Abstract:Background Institutional delivery rate among women in rural communities in Bangladesh remains low after several governmental interventions. A recent analysis of maternal mortality in Bangladesh revealed that women in rural communities were more likely to die from maternal complications than those in urban areas. Objective This study assessed the institutional delivery rate and associated factors among women in rural communities in Bangladesh. Design This was a cross-sectional study that used the 2017–2018 Bangladesh Demographic and Health Survey for analysis. To determine the factors associated with institutional delivery, multivariate logistic regression analysis was performed. Setting and participants The study was conducted in Bangladesh and among 3245 women who delivered live births 3 years before the survey. Main outcome measure The outcome variable was the place of delivery which was dichotomised into institutional and home delivery/other non-professional places. Results The institutional delivery rate was 44.82% (95% CI 42.02% to 47.65%). We found that women between the ages of 30 and 49 years (aOR=1.51, 95% CI 1.05 to 2.18), women whose partners attained higher education (aOR=2.02, 95% CI 1.39 to 2.94), women who had antenatal visits of 1–3 (aOR=2.54, 95% CI 1.65 to 3.90), 4–7 (aOR=4.79, 95% CI 3.04 to 7.53), and ≥8 (aOR=6.13, 95% CI 3.71 to 10.42), women who watched television (aOR=1.35, 95% CI 1.09 to 1.67) and women in the middle (aOR=1.38, 95% CI 1.05 to 1.82), rich (aOR=1.84, 95% CI 1.34 to 2.54) and richest (aOR=2.67, 95% CI 1.82 to 3.91) households were more likely to use institutional delivery. On the other hand, women who were working (aOR=0.73, 95% CI 0.60 to 0.89), women who were Muslims (aOR=0.62, 95% CI 0.44 to 0.89) and women who gave birth to two (aOR=0.61, 95% CI 0.48 to 0.77) or ≥3 children (aOR=0.46, 95% CI 0.35 to 0.60) were less likely to use institutional delivery. Conclusion The study revealed that a low proportion of women in rural communities in Bangladesh used institutional delivery. The results of this study should be taken into account by policy-makers and governmental efforts when creating interventions or programmes aimed at increasing institutional delivery in Bangladesh.

Abstract:Introduction: Institutional delivery is a major concern for a country's long-term growth. Rapid population development, analphabetism, big families, and a wider range of urban-rural health facilities have had a negative impact on institutional services in Sub-Saharan Africa (SSA) countries. The aim of this study was to look into the factors that influence women's decision to use an institutional delivery service in SSA. Methods: The most recent Demographic and Health Survey (DHS), which was conducted in nine countries (Senegal, Ethiopia, Malawi, Rwanda, Tanzania, Zambia, Namibia, Ghana, the Democratic Republic of Congo) was used. The service's distribution outcome (home delivery or institutional delivery) was used as an outcome predictor. Logistic regression models were used to determine the combination of delivery chances and different covariates. Results: The odds ratio of the experience of institutional delivery for women living in rural areas vs urban area was 0.44 (95% confidence interval (CI) 0.41-0.48). Primary educated women were 1.98 (95% CI 1.85-2.12) times more likely to deliver in health institutes than non-educated women, and secondary and higher educated women were 3.17 (95% CI 2.88-3.50) times more likely to deliver in health centers with facilities. Women aged 35-49 years were 1.17 (95% CI 1.05-1.29) times more likely than women aged under 24 years to give birth in health centers. The number of ANC visits: women who visited four or more times were 2.98 (95% CI 2.77-3.22) times, while women who visited three or less times were twice (OR = 2.03; 95% CI 1.88-2.18) more likely to deliver in health institutes. Distance from home to health facility were 1.18 (95% CI 1.11-1.25) times; media exposure had 1.28 (95% CI 1.20-1.36) times more likely than non-media-exposed women to delivery in health institutions. Conclusions: Women over 24, primary education at least, urban residents, fewer children, never married (living alone), higher number of prenatal care visits, higher economic level, have a possibility of mass-media exposure and live with educated husbands are more likely to provide health care in institutions. Additionally, the distance from home to a health facility is not observed widely as a problem in the preference of place of child delivery. Therefore, due attention needs to be given to address the challenges related to narrowing the gap of urban-rural health facilities, educational level of women improvement, increasing the number of health facilities, and create awareness on the advantage of visiting and giving birth in health facilities.
Level of institutional delivery service utilization and associated factors among women who gave birth in the past 12 months, Ga'an libah district, Marodijeh region, Somaliland: a community-based cross-sectional study

