Abstract:There is growing interest in the use of systems-based risk assessment methods in Human Factors and Ergonomics (HFE). The purpose of this study was to test the intra-rater reliability and criterion-referenced concurrent validity of three systems-based risk assessment approaches: (i) the Systems-Theoretic Process Analysis (STPA) method; (ii) the Event Analysis of Systemic Teamwork Broken Links (EAST-BL) method; and, (iii) the Network Hazard Analysis and Risk Management System (Net-HARMS) method. Reliability and validity measures were obtained using the Signal Detection Theory (SDT) paradigm. Whilst STPA identified the highest number of risks, the findings indicate a weak to moderate level of reliability and validity for STPA, EAST-BL and Net-HARMS. There were no statistically significant differences between the methods across analyses. The results suggest that there is merit to the continued use of systems-based risk assessment methods following a series of methodological extensions that aim to enhance the reliability and validity of future applications. Practitioner summary The three risk assessment methods produced weak to moderate levels of stability and accuracy regarding their capability to predict risks. There is a pressing need to further test the reliability and validity of safety methods in Human Factors and Ergonomics.

Abstract:The use of robotics in the process industry has shown a stable, increasing trend, according to survey reports, reducing the exposure of operators to critical operations, mainly during maintenance. Moving from full manual (FM) to human-robot teaming (HRT) operations is expected to reduce the risks for operators and increase operational efficiency. The methods used to assess system performances require an adaptation. Traditional risk assessment techniques are no longer adapted to analyze the new way of working; the previous methods proposed for HRT systems have been criticized for their fragmentation, complexity, and lack of validation. This study proposed an integrated framework, including the qualitative and quantitative stages, to investigate the risk-based system performance and compare the full manual and human-robot teaming scenarios in pressurized tank inspection operations. The outputs demonstrated that the integrated framework worked for FM and HRT system performance assessment, considering multiple element types and their interdependencies, generating knowledge that can be exploited to reduce the system's risk and choose among different operational alternatives.

Abstract:There are increasing calls for the application of systems ergonomics methods in healthcare, although evidence for their utility and uptake is limited. In this study, 67 Australian healthcare workers participated in a six-month longitudinal study where they were trained to apply the AcciMap adverse event analysis and Net-HARMS risk assessment methods. Data were gathered in line with the RE-AIM (Reach, Efficacy, Adoption, Implementation, and Maintenance) evaluation framework, including rates of organisational uptake and method validity, perceived workload, usability, and barriers and facilitators to use in practice. Overall RE-AIM ratings for AcciMap were relatively high, and more moderate for Net-HARMS. Time constraints was the most frequently identified barrier to the use of both methods in practice, while there was more organisational resistance to Net-HARMS uptake. Facilitators for the use of both methods include providing quality training and mentorship, additional time and software resources, and dedicated job roles. The implementation of systems ergonomics methods in healthcare is poorly understood. This study evaluated the longitudinal application of accident analysis (AcciMap) and risk assessment (Net-HARMS) methods by 67 healthcare workers. AcciMap was highly rated, while further training, additional time, and dedicated job roles may further support method use in healthcare.

Abstract:There is increasing interest in applying systems Human Factors and Ergonomics (HFE) methods in sport. The Networked Hazard Analysis and Risk Management System (Net-HARMS) is a recently developed risk assessment (RA) approach that draws on systems HFE methods to identify risks across overall work systems. What makes Net-HARMS unique is its capacity to identify ‘emergent risks’ – those unexpected risks that arise when risks from across the system combine and interact. Whilst the method was developed to support the identification of risks in safety-critical systems, its capacity to identify risks in other domains has been noted. This study sets out to use Net-HARMS in the context of elite women’s road cycling to predict all the credible risks that could degrade optimal sports performance. The findings demonstrate that Net-HARMS can be effectively applied in a context other than safety, and that multiple risks threatening the performance of the cycling team were identified.

Abstract:Teleoperation is becoming an essential feature in automated vehicle concepts, as it will help the industry overcome challenges facing automated vehicles today. Teleoperation follows the idea to get humans back into the loop for certain rare situations the automated vehicle cannot resolve. Teleoperation therefore has the potential to expand the operational design domain and increase the availability of automated vehicles. This is especially relevant for concepts with no backup driver inside the vehicle. While teleoperation resolves certain issues an automated vehicle will face, it introduces new challenges in terms of safety requirements. While safety and regulatory approval is a major research topic in the area of automated vehicles, it is rarely discussed in the context of teleoperated road vehicles. The focus of this paper is to systematically analyze the potential hazards of teleoperation systems. An appropriate hazard analysis method (STPA) is chosen from literature and applied to the system at hand. The hazard analysis is an essential part in developing a safety concept (e.g., according to ISO26262) and thus far has not been discussed for teleoperated road vehicles.

