Abstract:Drinking alcohol during adolescence has short-term and long-term effects on physical and mental health. At this stage, teenagers are greatly influenced by their schoolmates and friends. We conducted a multicenter cross-sectional study to investigate the association between school environment factors and adolescents' drinking behavior in China. Using multistage stratified cluster sampling, the study investigated 27,762 middle school students from six cities in China. The logistic regression model was used to explore the association between the school environment and adolescent alcohol drinking behaviors after adjusting for confounders, including gender, age, city, location, and smoking status. Compared with students with none of their close friends drinking, students with more than half of their close friends drinking were more likely to drink in a year (OR = 20.148, 95% CI: 17.722–22.905, p < 0.001) and in a month (OR = 13.433, 95% CI: 11.779–15.319, p < 0.001). In addition, classmates' drinking behaviors, friends' persuasion, and attending parties were risk factors for adolescents' drinking behavior, while the propaganda and regulations of banning drinking in school were protective factors. The school environment, especially friends drinking, is associated with students' drinking behavior. It is necessary to mobilize the strength of schools and peers to strengthen the prevention and control of adolescent drinking.

Individual and Regional Factors Affecting Stress and Problem Alcohol Use: A Representative Nationwide Study of China

Abstract:OBJECTIVES:The objective of this study was to examine the association between individual and environmental stressors and problem alcohol use among Chinese university students.METHODS:Participants were 11,942 students, who were identified through a multistage survey sampling process that included 50 universities. Individual information, including feelings of stress and perceptions of problem alcohol use, was obtained by self report. Urban and regional variables were retrieved from the National Bureau of Statistics database. Both unadjusted and adjusted methods were considered in the analyses.RESULTS:Almost one third (32.6%) of the students suffered from some form of severe stress while problem alcohol use prevalence was 7.3%, (95% CI: 4.1-10.4%). The multilevel logistic regression model found that uncertainty stress, gender, father's occupation and monthly expenses were associated with problem alcohol use. Of the contextual factors home region and the university city GDP and unemployment rate were important. When interactions were considered, the relationship between monthly expenses and financial uncertainty and problem drinking was most evident in high level universities. By contrast, the effects of uncertainty stress on problem drinking were most evident in middle and low level universities.CONCLUSIONS:The findings underscore that efforts to control problem alcohol use among students in China should pay greater attention to environmental determinants of stress and particularly to improvements in stress management in university settings.

Abstract:The current study was designed to investigate the drinking patterns and association between socio-demographic factors and adolescents' alcohol use among high school students from China's three metropolises, Beijing, Shanghai, and Guangzhou. Using a self-administered questionnaire, we conducted a cross-sectional survey among 13,811 high school students from 136 schools between May and June 2013. A two-stage stratified sampling method was used for subject selection. The prevalence of lifetime drinking was 52.5%; in addition, 38.5% of the students were past-year drinkers, while 20.1% of them had consumed alcohol in the past 30 days. During the past year, 29.7% of the students reported that they drank once per month or less, and 22.0% of the students drank less than one standard drink (SD) per occasion. For the students who were not living with their mothers, as well as the students in higher socioeconomic status (SES), the adjusted odds of past and current drinking were significantly higher, compared with those who lived with both parents and low SES. Due to the high prevalence of alcohol consumption among junior and senior high school students in metropolises, attention should be paid by parents, school administrators, educational and public health agencies for making efforts collectively to reduce alcohol availability and drinking among adolescents.

