Abstract:Big Data and Artificial Intelligence can profoundly transform medical practices, particularly in oncology. Comprehensive Cancer Centers have a major role to play in this revolution. With the purpose of advancing our knowledge and accelerating cancer research, it is urgent to make this pool of data usable through the development of robust and effective data warehouses. Through the recent experience of Comprehensive Cancer Centers in France, this article shows that, while the use of hospital data warehouses can be a source of progress by taking into account multisource, multidomain and multiscale data for the benefit of knowledge and patients, it nevertheless raises technical, organizational and legal issues that still need to be addressed. The objectives of this article are threefold: 1. to provide insight on public health stakes of development in Comprehensive Cancer Centers to manage cancer patients comprehensively; 2. to set out a challenge of structuring the data from within them; 3. to outline the legal issues of implementation to carry out real-world evidence studies. To meet objective 1, this article firstly proposed a discussion on the relevance of an integrated approach to manage cancer and the formidable tool that data warehouses represent to achieve this. To address objective 2, we carried out a literature review to screen the articles published in PubMed and Google Scholar through the end of 2022 on the use of data warehouses in French Comprehensive Cancer Centers. Seven publications dealing specifically with the issue of data structuring were selected. To achieve objective 3, we presented and commented on the main aspects of French and European legislation and regulations in the field of health data, hospital data warehouses and real-world evidence.

Abstract:We provide an overview of the recent trends towards digitalization and large scale data analytics in healthcare. It is expected that these trends are instrumental in the dramatic changes in the way healthcare will be organized in the future. We discuss the recent political initiatives designed to shift care delivery processes from paper to electronic, with the goals of more effective treatments with better outcomes; cost pressure is a major driver of innovation. We describe newly developed networks of healthcare providers, research organizations and commercial vendors to jointly analyze data for the development of decision support systems. We address the trend towards continuous healthcare where health is monitored by wearable and stationary devices; a related development is that patients increasingly assume responsibility for their own health data. Finally we discuss recent initiatives towards a personalized medicine, based on advances in molecular medicine, data management, and data analytics.

Abstract:Background and Objective : The increasing implementation and use of electronic health records over the last few decades has made a significant volume of clinical data being available. Over the past 20 years, hospitals have also adopted and implemented data warehouse technology to facilitate the reuse of administrative and clinical data for research. However, the implementation of clinical data warehouses encounters a set of barriers: ethical, legislative, technical, human and organizational. This paper proposes an overview of difficulties and barriers encountered during a clinical data warehouse (CDW) development and implementation project. Methods We conducted a focus group at the 2023 Medical Informatics Europe Conference and invited professionals involved in the implementation of CDW. These experts described their CDW and the difficulties and barriers they encountered at each phase: (i) launching of the data warehouse project, (ii) implementing the data warehouse and (iii) using a data warehouse in routine operations. They were also asked to propose solutions they were able to implement to address the barriers previously reported. Results After synthesis and consensus, a total of 26 barriers were identified, 10 pertained to tasks, 5 to tools and technologies, 4 to persons, 4 to organization, and 3 to the external environment. To address these challenges, a set of 15 practical recommendations was offered, covering essential aspects such as governance, stakeholder engagement, interdisciplinary collaboration, and external expertise utilization. Conclusions These recommendations serve as a valuable resource for healthcare institutions seeking to establish and optimize CDWs, offering a roadmap for leveraging clinical data for research, quality enhancement, and improved patient care.

Abstract:Objectives: Clinical Data Warehouses (CDW) are the designated infrastructures to enable access and analysis of large quantities of electronic health record data. Building and managing such systems implies extensive "data work" and coordination between multiple stakeholders. Our study focuses on the challenges these stakeholders face when designing, operating, and ensuring the durability of CDWs for research. Materials and methods: We conducted semistructured interviews with 21 professionals working with CDWs from France and Belgium. All interviews were recorded, transcribed verbatim, and coded inductively. Results: Prompted by the AI boom, healthcare institutions launched initiatives to repurpose data they were generating for care without a clear vision of how to generate value. Difficulties in operating CDWs arose quickly, strengthened by the multiplicity and diversity of stakeholders involved and grand discourses on the possibilities of CDWs, disjointed from their actual capabilities. Without proper management of the information flows, stakeholders struggled to build a shared vision. This was evident in our interviewees' contrasting appreciations of what mattered most to ensure data quality. Participants explained they struggled to manage knowledge inside and across institutions, generating knowledge loss, repeated mistakes, and impeding progress locally and nationally. Discussion and conclusion: Management issues strongly affect the deployment and operation of CDWs. This may stem from a simplistic linear vision of how this type of infrastructure operates. CDWs remain promising for research, and their design, implementation, and operation require careful management if they are to be successful. Building on innovation management, complex systems, and organizational learning knowledge will help.

