Abstract:ABSTRACT Aim: The aim of the study was to know the clinical and radiographic outcomes of regenerative endodontic procedures of immature versus mature in young permanent incisors with apical periodontitis. Materials and Methods: Twenty non-vital, young permanent incisors with traumatic exposure between the age of 7 and 13 years were taken. Patients were divided into two groups: Group I – immature (number of teeth included = 10) and Group II – mature (number of teeth included = 10). The procedure was performed by the blood clot method as a scaffold. Biodentine (about 2–3 mm) was placed as a capping material at the cementoenamel junction. Evaluation was based on the clinical (pain, sinus, tender on percussion, swelling, mobility, intact restoration) and radiographical (Nolla stage, thickening of walls, lamina dura continuity, calcification of pulp chamber and canal, periapical healing, PAI score) parameters. Results: All 19 teeth were clinically asymptomatic after the 12-month follow-up. Radiographically, the percentage of continuous root development (in the immature group) was 100% at 12 months ( P = 0.000**). Radiography revealed a reduction in the PAI scores at 12 months in the immature and mature groups. None of the treated teeth responded to the cold test after a 12-month follow-up. One patient did not come for intervention in the mature group. All the statistical tests (Chi-square, Mann–Whitney U -test, and Fisher’s exact test) were performed at a 5% significance level. Conclusion: Outcomes did not differ significantly between immature and mature groups.
Time‐Dependent Morphological Changes in Traumatic Immature Teeth With Necrotic Pulps Following Regenerative Endodontic Treatment: A Retrospective Study

Abstract:Background/Aim Regenerative endodontic treatment is a promising approach for healing periapical lesions and continuous root maturation. Although previous studies have reported its outcomes, the dynamics of morphological changes over time remain unclear. Therefore, this study aimed to evaluate changes in the periapical status and root dimensions over a 60‐month follow‐up period. Materials and Methods The follow‐up duration, periapical status changes, calcific barrier formation, degree of apical closure and radiographic root area changes were compared with those of the last follow‐up in this retrospective study. Radiographic root area changes were calculated as the difference between the total root and total canal areas. Results Fifty‐eight patients (81 teeth) underwent regenerative endodontic treatment during the study period, of whom 32 patients (36 teeth, 62%) were included. The survival and success rates of the treated teeth were 100% and 94.4%, respectively. All teeth developed a calcific bridge in the cervical third of the root canal, indicating the presence of vital tissue. Apical narrowing (partial or total) was observed in 75% of the cases. The root maturation stage affected the percentage increase in the radiographic root area. Teeth in Cvek stages II–III showed a higher radiographic root area increase than more mature teeth. All tooth radiographic root areas increased significantly in the initial 20 months of the treatment and moderately thereafter. Conclusions Regenerative endodontic treatment is a safe approach for traumatised immature teeth. The presence of a radiographic calcified bridge may be an early indication of treatment success. The main complete tooth morphological changes occur after approximately 20 months posttreatment. These findings may help clinicians better understand the time‐dependent changes in the root morphology after treatment, improve the follow‐up schedule and predict the progress of healing during follow‐up visits.

Abstract:Objective: to compare different protocols available for regenerative endodontic. Methodology: articles published between 2001 and 2021 were searched on the database PubMed using the following keywords: “pulp revascularization” or “pulp revitalization” or “apexification”. Inclusion criteria were studies on humans, patients with immature permanent teeth with necrosis or irreversible pulpitis, a follow-up of at least 12 months, clearly described protocols and in English language. Results: The search produced 944 results. After applying the inclusion and exclusion criteria, only 35 were selected. 11 articles were added following a bibliographic review of the selected articles and a total of 46 cases were included. The success rate outcome for regenerative endodontic was of 80% observed in a total of 278 teeth. During irrigation, NaOCL was used in most of the cases with concentration varying from 0.5% to 6% and TAP has been the most used intracanal medication (32.7%). Periapical healing and resolution of the symptoms were observed in 95.7%, while apexogenesis was observed in 80.2%. Increase in root length reach 60%, root thickness 64.8% and apical closure 61,9%. Pulp vitality was found in 7.2% of the cases and discoloration in 29.5%. Conclusion: Regenerative endodontic showed signs of apexogenesis in most cases, and a pronounced increase in root thickness, root length and closure of the apex. Due to a high number of protocols and their heterogenicity, the superiority of one over the others cannot be established. More randomized clinical trials are needed to determine the most effective protocol.
Three-dimensional qualitative and quantitative analyses of the effect of periradicular lesions on the outcome of regenerative endodontic procedures: A prospective clinical study

