Abstract:Oxygen saturation is a crucial metric used for monitoring patients with lung disease or other conditions who are at risk of hypoxemia. Recently, consumer wearable devices began incorporating oxygen saturation measurement tools, widening their use among individuals with lung disease. However, their performance has not been well characterized, particularly during blood oxygen desaturation, due to limitations in collecting comparison arterial blood gas measurements during exercise-induced hypoxemia. To address this knowledge gap, we performed a controlled oxygen desaturation study in nine healthy participants to mimic exercise-induced hypoxemia, reducing the blood oxygen saturation levels from 100% to 60% using a gas delivery system. We conducted a comparative analysis of the displayed oxygen saturation readings of both the Masimo MightySat Rx finger pulse oximeter and Apple Watch Series 7 with arterial blood oxygen saturation readings obtained from arterial blood gas samples. Both the Masimo MightySat Rx pulse oximeter and the Apple Watch Series 7 tended to overestimate the oxygen saturation. The Masimo pulse oximeter readings were more likely to fall within the acceptable error range than the Apple Watch (49.03% compared to 32.14%). Notably, both devices have limitations under low oxygen saturation levels (<88%) with an error of 1.96% and 4.99% for the Masimo and Apple Watch, respectively. Among the oxygen desaturation measurements at a desaturation rate larger than two percent per minute, a rate which is clinically correlated to sleep apnea, both devices increased their error slightly by 0.29% and 1.80% respectively for the Masimo and Apple Watch. However, no statistically significant increase was found.

Evaluation of Leading Smartwatches for the Detection of Hypoxemia: Comparison to Reference Oximeter

Abstract:Although smartwatches are not considered medical devices, experimental validation of their accuracy in detecting hypoxemia is necessary due to their potential use in monitoring conditions manifested by a prolonged decrease in peripheral blood oxygen saturation (SpO2), such as chronic obstructive pulmonary disease, sleep apnea syndrome, and COVID-19, or at high altitudes, e.g., during sport climbing, where the use of finger-sensor-based pulse oximeters may be limited. The aim of this study was to experimentally compare the accuracy of SpO2 measurement of popular smartwatches with a clinically used pulse oximeter according to the requirements of ISO 80601-2-61. Each of the 18 young and healthy participants underwent the experimental assessment three times in randomized order-wearing Apple Watch 8, Samsung Galaxy Watch 5, or Withings ScanWatch-resulting in 54 individual experimental assessments and complete datasets. The accuracy of the SpO2 measurements was compared to that of the Radical-7 (Masimo Corporation, Irvine, CA, USA) during short-term hypoxemia induced by consecutive inhalation of three prepared gas mixtures with reduced oxygen concentrations (14%, 12%, and 10%). All three smartwatch models met the maximum acceptable root-mean-square deviation (≤4%) from the reference measurement at both normal oxygen levels and induced desaturation with SpO2 less than 90%. Apple Watch 8 reached the highest reliability due to its lowest mean bias and root-mean-square deviation, highest Pearson correlation coefficient, and accuracy in detecting hypoxemia. Our findings support the use of smartwatches to reliably detect hypoxemia in situations where the use of standard finger pulse oximeters may be limited.
Performance of Wrist-Worn Pulse Oximeter for the Screening of Obstructive Sleep Apnea

Abstract:Introduction: A sleep apnea monitor (BM2000A) is a wrist-worn device that measures oxygen saturation and pulse rate during sleep. This study aimed to evaluate the efficacy of the watch-like BM2000A for screening obstructive sleep apnea (OSA). Materials and methods: 102 patients complaining of sleep breathing disorders were included; 81% were men and 19% were women. All participants underwent overnight simultaneous polysomnography (PSG) and BM2000A sleep monitoring. The number of apneas and hypopneas, apnea-hypopnea index (AHI), percentage of time spent with oxygen saturation under 90%, average oxygen saturation, lowest oxygen saturation, and duration of sleep were computed by the BM2000A and PSG. Then, these parameters were compared to validate the BM2000A. Results: All parameters, measured with BM2000A, had a good correlation (r ≥ 0.6, p < 0.0001) with PSG-derived indexes, except for sleep time (r = 0.19, p = 0.061) and hypopnea index (r = 0.4, p < 0.0001). AHI had the strongest correlation (r = 0.87, p < 0.0001). The mean difference between AHI values calculated with PSG and wrist-worn pulse oximeter (WPO) was -17.66 events/h (95% CI: -50.39 to 15.06). In AHI ≥ 5, BM2000A had 90.7% sensitivity, 100% specificity, 91.2% accuracy, and 0.994 area under the curve. Using AHI ≥ 5, ≥ 15, and ≥30 as the screening criteria, optimal WPO-AHI cutoffs to improve the screening accuracy were 3.10, 8.92, and 13.05. Conclusions: BM2000A-derived results properly correlate with PSG and can provide OSA screening with good sensitivity and specificity.
Validation of the Withings ScanWatch as a Wrist-Worn Reflective Pulse Oximeter: Prospective Interventional Clinical Study