Abstract:Background: Institutional delivery has been considered one of the important strategies to improve maternal and child health and significantly reduce birth-related complications. However, it is still low in developing countries though there are some improvements. even among the community who has access to the health institutions weather health center and hospital including Somaliland. Hence, the aim of this study was to assess the level of institutional delivery service utilization and associated factors among women who gave birth in the last 12 months in Ga'an libah district, Marodijeh region, Somaliland. Methods: The community-based cross-sectional study was conducted among women who gave birth in the last 12 months from September to December 2022. A simple random sampling technique was employed to select study participants from a total of sample population. Data was collected using semi-structural administered questionnaire through interviewing women. Data was collected with online mobile data collection (Kobo collect). SPSS version 25.0 was used for data management, entering and analysis. Bivariate and multivariable logistic regression models were fitted to determine the presence of a statistically significant association between independent variables and the outcome variable with p-value < 0.05. Result: Level of Institutional delivery services utilization in Ga'an libah district was 53.9% [95% Cl 48.2-59.6] gave birth at health institutions. Women who can read and write local were (AOR 2.18, 95% CI 1.08-4.56, p<0.01), Women with their husband can be capable to read and write are [(AOR = 6.95, 95% Cl 2.82-21.58, p<0.002]). Additionally, ability to cost transportation for referral [AOR 5.21, 95% Cl 2.44-11.13, p<0.001]. not good services available [AOR 0.07, 95%Cl 0.01-0.10, p<0.02]. lack of maternal health and child knowledge [AOR 0.034, 95% Cl 0.02-0.57, p<0.01]. were observed associated with level of institutional deliver services utilization. Conclusion: The institutional delivery service utilization was relatively high compared to national demographic health services in the study area. A large proportion of women gave both at home without a skilled attendant. Therefore, this finding has important policy implications since changes in the cost of the health service and perceived quality would mean changes in client satisfaction as well as their choice, as well emphasize to expectant women can receive medical advice and ambulance for referrals.
Individual- and community-level determinants of maternal health service utilization in southern Ethiopia: A multilevel analysis

Abstract:Background: Maternal health service utilization decreases maternal morbidity and mortality. However, the existing evidence is inadequate to design effective intervention strategies in Ethiopia. Objectives: This study aimed to examine the utilization of maternal health service and identify its determinants among women of reproductive age in southern Ethiopia. Design: A community-based cross-sectional study was conducted from October 21 to November 11, 2022 on a sample of 1140 women selected randomly from the Northern Zone of the Sidama region. Methods: Data were collected using the Open Data Kit mobile application and exported to Stata version 15 for analysis. We used a multilevel mixed-effects modified Poisson regression with robust standard error to identify determinants of maternal health service utilization. Results: Utilization of antenatal care, health facility delivery, and postnatal care was 52.0% (95% confidence interval: 49.0%, 55.0%), 48.5% (95% confidence interval: 45.6%, 51.4%), and 26.0% (95% confidence interval: 23.0%, 29.0%), respectively. Antenatal care use was associated with receiving model family training (adjusted prevalence ratio: 1.19; 95% confidence interval: 1.06, 1.35), knowledge of antenatal care (adjusted prevalence ratio: 1.54; 95% confidence interval: 1.31, 1.81), perceived quality of antenatal care (adjusted prevalence ratio: 1.02; 95% confidence interval: 1.01, 1.03), and having birth preparedness plan (adjusted prevalence ratio: 1.13; 95% confidence interval: 1.02, 1.25). The identified determinants of health facility delivery use were middle wealth rank (adjusted prevalence ratio: 1.35; 95% confidence interval: 1.03, 1.77), perceived quality of health facility delivery (adjusted prevalence ratio: 1.02; 95% confidence interval: 1.01, 1.03), antenatal care (adjusted prevalence ratio: 1.76; 95% confidence interval: 1.36, 2.26), and high community-level women literacy (adjusted prevalence ratio: 1.55; 95% confidence interval: 1.10, 2.19). Postnatal care use was associated with facing health problems during postpartum period (adjusted prevalence ratio: 1.79; 95% confidence interval: 1.18, 2.72), urban residence (adjusted prevalence ratio: 3.52; 95% confidence interval: 2.15, 5.78), knowledge of postnatal care (adjusted prevalence ratio: 1.11; 95% confidence interval: 1.04, 1.19), and low community-level poverty (adjusted prevalence ratio: 0.43; 95% confidence interval: 0.25, 0.73). Conclusion: Maternal health service use was low in the study area and was influenced by individual- and community-level determinants. Any intervention strategies must consider multi-sectorial collaboration to address determinants at different levels. The programs should focus on the provision of model family training, the needs of women who have a poor perception, and knowledge of maternal health service at the individual level.
Factors Affecting the Utilization of Antenatal Care Services During Pregnancy in Bangladesh and 28 Other Low- and Middle-income Countries: A Meta-analysis of Demographic and Health Survey Data

