Abstract:We report port site and distant metastases of unsuspected gallbladder cancer after laparoscopic cholecystectomy diagnosed by positron emission tomography (PET) in two patients. Patient 1, a 72-year-old woman was diagnosed as cholelithiasis and cholecystitis and received laparoscopic cholecystectomy. Unsuspected gallbladder cancer was discovered with histological result of well-differentiated squamous cell carcinoma of the gallbladder infiltrating the entire wall. A PET scan using F-18-fluorodeoxyglucose (FDG-PET) before radical resection revealed residual tumor in the gallbladder fossa and recurrence at port site and metastases in bilateral hilar lymph nodes. Patient 2, a 69-year-old woman underwent laparoscopic cholecystectomy more than one year ago with pathologically confirmed unsuspected adenosquamous carcinoma of stage pT1b. At 7-mo follow-up after surgery, the patient presented with nodules in the periumbilical incision. Excisional biopsy of the nodule revealed adenosquamous carcinoma. The patient was examined by FDG-PET, demonstrating increased FDG uptake in the right lobe of the liver and mediastinal lymph nodes consistent with metastatic disease. This report is followed by a discussion about the utility of FDG-PET in the gallbladder cancer.
18F-fluorodeoxyglucose positron emission tomography/computed tomography as an effective diagnostic workup in cervical metastasis of carcinoma from an unknown primary tumor

Abstract:Purpose: The purpose of this study is to assess the potential of F-18-fluorodeoxyglucose ((18)FDG) positron emission tomography/computed tomography (PET/CT) imaging for the diagnosis of cervical metastasis of carcinoma of an unknown primary tumor (CUP) and to determine whether the maximum standardized uptake value (SUVmax) is a prognostic factor. Methods: Twenty-five consecutive patients with cervical metastasis of CUP were retrospectively analyzed by PET/CT between July 2007 and July 2011. Results: FDG PET/CT suggested a primary tumor in 21 out of 25 patients (84.0%). The sensitivity of FDG PET/CT in detecting the primary tumor was 73.3% (11 of 15), and the positive predictive value was 52.4% (11 of 21). The median follow-up duration of survival patients was 10.4 months (range: 0-30 months). The estimated 2-year overall survival rate of all patients was 50.0%. Univariate analyses did not reveal a significant difference in overall survival between the group of 11 patients identified by pathology and the 14 patients not identified by pathology (overall survival was 57.1% and 49.1%, respectively; p = 0.468). The median SUVmax was 7.6. In the log-rank test, patients with a low SUVmax (<= 7.0) in cervical lymph nodes had a significantly higher survival rate at 2 years (87.5% vs. 21.2%; p = 0.007) than patients with a high SUVmax (>7.0). Conclusions: Although our study was inconclusive due to the small sample size, our results suggest that FDG PET/CT may be an effective diagnostic workup in the cervical metastasis of carcinoma from an unknown primary tumor (UPT). In the present study, SUVmax of PET/CT in the cervical lymph node may serve as a prognostic factor of cervical metastasis of carcinoma from a UPT based on the limited number of patients. Further studies are needed to confirm these findings.

Abstract:The present study aimed to explore the value of fludeoxyglucose F 18 positron emission tomography-computed tomography (PET/CT) for the early prediction of chemotherapy remission rates and survival in patients with recurrent and metastatic breast cancer. A total of 24 patients diagnosed with recurrent or metastatic breast cancer between 2009 and 2014 were enrolled. All patients underwent a PET/CT examination prior to (PET/CT1) and following (PET/CT2) chemotherapy. Differences of PET/CT1 maximal standardized uptake values (SUVmax), PET/CT2 SUVmax, ΔSUVmax and the ΔSUVmax% between objective remission (OR) and non-OR groups were measured. Survival differences between OR and non-OR groups and the overall survival (OS) between metabolic responsive and metabolic non-responsive groups were analyzed. In the present study, it was revealed that ΔSUVmax and ΔSUVmax% were significantly higher in the OR group compared with the non-OR group (P<0.001). Overall survival was significantly prolonged in the OR and metabolic responder groups compared with their respective control groups (P<0.001 and P<0.01, respectively). ΔSUVmax% were significantly positively associated with OS (r2=0.266; P<0.01). In conclusion, PET/CT may be valuable for the early prediction of the chemotherapy efficacy and survival of patients with recurrent or metastatic breast cancer.
The evaluation of 18F-FDG PET for clinical application of gallbladder carcinoma and cholangiocarcinoma