Abstract:Objectives: Qualitative and quantitative evaluation of the outcomes of regenerative endodontic procedure (REP) on human immature necrotic teeth with apical periodontitis using cone-beam computed tomography (CBCT) MATERIALS AND METHODS: Immature permanent teeth (n = 50) with necrotic pulp and periradicular pathosis were treated with a cell-homing concept-based REP. Following the procedure, a limited field-of-view CBCT scan was obtained. At each recall session (6, 12, 18 months), clinical tests were performed, and a digital periapical radiograph was taken. When significant radiographic changes were evident in the follow-up, a final CBCT scan was taken for qualitative and quantitative assessment. These initial and follow-up CBCT scans were assessed for quantification of changes in root length, pulp space diameter and periradicular lesion size. The data were statistically analysed using t test, one-way ANOVA, post hoc test and paired t test (P = 0.05). Results: Of the teeth, 94.6% were clinically successful based on the lack or regression of signs and symptoms after 48 months follow-up. REP resulted in a statistically significant increase in root length, decrease in pulp space diameter and periradicular radiolucency (P < 0.05). CBCT images illustrated various patterns of root maturation including an increased thickening of the canal walls and continued root maturation (37.1%), continued root development with the apical foramen remaining open (57.1%), severe calcification (obliteration) of the canal space (2.9%) and hard tissue barrier formation in the canal space between the coronal plug and the root apex (2.9%). Conclusion: This study highlighted that the expected outcome of radiographic root development was less predictable when immature permanent teeth with periradicular pathosis were treated with REP. Clinical relevance: The size and extent (expansion/destruction of the cortical plate) of periradicular lesions and abscesses influence the outcome of REP. These factors must be taken into consideration during treatment planning.
A 1-year Clinical and Radiographic Assessment of Regenerative Endodontic Therapy for Necrotic Primary Molars: A Randomized controlled Trial

