Abstract:Abstract Background In maintenance hemodialysis patients, intradialytic hypotension (IDH) is a frequent complication that has been associated with poor clinical outcomes. Prediction of IDH may facilitate timely interventions and eventually reduce IDH rates. Methods We developed a machine learning model to predict IDH in in-center hemodialysis patients 15 to 75 minutes in advance. IDH was defined as systolic blood pressure (SBP) < 90 mmHg. Demographic, clinical, treatment-related, and laboratory data were retrieved from electronic health records and merged with intradialytic machine data that were sent in real-time to the cloud. For model development, dialysis sessions were randomly split into training (80%) and testing (20%) sets. The area under the receiver operating characteristic curve (AUROC) was used as a measure of model's predictive performance. Results We utilized data from 693 patients who contributed 42 656 hemodialysis sessions and 355 693 intradialytic SBP measurements. IDH occurred in 16.2% of hemodialysis treatments. Our model predicted IDH 15 to 75 minutes in advance with an AUROC of 0.89. Top IDH predictors were the most recent intradialytic SBP, and IDH rate as well as mean nadir SBP of the previous 10 dialysis sessions. Conclusions Real-time prediction of IDH during an ongoing hemodialysis session is feasible and has a clinically actionable predictive performance. If and to what degree this predictive information facilitates the timely deployment of preventive interventions and translates into lower IDH rates and improved patient outcomes warrants prospective studies.

Abstract:Background and Objective Intradialytic hypotension (IDH) is closely associated with adverse clinical outcomes in HD-patients. An IDH predictor model is important for IDH risk screening and clinical decision-making. In this study, we used Machine learning (ML) to develop IDH model for risk prediction in HD patients. Methods 62,227 dialysis sessions were randomly partitioned into training data (70%), test data (20%), and validation data (10%). IDH-A model based on twenty-seven variables was constructed for risk prediction for the next HD treatment. IDH-B model based on ten variables from 64,870 dialysis sessions was developed for risk assessment before each HD treatment. Light Gradient Boosting Machine (LightGBM), Linear Discriminant Analysis, support vector machines, XGBoost, TabNet, and multilayer perceptron were used to develop the predictor model. Results In IDH-A model, we identified the LightGBM method as the best-performing and interpretable model with C- statistics of 0.82 in Fall30Nadir90 definitions, which was higher than those obtained using the other models (P<0.01). In other IDH standards of Nadir90, Nadir100, Fall20, Fall30, and Fall20Nadir90, the LightGBM method had a performance with C- statistics ranged 0.77 to 0.89. As a complementary application, the LightGBM model in IDH-B model achieved C- statistics of 0.68 in Fall30Nadir90 definitions and 0.69 to 0.78 in the other five IDH standards, which were also higher than the other methods, respectively. Conclusion Use ML, we identified the LightGBM method as the good-performing and interpretable model. We identified the top variables as the high-risk factors for IDH incident in HD-patient. IDH-A and IDH-B model can usefully complement each other for risk prediction and further facilitate timely intervention through applied into different clinical setting.

Personalized prediction of intradialytic hypotension in clinical practice: Development and evaluation of a novel AI dashboard incorporating risk factors from previous and current dialysis sessions