Abstract:Background Alcohol drinking among adolescents is associated with their health development. However, the prevalence of alcohol drinking among adolescents in Southwestern China remains largely unexplored. This study aimed to investigate the prevalence of alcohol drinking, with a particular focus on gender differences, among primary and middle school students in Zigong, a city in Southwestern China. Additionally, we examined the association between alcohol consumption and demographic and family factors. Methods A school-based cross-sectional survey was conducted in a city in Southwestern China, encompassing a total of 89 360 students from 132 different ordinary schools, including both primary and middle schools. Participants were recruited through cluster sampling. The Alcohol Use Disorders Identification Test Consumption was employed to assess alcohol consumption. Gender differences in the prevalence of alcohol drinkers across various schools and grades were analysed. Multivariable logistic regression analysis was used to investigate factors associated with hazardous drinking. Results Out of the 89 360 participants, 19.0% reported alcohol drinking, with 2.1% classified as hazardous drinkers. There was a higher prevalence of alcohol drinking among boys compared with girls, as well as hazardous drinking. There were significant gender disparities in alcohol drinking observed across various schools and grade levels. A notable divergence between boys and girls was observed starting from grade 10, with a rising prevalence of hazardous drinking among boys and a decline among girls. Additionally, older age, male gender and being left-behind children were identified as risk factors for hazardous drinking, while belonging to a nuclear family and having parents who do not drink were protective factors against hazardous drinking. Conclusions Alcohol consumption is prevalent among Chinese adolescents, with some even classified as hazardous drinkers. These findings may offer valuable insights for policymakers and caregivers, guiding them in formulating appropriate interventions and support strategies.

Abstract:Objective:Adolescents' alcohol consumption has lifetime adverse physical and mental health effects. Family environment factors have a significant influence in shaping adolescents' beliefs and habits. We conducted the multicenter cross-sectional study aiming to investigate the association between family environment factors and adolescent drinking behavior in China.Methods:The study investigated 27,762 middle school students from Beijing, Shanghai, Guangzhou, Jinan, Chengdu, and Harbin. A logistic regression model was used to explore the association between family environmental factors and adolescent drinking behavior. Participants were asked to self-report previous experiences of drinking and getting drunk to access their drinking status. Factors of family environment related to alcohol consumption included: parents' educational level, family economic status, family composition, the number of times parents drank alcohol in the past 30 days, and parents' attitudes toward their drinking behavior. The logistic regression model was used to adjust the demographic confounders, including gender, age, city, location, and smoking status, and to explore the association between family environmental factors and adolescent alcohol drinking behaviors.Results:Compared with students whose parents prohibited drinking, students who were approved drinking were more likely to drink in this year (OR = 16.544, 95%CI:15.265-17.929, P < 0.001; Full adjustment: OR = 13.111, 95% CI: 12.031-14.288, P < 0.001), drink in this month (OR = 7.791, 95% CI: 7.077-8.565, P < 0.001; Full adjustment: OR = 6.010, 95% CI: 5.439-6.641, P < 0.001). In addition, Low family economic status, not living with the mother, parents' ambivalent attitudes toward their children's drinking and parental drinking were risk factors for drinking among middle school students.Conclusion:The family environment, especially parents' attitudes, is associated with students' drinking and drunken behavior. Mobilizing the power of parents may play a positive role in the effective prevention and control of adolescent drinking.

Abstract:To describe frequency and patterns of alcohol use and explore the association between binge drinking and other health-compromising behaviors among adolescents in urban China.Data on alcohol use and other behavioral risk factors were obtained from the 2004 China Adolescent Behavioral Risk Factor Survey conducted in 18 provincial capitals. Chi-square test was used to estimate the prevalence of alcohol use and compare differences in drinking frequency by gender, school type, and grade. Logistic regression was used to estimate the association between binge drinking and other health-compromising behaviors.Data were available on 54,040 students in grades 7 to 12. In all, 51.1% of students reported ever using alcohol (male: 58.6%; female: 44.3%), 29.7% reported drinking before 13 years of age, and 14.1% had gotten drunk at least once during the past year. In the 30 days preceding the interview, 25.2% students reported consuming at least one alcoholic drink (male: 31.4%; female: 19.6%) and 10.3% reported at least one episode of binge drinking (male: 14.4%; female: 6.6%). Male students from vocational senior high schools reported the highest frequency of alcohol use. The prevalence of other risk behaviors, such as smoking, drug use, and fighting were significantly higher among students with a positive history of binge drinking.Alcohol use among urban adolescents is a major problem in China. A comprehensive alcohol control policy is needed, incorporating measures to educate the public about the dangers of adolescent alcohol use and regulating access and purchase of alcohol by minors.