Abstract:Background: Biomedical data warehouses (BDWs) have become an essential tool to facilitate the reuse of health data for both research and decisional applications. Beyond technical issues, the implementation of BDWs requires strong institutional data governance and operational knowledge of the European and national legal framework for the management of research data access and use. Objective: In this paper, we describe the compound process of implementation and the contents of a regional university hospital BDW. Methods: We present the actions and challenges regarding organizational changes, technical architecture, and shared governance that took place to develop the Nantes BDW. We describe the process to access clinical contents, give details about patient data protection, and use examples to illustrate merging clinical insights. More than 68 million textual documents and 543 million pieces of coded information concerning approximately 1.5 million patients admitted to CHUN between 2002 and 2022 can be queried and transformed to be made available to investigators. Since its creation in 2018, 269 projects have benefited from the Nantes BDW. Access to data is organized according to data use and regulatory requirements. Conclusions: Data use is entirely determined by the scientific question posed. It is the vector of legitimacy of data access for secondary use. Enabling access to a BDW is a game changer for research and all operational situations in need of data. Finally, data governance must prevail over technical issues in institution data strategy vis-à-vis care professionals and patients alike.

Abstract:In the era of Industry 4.0, hospitals face numerous obstacles when it comes to embracing digital disruption. One of the major challenges is dealing with big data. With the increasing amount of data being generated in healthcare, hospitals struggle to effectively manage and analyze this vast amount of information. This paper highlights the challenges faced by hospitals in adopting digital disruption in the era of Industry 4.0. Some of the challenges include big data, scalability, data management, security and privacy, standardization, interoperability and regulation, human factors, implementation costs, networking, integrated IT infrastructure, skills, and device/system heterogeneity. The study suggests that immediate implementation of information technology is required to achieve optimal data integration on a national scale. Various recommendations have been formulated, including improving knowledge management of big data analytics and systems, as well as empowering human resources within the organization. The study calls for more research in Healthcare 4.0, addressing key barriers and identifying trends and gaps that require further exploration.

Abstract:Digital health encompasses a wide range of novel digital technologies related to health and medicine. Such technologies rely on recent advances in the collection and analysis of ever increasing amounts of data from both patients and healthy citizens. Along with new opportunities, however, come new ethical and policy challenges. These range from the need to adapt current evidence-based standards, to issues of privacy, oversight, accountability and public trust as well as national and international data governance and management. This review illustrates key issues and challenges facing the rapidly unfolding digital health paradigm and reflects on the impact of big data in medical research and clinical practice both internationally and in Switzerland. It concludes by emphasising five conditions that will be crucial to fulfil in order to foster innovation and fair benefit sharing in digital health.

Abstract:The „true” fungi have been referred to as the KingdomFungi,the KingdomEumyceteae,or theEumycota[1].The fungi are eukaryotic organisms, characterized by: (i) a diversity of microbodies; (ii) cell walls that have a great similarity of architecture; (iii) hyphae that have a major chitin component, extended apically, and divide by centripetal invagination of the plasma membrane; (iv) lomasomes: sponge-like intumescences seen on the inside of the cell wall; (v) complete absence of the Golgi organelle in the terrestrial assemblages (zygomycetes, ascomycetes, and basidiomycetes) and some of the aquatic taxa; and (vi) nuclei in which most, if not all, gene products involved in mitosis probably have higher eukaryotic paramologues but which, in other ways, are exceptional [2]. Fungi are reproducing sexually or asexually, the diploid phase generally short-lived. Fungi parasitize a wide range of plants, animals, and other fungi [3].

Abstract:Existing data stored in a hospital's transactional servers have enormous potential to improve performance measurement and health care quality. Accessing, organizing, and using these data to support research and quality improvement projects are evolving challenges for hospital systems. The authors report development of a clinical data warehouse that they created by importing data from the information systems of three affiliated public hospitals. They describe their methodology; difficulties encountered; responses from administrators, computer specialists, and clinicians; and the steps taken to capture and store patient-level data. The authors provide examples of their use of the clinical data warehouse to monitor antimicrobial resistance, to measure antimicrobial use, to detect hospital-acquired bloodstream infections, to measure the cost of infections, and to detect antimicrobial prescribing errors. In addition, they estimate the amount of time and money saved and the increased precision achieved through the practical application of the data warehouse.