Abstract:Background: Intradialytic hypotension (IDH) is one of the most common and critical complications of hemodialysis. Despite many proven factors associated with IDH, accurately predicting it before it occurs for individual patients during dialysis sessions remains a challenge. Purpose: To establish artificial intelligence (AI) predictive models for IDH, which consider risk factors from previous and ongoing dialysis to optimize model performance. We then implement a novel digital dashboard with the best model for continuous monitoring of patients' status undergoing hemodialysis. The AI dashboard can display the real-time probability of IDH for each patient in the hemodialysis center providing an objective reference for care members for monitoring IDH and treating it in advance. Methods: Eight machine learning (ML) algorithms, including Logistic Regression (LR), Random Forest (RF), Support Vector Machine (SVM), K Nearest Neighbor (KNN), Light Gradient Boosting Machine (LightGBM), Multilayer Perception (MLP), eXtreme Gradient Boosting (XGBoost), and NaiveBayes, were used to establish the predictive model of IDH to determine if the patient will acquire IDH within 60 min. In addition to real-time features, we incorporated several features sourced from previous dialysis sessions to improve the model's performance. The electronic medical records of patients who had undergone hemodialysis at Chi Mei Medical Center between September 1, 2020 and December 31, 2020 were included in this research. Impact evaluation of AI assistance was conducted by IDH rate. Results: The results showed that the XGBoost model had the best performance (accuracy: 0.858, sensitivity: 0.858, specificity: 0.858, area under the curve: 0.936) and was chosen for AI dashboard implementation. The care members were delighted with the dashboard providing real-time scientific probabilities for IDH risk and historic predictive records in a graphic style. Other valuable functions were appended in the dashboard as well. Impact evaluation indicated a significant decrease in IDH rate after the application of AI assistance. Conclusion: This AI dashboard provides high-quality results in IDH risk prediction during hemodialysis. High-risk patients for IDH will be recognized 60 min earlier, promoting individualized preventive interventions as part of the treatment plan. Our approachis believed to promise an excellent way for IDH management.

Abstract:Both intradialytic hypotension (IDH) and hypertension (IDHTN) are associated with poor outcomes in hemodialysis patients, but a model predicting dual outcomes in real-time has never been developed. Herein, we developed an explainable deep learning model with a sequence-to-sequence-based attention network to predict both of these events simultaneously. We retrieved 302,774 hemodialysis sessions from the electronic health records of 11,110 patients, and these sessions were split into training (70%), validation (10%), and test (20%) datasets through patient randomization. The outcomes were defined when nadir systolic blood pressure (BP) < 90 mmHg (termed IDH-1), a decrease in systolic BP ≥ 20 mmHg and/or a decrease in mean arterial pressure ≥ 10 mmHg (termed IDH-2), or an increase in systolic BP ≥ 10 mmHg (i.e., IDHTN) occurred within 1 h. We developed a temporal fusion transformer (TFT)-based model and compared its performance in the test dataset, including receiver operating characteristic curve (AUROC) and area under the precision-recall curves (AUPRC), with those of other machine learning models, such as recurrent neural network, light gradient boosting machine, random forest, and logistic regression. Among all models, the TFT-based model achieved the highest AUROCs of 0.953 (0.952–0.954), 0.892 (0.891–0.893), and 0.889 (0.888–0.890) in predicting IDH-1, IDH-2, and IDHTN, respectively. The AUPRCs in the TFT-based model for these outcomes were higher than the other models. The factors that contributed the most to the prediction were age and previous session, which were time-invariant variables, as well as systolic BP and elapsed time, which were time-varying variables. The present TFT-based model predicts both IDH and IDHTN in real time and offers explainable variable importance.

Abstract:Introduction: Intradialytic hypotension (IDH) is prevalent and associated with high hospitalization and mortality rates. The purpose of this study was to explore the risk factors for IDH and use artificial intelligence to establish an early alert system before hemodialysis sessions to identify patients at high risk of IDH. Materials and methods: We obtained data on 314,534 hemodialysis sessions conducted at Sichuan Provincial People's Hospital from the renal disease treatment information system. IDH was defined as a systolic blood pressure drop ≥20 mm Hg, a mean arterial pressure drop ≥10 mm Hg during dialysis, or the occurrence of clinical hypotensive events requiring nursing intervention. After pre-processing, the data were randomly divided into training (80%) and testing (20%) sets. Four interpolation methods, three feature selection methods, and 18 machine learning algorithms were used to construct predictive models. The area under the receiver operating characteristic curve (AUC) was the main indicator for evaluating the performance of the models, while Shapley Additive ExPlanation was used to explain the contribution of each variable to the best predictive model. Results: A total of 3,906 patients and 314,534 dialysis sessions were included, of which 142,237 cases showed IDH (incidence rate, 45.2%). Nineteen parameters were identified through artificial intelligence feature screening. They included age, pre-dialysis weight, dry weight, pre-dialysis blood pressure, heart rate, prescribed ultrafiltration, blood cell counts (neutrophil, lymphocyte, monocyte, eosinophil, lymphocyte, and platelet counts), hematocrit, serum calcium, creatinine, urea, glucose, and uric acid. Random forest, gradient boosting, and logistic regression were the three best models, and the AUCs were 0.812 (95% confidence interval [CI], 0.811-0.813), 0.748 (95% CI, 0.747-0.749), and 0.743 (95% CI, 0.742-0.744), respectively. Conclusion: Our dialysis software-based artificial intelligence alert system can be used to predict IDH occurrence, enabling the initiation of relevant interventions.

