Abstract:Auditory stimuli are proposed as beneficial neurorehabilitation methods in patients with disorders of consciousness. However, precise and accurate quantitative indices to estimate their potential effect remain scarce. Fourteen patients were recruited from the Neuro-Rehabilitation Unit of Hangzhou Hospital of Zhejiang Armed Police Corps of China. Altogether, there were seven cases of unresponsive wakefulness syndrome (five males and two females, aged 45.7 ± 16.8 years) and seven cases of minimally conscious state (six males and one female, aged 42.3 ± 20.8 years). Simultaneously, fourteen healthy controls (10 males and 4 females, aged 51.7 ± 9.7 years) also participated in this case-control experiment. Brain response to music, subjects' own name, and noise was monitored by quantitative electroencephalography (QEEG) in the resting state and with acoustic stimulation. Predictive QEEG values in various brain regions were investigated. Our results show that cerebral activation was high in subjects stimulated by their own name, especially in the temporal lobe in patients with disorders of consciousness, and the frontal lobe in the control group. Further, during resting and stimulation, QEEG index (δ + θ/α + β ratio) negatively correlated with the Coma Recovery Scale-Revised score in traumatic disorders of consciousness patients. Hence, we speculate that a subject's own name might be an effective awakening therapy for patients with disorders of consciousness. Moreover, QEEG index in specific stimulation states may be used as a prognostic indicator for disorders of consciousness patients (sensitivity, 75%; specificity, 50%). Interhemispheric sleep EEG asymmetry in the rat is enhanced by sleep deprivation

Abstract:Vigilance state-related topographic variations of electroencephalographic (EEG) activity have been reported in humans and animals. To investigate their possible functional significance, the cortical EEG of the rat was recorded from frontal and parietal derivations in both hemispheres. Records were obtained for a 24-h baseline day, 6-h sleep deprivation (SD), and subsequent 18-h recovery. During the baseline 12-h light period, the main sleep period of the rat, low-frequency (<7.0 Hz) power in the non-rapid eye-movement (NREM) sleep EEG declined progressively. Left-hemispheric predominance of low-frequency power at the parietal derivations was observed at the beginning of the light period when sleep pressure is high due to preceding spontaneous waking. The left-hemispheric dominance changed to a right-hemispheric dominance in the course of the 12-h rest-phase when sleep pressure dissipated. During recovery from SD, both low-frequency power and parietal left-hemispheric predominance were enhanced. The increase in low-frequency power in NREM sleep observed after SD at the frontal site was larger than at the parietal site. However, frontally no interhemispheric differences were present. In REM sleep, power in the theta band (5.25-8.0 Hz) exhibited a right-hemispheric predominance. In contrast to NREM sleep, the hemispheric asymmetry showed no trend during baseline and was not affected by SD. Use-dependent local changes may underlie the regional differences in the low-frequency NREM sleep EEG within and between hemispheres. The different interhemispheric asymmetries in NREM and REM sleep suggest that the two sleep states may subserve different functions in the brain.

Abstract:The aim of this study is to detect changes in a functional state during the performance of monotonous psychomotor test. We propose that these changes represent the repeated episodes of short sleep and spontaneous awakenings from them. We also argue that spontaneous awakenings from sleep stage 2 are accompanied by high-amplitude slow oscillations (SO). 20 subjects participated in 27 experiments during daytime. They performed continuous psychomotor test in a supine position with eyes closed for 1 h. Expert scoring of SOs in the EEG, including K-complexes and other single slow waves, was performed in sleep episodes reaching sleep stage 2, defined as pauses in task performance longer than 3 s. 248 episodes with SOs occurred in time intervals without activity lasting from 3 s to 10 min. In 195 cases, at least one SO was recorded before the test performance was resumed. 248 sleep episodes with 1255 SOs were taken into analysis. It was shown that SOs occur much more frequently just before awakenings (SO 1 , 12 or less seconds before awakening) than within sleep episodes (12 of more seconds before awakening, SO 2 ). Some SOs were recorded within a few seconds after behavioral awakening (SO 3 ), which has not been previously reported. We propose the hypothesis that SOs which occurred just before resuming performance (SO 1 ) are associated with the unconscious episodic memory that triggers awakening followed by recovery of conscious activity performed prior to falling asleep. We also describe the novel type of SO (SO 3 ) which occurs just after awakening.

