Abstract:ObjectiveSystemic sclerosis (SSc) is a clinically heterogeneous disease characterized by increased collagen accumulation and skin stiffness. Our previous work has demonstrated that transforming growth factor β (TGFβ) induces extracellular matrix (ECM) modifications through lysyl oxidase–like 4 (LOXL‐4), a collagen crosslinking enzyme, in bioengineered human skin equivalents (HSEs) and self‐assembled stromal tissues (SAS). We undertook this study to investigate cutaneous fibrosis and the role of LOXL‐4 in SSc pathogenesis using HSEs and SAS.MethodsSSc‐derived dermal fibroblasts (SScDFs; n = 8) and normal dermal fibroblasts (NDFs; n = 6) were incorporated into HSEs and SAS. These 3‐dimensional skin‐like microenvironments were used to study the effects of dysregulated LOXL‐4 on ECM remodeling, fibroblast activation, and response to TGFβ stimulation.ResultsSScDF‐containing SAS showed increased stromal thickness, collagen deposition, and interleukin‐6 secretion compared to NDF‐containing SAS (P < 0.05). In HSE, SScDFs altered collagen as seen by a more mature and aligned fibrillar structure (P < 0.05). With SScDFs, enhanced stromal rigidity with increased collagen crosslinking (P < 0.05), up‐regulation of LOXL4 expression (P < 0.01), and innate immune signaling genes were observed in both tissue models. Conversely, knockdown of LOXL4 suppressed rigidity, contraction, and α‐smooth muscle actin expression in SScDFs in HSE, and TGFβ‐induced ECM aggregation and collagen crosslinking in SAS.ConclusionA limitation to the development of effective therapeutics in SSc is the lack of in vitro human model systems that replicate human skin. Our findings demonstrate that SAS and HSE can serve as complementary in vitro skin‐like models for investigation of the mechanisms and mediators that drive fibrosis in SSc and implicate a pivotal role for LOXL‐4 in SSc pathogenesis.
[Mrna Expression of Procollagen and SMADs in Lesional Skin of Progressive Systemic Sclerosis].

Abstract:OBJECTIVE:To investigate the mRNA expression of type I and type III procollagen and SMADs in the lesional skin of progressive systemic sclerosis (PSS) and analyze the role in pathogenesis of scleroderma thereof.METHODS:Real time PCR was used to detect the mRNA expression of type I and type III procollagen, and SMADs (SMAD 2, SMAD 3, SMAD 4, and SMAD 7) in the lesional skin specimens of 11 patients with PSS and 11 specimens of normal skin.RESULTS:Type I procollagen alpha1, type III procollagen alpha1, SMAD 4, and SMAD 7 were expressed at the mRNA level significantly higher in the lesional skin tissues of the PSS patients than in the normal skin tissues (all P < 0.05). SMAD 4 mRNA expression was positively correlated with type I procollagen alpha1 mRNA expression (r = 0.728, P < 0.05) and the type III procollagen alpha1 mRNA expression (r = 0.678, P < 0.05). SMAD3 mRNA expression was positively correlated with the type I procollagen alpha1 mRNA expression (r = 0.859, P < 0.01).CONCLUSION:Increased SMAD4 mRNA in the lesional skin of systemic sclerosis is positively correlation with increased type I and type III procollagen alpha1 mRNA expression and may be one of the main pathogenesis in resulting in excessive type I and type III collagen accumulation in PSS lesional skin.
Role of endothelin-1 in the skin fibrosis of systemic sclerosis

Abstract:Endothelin-1 (ET-1) acts as a key regulator of vasoconstriction and fibrosis. Many previous studies have focused on the role of ET-1 in scleroderma (systemic sclerosis, SSc).We investigated the effects of ET-1 on the production of extracellular matrix in SSc and normal skin fibroblasts. Primary cultured dermal fibroblasts from SSc patients and healthy controls were treated with ET-1 (25 ng/mL) for 0 min, 15 min, 1 h, 24 h, 48 h and 72 h, respectively. Our results showed that, in SSc fibroblasts, ET-1 upregulated collagen type I, connective tissue growth factor (CTGF), type I plasminogen activator inhibitor (PAI-1) and pAkt in a time-dependent manner within 72 h; in normal fibroblasts, 25 ng/mL ET-1 stimulation correlated with high levels of CTGF, PAI-1 and pAkt. The secretion of fibronectin (FN), collagen type I, and PAI-1 is markedly increased in the supernatant of both SSc fibroblasts and normal fibroblasts. Furthermore, ET-1 phosphorylates Smad2 and Smad3 in normal fibroblasts, but not in SSc fibroblasts. In conclusion, our results demonstrated that ET-1 may induce fibrosis in dermal fibroblasts through Akt signals.
Expression of activin receptor-like kinases 1 in dermal fibroblasts from patients with systemic scleroderma and its roles

