Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 2, February 2024. Background:Ruptures of the quadriceps tendon present most frequently in older adults and individuals with underlying medical conditions.Purpose:To examine the relationship between patient-specific factors and tear characteristics with outcomes after quadriceps tendon repair.Study Design:Case-control study; Level of evidence, 3.Methods:A retrospective review was conducted on all patients who underwent quadriceps tendon repair between January 1, 2016, and January 1, 2021, at a single institution. Patients 6 weeks to surgery) were excluded. Information was collected regarding patient characteristics, presenting symptoms, tear characteristics, physical examination findings, and postoperative outcomes. Poor outcome was defined as a need for revision surgery, complications, postoperative range of motion of (ROM) 5°.Results:A total of 191 patients met the inclusion criteria. Patients were aged 58.5 ± 13.2 years at the time of surgery, were predominantly men (90.6%), and had a mean body mass index (BMI) of 32.2 ± 6.3 kg/m2. Patients underwent repair with either suture anchors (15.2%) or transosseous tunnels (84.8%). Postoperatively, 18.5% of patients experienced knee flexion ROM of 5°, 8.5% had complications, and 3.2% underwent revision. Increasing age (odds ratio [OR], 1.03 [95% CI, 1.004-1.07]) and female sex (OR, 3.82 [95% CI, 1.25-11.28]) were significantly associated with postoperative knee flexion of 5°. Current smoking status (OR, 15.44 [95% CI, 3.97-65.90]) and concomitant retinacular tears (OR, 9.62 (95% CI, 1.67-184.14]) were associated with postoperative complications, and increasing age (OR, 1.05 [95% CI, 1.02-1.08]) and greater BMI (OR, 1.08 [95% CI, 1.02-1.14]) were associated with risk of acquiring any poor outcome criteria.Conclusion:Patient-specific characteristics—such as increasing age, greater BMI, female sex, retinacular involvement, and current smoking status—were found to be risk factors for poor outcomes after quadriceps tendon repair. Further studies are needed to identify potentially modifiable risk factors that can be used to set patient expectations and improve outcomes.

Clinical and radiological outcome of acute quadriceps tendon repair at 2 - year follow-up

Abstract:Purpose: Though previous studies on surgical quadriceps tendon repair reported good to satisfactory results its impact on knee extensor strength and patellofemoral joint is unknown. The purpose of the study was to assess the clinical and functional outcome by quantifying subjective knee related outcome, isokinetic extensor strength complemented by magnetic resonance imaging (MRI). Methods: For this retrospective clinical trial twenty-one patients with a mean age of 59.1 (±14.9) years were carried out for follow-up evaluation. For clinical outcome numeric rating scale for pain (NRS), Kujala anterior knee pain score, Knee osteoarthritis outcome score (KOOS) with its subscores, Tegener Activity Score (TAS), Isokinetic extensor strength testing (ISO) and the Limb symmetry index (LSI) were utilized. Muscle volume (Vmqf), atrophy and cartilage status were assessed on MRI. Tendon integrity and implant loosening were evaluated. Statistical analysis was carried out using Student´s t-test, Pearson and Spearman correlation coefficient. Results: Mean follow-up was 56.2 (± 17.5) months. Clinical outcomes in mean were as follows: NRS 1.7 (±2.0), Kujala score 75.8 (± 15.8) points; KOOS 62.9 (±30.6) % (Subscores: (KOOSPain 84 (± 18.3)%, KOOSSymptoms 63.4 (± 15.3) %, KOOSADL 79.8% (± 19.4)%, KOOSSports&Rec 61.5 (± 28.8))%, TAS 3.7 (± 1.2). Mean ISO difference was 3.7 (±32.3) %). 41.2% of patients had an LSI < 90% and showed non-significantly (p > 0.05) better outcomes in NRS, Kujala score and KOOS than patients with LSI < 90%. ISO correlated with better subjective outcome (r > 0.5) but not with MRI findings. Vmqf asymmetry > 10% was seen in 41,2% of patients. Degenerative muscle and cartilage changes were few and low grade. One tendon showed partial retear. Patients showed no signs of implant loosening. Conclusion: Distal quadriceps tendon repair leads to satisfactory subjective outcome in patients with low to moderate activity levels. Tendon integrity was reliably restored. However, extensor strength deficits remain in more than 40% of patients. Atrophy signs of advanced fatty infiltration were recorded in 15% and patellofemoral cartilage defects in 20% of all patients, indicating that previous assessment tools do not display functional outcome adequately.

