Abstract:Before 2022, monkeypox virus (Mpox) infection in humans was seldom reported outside Africa. During the May 2022 outbreak, most cases were detected among men who have sex with men (MSM). Since Mpox is largely unknown to the general population, through a self-completion questionnaire, we investigated the behaviours and knowledge of our at-risk population belonging to the sexually transmitted infection (STI) outpatient clinic of the Infectious Diseases Unit of the ASST Spedali Civili of Brescia, Italy, between August and October 2022. Most patients that took part in the compilation are HIV positive MSM. The other participants were HIV-seronegative patients with other STIs. Overall, 144 questionnaires were compiled. Most of the participants were Italians (130;90%) and males (139;96.5%) between 30 and 60 years (118;82%). Almost all (136;94%) reported having heard about Mpox and more than half (80;56%) received information about the transmission. Twenty-four respondents (16%) received information from health professionals and 14 (10%) believed that the information received was complete. Although 41% of respondents thought they were at risk of getting the infection and 62% were afraid to get it, the majority (56%) did not increase the precautions taken. When asked if they would accept a vaccine to prevent the disease, more than a third (32%) of respondents expressed hesitation or complete refusal to be vaccinated. Based on our results, what emerges is that there is still a lack of knowledge and awareness about Mpox. To address this issue, targeted health promotion and education strategies that provide the necessary resources to reduce risk behaviours and enhance connections with healthcare professionals are needed.

[Differential expression of proteins in Oryza sativa leaves in response to cadmium stress].

Abstract:In order to explore the cadmium (Cd)-tolerance mechanism of rice (Oryza sativa), a hydroponic experiment with Cd-tolerant rice cultivar PI312777 and Cd-sensitive rice cultivar IR24 was conducted. The cultivars were treated with 0 (control), 50, and 100 micromol L(-1) of Cd(2+) for 7 days, and the differential expression of proteins in their leaves were analyzed by two-dimensional electrophoresis and MALDI-TOF/MS approach. Under Cd stress, 31 protein spots in PI312777 leaves and 19 protein spots in IR24 leaves had significantly different expression, compared with the control. In PI312777 leaves, the 24 of 31 proteins had 4 proteins related to photosynthesis, 11 proteins related to cell metabolism and defense, 3 proteins related to other metabolism pathway, and 2 proteins were unknown proteins; in IR24 leaves, the 15 of 19 proteins were identified. Under Cd-stress, the heat shock protein, glutathione-diulfide reductase, poteasome subunit alpha type-6, 1,6-bisphosphate aldolase, thioredoxin, and DNA repair-recombination protein in PI312777 leaves were up-regulated expressed, while these proteins in IR24 were not significantly changed except the down-regulation of 1,6-bisphosphate aldolase and thioredoxin. The higher Cd-tolerance of PI312777 could have close relations with the differentially expressed proteins.
Knowledge and attitudes of health care workers about monkeypox virus infection in Southern Italy

Abstract:Background This present survey sought to investigate the level of knowledge and the attitudes pertaining the monkeypox (mpox) virus infection among a sample of health care workers (HCWs) in Italy, as well as the possible role of different factors on these outcomes. Methods The cross-sectional survey was performed from July through October, 2022 at four randomly selected hospitals located in Southern Italy. Results The questionnaire was completed by 421 HCWs, for an overall 59% response rate. Less than two-thirds were able to define the disease and the correct answer of the transmission mechanisms ranged from 22.8% for contact with contaminated objects to 75.8% through close contact with body fluids. Only 4% and 12.8% indicated HCWs and elderly/frail/people with underlying immune deficiencies as risk groups. The mean overall score of the knowledge assessment on mpox was 3.4 (0–9). The multivariate logistic regression analysis showed that HCWs with a lower number of years of working experience and those who had acquired information about mpox from scientific journals were more likely to have a higher level of knowledge. The average score of the perception of the severity of the disease was 6.3. A similar score with a value of 6.1 has been observed for the statement that mpox is a serious problem for the population. Regarding the level of concern about contracting mpox, the mean score was 5.1. Only 10.5% reported that they feel that this disease can be prevented, with an overall mean score of 6.5. Almost all HCWs reported that they are still living as usual, with no modification of their behavior for fear of contracting the mpox. The results of the multivariate logistic regression model showed that women, HCWs with a higher level of knowledge about mpox, and those who needed additional information about mpox were more likely to have a higher level of perception of the severity of the disease. Conclusion This survey has demonstrated that HCWs had an unsatisfactory level of knowledge toward mpox and only nearly half showed positive attitudes. Strategic health training programs should be made so that knowledge can be acquired.