Abstract:Background Adolescent alcohol use was a major social and public health concern given its negative impacts. Previous studies indicated gender role attitudes (GRA) were associated with alcohol use; however, few studies focused on early adolescents (10 to 14 years) and similar researches were not found in China. Objective: This study sought to explore the association between GRA and alcohol use among early adolescents. Methods: A cross-sectional study was conducted among students in grades six to eight across three public secondary schools in Shanghai, China. Data were collected by Computer-Assisted Self-Interview (CASI) questionnaire on mobile tablets. Alcohol use was measured by self-report, and six items with 5-point Likert-type options assessed the attitude toward traditional gender roles. The logistic regression model was adopted to examine the associations between GAR and alcohol use. Results: Totally 1,631 adolescents aged 10 to 14 years with 50.33% of boys included in this study. The mean score of GRA in drinkers was significantly higher than nondrinkers among boys (3.03 vs. 2.78, p < 0.001) but not girls (2.15 vs. 2.18, p = 0.499). After controlling the covariates of age, depression, peers' substance use, social cohesion, etc., we found that more traditional GRA was associated with a higher risk of alcohol use among boys (OR = 1.39, 95%CI: 1.11-1.75), whereas the association was not significant among girls (OR = 0.96, 95%CI: 0.74-1.24). Conclusions: Traditional GRA may increase the risk of alcohol use among early adolescent boys, suggesting that altering traditional GRA among this population group may help to prevent alcohol use.
The impact of the peer effect on adolescent drinking behavior: instrumental-variable evidence from China

Abstract:Background: Governments around the world have taken measures to limit adolescent drinking, however, rates are still alarmingly high. However, most of these measures ignore the peer effect of drinking among adolescents. Previous studies have not sufficiently considered the reciprocal relationship between adolescent alcohol consumption and peer alcohol consumption, which may lead to an overestimation of the peer effect and mask underlying issues. Good instrumental variables are powerful but rare tools to address these issues. Objective: This paper aims to correctly estimate the peer effect of drinking on adolescent drinking behavior in China. Methods: Owing to the detailed information of household background in the dataset of our survey, we were able to use the drinking behaviors of peers' fathers and their beliefs about the health risks of alcohol as instrumental variables, which are more powerful than school-average instrumental variables. We collected data from the 2017 Health and Nutrition Panel survey, which surveyed 10,772 primary school students from 59 urban migrant and 60 rural public schools. Results: The instrumental variable method estimation revealed that peer drinking significantly influences adolescent drinking behavior, with adolescents who have peers who drink alcohol being 10.5% points (2 stage least square, i.e., 2SLS, full sample estimation) more likely to engage in drinking compared to those without such peers. Furthermore, the effect differs significantly between migrant and rural adolescents. Conclusion: The study found that parental care plays a significant role in the degree of peer effect, with the absence of parental care being a key factor in the presence of the peer effect.