Abstract:Despite extensive research to demonstrate the influence of psychosocial hazards on the development of musculoskeletal and mental health disorders (MSDs and MHDs), current research demonstrates a significant evidence to practice gap in this area. This study investigated current risk management practices for prevention of these complex disorders in the Victorian Heavy Vehicle (HV) sector. A preliminary Systems Theoretic and Accident Model and Processes (STAMP) framework was constructed to represent management of Work Health and Safety (WHS) in this sector, based on Leveson's generic socio-technical control model, and review of grey literature and peer reviewed papers. The model was refined and updated following feedback from 13 subject matter experts. Key findings included: 1) risk management practices for preventing MSDs and MHDs were limited to reductionist approaches, including a narrow focus on management of physical hazards (MSDs) and modifying individuals' behaviour (MSDs and MHDs); and 2) a high number of actors at the upper levels of the WHS system are a key barrier to the transfer of critical information to and from actors at the lower levels of the WHS system. In conclusion, to reduce the high prevalence of MSDs and MHDs in the Victorian HV sector, an urgent need exists for HV organisations to expand the current focus on safety and trauma outcomes to include health and wellbeing risks (such as MSDs and MHDs), and transition from a reductionist approach to systems thinking that reflects the complex aetiology of these disorders.

Abstract:Automatic crane is a complex system affected by the external environment and the internal components of the system, information fusion, software and hardware combination, and man-machine integration. The improvement of its automation and informatization proposes various challenges in the accident model construction and safety analysis. However, the safety analysis methods based on fault types consider that the occurrence of accidents is linear and ignore the correlation among components of the system. This paper adopts the system-theoretic accident model and process (STAMP) and system-theoretic process analysis (STPA) mode is to implement safety analysis of the automatic crane trolley running system (ACTRs). The paper starts from the identification of system-level losses and hazards, clarifies the function and internal logical control relationships of the system’s components, and then finds potential unsafe control actions (UCAs) and loss scenarios during the trolley running. The results show that the control requirements for the regular operation of the trolley running system can be analyzed in detail. Therefore, the STAMP/STPA can apply to the safety investigation of automatic cranes.

Abstract:Industrial accidents are a major concern for companies and families alike. It is a high priority to all stakeholders that steps be taken to prevent accidents from occurring. In this paper, three approaches to safety are examined: fault trees (FT), Bayesian networks (BN), and the Functional Resonance Analysis Method (FRAM). A case study of a propane feed control system is used to apply these methods. In order to make safety improvements to industrial workplaces high understanding of the systems is required. It is shown that consideration of the chance of failure of the system components, as in the FT and BN approaches, may not provide enough understanding to fully inform safety assessments. The FT and BN methods are top-down approaches that are formed from the perspective of management in workplaces. The FRAM methodology uses a bottom-up approach from the operational perspective to improve the understanding of the industrial workplace. The FRAM approach can provide added insight to the human factor and context and increase the rate at which we learn by considering successes as well as failures. FRAM can be a valuable tool for industrial safety assessment and to consider industrial safety holistically, by providing a framework to examine the operations in detail. However, operations should be considered using both top-down and bottom-up perspectives and all operational experience to make the most informed safety decisions.

Abstract:Risk assessment of the operations utilized in processing products and services always deals with uncertainties and complexities. The ever-evolving complex and dynamic circumstances make it very difficult to identify and analyze potential events affecting workers' safety and health. Our first study was on managing the risky situations of a dynamic environment, the transport and storage of residual hazardous materials with high variation in operational times. It showed that the dynamicity of operational functions has a direct relation to the risk of accidents and suggested that such environments require a system to decide whether to perform each new action on a suspected risk condition or not. A practical framework, engaged close to the variable functions involved in potential events, is needed to provide reliable measures for risk assessment. Based on these measures, this framework would help to make decisions at the right time and to take preventive actions. It would support the decision-making process by recognizing the risk-associated features of available information and offer continuously updated alternatives for appropriate actions to prevent unsafe operations. In our second study, we developed a brain-inspired decision-making system for the real-time configuration of dynamic environments. That decision-making system builds knowledge from the least to the most similarities between experienced states to determine the most appropriate action(s) to rapidly reorient risky operations to a safe condition. This paper aims to verify the second study's proposed system performance in the simulated environment discussed in our first study on residual hazardous materials transportation. We extract information, including the effective factors, from that first study and use it in the decision-making system to prevent risky transportation. This model would be useful in daily risk management as a practical framework for establishing safe operations in today's industrial environments that involve dangerous chemical or radioactive products.