Abstract:Background A decrease in the level of pulse oxygen saturation as measured by pulse oximetry (SpO2) is an indicator of hypoxemia that may occur in various respiratory diseases, such as chronic obstructive pulmonary disease (COPD), sleep apnea syndrome, and COVID-19. Currently, no mass-market wrist-worn SpO2 monitor meets the medical standards for pulse oximeters. Objective The main objective of this monocentric and prospective clinical study with single-blind analysis was to test and validate the accuracy of the reflective pulse oximeter function of the Withings ScanWatch to measure SpO2 levels at different stages of hypoxia. The secondary objective was to confirm the safety of this device when used as intended. Methods To achieve these objectives, we included 14 healthy participants aged 23-39 years in the study, and we induced several stable plateaus of arterial oxygen saturation (SaO2) ranging from 100%-70% to mimic nonhypoxic conditions and then mild, moderate, and severe hypoxic conditions. We measured the SpO2 level with a Withings ScanWatch on each participant’s wrist and the SaO2 from blood samples with a co-oximeter, the ABL90 hemoximeter (Radiometer Medical ApS). Results After removal of the inconclusive measurements, we obtained 275 and 244 conclusive measurements with the two ScanWatches on the participants’ right and left wrists, respectively, evenly distributed among the 3 predetermined SpO2 groups: SpO2≤80%, 80%<SpO2≤90%, and 90%<SpO2. We found a strong association and a high level of agreement between the measurements collected from the devices, with high Pearson correlation coefficients of r=0.944 and r=0.954 on the correlation plots, low Pearson correlation coefficients of r=0.083 (P=.17) and r=0.23 (P=.001) on Bland-Altman plots, biases of 0.98% (95% CI 0.65-1.32) and 1.56% (95% CI 1.24-1.87), and root mean square errors of 2.97% and 3.00% from the participants’ right and left hands, respectively. Conclusions In conclusion, the Withings ScanWatch is able to measure SpO2 levels with adequate accuracy at a clinical grade. No undesirable effects or adverse events were reported during the study. Abstract:Peripheral blood oxygen saturation Sp02 is a vital measure in healthcare. Modern off-the-shelf wrist-worn devices, such as the Apple Watch, FitBit, and Samsung Gear, have an onboard sensor called a pulse oximeter. While pulse oximeters are capable of measuring both Sp02 and heart rate, current wrist-worn devices use them only to determine heart rate, as Sp02 measurements collected from the wrist are believed to be inaccurate. Enabling oxygen saturation monitoring on wearable devices would make these devices tremendously more useful for health monitoring and open up new avenues of research. To the best of our knowledge, we present the first study of the reliability of Sp02 sensing from the wrist. Using a custom-built wrist-worn pulse oximeter, we find that existing algorithms designed for fingertip sensing are a poor match for this setting, and can lead to over 90% of readings being inaccurate and unusable. We further show that sensor placement and skin tone have a substantial effect on the measurement error, and must be considered when designing wrist-worn Sp02 sensors and measurement algorithms. Based on our findings, we propose \codename, an alternative approach for reliable Sp02 sensing. By selectively pruning data, \codename achieves an order of magnitude reduction in error compared to existing algorithms, while still providing sufficiently frequent readings for continuous health monitoring.

