Abstract:BACKGROUND:Situs inversus totalis (SIT) is a rare genetic congenital disease, characterized with complete right-to-left inversion of all the internal organs. We herein describe a meaningful case which was diagnosed as gallbladder adenosquamous carcinoma, a rare histology type of gallbladder cancer, with SIT.CASE PRESENTATION:A 59-year-old Chinese woman was admitted for persistent epigastric distention and intermittent abdominal pain. The abdominal CT scan revealed a huge mass at the gallbladder bottom, involving the adjacent transverse colon and liver. En-bloc radical resection of the gallbladder cancer, including partial colectomy and hepatectomy with regional node dissection, followed by colocolostomy and Roux-en-Y choledochojejunostomy, was successfully performed. Pathology analysis indicated an adenosquamous carcinoma with positive adenocarcinoma markers (CK7, CK19) and squamous carcinoma markers (CK5/6, P63).CONCLUSION:The SIT anomaly might increase the risk of malignancies by sharing genome mutations, suggesting the importance of surveillance in the SIT settings.

Abstract:A 49-year-old female undergoing a periodic health examination at other hospital revealed a mass in the tail of pancreas. The patient denied any personal history of surgery except subtotal hysterectomy because of multiple myomas in uterus 7 years ago, family history of abdominal cancer and trauma. Physical examination and laboratory finding (including tumor marker) were unremarkable. Chest X-ray result was normal. Contrast enhanced ultrasound (CEUS) examination showed a well-defined hypoechoic pancreatic mass which was suggestive of solid pseudopapillary tumor. Contrast enhanced computed tomography (CE-CT) of the abdomen revealed a mass of hypodensity suggestive of intraductal papillary mucinous neoplasm. Because of the risk of bleeding and exclusion of surgical contraindications, patient underwent laparoscopic surgery. Intraoperatively, a solid mass was identified in the tail of pancreas, the intraoperative frozen pathological examination suggested a heterotopic accessary spleen (HAS) with squamous epithelial cyst. Partial pancreatectomy was performed. The uniqueness of this case is that the spleen can be ectopic to the pancreas, what is even more unexpected is that the HAS undergone cystic change. When encountering a pancreatic mass, we need to think about the possibility of HAS. In conclusion, it is important to diagnose HAS with squamous cyst in the pancreatic tail presenting as other pancreatic masses.
A case report of perihilar cholangiocarcinoma in a patient with situs inversus totalis

Abstract:Situs inversus totalis is a rare congenital malformation in which organs are positioned in a mirror-image relationship to normal conditions. It often presents with vascular and biliary malformations. Only a few reports have pointed out the surgical difficulties in patients with situs inversus totalis, especially in those with perihilar cholangiocarcinoma. This report describes a 66-year-old male patient who underwent left hemihepatectomy (S5, 6, 7, and 8) with combined resection of the caudate lobe (S1), extrahepatic bile duct, and regional lymph nodes for perihilar cholangiocarcinoma with situs inversus totalis. Cholangiocarcinoma was mainly located in the perihilar area and progressed extensively into the bile duct. Surgery was performed after careful evaluation of the unusual anatomy. Although several vascular anomalies required delicate manipulation, the procedures were performed without major intraoperative complications. Postoperatively, bile leakage occurred, but the patient recovered with drainage treatment. The patient was discharged on the 29th postoperative day. Adjuvant chemotherapy with S-1 was administered for approximately 6 months. There was no recurrence 15 months postoperatively. Appropriate imaging studies and an understanding of unusual anatomy make surgery safe and provide suitable treatment for patients with situs inversus totalis.

