Abstract:Background Increasing muscle strength may allow an athlete to improve their performance while reducing the risk of injury. Blood Flow Restriction (BFR) has emerged as an alternative for increasing muscle strengh because it can promote physiological changes and hypertrophy with a lower degree of mechanical overload. When combined with Neuromuscular Electrostimulation (NME) it offers the possibility of hypertrophy with greater activation of motor units, potentially increasing the effects of BFR. Objectives To evaluate the effects of strength training with BFR combined with NME of the quadriceps muscle in physically active individuals on parameters of strength and muscle activation. Methods This protocol is a randomized clinical trial with a blinded evaluator for groups and statistical analysis. Eligibility criteria will be age between 18 and 35 years old; both sexes and physically active according to the International Physical Activity Questionnaire. The following criteria are not considered appropriate: Body Mass Index over 30; use of stimulants in the 24 hours before the examinations; risk factors for thromboembolism and hypertension. Anthropometric data will be collected, followed by blood pressure measurement and kirtometers. Subjects will be randomly divided into three groups: Blood Flow Restriction Group (BFRG), Blood Flow Restriction and Electrostimulation Group (BFREG) and Conventional Exercise Group (CEG). A Vascular Doppler will be used to measure Total Occlusion Pressure (TOP). Maximum Repetition Test (1RM) unilateral will be used to determine the load during exercise, with adjustment in four weeks. An isokinetic dynamometer in concentric/eccentric mode at two angular speeds will be uses to assess muscle strenght: 60º/s and 180º/s, and isometric strength by the 30s test at 30º and 60º. Surface Electromyography (EMG) will be used to record the electrical activity of the quadriceps muscles. The intervention protocol consists of four sets of 30, 15, 15, 15 repetitions in the chair for extension, with one minute rest between sets, a load of 30% 1RM and 50% of POT, with adjustment of 5% each week up 80% of POT. The BFREG follows the same methodology, with an asymmetric biphasic current, frequency of 50Hz and a pulse duration of 400us. In the CEG the exercise will be performed without intervention in three sets of ten repetitions at 70% of 1RM. The training lasts eight weeks and takes place twice a week, with re-evaluation at the end of the training. The distribution of normality will be analysed by the Shapiro-Wilk test. To analyse the effect of group and the interventions, ANOVA for repeated measures and Bonferroni post test will be performed. The significant level adopted will be 5%. Conclusion It is reasonable to assume that BFR and the intervention associated with electrostimulation are superior to conventional training in terms of strength and muscle recruitment parameters. Implications Because BFR requires a reduced load and it has been hypothesised that its effects are similar to those of conventional training, BRF offers mechanical and physiological benefits. Reduced loading may produce the same results in terms of hypertrophy and increased muscle strength in individuals without joint overload and prolonged loading.

Effects of Resistance Exercise with and without Blood Flow Restriction on Acute Hemodynamic Responses: A Systematic Review and Meta-Analysis

Abstract:Low-load intensity resistance exercise with blood flow restriction (BFR) is an alternative method for enhancing strength and muscle mass. However, acute cardiovascular responses to a complete training session remain uncertain compared to high-load intensity resistance exercise (HI). Therefore, the objective of this study to examine acute and post-exercise hemodynamic responses to low-load BFR and HI protocols. This systematic review and meta-analysis (RD42022308697) followed PRISMA guidelines to investigate whether the responses of heart rate (HR), blood systolic (SBP), blood diastolic pressure (DBP), and rate pressure product (RPP) immediately after and up to 60 min post-exercise from BFR were consistent with those reported after resistance exercises performed at HI in healthy individuals. Searches using PICO descriptors were conducted in databases from January 2011 to December 2023, and effect sizes were determined by Hedge's g. The selected studies involved 160 participants in nine articles, for which the responses immediately after BFR and HI exercises showed no differences in HR (p = 0.23) or SBP (p = 0.57), but significantly higher DBP (p < 0.01) and lower RPP (p < 0.01) responses were found when comparing BFR to HI. Furthermore, the BFR and HI protocols showed no differences regarding SBP (p = 0.21) or DBP (p = 0.68) responses during a 15 to 60 min post-exercise period. Thus, these results indicated that hemodynamic responses are similar between BFR and HI, with a similar hypotensive effect up to 60 min following exercise.
Session rating of perceived exertion following resistance exercise with blood flow restriction