Abstract:Objective To describe the drinking behavior of high school students in Beijing,and explore possible preventive interventions.Methods The study was conducted between April and May in 2014.An anonymous questionnaire survey was conducted among 15 726 high school students who were selected by stratified cluster sampling method from 18 districts of Beijing.Results Of students surveyed,67.9％ of them reported ever-using alcohol,35.2％ reported drinking before 13 years of age.During the past 30 days,41.0％ students reported consuming at least one alcoholic drink and 16.6％ reported at least one episode of binge drinking.The main reason for drinking alcohol was to be with their families or peers.Boys were more likely to drink alcohol than girls.Key high school students were less likely to drink alcohol.Students who had other health related risk behaviors were more likely to have alcohol drinking related behaviors,i.e.students who smoked were more likely to drink alcohol during the past 30 days (OR=4.41,OR95％CI=3.90-4.98),who had fighting behavior were more likely to have binge drink(OR =2.00,95％CI=1.77-2.26),who had sexual intercourse were more likely to drink before 13 years of age(OR=1.58,95％CI=1.36-1.84),and who had suicidal behavior were more likely to have binge drink(OR=1.67,95％CI=1.27-2.19).Conclusion Age,family,sex and health related risk behaviors such as smoking,fighting and sexual intercourse etc.should be considered in preventing drinking related be-haviors among high school students.The comprehensive health education of drinking behavior should be implemented early and actively in families and schools.

Abstract:To examine the association between socioeconomic status (SES) and adolescent alcoholic beverage preferences and the associated drinking patterns in China. The study used cross-sectional data collected from 136 junior or senior high schools, using a self-administered questionnaire. A total number of 7,075 subjects of drinking students were selected from three metropolises (Beijing, Shanghai, and Guangzhou) via a two-stage stratified sampling method. Among the adolescent drinkers, 87.8% (95% CI: 86.5-89.0) reported that they drunk alcohol during the past years preceding the study, while 42.4% (95% CI: 40.4-44.4) of the subjects stated that they had drunk alcohol during the past 30 days. There were gradual increases in the usual quantity (>1 Standard Drink, SD) of alcoholic beverages with increasing SES, with highest rates reported by the high-level SES. Beer and grape wine were the most widely consumed alcoholic beverage, regardless of SES. Our findings suggest that high-level SES students have an increasing prevalence of drinking behaviour. Their confirmation by future studies which extend the sampling regions is required to further the prevention of adolescent alcohol abuse in China.

Abstract:Parental psychological control is strongly associated with adolescents' drinking. We used self-determination theory as a conceptual framework for testing the mediating effect of academic efficacy in the association between parental psychological control and adolescent drinking behavior, and the moderating effects of teacher-student relationship and sensation seeking in this indirect process. Moreover, we tested the above model in boys and girls separately to test gender differences. A total of 1,007 junior middle school students (Mean age = 13.16 years, SD = 0.67 years, 48.2% male) in south China completed questionnaires in their classrooms. We conducted a path analysis in Mplus 8.0 to test our moderated mediation model. The results showed that: (1) the influence of parental psychological control on drinking behavior was only significant among girls; (2) academic self-efficacy significantly mediated the relationship between parental psychological control and girls' drinking behavior; (3) the first and second parts of the mediating pathways were moderated by teacher-student relationship and sensation seeking, respectively. The current findings highlight the need to address individual, family, and school factors in prevention and intervention programs for girls' drinking in adolescence.

Abstract:Abstract Background Gender differences in alcohol use are more substantial among early adolescents in China than in the United States, presumably because of more permissive drinking norms for boys than girls in Chinese culture. This study tested a theory that gender differences in early experimentation with alcohol can be reduced through general parenting practices. Whereas traditional research has identified mediators of gender differences in alcohol use, the current research isolated moderators of gender differences and developed their implications for prevention programs. Methods The study analyzed the data from the China Global School-Based Student Health Survey ( n = 8805 middle school students in four cities). Youth completed anonymous surveys in classroom settings. The study examined interaction effects between gender and parenting variables using multiple regression with robust standard errors. Results Early adolescent boys exhibited higher levels of drinking than girls for all drinking outcomes. The gender differences in drinking were negatively associated with the level of perceived parental monitoring, parental involvement in adolescent school performance, and parental empathy in a nonlinear way. Conclusions Results suggested that early adolescents’ perceptions of general parenting practice nonlinearly moderated gender disparities in alcohol use.

The Epidemiology of Alcohol Consumption and Misuse among Chinese College Students.