Abstract:Objective: To assess the effectiveness of expiratory muscle raining on the pulmonary function of spinal cord injured patients. Design: Randomized controlled trial. Setting: Acute inpatient rehabilitation hospital. Participants: Patients (N 29, 22 men and 7 women) with ecent traumatic, motor complete, spinal cord injury (SCI) at or bove level T1 consecutively admitted to an SCI rehabilitation ervice. Main Outcome Measures: Pulmonary function tests were easured before and after the training program and included orced vital capacity (FVC); forced expiratory volume in 1 econd (FEV1); maximum expiratory pressure (MEP), which is ften referred to as forced expiratory pressure; maximum inpiratory pressure (MIP), which is often referred to as negative nspiratory force; inspiratory capacity (IC); expiratory reserve olume (ERV); total lung capacity (TLC); functional residual apacity (FRC); and residual volume (RV). Abstract:Background Monkeypox (mpox) is a zoonotic disease originating from the Congo Basin (Clade I) and West Africa (Clade II). In 2022, mpox spread to non-endemic European countries, predominantly through human transmission associated with sexual contact. The outbreak in Europe was primarily with the Clade IIb lineage, which is less virulent. The World Health Organization (WHO) declared this outbreak a Public Health Emergency of International Concern (PHEIC) in 2022, which ended in May 2023 after a decline in cases. However, in July 2024, a resurgence of the more virulent Clade I occurred in the Democratic Republic of Congo (DRC), leading WHO to declare mpox a PHEIC again, due to the risk of global spread. Understanding epidemiology and risk factors of mpox is vital for effective public health measures. Methodology and principal findings A search conducted from 2014 to 2024 across PubMed, Scopus and Embase identified 46 studies on mpox in Europe, which were included for qualitative analysis. The key themes identified were epidemiology, risk factors, and implications for public health actions. High-risk behaviors include sexual contact among men who have sex with men (MSM) with multiple partners, living with HIV, and frequent travel to endemic regions. Conclusions and significance With no definitive cure for mpox, public health measures such as surveillance, monitoring, and contact tracing are essential. Additionally, encouraging case-control studies is crucial for exploring other potential risk behaviors and design behavioral interventions, vaccination campaigns and awareness programs aimed at reducing high-risk behaviors among these populations. Although the number of cases in Europe did not surge in August 2024, proactive measures are necessary to prevent further spread.

Abstract:Background: In May 2022, the monkeypox virus outbreak in multiple countries on various continents marked a potential resurgence of the disease as a global health issue. Considering the crucial role of physicians in mitigating the monkeypox outbreak, we sought to evaluate physicians’ knowledge, attitude, concerns, and vaccine acceptance for monkeypox, in the shadow of the COVID-19 pandemic. Methods: A large-scale, cross-sectional survey was conducted among 283 physicians between 20 August−2 September 2022, in Turkey. The participants’ sociodemographic characteristics, knowledge, attitudes, concerns, and vaccine acceptance toward monkeypox infection were collected via a questionnaire. Results: Our study revealed that 32.5% of physicians achieved a good level of knowledge; similarly, 31.4% of the physicians planned to have the monkeypox vaccine. Multivariate binary logistic regression analysis showed that female physicians (p = 0.031) and older people (≥30 vs. <30) were more likely to be knowledgeable about monkeypox (p = 0.007). We found that participants from divisions of internal medicine (p = 0.033) who knew about the monkeypox disease during medical school or residency (p = 0.005) and were previously exposed to COVID-19 disease (p = 0.005) were more likely to have a good knowledge score of monkeypox. We also found that physicians with a good knowledge score were more worried about monkeypox compared to COVID-19 (AOR: 2.22; 95% CI:1.13−4.33; p = 0.019). Additionally, those who had information on monkeypox during medical education (AOR = 2.16, 95% CI = 1.10−4.21; p = 0.024) were more likely to receive the smallpox vaccine to prevent monkeypox viral infection when available. Conclusions: The present study pointed out that physicians in Turkey have unsatisfactory levels of knowledge about the emerging monkeypox. This study results can impede attempts to detect and manage cases of monkeypox and should be addressed through appropriate and timely awareness and educational programs, alerts, and seminars. These might serve as the basis for policymakers’ decisions about promoting national monkeypox vaccination strategies and addressing potential vaccine hesitancy and misinformation when needed.