Abstract:Objective To measure the expression of activin receptor-like kinases 1(ALK1)in dermal fibroblasts from patients with systemic scleroderma(SSc)and to estimate its role in the production of fibronectin and plasminogen activator inhibitor-1(PAI-1).Methods Dermal fibroblasts were isolated from the lesions of 12 patients with SSc as well as the normal skin of 14 healthy controls,and subjected to a primary culture.The third-passage fibroblasts were used in the next experiment.Western blot and indirect immunofluorescence technique were utilized to quantify the expression of ALK1.A specific siRNA targeting ALK1 was designed,constructed,and transiently transfected into the control dermal fibroblasts,which were then classified into 2 groups to be cultured with or without the presence of transforming growth factor(TGF)-β1 for 72 hours followed by the detection of fibronectin and PAI-1 expression with Western blot.Results As Western blot and direct immunofluorescence technique showed,both control and SSc fibroblasts showed an expression of ALK1 in the cytoplasm and membrane,and the expression intensity of ALK1 in SSc fibroblasts was significantly higher than that in the control fibroblasts(1.97 ± 0.05 vs.1.12 ± 0.03,t =50.96,P ＜ 0.05).The expression of ALK1,fibronectin and PAI-1 was decreased by 90％,58％ and 31％ respectively in specific siRNA-transfected SSc fibroblasts compared with the control siRNA-transfected fibroblasts.TGFβ1 significantly increased the expression of ALK1,fibronectin and PAI-1 in the control siRNA-transfected fibroblasts,but the increase was markedly inhibited by the siRNA-targeting ALK1.Conlusion TGFβ1 can promote the production of fibronectin and PAI-1 via ALK1 in fibroblasts,and ALK1 may be involved in the development of sclerosis in SSc.
Systemic sclerosis Th2 cells inhibit collagen production by dermal fibroblasts via membrane-associated tumor necrosis factor alpha

Abstract:Objective: In systemic sclerosis (SSc; scleroderma), T cells infiltrate organs undergoing fibrotic changes and may participate in dysregulated production of collagen by fibroblasts. The objective of this study was to functionally characterize T cells infiltrating skin lesions in early SSc and investigate their capacity to affect production of type I collagen and interstitial collagenase (matrix metalloproteinase 1 [MMP-1]) by dermal fibroblasts. Methods: Four-color cytometric analysis was used to characterize subset distribution and production of interferon-gamma (IFN gamma) and interleukin-4 (IL-4) in T cell lines generated from the skin of patients with SSc. T cell clones were generated, and their capacity to modulate collagen and MMP-1 production by fibroblasts derived from patients with SSc and from normal individuals was assessed. Neutralizing reagents were used to identify T cell mediators involved in fibroblast modulation. Results: The skin of individuals with early-stage SSc contained T cells preferentially producing high levels of IL-4. Cloned CD4+ Th2-like cells inhibited collagen production by normal fibroblasts. Th2 cell-dependent inhibition was, at least in part, contact-dependent, was essentially mediated by tumor necrosis factor alpha (TNF alpha), and was dominant over the enhancement induced by profibrotic IL-4 and transforming growth factor beta cytokines. The simultaneous induction of MMP-1 production confirmed the specificity of these observations. To be inhibitory, Th2 cells required activation by CD3 ligation. Th2 cells were less potent than were Th1 cells in inhibiting collagen production by normal fibroblasts via cell-to-cell interaction, and SSc fibroblasts were resistant to inhibition. Conclusion: These findings indicate that, despite their production of IL-4, Th2 cells reduce type I collagen synthesis by dermal fibroblasts because of the dominant effect of TNF alpha, and suggest that strategies based on TNF alpha blockade aimed at controlling fibrosis in SSc may be unwise.
Transcriptional Characteristics of Type Ⅰ Procollagen Gene of Fibroblast Clones from Patients with Systemic Sclerosis

