Abstract:Introduction: Lipoma Arborescens is a chronic, slow growing intra articular lesion of benign nature which is commonly seen in the supra patellar region of the knee joint. It is characterised by villous proliferation of the synovium, with the replacement of the subsynovial connective tissue with fat cells. It is not a neoplasm but non-specific reactive response to chronic synovial irritation, due to mechanical or inflammatory insults. We highlight this condition to raise awareness to be kept as a differential diagnosis in knee joint affected by various slow, progressive chronic inflammatory conditions. Case Report: We present case of 51-year female with severe knee swelling since 3 to 4 years with episodes of remission and regression on and off. She was diagnosed with lipoma arborescens after magnetic resonance imaging and confirmed after post operative histology. Conclusion: We in this case study highlight this rare condition with its imaging characteristics and arthroscopic treatment. Keeping in mind that lipoma arborescens though benign in nature and as one of the rare causes for knee swelling, has to be treated to get an optimal outcome.

Abstract:Lipoma arborescence is a rare, reactive disease process which commonly occurs due to chronic inflammation or repeated trauma. A limited number of cases have been documented in the literature and little is known regarding the pathogenesis of this condition. Fortunately, it is a benign condition with no known cases of neoplastic conversion. Here, we present the unique case of a lipoma arborescence occurring in a patient who was one year status post arthroscopic medial meniscectomy.

Abstract:Intermuscular lipomas are rare soft tissue tumors of mesenchymal origin. They are closely correlated with other benign and malign mesenchymal tumors in clinical presentation and imaging, causing problems in diagnosis and treatment. This study examined a rare giant intermuscular lipoma of the hip and its management, allowing clinicians to become familiar with this lesion. Furthermore, the clinical data of 13 patients with pathologically confirmed giant intermuscular lipomas (1 case treated by the authors, the remaining 12 cases reported in the Chinese literature during the period from January, 1992 to May, 2012) were also reviewed and analyzed. Based on the review, intermuscular lipomas were found to be more common in males and usually presented with an asymptomatic mass, with an average patient age of 39.7 years (range, 11-70), while symptom duration was between 1 month and 10 years. The most common tumor origin sites were the thigh and forearm and the option of treatment was surgical excision. The present study showed that it is crucial for the surgeon to consider infiltrating lipomas when dealing with deep soft tissue masses, while complete removal of the lesion was vital to avoid recurrence.
A rare coexistence of a non-dysraphic intradural lipoma and a cyst in the lumbar spine - A case report and literature review

Abstract:Introduction: Spinal intradural lipomas are rare entities, particularly when not associated with spinal dysraphism. Their diagnosis is often delayed due to their slow growth and presentation in dorsal components. Although the association between lipomas and cysts has been previously noted, the coexistence of lipomas with fluid-filled cysts is exceedingly rare. We present a case that highlights the presentation, management, and outcomes of a patient with the unusual coexistence of these two lesions in the lumbar region. Case report: An eight-year-old female presented to our outpatient neurology clinic with complaints of weakness and severe pain in her lower limbs, accompanied by urinary retention and lumbar focal hypertrichosis. Magnetic resonance imaging (MRI) of the spine revealed a lipoma alongside a cyst at the level of the first two lumbar vertebrae. A subtotal resection of both lesions was performed; the cyst was drained, revealing a clear fluid, while the lipoma was partially resected due to significant adhesions to the spine. The patient achieved full recovery within a few days, as confirmed by neurological and radiological evaluations. Clinical discussion: The origin of non-dysraphic lipomas remains unclear, with several congenital theories proposed to explain their formation. While associations with various anomalies and cysts have been documented, the coexistence of intradural fluid-filled cysts is exceptionally uncommon. This case presents several potential explanations for this phenomenon. Typically, lipomas are asymptomatic until they cause signs of cord compression. However, the presence of associated entities can lead to earlier symptom onset. Surgical intervention often involves subtotal resection due to severe tumor adhesions, which can yield satisfactory outcomes despite the presence of residual tumor postoperatively. Conclusion: The occurrence of an intradural fluid-filled cyst in conjunction with a lipoma is rare and may represent an arachnoid cyst or a form of weakened spinal dysraphism linked to the controversial formation of lipomas. Illustrating this association is crucial, as the adhesions and indistinct boundaries between these lesions and the spinal cord complicate complete excision, necessitating subtotal resection, which appears to favor better outcomes.