A Validation of Six Wearable Devices for Estimating Sleep, Heart Rate and Heart Rate Variability in Healthy Adults

Abstract:The primary aim of this study was to examine the validity of six commonly used wearable devices, i.e., Apple Watch S6, Garmin Forerunner 245 Music, Polar Vantage V, Oura Ring Generation 2, WHOOP 3.0 and Somfit, for assessing sleep. The secondary aim was to examine the validity of the six devices for assessing heart rate and heart rate variability during, or just prior to, night-time sleep. Fifty-three adults (26 F, 27 M, aged 25.4 ± 5.9 years) spent a single night in a sleep laboratory with 9 h in bed (23:00–08:00 h). Participants were fitted with all six wearable devices—and with polysomnography and electrocardiography for gold-standard assessment of sleep and heart rate, respectively. Compared with polysomnography, agreement (and Cohen's kappa) for two-state categorisation of sleep periods (as sleep or wake) was 88% (κ = 0.30) for Apple Watch; 89% (κ = 0.35) for Garmin; 87% (κ = 0.44) for Polar; 89% (κ = 0.51) for Oura; 86% (κ = 0.44) for WHOOP and 87% (κ = 0.48) for Somfit. Compared with polysomnography, agreement (and Cohen's kappa) for multi-state categorisation of sleep periods (as a specific sleep stage or wake) was 53% (κ = 0.20) for Apple Watch; 50% (κ = 0.25) for Garmin; 51% (κ = 0.28) for Polar; 61% (κ = 0.43) for Oura; 60% (κ = 0.44) for WHOOP and 65% (κ = 0.52) for Somfit. Analyses regarding the two-state categorisation of sleep indicate that all six devices are valid for the field-based assessment of the timing and duration of sleep. However, analyses regarding the multi-state categorisation of sleep indicate that all six devices require improvement for the assessment of specific sleep stages. As the use of wearable devices that are valid for the assessment of sleep increases in the general community, so too does the potential to answer research questions that were previously impractical or impossible to address—in some way, we could consider that the whole world is becoming a sleep laboratory.

Abstract:The aim of the study was to compare the WHOOP strap – a wearable device that estimates sleep based on measures of movement and heart rate derived from actigraphy and photoplethysmography, respectively. Twelve healthy adults (6 females, 6 males, aged 22.9 ± 3.4 years) participated in a 10-day, laboratory-based protocol. A total of 86 sleeps were independently assessed in 30-s epochs using polysomnography and WHOOP. For WHOOP, bed times were entered by researchers and sleeps were scored by the company based on proprietary algorithms. WHOOP overestimated total sleep time by 8.2 ± 32.9 minutes compared to polysomnography, but this difference was non-significant. WHOOP was compared to polysomnography for 2-stage (i.e., wake, sleep) and 4-stage categorisation (i.e., wake, light sleep [N1 or N2], slow-wave sleep [N3], REM) of sleep periods. For 2-stage categorisation, the agreement, sensitivity to sleep, specificity for wake, and Cohen's kappa were 89%, 95%, 51%, and 0.49, respectively. For 4-stage categorisation, the agreement, sensitivity to light sleep, SWS, REM, and wake, and Cohen's kappa were 64%, 62%, 68%, 70%, 51%, and 0.47, respectively. In situations where polysomnography is impractical (e.g., field settings), WHOOP is a reasonable method for estimating sleep, particularly for 2-stage categorisation, if accurate bedtimes are manually entered.

Abstract:Introduction: This study aimed to validate the ability of a prototype sport watch (Polar Electro Oy, FI) to recognize wake and sleep states in two trials with and without an interval training session (IT) 6 h prior to bedtime. Methods: Thirty-six participants completed this study. Participants performed a maximal aerobic test and three polysomnography (PSG) assessments. The first night served as a device familiarization night and to screen for sleep apnea. The second and third in-home PSG assessments were counterbalanced with/without IT. Accuracy and agreement in detecting sleep stages were calculated between PSG and the prototype. Results: Accuracy for the different sleep stages (REM, N1 and N2, N3, and awake) as a true positive for the nights without exercise was 84 ± 5%, 64 ± 6%, 81 ± 6%, and 91 ± 6%, respectively, and for the nights with exercise was 83 ± 7%, 63 ± 8%, 80 ± 7%, and 92 ± 6%, respectively. The agreement for the sleep night without exercise was 60.1 ± 8.1%, k = 0.39 ± 0.1, and with exercise was 59.2 ± 9.8%, k = 0.36 ± 0.1. No significant differences were observed between nights or between the sexes. Conclusion: The prototype showed better or similar accuracy and agreement to wrist-worn consumer products on the market for the detection of sleep stages with healthy adults. However, further investigations will need to be conducted with other populations.