Abstract:PURPOSE The purpose of this study was to ascertain if the simple practice of Gram stain, acridine orange stain and direct sensitivity determination of positive blood culture bottles could be used to guide early and appropriate treatment in trauma patients with clinical suspicion of sepsis. The study also aimed to evaluate the error in interpreting antimicrobial sensitivity by direct method when compared to standard method and find out if specific antibiotic-organism combination had more discrepancies. Findings from consecutive episodes of blood stream infection at an Apex Trauma centre over a 12-month period are summarized. MATERIALS AND METHODS A total of 509 consecutive positive blood cultures were subjected to Gram staining. AO staining was done in BacT/ALERT-positive Gram-stain negative blood cultures. Direct sensitivity was performed from 369 blood culture broths, showing single type of growth in Gram and acridine orange staining. Results of direct sensitivity were compared to conventional sensitivity for errors. RESULTS No 'very major' discrepancy was found in this study. About 5.2 and 1.8% minor error rates were noted in gram-positive and gram-negative bacteria, respectively, while comparing the two methods. Most of the discrepancies in gram-negative bacteria were noted in beta lactam - beta lactamase inhibitor combinations. Direct sensitivity testing was not reliable for reporting of methicillin and vancomycin resistance in Staphylococci. CONCLUSIONS Gram stain result together with direct sensitivity testing is required for optimizing initial antimicrobial therapy in trauma patients with clinical suspicion of sepsis. Gram staining and AO staining proved particularly helpful in the early detection of candidaemia.
No Clinically Significant Differences in Outcomes after Anterior Cruciate Ligament Reconstruction When Comparing Quadriceps, Bone Patellar Tendon Bone, and Hamstring Autografts of 9mm or Greater

Abstract:Purpose: The purpose of this study was to determine if there are patient-reported outcome differences in bone-patellar-tendon-bone (BTB), quadriceps tendon (QT), and hamstring (HS) grafts of comparable size. Methods: We performed a retrospective analysis of global registry data to include all patients that had an ACL reconstruction with BTB, QT, or HS autograft of at least 9mm diameter between 2010 and 2021 with complete 1- and 2-year outcome scores. We compared 1- and 2-year postoperative outcome scores (VAS, KOOS pain, MARS, VR-12) between BTB, QT and HS of the same size graft. Results: 2318 subjects were included in the analysis and all graft types showed improved PROMs at 1- and 2-years postoperatively when compared to baseline. The KOOS pain score for the BTB group was significantly better than the HS (between-group difference = 2.71, p<0.01) and QT (between-group difference = 2.51, p<0.01) groups at 1-year, and the BTB group was better than HS (between-group difference = 1.88, p<0.01) at 2-years. However, the differences were small and not clinically meaningful. When comparing graft type there were no differences in the percentage of patients who reached MCID, or clinical scores VAS, MARS at 2-years follow-up. Conclusion: There is no clinically meaningful difference in KOOS-pain, VR-12, VAS, and MARS at 1- and 2-years postoperatively in patients having anterior cruciate ligament reconstruction with BTB, HS or QT if graft size is at least 9mm in diameter. Introduction and Objectives:Anterior cruciate ligaments (ACL) accounted for the most frequent ligamentous injury. ACL reconstruction (ACLR) when indicated, has been the standard of care to treat instability and to restore functional stability.While hamstring tendon autografts (HT) are commonly used in ACLR, quadriceps tendon autografts (QT) have gained popularity in recent years Thus, this study performed a systematic review and meta-analysis comparing the outcomes of QT to HT for ACLR.Materials and Methods:Comprehensive database searches were performed in two databases, 452 studies were found and 14 studies were included in the study. This study was conducted based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Guideline. Included studies will be extracted for characteristics such as author, study year, design, and outcomes.Results:Fourteen studies were included in the study after a comprehensive literature search was done. 2219 patients, 847 were treated with QT and 1372 were treated with HT, functional outcomes such as International Knee Documentation Committee score (IKDC), Cincinnati score, Knee Injury and Osteoarthritis Outcome Score (KOOS), Lysholm score, Tegner score, VAS score, active knee flexion, return to previous activity, and Patient and Observer Scar Assessment Scale(POSAS) were measured, there were differences in KOOS pain score and Symptom score with mean difference of 2.40 (95%CI 0.75 to 4.06, I2=26%, p=0.004), and 4.40 (95%CI 0.81 to 7.89, I2= 60%, p=0.02) respectively. However, there were no significant differences in other functional outcomes and secondary outcomes.Conclusion:This study demonstrates comparable results between ACLR using QT and HT. QT appeared to have better functional outcomes in KOOS pain and KOOS symptoms than in HT