Abstract:BackgroundSitus inversus totalis (SIT) is an uncommon disorder characterized by mirror-image anatomy, which can present unique challenges and potential vascular anomalies in surgical interventions, particularly in gastric cancer patients.AimsWe aim to delineate a rare case of gastric adenocarcinoma in a SIT patient and conduct a thorough review of the existing literature concerning surgical strategies, vascular anomalies, and outcomes observed across varied geographic locales and technological approaches.MethodsA thorough examination of a case involving a 39-year-old male SIT patient who underwent a successful distal gastrectomy with D2 lymph node dissection is presented alongside an expansive literature review. The review encompasses 47 articles, collating data on surgical approaches and vascular anomalies across 49 patients diagnosed with SIT and gastric cancer.ResultsThe patient underwent curative distal gastrectomy and Billroth II with Braun anastomosis within 95 minutes, incurring minimal intraoperative blood loss (100ml). Postoperative pathology confirmed moderately to poorly differentiated gastric adenocarcinoma (pT3N0M0), with no signs of recurrence or metastasis after 6 months of S-1 adjuvant chemotherapy. The literature review revealed vascular anomalies in approximately 20% of reported cases, accentuating its surgical significance. Noteworthy variations in surgical strategies, operative times, blood loss, and complications across different surgical modalities were observed, providing a comprehensive view into the practical management of such cases.ConclusionDespite the inherent challenges associated with SIT, various surgical techniques can be successfully applied with meticulous preoperative planning and understanding vascular anomalies. This compilation of diverse surgical experiences across numerous documented cases seeks to provide a consolidated resource for refining surgical strategies and enhancing postoperative outcomes for gastric cancer patients with SIT, underscoring the imperativeness of further research in this niche domain.

Abstract:OBJECTIVE:To explore the diagnosis and surgical treatment of patients with periampullary carcinoma and situs inversus totalis.METHODS:The data of a patient with periampullary carcinoma and complete situs inversus totalis, a rare disease treated in our hospital on Mar. 2006, was reported, and relative articles were reviewed.RESULTS:This patient was diagnosed with stage I to II of periampullary carcinoma. Bilirubin was recovered one week postoperatively. Incomplete adhesive ileus at gastroenteral anastomosis appeared 2 weeks after the operation and was healed by nutritional support, acupuncture, endoscopic drainage and enteral nutrition. From 1936 to 2006, 15 malignant tumors with situs viscerum inversus totalis were reported, only 5 periampullary carcinomas with situs viscerum inversus totalis were reported.CONCLUSIONS:Surgical operation should be considered for malignant tumor patients with situs inversus totalis without contraindication. Attention should be paid to the opposite anatomical structure in this kind of situation.

Abstract:BackgroundSitus inversus totalis (SIT) is a congenital condition wherein organs in abdominal or thoracic cavity are mirrored from their normal positions. Abdominal cocoon, is a rare disease of unknown aetiology that is characterised by total or partial small intestine encapsulation by a compact fibrocollagenous membrane. Aside from having two extremely rare conditions (SIT and Abdominal cocoon), our patient developed renal cell carcinoma (RCC), which makes this case even more uncommon.Case PresentationWe report the case of a 64-year-old man who was admitted to our hospital with an extremely rare case of localized RCC in the left kidney complicated with SIT and abdominal cocoon. Computer tomography urography (CTU) and angiography (CTA) showed that the patient was confirmed as having SIT, for the space-occupying lesion in the left kidney, clear cell RCC (ccRCC) was considered, the lesion in the right kidney was probably cystic. We diagnosed our patient as having a cT1aN0M0 left RCC, and the RENAL score was 7x. With partial nephrectomy (PN) being the preferred treatment approach, robot-assisted laparoscopic partial nephrectomy (RALPN) was performed after obtaining informed consent. After insertion of the laparoscope, adhesions were observed between the entire colon and the anterior abdominal wall. Then, abdominal cocoon was diagnosed. The surgery was uneventful, and the tumour was resected successfully while preserving the tumour capsule. No intestinal injury or any other complication occurred in the intraoperative or postoperative, and the patient recovered well after the operation.ConclusionPN is an extremely challenging procedure in patients with SIT and abdominal cocoon. The da Vinci Xi surgical system and thorough preoperative assessment allowed the surgeon to overcome stereotyping, visual inversion, and successfully perform PN in a patient with SIT and abdominal cocoon without increasing the risk of complications and preserving as much renal function as possible. Considering the satisfactory outcomes, this report may hopefully provide a practical reference for the treatment of RCC in patients with other special conditions.
Port Site and Distant Metastases of Gallbladder Cancer after Laparoscopic Cholecystectomy Diagnosed by Positron Emission Tomography