Abstract:Objective To study the mechanisms underlying high collagen synthesis of systemic scle-rosis skin fibroblasts. Methods The mRNA expression levels 6f procollagen alpha 1 type 1 (COL1A1) were detected by in situ hybridization to select heterogeneic fibroblast clones, and monoclonal fibroblasts were divided into three groups according to their mRNA levels of COL1A1. Meanwhile, five deletion con-structs containing portions of human procollagen gene promoter were ligated into plasmids and used to trans-fect the three groups of fibroblast clones. The transcription of COL1A1 in these fibroblasts was assessed by transient transfection and luciferase assay. Re.milts Fibroblast clones from both patients and normal controls showed a heterogeneity in the mRNA expression of COL1A1. Patients with systemic sclerosis had a larger proportion of fibroblasts expressing high level of COL1A1mRNA than did normal controls (50% vs 12.5%). A significant increase was also observed in the mean mRNA levels of COL1A1 in fibroblast clones from patients with systemic sclerosis compared with those from normal controls (6.64 ± 2.48 vs 3.69 ± 2.01, t = 3.690, P < 0.01 ). Maximal transcriptional activity was observed with a construct containing COL1A1 pro-moter from -174 bp to +42 bp in all fibroblast clones. The activities of COL1A1 promoter positively corre-lated with the mRNA levels of this gene. Conclusions COL1A1 gene might be activated at the transcrip-tional level in systemic sclerosis fibroblast clones with a high rate of collagen synthesis. [Expression and distribution of type I, III and V collagens in skin lesions from patients with systemic sclerosis]

Abstract:Objective: To investigate the expressionand distribution of collagen I, III, and V (Col I, III, and V) in skin lesions of systemic sclerosis patients (SSc). Methods: Skin lesions were collected from 36 patients with SSc (mild fibrosis 9 patients, moderate 14, and severe 13). The expression and distribution of alpha1 chain of collagen type I, III and V [alpha1 (I), alpha1 (III) and alpha1 (IV)] in skin lesions were detected with immunohistochemical SP method, while 6 normal skin tissues were used as controls. Results: All the three types of collagens were observed with diffuse distribution in dermis of SSc skin lesions. The deep staining alpha1 (I) and alpha1 (III) clumps and bands were seen in reticular layer, while alpha1 (V) was more homogeneously andcommonly around small vessels and appendages. The expression of alpha (I), and alpha1 (III), and alpha1 (V) appeared to gradually increase in dermis and the area around small vessels, appendages with the increase of fibrosis severity (P < 0.05). The level of alpha1 (V) was obviously elevated at early stage, and persisted in whole fibrotic process, while al (I) and alpha1 (III) increased apparently later in middle and late stages. Conclusion: alpha1 (V) began to deposit prior to alpha1 (I) and alpha1 (III) at early stage in SSc and persisted in whole fibrotic process. The changes of alpha1 (V) contents started in dermal tissues around small vessels and appendages, which may involve in the development of vasculitis in SSc.
Transcriptional Regulation of TGF-β1、CTGF and IFN-γ on Type Ⅰ and Ⅲ Procollagen Gene in Scleroderma Fibroblast Clones with Different Collagen-Producing Capacity

Abstract:Objective:To study the transcriptional regulation of three cytokines in fibrosis(TGF-β1、CTGF and IFN-γ) on type Ⅰand Ⅲ procollagen gene in systemic scleroderma(SS) fibroblast clones.Methods:The effects of TGF-β1,CTGF and IFN-γ on human procollagen alpha 1 type 1(COL1A1) and COL3A1 proximal promoter activity in SS and normal control fibroblast clones with different collagen-producing capacity were detected by transient transfection and dual-luciferase reporter assay system.Results: TGF-β1(5μg/L) upregulated phCOLH10.2(COL1A1 proximal promoter) activity in SS and normal control high and low collagen-producing fibroblast clones and phCOLH30.1(COL3A1 proximal promoter) activity in normal control high and low collagen-producing fibroblast clones.CTGF(20μg/L) upregulated phCOLH10.2 and phCOLH30.1 activity in SS and normal control high and low collagen-producing fibroblast clones,and it preferentially upregulated phCOLH30.1 activity in SS low collagen-producing fibroblast clones.IFN-γ(100μg/L)preferentially upregulated phCOLH30.1 activity in SS low collagen-producing fibroblast clones,compared with that in controls,and it did not change phCOLH10.2 activity in SS and normal control high and low collagen-producing fibroblast clones and phCOLH30.1 activity in normal control high and low collagen-producing fibroblast clones.Conclusions: At transcriptional level,the responses of SS high collagen-producing fibroblast clones to exogenous cytokines decline.IFN-γ does not inhibit the activity of phCOLH10.2 and phCOLH30.1 in SS and normal control fibroblast clones.
The downregulation of microRNA let-7a contributes to the excessive expression of type I collagen in systemic and localized scleroderma