Abstract:This case report presents a 64-year-old male with a giant intramuscular lipoma on the right lumbar region's latissimus dorsi muscle. The patient presented with painless swelling, which gradually increased over six years. Magnetic resonance imaging (MRI) confirmed the presence of the lipoma, prompting surgical intervention. The surgical procedure involved meticulous dissection and complete excision of the tumor. Histopathological examination validated the diagnosis. Comparative analyses with similar cases highlighted variations in surgical approaches and the challenges in managing intramuscular lipomas. This case emphasizes the importance of considering intramuscular lipomas in soft tissue mass differentials and the significance of comprehensive management strategies for optimal patient outcomes.
An analysis of imaging characteristics of X-ray, computed tomography and magnetic resonance imaging of intraosseous lipoma

Abstract:Objective To investigate the imaging characteristics of X-ray, computed tomography ( CT ) and magnetic resonance imaging ( MRI ) of intraosseous lipoma and to improve the accuracy of diagnosis and differential diagnosis.Methods The imaging characteristics of X-ray, CT and MRI in 38 cases of intraosseous lipoma conifrmed by percutaneous puncture biopsy and pathological test were analyzed retrospectively, which was compared with pathological outcomes.Results Intraosseous lipoma was noticed in the calcaneus (n=11 ), in the femur (n=8 ), in the tibia (n=5 ), in the ilium (n=6 ), in the humerus (n=6 ), in the radius ( n=1 ) and in the talus (n=1 ) respectively. There were 38 cases of single cystic type in all. Based on the X-ray and CT, calciifcation was found in 11 cases, bone crest in 19 cases, double sclerotic band in 4 cases, sclerotic border in 21 cases and bone expansion in 16 cases. Lipo density could be determined by CT in all the cases. The CT value was -40HU - -133HU. The MRI showed there was fat component in all the cases. Among them, cyst formation was detected in 9 cases, and inhomogeneous signal in 20 cases. According to Milgram’s stage, 4 cases were at stage I, 4 cases at stage II and 30 cases at stage III. Conclusions The X-ray and CT can show the lesion site, morphology, calcification, bone crest, sclerotic border, bone expansion and with or without boundary. The MRI can make clear fat component, fat necrosis, cyst formation, myxoid change, inlfammation, range and border of the lesion. The X-ray, CT and MRI relfect imaging characteristics of intraosseous lipoma from different aspects.
Imaging review of lipomatous musculoskeletal lesions

Abstract:Lipomatous lesions are common musculoskeletal lesions that can arise within the soft tissues, bone, neurovascular structures, and synovium. The majority of these lesions are benign, and many of the benign lesions can be diagnosed by radiologic evaluation. However, radiologic differences between benign and malignant lipomatous lesions may be subtle and pathologic correlation is often needed. The use of sonography, computed tomography (CT), and magnetic resonance imaging (MRI) is useful not only in portraying fat within the lesion, but also for evaluating the presence and extent of soft tissue components. Lipomas make up most soft tissue lipomatous lesions, but careful evaluation must be performed to distinguish these lesions from a low-grade liposarcoma. In addition to the imaging appearance, the location of the lesion and the patient demographics can be utilized to help diagnose other soft tissue lipomatous lesions, such as elastofibroma dorsi, angiolipoma, lipoblastoma, and hibernoma. Osseous lipomatous lesions such as a parosteal lipoma and intraosseous lipoma occur less commonly as their soft tissue counterpart, but are also benign. Neurovascular and synovial lipomatous lesions are much rarer lesions but demonstrate more classic radiologic findings, particularly on MRI. A review of the clinical, radiologic, and pathologic characteristics of these lesions is presented.

Abstract:A 21 year old female patient presented with mild left ankle pain for 2 months. Left ankle radiograph was performed, showing a lucent lesion with narrow zone of transition at anterior body of the calcaneus. Further evaluation with MRI showed T1 hypointense and T2 hyperintense signals within said lesion, with fluid-fluid level. The working diagnosis was aneurysmal bone cyst. Follow-up MRI performed 3 years later showed predominant macroscopic fat within the lesion. This case demonstrates composition change of intraosseous lipoma over time.