Abstract:Session ratings of perceived exertion (SRPE) provide a valid and reliable indicator of resistance exercise session intensity. However, there is a lack of studies on the effects of resistance exercise with blood flow restriction (BFR) on SRPE. Thus, the aim of this study is to compare the effects of resistance exercise at high intensity versus low intensity with BFR on internal training load measured by SRPE. Thirteen young (22.2 ± 3.8 years) resistance-trained men (training experience 3.2 ± 2.4 years) participated in the study protocol. After determining one maximum repetition (1-RM), the subjects were assigned to two groups in a counterbalanced design (i) high-intensity exercise (HIE, performed one training session at 80% of 1-RM) and (ii) low intensity with BFR (BFR, performed an exercise session at 50% of 1-RM with BFR). During each session, subjects performed three sets of unilateral elbow flexion leading to concentric failure with a 1-min rest interval between sets. A cuff around the arm, inflated at 110 mmHg, was used continuously for BFR. The SRPE was reported 30 min after the end of the session. The low intensity with BFR showed lower total work (197.13 ± 63.49 versus 300.92 ± 71.81 kg; P = 0.002) and higher SRPE (9 versus 6; P = 0.007) than high-intensity resistance exercise. The present results indicate that BFR is an important factor to increase internal training load. Future studies should investigate the physiological stress imposed by different training methods rather than just quantify the external training load such as intensity or volume.

Abstract:ABSTRACT Introduction: Strength training with blood flow restriction (BFR) involves the use of low loads (20-30% of 1RM) with restriction of blood flow to promote gains in physical fitness. The restriction can be applied continuously or intermittently; however, it is unclear how it affects the hemodynamics of hypertensive women. Objective: To analyze the acute effect of resistance exercise (RE) on the upper and lower limbs with continuous and intermittent blood flow restriction (BFR) on the hemodynamic variables of women with hypertension. Methods: Thirteen women with controlled hypertension (40 to 65 years) underwent eight experimental protocols, with a randomized, counter balanced, crossover design; four exercise sessions for the right upper limb (elbow flexion) and four for the right lower limb (knee extension). The systolic arterial pressure (SAP), diastolic arterial pressure (DAP), and heart rate (HR) were measured before, during, immediately after and 15, 30, 45 and 60 minutes after the exercises. Results: There were no significant interactions between the protocols vs. segments vs. time, protocols vs. segments, protocols vs. time, segments vs. time, protocol, segment and time, on the variables SAP, DAP, and HR during and after the RE (p>0.05). Although all these protocols had significantly elevated SAP, DAP and HR, the values remained within the normal range. The protocols of this study did not cause hypotensive effect. Conclusion: Low-load RE combined with continuous and intermittent BFR, on the upper and lower limbs, appears to promote similar variations in the hemodynamic variables of women with hypertension. Physiological adaptations and myocellular stress in short-term, high-frequency blood flow restriction training: A scoping review

Abstract:High frequency (1–2 times per day) low-intensity blood flow restriction (BFR) training has been recommended as a prescription approach for short durations of time to maximize relevant physiological adaptations. However, some studies demonstrate negative physiological changes after short periods of high-frequency BFR training, including prolonged strength decline and muscle fiber atrophy. To provide a comprehensive overview of short-term, high-frequency blood flow restriction training, including main adaptations, myocellular stress, limitations in the literature, and future perspectives. A systematic search of electronic databases (Scopus, PubMed ® , and Web of Science) was performed from the earliest record to April 23, 2022. Two independent reviewers selected experimental studies that analyzed physical training protocols (aerobic or resistance) of high weekly frequency (>4 days/week) and short durations (≤3 weeks). In total, 22 studies were included in this review. The samples were composed exclusively of young predominantly male individuals. Muscle strength and hypertrophy were the main outcomes analyzed in the studies. In general, studies have demonstrated increases in strength and muscle size after short term (1–3 weeks), high-frequency low-intensity BFR training, non-failure, but not after control conditions (non-BFR; equalized training volume). Under failure conditions, some studies have demonstrated strength decline and muscle fiber atrophy after BFR conditions, accompanying increases in muscle damage markers. Significant limitations exist in the current HF-BFR literature due to large heterogeneities in methodologies. The synthesis presented indicates that short-term, high-frequency BFR training programs can generate significant neuromuscular adaptations. However, in resistance training to failure, strength declines and muscle fiber atrophy were reported. Currently, there are no studies analyzing low-frequency vs. high-frequency in short-term BFR training. Comparisons between resistance exercises of similar intensities (e.g., combined effort) are lacking, limiting conclusions on whether the effect is a product of proximity to failure or a specific effect of BFR.

