Sternalis Muscle: A Cadaveric Case Report of a Rare Variant With Multiple Branching

Abstract:The sternalis muscle is a rare anatomical variant located in the anterior thoracic wall. Understanding variations in the sternalis muscle anatomy is essential for clinicians, especially radiologists and surgeons to prevent misdiagnosis and avoid complications during surgical procedures in the anterior thoracic region. We present a unique case of bilateral branched sternalis muscles. On either side, the sternalis muscle lies deep to the breast and superficial fascia and superficial to pectoralis major muscle and pectoral fascia. Each sternalis muscle is branched into medial and lateral slips with the medial slip larger than the lateral slip. The medial slip of the right sternalis was larger than the medial slip of the left sternalis. The lateral slip of the left sternalis was larger than the lateral slip of the right sternalis. The lateral slip of the left sternalis muscle has a curved course with superior-lateral convexity and inferomedial concavity. Near its middle, the left sternalis lateral slip is branched into two smaller slips separated by a narrow cleft. The left sternalis muscle in our report is a new variant with multiple branching, which cannot be matched to any type of the previously described classifications. In this case report, we discussed the need of modification of the currently available sternalis classification system to accommodate all types of the previously reported sternalis muscles including the branching pattern of this muscle.

Abstract:The abnormal origin, presence of additional head and layered arrangement of fibers are the reported variations of sternocleidomastoid muscle in the past. In the present case we report a rare unusual origin of third head of the sternocleidomastoid muscle in a 60 year-old male cadaver. This additional head originated from the investing layer of cervical fascia in the roof of the subclavian triangle close to the clavicle and traversed obliquely upward, forward and fused with clavicular head. The insertion and nerve supply of the muscle was found to be normal. El origen anormal, presencia de una cabeza adicional y disposicin en capas de fibras son las variaciones reportadas del msculo esternocleidomastoideo. En el presente estudio, se presenta un origen poco habitual de la tercera cabeza del msculo esternocleidomastoideo en un cadver de sexo masculino de 60 aos. La cabeza supernumeraria se originaba en la lmina superficial de la fascia cervical en el techo del tringulo subclavio prximo de la clavcula y cruzaba oblicuamente hacia arriba y adelante para fusionarse con la cabeza clavicular. High Spatial Resolution Study of the Inner Environment around Two Young Planetary Nebulae

Abstract:We present observations of the dusty emission from the young planetary nebulae Hen 2-113 and CPD-56 obtained with VLT/NACO, VLTI/MIDI. The central stars of these two objects are Wolf-Rayet stars of the same spectral type [WC10]. They share an impressive number of characteristics and are located at similar distance, making the detection of any differences in their close environment of great interest. Hen 2-113 exhibits a clear ring-like structure of about 0.4 arcsec in diameter, superimposed to a more diffuse environment visible in L’, M’ and 8.7μm bands. No clear core could be detected for this object with MIDI through the N band. The dusty environment of CPD-568032 is much more compact, dominated by a bright, barely resolved, core whereas the visible nebula exhibits an amazing complexity. From MIDI 8.7μm acquisition images (dominated by PAHs emission), the extension and geometry of the core have been estimated and compared to the STIS/HST observations (De Marco et al., 1997 and 2002). Moreover, high SNR fringes at low level have been detected with projected baselines between 40 and 45 meters. This clear signal is interpreted in terms of the bright inner rim of a dusty disk exposed to the flux from the Wolf-Rayet star. The geometrical parameters of the N band flux distribution are well constrained by means of simple geometrical models and a simple radiative transfer model has been developed to extract the physical parameters of the disk. 1. Young PNs and B[e] stars The optical spectrum of many compact Planetary Nebulae (PNe) shows strong Balmer emission lines and forbidden emission lines but we will concentrate our discussion on the strong IR excess exhibited by these sources. First of all, it is important to note that the targets discussed in this paper, CPD-568032 (He 3-1333, hereafter CPD) and Hen 2-113 (He 3-1044, hereafter HEN), are not PNe which could be confused with B[e] star and classified as cPNB[e] stars or SymB[e] stars (Lamers et al. 1998). Nevertheless, their study is highly relevant on the subject of this workshop since the environment of these young PNe share many characteristics encountered on B[e] stars ones. Among them, the most striking one is the presence of an equatorial overdensity of dust material exhibited by a growing number of PNs thanks to the improvement of high spatial resolution instruments operating in the mid-IR domain. Abstract:PURPOSE:We report a unique case of duplicated posterior belly of digastric muscle and absence of omohyoid muscle, review literatures and discuss its clinical importance.MATERIALS AND METHODS:An abnormal strip of muscle was found during the routine functional neck dissection in a 58-year-old man, who suffered from moderately differentiated squamous cell carcinoma of right tongue. We check the anatomical features and search for similar variations in the past literatures.RESULTS:The abnormal strip of muscle was attached to mastoid process, passed anteroinferiorly into the infrahyoid muscles. The muscle was as long as, but narrower than the posterior belly of the digastric muscle. So far, only one case of this anomaly was reported. Meanwhile, the omohyoid muscle was confirmed to be absent in the right neck of this patient.CONCLUSIONS:To our knowledge, report of variations of both digastric muscle and omohyoid muscle, this variation mostly derives from abnormal development of the mesenchyme in the branchial arches. Attention should be paid to such variations, which might influence surgical procedures.