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 11, November 2024. Background:Patellar tendon ruptures are infrequent but disabling injuries. Surgical treatment is the gold standard to obtain good outcomes, and numerous techniques have been described. Biomechanical studies report better results when augmented techniques are used. However, there is a lack of consensus regarding the best standard technique.Purpose/Hypothesis:The purpose of this study was to assess a cohort of patients with acute patellar tendon rupture that was surgically treated and to compare the clinical outcomes of 2 groups: isolated repair and biological augmentation techniques with autograft or allograft. It was hypothesized that the biological augmentation group would have better clinical outcomes than the isolated repair group.Study Design:Cohort study; Level of evidence, 3.Methods:All patients with acute patellar tendon rupture surgically treated in our center between 2016 and 2022 were retrospectively reviewed. Patient and rupture characteristics, surgical technique, clinical outcomes, and complications were recorded. The primary outcome was rerupture rate, and secondary outcomes were infection rate, stiffness >15° of knee flexion deficit, and extensor mechanism lag >5°.Results:The study included 34 patients with 36 operated knees (2 patients with bilateral rupture). The mean age was 44.9 years. Isolated repair was performed in 20 knees (55.6%), and 16 knees underwent repair and biological augmentation with autograft or allograft. Both groups were comparable in terms of their demographic characteristics. A statistically significant association was found between the type of surgery and tendon rerupture. Of the patients in the isolated repair group, 5 of 20 (25%) experienced a failure, whereas in the biological augmentation group, no reruptures were recorded (P = .031). However, no statistically significant associations were found between the type of surgery and other complications, such as the development of stiffness (P = .54), residual extension lag >5° (P = .87), or the development of infection (P = .25).Conclusion:In this cohort of patients, biological augmentation reduced the rate of surgical failure for acute patellar tendon rupture without being associated with a higher risk of complications such as stiffness, residual extension lag, or the development of infection.

Physical Therapists' Prognosis of Outcomes After a Hip or Quadriceps Exercise Intervention in Patients With Patellofemoral Pain: A Secondary Analysis of a Randomized Trial

Abstract:OBJECTIVE: Can physical therapists who are treating patients with patellofemoral pain (PFP) predict the outcome of a 12-week exercise intervention based on initial assessment, and what are the physical therapists' reasons for prediction? DESIGN: Secondary analysis of a randomized trial. METHODS: After the initial assessment, physical therapists were asked to predict the prognosis of 200 patients with PFP who were allocated to 12 weeks of quadriceps exercises (QEs) or hip exercises (HEs) on a 1-to-10 Likert scale, and to describe their reasoning for the prediction score. OUTCOMES: measures were changes from baseline to weeks 12 and 26 on the Anterior Knee Pain Scale (range 0-100) and a transition questionnaire (TransQ). Linear mixed-effects models were used to assess the prediction. Secondly, we used a qualitative approach to summarize the physical therapists' reasoning (written notes) when predicting the outcome. RESULTS: There was no association between physical therapists' prognosis and changes in Anterior Knee Pain Scale for QE or HE at weeks 12 and 26 (slopes: -0.14 to -0.51 with wide 95% confidence intervals). There was no association between physical therapists' assessment of prognosis using TransQ for QE or HE at weeks 12 and 26 (odds ratio: 0.99 to 1.17 with wide 95% confidence intervals). CONCLUSION: Physical therapists' prognosis based on initial assessment was not associated with outcomes after 12 weeks of either quadriceps or hip exercise therapy among patients with PFP. Physical therapists' prognoses were not useful as a source of information and to identify PFP patients with poor or good projected outcomes. J Orthop Sports Phys Ther 2024;54(8):541-550. Association Between Proximal Hamstring Tear Characteristics and Achievement of Clinically Significant Outcomes After Endoscopic and Open Repair at Minimum 2-Year Follow-up

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:The effect of preoperative tear characteristics and the relative efficacy of open versus endoscopic surgical techniques have not been elucidated for the surgical treatment of proximal hamstring tendon injuries.Purpose:(1) To report on achievement rates of clinically significant outcomes at a minimum 2-year follow-up for multiple patient-reported outcomes (PROs) after surgical treatment of proximal hamstring injuries, stratified according to severity of proximal hamstring injury, and (2) to compare clinical outcomes associated with endoscopic versus open surgical repair for a subset of similarly classified tears.Study Design:Cohort study; Level of evidence, 3Methods:A single-surgeon clinical registry was queried for patients who underwent surgical repair for proximal hamstring ruptures between January 2012 and March 2021. Injuries were classified by magnetic resonance imaging as follows: grade 1, incomplete tear with the involvement of 1 or 2 tendons; grade 2, complete tear with the involvement of 3 tendons and ≤2-cm retraction; and grade 3, complete tear with the involvement of 3 tendons and >2-cm retraction. The Patient Acceptable Symptom State (PASS) for multiple PROs was calculated and compared among injury grades and between surgical techniques.Results:Among 75 patients with a mean follow-up of 30.5 ± 5.1 months, 20 had grade 1 tears, 24 had grade 2 tears, and 31 had grade 3 tears. Favorable 2-year postoperative PROs were demonstrated by each grade for all measured PROs. Patients with grade 3 tears were noted to have significantly lower rates of 2-year PASS achievement for the Hip Outcome Score–Activities of Daily Living (HOS-ADL) and Hip Outcome Score–Sports Specific (HOS-SS) scales (P≤ .032). Among grade 2 tears, patients treated endoscopically demonstrated significantly greater HOS-SS (endoscopic, 91.7%; open, 58.3%; P = .045) and Patient-Reported Outcomes Measurement Information System for Physical Function (endoscopic, 80.0%; open, 50.0%; P = .033) PASS achievement rates compared with those treated with the open technique. Complication rates were highest in patients with grade 3 tears (45.2%, P = .043).Conclusion:Surgical repair of proximal hamstring tendon tears with varying extents of tendon retraction resulted in high rates of achieving PASS at the 2-year follow-up. Among complete tears with 2 cm achieved lower rates of PASS on the HOS-ADL and HOS-SS scales and had a higher rate of complications.