Abstract:Background Photoplethysmography (PPG) is a low-cost and easy-to-implement method to measure vital signs, including heart rate (HR) and pulse rate variability (PRV) which widely used as a substitute of heart rate variability (HRV). The method is used in various wearable devices. For example, Samsung smartwatches are PPG-based open-source wristbands used in remote well-being monitoring and fitness applications. However, PPG is highly susceptible to motion artifacts and environmental noise. A validation study is required to investigate the accuracy of PPG-based wearable devices in free-living conditions. Objective We evaluate the accuracy of PPG signals—collected by the Samsung Gear Sport smartwatch in free-living conditions—in terms of HR and time-domain and frequency-domain HRV parameters against a medical-grade chest electrocardiogram (ECG) monitor. Methods We conducted 24-hours monitoring using a Samsung Gear Sport smartwatch and a Shimmer3 ECG device. The monitoring included 28 participants (14 male and 14 female), where they engaged in their daily routines. We evaluated HR and HRV parameters during the sleep and awake time. The parameters extracted from the smartwatch were compared against the ECG reference. For the comparison, we employed the Pearson correlation coefficient, Bland-Altman plot, and linear regression methods. Results We found a significantly high positive correlation between the smartwatch’s and Shimmer ECG’s HR, time-domain HRV, LF, and HF and a significant moderate positive correlation between the smartwatch’s and shimmer ECG’s LF/HF during sleep time. The mean biases of HR, time-domain HRV, and LF/HF were low, while the biases of LF and HF were moderate during sleep. The regression analysis showed low error variances of HR, AVNN, and pNN50, moderate error variances of SDNN, RMSSD, LF, and HF, and high error variances of LF/HF during sleep. During the awake time, there was a significantly high positive correlation of AVNN and a moderate positive correlation of HR, while the other parameters indicated significantly low positive correlations. RMSSD and SDNN showed low mean biases, and the other parameters had moderate mean biases. In addition, AVNN had moderate error variance while the other parameters indicated high error variances. Conclusion The Samsung smartwatch provides acceptable HR, time-domain HRV, LF, and HF parameters during sleep time. In contrast, during the awake time, AVNN and HR show satisfactory accuracy, and the other HRV parameters have high errors.

Abstract:Sleep tracking by consumers is becoming increasingly prevalent; yet, few studies have evaluated the accuracy of such devices. We sought to evaluate the accuracy of three devices (Oura Ring Gen3, Fitbit Sense 2, and Apple Watch Series 8) compared to the gold standard sleep assessment (polysomnography (PSG)). Thirty-five participants (aged 20–50 years) without a sleep disorder were enrolled in a single-night inpatient study, during which they wore the Oura Ring, Fitbit, and Apple Watch, and were monitored with PSG. For detecting sleep vs. wake, the sensitivity was ≥95% for all devices. For discriminating between sleep stages, the sensitivity ranged from 50 to 86%, as follows: Oura ring sensitivity 76.0–79.5% and precision 77.0–79.5%; Fitbit sensitivity 61.7–78.0% and precision 72.8–73.2%; and Apple sensitivity 50.5–86.1% and precision 72.7–87.8%. The Oura ring was not different from PSG in terms of wake, light sleep, deep sleep, or REM sleep estimation. The Fitbit overestimated light (18 min; p < 0.001) sleep and underestimated deep (15 min; p < 0.001) sleep. The Apple underestimated the duration of wake (7 min; p < 0.01) and deep (43 min; p < 0.001) sleep and overestimated light (45 min; p < 0.001) sleep. In adults with healthy sleep, all the devices were similar to PSG in the estimation of sleep duration, with the devices also showing moderate to substantial agreement with PSG-derived sleep stages.