Abstract:Systemic and localized scleroderma (SSc and LSc) is characterized by excessive deposition of collagen and tissue fibrosis in the skin. Although they have fundamental common characteristics including autoimmunity, little is known about the exact mechanism that mediates the excessive collagen expression in these disorders. In the current study, we tried to evaluate the possibility that microRNAs (miRNAs) play some roles in the pathogenesis of fibrosis seen in these diseases. miRNA expression patterns were evaluated by miRNA array analysis, real-time PCR, and in situ hybridization. The function of miRNAs in dermal fibroblasts was assessed using miRNA inhibitors, precursors, or protectors. In the mouse model of bleomycin-induced dermal sclerosis, the overexpression of miRNAs was performed by i.p. miRNA injection. We demonstrated let-7a expression was downregulated in SSc and LSc skin both in vivo and in vitro, compared with normal or keloid skin. The inhibition or overexpression of let-7a in human or mouse skin fibroblasts affected the protein expression of type I collagen or luciferase activity of collagen 3'-untranslated region. Also, we found let-7a was detectable and quantitative in the serum and investigated serum let-7a levels in patients with SSc or LSc. let-7a concentration was significantly decreased in these patients, especially in LSc patients. Moreover, we revealed that the intermittent overexpression of let-7a in the skin by i.p. miRNA injection improved the skin fibrosis induced by bleomycin in mice. Investigation of more detailed mechanisms of miRNA-mediated regulation of collagen expression may lead to new therapeutic approaches against SSc and LSc.
Atypical Chemokine Receptor 1-Positive Endothelial Cells Mediate Leucocyte Infiltration and Synergize with Secreted Frizzled-Related Protein 2/Asporin-Positive Fibroblasts to Promote Skin Fibrosis in Systemic Sclerosis.

Abstract:Background Skin fibrosis is the typical pathological manifestation of systemic sclerosis (SSc) and localized scleroderma (LS); it has an unclear aetiology and few effective treatments. Although excessive collagen secretion by fibroblasts is the primary cause of skin fibrosis, evidence has suggested that vascular damage is the initiating event and that various cell types, including fibroblasts, work together to contribute to the pathogenesis of skin fibrosis.Objectives To explore the relationship between vascular endothelial cell lesions and immune cell infiltration, along with the interactions between various cell types within the fibrotic skin ecosystem.Methods Single-cell RNA sequencing was performed on skin biopsies from three healthy donors and seven patients with SSc. Additional data from three patients with localized scleroderma available in the Gene Expression Omnibus (GSE160536) were integrated by Harmony. CellChat (version 1.5.0) was used to analyse the cell communication network. A Transwell (R) assay and a bleomycin (BLM) mouse model were used to explore the role of atypical chemokine receptor 1 (ACKR1; 'Duffy antigen') in immune cell infiltration. Milo single-cell Western blot was used to show fibroblast subcluster activation.Results A total of 62 295 cells were obtained and subpopulations of stromal and immune cells identified. Interaction network analysis found that multiple chemokines secreted by macrophages, pericytes and proinflammatory fibroblasts could bind with ACKR1, which was highly expressed by endothelial cells in lesional skin. The Transwell (R) assay revealed that overexpression of ACKR1 in human umbilical vein endothelial cells facilitated leucocyte infiltration following treatment with interleukin-8. BLM mice showed enhanced ACKR1 expression, massive immune cell infiltration and skin fibrosis that was attenuated by ACKR1 inhibition. Furthermore, infiltrated macrophages expressing high levels of transforming growth factor (TGF)-beta 1 or platelet-derived growth factor B (PDGFB) could activate secreted frizzled-related protein 2 (SFRP2)/asporin (ASPN)+ fibroblasts to contribute to the excessive accumulation of extracellular matrix. It was also found that the SOX4-ASPN axis plays an important role in the TGF-beta signalling cascade and the aetiology of skin fibrosis.Conclusions Our results reveal that high expression of ACKR1 by endothelial cells in fibrotic skin tissue promotes immune cell infiltration and that SFRP2/ASPN+ fibroblasts synergize to exacerbate skin fibrosis. Graphical Abstract We performed a systemic characterization of the interactions between various cell types in fibrotic skin tissues, providing novel insights into the mechanism of skin fibrosis. We identified a cluster of atypical chemokine receptor 1 (ACKR1)+ endothelial cells that are highly expressed in fibrotic skin tissue, and found that ACKR1+ endothelial cells could promote immune cell infiltration and mediate the development of skin fibrosis, providing a theoretical basis for potential anti-ACKR1 treatment in skin fibrosis. Infiltrating macrophages activated SFRP2/ASPN+ fibroblasts by secreting TGF-beta 1 and PDGFB, suggesting a role of macrophage-fibroblast interactions in skin fibrosis. The SOX4-ASPN axis plays an important role in the TGF-beta signalling cascade and aetiology of skin fibrosis. Skin fibrosis is characterized by thickening or hardening of the skin. It occurs in a number of skin diseases. It has been suggested that different cell types work together to contribute to the processes involved in skin fibrosis.In this study, we looked at interactions between different cell types involved in two different fibrotic skin diseases called 'systemic sclerosis' and 'localized scleroderma'. We found that many signalling proteins produced by certain cell types interacted strongly with a protein called 'ACKR1'. Other investigations showed that high levels of ACKR1 aggravated skin fibrosis. Finally, 'macrophages' (a type of white blood cell) that produced high levels of two different proteins caused skin fibrosis to worsen.Overall, our study findings provide a deeper insight into cellular interactions that might cause skin fibrosis to develop. This may lead to new treatments for these two diseases.