Abstract:The study aimed to identify the factors influencing the utilization of antenatal care (ANC) services among pregnant women to fulfill the Sustainable Development Goals (SDG) for maternal mortality ratio (MMR) by 2030; we also investigated the consistency of these factors. We have used the Demographic and Health Survey (DHS) data from 29 developing countries for analysis. A binary logistic regression model was run using Demographic and Health Survey data from Bangladesh to determine the factors influencing ANC utilization in Bangladesh. In addition, a random-effects model estimation for meta-analysis was performed using DHS data from 29 developing to investigate the overall effects and consistency between covariates and the utilization of ANC services. Logistic regression revealed that residence (odds ratio [OR] 1.436; 95% confidence interval [CI] 1.238, 1.666), respondent's education (OR 3.153; 95% CI 2.204, 4.509), husband's education (OR 2.507; 95% CI 1.922, 3.271) wealth index (OR 1.485; 95% CI 1.256, 1.756), birth order (OR 0.786; 95% CI 0.684, 0.904), working status (OR 1.292; 95% CI 1.136, 1.470), and media access (OR 1.649; 95% CI 1.434, 1.896) were the main significant factors for Bangladesh. Meta-analysis showed that residence (OR 2.041; 95% CI 1.621, 2.570), respondent's age (OR 1.260; 95% CI 1.106, 1.435), respondent's education level (OR 2.808; 95% CI 2.353, 3.351), husband's education (OR 2.267; 95% CI 1.911, 2.690), wealth index (OR 2.715; 95% CI 2.199, 3.352), birth order (OR 1.722; 95% CI 1.388, 2.137), and media access (OR 2.474; 95% CI 2.102, 2.913) were the most conclusive factors in a subjects decision to attend ANC. Our results support the augmentation of maternal education and media access in rural areas with ANC services. Particular focus is needed for women from Afghanistan since they have a lower level of ANC services.
Factors Affecting the Utilization of Delivery Care Services in Eastern Nepal

Abstract:Background Maternal health has been the utmost priority program implemented by the Government of Nepal, popularly known as the Aama program. The achievement of sustainable development goals in 2030 underlay the utilization of maternal and health care services. Institutional delivery is one of the key focused indicators to measure the country's efficacy in delivering maternal and health care service utilization. The study was conducted in eastern Nepal focusing on the factors affecting institutional delivery. Objective To assess the relationship of demographic, socio-cultural, and socio-behavioral factors with the utilization of delivery care services and to assess factors other than demographic, socio-cultural and socio-behavioral affecting the perception towards health services, accessibility to health services, and utilization of delivery care services. Method This study was cross-sectional and descriptive. A total of 401 recently delivered women were taken for the face-to-face interview. A multistage sampling technique has been taken to select the sample household as a primary sampling unit. Ethical approval has been obtained from Nepal Health Research Council. Result In the multivariate model, education level, family type of women, perceptions and awareness about the health facilities (should a pregnant woman go for a medical check-up, and quality of service of private health institutions is better than public health institutions were found to have a significant influence on the behaviors related to institutional delivery. Conclusion Our study stated that perception of quality of care, regular access to health services, receiving a higher level of education, and living in the joint family are very important which tend to improve the other determining factors for utilization of delivery care service in eastern Nepal.