Abstract:Objective We investigated how a commercially available smartwatch that measures peripheral blood oxygen saturation (SpO 2 ) can detect hypoxemia compared to a medical-grade pulse oximeter. Methods We recruited 24 healthy participants. Each participant wore a smartwatch (Apple Watch Series 6) on the left wrist and a pulse oximeter sensor (Masimo Radical-7) on the left middle finger. The participants breathed via a breathing circuit with a three-way non-rebreathing valve in three phases. First, in the 2-minute initial stabilization phase, the participants inhaled the ambient air. Then in the 5-minute desaturation phase, the participants breathed the oxygen-reduced gas mixture (12% O 2 ), which temporarily reduced their blood oxygen saturation. In the final stabilization phase, the participants inhaled the ambient air again until SpO 2 returned to normal values. Measurements of SpO 2 were taken from the smartwatch and the pulse oximeter simultaneously in 30-s intervals. Results There were 642 individual pairs of SpO 2 measurements. The bias in SpO 2 between the smartwatch and the oximeter was 0.0% for all the data points. The bias for SpO 2 less than 90% was 1.2%. The differences in individual measurements between the smartwatch and oximeter within 6% SpO 2 can be expected for SpO 2 readings 90%–100% and up to 8% for SpO 2 readings less than 90%. Conclusions Apple Watch Series 6 can reliably detect states of reduced blood oxygen saturation with SpO 2 below 90% when compared to a medical-grade pulse oximeter. The technology used in this smartwatch is sufficiently advanced for the indicative measurement of SpO 2 outside the clinic. Trial Registration ClinicalTrials.gov NCT04780724

Performance of the Verily Study Watch for Measuring Sleep Compared to Polysomnography

Abstract:This study evaluated the performance of a wrist-worn wearable, Verily Study Watch (VSW), in detecting key sleep measures against polysomnography (PSG). Methods: We collected data from 41 adults without obstructive sleep apnea or insomnia during a single overnight laboratory visit. We evaluated epoch-by-epoch performance for sleep versus wake classification, sleep stage classification and duration, total sleep time (TST), wake after sleep onset (WASO), sleep onset latency (SOL), sleep efficiency (SE), and number of awakenings (NAWK). Performance metrics included sensitivity, specificity, Cohens kappa, and Bland-Altman analyses. Results: Sensitivity and specificity (95% CIs) of sleep versus wake classification were 0.97 (0.96, 0.98) and 0.70 (0.66, 0.74), respectively. Cohens kappa (95% CI) for 4-class stage detection was 0.64 (0.18, 0.82). Most VSW sleep measures had proportional bias. The mean bias values (95% CI) were 14.0 minutes (5.55, 23.20) for TST, -13.1 minutes (-21.33, -6.21) for WASO, 2.97% (1.25, 4.84) for SE, -1.34 minutes (-7.29, 4.81) for SOL, 1.91 minutes (-8.28, 11.98) for light sleep duration, 5.24 minutes (-3.35, 14.13) for deep sleep duration, and 6.39 minutes (-0.68, 13.18) for REM sleep duration. Mean and median NAWK count differences (95% CI) were 0.05 (-0.42, 0.53) and 0.0 (0.0, 0.0), respectively. Core analytics: Detection and timing accuracy for the sleep onset times and wake times. Additional analytics and exploratory analyses: Discrepancy analysis both including and excluding the peri-sleep activity periods. Epoch-by-epoch analysis of rate and extent of wake misclassification during peri-sleep activity periods. Core outcomes: Oura and Fitbit were more accurate at detecting sleep/wake transitions than the actigraph and the lower-priced consumer sleep tracker devices. Detection accuracy was less reliable in participants with lower sleep efficiency. Important additional outcomes: With inclusion of peri-sleep periods, specificity and Kappa improved significantly for Oura and Fitbit, but not ActiGraph. All devices misclassified motionless wake as sleep to some extent, but this was less prevalent for Oura and Fitbit. Core conclusions: Performance of Oura and Fitbit is robust on nights with suboptimal bedtime routines or minor sleep disturbances. Reduced performance on nights with low sleep efficiency bolsters concerns that these devices are less accurate for fragmented or disturbed sleep.
Performance of seven consumer sleep-tracking devices compared with polysomnography