Abstract:Background: Preservation of necrotic primary teeth is important. Pulpectomy is the gold standard treatment in this situation. Reinfection is the main cause of failure in pulpectomy. The application of regenerative endodontic therapy in mature teeth has the rationale of restoring dental-pulp-like tissue and preventing reinfection. Aims and objectives: The current study was designed to clinically and radiographically assess regenerative endodontics therapy in necrotic primary molars in comparison to zinc oxide eugenol (ZOE) pulpectomy. Materials and methods: A double-blinded randomized controlled trial with three parallel arms and a 1:1:1 allocation ratio was conducted. A total of 54 necrotic primary molars in 39 healthy children aged 4-7 years old were randomly allocated as follows group I-control group, in which ZOE pulpectomy was performed. Group II and III-experimental groups, in which regenerative endodontic therapy (RET) was performed. Modified triple antibiotic paste (mTAP) and Metapex™ were used as intracanal medicaments in groups II and III, respectively. Clinical and radiographic assessments were recorded at baseline, 6 and 12 months. Chi-squared and Fisher's exact tests were used to compare the qualitative data, while Friedman's test was used to study the changes by time within each group. Results: Nonstatistically significant differences were reported between the three groups at the 6 and 12 months follow-ups regarding the clinical or radiographic assessment (p-value = 0.327 and effect size = 0.22), (p-value = 0.055 and effect size = 0.118), respectively. Conclusion: Regenerative endodontic therapy (RET) yielded comparable results to pulpectomy. However, the use of 5 mg/mL mTAP in RET represented the highest level of clinical as well as radiographic insignificant failure. Clinical significance: Regenerative endodontic therapy (RET) provides an acceptable biological alternative to pulpectomy. Abstract:Introduction The primary aim of this study was to compare the radiographic changes of immature incisors with periapical radiolucency after treatment with platelet-rich fibrin (PRF) and concentrated growth factor (CGF) platelet concentrate scaffolds as well as assessment of the clinical success rate over 12 months. The secondary aim was to monitor the radiographic changes in terms of reduction of periapical lesion diameter (PALD), root dentine thickness (RDT), root length (RL), and apical foramen width (AFW). The tertiary aim was to assess and pulp responses, after 12 months Methods Fifty six children with seventy necrotic , single-rooted maxillary incisors with periapical radiolucency were treated with either CGF or PRF scaffolds (35 teeth per group). Two patients with four teeth (two teeth in each group) failed to attain the follow-up recalls. Radiographic changes in terms of reduction of periapical lesion diameter (PALD), root dentine thickness (RDT), root length (RL), and apical foramen width (AFW) were monitored using a two-dimensional radiograph and CBCT scan. The clinical performance of teeth receiving both scaffolds was assessed after 6 and 12 months. Categorical and continuous data were analyzed using the chi-square test and the t-test , respectively. The time and group effects on the means of different radiographic dimensions were tested using the general linear model (GLM). Bland-Altman plots were used to assess the level of agreement between the 2D radiographs and CBCT. The level of significance was defined at 0.05 and a 95% CI . Results The means of PALD and RL showed significant enhancement in the CGF group compared to the PRF group ( p < 0.05) . While the difference between the two scaffolds in terms of RDT and AFW was not significant ( p > .05) . The findings of the 2D radiograph and CBCT were consistent. Clinically, both scaffold success rates were similar (93.9%) over the follow-up intervals. The influence of study independent variables had no significant effect on the success of the REPs outcome ( p > 0.05). There was no significant difference in the positive pulp responses to the thermal and electric pulp tests after one year of treatment ( p > 0.05) . Conclusions According to the short-term follow-up, PRF and CGF were successful in treating immature teeth with periapical radiolucency by regenerative endodontics. Both scaffold systems induced periapical healing and root lengthening with significant superiority of CGF.

Regenerative Endodontic Treatment versus Apical Plug in Immature Teeth: Three-Year Follow-Up

Abstract:This report compares and evaluates the treatment outcomes of regenerative endodontic treatment and apical plug as two accepted treatment protocols in a pair of necrotic immature maxillary central incisors of a 12-year old female. The patient was referred complaining of a dull pain and swelling in her upper lip area. She had a history of trauma to the anterior maxilla two years earlier. Both teeth were clinically diagnosed with pulp necrosis and periapical radiographs revealed that separate periapical radiolucent lesions surrounded the immature apices of both teeth. The left and right incisors were treated with apical plug and regenerative endodontic treatment, respectively, using calcium-enriched mixture (CEM) cement. The patient was followed-up for three years. During this period, both teeth were clinically asymptomatic and showed complete radiographic healing of the periapical lesions. The right central incisor showed root development. No tooth discoloration was evident. Apexification by apical plug placement and pulp regeneration are both reliable treatments for immature non-vital teeth. In order to choose the right treatment the advantages of either technique should be weighed against its drawbacks. CEM cement can be successfully applied for both purposes. This biomaterial causes less discoloration of the tooth.
Studies on human placental carbon monoxide-binding cytochromes.

Abstract:Studies with marker enzymes, difference spectra, and electron microscopy have confirmed the presence of a carbon monoxide-binding cytochrome in both the mitochondria and endoplasmic reticulum of human placentas. The position of the absorption maximum of the CO complex (450-form) was dependent on the method of spectral analysis utilized but was independent of aryl hydrocarbon hydroxylase activity and gestational age. An altered form of the cytochrome ("428-form) exhibited absorption maxima which varied between 421 and 434 nm. The rate of conversion of the 450-form to the "428"-form could be retarded markedly if buffered solutions containing 10-4 M dithiothreitol and 20% glycerol were utilized for the resuspension of placental particulate fractions. The cytochrome could be solubilized with sodium cholate and partially purified without significant conversion to the "428"-form. The pigment was detectable both at term and during the first two trimesters of pregnancy. Preliminary observations indicated a possible inverse correlation between the cytochrome P-450 content and aryl hydrocarbon hydroxylase activities observed in placental particulate fractions. No difference in the positions of the absorption maxima could be observed in placentas with very high vs . very low aryl hydrocarbon hydroxylase activities. Differences between rat hepatic microsomal cytochrome P-450 and the placental cytochrome were noted with respect to ethyl isocyanide-, n -octylamine-, and CO-binding spectra, rates of temperature-dependent conversion to the "428"-form, and the effects of p -chloromecuribenzoate on the CO difference spectra.
Regenerative Endodontic Procedures of Immature Permanent Premolars with Periapical Lesions: A Report of Two Cases Using Two Different Materials, 18-Month Follow-Up