Abstract:Intradialytic hypotension (IDH) is a common and potentially life-threatening complication in hemodialysis patients. Traditional preventive measures have shown limited effectiveness in reducing IDH incidence. This systematic review evaluates the existing literature on the use of artificial intelligence (AI) and machine learning (ML) models for predicting IDH in hemodialysis patients. A comprehensive literature search identified five eligible studies employing diverse AI/ML algorithms, including artificial neural networks, decision trees, support vector machines, XGBoost, random forests, and LightGBM. These models utilized various features such as patient demographics, clinical data, laboratory findings, and dialysis-related parameters. The studies reported promising results, with several models achieving high prediction accuracies, sensitivities, specificities, and area under the receiver operating characteristic curve values for predicting IDH. However, limitations include variations in study populations, retrospective designs, and the need for prospective validation. Future research should focus on multicenter prospective studies, assessing clinical utility, and integrating interpretable AI/ML models into clinical decision support systems.

Abstract:BACKGROUND Intradialytic hypotension (IDH) is one of the most common complications during hemodialysis,and is an important risk factor for cardiovascular events, hospitalization, and mortality. The reported incidence of IDH ranges from 7.5% to 69% according to different definitions.According to the recommended definition of Kidney Disease Outcome Quality Initiative (KDOQI), the incidence rate is 20%-30%. The mechanism of IDH is complicated and is associated with decreased blood volume and insufficient vascular response. The occurrence of IDH depends on various risk factors, including demographic factors (age, sex), comorbidities (diabetes, coronary heart disease, left ventricular hypertrophy), dialysis characteristics, complications (dialysis vintage, weight gain between dialysis sessions, anemia), dialysis treatment prescription (ultrafiltration), medications (antihypertensive medications), and laboratory findings.Therefore, pre-assessment and monitoring of all patients during hemodialysis are critical for IDH prevention and treatment.However, IDH is associated with many factors and is difficult to predict, even for experienced nephrologists. Many efforts have been made to predict and initiate interventions to prevent IDH, however, in clinical settings, the accuracy and applicability remain poor. Artificial intelligence (AI) is a branch of computer science that can analyze complex medical data and can be used for clinical diagnosis, treatment, prediction, and efficacy evaluation.[14]Machine learning can merge a large amount of different data into a unified algorithm, such that a large amount of data can be used to develop reliable predictive analysis. AI has been applied to predict IDH,however, they can only predict IDH more than 1 h after initiating hemodialysis, which results in a compromised possibility of early intervention. The purpose of this research was to establish a software-based early alert system using artificial intelligence to identify patients at a high risk of IDH just before the initiation of hemodialysis, thereby allowing the medical team to manage and prevent IDH during hemodialysis. Abstract:Abstract Background and Aims Although there have been many studies on the machine learning prediction model of intradialytic hypotension, the high prediction accuracy in the academic field needs to be transferred to the clinical environment to aid in medical professionals' decision-making process. Before this clinical trial, our team built a data storage, edge computing, and decision support system. However, to achieve real-time decision support, real-time data transmission is also needed along with the system to achieve clinical decision needs. Therefore, this study aims to integrate the AI predictive model with edge computing to build an overall decision support system for intradialytic hypotension in the real-world clinical environment; we have developed a system architecture to deploy the system. Method Development of the IDH decision support system: We developed a complete IDH prediction system to be implemented in dialysis centers and hospitals, which incorporates patient data retrieval to AI prediction models to the web application and monitoring dashboard for medical professionals. The system architecture includes a bridge service used to retrieve the HIS database (patient history), DIS database (dialysis information), and NIS database (nursing care). The data is stored in a data warehouse and then converted into a structured format for AI model training and prediction. Furthermore, an API Server, a communication service for the database and front-end web, is used to present the warning signals in real-time. Lastly, there is a multi-bed monitoring dashboard presenting personalized patient information, the incidence rate of hypotension and numerical prediction of the actual blood pressure value. Design and validation of the online AI prediction model The real-time AI prediction model is built through automatic machine-learning training, which retrains the model every week, utilizing the data of the past 180 days. The features of the input data include biochemistry lab data, nursing records, demographical characteristics, hemodialysis machine parameters and vital signs; it then uses the SMOTE method to fill in missing values and via RFECV to do feature extraction and selection. Lastly, the model is trained through improved RNN models of LSTM (Long short-term memory) and GRU (Gated Recurrent Unit) integrated with machine learning models of XGboost and LightGBM algorithms to generate a combination of binary classification and numerical prediction. Hypotension's output label is defined when systolic blood pressure drops below 90 mmHg. Finally, the validation of the AI machine learning models for binary classification used accuracy score, specificity and sensitivity, whereas the numerical prediction model used RMSE and MAE. Results With the implementation of the edge computing and decision support system, clinical and dialysis data of the patients upload directly to the data warehouse in real-time, and it presents the hypotension prediction information to the medical professional's computer. As a result, the online real-time AI prediction system for the binary classification model yielded an accuracy score, specificity and sensitivity of 0.95, 0.99 and 0.86, respectively, and the RMSE and MAE yielded 17.5 and 11.6, respectively. Furthermore, the model predicted within 15 seconds to ensure a real-time and automatic parameters update. Conclusion Introducing and implementing this integrated intelligence AI prediction system reduces the occurrence of hypotension in dialysis, improves patient safety and satisfaction of patients, and enhances the work environment of medical professionals. Furthermore, this framework and validation can also be provided to home dialysis patients to improve their quality of care. In addition, this system architecture can also be extended to other medical conditions in developing intelligent decision support systems for practical clinical applications.