Abstract:The aim of this study was to compare the acute effect of resistance exercise (RE) with and without blood flow restriction (BFR) on heart rate (HR), double product (DP), oxygen saturation (SpO2 ) and rating of perceived exertion (RPE). Twenty-four men (21·79 ± 3·21 years) performed three experimental protocols in a random order (crossover): (i) high-intensity RE at 80% of 1RM (HI), (ii) low-intensity RE at 20% of 1RM (LI) and (iii) low-intensity RE at 20% of 1RM combined with partial blood flow restriction (LI+BFR). HR, blood pressure, SpO2 and RPE were assessed. The data were analysed using repeated measures analysis of variance and the Wilcoxon test for RPE. The results indicated that all protocols significantly increased HR, both immediately postexercise and during the subsequent 60 min (P<0·05), and postexercise DP (P<0·05), but there were no differences between protocols. The protocols of LI and LI+BFR reduced postexercise SpO2 (P = 0·033, P = 0·007), and the LI+BFR protocol presented a perception of greater exertion in the lower limbs compared with HI (P = 0·022). We conclude that RE performed at low intensity combined with BFR seems to reduce the SpO2 after exercise and increase HR and DP while maintaining a perception of greater exertion on the lower limbs.

Very-low-load resistance exercise in the upper body with and without blood flow restriction: cardiovascular outcomes

Abstract:It is proposed that, at very low loads, greater blood flow restriction (BFR) pressures might be required for muscular adaptation to occur. The cardiovascular and hyperemic response to very low loads combined with relative levels of BFR is unknown. Ninety-seven participants were recruited and assigned to 1 of 4 exercise conditions: 15% of 1-repetition maximum (1RM) without BFR (15/00), 15% 1RM with BFR at 40% of arterial occlusion pressure (AOP) (15/40), 15% of 1RM with BFR at 80% of AOP (15/80), and 70% of 1RM without BFR (70/00). Participants performed 4 sets of unilateral biceps curls. Blood pressure was measured before and after exercise; brachial artery blood flow was measured before exercise, following the second set, and 1 min following exercise. Systolic blood pressure increased following exercise in all conditions (+10 (11) mm Hg, P < 0.0005). Diastolic pressure increased in all but 70/00 (+2 (11) mm Hg, P = 0.107). Brachial artery blood flow increased following the second set of exercise in all but 15/80 (+43.4 (76.8) mL·min −1 , P = 0.348). One minute following exercise and cuff deflation, there were no differences in blood flow between conditions (P > 0.05). Similarly, artery diameter was increased in all conditions except 15/80 (+0.002 (0.041) cm, P = 0.853) following the second set, and increased in all conditions by 1 min following exercise (P < 0.05). In conclusion, exercise-induced hyperemia is blunted with increasing pressures of BFR. There is a modest increase in blood pressure at very low loads of resistance exercise in the upper body.

Abstract:Background: Changes in muscle mass, strength, vascular function, oxidative stress, and inflammatory biomarkers were compared in older adults after resistance training (RT) performed with low-intensity without blood flow restriction (RT-CON); low-intensity with BFR (RT-BFR); and high-intensity without BFR (RT-HI). Methods: Thirty-two untrained individuals (72±7 y) performed a 12-week RT after being randomized into three groups: RT-CON -30% of 1 repetition maximum (RM); RT-BFR -30% of 1RM and mild BFR (50% of arterial occlusion pressure); RT-HI -70% of 1 RM. Results: Improvements in handgrip strength were similar in RT-BFR (17%) and RT-HI (16%) vs. RT-CON (-0.1%), but increases in muscle mass (6% vs. 2% and -1%) and IGF-1 (2% vs. -0.1% and -1.5%) were greater (p < 0.05) in RT-BFR vs. RT-HI and RT-CON. Changes in vascular function, morphology, inflammation, and oxidative stress were similar between groups, except for time to reach maximum red blood cell velocity which showed a greater reduction (p < 0.05) in RT-BFR (-55%) vs. RT-HI (-11%) and RT-CON (-4%). Conclusion: RT with low intensity and mild BFR improved muscle strength and mass in older individuals while preserving vascular function. This modality should be considered an adjuvant strategy to improve muscle function in older individuals with poor tolerance to high loads.
Aerobic Training With Blood Flow Restriction on Muscle Hypertrophy and Strength: Systematic Review and Meta-analysis