Abstract:Regenerative endodontic procedure is an emerging alternative to traditional therapies for immature teeth with necrotic pulp with or without periapical lesions. This innovative approach, also known as revitalization procedures, is aimed at enhancing canal wall thickness, stimulating root lengthening, and promoting apical closure. The regenerative endodontic procedures involve minimally invasive cleaning to preserve stem cells, stimulation of bleeding and clot formation within the canal, and the use of biomaterials to stimulate differentiation. This method is the first choice in biologically based treatments for immature permanent teeth. We present two successful clinical cases in which regenerative endodontic procedures were performed on permanent premolars with necrotic pulp with symptomatic apical periodontitis and chronic apical abscess due to dens evaginatus. The same procedure was employed for both cases, utilizing two differential materials: ProRoot MTA (Dentsply Tulsa Dental Specialities, USA) and Biodentine™ (Septodont, Saint-Maur-des-Fossés, France). Both cases exhibited positive clinical and radiographic outcomes after an 18-month follow-up period including periapical healing, increased dentin thickness, root lengthening, and apical closure.

Abstract:Primary hyperparathyroidism is a common disease and many patients experience symptoms and demonstrate complications of their hypercalcaemia; surgical intervention in the form of cervical exploration is now being implemented with increasing frequency. In a high proportion (80-90 per cent) of individuals the underlying pathological lesion is a solitary parathyroid adenoma. In the remaining patients multiglandular disease is present. The standard operative strategy has been bilateral neck exploration, attempted identification of all four parathyroids, and removal of obviously enlarged glands. This approach has resulted in unnecessary surgical exploration of the contralateral side of the neck in the great majority of patients whose hyperparathyroidism was caused by a solitary adenoma. Against this background, unilateral cervical exploration for patients with a single parathyroid adenoma represents an attractive surgical option, if the side of the solitary tumour can be identified; this may occur either by chance at operation (i.e. on the first side exposed) or before surgery using ultrasonographic or scintigraphic localization techniques. The philosophy of unilateral parathyroid exploration was first expounded by Roth and colleagues’ and developed by Tibblin and associates’. Both of these groups practised blind exploration of the neck with termination of the operation if an adenoma was located on the side initially explored. Compared with patients who underwent bilateral cervical exploration, Tibblin et al. found that those who had a unilateral operation experienced less hypocalcaemia after surgery and had a significantly shorter mean operation time’. More important, no patient who underwent unilateral exploration experienced recurrent hypercalcaemia during a follow-up of between 2 and 7 years. The availability of non-invasive localization techniques, such as ultrasonography and, particularly, thallium-technetium isotope subtraction scanning, now allows accurate lateralization of a significant proportion of solitary parathyroid adenomas before operation. In this hospital thallium-technetium scanning has been used routinely since 1983 for patients who are about to undergo neck exploration for primary hyperparathyroidi~m~. After experience with the investigation developed, and with an awareness of the very high incidence of single adenoma in this disease, a prospective study of scan-directed unilateral parathyroid exploration for those patients who had a positive scan was started. Such a surgical approach produced encouraging results in a small series of patients reported by Biller and colleagues4 in 1986. Since 1985 the surgical strategy, for patients coming to first-time cervical exploration for primary hyperparathyroidism and who have an isotope scan showing a single ‘hot-spot’, has involved a scan-directed unilateral operation with removal of the parathyroid adenoma and biopsy of the ipsilateral normal gland. Bilateral cervical exploration is performed if: ( 1 ) the isotope scan fails to demonstrate an adenoma; ( 2 ) the scan shows more than one ‘hot-spot’; ( 3 ) two enlarged parathyroids are encountered on the side initially explored; or (4 ) familial hyperparathyroidism or multiple endocrine neoplasia is suspected. If an adenoma is not located on the side suggested by the scan, exploration of the contralateral side of the neck is, of course, carried out. This policy has resulted in a significant reduction in operating time for those patients submitted to unilateral exploration (mean 26 min) and, critically, leaves them no more vulnerable to persistent or recurrent hypercalcaemia than those undergoing standard bilateral cervical exploration5. It is acknowledged that the isotope subtraction scan will prove to be of positive help in guiding the surgeon to the site of the tumour in only 55-60 per cent of patients with solitary parathyroid adenoma, but this is not a reason for failing to use the scan information for those individuals in whom the investigation is positive. The logic of a scan-directed but routine bilateral cervical exploration for patients with a single parathyroid adenoma is difficult to follow6. The advantages of unilateral parathyroid exploration for solitary adenoma include not only a reduced operating time without jeopardizing the potential for cure, but also elimination of the risk of recurrent laryngeal nerve and parathyroid injury on the unexplored side and, possibly, a consequent reduction in the incidence of hypocalcaemia after operation. Comparative Evaluation of Platelet-rich Fibrin and Treated Dentin Matrix in Regenerative Endodontic Treatment of Nonvital Immature Permanent Teeth: A Randomized Clinical Trial