Mitral regurgitation in left ventricular noncompaction cardiomyopathy assessed by cardiac MRI.

Abstract:BACKGROUND AND AIM OF THE STUDY Previous case reports have described patients with left ventricular noncompaction cardiomyopathy (LVNC) with significant mitral regurgitation (MR). The study aim was to determine if LVNC is associated with MR, as assessed by cardiac magnetic resonance imaging (cMRI). METHODS LVNC, assessed with cMRI, was observed retrospectively among 122 consecutive cases, 31 of whom had an end-systolic noncompacted-to-compacted ratio (ESNCCR) ≥ 2. In addition, 40 normal subjects undergoing cMRI and 40 with moderate to severe MR were included as controls. Using cine images, the ESNCCR and left ventricular (LV) and right ventricular (RV) stroke volumes were measured. The mitral regurgitant fraction (MRF) was calculated by dividing the difference between the RV and LV stroke volumes by the LV stroke volume. The total papillary muscle (TPM) area was measured from the mid short-axis view. Adjusting for age, race, gender and body surface area, an analysis of covariance was conducted to determine whether MRF and TPM were associated with ESNCCR ≥ 2. Adjusted means were presented with 95% confidence intervals. RESULTS After adjustment, the MRF in patients with ESNCCR ≥ 2 was higher than in controls, but did not differ from that in patients with significant MR (21 ± 6.5% versus 2.7 ± 12% versus 29.2 ± 11%, p = 0.039 and p = 0.3, respectively). Further, patients with ESNCCR ≥ 2 had a lower TPM than normal controls or those with moderate to severe MR (1.89 ± 0.28 cm2 versus 3.6 ± 0.26 cm2 versus 3.7 ± 0.24 cm2; p < 0.001 and p < 0.001, respectively). CONCLUSION LVNC is associated with increased MR, as assessed using cMRI. LVNC is also associated with abnormalities in the papillary muscle anatomy, which may predispose to MR.
Knee joint mechanics during gait after anterior cruciate ligament reconstruction using a partial or full thickness quadriceps tendon autograft at 2 years after surgery

Abstract:Background Despite quadriceps weakness in individuals after quadriceps tendon anterior cruciate ligament reconstruction (QT‐ACLR), and its association with knee joint mechanics, no studies have addressed gait mechanics in both partial‐thickness (PT‐Q) and full‐thickness (FT‐Q) options for QT‐ACLR. Objective To assess gait mechanics across a QT‐ACLR cohort. We hypothesized that QT‐ACLR would show changes in knee joint mechanics compared to control participants (CON) and nonoperated limbs. Additionally, we hypothesized that FT‐Q operated limbs would show greater changes compared to PT‐Q and CON. Design Retrospective cohort study. Setting University‐affiliated sports medicine institute. Participants Sixteen patients who underwent QT‐ACLR (7 FT‐Q: Age (years) = 28.6 ± 7.3, post‐op (months) = 23.5 ± 10.7, 9 PT‐Q: Age = 25.2 ± 4.3, post‐op = 24.4 ± 11.7) were recruited and compared to 11 CON (age = 23.4 ± 4.8). Intervention Participants underwent gait testing with force plate integrated motion capture. Main Outcome Measures Mixed repeated‐measures analyses of covariance, adjusted for gait speed, were used to determine significant main effects or interactions in peak knee flexion angle, sagittal knee range of motion, peak internal knee extension moment (KEM), and peak internal knee flexion moment. Results When measured an average of 2 years after surgery, no main effect for limb or limb by depth interaction were detected. A significant effect by group was observed for peak KEM (p = .03, η2 = .27) and peak knee flexion angle (p = .04, η2 = .24) in the loading response phase. FT‐Q (p = .02) and PT‐Q (p = .03) showed lower KEM compared to the CON group in both limbs. The FT‐Q group showed lower peak knee flexion angle compared to the CON group (p = .01). Conclusions Knee joint symmetry may be recovered 2 years following QT‐ACLR, but lower KEM compared to CON for both graft options and lower peak knee flexion angle than CON for the FT‐Q group may indicate a need for further investigation in QT‐ACLR.