Abstract:Objective: Obstructive Sleep Apnea (OSA) is associated with deficits in vigilance. This work explored the temporal patterns of OSA-related events during sleep and vigilance levels measured by the Psychomotor Vigilance Test (PVT) in patients undergoing Polysomnography (PSG) for suspected OSA. Approach: The PVT was conducted prior to in-laboratory PSG for 80 patients suspected of having OSA. Three groups were formed based on PVT-RT-outcomes and participants were randomly allocated into Training (n=55) and Test (n=25) samples. Sleep epochs of non-rapid-eye movement (NREM) Electroencephalographic (EEG) asynchrony data, and REM and NREM data for respiratory, arousal, limb movement and desaturation events were analysed. The data were segmented by sleep stage, by Sleep Blocks (SB) of stable Stage N2, Stage N3, mixed-stage NREM sleep, and, by Time of Night (TN) across sleep. Models associating this data with PVT groups were developed and tested. Main Results: A model using NREM EEG asynchrony data segmented by SB and TN achieved 81.9% accuracy in the Test Cohort. Models based on interhemispheric asynchrony SB data and OSA data segmented by TN achieved 80.6% and 79.5% respectively. Significance: Novel data segmentation methods via blocks of NREM sleep and TN have improved our understanding of the relationship between sleep, OSA and vigilance.

Abstract:Sleep deprivation, a widespread phenomenon that affects one-third of normal American adults, induces adverse changes in physical and cognitive performance, which in turn increases the occurrence of accidents. Sleep deprivation is known to increase resting blood pressure and decrease muscle sympathetic nerve activity. Monitoring changes in the interplay between the central and autonomic sympathetic nervous system can be a potential indicator of human’s readiness to perform tasks that involve a certain level of cognitive load (e.g., driving). The electroencephalogram (EEG) is the standard to assess the brain’s activity. The electrodermal activity (EDA) is a reflection of the general state of arousal regulated by the activation of the sympathetic nervous system through sweat gland stimulation. In this work, we calculated the mutual information between EDA and EEG recordings in order to consider linear and non-linear interactions and provide an insight of the relationship between brain activity and peripheral autonomic sympathetic activity. We analyzed EEG and EDA data from ten participants performing four cognitive tasks every two hours during 24 h (12 trials). We decomposed EEG data into delta, theta, alpha, beta, and gamma spectral components, and EDA into tonic and phasic components. The results demonstrate high values of mutual information between the EDA and delta component of EEG, mainly in working memory tasks. Additionally, we found an increase in the theta component of EEG in the presence of fatigue caused by sleep deprivation, the alpha component in tasks demanding inhibition and attention, and the delta component in working memory tasks. In terms of the location of brain activity, most of the tasks report high mutual information in frontal regions in the initial trials, with a trend to decrease and become uniform for all the nine analyzed EEG channels as a consequence of the sleep deprivation effect. Our results evidence the interplay between central and sympathetic nervous activity and can be used to mitigate the consequences of sleep deprivation.
Intracerebral dynamics of sleep arousals: a combined scalp-intracranial EEG study