Abstract:Background: To date, most intradialytic hypotension (IDH) studies have proposed technologies to comprehensively predict the occurrence of IDH using the patient's baseline information and ultrafiltration (UF) information, but it is difficult to apply the technologies while identifying the patient's condition in real time. Methods: In this study, we propose an IDH indicator that uses heart rate (HR) change information to identify the patient's condition in real time and visually shows whether IDH has occurred. The data used were collected from 40 dialysis patients in a clinical trial conducted in the Artificial Kidney Unit at Yeungnam University Medical Center, Korea, from 18 July to 29 November 2023. Results: The IDH indicator infers changes in the patient's blood pressure during dialysis by analyzing the upper and lower maximum HRs based on the real-time average HR. Medical staff can respond to IDH in real time by looking at the IDH indicator, which visually expresses changes in the patient's HR. In addition, we propose a multilayer perceptron structure that inputs upper and lower maximum HR information based on the average HR for the time interval accumulated in real time. In learning using 40 min of data up to 5 min before IDH occurred, models using two and five layers showed excellent performance, with accuracy of 88.6% and 85.2%, respectively. Conclusions: By combining IDH visual indicators and the multi-layer perceptron method, medical staff can effectively respond to IDH in real time.

Abstract:Intradialytic hypotension (IDH) is a serious complication of hemodialysis (HD), with an incidence of more than 20%. IDH induces ischemic organ damage and even reduces the ultrafiltration and duration of HD sessions. Frequent attacks of IDH are a risk factor for death in HD patients. Malnutrition is common in HD patients and is also associated with mortality. Although the link between IDH episodes and malnutrition has been observed in practice, it has not been supported by the data. To study the relationship, we propose a promising hybrid model called BSCWJAYA_KELM, which is a wrapper feature selection method based on a variant of the JAYA optimization algorithm (SCWJAYA) and Kernel extreme learning machine (KELM). In this paper, we verify the optimization capability of the SCWJAYA algorithm in the model by comparing experiments with some state-of-the-art methods for IEEE CEC2014, IEEE CEC2017, and IEEE CEC2019 benchmark functions. The prediction accuracy of BSCWJAYA_KELM is validated by the public datasets and the HD dataset. In the experiments on the HD dataset, 1940 HD sessions of 178 HD patients are analyzed by the developed BSCWJAYA_KELM model. The key indicators selected from vast amounts of data are serum uric acid, dialysis vintage, age, diastolic pressure, and albumin. The BSCWJAYA_KELM method is a stable and excellent prediction model that can achieve a more accurate prediction of IDH.