J Strength Cond Res 38(7): 1341-1349, 2024-Integrating strength and endurance training in a single exercise session, even on separate days, can be physically demanding and time-consuming. Therefore, there is a growing interest in identifying efficient training methods that can concurrently enhance cardiovascular and neuromuscular performance through a singular training modality. This study conducted a systematic review and meta-analysis to explore the effects of aerobic training with blood flow restriction (AT + BFR) on muscle hypertrophy and strength gains in healthy individuals. Our study was registered at PROSPERO and used multiple databases (PubMed, Embase, Scopus, and Web of Science), seeking clinical trials that examined AT + BFR influence on muscle hypertrophy and strength gains in individuals aged 18-60 years and comparing with aerobic training without BFR. The risk of bias and method quality were assessed using the ROB2.0 tool and PEDro scale, respectively, and the quality of evidence was evaluated with the GRADE method. A random-effects model was used for meta-analysis, and standardized mean difference (SMD) was calculated for each outcome. Of 4,462 records, 29 full texts were assessed for eligibility, with 7 articles meeting the inclusion criteria. The results indicated that AT + BFR was more beneficial for inducing muscle hypertrophy than aerobic training without BFR (SMD [95% CI] = 0.86 [0.37-1.35]; I2 = 42%). Furthermore, AT + BFR was associated with greater improvements in muscle strength (SMD [95% CI] = 0.41 [0.10-0.72]; I2 = 0%). Despite the generally high risk of bias for both outcomes, these encouraging findings underscore the clinical significance of AT + BFR as a compelling tool for enhancing neuromuscular parameters.
Impact of Complete Intermittent Blood Flow Restriction in Upper Limbs Strength and Neural Function

Abstract:Purpose: We aimed to investigate the chronic effects of low-load strength training (LT) with complete intermittent blood flow restriction (IBFR) on neural adaptations and strength in biceps brachii. Methods: Nineteen volunteers were randomly assigned into two different 9-week training protocols consisting of three assessment weeks and six training weeks: (a) LT with complete IBFR (LT-IBFR; n = 10) and (b) LT without complete IBFR (LT; n = 9). Strength was evaluated by predicted 1 repetition maximum (1RM) at weeks 1, 5, and 9 and neural function by root mean square (RMS) and median frequency (MDF) at sessions 1, 7, and 12 during the first three and last three repetitions. Both groups performed three sets of Scott curl with 20% of predicted 1RM interspersed with 90s rest twice a week. Results: No changes were found in predicted 1RM throughout the training protocols nor between groups. LT-IBFR group showed lower RMS in the first set than LT for the first three repetitions and higher RMS in all sets for the last three repetitions with decreases in this value across the sets with no longitudinal changes for both groups. MDF in the first three repetitions did not differentiate between groups, however, in the last three repetitions, MDF were lower for LTIBFR group in all sets and it increased across the sets for this condition with no chronical changes for both groups in both repetitions zones. These results suggest that LT-IBFR may be ineffective for increasing Q5 strength and it did not promote chronic neural adaptations.

Acute neuromuscular, cardiovascular, and muscle oxygenation responses to low-intensity aerobic interval exercises with blood flow restriction

Abstract:We investigated the influence of short- and long-interval cycling exercise with blood flow restriction (BFR) on neuromuscular fatigue, shear stress and muscle oxygenation, potent stimuli to BFR-training adaptations. During separate sessions, eight individuals performed short- (24 × 60 s/30 s; SI) or long-interval (12 × 120 s/60 s; LI) trials on a cycle ergometer, matched for total work. One leg exercised with (BFR-leg) and the other without (CTRL-leg) BFR. Quadriceps fatigue was quantified using pre- to post-interval changes in maximal voluntary contraction (MVC), potentiated twitch force (QT) and voluntary activation (VA). Shear rate was measured by Doppler ultrasound at cuff release post-intervals. Vastus lateralis tissue oxygenation was measured by near-infrared spectroscopy during exercise. Following the initial interval, significant (P < 0.05) declines in MVC and QT were found in both SI and LI, which were more pronounced in the BFR-leg, and accounted for approximately two-thirds of the total reduction at exercise termination. In the BFR-leg, reductions in MVC (-28 ± 15%), QT (-42 ± 17%), and VA (-15 ± 17%) were maximal at exercise termination and persisted up to 8 min post-exercise. Exercise-induced muscle deoxygenation was greater (P < 0.001) in the BFR-leg than CTRL-leg and perceived pain was more in LI than SI (P < 0.014). Cuff release triggered a significant (P < 0.001) shear rate increase which was consistent across trials. Exercise-induced neuromuscular fatigue in the BFR-leg exceeded that in the CTRL-leg and was predominantly of peripheral origin. BFR also resulted in diminished muscle oxygenation and elevated shear stress. Finally, short-interval trials resulted in comparable neuromuscular and haemodynamic responses with reduced perceived pain compared to long-intervals.
Muscle Adaptations to High-Load Training and Very Low-Load Training With and Without Blood Flow Restriction