Abstract:AIMS:To understand alcohol-related risk behaviours among Chinese college students.METHODS:As part of the first China National Youth Risk Behaviour Survey, undertaken in 2009, 52,150 students at 119 colleges were randomly sampled. Information was obtained from self-administered questionnaires.RESULTS:Prevalences were: lifetime drinkers 80.8%, current drinkers 49.3% (drank alcohol in past 30 days) and binge drinkers 23.5% ('binge drinkers' reporting at least five alcoholic drinks on a single occasion at least six times during the past 30 days). Multinomial logistic analysis revealed the contribution of sociodemographic factors to three high-risk drinking behaviours: odds ratio (95% confidence interval) = 3.64 (2.69-4.60) with frequent drinking; 3.27 (1.82-4.72) with binge drinking; and 5.48 (3.20-7.77) with heavy binge drinking. These three rates were greater among males than females, in the Western more than the Eastern region, among students living off-campus and among those whose mothers had higher education. Heavy drinking was linked to lower academic self-rating.CONCLUSION:There is a trend towards risky drinking among Chinese college students. Measures such as a minimum drinking age, advertisement restrictions, taxation, drunk-driving penalties and campaigns to heighten public awareness of alcohol-related health risks should be instituted in order to improve the situation on college campuses where alcohol abuse is particularly prevalent.

Combined Associations of Smoking and Bullying Victimization with Binge Drinking among Adolescents in Beijing, China

Abstract:Background: Binge drinking and smoking among adolescents are serious public concerns. However, very few studies have explored the reinforcement of bullying victimization by such behavior. Our study aimed at examining the individual and combined associations of smoking and bullying victimization with binge drinking among adolescents in Beijing, China.Methods: A total of 33,694 students aged 13–17 years old in Beijing, China were anonymously investigated via the cross-sectional Chinese Youth Risk Behavior Surveillance Survey from April to May 2014. A three-stage stratified sampling was used to select participants. Factors such as sociodemographic variables and indicators of smoking, bullying victimization, and binge drinking were analyzed with multiple logistic regressions, and joint and additive interaction effects were tested.Results: Overall, ever-drinking prevalence was 59.1% (boys: 64.4%; girls: 53.7%). Past 30-day binge drinking was 11.5% (boys: 15.6%; girls: 7.4%) and frequent binge drinking was 2.3% (boys: 3.3%; girls: 1.0%). Past 30-day smoking was 10.7% (boys: 16.4%; girls: 5.0%) and past 30-day bullying victimization was 48.7% (boys: 57.3%; girls: 40.1%). The combined effects of smoking and bullying victimization on occasional binge drinking (OR = 6.49, 95% CI = 5.60–7.52) and frequent binge drinking (OR = 10.32, 95% CI = 7.52–14.14) were significant, and the additive interaction effect was significant for current smoking and bullying victimization on frequent binge drinking (OR = 10.22, 95% CI = 9.43–11.07). The additive interaction effect for current smoking and bullying victimization on frequent binge drinking was significant among boys.Conclusion: Bullying victimization reinforced the association of smoking with frequent binge drinking, especially with findings specific to boys. Programs to prevent smoking or bullying or both may reduce binge drinking among adolescents in China.

Abstract:This study assessed the association between accessibility of catering service venues and adolescents' alcohol use over the previous 30 days. The data were collected from cross-sectional surveys conducted in 2014, 2223 students at 27 high schools in Chaoyang and Xicheng districts, Beijing using self-administered questionnaires to collect the adolescents information on socio-demographic characteristics and recent alcohol experiences. The accessibility of, and proximity to, catering service venues were summarized by weights, which were calculated by multiplication of the type-weight and the distance-weight. All sampled schools were categorized into three subgroups (low, middle, and high geographic density) based on the tertile of nearby catering service venues, and a multi-level logistic regression analysis was performed to explore variance between the school levels. Considering the setting characteristics, the catering service venues weighted value was found to account for 8.6% of the school level variance of adolescent alcohol use. The odds ratios (OR) and 95% confidence intervals (CI) of drinking over the past 30-days among adolescents with medium and high accessibility of catering service venues were 1.17 (0.86, 1.57) and 1.47 (1.06, 2.02), respectively (p < 0.001 for trend test). This study addressed a gap in the adolescent drinking influence by the catering service venues around schools in China. Results suggest that the greater accessibility of catering service venues around schools is associated with a growing risk of recent drinking.