Abstract:We report port site and distant metastases of unsuspected gallbladder cancer after laparoscopic cholecystectomy diagnosed by positron emission tomography (PET) in two patients. Patient 1, a 72-year-old woman was diagnosed as cholelithiasis and cholecystitis and received laparoscopic cholecystectomy. Unsuspected gallbladder cancer was discovered with histological result of well-differentiated squamous cell carcinoma of the gallbladder infiltrating the entire wall. A PET scan using F-18-fluorodeoxyglucose (FDG-PET) before radical resection revealed residual tumor in the gallbladder fossa and recurrence at port site and metastases in bilateral hilar lymph nodes. Patient 2, a 69-year-old woman underwent laparoscopic cholecystectomy more than one year ago with pathologically confirmed unsuspected adenosquamous carcinoma of stage pT1b. At 7-mo follow-up after surgery, the patient presented with nodules in the periumbilical incision. Excisional biopsy of the nodule revealed adenosquamous carcinoma. The patient was examined by FDG-PET, demonstrating increased FDG uptake in the right lobe of the liver and mediastinal lymph nodes consistent with metastatic disease. This report is followed by a discussion about the utility of FDG-PET in the gallbladder cancer.

Abstract:Objective\r\nTo explore the impact of abdominal situs inversus on the diagnosis and treatment of abdominal diseases.\r\n\r\n\r\nMethods\r\nClinical data and course of 13 abdominal situs inversus cases from January 2012 to December 2016 were retrospectively analyzed.\r\n\r\n\r\nResults\r\nA total of 13 cases accounting for 1.5/10 000 of all cases who had abdominal CT were diagnosed situs inversus. 6 of them were diagnosed for routine physical examination, 4 of them for urinary tract symptoms, and the other 3 for digestive complaints. Conservative treatment or follow-up was prescribed for 8 patients who were asymptomatic and healthy; Five patients received operation, including 2 nephrecctomy, 1 pancreatico duodenectomy, 1 laparoscopic cholecystectomy, and 1 endoscopic retrograde cholangiopancreatography. All the cases recovered uneventfully.\r\n\r\n\r\nConclusions\r\nAbdominal situs inversus is rare, non-disease entity, usually an incidental finding during clinical examination. Abstract:Introduction and significance: Situs inversus totalis is characterized by the complete transposition of thoracic and abdominal viscera. Individuals can live asymptomatically with this condition; however, it may be associated with certain abnormalities of the organs involved. Case presentation: Herein, we present a case of a situs inversus totalis woman presented with choledocholithiasis. Elective laparoscopic cholecystectomy was performed on the patient with intraoperative modifications. The patient was discharged in a healthy condition. Clinical discussion: Several case reports have documented the typical presentation of cholelithiasis, which is characterized by pain in the left upper quadrant and epigastric region. Our patient exhibited similar symptoms and was diagnosed with choledocholithiasis. Conclusions: Diagnostic and therapeutic management of morbidities in SIT individuals can be performed with recommended modifications that can lead to favorable outcomes.

Abstract:A rare disorder called situs inversus partialis (SIP) is characterized by the transposition of organs in the abdomen or thoracic cavity from one side of the body to the other (the mirror image of normal). Autosomal dominant, autosomal recessive, rare genetic mutations, and X-linked recessive inheritance patterns have been identified to be involved in this condition. Laparoscopic cholecystectomies have been successfully performed on patients with SIT. Due to challenges in spatial orientation and the identification of anatomical variations brought on by the abdominal organs' mirror image, surgery is more complicated and takes longer. We describe a 40-year-old female case who had acute cholecystitis. Laparoscopic cholecystectomy was used to treat this patient, a highly effective procedure for both the treatment and care of these patients. Post-surgical examination and follow-up revealed improvement in the patient's condition without subsequent complications.