Abstract:Aim: Clinical and radiographic evaluation of the efficacy of platelet-rich fibrin (PRF) and treated dentin matrix (TDM) in regenerative endodontic treatment and periapical healing of nonvital immature permanent teeth with chronic apical periodontitis. Materials and methods: Twenty-four children aged between 7 and 11 years, each presenting with a nonvital immature permanent upper central incisor, were selected. They were randomly allocated into two groups (n = 12), group I (PRF) and group II (TDM). Baseline clinical findings were recorded, and preoperative cone-beam computed tomography (CBCT) was taken. Follow-up was done clinically for 15 months at 3-month intervals (3, 6, 9, 12, and 15 months), and CBCT was taken at the end of the 15-month follow-up. Root length, apical diameter, radiographic root area (RRA), and size of the periapical lesion were quantitively assessed at the end of follow-up period and compared to the preoperative CBCT. Results: Clinical success was 100% in both groups by the end of the follow-up period. Radiographically, after a 15-month follow-up, there was a significant increase in root length and RRA, and there was also a significant reduction in apical diameter and lesion size within each group (p < 0.05). However, there was no statistically significant difference between both groups regarding the mean percentage of increase in root length and mean percentage of reduction of apical diameter (p > 0.05). On the other hand, PRF showed more increase in RRA and more reduction in lesion size, with a statistically significant difference between both groups (p < 0.05). Conclusion: Both PRF and TDM were clinically successful. Platelet-rich fibrin showed better radiographic outcomes and periapical healing. Clinical significance: Platelet-rich fibrin is a viable scaffold to aid further root development and resolution of periapical lesions of nonvital immature permanent teeth. Further studies with different forms of TDM are needed to assess the efficacy of TDM in regenerative endodontic treatment of nonvital immature permanent teeth. [A new variety of acute non-promyelocytic leukemia with t(15;17)].

Abstract:Three cases of a new variety of acute leukemia have been reported. The main features were: hyperleukocytosis made of large-sized blasts with a double shaped nucleus, few or no granulations in the cytoplasm, and in a few cell faggots or unique Auer rods; mycloperoxydase reaction was positive. This feature was associated with disseminated intravascular coagulation syndrome and t(15;17)(q22;q21) translocation in the majority of mitoses.