Abstract:UNLABELLED Laryngotracheal stenosis, although not very common, is an important medical condition that represents a true diagnostic and therapeutic challenge. We present our experiences in treating 22 patients with laryngotracheal stenosis between 1995 and 2004 at Department of ENT, Head and Neck Surgery, Dr. Josip Bencević General Hospital in Slavonski Brod. PATIENTS AND METHODS Stenosis was caused by trauma in 9 patients, by endotrauma due to intubation in 3, and by war penetrating injury caused by shell and mine fragments in 4 patients. Hypertrophic glottic stenosis, after supraglottic laryngectomy and postoperative radiotherapy caused laryngeal obstruction in 2 patients. Tumors caused stenosis in 11 patients: tumors of the thyroid with compression in 9, and malignant infiltration of the trachea in 2 patients. Glottic stenosis was present in 4, glottic and subglottic in 4, subglottic and tracheal in 2, isolated tracheal stenosis in 12 patients. Surgical treatment was performed in 14 patients: endoscopic in two and classic open procedure in 12 patients. Laryngeal and tracheal stents were inserted in 3 patients with inopeable thyroid tumor. All treated patients except one were extubated. RESULTS Excellent functional results were achieved upon endoscopic resection, resection of stenosis and end-to-end anastomosis. Minimally invasive and good palliative treatment was achieved by the stent placement into the airway.
[Clinical value of high-resolution magnetic resonance tomography in the staging of tumors of the oral cavity, the pharynx and the larynx].

Abstract:In a prospective study 102 patients with malignant tumors of the tongue, oropharynx, hypopharynx, and larynx were staged by means of MRI. Special attention was directed at those tumour extensions that could influence the treatment strategy. The MR findings were correlated with the results of palpation, endoscopy, ultrasound, computerised tomography and histopathological findings. MRI showed a good delineation of pT2 to pT4 tumours. However, visualisation of small tumours in the soft palate, the tonsils, the pyriform sinuses and vocal cords was difficult. In conclusion our results suggest that in addition to endoscopy MRI is a valuable tool for the examination of tumours of tongue, oropharynx, hypopharynx and larynx.
Monkeypox: EpiMPX Surveillance System and Open Data with a Special Focus on European and Italian Epidemic

Abstract:Background: The current out-of-Africa 2022 outbreak of Monkeypox requires a coordinated, international response through the rapid sharing of data and research results, as we have seen with COVID-19 and the previous Ebola and Zika outbreaks, which demonstrated how important real-world data are to inform public health, to create surveillance systems, to determine policy decisions and to improve clinical trials. Objectives: To support global response efforts by providing public access to real-time Monkeypox-related data for effective use of open data that could accelerate scientific knowledge and discoveries in terms of understanding, preventing, and treating the disease. In practice, to create a new surveillance system easy to consult and utilize. Study design: This work aims to build a surveillance system, namely EpiMPX, that allows researchers and policymakers to monitor the impact of Monkeypox in Europe, with a special focus on the epidemic trends in the Italian regions, based on an open-access database containing information on the laboratory confirmed Monkeypox cases reported by EU/EEA countries and updated once a week. In addition, users will be provided open-access R codes to estimate key epidemiological parameters such as the reproduction number (updating the Serial Interval distribution when new estimates will be published) and produce real-time results on their devices. Results: EpiMPX monitors the space-time distribution of cases and their characteristics, such as age, gender, symptoms, clinical status, and sexual orientation, when available. Even if it is currently too early for reliable calculation of epidemiological parameters, we estimated reproduction number R t in European countries with more than 28 days of observed incidence, assuming that the Serial Interval (SI) early estimate in Italy is valid for other countries too. This provides a direct visual assessment of the geographic distribution of risk areas as well as insights into the evolution of the outbreak over time. Italian data were evaluated concerning gender, region prevalence and cumulative data. Conclusions: The proposed EpiMPX surveillance system provides an overview of the European and Italian Monkeypox epidemiological situation with an open-access database to support epidemiological understanding of the origins and transmission dynamics of the disease with informative graphical outputs. These data confirmed the prevalent expression of Monkeypox within males, both in Europe and Italy. European MSM patients were affected by Monkeypox in a high percentage, confirming close sexual contact and possible sexual transmission. For the first time, Italian data on the regional distribution of cases and gender distribution were graphically evaluated. The data and research results are freely available and can be easily enriched to provide a prompt response to the scientific community and accelerate global efforts to contain the Monkeypox virus.