Objectives:To report patient-reported outcome measures (PROMs) following isolated combined MPFL and MQTFL reconstruction in cases of patellar instability at a minimum follow-up of 2 years and to assess potential complications.Methods:A retrospective analysis of prospectively collected data was performed on consecutive skeletally mature patients who underwent isolated combined MPFL and MQTFL reconstruction between January 1, 2020, and May 31, 2022. Data were collected on physical examination, return to sport, and occurrence of complications. Patients completed Kujala and International Knee Documentation Committee (IKDC) Scores (PROMs) pre-operatively and at final follow-up.Results:A total of 82 patients underwent combined MPFL and MQTFL reconstruction, with 7 patients (8%) lost to follow-up. The mean follow-up time was 3.2 ± 0.7 years (range, 2.2 to 5.8). The mean pre-operative Kujala knee function score was 62 points (range, 39-76), which increased to 86 points (range, 86-93) at final follow-up (P = 0.00004). The mean pre-operative IKDC score was 42.5 (range, 39.1-44.8) points, increasing to 73 points (range, 70.1-75.9) at final follow-up (P = 0.00894). No instances of post-operative patellar fracture, infection, hematoma, or instability were reported. Minor complications were observed in 13.33% (10/75) of cases.Conclusion:In this series, combined MPFL and MQTFL reconstruction resulted in patellar stability, significant improvements in clinical and functional outcomes, and no major complications.

Abstract:Background: The best type of autograft for anterior cruciate ligament (ACL) reconstruction remains debatable. Hypothesis: Compared with bone-patellar tendon-bone (BPTB) and hamstring tendon (HT) autografts, the quadriceps tendon (QT) autograft has comparable graft survival as well as clinical function and pain outcomes. Study Design: Meta-analysis; Level of evidence, 4. Methods: A systematic literature search was conducted in PubMed, Embase, Scopus, and the Cochrane Library to July 2020. Randomized controlled trials (RCTs) and observational studies reporting comparisons of QT versus BPTB or HT autografts for ACL reconstruction were included. All analyses were stratified according to study design: RCTs or observational studies. Results: A total of 24 studies were included: 7 RCTs and 17 observational studies. The 7 RCTs included 388 patients, and the 17 observational studies included 19,196 patients. No significant differences in graft failure (P = .36), the International Knee Documentation Committee (IKDC) subjective score (P = .39), or the side-to-side difference in stability (P = .60) were noted between QT and BPTB autografts. However, a significant reduction in donor site morbidity was noted in the QT group compared with the BPTB group (risk ratio [RR], 0.17 [95% CI, 0.09-0.33]; P < .001). No significant differences in graft failure (P = .57), the IKDC subjective score (P = .25), or the side-to-side stability difference (P = .98) were noted between QT and HT autografts. However, the QT autograft was associated with a significantly lower rate of donor site morbidity than the HT autograft (RR, 0.60 [95% CI, 0.39-0.93]; P = .02). A similar graft failure rate between the QT and control groups was observed after both early and late full weightbearing, after early and late full range of motion, and after using the QT autograft with a bone plug and all soft tissue QT grafts. However, a significantly lower rate of donor site morbidity was observed in the QT group compared with the control group after both early and late full weightbearing, after early and late full range of motion, and after using the QT autograft with a bone plug and all soft tissue QT grafts. No difference in effect estimates was seen between RCTs and observational studies. Conclusion: The QT autograft had comparable graft survival, functional outcomes, and stability outcomes compared with BPTB and HT autografts. However, donor site morbidity was significantly worse with the QT autograft than with BPTB and HT autografts.

Background:Impaired quadriceps muscle strength after anterior cruciate ligament reconstruction (ACLR) is associated with worse clinical outcomes and a risk of reinjuries. Yet, we know little about quadriceps muscle strength in patients reconstructed with a quadriceps tendon (QT) graft, which is increasing in popularity worldwide.Purpose:To describe and compare isokinetic quadriceps strength in patients undergoing ACLR with a QT, hamstring tendon (HT), or bone–patellar tendon–bone (BPTB) autograft.Study Design:Cross-sectional study; Level of evidence, 3.Methods:We included patients with QT grafts (n = 104) and matched them to patients with HT (n = 104) and BPTB (n = 104) grafts based on age, sex, and associated meniscal surgery. Data were collected through clinical strength testing at a mean of 7 ± 1 months postoperatively. Isokinetic strength was measured at 90 deg/s, and quadriceps strength was expressed as the limb symmetry index (LSI) for peak torque, total work, torque at 30° of knee flexion, and time to peak torque.Results:Patients with QT grafts had the most impaired isokinetic quadriceps strength, with the LSI ranging between 67.5% and 75.1%, followed by those with BPTB grafts (74.4%-81.5%) and HT grafts (84.0%-89.0%). Patients with QT grafts had a significantly lower LSI for all variables compared with patients with HT grafts (mean difference: peak torque: −17.4% [95% CI, −21.7 to −13.2], P < .001; total work: −15.9% [95% CI, −20.6 to −11.1], P < .001; torque at 30° of knee flexion: −8.8% [95% CI, −14.7 to −2.9], P = .001; time to peak torque: −17.7% [95% CI, −25.8 to −9.6], P < .001). Compared with patients with BPTB grafts, patients with QT grafts had a significantly lower LSI for all variables (mean difference: peak torque: −6.9% [95% CI, −11.2 to −2.7], P < .001; total work: −7.7% [95% CI, −12.4 to −2.9], P 90% for peak torque (QT: 67.5% [95% CI, 64.8-70.1]; HT: 84.9% [95% CI, 82.4-87.4]; BPTB: 74.4% [95% CI, 72.0-76.9]).Conclusion:At 7 months after ACLR, patients with QT grafts had significantly worse isokinetic quadriceps strength than patients with HT and BPTB grafts. None of the 3 graft groups reached a mean LSI of >90% in quadriceps strength.