Abstract:Background: Cardiac lipomas are rare benign tumors of the heart. They are usually asymptomatic and are thus most often diagnosed on autopsies. Symptoms, when present, depend upon the location within the heart. Typical locations are the endocardium of the right atrium and the left ventricle. Diagnostic modality of choice is cardiac MRI. Treatment guidelines have not yet been established due to the very low prevalence of these tumors and are thus guided by the patient's symptomatology. Case presentation: We describe a case of an invasive cardiac lipoma, wherein the initial symptom of the patient was shortness of breath. Although the echocardiogram visualized the tumor in the right atrium, a cardiac MRI was performed for better tissue characterization. The MRI revealed a large, fat containing, septated mass in the right atrium with invasion into the inter-atrial septum and inferior cavoatrial junction. There was also invasion of the coronary sinus along the inferior and left lateral aspect of the posterior atrioventricular groove. Although the mass appeared to represent a lipoma by imaging characteristics, the unusual extension into the coronary sinus led to consideration of a low-grade liposarcoma in the differential. Thus a pre-operative biopsy was performed along with MDM2 gene amplification to rule out a liposarcoma preceding surgical excision. Conclusion: Cardiac lipomas are well-characterized on cardiac MRI, which is the diagnostic modality of choice. Typical locations are the right atrium and the left ventricle. However, in those with atypical features such as invasion of the coronary sinus, pre-operative biopsy for histopathologic confirmation is imperative to exclude well-differentiated liposarcoma. Our patient with a simple lipoma underwent partial resection to relieve symptoms. We discuss prognosis and treatment of symptomatic cardiac lipomas.

Abstract:Despite its characteristically low sensitivity, sputum smear microscopy remains the standard for diagnosing tuberculosis (TB) in resource-poor countries. In an attempt to develop an alternative or adjunct to microscopy, researchers have recently examined the ability of pouched rats to detect TB-positive human sputum samples and the microbiological variables that affect their detection. Ten published studies, reviewed herein, suggest that the rats are able to detect the specific odor of Mycobacterium tuberculosis, which causes TB, and can substantially increase new-case detections when used for second-line TB screening following microscopy. Further research is needed to ascertain the rats’ ability to detect TB in children and in HIV-positive patients, to detect TB when used for first-line screening, and to be useful in broad-scale applications where cost-effectiveness is a major consideration.

Abstract:Lipomas are the most common soft tissue tumours, accounting for nearly 50% of all soft tissue neoplasms. Among the types of lipomas, intermuscular lipomas are rare. We report the case of a 46-year-old man with a deep-seated lipoma located between the trapezius and splenius capitis muscles in the posterior neck triangle. A successful surgical excision was performed. This case highlights challenges in diagnosis, surgical management, and the importance of complete excision in minimizing the risk of recurrence. This type of lipoma remains relatively understudied; therefore, further studies are needed to increase awareness of its incidence, clinical presentation, diagnostic evaluation, and optimal management.
The Diagnosis and Arthroscopic Treatment of Angioleiomyoma Presenting Loose Body in the Knee Joint: Two Case Reports

Abstract:Background Angioleiomyoma is a very rare benign solitary soft tissue neoplasm originating from smooth muscle layer of blood vessels. The tumor is usually located in the subcutis or the superficial fasciae, but less often in the deep fasciae, especially rare in the knee joint cavity. Diagnosis is frequently delayed or misdiagnosed as loose body or anterior knee pain because of its rare occurrence and poor awareness of physicians. Few studies have presented intra-articular angioleiomyoma and such cases become rarer and more difficult to diagnose when it presents as loose body. Case presentation Two patients, a middle-aged man and an old woman, presented to our outpatient clinic with persistent anterior knee pain and both of them suffered from a solitary mass in the right knee that had slowly enlarged. One of two patients showed negative in the routine radiographic imaging and the other showed a “loose body” beside the lateral femoral condyle in the knee. MRI showed both a well-demarcated intra-articular mass of isointense signal to muscle on T1-weighted images and heterogeneous intensity on T2-weighted images. Their tumors were excised under arthroscopy finally, with the pathological results revealed vascular leiomyomas. They both recovered well with pain free after operation and no signs of recurrence were seen at the 7-year follow-up. Conclusions This case report illustrates the atypical locations of angioleiomyoma in the knee joint should arouse our attention and be included in the differential diagnosis of nodular lesions mimicking loose bodies.

