Abstract:ABSTRACT Background: The oral multikinase inhibitor regorafenib significantly improved overall survival vs placebo (HR 0.77, p = 0.0052) in patients with mCRC that progressed on available treatments in the randomized, double-blind, placebo-controlled CORRECT trial (Grothey, Van Cutsem et al. Lancet 2013). CORRELATE will characterize the safety and efficacy of regorafenib in real-world clinical practice. Trial design: This prospective, observational, multicenter trial (ClinicalTrials.gov identifier NCT02042144) will be conducted in routine clinical practice settings in more than 25 countries in Europe, Latin America, and the Asia-Pacific region. The trial will recruit 3,000 patients with mCRC previously treated with other approved therapies and for whom a decision has been made to treat with regorafenib. Patients will receive oral regorafenib 160 mg once daily for weeks 1–3 of each 4-week cycle. Dose interruptions and reductions will be permitted for the management of adverse events. The primary endpoint is the incidence of treatment-emergent adverse events, assessed using National Cancer Institute Common Terminology Criteria for Adverse Events. Secondary endpoints are overall survival, progression-free survival, disease control rate, health-related quality of life (assessed using the EQ-5D questionnaire), and healthcare resource use. Data sources will include medical records, routine measurements, and patient-reported outcome questionnaires. All patients receiving ≥1 dose of regorafenib will be included in the overall analysis. Two planned interim assessments will occur after 1,000 and 2,000 patients have been observed for ≥3 months. The final analysis will be performed when all patients have been followed for ≥18 months from the time they discontinue regorafenib (unless they withdrew from the trial early because of death, consent withdrawal, or patient/investigator decision to stop). Abstract:As a specific tumor marker, prostate-specific antigen (PSA) is widely used for the early diagnosis of prostate cancer. Sensitive and specific methods are required to improve the diagnostic accuracy of PSA detection. In the current study, we compared the immuno-polymerase chain reaction (immuno-PCR) method with the solid-phase proximity ligation assay (SP–PLA) with respect to the detection of PSA. Using oligonucleotide-labeled antibody probes, we used both immuno-PCR and SP–PLA to detect trace levels of PSA. The nucleic acid sequences can be monitored using real-time PCR. SP–PLA, however, was found to be superior in terms of both the detection limit and the dynamic range. To detect even lower levels of PSA, we used the loop-mediated isothermal amplification (LAMP) method to measure the levels of reporter DNA molecules in SP–PLA. The sensitivity of the LAMP method is 0.001pM, which is approximately 100-fold higher than the sensitivities of the other assays. The results suggest that an SP–PLA- and LAMP-based protocol with oligonucleotide-labeled antibody probes may have great application in detecting PSA or other proteins present at trace levels.
SERS-based biosensor for detection of f-PSA%: Implications for the diagnosis of prostate cancer

Abstract:In diagnosing prostate cancer and distinguishing it from other prostate diseases, the ratio of the concentration of free prostate-specific antigen (f-PSA) to total prostate-specific antigen (t-PSA), i.e., (f-PSA%) is more accurate than the concentration of t-PSA alone. Immunoassay based on surface-enhanced Raman scattering (SERS) frequency shift has been proven to be particularly suitable for detecting large biomolecules with high reproducibility. Along similar lines, the present study developed a SERS-based biosensor that simultaneously detects t-PSA and f-PSA. The 4-mercaptobenzoic acid (MBA) on the immunocapture substrate is coupled to the t-PSA antibody through the carboxyl group, and the combination of t-PSA induces the Raman frequency shifts of MBA. The immunocolloidal gold attached with f-PSA antibodies selectively capture the f-PSA that immobilized on the MBA-modified SERS substrates, allowing for f-PSA quantification according to the SERS intensities of the 5, 5'-Dithiobis (succinimidyl-2-nitrobenzoate) (DSNB) probe. The results show that f-PSA and t-PSA have good linear response in the concentration scale of 0.1-20 ng/mL, and 1-200 ng/mL, respectively. The biosensor combines Raman frequency shifts and intensities, which greatly simplifies traditional procedures for f-PSA% detection. All the results demonstrated the great potential of the proposed biosensor in highly reproducible and accurate diagnosis of prostate cancers.
A label-free fluorescent aptasensor based on HCR and G-quadruplex DNAzymes for the detection of prostate-specific antigen