Abstract:Purpose Bone patella‐tendon bone (BPTB) and hamstring tendon (HT) autografts are the most used grafts in primary anterior cruciate ligament (ACL) reconstructions (ACLR) in Norway. Quadriceps tendon (QT) autograft has gained more popularity during the past years. The purpose of this study is to compare revision rates and patient‐reported outcomes of primary QT with BPTB and HT autograft ACL reconstructions in Norway at 2‐year follow‐up. It was hypothesized that there would be no difference in 2‐year revision rates between all three autografts. Methods Data included primary ACLR without concomitant ligament surgeries, registered in the Norwegian Knee Ligament Register from 2004 through 2021. Revision rates at 2 years were calculated using Kaplan–Meier analysis. Hazard ratios (HR) for revision were estimated using multivariable Cox regression analysis with revision within 2 years as endpoint. Mean change in patient‐reported outcome was recorded preoperatively and at 2 years through the Knee Injury and Osteoarthritis Outcome Score (KOOS) subcategories 'Sport' and 'Quality of Life' was measured for patients that were not revised and analysed with multiple linear regression. Results A total of 24,790 primary ACLRs were identified, 10,924 with BPTB, 13,263 with HT and 603 with a QT graft. Patients in the QT group were younger (23.5 years), more of them were women (58.2%) and over 50% had surgery <3 months after injury. Abstract:Objective: The purpose is to compare functional outcomes, return to soccer rates, and revision rates in an all-female soccer player cohort undergoing quadriceps tendon (QT) autograft ACLR versus bone-patellar tendon-bone (BPTB) autograft ACLR. Methods: Female soccer players who sustained an ACL rupture and underwent primary anatomic, single-bundle ACLR with BPTB autograft or QT autograft were included. Demographic and surgical characteristics were collected. Outcomes of interest included Tegner score, International Knee Documentation Committee (IKDC) score, Marx score, return to soccer rates, and failure rates. Results: Data on 23 patients undergoing BPTB autograft ACLR and 14 undergoing QT autograft ACLR was available. Average age was 18.7 years, and average follow up was 4.8 years. Overall, 76 % (28/37) returned to soccer and 5.4 % (2/37) underwent revision ACLR. No major significant differences were found in demographic or surgical characteristics. No differences were found in postoperative IKDC scores, preoperative, postoperative, or change from pre-to postoperative Marx activity scores, or pre-and postoperative Tegner scores between the groups. QT autograft ACLR patients had significantly less change in Tegner scores pre-to postoperatively compared to the BTPB autograft ACLR group (0.6 ± 1.2 versus 2.1 ± 1.8; p = 0.02). Both groups had similar rates of return to soccer [78 % (18/23) BPTB autograft ACLR versus 71 % (10/14) QT autograft ACLR; p = 0.64] and rates of revision (8.7 % (2/23) BPTB autograft ACLR; 0 % (0/14) QT autograft ACLR. Conclusion: Results of this study suggest that BPTB autograft ACLR and QT autograft ACLR produce comparable, successful functional and return to soccer outcomes in this all-female soccer player cohort study. Larger, prospective studies are needed to improve the strength of conclusions and provide more information on the optimal graft choice for female soccer players. Surgeons can use the results of this study to counsel female soccer players on expected outcomes after ACLR. Level of evidence: III.

Abstract:We report two cases of primary pulmonary hypertension (PPH) who benefited from oral sildenafil therapy. Both demonstrated a substantial improvement in exercise ability, which has been sustained at 3 and 6 months. Sildenafil acting as a phosphodiesterase 5 inhibitor may have an important role to play in the management of PPH and we believe further study to be of importance.