Abstract:Objective The aim of this retrospective study was to investigate the value of PET with ~(18)F-FDG in the diagnosis,staging and restaging of gallbladder carcinoma and cholangiocarcinoma.Methods The results of ~(18)SF-FDG PET or PET/CT over 65 patients with gallbladder and bile ductal diseases from four hospitals were retrospectively reviewed.Among them,46 cases had gallbladder diseases(7 cases had PET before operation,39 cases after);19 cases had bile ductal diseases(2 before,17 after operation).All re- suits were analyzed by visual inspection and semi-quantitative method(the maximum SUV,SUV_(max)).Re- suits In 46 cases suffered from gallbladder diseases,7 cases with gallbladder carcinoma were diagnosed before surgery based on PET scans,and 3 of them were proved pathologically,2 lost follow-up,and 2 were false positive(1 adenoma and 1 cholecystitis).The other 39 gallbladder carcinoma underwent PET scan af- ter operation,21 had tumor residual and(or)metastases,while 15 were negative on PET scan,and 3 lost follow-up.The metastases were mainly confined in celiac lymph nodes and retroperitoneal lymph nodes.The sensitivity and specificity of PET in detecting gallbladder tumor residual and metastases were 91.3% and 84.6% respectively.In 19 cases with bile ductal diseases,2 were scanned before operation,1 cholangio- carcinoma was positive,1 adenocarcinoma was false negative.Nine of seventeen cholangiocarcinoma studied after operation had residual and(or)metastases,1 was negative,and 7 lost follow-up.The metastases were mainly confined in liver.The sensitivity and specificity of PET in detecting bile ductal tumor residual and metastases were 9/9 and 1/1.Conclusions ~(18)F-FDG PET is a useful tool in diagnosing gallbladder carci- noma and cholangiocarcinoma,and is of particular value in post-operation restaging.

Abstract:Background: The incidence of gallbladder adenocarcinoma (GBA) is relatively low, yet it exhibits a high degree of malignancy and a significantly low 5-year survival rate. The aim of this study was to investigate the prognostic value of pretreatment 2-[18F]fluoro-D-glucose positron emission tomography {2-[18F]FDG PET} parameters in predicting outcomes for patients with GBA. Methods: In total, 67 patients with GBA who underwent 2-[18F]FDG PET/computed tomography (CT) before treatment were retrospectively analyzed at Chinese PLA General Hospital from January 2012 to June 2022. All patients were diagnosed by pathology, and their baseline characteristics and clinical data were collected. The metabolic PET parameters of the primary and metastatic lesions were measured, including the maximum and average standardized uptake values (SUVs), metabolic tumor volume (MTV), and total lesion glycolysis (TLG). The prognostic significance of metabolic parameters and other clinical variables was assessed using Cox proportional hazards regression models. Differences in progression-free survival (PFS) and overall survival (OS) in relation to metabolic parameters were examined using the Kaplan-Meier method. Results: During a median follow-up period of 14.2 months, 43 patients (64.2%) experienced tumor recurrence or progression, and 38 patients (56.7%) died of cancer. In the univariate Cox regression analysis, liver parenchymal invasion (P=0.001), lymph node metastasis (P=0.007), distant metastases (P=0.049), tumor differentiation (P=0.028), surgery (P=0.014), carcinoembryonic antigen (CEA) level (P=0.030), carbohydrate antigen 19-9 (CA19-9) level (P=0.003), TLG (P=0.005), MTV (P<0.001), sum of the TLGs of the primary and metastatic lesions (total TLG, tTLG) (P=0.001), and sum of the MTVs of the primary and metastatic lesions (total MTV, tMTV) (P<0.001) were significant predictors of PFS. In multivariate analysis, MTV was an independent predictor of PFS [hazard ratio (HR) =2.785; 95% confidence interval (CI): 1.204-6.441; P=0.017]. In the univariate Cox regression analysis, liver parenchymal invasion (P=0.001), lymph node metastasis (P=0.027), distant metastases (P=0.036), tumor differentiation (P=0.047), surgery (P=0.002), neutrophil-to-lymphocyte ratio (NLR) (P=0.011), CEA level (P=0.036), CA19-9 level (P<0.001), TLG (P=0.007), MTV (P<0.001), tTLG (P=0.003), and tMTV (P<0.001) were significant predictors of OS. In the multivariate analysis, higher CA19-9 levels >37 U/mL and a greater tMTV (HR =2.961; 95% CI: 1.092-8.024; P=0.033) were predictive of OS. Conclusions: Our study results suggest that pretreatment 2-[18F]FDG PET parameters can not only assist in the diagnosis of patients with GBA but may also serve as predictive factors for the prognosis of these patients and should thus be applied in their treatment.
Imaging Gallbladder Lesions: What Can Positron Emission Tomography/Computed Tomography Add to the Conventional Imaging Approach?