Several models have been developed to characterize the mobile satellite channel. Some of them are based on different states of signal reception. Abstract:Objectives: Antenatal care (ANC) during pregnancy and skilled birth attendance (SBA) during delivery are important policy concerns to reduce maternal deaths. Bangladesh is one of the developing countries which has made remarkable progress in both services during the last couple of decades by improving the SBA service rate from 16% in 2004 to 42.1% in 2014. However, this rate remains below the targeted level (50%) of the Health Population and Nutrition Sector Development Program set by the Ministry of the Health and Family Welfare of Bangladesh. This article explored the sociodemographic factors associated with the ANC and SBA service attainment. Furthermore, the possible implication of using ANC on SBA was also investigated. Study design: The study followed a cross-sectional design using the Bangladesh demographic and health survey 2014, with a sample of size 4603 women with at least one live birth 3 years preceding the survey. Methods: Following a bivariate analysis, linear mixed-effect models were used to assess the relationship between sociodemographic factors and the outcome indicators (ANC and SBA). Finally, the association between SBA and ANC was evaluated through another mixed-effect model. Results: Wealth index, participation in household decisions, and partner's and respondent's education were significant predictors of ANC; whereas, residence, age at first birth, wealth index, working status, participation in household decisions, and partner and respondent's education were significant for SBA. Female education and household affordability were the strongest predictors for both ANC and SBA. ANC showed significant association with SBA as women accessing essential ANC during delivery seemed to be 4 times more likely (95% confidence interval: 3.05-5.93) to avail SBA services. Conclusions: Overall, four factors were significant: residence, wealth index, education, and ANC access. Women residing in urban areas, having higher financial solvency, completing higher education, and accessing ANC by skilled personnel were more likely to receive SBA at delivery than their counterparts. Accessibility to skilled care during pregnancy leads to increased professional care during delivery. Thus, policies to encourage women and heads of families to seek skilled care during pregnancy would be beneficial to reach the maternal healthcare targets of Bangladesh.

Abstract:In recent years Caesarean Section (C-S)is considered as an important mode of child delivery in lowering mother and infant mortality. However, the unnecessary use of C-S has detrimental impacts on both mothers’ and newborns’ health. The purpose of this study is to evaluate the prevalence of caesarean delivery and to find out the related factors influencing the rapid rise in C-Sin Bangladesh by considering an appropriate statistical model in line with the data structure. The study is based on the data from the 2017-2018 BDHS which is a nationally representative survey. BDHS is conducted periodically and 2017-2018 is the latest of all the BDHS conducted so far. Information used in the present study was collected from 5230 mothers who provided complete information about all the variables considered in the study. Among the Bangladeshi women, the rate of Caesarean sections (C-S) was reported to be 32.94% which is high in any international standard. Multilevel analysis suggests that the highest education level, current working status, wealth index, media exposure, number of decisions in which women participated, age at birth, BMI, birth order of the index child, number of ANC visits during the period of pregnancy, and childbirth weight have significant influence on whether mothers deliver babies through the C-S. In order to prevent needless caesarean deliveries and safeguard mothers from the consequences, Bangladesh urgently needs caesarean delivery policy guidelines. Factors influencing the utilization of institutional delivery services by Chepang women in Chitwan district of Nepal

Abstract:Factors influencing the utilization of instituional delivery services by Chepang women in Chitwan district of Nepal Background To reduce the maternal death rates, a safe motherhood program has been implemented across Nepal to provide pregnant women with free delivery care in a healthcare facility, as well as transportation incentives promoting institutional delivery. Being a marginalized group of people residing in hilly slope areas, the Chepang population lacks significant usage of healthcare services. Objective This study aimed to assess the incidence of delivery complications, and to investigate the factors associated with the utilization of institutional delivery of the Chepang women in the Chitwan district. Methods A cross-sectional study was conducted with a proportional random sample of 557 mothers. A face-to-face interview was performed to collect the data. Using stepwise logistic regression analysis, the data were analyzed. Results The results showed low incidence of delivery complications among those who utilized an institutional delivery. The literate parents were strongly associated with the increased usage of an institutional delivery (OR=12.08, 95%CI=6.32-23.06, and OR=5.45, 95%CI=2.82-10.51 for literate mothers and literate spouses, respectively). Other significant factors were older ages at delivery of mothers as compared to younger ages mothers (OR= 3.28, 95%CI=1.83-5.90 for ages 20-29 yr.; OR=3.75, 95%CI=1.80-7.83) for ages ≥30 yr.), and engagement in business and services occupation of mothers (OR=2.01, 95%CI=1.01-4.01). The associations were observed for vehicle transportation to the delivery facility (OR =1.99, 95% CI =1.28-3.11) and travel time ≤30 minutes to the delivery facility (OR=2.80, 95%CI=1.58-4.96). Conclusions The results revealed a lower incidence of delivery complications among those who utilized the institutional delivery. The analysis also demonstrated that age, education, income, and convenient access to an institutional delivery facility were associated with the utilization of institutional delivery. Mapping the prevalence and covariates associated with home delivery in Bangladesh: A multilevel regression analysis