Abstract:Multiple ergonomic risk assessment methods of unique structure are currently being used to direct industrial prevention initiatives focused on musculoskeletal injuries. In this study, the physical exposures required to perform an at-risk sawmill occupation were collected from 29 subjects via quantified means (surface electromyography and electrogoniometery) and used to calculate several ergonomic risk assessment methods. The aims of this study are to: 1) compare the output of the RULA, REBA, ACGIH TLV, Strain Index and OCRA ergonomic risk assessment methods, 2) examine the assessments' ability to differentiate between facilities reporting meaningfully different incidence rates, and 3) examine the effect of varying the definition of end range posture and exertion required on risk assessment scores. Risk level output assigned by all methods were not sensitive to inter facility differences in risk of injury, suggesting interpretation of risk index and component scores are needed to direct intervention. Components of all methodologies were sensitive to worker technique and facility assessed. Varying variable definition resulted in significantly different component, combined component and/or risk output scores in all methods assessed. The significant effect of posture and exertion variable definition suggests definitions taken to be interchangeable by work site evaluators are not equivalent.

Abstract:The level of automation in transportation systems is steadily on the rise, particularly in railways. However, this increase in automation implies a review of the previously established safety assessments, safety cases, and safety principles. Concretely, to ensure the overall safety of automated systems, it is crucial to provide conclusive assurance not only for their functional systems but also for any potential operational interactions that may arise unexpectedly. In this paper, we propose a risk assessment method to deal with the operational safety of the remote driving of freight trains. The established approach is based on the GAME principle (i.e. the French safety principle ' Globally at least equivalent '), which tends to assure the non-regression of the railway safety level. Firstly, the approach aims to perform a risk gap analysis with respect to driver's handbook procedures to identify the (operational) hazards and dangerous situations/scenarios generated by remote driving on the rail system. This analysis is completed by the identification of hazards due to the fact that the remote driving is not able to fully comply with the requirements of the driver's handbook. Then, the methodology is used to identify the risk control measures and the safety requirements to meet safety targets (i.e. maintaining the safety level at least equivalent to that obtained so far with in-cab driver). The obtained results are presented and structured in a table inspired by the preliminary operational hazards analysis (P-OHA) process.

Abstract:Safety has become of paramount importance in the development lifecycle of the modern automobile systems. However, the current automotive safety standard ISO 26262 does not specify clearly the methods for safety analysis. Different methods are recommended for this purpose. FTA (Fault Tree Analysis) and FMEA (Failure Mode and Effects Analysis) are used in the most recent ISO 26262 applications to identify component failures, errors and faults that lead to specific hazards (in the presence of faults). However, these methods are based on reliability theory, and they are not adequate to address new hazards caused by dysfunctional component interactions, software failure or human error. A holistic approach was developed called STPA (Systems-Theoretic Process Analysis) which addresses more types of hazards and treats safety as a dynamic control problem rather than an individual component failure. STPA also addresses types of hazardous causes in the absence of failure. Accordingly, there is a need for investigating hazard analysis techniques like STPA. In this paper, we present a concept on how to use STPA to extend the safety scope of ISO 26262 and support the Hazard Analysis and Risk Assessments (HARA) process. We applied the proposed concept to a current project of a fully automated vehicle at Continental. As a result, we identified 24 system- level accidents, 176 hazards, 27 unsafe control actions, and 129 unsafe scenarios. We conclude that STPA is an effective and efficient approach to derive detailed safety constraints. STPA can support the functional safety engineers to evaluate the architectural design of fully automated vehicles and build the functional safety concept.
Testing the reliability of accident analysis methods: a comparison of AcciMap, STAMP-CAST and AcciNet