Abstract:Prostate specific antigen (PSA) has been considered as the most potential serological biomarker for the early stage detection of prostate cancer. Here, a label-free fluorescence aptasensing strategy for detecting PSA based on hybridization chain reaction (HCR) and G-quadruplex DNAzymes has been developed. This designed strategy consists of three DNA probes, aptamer probe (AP), hairpin probe 1 (H1) and hairpin probe 2 (H2). In the presence of target PSA, the aptamer sequences in AP specifically recognized PSA to form a PSA-aptamer complex, causing an AP conformation change and thus releasing the initiator, which triggered the chain-like assembly of H1 and H2 that yielded extended nicked double-stranded DNA through HCR. Upon the addition of hemin, the G-rich segments at the end of H1 and H2 self-assembled into the peroxidase-mimicking hemin/G-quadruplex DNAzymes, which catalyzed the hydrogen peroxide-mediated oxidation of thiamine to give a fluorescence signal dependent on the concentration of PSA. Under optimal conditions, a limit of detection of 0.05 nM and a linear range from 0.1 nM to 1 nM (R2 = 0.9942) were achieved by this assay. In addition, other interfering proteins, such as IgG, AFP and CEA, did not produce any significant change in the fluorescence intensity response, indicating good selectivity of this sensor for PSA detection. Finally, this proposed aptasensor was successfully used for diluted serum samples.

Abstract:Highly sensitive analysis of cancer biomarkers demonstrates an important impact in early diagnosis and therapies of cancer. A novel surface-enhanced Raman scattering (SERS) based immunoassay using microfluidic technique was reported for rapid analysis of prostate-specific antigen (PSA) biomarker. It is a useful screening test to discriminate prostate cancer and other diseases related to prostate. A "sandwich" immunoassay based on SERS nanotags, PSA biomarkers, and magnetic beads was applied on a pump-free microfluidic sensor. Magnetic immunocomplexes are isolated and trapped at the detection chamber by a permanent magnet integrated into the chip. The PBS buffer washed magnetic immunocomplexes and brought the free gold nanoparticles to the downsteam channel for waste. Our results show a good linear response in the range from 0.01 to 100 ng mL-1. The limit of detection of the PSA level is estimated to be below 0.01 ng mL-1 using this chip. This detection level of PSA biomarker in human serum can be accomplished in 5 min without manual incubation and a heavy syringe pump. To the best of our knowledge, this is the first SERS-based immunoassay which applied a pump-free microfluidic chip as a detection platform. We believe that the proposed method reveals a valuable potential tool for the diagnosis of prostate cancer.

Abstract:Herein, a digital immunoassay was developed by counting highly confined gold nanorod (AuNR) probes with the aid of magnetic nanoparticles (MNPs). MNPs were conjugated with capture antibodies to concentrate the target analytes. AuNRs were conjugated with a mixture of the detection antibody and randomly selected DNA, where the detection antibody was applied to signal the target binding event and DNA was applied to provide sufficient negative charges. The bicomponent AuNR probes in the target-mediated immunocomplexes were eluted and confined onto a positively charged glass slide on the basis of electrostatic attraction for accurate counting. The combination of the magnetic concentration of the target analytes and the confinement of the DNA/antibody AuNR probes results in improved detection efficiency; a good linear range from 0.01 to 10 pg mL(-1) and a detection limit of 8 fg mL(-1) (similar to 226 aM) were achieved for the detection of prostate-specific antigen (PSA). The proposed method offers the simultaneous advantages of high sensitivity, digital quantification accuracy, lack of requirement for signal or target amplification, wide dose response curve and low detection cost; thus, it shows good potential in practical applications.