Abstract:The primary objective of this study is to enhance the prediction accuracy of intradialytic hypotension in patients undergoing hemodialysis. A significant challenge in this context arises from the nature of the data derived from the monitoring devices and exhibits an extreme class imbalance problem. Traditional predictive models often display a bias towards the majority class, compromising the accuracy of minority class predictions. Therefore, we introduce a method called UnderXGBoost. This novel methodology combines the under-sampling, bagging, and XGBoost techniques to balance the dataset and improve predictive accuracy for the minority class. This method is characterized by its straightforward implementation and training efficiency. Empirical validation in a real-world dataset confirms the superior performance of UnderXGBoost compared to existing models in predicting intradialytic hypotension. Furthermore, our approach demonstrates versatility, allowing XGBoost to be substituted with other classifiers and still producing promising results. Sensitivity analysis was performed to assess the model's robustness, reinforce its reliability, and indicate its applicability to a broader range of medical scenarios facing similar challenges of data imbalance. Our model aims to enable medical professionals to provide preemptive treatments more effectively, thereby improving patient care and prognosis. This study contributes a novel and effective solution to a critical issue in medical prediction, thus broadening the application spectrum of predictive modeling in the healthcare domain.

Machine Learning Analysis of Time-Dependent Features for Predicting Adverse Events During Hemodialysis Therapy: Model Development and Validation Study

Abstract:Background Hemodialysis (HD) therapy is an indispensable tool used in critical care management. Patients undergoing HD are at risk for intradialytic adverse events, ranging from muscle cramps to cardiac arrest. So far, there is no effective HD device–integrated algorithm to assist medical staff in response to these adverse events a step earlier during HD. Objective We aimed to develop machine learning algorithms to predict intradialytic adverse events in an unbiased manner. Methods Three-month dialysis and physiological time-series data were collected from all patients who underwent maintenance HD therapy at a tertiary care referral center. Dialysis data were collected automatically by HD devices, and physiological data were recorded by medical staff. Intradialytic adverse events were documented by medical staff according to patient complaints. Features extracted from the time series data sets by linear and differential analyses were used for machine learning to predict adverse events during HD. Results Time series dialysis data were collected during the 4-hour HD session in 108 patients who underwent maintenance HD therapy. There were a total of 4221 HD sessions, 406 of which involved at least one intradialytic adverse event. Models were built by classification algorithms and evaluated by four-fold cross-validation. The developed algorithm predicted overall intradialytic adverse events, with an area under the curve (AUC) of 0.83, sensitivity of 0.53, and specificity of 0.96. The algorithm also predicted muscle cramps, with an AUC of 0.85, and blood pressure elevation, with an AUC of 0.93. In addition, the model built based on ultrafiltration-unrelated features predicted all types of adverse events, with an AUC of 0.81, indicating that ultrafiltration-unrelated factors also contribute to the onset of adverse events. Conclusions Our results demonstrated that algorithms combining linear and differential analyses with two-class classification machine learning can predict intradialytic adverse events in quasi-real time with high AUCs. Such a methodology implemented with local cloud computation and real-time optimization by personalized HD data could warn clinicians to take timely actions in advance.