Abstract:Systemic sclerosis (SSc) is a connective tissue disease of autoimmune origin characterized by fibrosis of the skin and visceral organs, and peripheral circulatory disturbance. α2‑antiplasmin (α2AP) is the major circulating inhibitor of plasmin and is a key regulator of fibrinolysis. It has been demonstrated that the expression of α2AP is elevated in dermal fibroblasts obtained from patients with SSc patients. It has also been determined that α2AP is associated with the development and progression of fibrosis in SSc. The present study assessed the relationship between α2AP and matrix metalloproteinase‑3 (MMP‑3), an extracellular matrix (ECM)‑degrading enzyme. Serum levels of α2AP and MMP‑3 were measured in healthy controls and patients with SSc using ELISA. No significant differences were determined between these two groups. α2AP, MMP‑3 and tissue inhibitor of metalloproteinase‑1 (TIMP‑1) expression was subsequently evaluated in normal and SSc fibroblasts via western blotting. The results revealed that α2AP expression increased in SSc dermal fibroblasts, while the ratio of MMP‑3/TIMP‑1 decreased. Additionally, incubation of recombinant α2AP with MMP‑3 caused α2AP degradation. The mixture of recombinant α2AP with MMP‑3 was subsequently added to normal fibroblasts prior to western blotting. The results revealed decreased α‑smooth muscle actin (α‑SMA; a marker of the myofibroblast phenotype) and type I collagen expression. The stimulation of SSc fibroblasts with MMP‑3 decreased protein levels of α2AP, α‑SMA and type I collagen, thus reversing the pro‑fibrotic phenotype of SSc fibroblasts. SSc fibroblast transfection with microRNA‑29a resulted in a decreased TIMP‑1 expression, but also decreased the protein expression of α2AP. The results indicated that MMP‑3 attenuated fibrosis progression by degrading α2AP and ECM, and might therefore contribute to a novel therapeutic approach for SSc treatment.

Abstract:BACKGROUND: Systemic scleroderma (SSc) is an acquired disorder characterized by excessive deposition of extracellular matrix in the skin and internal organs. Collagen triple helix repeat containing-1 (CTHRC1) has been indicated to be a cell type-specific inhibitor of transforming growth factor-β (TGF-β), which could have the potential for extensive clinical application owing to its ability to reduce collagen deposition. Our previous studies showed that CTHRC1 inhibited TGF-β1-induced collagen type I synthesis in keloid fibroblasts. In our present research, we attempted to probe the role of CTHRC1 in dermal fibrosis in bleomycin (BLM)-treated mice.METHODS: CTHRC1 and TGF-β1 expression was detected in dermal tissues from patients with SSc and BLM-treated mice by immunohistochemistry. A 3-(4,5-dimethyl-2-thiazolyl)-2,5-diphenyl-2H-tetrazolium bromide assay was used to assess TGF-β1-induced proliferation of human dermal fibroblasts. Collagen expression and fibroblast synthesis were evaluated by quantitative real-time polymerase chain reaction and the 3H-proline incorporation. Masson's trichrome staining and western blotting were carried out to analyze the deposits and protein levels of type I collagen, respectively.RESULTS: Compared with those in normal tissues, the levels of CTHRC1 and TGF-β1 were elevated in dermal tissues from patients with SSc and in skin tissues from BLM-treated mice, respectively. Furthermore, recombinant CTHRC1 was found to inhibit TGF-β1-stimulated collagen deposition by fibroblasts. Finally, the in vivo experiments showed that CTHRC1 alleviated BLM-induced dermal fibrotic changes.CONCLUSIONS: CTHRC1 can inhibit human dermal fibroblast collagen deposition and can also exert protective effects against BLM-induced dermal fibrosis in mice. This research provides an indication that CTHRC1 may be a promising treatment choice for dermal fibrosis in SSc patients.