Abstract:Introduction: Lipomas are the most common benign mesenchymal tumors which can be found in any part of the body. Nevertheless, their etiology and pathogenesis remain unknown. It is hypothesized that some of these lesions could result from an acute or chronic trauma. Patients and methods: We report a case of a 54-year-old man presenting a perineal lipoma which volume grew rapidly after he fell on his buttock, in the context of inaugural epileptic seizure. Pelvic MRI showed a voluminous fatty mass, measuring 6.6 × 5 × 9 cm without any signs of local invasion. Furthermore, we review the latest research on lipomas originating from traumatic lesion. Results: The mass was completely excised in one block under general anesthaesia, using an elliptical incision and a deep dissection. We did not close the skin incision in view of the cutaneous defect. Post-operative recovery was uneventful and the patient was discharged from hospital two days after the operation. Histopathology indicated a reorganised lipoma with no evidence of malignancy. Conclusion: Perineal lipomas are extremely rare, pathological examination of imaging guided biopsies are needed to exclude malignancy especially a well-differentiated liposarcoma. MRI remains the first option and radical surgical excision is the gold standard treatment.

Abstract:Abstract Synovial lipomatosis is a rare condition characterized by adipocyte proliferation within joint synovial tissue. It most commonly affects the knee and is typically intra-articular. Only 5 published case reports describe extra-articular synovial lipomatosis of the wrist. We present a case of a sexagenarian patient seen for his wrist arthropathy. His x-ray revealed pan-wrist arthritis and inflammatory soft tissue swelling. The patient was slated for a wrist fusion and Darrach procedure. Following the dorsal skin incision in the operating room, an unusual adipose mass was identified infiltrating all extensor compartments: midcarpal, radiocarpal, and distal radioulnar joints. The mass was excised and sent to pathology prior to proceeding with the slated surgery. Synovial lipomatosis was diagnosed postoperatively based on histopathology. Six weeks postoperatively, the wrist fusion had healed clinically and radiographically, and his pain had improved. There was no evidence of recurrence. Synovial lipomatosis is a rare entity that may imitate multiple other pathologies. It is possible that synovial lipomatosis may represent a secondary occurrence following degenerative articular disease or trauma in older patients. This is the first case report to date describing synovial lipomatosis of the wrist with extra-articular extension in the setting of pan-carpal wrist arthritis.

Abstract:Synovial hemangiomas are rare benign vascular anomalies surrounded by a synovial lining and were first described by Bouchut in 1856. These neoplasms can develop in the intra-articular region, resulting in effusions and knee pain. However, their cause remains unknown. Prompt diagnosis and intervention are critical to prevent chondral damage. Histopathological examination is used to achieve the diagnosis, which is often delayed because of a lack of specific clinical signs. This report describes a unique case in which a painful infrapatellar mass was diagnosed as a synovial hemangioma. The absence of typical magnetic resonance imaging (MRI) findings highlights the importance of arthroscopic excision for diagnosis and symptom relief.