Abstract:Objective: Incidental gallbladder lesions are common in imaging studies, although it is not always easy to discriminate benign lesions from gallbladder cancer with conventional imaging procedures. The present study aims to assess the capacity of positron emission tomography/computed tomography (PET/CT) with 2-[ 18 F]FDG to distinguish between benign and malignant pathology of the gallbladder, compared with conventional imaging techniques (contrast-enhanced CT or magnetic resonance imaging). Methods: Positron emission tomography/CT and conventional imaging studies of 53 patients with gallbladder lesions were evaluated and visually classified as benign, malignant, or inconclusive. Agreement between PET/CT and conventional imaging was determined, and imaging findings were correlated with histology or follow-up. Positron emission tomography/CT images were also analyzed semiquantitatively (SUV max and maximum tumor-to-liver ratio [TLR max ]). The presence of adenopathies and distant metastases was assessed and compared between both imaging procedures. Results: According to histology or follow-up, 33 patients (62%) had a malignant process and 20 (38%) had benign lesions. Positron emission tomography/CT and conventional imaging showed a moderate agreement ( κ = 0.59). Conventional imaging classified more studies as inconclusive compared with PET/CT (17.0% and 7.5%, respectively), although both procedures showed a similar accuracy. Malignant lesions had significantly higher SUV max and, especially, TLR max (0.89 and 2.38 [ P = 0.00028] for benign and malignant lesions, respectively). Positron emission tomography/CT identified more pathologic adenopathies and distant metastases, and patients with regional or distant spread had higher SUV max and TLR max in the gallbladder. Conclusions: Positron emission tomography/CT is accurate to distinguish between benign and malignant pathology of the gallbladder, with a similar performance to conventional imaging procedures but with less inconclusive results. Malignant lesions present higher SUV max and TLR max values.

Delayed post-diuretic 18F-FDG PET/CT for preoperative evaluation of renal pelvic cancer

Abstract:Background: Application of 18F-fluorodeoxyglucose positron emission tomography (18F-FDG PET) in urological oncology was relatively slowly due to the urinary elimination of 18F-FDG. We investigated whether delayed post-diuretic 18F-FDG PET/CT could be used for diagnosing renal pelvic cancer. Methods: 51 patients were included who underwent delayed post-diuretic 18F-FDG PET/CT for detecting renal pelvic space-occupying lesions. The comparations of delayed PET/CT parameters and clinical characteristics between renal pelvic cancer and benign polyp were investigated. Results: Among the 51 patients, 47 were found to have renal pelvic urothelial carcinoma, and 4 had benign polyp. ROC analysis identified the lesion maximum standardized uptake value (SUVmax) of 6.2 as the optimal cut-off value to distinguish from renal pelvic urothelial carcinoma to benign polyp. With the SUVmax cut-off of 6.2, the sensitivity, and specificity for predicting of renal pelvic urothelial carcinoma were 91.5% (43/47), and 100% (4/4). We also found a significant difference in tumor size between the positive (SUVmax > 6.2) and negative (SUVmax ≤ 6.2) PET groups in renal pelvic cancers. In patients with tumor size < 1.1 cm, the probability of being in the negative PET group was 75%. In such patients, a substantial proportion of renal pelvic cancer demonstrated negative SUVmax similar to that in patients with benign polyp. Conclusion: Delayed 18F-FDG PET/CT could be used for differentiating renal pelvic cancer from benign polyp. In patients with small tumor size, renal pelvic cancer may present low 18F-FDG uptake, mimicking the metabolic phenotypes of patients with benign polyp.