Abstract:Systemic sclerosis (SSc) is characterized by dermal fibrosis with a female predominance, suggesting a hormonal influence. Patients with SSc have elevated interleukin (IL)-6 levels, and post-menopausal women and older men also have high estradiol (E2) levels. In the skin, IL-6 increases the enzymatic activity of aromatase, thereby amplifying the conversion of testosterone to E2. Therefore, we hypothesized that an interplay between E2 and IL-6 contributes to dermal fibrosis. We used primary dermal fibroblasts from healthy donors and patients with diffuse cutaneous (dc)SSc, and healthy donor skin tissues stimulated with recombinant IL-6 and its soluble receptor (sIL-6R) or E2. Primary human dermal fibroblasts and tissues from healthy donors stimulated with IL-6+sIL-6R produced E2, while E2-stimulated dermal tissues and fibroblasts produced IL-6. Primary dermal fibroblasts from healthy donors treated with IL-6+sIL-6R and the aromatase inhibitor anastrozole (ANA) and dcSSc fibroblasts treated with ANA produced less fibronectin (FN), type III collagen A1 (Col IIIA1), and type V collagen A1 (Col VA1). Finally, dcSSc dermal fibroblasts treated with the estrogen receptor inhibitor fulvestrant also generated less FN, Col IIIA1, and Col VA1. Our data show that IL-6 exerts its pro-fibrotic influence in human skin in part through E2 and establish a positive feedback loop between E2 and IL-6.

Abstract:Introduction: Although immune dysfunction plays a role in the pathogenesis of systemic sclerosis (SSc), involvement of T helper 17 (Th17) and T regulatory (Treg) cells remains unclear. We aimed to investigate the presence of Th17 and Treg cells in SSc patients and the role of Th17 cells in collagen production in SSc fibroblasts.Methods: We analyzed inflammatory cell profiles in the skin of 13 SSc patients by immunohistochemistry, the percentage of Th17 and Treg cells in peripheral blood mononuclear cells (PBMCs) of 45 SSc patients and 24 healthy controls by flow cytometry, gene expression in PBMCs by real-time reverse transcription-polymerase chain reaction and interleukin-17 (IL-17) in sera and culture supernatants by enzyme-linked immunosorbent assay. We also investigated the effect of Th17 cell-derived IL-17 on fibroblast growth and collagen production.Results: Infiltration of inflammatory cells including IL-17(+) and Foxp3(+) lymphocytes was detected in the skin of patients with early SSc. The percentages of circulating Th17 cells and IL-17 production were elevated in samples from patients with active SSc, whereas the percentage of circulating Treg cells was not affected. The number of Th17 cells was closely related to disease activity. IL-17 from SSc patients promoted fibroblast growth and collagen production, whereas IL-17 neutralizing antibody effectively blocked collagen production.Conclusion: SSc progression might be linked to expansion of circulating Th17 cells and increased infiltration of IL-17(+) cells in skin. Th17-derived IL-17 is involved in fibroblast growth and collagen production. IL-17 blocking antibody may be a useful tool for intervention in the fibrotic course of SSc.

Abstract:The hallmark of scleroderma is fibrosis by excessive extracellular matrix (ECM) deposition in the skin, lung, and other organs. Increasing evidence suggests that overexpression of transforming growth factor-beta (TGF-beta) and its receptors play a key pathogenic role in the development of tissue fibrosis in scleroderma. TGF-beta is known to induce the expression of ECM proteins in the pathogenesis of fibrosis in systemic sclerosis. Investigations into TGF-beta pathways will suggest new treatment strategies for fibrotic diseases.