Abstract:OBJECTIVE Paired DNA samples from 55 primary uterine cervical carcinomas and normal bloods were studied for chromosomal allelic loss (loss of heterozygosity; LOH) of the retinoblastoma (Rb) and p53 gene loci by polymerase chain reaction (PCR) or PCR-restriction fragment length polymorphism analysis. All the study samples contained at least one of the oncogenic human papillomavirus (HPV) type 16 and/or 18 sequences. And the relationships between allelic losses of these genes and conventional clinicopathological parameters were evaluated. METHODS In order to detect LOH of the Rb gene in cervical cancers, we analyzed four polymorphic intronic sites (intron 1, 17, 20, and 25) of the Rb gene and one additional microsatellite near the Rb locus (D13S118). For detection of the LOH in p53, three intragenic polymorphisms (exon 1, exon 4, intron 6) and one microsatellite distal to the p53 gene (D17S5) were examined. RESULTS By analyzing this system, we could increase the heterozygosity of the Rb and p53 loci up to 0.91 and 1, respectively. The observed allelic loss rates of the Rb and p53 loci in informative cases were 14% (7/50) and 5.5% (3/55), respectively. The patients with LOH at the D13S118 locus also had the allelic loss of the Rb gene, whereas only one of the four patients with LOH at the D17S5 locus showed a concomittant allelic loss of the p53 gene. The frequency of cervical cancer with one LOH at the Rb or p53 loci was 20% (11/55). No shifted bands were observed in the PCR-single-strand conformation polymorphism analysis of the p53 gene. The LOH of the Rb or p53 gene was not significantly associated with other parameters including clinical stage, histological type, degree of differentiation, status of HPV infection, and p53 gene mutation. CONCLUSION Concerning the results above, we conclude that the allelic imbalance of the Rb or p53 gene itself is not implicated as a major contributing factor in the malignant transformation or the tumor progression in HPV-positive uterine cervical cancers.
Intramuscular lipomas posing diagnostic and pre-operative counselling challenges in a low-resource setting: A case series

Abstract:Introduction: Intramuscular lipomas (IMLs) are uncommon primary adipose tissue tumours deep within the muscle. A high likelihood of misdiagnosing them as other benign and malignant masses necessitates imaging studies to confirm the diagnosis and plan treatment. Ultrasonography is useful but CT and MRI provide a more accurate diagnosis. While diagnostic tests are suitable, they may not always be accessible or affordable in low-resource settings. We present three cases of IMLs that emphasise the difficulties posed by limited resources and the significance of a comprehensive medical history and physical examination in low-resource settings. Presentation of cases: The patients included a 57-year-old male with a distal right thigh mass, a 65-year-old female with a proximal right thigh mass, and a 60-year-old female with a mass at the left scapular area. The three patients underwent surgical excision and had an uneventful postoperative course, with no reported recurrence during their ongoing follow-up. Discussion: The management of IMLs is not complicated if the requisite resources are available. Conversely, in low-resource settings with limited diagnostic facilities and human expertise, management may take a challenging path. Patient 1, despite undergoing diagnostic tests confirming IML, initially declined treatment due to challenges with pre-operative counselling. Patients 2 and 3 lacked health insurance and could not afford diagnostic imaging tests. Conclusion: Healthcare professionals in low-resource settings should familiarise themselves with the clinical characteristics and pathology of IMLs to minimise misdiagnosis and ensure appropriate counselling is provided to patients. IMLs are slow-growing mostly asymptomatic benign swelling. On physical examination, they are usually non-tender, soft, masses, not fixed to the bed or overlying tissue. The overlying skin is normal and lymphadenopathy is absent.

Previous studies of inherited factor H deficiency revealed single amino acid substitutions at conserved cysteine residues, on one allele arginine for cysteine 518 (C518R) and on the other tyrosine for cysteine 941 (C941Y) (Ault, B. H., Schmidt, B. Z., Fowler, N. L., Kashtan, C. E., Ahmed, A. E., Vogt, B. A., and Colten, H. R. (1997) J. Biol. Chem. 272, 25168–25175). To ascertain if the phenotype, impaired secretion of factor H, is due to the C518R substitution or the C941Y substitution and to ascertain the mechanism by which secretion is impaired, we studied COS-1 and HepG2 cells transfected with wild type and several mutant factor H molecules. The results showed markedly impaired secretion of both C518R and C941Y factor H as well as that of factor H molecules bearing alanine or arginine substitutions at the Cys518–Cys546 disulfide bond (C518A, C546A, C546R, C518A-C546A). In each case, mutant factor H was retained in the endoplasmic reticulum and degraded relatively slowly as compared with most other mutant secretory and membrane proteins that are retained in the endoplasmic reticulum. These data indicate that impaired secretion of the naturally occurring C518R and C941Y mutant factor H proteins is due to disruption of framework-specific disulfide bonds in factor H short consensus repeat modules.