Abstract:Abstract Study Objectives Consumer sleep-tracking devices are widely used and becoming more technologically advanced, creating strong interest from researchers and clinicians for their possible use as alternatives to standard actigraphy. We, therefore, tested the performance of many of the latest consumer sleep-tracking devices, alongside actigraphy, versus the gold-standard sleep assessment technique, polysomnography (PSG). Methods In total, 34 healthy young adults (22 women; 28.1 ± 3.9 years, mean ± SD) were tested on three consecutive nights (including a disrupted sleep condition) in a sleep laboratory with PSG, along with actigraphy (Philips Respironics Actiwatch 2) and a subset of consumer sleep-tracking devices. Altogether, four wearable (Fatigue Science Readiband, Fitbit Alta HR, Garmin Fenix 5S, Garmin Vivosmart 3) and three nonwearable (EarlySense Live, ResMed S+, SleepScore Max) devices were tested. Sleep/wake summary and epoch-by-epoch agreement measures were compared with PSG. Results Most devices (Fatigue Science Readiband, Fitbit Alta HR, EarlySense Live, ResMed S+, SleepScore Max) performed as well as or better than actigraphy on sleep/wake performance measures, while the Garmin devices performed worse. Overall, epoch-by-epoch sensitivity was high (all ≥0.93), specificity was low-to-medium (0.18–0.54), sleep stage comparisons were mixed, and devices tended to perform worse on nights with poorer/disrupted sleep. Conclusions Consumer sleep-tracking devices exhibited high performance in detecting sleep, and most performed equivalent to (or better than) actigraphy in detecting wake. Device sleep stage assessments were inconsistent. Findings indicate that many newer sleep-tracking devices demonstrate promising performance for tracking sleep and wake. Devices should be tested in different populations and settings to further examine their wider validity and utility.

Abstract:Quality of sleep is an important indicator of health and well being. Recent developments in the field of in-home sleep monitoring have the potential to enhance a person’s sleeping experience and contribute to an overall sense of well being. Existing in-home sleep monitoring devices either fail to provide adequate sleep information or are obtrusive to use. To overcome these obstacles, a noncontact and cost-effective sleep monitoring system, named SleepSense, is proposed for continuous recognition of the sleep status, including on-bed movement, bed exit, and breathing section. SleepSense consists of three parts: a Doppler radar-based sensor, a robust automated radar demodulation module, and a sleep status recognition framework. Herein, several time-domain and frequency-domain features are extracted for the sleep recognition framework. A prototype of SleepSense is presented and evaluated using two sets of experiments. In the short-term controlled experiment, the SleepSense achieves an overall 95.1% accuracy rate in identifying various sleep status. In the 75-minute sleep study, SleepSense demonstrates wide usability in real life. The error rate for breathing rate extraction in this study is only 6.65%. These experimental results indicate that SleepSense is an effective and promising solution for in-home sleep monitoring.

Abstract:Consumer sleep trackers provide useful insight into sleep. However, large scale performance evaluation studies are needed to properly understand sleep tracker accuracy. This study evaluated performance of an under-mattress sensor to estimate sleep and wake versus polysomnography in a large sample, including individuals with and without sleep disorders and during day versus night sleep opportunities, across multiple in-laboratory studies. 183 participants (51/49% male/female, mean[SD] age=45[18] years) attended the sleep laboratory for a research study including simultaneous polysomnography and under-mattress sensor (Withings Sleep Analyzer [WSA]) recordings. Epoch-by-epoch analyses determined accuracy, sensitivity, and specificity of the WSA versus polysomnography. Bland-Altman plots examined bias in sleep duration, efficiency, onset-latency, and wake after sleep onset. Overall WSA sleep-wake classification accuracy was 83%, sensitivity 95%, and specificity 37%. The WSA significantly overestimated total sleep time (48[81]minutes), Sleep efficiency (9[15]%), sleep onset latency (6[26]minutes), and underestimated wake after sleep onset (54[78]minutes). Accuracy and specificity were higher for night versus daytime sleep opportunities in healthy individuals (89% and 47% versus 82% and 26% respectively, p<0.05). Accuracy and sensitivity were also higher for healthy individuals (89% and 97%) versus those with sleep disorders (81% and 91%, p<0.05). WSA performance is comparable to other consumer sleep trackers, with high sensitivity but poor specificity compared to polysomnography. WSA performance was reasonably stable, but more variable in daytime sleep opportunities and in people with a sleep disorder. Contactless, under-mattress sleep sensors show promise for accurate sleep monitoring, noting the tendency to over-estimate sleep particularly where wake time is high.