Abstract:Systemic sclerosis (SSc) is a multisystem connective tissue disease. Skin fibrosis, the hallmark of this disease, is defined as the excess deposition and accumulation of extracellular matrix, mainly type 1 collagen, in the dermis. SLC39A7 is an intracellular zinc transporter that plays a unique role in connective tissue formation. Therefore, we investigated the expression and role of SLC39A7 in SSc. Using immunohistochemical staining we demonstrated the overexpression of SLC39A7 in the skin of SSc patients. Quantitative real‐time PCR and western blot data analysis showed that both SLC39A7 mRNA and protein levels were significantly upregulated in dermal fibroblasts from SSc patients compared to healthy controls. We used the shRNA lentiviral particle transduction system to stably knockdown the expression of SLC39A7 in SSc fibroblasts. The results showed that knockdown of SLC39A7 suppressed the production of type 1 collagen. These findings provide evidence that SLC39A7 is involved in the pathogenesis of SSc and that SLC39A7 plays a positive role in its progression.

Abstract:We previously demonstrated that the non-calcemic pregnacalciferol (pD) analog 17,20S (OH)2pD suppressed TGF-β1-induced type I collagen production in cultured normal human dermal fibroblasts. In the present studies, we examined fibroblasts cultured from the lesional skin of patients with systemic sclerosis (scleroderma (SSc)) and assessed the effects of 17,20S(OH)2pD on fibrosis-related mediators. Dermal fibroblast lines were established from skin biopsies from patients with SSc and healthy controls. Fibroblasts were cultured with either 17,20S(OH)2pD or 1,25(OH)2D3 (positive control) with/without TGF-β1 stimulation and extracted for protein and/or mRNA for collagen synthesis and mediators of fibrosis (MMP-1, TIMP-1, PAI-1, BMP-7, PGES, GLI1, and GLI2). 1 7,20S(OH)2pD (similar to 1,25(OH)2D3) significantly suppressed net total collagen production in TGF-β1-stimulated normal donor fibroblast cultures and in cultures of SSc dermal fibroblasts. 17,20S(OH)2pD (similar to 1,25(OH)2D3) also increased MMP-1, BMP-7, and PGES and decreased TIMP-1 and PAI1 expression in SSc fibroblasts. Although 17,20S(OH)2pD had no effect on Gli1 or Gli2 in SSc fibroblasts, it increased Gli2 expression when cultured with TGF-β1 in normal fibroblasts. These studies demonstrated that 17,20S(OH)2pD modulates mediators of fibrosis to favor the reduction of fibrosis and may offer new noncalcemic secosteroidal therapeutic approaches for treating SSc and fibrosis.

Abstract:Objectives: Systemic sclerosis (SSc) is an autoimmune connective tissue disease involving multiple organs. The most common clinical symptom of SSc is progressive fibrosis of the skin, and the pathologically manifestations of skin were activation and proliferation of fibroblasts and continuous proliferation of extracellular matrix. Transforming growth factor β (TGFβ) can promote the proliferation and activation of fibroblasts, causing excessive deposition of collagen and structural proteins. Therefore, exploring the specific mechanism of TGFβ-related pathway on fibrosis is of great significance for improving skin fibrosis in SSc. Methods: Genes related to TGFβ pathway were screened through bioinformatics analysis, and SSc phenotypes were verified in vivo and vitro. The relevant molecular mechanisms were preliminarily discussed in combination with transcriptome sequencing. Results: Human cysteine-rich secreted protein LCCL domain protein 2 (CRISPLD2) was found increased reactivity in TGFβ induced fibroblasts, and the expression of ACTA2 (ɑ-SMA) decreased significantly in TGFβ-mediated fibroblasts with up-regulation of CRISPLD2. Conclusion: CRISPLD2 was found increased reactivity in TGFβ induced fibroblasts, and we further confirmed that CRISPLD2 can participate in TGFβ induced fibroblast fibrosis from multiple perspectives and levels in negative feedback regulation, and investigated the mechanism of CRISPLD2 in fibrosis.