Abstract:Objective: To evaluate the diagnostic value of 18F-fluorodeoxy glucose (FDG) positron emission tomography (PET)/CT in patients with hepatic tumor. Methods: One hundred and sixteen patients with clinical diagnosis of hepatic tumor (85 males, 31 females; average age was 56 years old) had undergone 18F FDG PET/CT scan before treatment from January 2010 to June 2017, and the imaging characteristics were retrospectively analyzed. The pathological results were used as a gold standard for evaluating the diagnostic value of 18F FDG PET/CT. The maximum diameter (dmax), the maximum standardized uptake value (SUVmax) and the tumor to non-tumor SUV ratio (TNR) were measured. Two-sample t test, one-way ANOVA, paired Chi-square test and receiver operating characteristic (ROC) curve were used for data analysis. Results: Of the 116 patients, 11 patients had benign tumor, and 105 patients had malignant tumor. The dmax, SUVmax and TNR of 1 05 patients with malignant tumor were significant higher than those of 11 patients with benign tumor (all P < 0.05). The sensitivity and specificity of visual analysis in the diagnosis of benign tumors were 84.8％ and 54.5％, respectively. The sensitivity and specificity of the SUVmax method were 83.8％ and 63.6％, respectively. The sensitivity and specificity of the TNR method were 57.1％ and 90.9％, respectively. From the perspective of positive rate, visual analysis and SUVmax were better than TNR. The areas under receiver operating characteristic curve for diagnosis of intrahepatic cholangiocarcinoma and liver metastases by SUVmax and TNR methods were larger. Conclusion: 18F-FDG PET/CT is a useful examination in hepatic tumor, especially for intrahepatic cholangiocarcinoma and metastatic carcinoma.
The Clinical Value of Fluorine-18 Fluorodeoxyglucose Positron Emission Tomography/Computed Tomography in Postoperative Patients with Gastrointestinal Mucinous Adenocarcinoma

Abstract:ObjectiveTo evaluate the clinical value of 18F-fluorodeoxyglucose positron emission tomography and computed tomography (18F-FDG PET/CT) in postoperative patients with gastrointestinal mucinous adenocarcinoma. MethodsFrom July 2007 to March 2009, 30 patients who had previous surgical resection of histopathologically diagnosed gastrointestinal mucinous adenocarcinoma underwent 18F-FDG PET/CT scans in our center. The standard of reference for tumor recurrence, regional lymph node (LN) metastasis, peritoneal and distant metastasis consisted of histopathologic confirmation or clinical follow-up information for at least 6 months after PET/CT examinations. ResultsWith final diagnosis, tumor recurrences were confirmed in eight of the 30 patients (26.7%). If a maximum standardized uptake value (SUVmax) of 2.5 or more was used as a cut-off point, the sensitivity, specificity, and accuracy of PET/CT were 87.5, 77.3, and 80.0%, respectively. However, if an SUVmax of 4.0 or more was the criterion, the sensitivity, specificity, and accuracy were 25.0, 86.4, and 70.0%, respectively. A cut-off point of 2.5 showed a higher sensitivity (P=0.041), and there was no statistical difference in the specificity and the accuracy of these two criteria. For the diagnosis of metastasis in regional LNs and peritoneum, the detection rate was 95.2 and 86.4%, respectively. In addition, we followed up 20 patients with 26 suspicious distant lesions. The sensitivity, specificity, and accuracy were 58.3, 92.9, and 76.9%, respectively. Conclusion18F-FDG PET/CT may be effective to discriminate tumor recurrence, and to detect regional LNs, peritoneal and distant metastasis in postoperative patients with gastrointestinal mucinous adenocarcinoma.
18-FDG PET-CT should be included in preoperative staging of gall bladder cancer