Abstract:Introduction: Studies on the clinical outcomes and prognostic factors of regenerative endodontic procedures (REPs) in a large population and long-term recall period have been limited. Therefore, the aims of this study were to evaluate treatment outcomes and prognostic factors of REPs. Methods: Immature permanent teeth treated with REPs with a minimum one-year follow-up period were included. Treatment outcomes (functional retention, healed rate, root development and sensibility test response) and any prognostic factors were analyzed with multivariable Cox regression, linear regression, and risk-ratio regression. Results: One-hundred-twenty REPs teeth with a mean 41.7-month recall period were included with a functional retention rate of 97.5%. The healed, healing and diseased rates of REPs were 80%, 9.2%, and 10.8%, respectively. Significant prognostic factors for healed were age (<12 years old) and root development stage (stages 4 and 5). Changes in the apical diameter, root length, root width, and radiographic root area after REPs were 56.8%, 8.3%, 23.2%, and 21.7%, respectively. A significant prognostic factor for continued root development was etiology of pulpal disease (from caries or dens evaginatus). The sensibility test response rate was 41.7% with significant positive factors of EDTA irrigation and capping material level below the cemento-enamel junction. Conclusion: REPs demonstrated high functional retention and healed rates. Patients <12 years old presented a higher healed rate and continued root development (excluding root length). Dental caries or anomalies had higher continued root development compared with dental trauma. The sensibility test response was related to EDTA irrigation and low-level capping material.
Prognostic Factors Affecting the Outcome of Surgical Root Canal Treatment—A Retrospective Cone-Beam Computed Tomography Cohort Study

Abstract:Aim: To assess the association between demographic and clinical variables and the outcome of root-end surgery using digital periapical radiographs (PA) and cone-beam computed tomography (CBCT). Methodology: One hundred and fifty teeth that received endodontic microsurgery were clinically and radiographically examined (PA and CBCT scans) after 1 and 2 years. Two calibrated endodontists evaluated the radiographic healing based on a six-point outcome classification. The outcome was classified using both strict (healed) and loose (healing) criteria. The success rates were calculated, and several outcome prognostic factors were assessed. Results: One hundred and fifty teeth were assessed with both radiographic systems. When "loose" success criteria were applied using PA, 90% (n = 135) of teeth were assessed as having had a favourable outcome (90%; 95% CI: 85.2–94.8%), whereas 90.7% (n = 136) of teeth showed a successful outcome when assessed with CBCT (90.7%; 95% CI: 86.0–95.3%). When "strict" success criteria were applied, there was a statistically significant difference (p = 0.018) between the success rates of mineral trioxide aggregate (MTA) (63.7%) and Biodentine (95.5%). Conclusions: Within the limitations of this study, endodontic microsurgery showed a high success rate. Among all the demographic and clinical variables assessed, in the multiregression analysis, only the use of Biodentine was associated with a higher proportion of "complete" healings compared to MTA or Intermediate Restorative Material (IRM) when assessed using CBCT. MTA and Biodentine performed similarly when "incomplete" healings were regarded as successful outcomes.

Abstract:Objective This study explored the achievement of primary and secondary goals and factors influencing their achievement in regenerative endodontic procedures (REPs) for immature permanent teeth. Methods Dental records of all patients who received REPs for immature permanent teeth at the Department of Pediatric Dentistry, Peking University School and Hospital of Stomatology between January 2012 and January 2023 were retrieved. The evaluation of the primary goal was based on medical and radiographic records. The achievement of the primary goal was defined as the absence of clinical signs and symptoms, such as pain, swelling, or sinus tract and the absence of periapical radiolucency, as assessed on postoperative periapical radiographs. The achievement of secondary goal represented increased root wall thickness and/or increased root length, that is, continued root development. Periapical radiographs before and after treatment were used to evaluate the achievement of the secondary goal. The secondary goal was required to be achieved alongside the achievement of primary goal. Results A total of 436 teeth (136 anterior and 300 posterior teeth) were included in this study, 96.1% of which demonstrated achievement of the primary goal. Signs of failure (17 teeth) included crown fracture and uncontrolled and recurrent periapical lesions. In addition, 77.8% of teeth demonstrated achievement of the secondary goal, and more than half of the teeth exhibited a complete root development. Evaluation factors included patients’ age, sex, tooth type, etiology, preoperative periapical lesion, duration of clinical signs and symptoms, follow-up period, and stage of root development. The achievement of the primary and secondary goals were significantly related to age and tooth type (p < 0.05). Conclusions Children with a younger initial visit age are more likely to achieve primary and secondary goals. Additionally, posterior teeth had an advantage over anterior teeth in achieving primary and secondary goals.