Abstract:Introduction: The zoonotic features and potential for transmission between animals and humans make the monkeypox (MPX) virus, a member of the orthopoxvirus family, a serious threat. Unfortunately, healthcare staff's lack of knowledge and readiness about MPX has made it harder to implement effective prevention and response plans. Our current understanding of the KAPs (Knowledge, Attitudes, and Practices) among Jordanian clinicians could benefit from additional research. Methods: This cross-sectional study aimed to evaluate the MPX KAP among 300 healthcare workers (HCWs) in two private hospitals in the Irbid governorate of Jordan. We employed descriptive statistics, such as percentages and frequencies, as well as an independent sample t-test, one-way ANOVA, and multiple linear regression (enter method) for data analysis. Results: Although differences existed between demographic groups, the study found that participants had an average level of MPX knowledge proficiency. Out of a total of 300 participants, 196 were female and 104 were male, with 52.7% in the 20–30 age bracket. Compared to their female counterparts, male respondents exhibited higher levels of knowledge, attitudes, and practices regarding MPX. Attitudes and practices towards MPX varied by age group and level of education, demonstrating how demographic factors impact these aspects of public health. Additionally, monkeypox attitudes were lower among women, those with a diploma degree, and those aged 31–40. Conclusions: Healthcare staff require adequate training and continuous education to address their lack of knowledge and attitudes regarding MPX protection. Staying updated about new illnesses like MPX is crucial, as the COVID-19 pandemic demonstrated. To effectively tackle global health concerns, continuous learning and up-to-date knowledge are essential.
Global knowledge and attitudes towards mpox (monkeypox) among healthcare workers: a systematic review and meta-analysis

Abstract:Abstract Background The recent increase in human mpox (monkeypox) cases emphasizes the importance of early detection, prompt response and preventive management to control the spread of the disease. Healthcare workers (HCWs) play a crucial role in this process. This study aimed to determine the global knowledge and attitudes towards mpox among HCWs. Methods This study searched multiple databases, including Google Scholar, Scopus, PubMed/MEDLINE, Science Direct, Web of Science, Embase, Springer and ProQuest, to locate various publications. The search was limited to English-language articles published between May 2022 (when the increase in mpox incidence was reported) and August 2023. The Joanna Briggs Institute (JBI) quality checklist was utilized to evaluate the quality of the included studies. Data were obtained using a Microsoft Excel spreadsheet and subsequently scrutinized through STATA software, version 14. The heterogeneity of the studies was assessed using the inverse variance and Cochran Q statistics based on the I2 test statistics. The Dersimonian and Liard random effects models were used where heterogeneity existed. Subgroup analysis and univariate and multivariable metaregression techniques were used to examine the causes of heterogeneity. Results A total of 22 studies, including 22 studies for knowledge (27 731 HCWs) and 6 studies for attitudes (14 388 HCWs), were included in the meta-analysis. The pooled estimates for good knowledge and positive attitudes among HCWs were 26.0% (95% confidence interval [CI] 17.8 to 34.2) and 34.6% (95% CI 19.0 to 50.2), respectively. Moreover, the knowledge was 34.8% (95% CI 24.1 to 45.6) among HCWs with <5 y of work experience and 41.6% (95% CI 33.1 to 50) among individuals possessing >5 y of professional background. Conclusions Good knowledge of HCWs is at a low level. It is suggested that training sessions should be tailored towards younger HCWs with less healthcare experience. Additionally, it is essential to identify strategies on how to improve the knowledge and attitudes for better practice about the disease in HCWs worldwide.