Abstract:Background: Only few retrospective studies have looked into the ability of PET-CT to diagnose distant metastases in gall bladder cancer (GBC) patients with variable results. This study aims to determine the utility of PET -CT in potentially resectable GBC. Methods: All GBC patients with resectable disease on CECT chest, abdomen & pelvis were subjected to FDG- PET-CT scan. Incidental GBC was excluded. All additional findings and change in management plan was recorded. Results: Out of 149 patients, 99 (66.4%) were females and the mean age was 56.7 ± 11.0 years,. After PET scan, additional findings were seen in 46/149 (30.9%) patients and it lead to change in management plan in 35 (23.4%) patients due to the presence of distant metastases. Impact of PET scan in changing the stage was higher in patients having node positive disease on CECT (26/96, 27%) as compared to node negative patients (9/53, 16.9%), but this difference was not statistically significant (p = 0.233). After assessment on CECT, 76 patients were planned for NACT in view of locally advanced disease but after PET-CT in these patients, the management plan changed to palliative chemotherapy in 26 (34.2%) cases whereas it changed in only 9 out of 73 (12.3%) patients who were planned for upfront surgery (p = 0.003). Conclusion: Our results show that preoperative staging workup for GBC should include PET-CT as it changed the management plan in approximately one-fourth of all resectable GBC patients and in one-third of locally advanced cases.
Performance of 18F-FDG PET/CT as a postoperative surveillance imaging modality for asymptomatic advanced gastric cancer patients

Abstract:ObjectiveThe purpose of this study was to investigate the diagnostic performance of postoperative fluorine-18 fluoro-2-deoxy-d-glucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) as a surveillance modality for advanced gastric cancer patients who were asymptomatic and negative by conventional follow-up.MethodsWe retrospectively collected 46 advanced gastric cancer patients who received approximately 1-year-postoperative 18F-FDG PET/CT surveillance following curative resection (mean age 60.6 ± 11.5 years). 18F-FDG PET/CT was interpreted by nuclear medicine physicians who were blind to the clinical information. Final confirmation was determined by clinical follow-up using tumor markers, conventional CT scan, upper gastrointestinal endoscopy and with/without subsequent histopathologic diagnosis.ResultsFour patients developed recurrence (8.7 %; 1 local and 3 distant recurrences). For local recurrence, 18F-FDG PET/CT found four hypermetabolic lesions and one was local recurrence. For distant recurrence, seven hypermetabolic lesions were found in six patients and true-positive was three lesions. False-positive cases were mainly turned out to be physiologic small bowel uptake. Regardless of the recurrence site, the sensitivity, specificity, positive predictive value and negative predictive value of 18F-FDG PET/CT were 100 % (4/4, 95 % confidence interval (CI) 39.6–100 %), 88.1 % (37/42, 95 % CI 73.6–95.5 %), 44.4 % (4/9, 95 % CI 15.3–77.3 %) and 100 % (37/37, 95 % CI 88.3–100 %), respectively in the patient-based analysis.ConclusionOur study showed good specificity of postoperative surveillance 18F-FDG PET/CT for detecting recurrence. Careful caution should be made for interpreting some false-positive hypermetabolic lesions in postoperative 18F-FDG PET/CT, especially at the local anastomosis site.