Abstract:Early mortality after hemodialysis (HD) initiation significantly impacts the longevity of HD patients. This study aimed to quantify the effect sizes of risk factors on mortality using various machine learning approaches. A cohort of 3284 HD patients from the CRC-ESRD (2008–2014) was analyzed. Mortality risk models were validated using logistic regression, ridge regression, lasso regression, and decision trees, as well as ensemble methods like bagging and random forest. To better handle missing data and time-series variables, a recurrent neural network (RNN) with an autoencoder was also developed. Additionally, survival models predicting hazard ratios were employed using survival analysis techniques. The analysis included 1750 prevalent and 1534 incident HD patients (mean age 58.4 ± 13.6 years, 59.3% male). Over a median follow-up of 66.2 months, the overall mortality rate was 19.3%. Random forest models achieved an AUC of 0.8321 for first-year mortality prediction, which was further improved by the RNN with autoencoder (AUC 0.8357). The survival bagging model had the highest hazard ratio predictability (C-index 0.7756). A shorter dialysis duration (< 14.9 months) and high modified Charlson comorbidity index scores (7–9) were associated with hazard ratios up to 7.76 (C-index 0.7693). Comorbidities were more influential than age in predicting early mortality. Monitoring dialysis adequacy (KT/V), RAAS inhibitor use, and urine output is crucial for assessing early prognosis.

Abstract:Intradialytic hypotension (IDH) is common in hemodialysis patients and can lead to several complications. Risk factors for IDH include demographic characteristics, comorbidities, dialysis procedure factors, and so on. Clinical studies on predictive models for dialysis-induced hypotension have shown inconsistent results. This systematic review aims to evaluate published prediction models for IDH, analyzing their characteristics, predictors, efficacy, and the methodological quality and applicability. The protocol has been prepared using the Preferred Items for Systematic Review and Meta-analysis Protocols (PRISMA-P) guidelines. The systematic review protocol for IDH prediction in hemodialysis patients has been registered with the International Platform of Registered Systematic Review and Meta-analysis Protocols (INPLASY2023110081, DOI: 10.37766/inplasy2023.11.0081). A comprehensive search across five major databases (PubMed, Web of Science, Cochrane Library, CNKI, and Wanfang) will be conducted for studies on prediction models of IDH among hemodialysis patients. Two researchers will independently screen literature, extract data, and evaluate the bias risk and applicability of included studies using prediction modelling study tools. This systematic review will provide critical insights into the efficacy and quality of reporting of the IDH model in hemodialysis patients. This will guide clinical staff in selecting the most appropriate IDH prediction model and inform future research endeavors in IDH prediction.

Machine Learning for the Prediction of First-Year Mortality in Incident Hemodialysis Patients

Abstract:Rationale: The current mortality risk scores for hemodialysis (HD) patients employ regression techniques and have limited applicability and robustness. Objective: We aimed to develop a machine learning model utilizing clinical factors to predict first-year mortality in HD patients that could assist physicians in classifying high-risk patients.Methods: Training and testing cohorts consisted of 5351 incident HD patients from a single center and 5828 incident HD patients from 97 renal centers, respectively. The outcome was all-cause mortality during the first year of dialysis. eXtreme Gradient Boosting (XGBoost) was used for algorithm training and validation. Two models were established based on the data obtained at dialysis initiation (model 1) and data from 0-3 months after dialysis initiation (model 2). Ten-fold cross-validation was applied to each model. The C statistic (area under the receiver operating characteristic curve, AUC) was used to assess the predictive ability of the models.Results: In the training and testing cohorts, 585 (10.93%) and 764 (13.11%) patients, respectively, died during the first-year follow-up. Fifteen most important features were selected from 42 candidate features. The performance of model 1 (C statistic of 0.8318, 95% CI 0.7771 - 0.8374) was similar to that of model 2 (C statistic of 0.8530, 95% CI 0.8057 - 0.8593).Conclusions: We developed and validated two machine learning models to predict first-year mortality in HD patients. Abstract:Intradialytic hypotension (IDH) is the most common acute complication in hemodialysis (HD) sessions and is associated with increased morbidity and mortality in HD patients. To prevent the episode of IDH, it is critical to predict its occurrence. Chronic kidney disease-mineral and bone disorders (CKD-MBD) induce cardiac and vascular calcification, which impairs the compensatory mechanisms of blood pressure during HD. In this study, we proposed a feature selection framework called BSWEGWO_KELM to analyze 1940 records from 178 HD patients, which was based on an enhanced grey wolf optimization (GWO) algorithm and the kernel extreme learning machine (KELM). Then, global optimization experiments, together with feature selection experiments on public data sets and HD dataset, were performed to verify the effectiveness of the BSWEGWO_KELM method. The experimental results showed that the established BSWEGWO_KELM had the capability of screening out the key indicators such as dialysis vintage, mean arterial pressure (MAP), alkaline phosphatase (ALP), and intact parathyroid hormone (iPTH). Consequently, BSWEGWO_KELM can be applied as a practical and accurate method to predict IDH.