Abstract:Accident analysis methods are used to model the multifactorial cause of adverse incidents. Methods such as AcciMap, STAMP-CAST and recently AcciNet, are systemic approaches that support the identification of safety interventions across sociotechnical system levels. Despite their growing popularity, little is known about how reliable systems-based methods are when used to describe, model and classify contributory factors and relationships. Here, we conducted an intra-rater and inter-rater reliability assessment of AcciMap, STAMP-CAST and AcciNet using the Signal Detection Theory (SDT) paradigm. A total of 180 hours' worth of analyses across 360 comparisons were performed by 30 expert analysts. Findings revealed that all three methods produced a weak to moderate positive correlation coefficient, however the inter-rater reliability of STAMP-CAST was significantly higher compared to AcciMap and AcciNet. No statistically significant or practically meaningful differences were found between methods in the overall intra-rater reliability analyses. Implications and future research directions are discussed. Practitioners who undertake accident analysis within their organisations should consider the use of STAMP-CAST due to the significantly higher inter-rater reliability findings obtained in this study compared to AcciMap and AcciNet, particularly if they tend to work alone and/or part of relatively small teams

Abstract:Statement of a problem. The increasing complexity in high-tech systems leads to potentially disastrous failure models and new kinds of safety issues. This led to the development of new approaches, for modeling accidents and risk management. In recent years, extended and hybrid approaches have been gaining popularity due to their effectiveness in decision-making for the design and operation of socio-technical systems. The proliferation of these approaches makes it difficult to select the appropriate approach for a particular system. Purpose. Conduct a comparative analysis of various hybrid approaches of accidents in complex systems, identify the strengths and weaknesses of each one, and study the feasibility of their use in risk management in socio-technical systems. Results. The paper analyzes the main approaches of accident modeling (FRAM, STAMP, failure tree, AcciMap) and their limitations in determining cause-effect relationships and dynamics of modern complex systems. New approaches to safety and accident modeling in sociotechnical systems are discussed, these approaches depend on combining several models into one hybrid approach: FuzzyFTA, FRAM-ANP, ACAT-FRAM, STAMP-HFACS, AcciMap-ANP, and SD-ET-FT-ANN. A review of hybrid approaches of accident modeling in complex systems and identify weaknesses and strengths, as well as the application field of each one of these approaches. Practical importance. This study will be a guide for researchers in the field of accident modeling and risk management in sociotechnical systems. It also concludes that it is necessary to use different approaches to risk management depending on the type of risk and the complexity of the system.
Human reliability analysis: A critique and review for managers

Abstract:In running our increasingly complex business systems, formal risk analyses and risk management techniques are becoming more important part to managers: all managers, not just those charged with risk management. It is also becoming apparent that human behaviour is often a root or significant contributing cause of system failure. This latter observation is not novel; for more than 30years it has been recognised that the role of human operations in safety critical systems is so important that they should be explicitly modelled as part of the risk assessment of plant operations. This has led to the development of a range of methods under the general heading of human reliability analysis (HRA) to account for the effects of human error in risk and reliability analysis. The modelling approaches used in HRA, however, tend to be focussed on easily describable sequential, generally low-level tasks, which are not the main source of systemic errors. Moreover, they focus on errors rather than the effects of all forms of human behaviour. In this paper we review and discuss HRA methodologies, arguing that there is a need for considerable further research and development before they meet the needs of modern risk and reliability analyses and are able to provide managers with the guidance they need to manage complex systems safely. We provide some suggestions for how work in this area should develop. But above all we seek to make the management community fully aware of assumptions implicit in human reliability analysis and its limitations.

Abstract:This paper presents a systematic literature review of risk assessment methods in the chemical process industry (CPI), focusing on process safety, process security, and resilience. We analyzed peer-reviewed articles from 2000 to 2022 using the PRISMA methodology and identified twelve predominant methods. Our findings reveal a shift towards dynamic, systemic-based assessments like the Functional Resonance Analysis Method (FRAM) and System-Theoretic Accident Model and Processes (STAMP). These methods are particularly effective at capturing the complexities of sociotechnical systems in the CPI. However, a significant observation from our review is the limited emphasis on the resilience paradigm within many existing methods when addressing both process safety and process security risks, which is crucial for preventing and recovering from disruptions. Given the evolving challenges in system safety and security threats, there is an urgent need for holistic methods that integrate process safety, process security, and resilience. Our review highlights the opportunity for further research to better prepare the industry for future challenges, ensuring safer, more secure, reliable, and resilient operations.