Abstract:An inability to lift loads great enough to disrupt muscular blood flow may impair the ability to fatigue muscles, compromising the hypertrophic response. It is unknown what level of blood flow restriction (BFR) pressure, if any, is necessary to reach failure at very low-loads [i.e., 15% one-repetition maximum (1RM)]. The purpose of this study was to investigate muscular adaptations following resistance training with a very low-load alone (15/0), with moderate BFR (15/40), or with high BFR (15/80), and compare them to traditional high-load (70/0) resistance training. Using a within/between subject design, healthy young participants (n = 40) performed four sets of unilateral knee extension to failure (up to 90 repetitions/set), twice per week for 8 weeks. Data presented as mean change (95% CI). There was a condition by time interaction for 1RM (p < 0.001), which increased for 70/0 [3.15 (2.04,4.25) kg] only. A condition by time interaction (p = 0.028) revealed greater changes in endurance for 15/80 [6 (4,8) repetitions] compared to 15/0 [4 (2,6) repetitions] and 70/0 [4 (2,5) repetitions]. There was a main effect of time for isometric MVC [change = 10.51 (3.87,17.16) Nm, p = 0.002] and isokinetic MVC at 180°/s [change = 8.61 (5.54,11.68) Nm, p < 0.001], however there was no change in isokinetic MVC at 60°/s [2.45 (-1.84,6.74) Nm, p = 0.261]. Anterior and lateral muscle thickness was assessed at 30, 40, 50, and 60% of the upper leg. There was no condition by time interaction for muscle thickness sites (all p ≥ 0.313). There was a main effect of time for all sites, with increases over time (all p < 0.001). With the exception of the 30% lateral site (p = 0.059) there was also a main effect of condition (all p < 0.001). Generally, 70/0 was greater. Average weekly volume increased for all conditions across the 8 weeks, and was greatest for 70/0 followed by 15/0, 15/40, then 15/80. With the exception of 1RM, changes in strength and muscle size were similar regardless of load or restriction. The workload required to elicit these changes lowered with increased BFR pressure. These findings may be pertinent to rehabilitative settings, future research, and program design.

Abstract:The objective of this meta-analysis is to compare the effects of low and high-intensity aerobic training with BFR (LI-BFR and HI-BFR) with low and high-intensity aerobic training without BFR (LI and HI) on muscle strength and hypertrophy. The search was performed in five databases, by two independent researchers, and the terms and keywords used to optimize the searches were related to blood flow restriction and aerobic training. All studies were evaluated for methodological quality using the PEDro scale and for quality of evidence using the GRADE system. Meta-analyses were conducted using RevMan software. After data extraction, 11 studies met all eligibility criteria and were included in the systematic review. The results of the overall analysis between LI-BFR vs. LI showed a significant difference in muscle strength of knee extensors, for hypertrophy, LI was superior to LI-BFR with clinical relevance. Comparing HI-BFR vs. HI there was no superiority for muscle strength. In conclusion, for strength gains very low-quality evidence was found to support no superiority between LI-BFR and HI-BFR compared to LI and HI, respectively. For muscle hypertrophy, superiority of LI was found compared to LI-BFR, with a very low level of evidence.