Abstract:The most commonly used method to assess peripheral oxygen saturation (SpO2) in clinical practice is pulse oximetry. The smartwatch Apple Watch 6 was developed with a new sensor and an app that allows taking on-demand readings of blood oxygen and background readings, day and night. The present study aimed to assess the feasibility and agreement of the Apple Watch 6 compared with a standard SpO2 monitoring system to assess normal and pathological oxygen saturation. We recruited study participants with lung disease or cardiovascular disease and healthy subjects. A total of 265 subjects were screened for enrolment in this study. We observed a strong positive correlation between the smartwatch and the standard commercial device in the evaluation of SpO2 measurements (r = 0.89, p < 0.0001) and HR measurements (r = 0.98, p < 0.0001). A very good concordance was found between SpO2 (bias, −0.2289; SD, 1.66; lower limit, −3.49; and upper limit, 3.04) and HR (bias, −0.1052; SD, 2.93; lower limit, −5.84; and upper limit, 5.63) measured by the smartwatch in comparison with the standard commercial device using Bland–Altman analysis. We observed similar agreements and concordance even in the different subgroups. In conclusion, our study demonstrates that the wearable device used in the present study could be used to assess SpO2 in patients with cardiovascular or lung diseases and in healthy subjects.

Abstract:Obstructive Sleep Apnea (OSA) is a prevalent respiratory disorder, often diagnosed through polysomnography. Moderate-severe OSA significantly raises morbidity and mortality. However, patients commonly encounter accessibility challenges and prolonged waiting times. The STOP-BANG score, while sensitive, has low specificity. Studies using pulse oximeters, particularly with the oxygen desaturation index (ODI) as the diagnostic threshold, have been conducted. In an effort to enhance screening precision and accessibility, we utilized the Wellue O2 ring, a commercial pulse oximeter ring, for screening moderate-severe OSA. This prospective pilot study, conducted at the Thammasat Sleep Center, enrolled patients aged ≥ 15 years. Patients wore the O2 ring on their thumb during polysomnography, and the O2 ring data were compared with polysomnography data post-examination. Among the 192 participants, 53.1% were male, with an average age of 49 years. 15.1% exhibited normal or mild OSA, while 84.9% had moderate-severe OSA. The Wellue O2 ring demonstrated high accuracy in screening moderate-severe OSA, with a sensitivity and specificity of 86.5% and 86.2%, respectively, using an ODI of 15 for 20 seconds. The test's area under the curve was 0.92. The Wellue O2 ring emerges as a promising alternative for screening moderate-severe OSA. This pilot study suggests its high sensitivity and specificity, positioning it as a convenient and accessible substitute for in-laboratory polysomnography. However, larger studies with diverse populations are essential to validate results and establish the device's clinical utility in OSA diagnosis.

Abstract:Recently, photoplethysmography-based vital parameter measurements have increased in popularity. However, clinical evaluation of these measurements is lacking. The objective of this study was to rigorously evaluate the clinical accuracy and reliability of a novel photoplethysmography-based wristband for measuring key vital parameters—oxygen saturation (SpO2), respiratory rate (RR), and pulse rate (PR)—during heart catheterisations. Vital parameters obtained during heart catheterisations by means of a photoplethysmography-based wristband (CardioWatch 287-2, Corsano Health) were compared to reference measurements performed by a Nellcor fingerclip (SpO2, PR) as well as a 5-lead ECG (RR) (QMAPP Haemodynamic Monitoring module, Fysicon B.V.) by means of correlation coefficients and root means squared error (RMSE). Effects of skin colour and arm hair density were additionally evaluated. In total, 945 samples from a total of 100 patients were included in the analysis. The correlation coefficients and RSME obtained for the difference between reference and photoplethysmography-based wristband measurements were r = 0.815 and 1.6% for SpO2, r = 0.976 and 0.9 brpm for RR, and r = 0.995 and 1.3 bpm for PR. Similar results were obtained across all skin colour and arm hair density subcategories. This study shows that photoplethysmography-based SpO2, RR, and PR measurements can be accurate during heart catheterisations. Future investigations are required to evaluate the wristband's performance under dynamic circumstances as well as over an extended time period.