Abstract:Splenosis is the autotransplantation of normal splenic tissue mostly following splenic rupture or splenectomy. Isolated hepatic splenosis is rare and frequently misinterpreted as a neoplasm. Despite its rarity, hepatic splenosis should always be considered because its clinical management diverges from that of a neoplasm. Here, we report a case of a 59-year-old man with an occupied lesion mimicking a tumor in the right lobe of the liver. Physical examination and laboratory tests revealed uneventful findings except for the postoperative abdominal scar. The patient had a history of acute icteric hepatitis more than 10 years ago and a splenectomy after trauma 40 years ago. We describe the computed tomography and magnetic resonance imaging findings in this case and also retrospective reports of hepatic splenosis to summarize some features assisting the diagnosis.

Abstract:BACKGROUND:Situs inversus (SI) is a rare congenital anomaly characterized by the transposition of thoracic and abdominal viscera. Laparoscopic pancreaticoduodenectomy (LPD) is increasingly used in patients with periampullar and pancreatic carcinomas. For patients with SI, LPD can be more complicated because of reversed anatomy and possible other associated anomalies that have not been expected before surgery.CASE PRESENTATION:A female patient with SI totalis presented with inappetence, vomiting, and weight loss for 2 months. Imaging modalities and angiography revealed a mass in the periampullary region without obvious vascular abnormalities. The mass was successfully resected via LPD based on an elaborate preoperative plan. The surgical pathology report demonstrated adenocarcinoma of the duodenal papilla. The patient has been followed up for 4 months and no tumor recurrence or long-term complications were observed.CONCLUSION:LPD is technically difficult but feasible in patients with SI.

Abstract:Introduction and importance: Situs inversus totalis, a rare congenital condition, is characterized by the mirror-image reversal of the abdominal and thoracic organs. Amoebic liver abscess and emphysematous gallbladder are severe gastrointestinal infections. The coexistence of these three conditions is extremely rare. Case presentation: We present the case of a 65-year-old male who presented with abdominal pain, fever, and jaundice. Investigations revealed situs inversus totalis, amoebic liver abscess, and emphysematous gallbladder. The patient underwent successful treatment with antibiotics and open surgery drainage of the liver abscess, and subtotal cholecystectomy. Clinical discussion: The co-occurrence of situs inversus totalis, amoebic liver abscess, and emphysematous gallbladder poses a diagnostic challenge and requires thorough evaluation and appropriate management. Conclusion: Situs inversus totalis may coincide with other conditions like amebic abscess and emphysematous gallbladder, complicating diagnosis and treatment. Early diagnosis and prompt intervention are crucial to improve outcomes.