Abstract:In the current study, we conducted daily telephone interviews with a sample of Chinese workers (N = 57) for 5 weeks to examine relationships between daily work family conflict and alcohol use. Drawn from the tension reduction theory and the stressor vulnerability model, daily work family conflict variables were hypothesized to predict employees' daily alcohol use. Further, social variables (i.e., peer drinking norms, family support, and coworker support) were hypothesized to moderate the relationship between work family conflict and alcohol use. Results showed that daily work-to-family conflict but not family-to-work conflict had a significant within-subject main effect on daily alcohol use. In addition, there was significant between-subject variation in the relationship between work-to-family conflict and alcohol use, which was predicted by peer drinking norms, coworker support, and family support. The current findings shed light on the daily health behavior consequences of work family conflict and provide important theoretical and practical implications.
Daily Social Pressure and Alcohol Consumption among Chinese Women: A Cross-Sectional Study

Abstract:The purpose of this study was to examine the daily social pressure and socioeconomic factors related to women's alcohol consumption in China. Cross-sectional data were obtained from the 2012 China Family Panel Studies. A multivariate logistic regression analysis of a sample of 16 339 female adults with the mean age of 45.3 years was used to examine the relationships between dependent and independent variables. According to the results, first, the greater the daily social pressure, the more likely women were to engage in general alcohol consumption (odds ratio = 1.061) and risk drinking (odds ratio = 1.057). Second, while there is a positive relationship between the general level of social pressure and women's alcohol consumption, the relationship between the severe level of social pressure and women's alcohol consumption was not significant. Finally, women in the Central region were less likely to engage in risk drinking than women in the Western region; women with secondary school education were more likely to engage in risk drinking than women with primary school education or below; and age was significantly positively associated with both general and risk drinking. In conclusion, increasing alcohol consumption among women may be due to increased social pressure.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Adolescent alcohol use was found to be influenced by parental and peer alcohol use. Drinking behaviors also differed by gender. However, previous studies mainly focused on adolescents' substance use in relation to the same substance use among their salient others. Hence, this study investigated the cross-substance relationships of alcohol and tobacco use of parents and peers with adolescents' problematic alcohol use, separately by gender.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Data were derived from 4,445 adolescents aged 12–17 years from the 2014 National Survey of Substance Use, a nationally representative survey in Taiwan. Problematic alcohol use was assessed using the Alcohol Use Disorders Identification Test (AUDIT). Multivariate multinomial logistic regression was used, stratified by gender.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>For males, maternal (adjusted odds ratio [aOR] = 1.73) and peer (aOR = 2.57) alcohol use was related to social drinking (AUDIT < 2); paternal (aOR = 3.58), maternal (aOR = 2.18), peer alcohol use (aOR = 5.37), and their own tobacco use (aOR = 4.72) were related to problem drinking (AUDIT ≥ 2). For females, maternal (aOR = 2.26) and peer (aOR = 2.84) alcohol use was related to social drinking; maternal (aOR = 2.35) and peer tobacco use (aOR = 3.48), and paternal (aOR = 4.56) and peer alcohol use (aOR = 3.36) were linked to problem drinking.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Both male and female adolescents' alcohol use was associated with their peer alcohol use, and gender differences were found in relation to their parental and peer substance use. Specifically, the parental role-modeling of smoking was only significant in mother-daughter dyads. These findings could inform multifaceted adolescent alcohol prevention programs, tailoring for males and females and also targeting their parental and peer substance use.</p>