The effect of eccentric exercise with blood flow restriction on neuromuscular activation, microvascular oxygenation, and the repeated bout effect

Abstract:Purpose: To examine the effect of low-intensity eccentric contractions with and without blood flow restriction (BFR) on microvascular oxygenation, neuromuscular activation, and the repeated bout effect (RBE). Methods: Participants were randomly assigned to either low-intensity (LI), low-intensity with BFR (LI-BFR), or a control (CON) group. Participants in LI and LI-BFR performed a preconditioning bout of low-intensity eccentric exercise prior to about of maximal eccentric exercise. Participants reported 24, 48, 72, and 96 h later to assess muscle damage and function. Surface electromyography (sEMG) and near-infrared spectroscopy (NIRS) were used to measure neuromuscular activation and microvascular deoxygenation (deoxy-[Hb + Mb]) and [total hemoglobin] ([THC]) during the preconditioning bout, respectively. Results: During set-2, LI-BFR resulted in greater activation of the VM-RMS (47.7 ± 11.5% MVIC) compared to LI (67.0 ± 20.0% MVIC), as well as during set-3 (p < 0.05). LI-BFR resulted in a greater change in deoxy-[Hb + Mb] compared to LI during set-2 (LI-BFR 13.1 ± 5.2 µM, LI 6.7 ± 7.9 µM), set-3 (LI-BFR 14.6 ± 6 µM, LI 6.9 ± 7.4 µM), and set-4 (p < 0.05). [THC] was higher during LI-BFR compared to LI (p < 0.05). All groups showed a decrease in MVIC torque immediately after maximal exercise (LI 74.2 ± 14.1%, LI-BFR 75 ± 5.1%, CON 53 ± 18.6%). At 24, 48, 72, and 96 h post maximal eccentric exercise, LI and LI-BFR force deficit was not different from baseline. Conclusion: This study suggests that the neuromuscular and deoxygenation (i.e., metabolic stress) responses were considerably different between LI and LI-BFR groups; however, these differences did not lead to improvements in the RBE inferred by performing LI and LI-BFR.
Acute impact of autoregulation of applied blood flow restriction pressures on bilateral single-joint upper limb resistance exercise

Abstract:To investigate the acute effects of 4 sets of autoregulated (AR-BFR) versus non-autoregulated (NAR-BFR) applied pressures during blood flow restriction (BFR) resistance exercise to volitional failure compared with low-load exercise without BFR. A randomized crossover design study was conducted on 32 healthy adults (20.8 ± 2.3 years; 11 females). Outcome measures were as follows: (1) arterial stiffness, (2) peak perceptual responses and likelihood to perform again, and (3) performance. Results: Post-exercise changes in central and brachial diastolic blood pressure were decreased in all groups. Post-exercise supine systolic blood pressure in no-BFR increased (mean difference (MD) = 4 ± 1 mmHg, 95% CI (1–7), p = 0.003, η 2 = 0.13). Total repetitions performed and volume workload were similar between BFR conditions but less than no-BFR. AR-BFR reported significantly higher exertion (MD = 0.53 ± 0.2, 95% CI (0.04–1.0), p = 0.03, η 2 = 0.19) than other conditions, and induced greater discomfort (MD = 2.50 ± 0.36, 95% CI (1.63–3.37), p < 0.001, η 2 = 0.28) than no-BFR. Conclusion: Biceps curl exercise to volitional failure appears to induce negligible arterial stiffness or blood pressure changes regardless of the application of autoregulation, yet autoregulation appears to enhance the perceptual response to BFR exercise compared to NAR-BFR without impacting exercise performance.

Abstract:Abstract Heart rate variability (HRV), blood pressure variability (BPV), and baroreflex sensitivity (BRS) provide important information on cardiovascular autonomic control. However, little is known about the reorganization of HRV, BPV, and BRS after aerobic exercise. While there is a positive relationship between heart rate (HR) recovery rate and cardiorespiratory fitness, it is unclear whether there is a relationship between cardiorespiratory fitness and reorganization of cardiovascular autonomic modulation during recovery. Thus, this study aimed to investigate whether cardiorespiratory fitness influences the cardiovascular autonomic modulation recovery, after a cardiopulmonary exercise test. Sixty men were assigned into groups according to their cardiorespiratory fitness: low cardiorespiratory fitness (LCF = VO2: 22–38 mL kg −1 min −1 ), moderate (MCF = VO2: 38–48 mL kg −1 min −1 ), and high (HCF = VO2 > 48 mL kg −1 min −1 ). HRV (linear and non-linear analysis) and BPV (spectral analysis), and BRS (sequence method) were performed before and after a cardiopulmonary exercise test. The groups with higher cardiorespiratory fitness had lower baseline HR values and HR recovery time after the cardiopulmonary exercise test. On comparing rest and recovery periods, the spectral analysis of HRV showed a decrease in low-frequency (LF) oscillations in absolute units and high frequency (HF) in absolute and normalized units. It also showed increases in LF oscillations of blood pressure. Nonlinear analysis showed a reduction in approximate entropy (ApEn) and in Poincare Plot parameters (SD1 and SD2), accompanied by increases in detrended fluctuation analysis (DFA) parameters α1 and α2. However, we did not find differences in cardiovascular autonomic modulation parameters and BRS in relation to cardiorespiratory fitness neither before nor after the cardiopulmonary test. We concluded that cardiorespiratory fitness does not affect cardiovascular autonomic modulations after cardiopulmonary exercise test, unlike HR recovery.