Abstract:As an intrinsic component of sleep architecture, sleep arousals represent an intermediate state between sleep and wakefulness and play an important role in sleep-wake regulation. They have been defined in an all-or-none manner, whereas they actually present a wide range of scalp-electroencephalography (EEG) activity patterns. It is poorly understood how these arousals differ in their mechanisms. Stereo-EEG (SEEG) provides the unique opportunity to record intracranial activities in superficial and deep structures in humans. Using combined polysomnography and SEEG, we quantitatively categorized arousals in non-rapid eye movement sleep into slow wave (SW) and Non-SW arousals based on whether they co-occurred with a scalp-EEG SW event. We then investigated their intracranial correlates in up to 26 brain regions from 26 patients (12 females). Across both arousal types, intracranial theta, alpha, sigma, and beta activities increased in up to 25 regions ( p <0.05, d =0.06-0.63), while gamma and high frequency (HF) activities decreased in up to 18 regions across five brain lobes ( p <0.05, d =0.06-0.44). Intracranial delta power widely increased across five lobes during SW arousals ( p <0.05 in 22 regions, d =0.10-0.39), while it widely decreased during Non-SW arousals ( p <0.05 in 19 regions, d =0.10-0.30). Despite these main patterns, unique activity was observed locally in some regions such as the hippocampus and middle cingulate cortex, indicating spatial heterogeneity of arousal responses. Our results suggest that Non-SW arousals correspond to a higher level of brain activation than SW arousals. The decrease in HF activities could potentially explain the absence of awareness and recollection during arousals. Significance Statement Intrinsic to sleep architecture, sleep arousals play an important role in sleep-wake regulation. They are defined in an all-or-none manner, whereas they actually present various scalp electroencephalography (EEG) patterns. Using simultaneous scalp and intracranial EEG in humans, we analyzed the intracranial activity during two types of arousals marked on scalp EEG, quantitatively categorized by whether they co-occurred with a scalp-EEG slow wave (SW). Non-SW arousals present prevalent low-voltage fast activity, while SW arousals exhibit high-voltage slow waves alongside fast activities. This work represents the first intracranial study of different types of NREM sleep arousals and provides a comprehensive description of local brain activities during both arousal types, serving as a foundation for future studies investigating regional behaviors during sleep-wake transition.

Abstract:Rapid eye movement (REM) sleep is characterized by the alternation of two markedly different microstates, phasic and tonic REM. These periods differ in awakening and arousal thresholds, sensory processing, and spontaneous cortical oscillations. Previous studies indicate that although in phasic REM, cortical activity is independent of the external environment, attentional functions and sensory processing are partially maintained during tonic periods. Large-scale synchronization of oscillatory activity, especially in the α- and β-frequency ranges, can accurately distinguish different states of vigilance and cognitive processes of enhanced alertness and attention. Therefore, we examined long-range inter- and intrahemispheric as well as short-range electroencephalographic synchronization during phasic and tonic REM periods quantified by the weighted phase lag index. Based on the nocturnal polysomnographic data of 19 healthy adult participants, we showed that long-range inter- and intrahemispheric α and β synchrony was enhanced in tonic REM states in contrast to phasic ones, and resembled α and β synchronization of resting wakefulness. On the other hand, short-range synchronization within the γ-frequency range was higher in phasic compared with tonic periods. Increased short-range synchrony might reflect local and inwardly driven sensorimotor activity during phasic REM periods, whereas enhanced long-range synchrony might index frontoparietal activity that reinstates environmental alertness after phasic REM periods.

Abstract:This study investigates intra-regional connectivity and regional hemispheric asymmetry under two vigilance states: alertness and vigilance decrement. The vigilance states were induced on nine healthy subjects while performing 30 min in-congruent Stroop color-word task (I-SCWT). We measured brain activity using Electroencephalography (EEG) signals with 64-channels. We quantified the regional network connectivity using the phase-locking value (PLV) with graph theory analysis (GTA) and Support Vector Machines (SVM). Results showed that the vigilance decrement state was associated with impaired information processing within the frontal and central regions in delta and theta frequency bands. Meanwhile, the hemispheric asymmetry results showed that the laterality shifted to the right-temporal in delta, right-central, parietal, and left frontal in theta, right-frontal and left-central, temporal and parietal in alpha, and right-parietal and left temporal in beta frequency bands. These findings represent the first demonstration of intra-regional connectivity and hemispheric asymmetry changes as a function of cognitive vigilance states. The overall results showed that vigilance decrement is region and frequency band-specific. Our SVM model achieved the highest classification accuracy of 99.73% in differentiating between the two vigilance states based on the frontal and central connectivity networks measures.