Abstract:Objective The aim of this study was to evaluate the utility of 3-deoxy-3-F-18-fluorothymidine (F-18-FLT) PET/computed tomography (CT) imaging in the pretreatment evaluation of metastatic gastric cancer in comparison with F-18-fluorodeoxyglucose (F-18-FDG) PET/CT imaging. Methods A total of 39 metastatic gastric cancer patients were enrolled in the study. Attenuation-corrected whole-body F-18-FLT and F-18-FDG PET/CT (low-dose CT) imaging was performed on two consecutive days before chemotherapy. Results Accumulation of focal activity was visible in primary tumors on F-18-FLT PET/CT in 36/39 patients and on F-18-FDG PET/CT in 37/39 patients, with sensitivities of 92.3 and 94.9%, respectively. Further, three of the 36 FLT-avid primary tumors were almost undetected because they were covered by a high background hepatic uptake. Because of the high physiological uptake of F-18-FLT in the liver [median maximum standardized uptake value (SUVmax) 5.5, range 4.5-8.3] and the bone marrow (median SUVmax 14.8, range 10.8-22.0), the sensitivity of F-18-FLT PET/CT versus F-18-FDG PET/CT for detecting liver metastases and bone metastases was 30.0% (6/20) versus 100% (20/20) and 1/5 (20.0%) versus 5/5 (100%), respectively (P<0.05). Metabolically positive findings of lymph node, peritoneal, and ovarian metastases were similar between the two modalities: 96.8% (30/31) versus 93.5% (29/31), 89.5% (17/19) versus 94.7% (18/19), and 90.9% (10/11) versus 90.9% (10/11) of patients for F-18-FLT versus F-18-FDG, respectively (P>0.05). Conclusion F-18-FLT PET/CT imaging is not recommended for pretreatment assessment of metastatic gastric cancer as it is not competent enough to evaluate liver and bone metastases; moreover, the high background hepatic uptake may cover the gastric primary tumors located adjacent to the liver. Implications of False Negative and False Positive Diagnosis in Lymph Node Staging of NSCLC by Means of 18F-FDG PET/CT

Abstract:BACKGROUND:Integrated ¹⁸F-fluorodeoxyglucose positron emission tomography/computed tomography (¹⁸F-FDG PET/CT) is widely performed in hilar and mediastinal lymph node (HMLN) staging of non-small cell lung cancer (NSCLC). However, the diagnostic efficiency of PET/CT remains controversial. This retrospective study is to evaluate the accuracy of PET/CT and the characteristics of false negatives and false positives to improve specificity and sensitivity. METHODS:219 NSCLC patients with systematic lymph node dissection or sampling underwent preoperative PET/CT scan. Nodal uptake with a maximum standardized uptake value (SUV(max)) >2.5 was interpreted as PET/CT positive. The results of PET/CT were compared with the histopathological findings. The receiver operating characteristic (ROC) curve was generated to determine the diagnostic efficiency of PET/CT. Univariate and multivariate analysis were conducted to detect risk factors of false negatives and false positives. RESULTS:The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of PET/CT in detecting HMLN metastases were 74.2% (49/66), 73.2% (112/153), 54.4% (49/90), 86.8% (112/129), and 73.5% (161/219). The ROC curve had an area under curve (AUC) of 0.791 (95% CI 0.723-0.860). The incidence of false negative HMLN metastases was 13.2% (17 of 129 patients). Factors that are significantly associated with false negatives are: concurrent lung disease or diabetes (p<0.001), non-adenocarcinoma (p<0.001), and SUV(max) of primary tumor >4.0 (p=0.009). Postoperatively, 45.5% (41/90) patients were confirmed as false positive cases. The univariate analysis indicated age > 65 years old (p=0.009), well differentiation (p=0.002), and SUV(max) of primary tumor ≦4.0 (p=0.007) as risk factors for false positive uptake. CONCLUSION:The SUV(max) of HMLN is a predictor of malignancy. Lymph node staging using PET/CT is far from equal to pathological staging account of some risk factors. This study may provide some aids to pre-therapy evaluation and decision-making.
Diagnostic performance of FDG PET/CT for surveillance in asymptomatic gastric cancer patients after curative surgical resection