Abstract:Bangladesh has made an intense effort to improve maternal healthcare facilities including facility delivery, but the number of home deliveries is still very high. Therefore, this study aims to find out district-wise prevalence and determine the individual and community-level predictors of home delivery among women in Bangladesh. Data were derived from the Multiple Indicator Cluster Survey (MICS) 2019, a nationwide cross-sectional survey in Bangladesh. A final sample of 9,166 (weighted) women who gave birth in the two years preceding the survey were included in this study. Considering the two-stage cluster sampling strategy adopted by MICS, we used multilevel (2-level) logistic regression analysis to find out the correlates of home delivery. The overall weighted prevalence of home delivery was 46.41% (95% confidence interval [CI]: 45.39–47.43). The highest prevalence was observed in Bandarban district (84.58%), while the lowest was found in Meherpur district (6.95%). The intercept-only regression model demonstrates that the likelihood of women from various clusters having home delivery varied significantly (variance: 1.47, standard error [SE]: 0.117), indicating the applicability of multilevel regression modeling. The multilevel regression analysis showed that women with higher education, wealth status and ANC visit, and those aged >18 years at first marriage/union were associated with lower odds of delivering child at home compared to their counterparts. While women from age group of 35–49 years, whose last pregnancy was unintended were more likely to deliver child at home. In addition, those respondents belonging to a community that had higher wealth status, women's education level, and exposure to media showed lower odds of having delivery at home. The finding indicates that delivery at home is still high in Bangladesh. Targeted interventions to reduce home delivery are urgently needed in Bangladesh to tackle adversities during deliveries and save mothers from the consequences.

Abstract:Background Utilization of skilled birth attendance during home delivery of pregnant women is proven to reduce complications during and after childbirth. Though the utilization of skilled birth attendance (SBA) during home delivery has increased significantly in recent times, the rate of utilizing skilled birth attendance is still low in several regions across India. The objective of this study is to analyze the prevalence and to identify the determinants of the utilization of skilled birth attendance during home delivery of pregnant women in India. Methods To conduct this study, data and information from the Indian Demographic and Health Survey 2015–16 have been utilized. The sample size for this study is a weighted sample of 41,171 women. The sample consisted of women who had given a live birth in the three years preceding the survey. For women with more than one child, only the first live birth was considered. The binary logistic regression model and the log-binary logistic regression analysis have been applied as the adjusted odds ratios (AORs) with 95% confidence intervals for identifying the determinants of home-based skilled birth attendance during delivery. That allows us to select the most appropriate model for our study objective by ensuring that the determinants of skilled birth attendance for home delivery are accurately assessed based on the characteristics of the data. Results The analyses show that only 18.8% of women had utilized skilled birth attendance during delivery. Women residing in urban areas are more likely to utilize skilled birth attendance during home delivery (AOR: 1.14; 95% CI: 1.08–1.20). Women having higher education levels are associated with increased use of SBA during home delivery (AOR: 1.15; 95% CI: 1.04–1.28). Exposure to media is associated with increased utilization of SBA (AOR: 1.17; 95% CI: 1.11–1.23). Overweight women are also more likely to avail the SBA during home delivery (AOR: 1.11; 95% CI: 1.03–1.19). Women belonging to affluent households have higher odds of utilizing skilled birth attendance (AOR: 1.41; 95% CI: 1.33–1.49). Having 3+ tetanus injections is associated with the utilization of SBA (AOR: 1.56; 95% CI: 1.43–1.69). Women having 4+ antenatal care visits were more likely to utilize SBA (AOR: 1.81; 95% CI: 1.71–1.92). Women belonging to the Hindu religion were 1.12 times more likely to utilize SBA (AOR: 1.12; 95% CI: 1.07–1.18). Women with 1 to 3 birth orders were 1.40 times more likely to utilize skilled birth attendance during home delivery (AOR: 1.40; 95% CI: 1.30–1.51). Conclusion The percentage of women utilizing skilled birth attendance during home delivery is still very low which is a matter of serious concern. Several factors have been found to be associated with the utilization of SBA during home delivery in India. As skilled birth attendance has significant positive health outcomes for pregnant women and newborns, efforts to increase the rate of SBA utilization during home delivery should be undertaken.