Abstract:Background: High incidence mpox rates suggest asymptomatic individuals may contribute to virus transmission. We undertook this study to assess the seroprevalence of IgG anti-MPXV in a cohort of asymptomatic PLWH, to analyze the size of the phenomenon of asymptomatic infections. Materials and methods: From October 2022 to March 2023 we serially collected serum samples from PLWH attending our Clinic. IgG against MPXV have been assessed on stored cryopreserved samples with an ELISA. Only people with no previous reported vaccine against smallpox or mpox nor previous clinical manifestations consistent with an mpox diagnosis were included. Results: 285 PLWH were included. Twenty-one participants tested positive for IgG anti MPXV (7.37%, 95% CI 4.62-11.0). Seropositivity was predominant in male (15/285, 71.4) with a small fraction of female (6/285,28.6%) and PWID (1/285,4.8%). Conclusions: Our findings suggest the possibility of an asymptomatic course of the mpox infection even in populations beyond traditional high-risk groups.
Epidemiological trends and clinical features of the ongoing monkeypox epidemic: a preliminary pooled data analysis and literature review

Abstract:An emerging outbreak of monkeypox infection is quickly spreading worldwide, being currently reported in more than 30 countries, with slightly less than one thousand cases. In the present preliminary report, we collected and synthesized early data concerning epidemiological trends and clinical features of the ongoing outbreak and we compared them with those of previous outbreaks. Data were pooled from six clusters in Italy, Australia, the Czech Republic, Portugal, and the UK, totaling 124 cases (for 35 of which it was possible to retrieve detailed information). The ongoing epidemic differs from previous outbreaks in terms of age (54.29% of individuals in their thirties), sex/gender (most cases being males), risk factors, and transmission route, with sexual transmission being highly likely. Also, the clinical presentation is atypical and unusual, being characterized by anogenital lesions and rashes that relatively spare the face and extremities. The most prevalent sign/symptom reported was fever (in 54.29% of cases) followed by inguinal lymphadenopathy (45.71%) and exanthema (40.00%). Asthenia, fatigue, and headache were described in 22.86% and 25.71% of the subjects, respectively. Myalgia was present in 17.14% of the cases. Both genital and anal lesions (ulcers and vesicles) were reported in 31.43% of the cases. Finally, cervical lymphadenopathy was described in 11.43% of the sample, whilst the least commonly reported symptoms were diarrhea and axillary lymphadenopathy (5.71% of the case series for both symptoms). Some preliminary risk factors can be identified (being a young male, having sex with other men, engaging in risky behaviors and activities, including condomless sex, HIV positivity (54.29% of the sample analyzed), and a story of previous sexually transmitted infections, including syphilis). On the other hand, being fully virally suppressed and undetectable may protect against a more severe infectious course. Abstract:The 2022 multi-country monkeypox outbreak in humans has brought new public health adversity on top of the ongoing coronavirus disease 2019 (COVID-19) pandemic. The disease has spread to 104 countries throughout six continents of the world, with the highest burden in North America and Europe. The etiologic agent, monkeypox virus (MPXV), has been known since 1959 after isolation from infected monkeys, and virulence among humans has been reported since the 1970s, mainly in endemic countries in West and Central Africa. However, the disease has re-emerged in 2022 at an unprecedented pace, with particular concern on its human-to-human transmissibility and community spread in non-endemic regions. As a mitigation effort, healthcare workers, public health policymakers, and the general public worldwide need to be well-informed on this relatively neglected viral disease. Here, we provide a comprehensive and up-to-date overview of monkeypox, including the following aspects: epidemiology, etiology, pathogenesis, clinical features, diagnosis, and management. In addition, the current review discusses the preventive and control measures, the latest vaccine developments, and the future research areas in this re-emerging viral disease that was declared as a public health emergency of international concern.