Abstract:Abstract Background Resistance training with blood flow restriction (BFR) results in hypertrophy, and its magnitude depends on various training variables. This study aimed to compare the long-term effect of passive recovery (PR) and active recovery (AR) during low-intensity resistance training with BFR on hormonal levels and performance in young men. Methods In the randomized clinical trial, 20 men were randomly divided into PR and AR groups during resistance training with BFR. The intervention consisted of six upper and lower body movements with 30% of one maximum repetition (1RM), three sessions per week for six weeks. Both groups wore pneumatic cuffs on the proximal part of thighs and arms. The cuff pressure was 60% of the calculated arterial blood occlusion and increased 10% every two weeks. The AR group performed seven repetitions in 30 s break between sets by one second for concentric and eccentric phases and two seconds rest, and the other group had passive rest. The blood samples and a series of performance tests were gathered before and after the intervention. A repeated measure ANOVA was used to analyze data. Results AR and PR interventions significantly improved the C-reactive protein (CRP) (− 38% vs. − 40%), Lactate dehydrogenase (LDH) (− 11% vs. − 3%), Sargent jump (9% vs. 10%), peak power (20% vs.18%), and average power (14% vs. 14%), upper 1RM (8% vs. 8%) and no significant differences were observed between groups. The AR intervention significantly increased growth hormone (GH) (423% vs. 151%, p = 0.03), lower body 1RM (18% vs. 11%) and muscle endurance (34% vs. 22% for the upper body, p = 0.02 and 32% vs. 24% for the lower body, p = 0.04) than the PR group. The PR intervention further increased the minimum power than the AR group (19% vs. 10%). There were no significant changes in testosterone ( p = 0.79) and cortisol ( p = 0.34) following interventions. Conclusion The findings indicated that by increasing muscle activation and higher metabolic load, AR during resistance training with BFR might cause more remarkable improvements in serum GH, muscle strength, and endurance. Thus, to gain further benefits, AR during training with BFR is recommended. Trial registration : IRCT20191207045644N1. Registration date: 14/03/2020. Abstract:BackgroundLow-load resistance training (< 50% of one-repetition maximum [1RM]) associated with blood-flow restriction (BFR-RT) has been thought to promote increases in muscle strength and mass. However, it remains unclear if the magnitude of these adaptations is similar to conventional high-load resistance training (> 65% 1RM; HL-RT).ObjectiveTo compare the effects of HL- versus BFR-RT on muscle adaptations using a systematic review and meta-analysis procedure.MethodsStudies were identified via electronic databases based on the following inclusion criteria: (a) pre- and post-training assessment of muscular strength; (b) pre- and post-training assessment of muscle hypertrophy; (c) comparison of HL-RT vs. BFR-RT; (d) score ≥ 4 on PEDro scale; (e) means and standard deviations (or standard errors) are reported from absolute values or allow estimation from graphs. If this last criterion was not met, data were directly requested from the authors.ResultsThe main results showed higher increases in muscle strength for HL- as compared with BFR-RT, even when considering test specificity, absolute occlusion pressure, cuff width, and occlusion pressure prescription. Regarding the hypertrophic response, results revealed similar effects between HL- and BFR-RT, regardless of the absolute occlusion pressure, cuff width, and occlusion pressure prescription.ConclusionsBased on the present data, maximum muscle strength may be optimized by specific training methods (i.e., HL-RT) while both HL- and BFR-RT seem equally effective in increasing muscle mass. Importantly, BFR-RT is a valid and effective approach for increasing muscle strength in a wide spectrum of ages and physical capacity, although it may seem particularly of interest for those individuals with physical limitations to engage in HL-RT.