Abstract:Mining is an important contributor to the social and economic fabric of our society. However, it is also considered to be one of the most dangerous industries. Compared to manufacturing, mining is generally regarded as a more complex industry to work in, creating additional challenges for policy makers, researchers and practitioners. This paper first discusses the state of mining health and safety in Australia, followed by an examination of some of the complexities that characterizes the industry. Next one contemporary approach, permit-to-work systems (PTW), is introduced, followed by a review of the literature relating to its use as a health and safety risk control strategy. This is followed by a discussion of Resilience engineering (RE) as an innovation in health and safety management, and a case made for investigating RE as a safety management strategy using PTW systems. The paper concludes by suggesting a pragmatism research framework and two organizational theories upon which such research can be advanced.
Comparison of the HAZOP, FMEA, FRAM, and STPA Methods for the Hazard Analysis of Automatic Emergency Brake Systems

Abstract:As autonomous vehicle (AV) intelligence for controllability continues to develop, involving increasingly complex and interconnected systems, the maturity level of AV technology increasingly depends on the systems reliability level, also considering the interactions among them. Hazard analysis is typically used to identify potential system risks and avoid loss of AV system functionality. Conventional hazard analysis methods are commonly used for traditional standalone systems. New hazard analysis methods have been developed that may be more suitable for AV system-of-systems complexity. However, a comprehensive comparison of hazard analysis methods for AV systems is lacking. In this study, the traditional hazard analysis methods, hazard and operability (HAZOP) and failure mode and effects analysis (FMEA), as well as the most recent methods, like functional resonance analysis method (FRAM) and system-theoretic process analysis (STPA), are considered for implementation in the automatic emergency braking system. This system is designed to avoid collisions by utilizing the surrounding sensors to detect objects on the road, warning drivers with alerts about any collision risk, and actuating automatic partial/full braking through calculated adaptive braking deceleration. The objective of this work is to evaluate the methods with the unified theory of acceptance and use of technology (UTAUT) approach, in terms of their applicability to AV technologies. The advantages of HAZOP, FMEA, FRAM, and STPA, as well as the possibility of combining them to achieve systematic risk identification in practice, are discussed.

Abstract:Identifying and mitigating safety risks is paramount in a number of industries. In addition to guidelines and best practices, many industries already have safety management systems (SMSs) designed to monitor and reinforce good safety behaviors. The analytic capabilities to analyze the data acquired through such systems, however, are still lacking in terms of their ability to robustly quantify risks posed by various occupational hazards. Moreover, best practices and modern SMSs are unable to account for dynamically evolving environments/behavioral characteristics commonly found in many industrial settings. This article proposes a method to address these issues by enabling continuous and quantitative assessment of safety risks in a data-driven manner. The backbone of our method is an intuitive hierarchical probabilistic model that explains sparse and noisy safety data collected by a typical SMS. A fully Bayesian approach is developed to calibrate this model from safety data in an online fashion. Thereafter, the calibrated model holds necessary information that serves to characterize risk posed by different safety hazards. Additionally, the proposed model can be leveraged for automated decision making, for instance solving resource allocation problems -- targeted towards risk mitigation -- that are often encountered in resource-constrained industrial environments. The methodology is rigorously validated on a simulated test-bed and its scalability is demonstrated on real data from large maintenance projects at a petrochemical plant.
A probabilistic risk-based decision framework for structural health monitoring

Abstract:Obtaining the ability to make informed decisions regarding the operation and maintenance of structures, provides a major incentive for the implementation of structural health monitoring (SHM) systems. Probabilistic risk assessment (PRA) is an established methodology that allows engineers to make risk-informed decisions regarding the design and operation of safety-critical and high-value assets in industries such as nuclear and aerospace. The current paper aims to formulate a risk-based decision framework for structural health monitoring that combines elements of PRA with the existing SHM paradigm. As an apt tool for reasoning and decision-making under uncertainty, probabilistic graphical models serve as the foundation of the framework. The framework involves modelling failure modes of structures as Bayesian network representations of fault trees and then assigning costs or utilities to the failure events. The fault trees allow for information to pass from probabilistic classifiers to influence diagram representations of decision processes whilst also providing nodes within the graphical model that may be queried to obtain marginal probability distributions over local damage states within a structure. Optimal courses of action for structures are selected by determining the strategies that maximise expected utility. The risk-based framework is demonstrated on a realistic truss-like structure and supported by experimental data. Finally, a discussion of the risk-based approach is made and further challenges pertaining to decision-making processes in the context of SHM are identified.