Abstract:Situs inversus is a rare congenital anatomical variant that involves a group of abnormalities regarding the disposition of intrathoracic and intraabdominal organs. This is opposed to situs solitus, which is the normal anatomical disposition of all bodily organs. [ 1 ] There are 2 types of situs inversus: situs inversus totalis (SIT) which corresponds to the mirror image of situs solitus, including the heart (dextrocardia), and partial situs inversus, where intraabdominal and thoracic organs are seen in mirror image. However, the heart is in a normal arrangement (levocardia). In the SIT we find the cardiac apex, stomach, spleen, ligament of Treitz, descending colon, and aorta in the right side of the abdomen, while the inferior vena cava, liver, gallbladder, and ascending colon are found in the left side of the abdomen (Fig. 1). Besides, it presents a bilobed right lung. SIT differs from situs ambiguous in that the latter corresponds to any other abnormality in the development of the left-right axis of the organs, and is characterized by right or left isomerism, heteroataxia, and multiple thoracoabdominal malformations. Among its subdivisions we find: situs ambiguous with polysplenia, situs ambiguous with asplenia, and Ivemark syndrome. [ 2 ] Classically, situs inversus totalis is asymptomatic, although a third of the cases are associated with a recessive genetic condition: primary ciliary dyskinesia. [ 2 ] Other concomitant pathologies are increasingly being reported, including: cardiovascular malformations (tetralogy of Fallot, pulmonary stenosis, defects in the development of the atrial or ventricular septum), lung adenocarcinoma, metastasis of gallbladder carcinoma, and biliary atresia. [ 2 , 3 ] The pathogenesis of SIT is still not well established. The involvement of an autosomal recessive gene on the long arm of chromosome 14 and deletions on chromosomes 7 and 8 have been established. [ 2 ] The correlation with deletions of KIF3-A and KIF3-B, 2 microtubule-dependent motor kinesins involved in cancer development, and whose nonfunctional forms prevent the transport of the cell adhesion factors N-cadherin and β-catenin to the cell surface has also been studied. [ 4 ] The epidemiology of these anatomical variations differs according to the literature. One review estimates that the incidence of situs inversus totalis in general population is 1:10,000. [ 5 ] The most current reviews estimate an incidence of 1:20,000 to 1:25,000 at a general level. [ 4 ] In addition, it tends to occur commonly in men (1.5:1) and there is no racial predilection. [ 2 , 4 ]

Influences of spinal decompression therapy and general traction therapy on the pain, disability, and straight leg raising of patients with intervertebral disc herniation

Abstract:[Purpose] The purpose of this study was to identify how spinal decompression therapy and general traction therapy influence the pain, disability, and straight leg raise (SLR) ability of patients with intervertebral disc herniation. [Subjects] The subjects were 30 patients with chronic lumbar pain who were divided into a spinal decompression therapy group (SDTG, n=15), and a general traction therapy group (GTTG, n=15). [Methods] The SDTG used a spinal decompression device, and the GTTG used a lumbar traction device. Both groups received conservative physical therapy three times a week for four weeks. A visual analog scale (VAS) was used to measure the degree of pain the patients with chronic lumbar pain. The Oswestry Disability Index (ODI) was used to measure the degree of functional disability. A goniometer was used to measure the patients’ SLR ability. [Results] Both SDTG and GTTG showed statistically significant decreases in VAS and ODI scores and a statistically significant increase in SLR angle. A comparison of the two groups found no statistically significant differences. [Conclusion] Spinal decompression therapy and general traction therapy are effective at improving the pain, disability, and SLR of patients with intervertebral disc herniation. Thus, selective treatment may be required.

Abstract:Situs inversus is a condition in which abdominal and thoracic organs are laterally transposed. The organs which are supposed to be on the right side of the abdomen are on the left and vice versa. It is a rare congenital condition; however, the exact incidence is difficult to determine as most of the cases go unnoticed until they undergo an imaging study. We report a case of a 30-year-old female presenting with situs inversus in association with a choledochal cyst and a horseshoe kidney. She underwent imaging evaluation for non-specific abdominal pain. Her routine clinical examination revealed a soft abdomen without any tenderness. Routine laboratory tests were within normal limits. Since there was abdominal pain, ultrasonography of the whole abdomen was advised. It revealed the presence of abdominal organs on the opposite side as normally seen. The common bile duct was dilated, and lower poles of the kidneys were fused. The cardiac apex was found to be on the right. It was followed up with a computed tomography scan which confirmed situs inversus. The common bile duct was dilated without any obstructive pathology in the pancreatic head or periampullary region. Lower poles of the kidneys were found to be fused together in front of the retroperitoneal vessels through an isthmus. Based on these findings, a diagnosis of situs inversus in a case of a choledochal cyst and a horseshoe kidney was made. Situs inversus is a rare entity. Its association with choledochal cysts and horseshoe kidneys has never been reported in the literature to the best of our knowledge.