Abstract:Variant anatomy in the axillary region is of great clinical significance. It is one of the most frequently accessed regions for radical dissection surgery. During routine dissection of embalmed cadavers, we found a rare case of two accessory muscular slips emerging from the lateral border of latissimus dorsi (LD) and the inferolateral border of pectoralis major (PM), crossing the neurovascular structures in the axilla and merging distally together to the brachial fascia at the upper end of humerus below the bicipital groove. The accessory slip from LD is commonly referred to as the "axillary arch" in literature. We identified the accessory slip from the PM crossing over the axilla as pectoralis quartus. These aberrant slips can cause neurovascular compression in the axilla and can have clinical implications. Prior knowledge of the variant anatomy is the key to successful surgery in the axilla, thereby avoiding inadvertent injuries and post-operative complications.
A prevalence study of the atlantomastoid muscle

Abstract:Purpose: To contribute to the documentation and consideration of the prevalence of the atlantomastoid muscle in humans. Methods: Sixty formalin-fixed cadaveric sides were dissected for the presence of the atlantomastoid muscle (2 sides per donor). Laterality, origin, insertion, and other observable characteristics of the variant were recorded along with donor sex for comparative analyses. Results: The atlantomastoid muscle was observed in nine sides (15%): two left side only, three right side only, and 2 bilaterally. The origin of the muscle was consistent from the transverse process of the atlas, lateral to both the insertion of obliquus capitis inferior and the origin of obliquus capitis superior. In one instance, the origin of atlantomastoid was continuous with the most superior tendinous slip of levator scapulae. The insertion of each atlantomastoid variant was the mastoid process, however, the precise location was variable. Conclusion: The atlantomastoid muscle is a common accessory muscle of the suboccipital region; the muscle was present in 15% of the cadaveric sides dissected in this study. Despite the notable prevalence of atlantomastoid muscles, there is little current literature dedicated to its study. Here, we document the first new information related to the prevalence of atlantomastoid since 1964 and discuss the development and potential clinical significance of the muscle.

Abstract:During routine dissections of cadavers as part of the medical curriculum, we identified a rare unilateral variation in the brachial plexus on the right side of a female body donor. This variation consisted of four unusual changes to the regular pattering of nerve bundles and the dorsal scapular artery permeating the complex neural network. The variation included contributions of root C4 to the plexus by a root C4/C5 anastomosis, a rare fusion of the superior and middle trunks to a 'superomiddle' trunk, a preliminary, proximal branching of the suprascapular nerve off the C5 root. We further observed an accessory 'medial anterior division' branching off the fused upper and middle trunks merging with the anterior division of the inferior trunk forming the medial cord. The latter event potentially introduced nerve fibers from C5 to C7, which are absent in common patterns. We aim to relate these observations to previous categorizations and quantifications of brachial plexus patterns. We believe that the combination of different variations in this case resulted in a unique pattern. Since this observation was made in the dissection class, we further aim to raise awareness among medical students and anatomical instructors for the likelihood of variations to textbook patterns. This will hopefully foster an appreciation of uniqueness and individuality in the interaction with future patients demonstrating that proper preparation prior to surgical interventions is always a necessary prerequisite.