Abstract:Monkeypox (MPX) is a zoonotic disease caused by the MPX virus from the poxviridae family of orthopoxviruses. Typically, endemic in central and west Africa, it has now become a matter of concern since cases have been reported in non-endemic countries around mid-June 2022, especially in the European region, with the transmission not related to travel. The diagnosis is made by PCR testing of the skin lesions. Even though treatment is symptomatic, antiretrovirals, such as tecovirimat, are used in severe cases. Vaccination with second and third generation vaccines is approved for prophylaxis in high risk individuals. Unfortunately, these options of treatment and prevention are only available in high income countries at the moment. This review, through a thorough literature search of articles from 2017 onward, focuses on epidemiology, clinical manifestations, challenges, treatment, prevention and control of MPX virus and how they can be corelated with other viral outbreaks including COVID-19, Acute Hepatitis of unknown origin, Measles and Dengue, to better predict and therefore prevent its transmission. The previous COVID-19 pandemic increased the disease burden on healthcare infrastructure of low-middle income countries, therefore, this recent MPX outbreak calls for a joint effort from healthcare authorities, political figures, and NGOs to combat the disease and prevent its further spread not only in high income but also in middle- and low-income countries.

Abstract:Monkeypox virus (MPXV) is another zoonotic virus spilled over to the man and resulted in pandemic. World Health Organization declared it as a 'Public Health Emergency of International Concern (PHEIC) on July 22, 2022. Mpox affected over 95226 individuals among them claimed the lives of 185. Despite the fact that Mpox is generally mild and self-limited, immunocompromised people with low CD4 counts may experience severe disease course. Management of Mpox patients has three pillars. First symptomatic approach includes pain management, prophylaxis for secondary infections and when needed effective treatment of superinfections. Second, vaccines developed against smallpox can be used in preexposure or postexposure prophylaxis strategies against Mpox. Third, current antiviral options include tecovirimat, cidofovir and birincidofovir all of which have been recommended relying on experience from animal studies, clinical case reports or case series. Results of well-planned randomized control trials are not available. Occupational exposure to MPXV is especially a manageable risk for health care workers. Prevention of Mpox also requires risk communication with vulnerable population and their involvement in mitigation efforts.

Abstract:Monkeypox virus (MPXV) is an emerging zoonotic virus that belongs to the Orthopoxvirus genus and presents clinical symptoms similar to those of smallpox, such as fever and vesicular–pustular skin lesions. However, the differential diagnosis between smallpox and monkeypox is that smallpox does not cause lymphadenopathy but monkeypox generates swelling in the lymph nodes. Since the eradication of smallpox, MPXV has been identified as the most common Orthopoxvirus to cause human disease. Despite MPXV being endemic to certain regions of Africa, the current MPXV outbreak, which began in early 2022, has spread to numerous countries worldwide, raising global concern. As of the end of May 2023, over 87,545 cases and 141 deaths have been reported, with most cases identified in non-endemic countries, primarily due to human-to-human transmission. To better understand this emerging threat, this review presents an overview of key aspects of MPXV infection, including its animal reservoirs, modes of transmission, animal models, epidemiology, clinical and immunological features, diagnosis, treatments, vaccines, and prevention strategies. The material presented here provides a comprehensive understanding of MPXV as a disease, while emphasizing the significance and unique characteristics of the 2022 outbreak. This offers valuable information that can inform future research and aid in the development of effective interventions.

Abstract:The COVID-19 pandemic, starting in 2020, has presented a major challenge in terms of early diagnosis and the subsequent containment and management of severe cases. The spread of viruses such as monkeypox in non-endemic countries is now creating new difficulties for healthcare professionals. Proper case definition and clinical examination are crucial for the early identification of suspected cases. For this reason, we performed a review of the literature in order to report the first signs, which are useful for healthcare providers for early case identification. Since 2022 to date, 86,930 laboratory-confirmed cases and 1051 probable cases have been reported worldwide, and of these, 116 were fatal cases and, for the first time, most of the cases were registered in countries that have not historically reported monkeypox and that lack direct or immediate epidemiological links to areas of West or Central Africa where the disease is endemic. Patients with Monkeypox experience prodromal symptoms, such as fever, fatigue, headache, muscle aches, and a rash after an incubation period of 5–21 days. The disease is usually self-limiting within 2–4 weeks but can lead to complications, such as pneumonia, encephalitis, kidney injury, and myocarditis in children, pregnant individuals, and those with weakened immune systems. The case–fatality ratio is between 1 and 10%. Today, prevention campaigns and the control of human monkeypox are the best weapons to prevent infection and stop transmission. Prevention strategies, such as avoiding contact with sick or dead animals, and the proper preparation of all foods containing animal meat or parts, should be adopted. Furthermore, close contact with infected people or contaminated materials should be avoided to prevent human-to-human transmission.