Novel Electrophysiological Signatures of Learning and Forgetting in Human Rapid Eye Movement Sleep

Abstract:Despite the known behavioral benefits of rapid eye movement (REM) sleep, discrete neural oscillatory events in human scalp electroencephalography (EEG) linked with behavior have not been discovered. This knowledge gap hinders mechanistic understanding of the function of sleep, as well as the development of biophysical models and REM-based causal interventions. We designed a detection algorithm to identify bursts of activity in high-density, scalp EEG within theta (4-8 Hz) and alpha (8-13 Hz) bands during REM sleep. Across 38 nights of sleep, we characterized the burst events (i.e., count, duration, density, peak frequency, amplitude) in healthy, young male and female human participants (38; 21F) and investigated burst activity in relation to sleep-dependent memory tasks: hippocampal-dependent episodic verbal memory and nonhippocampal visual perceptual learning. We found greater burst count during the more REM-intensive second half of the night (p < 0.05), longer burst duration during the first half of the night (p < 0.05), but no differences across the night in density or power (p > 0.05). Moreover, increased alpha burst power was associated with increased overnight forgetting for episodic memory (p < 0.05). Furthermore, we show that increased REM theta burst activity in retinotopically specific regions was associated with better visual perceptual performance. Our work provides a critical bridge between discrete REM sleep events in human scalp EEG that support cognitive processes and the identification of similar activity patterns in animal models that allow for further mechanistic characterization.
Spatiotemporal patterns of theta-band activity during rapid-eye movement sleep: a magnetoencephalography analysis

Abstract:Theta oscillations (4-8 Hz) in frontal cortical regions are present to different degrees across states of consciousness. In sleep, theta is prominent in periods of rapid eye-movement (REM) sleep. Theta has been linked to processes of memory consolidation; however, its mechanistic contribution specifically during REM sleep is not well understood. Interestingly, in the wake state, frontal theta activity increases during effortful cognitive tasks involving executive functions such as working memory, hinting at similarities in circuitry, and potentially, function. The aim of the present work is to create a spatially resolved, whole-brain characterisation of REM oscillatory activity in healthy human subjects, distinguishing theta from neighbouring frequency bands, differentiating substages of REM sleep (phasic and tonic REM), and comparing REM theta to that which is evoked during a working memory task. To that end, we analysed magneto- and electroencephalography (M/EEG) data recorded during overnight sleep in 10 healthy subjects, and similar data from 17 healthy subjects who performed a working memory task, using a novel whole-brain, source-localised MEG approach. Our results show that (i) theta activity has a frontal midline topography that is distinct from those of other prominent frequency bands in REM (delta, alpha, beta), (ii) theta activity in frontal midline regions is best observed within a focused 5-7 Hz range, separating it from occipital alpha activity, (iii) REM theta is dominant over the frontal midline but is also observed in several sub-cortical areas, (iv) theta is more widespread in tonic than phasic REM sleep, and (v) the focused frontal midline theta pattern observed in REM phasic sleep is the most similar of all observed sleep substages to theta evoked by a working memory task. These results enhance our understanding of theta physiology in REM sleep and suggest future targets for research into REM's role in learning and memory.

Structure of Spectral Composition and Synchronization in Human Sleep on the Whole Scalp: A Pilot Study

Abstract:We used numerical methods to define the normative structure of the different stages of sleep and wake (W) in a pilot study of 19 participants without pathology (18–64 years old) using a double-banana bipolar montage. Artefact-free 120–240 s epoch lengths were visually identified and divided into 1 s windows with a 10% overlap. Differential channels were grouped into frontal, parieto-occipital, and temporal lobes. For every channel, the power spectrum (PS) was calculated via fast Fourier transform and used to compute the areas for the delta (0–4 Hz), theta (4–8 Hz), alpha (8–13 Hz), and beta (13–30 Hz) bands, which were log-transformed. Furthermore, Pearson's correlation coefficient and coherence by bands were computed. Differences in logPS and synchronization from the whole scalp were observed between the sexes for specific stages. However, these differences vanished when specific lobes were considered. Considering the location and stages, the logPS and synchronization vary highly and specifically in a complex manner. Furthermore, the average spectra for every channel and stage were very well defined, with phase-specific features (e.g., the sigma band during N2 and N3, or the occipital alpha component during wakefulness), although the slow alpha component (8.0–8.5 Hz) persisted during NREM and REM sleep. The average spectra were symmetric between hemispheres. The properties of K-complexes and the sigma band (mainly due to sleep spindles—SSs) were deeply analyzed during the NREM N2 stage. The properties of the sigma band are directly related to the density of SSs. The average frequency of SSs in the frontal lobe was lower than that in the occipital lobe. In approximately 30% of the participants, SSs showed bimodal components in the anterior regions. qEEG can be easily and reliably used to study sleep in healthy participants and patients.