Abstract:A sensitive ratiometric electrochemical readout was designed with an immunoreaction-triggered DNA assembly for one-step, fast and flexible assay of protein biomarker. The sensing interface was prepared by immobilizing a ferrocene (Fc)-labeled hairpin DNA on a gold electrode. In the presence of DNA2-antibody2 (Ab2) and methylene blue (MB)-labeled DNA1-Ab1 probes, the addition of target protein could induce the sandwich immunoreaction among two probes and the protein to trigger the hybridization of DNA1 and DNA2, which subsequently unfolded the hairpin DNA to form a three-arm DNA structure on the sensing interface. The DNA assembly caused the departure of Fc from the electrode and the approach of MB to the electrode, which led to the signal decrease and increase of Fc and MB respectively for ratiometric readout. Using prostate specific antigen (PSA) as a model target, the ratiometric electrochemical assay showed a linear detection range from 0.01 to 200ng/mL with a detection limit of 4.3pg/mL (the mean signal of blank measures+3σ). By changing the affinity probe pairs this method could be easily expanded for other protein analytes, showing promising potential for point-of-care testing and extensive applications in bioanalysis.

Abstract:PSA is a member of low abundance proteins and serves as a critical indicator of the development and therapy efficacy for prostate cancer. In this study, a facile and high sensitive method was developed for serum PSA detection by integrating the immunomagnetic separation and cation exchange based signal amplification. On the basis of nanoparticle preparation and immunoprobe construction, PSA in serum was captured, separated by the immunomagnetic probe and then interacted with the quantum dots (QDs) based immunofluorescence probe; Zn2+ inside QDs was replaced by Ag+ within seconds, after which fluorescence signal was amplified by Fluozin-3, the Zn2+ responsive dye. Under optimized conditions, low detection limit (1.56 pg/mL), wide linear range (1.56-25 ng/mL) and good repeatability (intra-coefficient variation=3.18%) were achieved, which is superior to commercialized ELISA kit. These results demonstrated the potential of our high sensitive method for PSA detection in clinical.
A Sensitive Fluorescent Immunoassay for Prostate Specific Antigen Detection Based on Signal Amplify Strategy of Horseradish Peroxidase and Silicon Dioxide Nanospheres

Abstract:A simple, sensitive, and fluorescent immunoassay for the detection of prostate-specific antigen (PSA) based on horseradish peroxidase and silicon dioxide nanospheres as a signal amplification strategy has been described. In the design, the primary antibody (Ab1) of PSA was first immobilized on the 96-well plates via physical adsorption between polystyrene and hydrophobic groups of the antibody molecule. The silicon dioxide nanospheres (SiO2 NSs), with large surface area and good biocompatibility, were loaded with horseradish peroxidase (HRP) and horseradish peroxidase-labeled secondary antibodies (HRP-Ab2) as signal amplification systems. In the presence of PSA, a sandwich-type immunocomplex composed of Ab1-Ag-HRP-Ab2 had been formed. Fluorescence technology was employed to obtain the response signal of the immunoassay in the L-tyrosine and H2O2 systems. Experiment results showed that a strong and stable fluorescent signal at 416 nm (excitation wavelength: 325 nm) was observed, and the changes in fluorescent intensity were related to the levels of PSA. Under the optimal conditions, the relative fluorescence intensity was linear with the logarithm of PSA concentration from 0.03 to 100 ng·mL-1, with a detection limit of 0.01 ng·mL-1 (at a signal/noise ratio of 3). In contrast to other fluorescent immunoassays, the sandwich-type immunoreaction based on the high binding ELISA plates has the advantages of being simple, stable, and easy to operate, high selectivity, small sample quantity, etc., which can be widely used in the selective detection of a variety of targets, from DNA to proteins and small molecules. Such fluorescent immunoassays should be feasible for the fields of molecular diagnosis and other life science fields in the future.