Abstract:Regenerative endodontic therapy (RET) offers a novel treatment option for developing teeth with pulp necrosis. In the current instance, RET was used to treat an immature mandibular permanent first molar that had been identified with irreversible pulpitis. The root canals were treated with triple antibiotic paste (TAP) and 1.5% sodium hypochlorite (NaOCl) irrigation. TAP was removed, and 17% ethylenediaminetetraacetic acid (EDTA) was used to treat the root canals during the second visit. As a scaffold, Platelet-rich fibrin (PRF) was applied. Mineral trioxide aggregate (MTA) was applied over PRF, and composite resin was used to repair the teeth. Radiographs taken from the posterior were utilized to assess the healing. The teeth displayed no signs of pain and healing after the six-month follow-up periods, and pulp sensibility tests using a cold and electric pulp tester produced no results. Conservative treatment options should be considered to save immature permanent teeth and assist in the regeneration of the root apex.

Abstract:Background Regenerative endodontic procedures (REPs) is effective for treating young permanent teeth with pulp necrosis. However, its efficacy on delayed replanted avulsed teeth is unclear. Aim This retrospective study aimed to assess the efficacy of REPs in treating delayed replanted immature permanent teeth with apical periodontitis. Design Avulsed teeth receiving REPs were systematically screened based on predetermined criteria. This study assessed the REP outcomes, postoperative periodontal healing, and overall treatment efficacy. Samples were grouped by REP outcomes and root development stage, with Fisher's exact tests used to compare outcomes among different groups. Results Among the included 17 teeth, 47.1% exhibited successful REPs and periodontal healing. Another 47.1%, due to replacement resorption or REP failure, were categorized as tooth survival. Healing of periapical lesions was observed in 88.2% of the cases, but only 41.2% demonstrated continued root development. Although differences were not significant (p = 0.05), teeth with continued root development had a higher rate of functional healing (85.7%) compared to those without (30%). Conclusion Within the limitations of this study, REPs presented reliable outcomes for treating delayed replanted immature permanent teeth with apical periodontitis mainly in periapical lesion healing. Teeth with continued root development after REPs exhibited a higher rate of functional healing. Further investigation is required to explore potential synergies between REP outcomes and periodontal healing.

Abstract:Bottom-up factors such as nutrient availability have long been thought to be the primary regulators of plant growth in salt marshes. However, that paradigm has been challenged by investigations showing that grazing by the periwinkle snailLittoraria irrorata regulatesSpartina alterniflora growth through top-down forces. Our investigation was conducted between November 2003 and March 2007 within the North Inlet Estuary (NIE) salt marsh to examine relationships betweenS. alterniflora andL. irrorata. The primary goal was to determine if observed in situ densities ofL. irrorata significantly decreasedS. alterniflora productivity at the study sites. The results indicate thatS. alterniflora productivity positively correlated withL. irrorata snail density during winter. However, no correlation was observed during summer. Hence, there was no inverse relationship between snail density and primary productivity and no support for the hypothesis of ‘top-down’ control of marsh plant production. A significant relationship betweenS. alterniflora stem density andL. irrorata density was observed during summer and winter, suggesting that stem density may play a key role in determining the distribution of periwinkle snails in NIE. These results challenge the applicability of theL. irrorata-S. alterniflora top-down control model as a generalized phenomenon in southeastern salt marshes.

Abstract:Injuries to immature permanent teeth may cause pulp necrosis. Conventional treatment for such teeth is root canal treatment and apexification, but recently a new treatment option of revascularization has been introduced, also known as regenerative endodontic treatment. This study compared the treatment outcome of apexification and pulp revascularization in immature necrotic teeth. All the patients were treated, and data were acquired from the dental outpatient department of Shahida Islam Medical and Dental College, Lodhran, between November 15, 2021, to November 14, 2022, duration. To complete this research, Randomized Controlled Clinical Trial was used as a study design to collect the patients, later divided into two groups. Means age of all the patients in this study was 11.41 ± 4.20 years; the mean age of the patients in Group A=11.50 ± 4.24 years and Group B=11.32 ± 4.26 years. The success rate in Group A= 60 (87.32%) was lower than that of Group B= 67 (97.04%). The results of this study revealed that pulp revascularization has a higher success rate than apexification by mineral trioxide aggregate.