Background:Patellar fracture, femoral physis injury, and recurrent instability are concerning complications in medial patellofemoral ligament (MPFL) reconstruction (MPFLR) techniques for recurrent patellar dislocation in children and adolescents.Purpose:To evaluate the outcomes of an anatomic all–soft tissue fixation technique for reconstruction of the medial patellofemoral complex (MPFC) using a double-bundle quadriceps tendon (QT) autograft for recurrent patellar dislocation in skeletally immature patients.Study Design:Case series; Level of evidence, 4.Methods:This retrospective study involved 24 skeletally immature patients (24 knees; 16 women and 8 men; age range, 9.5-15 years) with recurrent patellar dislocation who underwent MPFC reconstruction using a double-bundle QT autograft between September 2018 and January 2021. Only soft tissue suture fixation was used on the femoral and patellar sides of the 2 bundles of the QT. Radiographs, computed tomography, and magnetic resonance imaging were used to evaluate physeal status, lower limb alignment, patellar height and tilt, trochlear morphology, tibial tubercle-trochlear groove distance, and any associated knee pathology. Functional outcomes were assessed with the Kujala score, the visual analog scale (VAS) for pain, and the grading system of Insall et al.22Results:The mean follow-up time was 40 ± 9.6 months (range, 28-56 months). At the final follow-up, the Kujala and VAS pain scores showed a significant improvement versus preoperative scores (P < .001), and the passive lateral patellar glide showed a significant reduction (P < .001). All patients had negative apprehension and J signs. Of the 24 patients, 23 regained full range of motion, while 1 patient had a knee flexion deficit. The patellar tilt angle improved significantly at the final follow-up (P < .001). There was no patellar fracture, femoral physis injury, or recurrence of patellar dislocation. According to the grading system of Insall et al, the results were excellent in 15 knees (62.5%), good in 8 knees (33.3%), fair in 1 knee (4.2%), and no knees showed poor results.Conclusion:Reconstruction of the MPFC using a double-bundle QT autograft with an all–soft tissue fixation technique was an effective method for treating patellar instability in skeletally immature patients.

No Clinical Advantage of Harvesting a Patellar Bone Block Compared to All Soft Tissue Graft in Primary Quadriceps Tendon Anterior Cruciate Ligament Reconstruction

Abstract:Purpose: To compare patient-reported outcomes measurements (PROMs) and subsequent surgical interventions in patients treated with anterior cruciate ligament reconstruction (ACLR) using either quadriceps tendon autograft with a patellar bone block (bQT) or soft tissue only (sQT). Methods: All ACLRs performed between 01.2010 and 03.2022 were prospectively followed for 24 months and retrospectively evaluated. All primary ACLRs with full 24 months of follow-up data, without any previous surgery or any additional ligamentous interventions, were matched for major ACL risk factors, including gender, age, Tegner activity level (TAL), pivoting sports, and concomitant injuries. PROMs (Lysholm score, Visual Analog Scale (VAS) for pain, and TAL) and subsequent surgical interventions were registered after 6, 12, and 24 months postoperative. Binary logistic regression was used to assess the influence of graft type, age, preinjury TAL, gender, pivoting sports and concomitant interventions on the need to undergo subsequent surgery. Results: After matching, 246 patients were included in the final analysis. Both groups did not differ regarding any preoperative patient demographics or intraoperative details. At final follow-up, no significant difference in mean Lysholm score (sQT: 90.8±10.6, bQT: 91.8±10.6, p= .46), median TAL (sQT: 6 [1-10], bQT: 6 [1-10], p= .53) and VAS for pain (sQT: 0.7±1.1, bQT: 0.7±1.2, p= .70) was reported between both groups. 70.3% (sQT-A: 70.7%, bQT: 69.9%, p= .89) of patients returned to or exceeded their preinjury activity level. In terms of revision ACLR, there was no statistically significant difference between bQT (3.3%) and sQT (4.1%). Similarly, no difference was observed in contralateral ACLR (bQT: 7.3% sQT: 11.4%). Regression analysis indicated that none of the studied factors, including the use of a patellar bone block, influenced subsequent surgery, revision ACLR, or contralateral ACLR. Conclusion: Harvesting an additional patellar bone block in quadriceps tendon ACL reconstruction does not seem to impact postoperative patient-reported outcomes, ACL revision- or contralateral ACL reconstruction rates. Background:The incidence of anterior cruciate ligament (ACL) reconstruction is increasing, and quadriceps tendon (QT) autograft is gaining popularity for both primary and revision ACL reconstruction.Purpose:To evaluate the differences in the patient-reported functional outcomes, concomitant injuries, and graft failure in primary versus revision ACL surgery using QT autograft.Study Design:Cohort study; Level of evidence, 3.Methods:A total of 376 patients with primary ACL reconstruction and 138 patients with revision ACL reconstruction were retrospectively retrieved from a prospectively collected ACL registry. A minimally invasive technique was used for QT autograft harvesting. The surgical procedure and rehabilitation protocol were identical in both groups. To maintain a homogeneous cohort for the study, the groups were matched by age, sex, and preinjury outcome scores (Lysholm knee score, Tegner activity level, and visual analog scale [VAS] for pain). Initial baseline assessments of outcome scores were compared with scores collected at the 2-year postoperative mark.Results:The mean age of the primary group and revision group was 32.9 ± 10.2 years (range, 18-55 years) and 32.3 ± 9.9 years (range, 19-55 years) respectively. Significant preinjury to postoperative improvements were noted in Lysholm (88.2 ± 16.4 vs 83.5 ± 15.0; P = .007) and VAS pain (0.9 ± 1.3 vs 1.5 ± 1.6; P = .001) scores after primary ACL reconstruction compared with revision reconstruction. However, no significant difference was found in Tegner activity level (6.7 ± 1.8 vs 5.9 ± 1.8; P > .430). Primary ACL injury was associated with significantly higher concomitant medial collateral ligament injuries (P = .019), while the revision group was associated with significantly higher concomitant cartilage (P = .001) and meniscal (P = .003) injuries. A significantly higher graft failure rate was noted in the revision group compared with the primary ACL reconstruction group (13.0% vs 5.6%; P = .005).Conclusion:Both primary and revision ACL reconstruction with QT autograft had acceptable functional outcomes. The primary group had better outcomes than the revision group, possibly due to the lower prevalence of meniscal and cartilage injuries in the primary group compared with the revision group. The revision group was associated with higher graft failure than the primary group. QT autograft is a viable graft choice for both primary and revision ACL reconstruction.