Abstract:PurposeThe present study evaluated the diagnostic performance of 2-[18F] fluoro-2-deoxy-d-glucose (FDG) positron emission tomography/computed tomography (PET/CT) for surveillance in asymptomatic gastric cancer patients after curative surgical resection.MethodsWe retrospectively recruited 190 gastric cancer patients (115 early gastric cancer patients and 75 advanced gastric cancer patients) who underwent 1-year (91 patients) or 2-year (99 patients) postoperative FDG PET/CT surveillance, along with a routine follow-up program, after curative surgical resection. All enrolled patients were asymptomatic and showed no recurrence on follow-up examinations performed before PET/CT surveillance. All PET/CT images were visually assessed and all abnormal findings on follow-up examinations including FDG PET/CT were confirmed with histopathological diagnosis or clinical follow-up.ResultsDuring follow-up, 19 patients (10.0 %) developed recurrence. FDG PET/CT showed abnormal findings in 37 patients (19.5 %). Among them, 16 patients (8.4 %) were diagnosed as cancer recurrence. Of 153 patients without abnormal findings on PET/CT, three patients were false-negative and diagnosed as recurrence on other follow-up examinations. The sensitivity, specificity, positive predictive value, and negative predictive value of FDG PET/CT were 84.2 %, 87.7 %, 43.2 %, and 98.0 %, respectively. Among 115 early gastric cancer patients, PET/CT detected recurrence in four patients (3.5 %) and one patient with local recurrence. Among 75 advanced gastric cancer patients, PET/CT detected recurrence in 12 patients (16.0 %), excluding two patients experiencing peritoneal recurrence. In addition, FDG PET/CT detected secondary primary cancer in six (3.2 %) out of all the patients.ConclusionsPost-operative FDG PET/CT surveillance showed good diagnostic ability for detecting recurrence in gastric cancer patients. FDG PET/CT could be a useful follow-up modality for gastric cancer patients, especially those with advanced gastric cancer. However, further careful evaluation is needed because of false-positive findings on PET/CT.

Evaluation of 68Ga-FAPI PET/CT and 18F-FDG PET/CT for the diagnosis of recurrent colorectal cancers

Abstract:Objective: The present study aimed to compare the diagnostic value of gallium-68-labeled fibroblast activation protein inhibitor positron emission tomography/computed tomography (68Ga-FAPI PET/CT) and fluorine-18-labeled fluorodeoxyglucose PET/CT (18F-FDG PET/CT) for detecting recurrent colorectal cancers (CRCs). Materials and methods: Fifty-six patients (age: 18-80 years, 31 men and 25 women) with suspected recurrent CRC were enrolled and underwent 18F-FDG PET/CT and 68Ga-FAPI PET/CT sequentially within 1 week. The maximum standard uptake value (SUVmax), tumor-to-background ratio (TBR), and diagnostic accuracy were estimated and compared between the two modalities by using Student's t-test. The Wilcoxon signed-rank test was used to compare peritoneal carcinoma index (PCI) scores between the two imaging modalities. Results: 68Ga-FAPI PET/CT showed higher sensitivity for detecting recurrence (93 % vs. 79 %); lymph node metastasis (89 % vs. 78 %), particularly peritoneal lymph node metastasis (92 % vs. 63 %); and metastatic implantation on the intestinal wall (100 % vs. 25 %) compared to 18F-FDG PET/CT. However, 68Ga-FAPI PET/CT showed lower sensitivity for detecting bone metastasis (67 % vs. 100 %). The mean SUVmax values of peritoneal metastases and metastatic implantation on the intestinal wall were 4.28 ± 2.70 and 7.58 ± 1.66 for 18F-FDG PET/CT and 5.66 ± 1.97 and 6.70 ± 0.25 for 68Ga-FAPI PET/CT, respectively. Furthermore, 68Ga-FAPI PET/CT showed significantly higher TBR for peritoneal metastatic lesions (4.22 ± 1.47 vs. 1.41 ± 0.89, p < 0.0001) and metastatic implantation on the intestinal wall (5.63 ± 1.24 vs. 2.20 ± 0.5, p = 0.02) compared to 18F-FDG PET/CT. For the same patient, 68Ga-FAPI PET/CT yielded a more accurate PCI score and a greater area under the curve value for the receiver operating characteristic curve (p < 0.01) than 18F-FDG PET/CT. Conclusion: 68Ga-FAPI PET/CT was superior to 18F-FDG PET/CT for detecting recurrence and peritoneal metastases. Hence, we propose the combination of these two modalities for better clinical diagnosis and management of patients with CRC.