Abstract:Brain dynamics is highly non-stationary, permanently subject to ever-changing external conditions and continuously monitoring and adjusting internal control mechanisms. Finding stationary structures in this system, as has been done recently, is therefore of great importance for understanding fundamental dynamic trade relationships. Here we analyse electroencephalographic recordings (EEG) of 13 subjects with unresponsive wakefulness syndrome (UWS) during rest and while being influenced by different acoustic stimuli. We compare the results with a control group under the same experimental conditions and with clinically healthy subjects during overnight sleep. The main objective of this study is to investigate whether a stationary correlation pattern is also present in the UWS group, and if so, to what extent this structure resembles the one found in healthy subjects. Furthermore, we extract transient dynamical features via specific deviations from the stationary interrelation pattern. We find that (i) the UWS group is more heterogeneous than the two groups of healthy subjects, (ii) also the EEGs of the UWS group contain a stationary cross-correlation pattern, although it is less pronounced and shows less similarity to that found for healthy subjects and (iii) deviations from the stationary pattern are notably larger for the UWS than for the two groups of healthy subjects. The results suggest that the nervous system of subjects with UWS receive external stimuli but show an overreaching reaction to them, which may disturb opportune information processing.

Reflection of the pattern of cortical activation in the phase structure of the human EEG

Abstract:A total of 22 healthy subjects in the EEG laboratory and 62 patients in the clinical functional diagnostic unit were studied. Spontaneous EEG recordings were made using the 10–20 scheme relative to combined ear electrodes in the state of rest with the eyes closed and open and during various functional loads. Traces were analyzed by computer animation of the EEG phase structure. The main concept of the method for extracting phase structure was based on not using a single reference lead. Time shifts were measured only between neighboring electrodes, with the result that the oscillations being compared were highly coherent. Time discordance was assessed in terms of the shift in the peak of the cross-correlation function. The results showed that from the point of view of phase structure, the differences between the high-and low-frequency EEG rhythms were purely quantitative. Qualitatively, the properties of the rhythms were identical and were reduced to slow (in the seconds range) oscillations of phase shifts. Low-frequency activity was characterized by large (in absolute terms, msec) phase shifts from electrode to electrode as compared with high-frequency activity. The phase shifts of potentials formed a structure which was overall very similar in different subjects and was reproduced in different leads. The initial appearance of EEG waves was statistically linked with the main sensory projections-the visual (occipital areas), auditory (temporal areas), and somatic (parietal areas), with addition of the frontal areas. Rearrangement of phase leadership in favor of the occipital pole at the expense of both temporal areas was observed on opening the eyes. This appears to depend on the level of sensory influx to this cortical area from the thalamus. It is suggested that the direction of the phase gradient reflects a gradient of cortical current density parallel to the surface. This can be used to locate compact sources lying close to the surface.