Abstract:Introduction: Cardiovascular (CV) events are the major cause of morbidity and mortality associated with blood pressure (BP) in hemodialysis (HD) patients. BP varies significantly during HD treatment, and the dramatic variation in BP is a well-recognized risk factor for increased mortality. The development of an intelligent system capable of predicting BP profiles for real-time monitoring is important. Our aim was to build a web-based system to predict changes in systolic BP (SBP) during HD. Methods: In this study, dialysis equipment connected to the Vital Info Portal gateway collected HD parameters that were linked to demographic data stored in the hospital information system. There were 3 types of patients: training, test, and new. A multiple linear regression model was built using the training group with SBP change as the dependent variable and dialysis parameters as the independent variables. We tested the model's performance on test and new patient groups using coverage rates with different thresholds. The model's performance was visualized using a web-based interactive system. Results: A total of 542,424 BP records were used for model building. The accuracy was greater than 80% in the prediction error range of 15%, and 20 mm Hg of true SBP in the test and new patient groups for the model of SBP changes suggested the good performance of our prediction model. In the analysis of absolute SBP values (5, 10, 15, 20, and 25 mm Hg), the accuracy of the SBP prediction increased as the threshold value increased. Discussion: This databae supported our prediction model in reducing the frequency of intradialytic SBP variability, which may help in clinical decision-making when a new patient receives HD treatment. Abstract:Abstract Background and Aims Intradialytic hypotension (IDH) plays an essential role in hemodialysis. IDH prevalence can be up to 8% to 40% [1], according to research. Furthermore, research shows that IDH positively correlates with severe complications such as non-occlusive mesenteric ischemia[2], critical limb ischemia[3] and tissue ischemia, which leads to irreversible damage, even increasing the chance of infection. Many causes and risk factors result in IDH, such as excessive weight gain during dialysis, cardiovascular dysfunction, and improper use of blood pressure-lowering drugs[1]. Therefore, it still relies on experienced medical professionals to monitor patients’ blood pressure to beware of the risk of developing IDH. However, this increases the workload on medical professionals and does not provide standard or more objective early warning indicators of hypotension for new medical professionals. Method Selection of experimental and control group: In this study, we included 81 subjects that received hemodialysis in Tainan City Municipal Annan Hospital from January 2021 to August 2022. The control group was retrospective data collected from January 2021 to August 2021. Then, from January 2022 to August 2022, we implemented our IDH predicting system and took subjects’ data from this interval as the experimental group to compare the incidence of IDH with the control group. IDH prediction system monitoring interface: After integrating with the hemodialysis dataset, our prediction system will predict patients’ systolic pressure and the likelihood of IDH in the next 30 minutes. As soon as the system detects that the patient will encounter hypotension, it sends warning signals, blood pressure and IDH incidence probability predictions to the monitoring interface to alert medical professionals. Furthermore, our system also supports multi-bed monitoring, which can significantly increase the efficiency of monitoring during hemodialysis. Results After implementing the IDH prediction system, the outcome yielded an IDH incidence of 6.12% in August 2022. Compared to the control group, when the IDH system was not yet implemented, which yielded an IDH incidence of 9.34%, it showed a reduction of 34.5% in IDH cases. Furthermore, the average reduction in IDH rate each month was 12%. Conclusion The present study demonstrated that implementing the IDH predicting model may efficiently reduce the incidence of IDH. However, along with the IDH warning multi-bed monitoring interface, medical professionals must develop a standard procedure to deploy when noticing the warning signals. As a result, this may further reduce the incidence of IDH during hemodialysis sessions.