Unusual insertion of the levator scapulae muscle: a case report

Abstract:The current case report describes an uncommon insertion of the levator scapulae (LS), which broadly attaches to multiple structures, including the serratus posterior superior, ligamentum nuchae, and the 6th and 7th spinous processes of the cervical vertebrae. The most superior portion of this aponeurosis merged with the ligamentum nuchae at the C7 level. The middle and inferior portions of the head were inserted (or fused) into the tendon of the rhomboid minor as well as the fibers of the serratus posterior superior muscle. Consequently, recognizing this anatomical variation, characterized by an additional slip of the LS attaching to the 7th vertebra and other muscular structures, is crucial not only for anatomists but also for surgeons performing procedures on the posterior neck related to cervical or shoulder pain and cervical dystonia.

Abstract:A supraglottic laryngeal neuroendocrine tumour in a 71‐year‐old female is presented. In addition to the typical features of extra‐adrenal paragangliomas it showed unusual, numerous intracytoplasmic lumina and occasional true acini with accumulation of alcian blue and PAS positive secretory product in them which led originally to an erroneous diagnosis of metastatic adenocarcinoma. Ultrastructurally, the tumour was composed of light and dark chief cells containing varying numbers of dense‐core secretory granules. Intracytoplasmic lumina and true acini contained microvilli with glycocalyceal material and varying amounts of membranous and amorphous material, possibly secreted via small, smooth‐surfaced cytoplasmic vesicles but typical mucin granules were not seen. These features have not been previously described in laryngeal paragangliomas but are seen in endocrine tumours of other sites and have been used as an argument in favour of an endodermal rather than ectodermal origin for endocrine cells in those sites. Laryngeal paragangliomas are often malignant and the presence of spasmodic pain appears to be the most reliable indication of possible malignancy, histological criteria being as yet poorly defined.
Isolated Unilateral Total Absence of the Pectoralis Minor Muscle: A Rare Finding in Two Cases

Abstract:Anatomical variations are observed at times during a routine dissection process and some of them are clinically relevant as they can lead to certain clinical presentations or situations that are difficult to anticipate without the knowledge about their possibility. The unilateral non-syndromic complete absence of pectoral muscles is very rare. Their absence is always found to be associated with syndromes like Poland syndrome or Sprengel's deformity. During the routine anatomical dissection, we encountered two cases of non-syndromic complete unilateral absence of the pectoralis minor muscle. On further inspection of the cadaver in both the cases, no other bony (ribs, scapula), vascular, breast abnormalities, or muscular aplasia (fibers of the serratus anterior or pectoralis major) was noted. As the pectoralis minor muscle serves as the potential surgical landmark and can also be used as the myo-cutaneous flaps for facial reanimation surgeries and in thumb opponensplasty, the absence of the pectoralis minor muscle would come as a surprise for the surgeons during the process of harvesting the flap for these procedures, so the possibility of this kind of rare variation should be documented.

Abstract:The digastric, a prominent anatomic feature of the head and neck, exhibits clear ethnographic differences in the incidence of variation in its anterior belly (AB) and the relationship of its intermediate tendon (IT) to the stylohyoid (SH). There is a limited amount of information available regarding the digastric in the Chinese population, with most descriptions being individual cases. The present study aims to investigate the variability of AB, elucidate the relationship between IT and SH, and further address the arrangement of individual digastric muscles according to the type of AB and posterior belly (PB) of the digastric, as well as IT, in a Chinese population. Anatomic dissections of the head and neck were carried out bilaterally after student-performed neck dissections during a topographic anatomy course. The types of AB, PB, and IT were recorded to characterize and classify each digastric muscle. The incidence of AB variation was 21.05%. IT type I and IT type II accounted for 31.25% and 68.75%, respectively. The gross anatomy of the digastric was dominated by the arrangement of AB type I, IT type II, and PB type I (63.16%). In addition, 2 rare AB variations were identified. To the best of our knowledge, this is the first report describing individual digastric arrangements and anatomically defining the normal digastric arrangement in the Chinese population. Careful consideration of the variant and anomaly of the digastric is critical for both surgeons and radiologists to provide accurate diagnostic and therapeutic procedures.