Abstract:Sleep inertia (SI) is a time period during the transition from sleep to wakefulness wherein individuals perceive low vigilance with cognitive impairments; SI is generally identified by longer reaction times (RTs) in attention tasks immediately after awakening followed by a gradual RT reduction along with waking time. The sluggish recovery of vigilance in SI involves a dynamic process of brain functions, as evidenced in recent functional magnetic resonance imaging (fMRI) studies in within-network and between-network connectivity. However, these fMRI findings were generally based on the presumption of unchanged neurovascular coupling (NVC) before and after sleep, which remains an uncertain factor to be investigated. Therefore, we recruited 12 young participants to perform a psychomotor vigilance task (PVT) and a breath-hold task of cerebrovascular reactivity (CVR) before sleep and thrice after awakening (A1, A2, and A3, with 20 min intervals in between) using simultaneous electroencephalography (EEG)-fMRI recordings. If the NVC were to hold in SI, we hypothesized that time-varying consistencies could be found between the fMRI response and EEG beta power, but not in neuron-irrelevant CVR. Results showed that the reduced accuracy and increased RT in the PVT upon awakening was consistent with the temporal patterns of the PVT-induced fMRI responses (thalamus, insula, and primary motor cortex) and the EEG beta power (Pz and CP1). The neuron-irrelevant CVR did not show the same time-varying pattern among the brain regions associated with PVT. Our findings imply that the temporal dynamics of fMRI indices upon awakening are dominated by neural activities. This is the first study to explore the temporal consistencies of neurovascular components on awakening, and the discovery provides a neurophysiological basis for further neuroimaging studies regarding SI.

Intracortical Causal Information Flow of Oscillatory Activity (Effective Connectivity) at the Sleep Onset Transition

Abstract:We investigated the sleep onset transition in humans from an effective connectivity perspective in a baseline condition (approx. 16 h of wakefulness) and after sleep deprivation (40 h of sustained wakefulness). Using EEG recordings (27 derivations), source localization (LORETA) allowed us to reconstruct the underlying patterns of neuronal activity in various brain regions, e.g., the default mode network (DMN), dorsolateral prefrontal cortex and hippocampus, which were defined as regions of interest (ROI). We applied isolated effective coherence (iCOH) to assess effective connectivity patterns at the sleep onset transition [2 min prior to and 10 min after sleep onset (first occurrence of stage 2)]. ICOH reveals directionality aspects and resolves the spectral characteristics of information flow in a given network of ROIs. We observed an anterior-posterior decoupling of the DMN, and moreover, a prominent role of the posterior cingulate cortex guiding the process of the sleep onset transition, particularly, by transmitting information in the low frequency range (delta and theta bands) to other nodes of DMN (including the hippocampus). In addition, the midcingulate cortex appeared as a major cortical relay station for spindle synchronization (originating from the thalamus; sigma activity). The inclusion of hippocampus indicated that this region might be functionally involved in sigma synchronization observed in the cortex after sleep onset. Furthermore, under conditions of increased homeostatic pressure, we hypothesize that an anterior-posterior decoupling of the DMN occurred at a faster rate compared to baseline overall indicating weakened connectivity strength within the DMN. Finally, we also demonstrated that cortico-cortical spindle synchronization was less effective after sleep deprivation than in baseline, thus, reflecting the reduction of spindles under increased sleep pressure.
The Theta Paradox: 4-8 Hz EEG Oscillations Reflect Both Sleep Pressure and Cognitive Control

Abstract:Human electroencephalographic (EEG) oscillations characterize specific behavioral and vigilance states. The frequency of these oscillations is typically sufficient to distinguish a given state; however, theta oscillations (4-8 Hz) have instead been found in near-opposite conditions of drowsiness during sleep deprivation and alert cognitive control. While the latter has been extensively studied and is often referred to as "frontal midline theta," (fmTheta) the former has been investigated far less but is considered a marker for sleep pressure during wake. In this study we investigated to what extent theta oscillations differed during cognitive tasks and sleep deprivation. We measured high-density EEG in 18 young healthy adults (nine female) performing six tasks under three levels of sleep deprivation. We found both cognitive load and sleep deprivation increased theta power in medial prefrontal cortical areas; however, sleep deprivation caused additional increases in theta in many other, predominantly frontal, areas. The sources of sleep deprivation theta (sdTheta) were task dependent, with a visual-spatial task and short-term memory (STM) task showing the most widespread effects. Notably, theta was highest in supplementary motor areas during passive music listening, and highest in the inferior temporal cortex (responsible for object recognition) during a spatial game. Furthermore, while changes in task performance were correlated with increases in theta during sleep deprivation, this relationship was not specific to the EEG of the same task and did not survive correction for multiple comparisons. Altogether, these results suggest that both during sleep deprivation and cognition theta oscillations may preferentially occur in cortical areas not involved in ongoing behavior.SIGNIFICANCE STATEMENT Electroencephalographic (EEG) research in sleep has often remained separate from research in cognition. This has led to two incompatible interpretations of the function of theta brain oscillations (4-8 Hz): that they reflect local sleep events during sleep deprivation, or that they reflect cognitive processing during tasks. With this study, we found no fundamental differences between theta oscillations during cognition and theta during sleep deprivation that would suggest different functions. Instead, our results indicate that in both cases, theta oscillations are generated by cortical areas not required for ongoing behavior. Therefore, at least in humans, theta may reflect either cortical disengagement or inhibition.
High-density EEG power topography and connectivity during confusional arousal