Abstract:Essential tremor (ET) is a common movement disorder in adults and the condition has also been reported in children [1,2]. In the vast majority of patients, a positive family history can frequently be elicited. Current evidence suggests that the inheritance of ET can be autosomal dominant or follow a complex inheritance pattern. Efforts to identify ET susceptibility loci have proved challenging. Thus far, linkage analysis studies in Icelandic and North American families have identified three genetic loci in chromosome 3q13 (ETM1) [3], 2p22-p25 (ETM2) [4] and 6p23 [5], where the causative ET genes may be located. As many families do not show evidence for linkage to any of these loci, multiple yet undiscovered ET genes probably exist. Like in other complex disorders, there have been attempts to unravel common genetic risk factors that might contribute to the etiology of apparent sporadic forms of ET [6–12]. Higgins et al. [9] described a putative mutation (A265G) in the HS1-BP3 gene that was found to associate with familial ET. However, the excitement for this observation abated when the variant was subsequently found in normal controls and could not be consistently replicated in patients [10]. More recently, a Ser9Gly variant in the DRD3 gene located in the ETM1 locus was demonstrated to associate with risk and age at onset of ET [11]. The variant also appears to co-segregate with ET in some families [12]. The DRD3 is of interest as it encodes the dopamine D3 receptor, a member of the D2 family of dopamine receptors. D3 receptors may be involved in the regulation of locomotion as DRD3 protein is expressed in the Purkinje cells of the rat cerebellum, which has been implicated in the pathogenesis of ET [11]. In vitro, DRD3 has been shown to mediate ERK activation and the functional Gly9 variant elevates dopamine affinity and dopamine-mediated cyclic-AMP response and prolongs the mitogen-associated protein kinase signal. Replication of a genetic association in independent populations is the litmus test of its validity. Two studies in this issue of EJN attempted to address the replicability involving the DRD3 variant. Vitale et al. [13] in a case control study in the Italian population did not find such an association in their familial ET patients. The authors also genotyped the variant in 76 affected and 20 non-affected subjects from 16 ET families. Co-segregation of the Gly-9 allele was suggested in only three families. However, the strength of this co-segregation is not clear as details were not provided. Inashkina et al. [14] analyzed short tandem repeat markers located within the ETM 1 and ETM2 loci, but no association of these markers with ET was detected in the Latvian population. Furthermore, they found no significant difference in the frequency of the DRD3 Ser9Gly variant between cases and controls. A similar study also did not detect the association in the Asian population [6]. Blair et al. [15] analyzed the association with this variant using linkage analysis, association study and transmission disequilibrium test in a much larger group of ET patients and controls and arrived at a similar conclusion. The failure by various groups to replicate the DRD3 association with ET is disappointing and suggests that the Ser9Gly variant is unlikely to play a major role in ET susceptibility in the majority of patients. However, it is timely to remind ourselves that findings from both hypothesis-generating and replication studies in ET genetic association studies are frequently influenced by various confounding variables. For most published studies, the sample size remains relatively small, the study populations vary (including a mix of familial and sporadic cases), family history is frequently unreliable with no validation [16], and clinical diagnostic criteria are variably applied. To compound these problems, there is significant clinical and genetic heterogeneity among ET patients, and incomplete penetrance is an important confounding factor in linkage analysis. Misdiagnosis can occur as ET can also be an initial presentation of other genetic disorders [17,18]. Several environmental factors have been postulated in ET pathogenesis, and the magnitude of the gene–environmental interaction in the common garden variety of ET is unclear. Because of all these reasons, cautious interpretation of genetic association studies in ET is advised. To surmount these problems, multiple approaches are needed to unravel causative genes of ET. Perhaps, one should focus on selected groups of ET patients who are genetically enriched. In ET families with clear Mendelian inheritance, a mapping–cloning strategy including parametric linkage analysis and candidate gene screening remains the norm. However, long-term follow-up and re-evaluation of the initial analysis of non-affected subjects will be important. In sporadic patients, validation of a link through comparative analysis of linkage disequilibrium and disease-association patterns in the very young onset patients may be potentially fruitful. These can be reinforced by studies involving sibpairs, monozygotic twins, and nuclear families.