Abstract:Prostate-specific antigen (PSA) is the most widely used biomarker for the early diagnosis of prostate cancer. Existing methods for PSA detection are burdened with some limitations and require improvement. Herein, we developed a novel microfluidic-electrochemical (μFEC) detection system for PSA detection. First, we constructed an electrochemical biosensor based on screen-printed electrodes (SPEs) with modification of gold nanoflowers (Au NFs) and DNA tetrahedron structural probes (TSPs), which showed great detection performance. Second, we fabricated microfluidic chips by DNA TSP-Au NF-modified SPEs and a PDMS layer with designed dense meandering microchannels. Finally, the μFEC detection system was achieved based on microfluidic chips integrated with the liquid automatic conveying unit and electrochemical detection platform. The μFEC system we developed acquired great detection performance for PSA detection in PBS solution. For PSA assays in spiked serum samples of the μFEC system, we obtained a linear dynamic range of 1-100 ng/mL with a limit of detection of 0.2 ng/mL and a total reaction time <25 min. Real serum samples of prostate cancer patients presented a strong correlation between the "gold-standard" chemiluminescence assays and the μFEC system. In terms of operation procedure, cost, and reaction time, our method was superior to the current methods for PSA detection and shows great potential for practical clinical application in the future.

Abstract:Nanozymes have been regarded as an excellent alternative for natural enzymes because of their high stability, low cost, and high activity. However, their use in disease diagnosis is still challenging, since the complex biological samples foul the nanozymes' surface and generate interference signals, thereby compromising the performance of nanozyme-based assays. Here, we report a dual-readout, CsPbBr3 NCs-based sandwich immunoassay for the detection of prostate specific antigen (PSA). Thanks to their excellent fluorescence and intrinsic peroxidase-like catalytic activity, the designed phospholipid-coated CsPbBr3 NCs (PL-CsPbBr3 NCs) served as an attractive dual signal generator (fluorescent and colorimetric), which is hardly achieved by other nanozymes. The Michaelis-Menten constant (KM) values of PL-CsPbBr3 NCs for H2O2 and tetramethylbenzidine are 2.85 mM and 1.42 mM, respectively. Meanwhile, the lipid shell around CsPbBr3 NCs not only greatly improves their aqueous stability, but also helps them resist the unspecific adsorption of biological impurities. Thus, the proposed dual-readout immunoassay enables precise, cost-effective, and anti-jamming detection of PSA in real serum samples with a low detection limit of 0.29 ng mL-1 (colorimetric) and 0.081 ng mL-1 (fluorescence). This enhanced immunoassay opens new insights for the application of perovskites in bioanalysis, especially for protein assay, holding great potential for disease diagnosis.

Abstract:A novel chemiluminescence (CL) imaging platform was constructed for prostate specific antigen (PSA) detection in a multiple signal amplifying manner. To construct the platform, the primary antibody for PSA was firstly immobilized on a O-ring area of a glass slide for recognizing the PSA. The horseradish peroxidase (HRP) and the secondary antibody of PSA (Ab2) functionalized Au NPs (HRP-Au NPs-Ab2) were modified on the platform through immunoreaction between PSA and Ab2. The excellent catalytic effect of Au NPs and HRP on the HRP-Au NPs-Ab2 to the luminol-H2O2 CL system provided the dual-signal amplification for PSA detection. To further enhance the sensitivity, tyramine signal amplification (TSA) strategy was introduced: tyramine-HRP conjugates were added into the O-ring reservoir and thus tyramine-HRP repeats formed in the presence of H2O2, generating a multiple signal amplification because of the large amounts of HRP on the sensing interface. The excellent performance of HRP-Au NPs-Ab2 and TSA strategy endows the CL platform with high sensitivity. The PSA was detected with a photomultiplier tube (PMT) and visually analyzed by a charge coupled device (CCD), respectively. The linear ranges of PMT and CCD for PSA are 0.1 – 100.0 ng mL-1 with a detection limit of 0.05 pg mL-1 and 0.5–100.0 ng mL-1 with a detection limit of 0.1 pg mL-1, respectively. The levels of PSA in several human serum samples were determined and the recoveries are ranged from 82.5%–117.0%. This CL immunosensing platform holds great potential for bioactive molecules detection visually and sensitively.