Abstract:Purpose: Hepatocellular carcinoma(HCC) is one of the most common malignancies in China, but its 18F-FDG-PET diagnosis was reported far from satisfactory. The aim of this study was to evaluate the value of 18F-2-fluoro-2-deoxy-D-glucose(18FDG) PET/CT in patients with HCC. Materials and Methods: 28 patients were divided into 3 groups. In Group 1, 13 HCCs had not received any treatment before; In Group 2, 9 cases were confirmed to have benign liver tumors. The remaining 6 patients (Group 3)were coming for follow-up post therapy for their HCCs. Images were compared with the histopathologic findings at surgery or with clinical follow-up findings. The sensitivity of 18FDG PET/CT was calculated and the influencing factors of standardized uptake value (suv) were analyzed. Results: The 18FDG PET/CT findings in Group 1 showed 7 ‘hot’ lesions in liver,(TypeⅠ), 4 and 1 of equal or less uptake than surrounding liver tissue( TypeⅡand Type Ⅲ), respectively. None in Group 2 showed TypeⅠuptake, but 4 TypeⅡand 5 Type Ⅲ, respectively. The sensitivity of 18FDG PET/CT diagnosis of HCC was 53.8%. The SUV had the relationship with the biological properties of HCCs. Five HCCs in Group 3 had TypeⅠ uptake, the remaining one had Type Ⅲ uptakes. The SUV related with the tumor residue. Conclusion: 18FDG PET/CT had obvious advantages in evaluating the biological properties of HCCs and in monitoring the response after therapy.

Abstract:Background/Aims: To evaluate the value FDG-PET/ CT on the pre-operative staging of pancreatic cancer and Its impact on clinical management. Methodology: From Deceinber 2006 to January 2013, data of pancreatic carcinoma patients who underwent surgical treatment at our center was collected retrospectively. MDCT and FDG-PET/CT were used separately to diagnose and stage the tumor. Pre-operation staging by MDCT with chest x-ray and by FDG-PET/CT was compared according to the final pathological staging. Results: A total of 79 histologially proven pancreatic cancer patients were enrolled in this study. FDG-PET/CT was more accurate in the detection of tumor (PET/CT vs. MDCT: 93.67% vs. 88.61% p =0.402). The SE (60.00%.vs. 24.00%, p=0.01) and accuracy (87.81% vs. 76.83%, p=0.015) of PET/CT to detect distant the metastasis is significantly higher than those of MDCT. FDG-PET/CT also sfidwed advantage over CT in the detection of metastatic lymph nodes (52.83% vs 16.98%, p<0.001; accuracy: 66.67% vs. 41.33, p=0.002) The extra staging information PET/CT provided could have skipped eight patients (10.13%) of unnecessary surgical exploration. Conclusion: FDG-PET/CT is an important staging procedure and helps to make the clinical decision for the patients with pancreatic carcinoma.
Diagnostic Value of 18F-FDG PET/CT in Patients with Pancreatic Neoplasm

Abstract:Objective To evaluate the diagnostic value of 18F-FDG PET/CT in patients with pancreatic neoplasm. Methods Fifty-three patients (36 males, 17 females; age:(60.3±8.9) years) who underwent 18F-FDG PET/CT examination for suspected pancreatic tumor were retrospectively analyzed. Thirty-two of them underwent dual-time point imaging. The characteristics of PET/CT images and serum CA19-9 were reviewed. Pathological results were used as a gold standard for evaluating the diagnostic value of PET/CT. Two-sample t test, paired t test and ROC curve analysis were used for data analysis. Results Thirty-nine patients were finally diagnosed with pancreatic cancer and the other fourteen patients with benign disease. With SUVmax cutoff value of 3.13, the sensitivity and specificity of single-time point 18F-FDG PET/CT for diagnosis of pancreatic cancer were 92.3%(36/39) and 9/14, respectively. The SUVmax was significant different between pancreatic cancer group and benign disease group (6.16±2.89 vs 3.37±1.58; t=4.46, P 0.05). With the SUVmax cutoff value of 3.3, the sensitivity, specificity and accuracy in the early scan were 87.0%(20/23), 6/9 and 81.2%(26/32), respectively. While with the SUVmax cutoff value of 3.0, the sensitivity, specificity and accuracy in the delayed scan were 95.7%(22/23), 5/9 and 84.4%(27/32), respectively. Conclusion 18F-FDG PET/CT is a useful tool in the differential diagnosis of benign from malignant pancreatic diseases.