French Health big data experience: The evolution of the national health insurance information system

Abstract:Abstract The French national health insurance information system created in 1999 is one of the largest global data warehouse of medico-administrative data, covering nearly 66 million inhabitants and containing more than 160 billion records, including all outpatient health expenditures. Recent legislation has enlarged the scope of information and access to the data warehouse. Initially created to reimburse patients, the national health insurance data warehouse has developed into a powerful tool for conducting public health research. Public health researchers and epidemiologists have long studied population health determinants, including access to and impact of health care interventions, through hypothesis driven investigations based on experimental (randomized controlled trials) or quasi-experimental designs (e.g. cohort or case control studies). In such studies, efforts are centred on data validity rather than scope or volume. Nowadays the availability of medical claims and electronic health records (eHR) allows users not only to address similar research questions at no or little cost for data collection, but also to extend the scope of research questions to the analysis of care pathways, the evaluation of public health policies, the impact of interventions in real-life conditions. Striding away from the conventional hypothesis driven research paradigm, public health researchers have started exploring the power of applying data mining approaches on eHR. As demonstrated in the omics revolution, data driven approaches are discovery-generating tools with explanatory and predictive power. Could data mining methods applied to eHR deliver similar breakthroughs in Health Services Research? What new knowledge and tools must the public health research community acquire to embrace this new research paradigm? These are the issues that we will discuss, with illustrations, during this presentation.

Abstract:Big data, coupled with the use of advanced analytical approaches, such as artificial intelligence (AI), have the potential to improve medical outcomes and population health. Data that are routinely generated from, for example, electronic medical records and smart devices have become progressively easier and cheaper to collect, process, and analyze. In recent decades, this has prompted a substantial increase in biomedical research efforts outside traditional clinical trial settings. Despite the apparent enthusiasm of researchers, funders, and the media, evidence is scarce for successful implementation of products, algorithms, and services arising that make a real difference to clinical care. This article collection provides concrete examples of how "big data" can be used to advance healthcare and discusses some of the limitations and challenges encountered with this type of research. It primarily focuses on real-world data, such as electronic medical records and genomic medicine, considers new developments in AI and digital health, and discusses ethical considerations and issues related to data sharing. Overall, we remain positive that big data studies and associated new technologies will continue to guide novel, exciting research that will ultimately improve healthcare and medicine-but we are also realistic that concerns remain about privacy, equity, security, and benefit to all.
Good practices for clinical data warehouse implementation: a case study in France

Abstract:Real World Data (RWD) bears great promises to improve the quality of care. However, specific infrastructures and methodologies are required to derive robust knowledge and brings innovations to the patient. Drawing upon the national case study of the 32 French regional and university hospitals governance, we highlight key aspects of modern Clinical Data Warehouses (CDWs): governance, transparency, types of data, data reuse, technical tools, documentation and data quality control processes. Semi-structured interviews as well as a review of reported studies on French CDWs were conducted in a semi-structured manner from March to November 2022. Out of 32 regional and university hospitals in France, 14 have a CDW in production, 5 are experimenting, 5 have a prospective CDW project, 8 did not have any CDW project at the time of writing. The implementation of CDW in France dates from 2011 and accelerated in the late 2020. From this case study, we draw some general guidelines for CDWs. The actual orientation of CDWs towards research requires efforts in governance stabilization, standardization of data schema and development in data quality and data documentation. Particular attention must be paid to the sustainability of the warehouse teams and to the multi-level governance. The transparency of the studies and the tools of transformation of the data must improve to allow successful multi-centric data reuses as well as innovations in routine care.

Big data is made up of a lot of data that can't be handled and stored in the conventional way. This article primarily examines big data deployment strategies in the healthcare industry. The difficulties and resources for implementing big data in the healthcare industry are also hinted to in this research. Additionally, it covers various machine learning algorithms that are helpful in upholding the trade-off between accuracy and efficiency.

Abstract:Health Information Technologies HIT are being deployed world- wide to improve access to individual patient information. Primarily this is through the development of electronic health records EHR and electronic medical records EMR. While the proper collection of this data has reached a high level of maturity, the use and analysis of it is only in its infancy. This data contains information which can potentially improve treatment for the individual patient and for the cohort of patients suffering a similar disease. The data can also provide valuable information for broader research purposes. In this paper we discuss the research contributions we are making in improving the use and analysis of patient data. Our projects include the analysis of physiological data, the extraction of information from multi-modal data types, the linking of data stored in heterogeneous data sources and the semantic integration of data. Through these projects we are providing new ways of using health data to improve health care delivery and provide support for medical research.