Abstract:For the purpose of detecting prostate specific antigen (PSA), a ratiometric electrochemical aptasensor has been developed based on target-aptamer binding triggered isothermal strand displacement polymerase reaction (ISDPR). A ferrocene (Fc)-modified hairpin probe 2 (Fc-HP2) was self-assembled on the modified electrode surface. One terminus of hairpin probe 1 (HP1) bound to PSA and formed the HP1/PSA complex, and the other hybridized with Fc-HP2. Then, the S1 strand hybridized with the stem of Fc-HP2 and was extended with the aid of phi29 DNA polymerase (phi 29) and deoxyribonucleoside triphosphates (dNTPs), and the HP1/PSA complex could be displaced to trigger a new cycle of polymerization reaction. Finally, numerous double-stranded DNAs (dsDNAs) were produced for methylene blue (MB) intercalation. With the raising of PSA concentrations, the electrochemical responses of Fc (IFc) and MB (IMB) decreased and increased, respectively. When IMB/IFc was used as the response signal for quantitative determination of PSA, the proposed strategy possessed a wide linear range (10 pg mL−1–100 ng mL−1) and a low detection limit of 3.2 pg mL−1. Moreover, the proposed aptasensor displayed good performance for PSA determination in spiked human serum samples, showing great application prospects in bioanalysis.

A Universal and Enzyme-Free Immunoassay Platform for Biomarker Detection Based on Gold Nanoparticle Enumeration with a Dark-Field Microscope

Abstract:Developing an enzyme-free, non-amplification strategy for biomarker detection with universality and easy implementation is of central importance in clinical diagnosis and therapeutic monitoring. Herein, we report for the first time a universal and enzyme-free magnetic bead-based sandwich-format immunoassay platform for biomarker detection by combining secondary antibody functionalized AuNPs and automatic AuNP counting readout. For the prostate specific antigen (PSA), the detection limit is found to be 1 ng mL-1, and the spike recoveries (n = 3) with 10% fetal bovine serum are 113.5% for 2 ng mL-1 and 107.7% for 10 ng mL-1. The assay also presents reasonable repeatability as indicated by the coefficient of variance of 13.1% with 5 measurements in 60 days. This strategy has been successfully applied to the determination of carcinoembryonic antigen (CEA) and alpha-fetoprotein (AFP), demonstrating the universality of this strategy. Our proposed non-amplification platform presents sensitivity comparable to that of the enzyme-linked immunosorbent assay (ELISA) with better repeatability; and more importantly, our method has better simplicity than most of the amplification-based methods, and thus is more suitable for routine analysis. The highlights of our work suggest that it is a promising method and would be potentially an alternative for ELISA in laboratories where routine analyses are intensively performed.
Nanogold-functionalized G-C3n4 Nanohybrids for Sensitive Impedimetric Immunoassay of Prostate-Specific Antigen Using Enzymatic Biocatalytic Precipitation

Abstract:This work reports on a new impedimetric immunosensing strategy for sensitive detection of prostate-specific antigen (PSA) in biological fluids. The assay was carried out on monoclonal anti-PSA capture antibody-modified glassy carbon electrode with a sandwich-type detection format. Gold nanoparticles-decorated g-C3N4 nanosheets (AuNP/g-C3N4), synthesized by the wet-chemistry method, were utilized for the labeling of polyclonal anti-PSA detection antibody and horseradish peroxidase (HRP). Upon target PSA introduction, the sandwiched immunocomplex could be formed between capture antibody and detection antibody. Followed by the AuNP/g-C3N4, the labeled HRP could catalyze 4-choloro-1-naphthol into benzo-4-chlorohexadienone. The as-generated insoluble product was coated on the electrode surface, thus increasing the Faradaic impedance of Fe(CN)6(4-/3)(-) indicator between the solution and the base electrode. Under the optimal conditions, the impedance increased with the increasing target PSA in the sample, and exhibited a wide linear range from 10pgmL(-1) and 30ngmL(-1) with a detection limit of 5.2pgmL(-1). A repeatability and intermediate precision of <14% was accomplished. The specificity and method accuracy in comparison with commercial PSA ELISA kit for analysis of human serum specimens were relatively satisfactory.