Abstract:Confusional arousal is the milder expression of a family of disorders known as Disorders of Arousal (DOA) from non-REM sleep. These disorders are characterized by recurrent abnormal behaviors that occur in a state of reduced awareness for the external environment. Despite frequent amnesia for the nocturnal events, when actively probed, patients are able to report vivid hallucinatory/dream-like mental imagery. Traditional (low-density) scalp and stereo-electroencephalographic (EEG) recordings previously showed a pathological admixture of slow oscillations typical of NREM sleep and wake-like fast-mixed frequencies during these phenomena. However, our knowledge about the specific neural EEG dynamics over the entire brain is limited. We collected 2 consecutive in-laboratory sleep recordings using high-density (hd)-EEG (256 vertex-referenced geodesic system) coupled with standard video-polysomnography (v-PSG) from a 12-year-old drug-naïve and otherwise healthy child with a long-lasting history of sleepwalking. Source power topography and functional connectivity were computed during 20 selected confusional arousal episodes (from -6 to +18 sec after motor onset), and during baseline slow wave sleep preceding each episode (from - 3 to -2 min before onset). We found a widespread increase in slow wave activity (SWA) theta, alpha, beta, gamma power, associated with a parallel decrease in the sigma range during behavioral episodes compared to baseline sleep. Bilateral Broadman area 7 and right Broadman areas 39 and 40 were relatively spared by the massive increase in SWA power. Functional SWA connectivity analysis revealed a drastic increase in the number and complexity of connections from baseline sleep to full-blown episodes, that mainly involved an increased out-flow from bilateral fronto-medial prefrontal cortex and left temporal lobe to other cortical regions. These effects could be appreciated in the 6 sec window preceding behavioral onset. Overall, our results support the idea that DOA are the expression of peculiar brain states, compatible with a partial re-emergence of consciousness.