Abstract:Abstract Elite gymnastics, and other sports where athletes and coaches are particularly concerned with aesthetic considerations, weight and shape, are fields within which the risk of eating disorders may be unusually high. Adolescent gymnasts, developing their own sense of self, at a time of life where body image concerns are common, often compete at the very top of the sport with a need to maintain a body shape and weight optimal for elite performance. Research into this field should address the range of sociological and ethical aspects of eating disorders in elite sport, their prevalence as well as the ethos of the sport itself. This paper addresses a range of conceptual, ethical and methodological issues relevant to conducting research in this sensitive yet important field.
Anatomical variation of the sympathetic trunk and aberrant rami communicantes and their clinical implications

Abstract:Surgical interventions involving the sympathetic trunk are increasingly performed to alleviate symptoms of several disorders such as hyperhidrosis. Anatomical variation has been highlighted as one of the main causes behind surgical failure and symptoms recurrence following surgeries conducted on the chain or its surroundings. This study therefore aimed to record anatomical variants within spinal segments C8-T10 of the sympathetic trunk. Thirty Thiel-embalmed cadavers were investigated bilaterally. The stellate ganglion was recorded on 29 sides. Its size was significantly greater in males and on the right side when the coalescence extended to the subsequent ganglion. The intrathoracic nerve of Kuntz was observed on 21 sides and was significantly more prevalent in males. There was a significant positive association between the presence of this nerve and the descending ramus in the first intercostal space. Aberrant rami found between spinal root C8 and the ventral ramus of the first intercostal nerve were introduced as rami communicantes superi. Aberrant rami communicantes were recorded 50 times in total, of which 70% were found in males. Descending rami showed the highest prevalence in upper intercostal levels, especially in males within the first intercostal space. Aberrant neuronal pathways in upper levels were significantly more prevalent when the stellate ganglion was present. The scientific literature has proven to be stochastic as results were significantly higher in past studies in regard to some sympathetic variants. Anatomical findings of the current study as well as the inconsistency of previous data should be acknowledged and considered for better surgical planning.

The Sternocleidomastoid Muscle Reverse Pad: A "Safety Net" in Catastrophic Tracheal Surgery Situation

Abstract:Background: This paper presents the outcomes of employing the inferiorly based rotated sternocleidomastoid muscle flap in complex tracheal reconstruction/repair scenarios, focusing on the key objectives of ensuring stable airway, functional digestive tract and patient survival. Methods: A retrospective analysis was performed for patients treated at two medical centers (A.O. San Camillo Forlanini, Rome, and A.O.U. Careggi, Florence) from 2011 to 2023, in which the sternocleidomastoid muscle (SCM) flap, detached from the mastoid and basicranium, was rotated on the lower pivot directly onto the repair site and pedicled to the sternal origin to ensure the continuity of the airway. Average postoperative hospital stay, follow-up period and patient survival were analyzed. Follow-up assessments encompassed bronchoscopies and CT scans conducted at intervals of 15 and 28 days, and subsequently at 3 and 9 months. Results: A total of five patients were enrolled in this study. These cases included one patient with anterior tracheal wall lesions with abundant tissue loss, one patient with an anterior wall necrosis due to descending cervical mediastinitis and three patients with extra-long tracheoesophageal fistulas (TEFs) (greater than 4.5 cm or >30% of the total tracheal length). In the case of the direct repair of a TEF with a proximal tracheal stenosis, the sternocleidomastoid muscle was used to reconstruct the tissue deficit caused by extensive loss of substance in the left lateral side of the tracheal wall. In case of repair through exclusion of the TEF, the sternocleidomastoid muscle was interposed between the visceral sutures after exclusion of the TEF by an endomechanical device, in one case even substituting the membranous part of the tracheal wall. Our technique allows rotation on the sternal head of the sternocleidomastoid muscle with the lowest rotation radius, pedicled to the sternal origin, detached from the mastoid process and superior nuchal line, thus providing optimal vascularization from the superior thyroid artery/external carotid artery and accessory vasculature from the suprascapular artery. Patients exhibited uneventful postoperative recovery concerning airway and digestive patency. The mean postoperative hospitalization duration was 41 days. The follow-up assessments were negative for postoperative complications. Conclusions: The use of sternocleidomastoid muscle flap was proposed to ensure repair and protection of the suture margin or to constitute a portion, as a scaffold, of the wall by leveraging the muscle's vascularization and thickness. This technique may be considered a leading component in managing complex situations in tracheal surgery.