Abstract:We reported a novel multifunctional nanocone array as solid immunoassay plate and surface-enhanced Raman scattering (SERS) substrate for the simultaneous immunoassay of prostate-specific antigen (PSA) and thrombin based on microfluidic chip. In traditional immunoassays, microtiter plates are often used as solid capture plates to form immunocomplexes. However, the two-dimensional surface of the microtiter plate limits the efficiency of target antigen capture due to its steric inhibition. To solve this problem, a nanocone array covered with deposited gold film was successfully fabricated on glass slides that could be directly sealed with PDMS microfluidic chip. As a SERS active substrate, it can also provide a large surface area for capture aptamer conjugation to form a “sandwich” immunocomplex. On the strength of this integrated microfluidic chip, we could detect PSA and thrombin simultaneously with the detection limits of 0.01 ng mL−1 and 0.01 nM. Compared to previous reports, this detection strategy demonstrated the rapid and automatic performance with comparable detection limits. It is believed that the proposed microfluidic sensor has great potential for the early diagnosis of prostate cancer.

Fluorescent Immunosensor Based on CuS Nanoparticles for Sensitive Detection of Cancer Biomarker

Abstract:A novel fluorescent immunosensor was developed based on the use of CuS nanoparticles (CuS NPs) as labels for the highly sensitive detection of human prostate cancer biomarker prostate specific antigen (PSA). In the presence of CuS NPs, the non-fluorescent substrate o-phenylenediamine could be oxidized into the stable fluorescent product 2,3-diamiophenazine at physiological pH. Throughout the reaction, no other oxidizing agents (e.g. hydrogen peroxide) were needed. The relatively mild oxidation conditions made the immunoassay robust, reliable and facile. The proposed immunoassay exhibited high sensitivity and specificity for the detection of PSA. A linear relationship between the fluorescent signals and the concentration of PSA was obtained in the range of 0.5 pg mL(-1) to 50 ng mL(-1), with a detection limit of 0.1 pg mL(-1) (S/N = 3). The proposed fluorescent immunoassay can be used as a promising platform for the detection of a variety of other biomarkers.
Enhanced colorimetric immunoassay accompanying with enzyme cascade amplification strategy for ultrasensitive detection of low-abundance protein

Abstract:Methods based on enzyme labels have been developed for colorimetric immunoassays, but most involve poor sensitivity and are unsuitable for routine use. Herein, we design an enhanced colorimetric immunoassay for prostate-specific antigen (PSA) coupling with an enzyme-cascade-amplification strategy (ECAS-CIA). In the presence of target PSA, the labeled alkaline phosphatase on secondary antibody catalyzes the formation of palladium nanostructures, which catalyze 3,3',5,5'-tetramethylbenzidine-H2O2 system to produce the colored products, thus resulting in the signal cascade amplification. Results indicated that the ECAS-CIA presents good responses toward PSA, and allows detection of PSA at a concentration as low as 0.05 ng mL(-1). Intra- and inter-assay coefficients of variation are below 9.5% and 10.7%, respectively. Additionally, the methodology is validated for analysis of clinical serum specimens with consistent results obtained by PSA ELISA kit. Importantly, the ECAS-CIA opens a new horizon for protein diagnostics and biosecurity.

Abstract:Sensitive biomarker detection techniques are beneficial for both disease diagnosis and postoperative examinations. In this study, we report an integrated microfluidic chip designed for the immunodetection of prostate‐specific antigens (PSAs). The microfluidic chip is based on the three‐dimensional structure of quartz capillaries. The outlet channel extends to 1.8 cm, effectively facilitating the generation of uniform droplets ranging in size from 3 to 50 μm. Furthermore, we successfully immobilized the captured antibodies onto the surface of magnetic beads using an activator, and we constructed an immunosandwich complex by employing biotinylated antibodies. A key feature of this microfluidic chip is its integration of microfluidic droplet technology advantages, such as high‐throughput parallelism, enzymatic signal amplification, and small droplet size. This integration results in an exceptionally sensitive PSA detection capability, with the detection limit reduced to 7.00 ± 0.62 pg/mL.