Abstract:Objective To study the transcriptional regulation of type Ⅰ procollagen gene in systemic scleroderma(SS)-derived high collagen-producing fibroblast clones by Radix Salviae Miltiorrhizae(RSM).Methods Fibroblast clones with different collagen-producing capacity were previously obtained from patients with SS and normal human controls,and divided into 5 groups to be treated with RSM(1 g/L)injection,its water-soluble active monomers including sodium danshensu(20 mg/L),salvianolic acid B(5 mg/L)and protocatechuic aldehyde(5 mg/L),and lipid-soluble active monomer(tanshinone Ⅱ A,5mg/L)respectively.The fibroblast clones incubated with no drugs served as the water soluble negative control group,and those with dimethyl sulfoxide(DMSO)as the lipid soluble negative control group.MTT assay was performed to evaluate the proliferation of the fibroblast clones after 1-,3-,5-,and 7-day treatment,transient transfection and dualluciferase reporter assay system to quantify the relative activity of collagen type Ⅰ,alpha 1(COL1A1)proximal promoter in these fibroblast clones.Results The inhibitory effect of RSM and its active monomers on the proliferation of fibroblast clones was inapparent within the initial 3 days(P ＞ 0.05),but was enhanced with incubation time.A significant difference was observed in the proliferation level of fibroblast clones between RSM group and water-soluble negative control group on day 5(q′ =3.22,P ＜ 0.01),between RSM,salvianolic acid B,protocatechuic aldehyde groups and the water-soluble negative control group(q′ =4.74,3.03,2.56,all P ＜0.05)on day 7,and between tanshinone Ⅱ A and lipid-soluble negative control group on day 5 and 7(t =2.22,2.15,both P ＜ 0.05).RSM injection,tanshinone Ⅱ A and protocatechuic aldehyde significantly inhibited COL1A1 proximal promoter activity in SS-derived and normal control fibroblast clones(all P ＜ 0.01),and the former two drugs preferentially downregulated COL1A1 proximal promoter activity in SS-derived high collagenproducing fibroblast clones.Significantly different COL1A1 proximal promoter activity was observed in SS-derived high and low collagen-producing fibroblast clones between water-soluble negative control group and RSM injection group(12.019 ± 0.830 vs.4.445 ± 1.061,5.388 ± 0.480 vs.2.856 ± 0.597,F=31.78,P＜ 0.01),and between lipid-soluable negative control group and tanshinone Ⅱ A group(14.155 ± 0.672 vs.9.638 ±0.854,4.299 ± 0.252 vs.3.192 ± 0.450,F=24.10,P＜ 0.01).Conclusions RSM inhibits the transcription of COL1A1 gene in SS-derived high collagen-producing fibroblast clones,which may be mainly attributed to tanshinone Ⅱ A and protocatechuic aldehyde.

Abstract:Abstract Background Skin fibrosis is the most typical pathological manifestation of systemic sclerosis (SSc) and localized scleroderma (LS) with unclear etiology and few effective treatments. Though excessive collagen secretion by fibroblasts is the primary cause of skin fibrosis, many lines of evidence suggested that vascular damage was the initiating event and various cell types along with fibroblasts worked together to contribute to the pathogenesis of skin fibrosis. Objectives We sought to explore the relationships between vascular endothelial cell lesions and immune cell infiltration, along with the cell-cell interactions among various cell types within the fibrotic skin ecosystem. Methods Single-cell RNA-seq (10x Genomics) was performed on skin biopsies of 3 healthy donors and 7 SSc patients in Chinese. The additional 3 localized scleroderma patients’ data from NCBI database (GSE160536) were integrated by Harmony. CellChat package (v1.5.0) was applied to analyze cell communication network. Transwell assay and subcutaneous bleomycin (BLM) injection in mice were used to explore the role of ACKR1 on immune cell infiltration. Milo single-cell western blot was applied to show the activation of fibroblast subclusters. Results A total of 62,295 cells were obtained and subpopulations of stromal and immune cells were identified. Interaction network analysis revealed that multiple chemokines secreted by macrophages, pericytes, and pro-inflammatory fibroblasts could bind with Duffy antigen/receptor for chemokines (ACKR1), which is highly expressed on ACKR1+ endothelial cells of lesion skin. Transwell assay revealed that over-expressed ACKR1 in HUVEC facilitated leukocyte infiltration under the treatment of IL8. The BLM mice showed enhanced ACKR1 expression, massive immune cell infiltration, and fibrosis in skin, which could be attenuated by ACKR1 inhibition. Furthermore, infiltrated macrophages with TGFB1 or PDGFB high production could activate SFRP2/ASPN+ fibroblasts to contribute to excessive accumulation of extracellular matrix (ECM), and the SOX4-ASPN axis plays an important role in the TGF-β signaling cascade and the etiology of skin fibrosis. Conclusions Our results reveal that highly expressed ACKR1 in endothelial cells of fibrotic skin tissue promotes immune cell infiltration, and SFRP2/ASPN+ fibroblasts synergize to exacerbate skin fibrosis.