Identification and characterization of a potentially novel dorsal cutaneous muscle in rodents

Abstract:In the course of performing a detailed dissection of adult rat to map the cutaneous nerves of cervical, thoracic, and lumbar levels a small and unexpected structure was isolated. It appeared to be a cutaneous striated muscle and was observed in both male and female rats and in mice but absent from cats and humans. With the skin reflected laterally from midline, the muscle lies closely apposed to the lateral border of the Thoracic Trapezius (Spinotrapezius) muscle and is easily missed in standard gross dissections. Focussed prosections were performed to identify the origin, insertion, and course of gross innervation. Identification of each of these elements showed them to be distinct from the nearby Trapezius and Cutaneous Trunci (Cutaneous Maximus in mouse) muscles. The striated muscle nature of the structure was validated with whole-mount microscopy.Consulting a range of published rodent anatomical atlases and gross anatomical experts revealed no prior descriptions. This preliminary report is an opportunity for the anatomical and research communities to provide input to either confirm the novelty of this muscle or refer to prior published descriptions in rodents or other species while the muscle, its innervation, and function are further characterized. Presuming this muscle is indeed novel, the name “Cutaneous Scapularis muscle” is proposed in accord with general principles of the anatomical field.

Abstract:Objective: This study aimed to investigate morphologic features of the sternocleidomastoid muscle (SCM) in children for pediatric head-neck surgeries. Methods: Magnetic resonance imaging images of 84 (46 males/38 females) pediatric patients aged ages 1 to 18 years (mean age: 9.38 ± 4.46 y) constituted the study universe. The size of SCM (including its height, width, and angulation) was measured, and the origin types of SCM were noted. Results: All patients had SCM bilaterally. The origin variation of SCM was observed in 22 sides (13.10%). The authors observed type 1 in 146 sides (86.90%), type 2a in 15 sides (8.90%), type 3 in 2 sides (1.20%), type 4 in 1 side (0.60%), and type 5 in 4 sides (2.40%). The absence and presence of SCM variation were not affected by sex ( P = 0.370) and side ( P = 0.360). All measured parameters were similar in right and left sides, and also in males and females, except from the angle between the junction of the sternal and clavicular heads of SCM. The angle was greater in males than females ( P = 0.004). All measured parameters increased with advancing age (between 1 and 18 y; P < 0.05). Conclusion: The authors’ SCM-related data may be useful for clinicians to estimate the size origin variations of SCM in children.

Abstract:The thyrohyoid muscle belongs to the infrahyoid group located in the carotid triangle. It normally originates from thyroid cartilage and inserts into hyoid bone. Quite often, it is continuous with the sternohyoid muscle. Furthermore, there are variants that have their origin in the cricoid cartilage only, however, this occurs very rarely. During anatomical dissection, a two-headed variant of this muscle was found. One head had its origin in the cricoid cartilage and the other in the thyroid cartilage. This variant of thyrohyoid had not been previously described in the available literature. Therefore, we believe that it may be referred to as the cricothyrohyoid muscle. As the thyrohyoideus is often used as a landmark during surgical procedures in the prelaryngeal area and as a muscle graft, a thorough knowledge of its anatomy and variation is extremely important. We speculate that the two-headed version of this muscle may be problematic during surgical procedures in this region, however, it may also provide more options as a muscular graft.