Abstract:Abstract Introduction Alpha-delta sleep, also known as alpha wave intrusion during sleep, is an electroencephalography (EEG) pattern often observed in clinical settings, and is associated with unrefreshing sleep and chronic fatigue or pain. The neural basis of alpha-delta sleep is poorly understood, and the extent of the cerebral cortex’s involvement is unknown. Here we use overnight intracranial and scalp EEG in adult humans to explore the cortical neural mechanisms of alpha-delta sleep. Methods We studied neurosurgical epilepsy patients who underwent longitudinal intracranial EEG (iEEG) monitoring. In each patient, data were simultaneously recorded overnight from iEEG electrodes and electrodes placed on scalp for polysomnography. To focus on local neural activity, we derived each iEEG channel as the signal difference between two adjacent recording sites separated by 5 mm within the cerebral cortex. For each 30-second epoch, we calculated EEG power in delta (1-4 Hz), theta (4-8 Hz), and alpha (8-15 Hz) frequency bands for all iEEG channels, and for sleep-scoring scalp EEG channels (e.g. C3-A2). Other bands were studied to rule out the effects of sleep spindles. For each night (comprising around 1000 epochs), we calculated the Spearman’s rank correlation coefficients of the EEG power values across epochs between the delta-band and the alpha-band power in each channel. Results All scalp EEG and most iEEG channels showed large increases of delta-band power during non-REM sleep compared to awake. Many (> 75%) of these channels also showed significant increases of alpha-band power, and positive correlations across overnight epochs between delta-band and alpha-band power, indicating involvement of alpha-delta sleep. However, a minority (< 25%) of iEEG channels, often located in the temporal lobe, showed significant decreases of alpha-band power during non-REM sleep compared to awake, and negative correlations between delta-band and alpha-band power, therefore appearing to be not involved in alpha-delta sleep. Conclusion Our iEEG study showed for the first time that alpha-delta sleep is not homogeneous across the cerebral cortex. Instead, the cortical distribution of alpha waves is more spatially restricted than the distribution of delta waves during non-REM sleep. Abstract:Despite the known behavioral benefits of rapid eye movement (REM) sleep, discrete neural oscillatory events in human scalp electroencephalography (EEG) linked with behavior have not been discovered. This knowledge gap hinders mechanistic understanding of the function of sleep, as well as the development of biophysical models and REM-based causal interventions. We designed a detection algorithm to identify bursts of activity in high-density, scalp EEG within theta (4–8 Hz) and alpha (8–13 Hz) bands during REM sleep. Across 38 nights of sleep, we characterized the burst events (i.e., count, duration, density, peak frequency, amplitude) in healthy, young male and female human participants (38; 21F) and investigated burst activity in relation to sleep-dependent memory tasks: hippocampal-dependent episodic verbal memory and non-hippocampal visual perceptual learning. We found greater burst count during the more REM-intensive second half of the night (p < .05), longer burst duration during the first half of the night (p .05). Moreover, increased alpha burst power was associated with increased overnight forgetting for episodic memory (p < .05). Furthermore, we show that increased REM theta burst activity in retinotopically specific regions was associated with better visual perceptual performance. Our work provides a critical bridge between discrete REM sleep events in human scalp EEG that support cognitive processes, and the identification of similar activity patterns in animals models that allow for further mechanistic characterization. Significance Statement Current understanding of sleep and its role cognitive processes is incomplete due to a lack of discrete electrophysiological events in human rapid eye movement (REM) sleep detectable via scalp EEG. Our work remedies this gap in knowledge by designing an open-source, computational approach to identify electrophysiological alpha and theta burst events in REM sleep. Additionally, we provide evidence that these burst events are functionally important for learning and memory. Defining burst events in human REM will contribute to the development of a comprehensive mechanistic model of how sleep as a whole, and REM specifically, facilitate cognitive processes, and provide a deeper understanding of the fundamental electrophysiological properties of REM sleep that are distinct from non-REM sleep.

Abstract:Purpose: Interoceptions are a combination of sensation, integration, and interpretation of internal bodily signals. Interoceptions are bidirectionally related to the human being mental and physiological health, and well-being. Sleep and different interoceptive modalities are proven to share common relations. Heartbeat Evoked Potential (HEP) is known as a robust readout to interoceptive processes. In this study, we focused on the relation between HEP modulations and sleep-related disorders. Materials and Methods: We investigated four different sleep-related disorders, including insomnia, rapid eye movement behavior disorder, periodic limb movements and nocturnal frontal lobe epilepsy, and provided HEP signals of multiple Electroencephalogram (EEG) channels over the right hemisphere to compare these disorders with the control group. Here, we investigated and compared the results of 35 subjects, including seven subjects for the control group and seven subjects for each of above-mentioned sleep disorders. Results: By comparing HEP responses of the control group with sleep-related patients’ groups, statistically significant HEP differences were detected over right hemisphere EEG channels, including FP2, F4, C4, P4, and O2 channels. These significant differences were also observed over the grand average HEP amplitude activity of channels over the right hemisphere in the sleep-related disorders as well. Conclusion: Our results between the control group and groups of patients suffering from sleep-related disorders demonstrated that during different stages of sleep, HEPs show significant differences over multiple right hemisphere EEG channels. Interestingly, by comparing different sleep disorders with each other, we observed that each of these HEP differences’ patterns over specific channels and during certain sleep stages bear considerable resemblances to each other.