Abstract:Abstract Background Caesarean section (CS) delivery has a significant effect on maternal and neonatal health especially in a developing country like Bangladesh. The aim of the study was to determine the risk factors and their individual contribution to CS delivery among Bangladeshi married women in reproductive age. Methods The cross sectional secondary data was used in this study. Data was extracted from Bangladesh Demographic and Health Survey (BDHS), 2014 dataset. BDHS-2014 collected data from all over Bangladesh. Stepwise logistic regression analysis and population attributable fractions (PAF) were utilized in this study. Results A total number of 4422 married Bangladeshi women having at least one child (age ≤ 5 years) were considered in this study. The prevalence of CS delivery among Bangladeshi women was 23.94%. The stepwise logistic regression model showed that location (division), type of residence, education of respondent and her husband, working status, age at first birth, number of children, wealth index and baby’s birth weight were most important predictors of CS delivery among Bangladeshi mothers. PAF demonstrated that overweight or obese women had highest contribution (23.36%) among the risk factors of CS delivery, followed by age at first birth (age > 20 years) (18.97%), highest wealth quintile (17.39%), higher education (15.93%), living in urban environment (14.39%), having lower number of ever born children (1–2 children) (13.58%), living in Dhaka division (12.11%), delivering large size of child at birth (11.13%) and housewife (6.55%). Conclusions In the present study, we have identified the important risk factors and their individual contribution to CS delivery in Bangladesh. Consequently, these factors can be considered for reducing the rate of CS delivery in Bangladesh.

Abstract:Background High maternal mortality rates still today remain a significant public health concern in South Asian countries. The majority of maternal deaths occur during pregnancy, and these deaths may typically be avoided by ensuring that women have access to reliable maternity care such as antenatal care (ANC) and facility delivery. The objectives of this research were to assess socioeconomic disparities in the utilization of health care services by mothers and to determine the factors influencing this utilization among women aged 15 to 49 in five South Asian countries. Methods For this study, nationally representative data from the Demographic and Health Survey (DHS) were analyzed. This research included a total of 262,531 women between the ages of 15 and 49. To determine the likely causes of maternal health care utilization, simple bivariate statistics and binary logistic regression were applied, and decomposition analysis and the concentration curve were used to quantify disparity (Lorenz curve). Results ANC and institutional delivery were both prevalent in 59.27% and 86.52% of cases, respectively. Among the five nations, Maldives has the greatest ANC (96.83%) and institutional delivery (99.39%), while Bangladesh has the lowest ANC (47.01%) and institutional delivery (49.81%). Women’s and husbands’ education, household wealth status, BMI, and urban residents are the most important factors influencing maternal health service utilization, whereas higher education level, affluent wealth quintiles, and place of residence are the major contributors to socioeconomic inequalities in access to maternal health care that favor the wealthy. Conclusion Maternal health care services must be utilized properly in order to promote optimal health and prevent maternal mortality. Several socioeconomic and sociodemographic variables of the individual population, as well as policy issues, all have an impact on maternal mortality. This research recommends for concerted action to enhance how successfully women use maternity care services.

Maternal mortality in Bangladesh: Who, when, why, and where? A national survey-based analysis

Abstract:Background: Despite a notable decline in recent decades, maternal mortality in Bangladesh remains high. A thorough understanding of causes of maternal deaths is essential for effective policy and programme planning. Here we report the current level and major causes of maternal deaths in Bangladesh, focusing on care-seeking practices, timing, and place of deaths. Methods: We analysed data from the 2016 Bangladesh Maternal Mortality and Health Care Survey (BMMS), conducted with nationally representative sample of 298 284 households. We adapted the World Health Organization's 2014 verbal autopsy (VA) questionnaire. Trained physicians reviewed the responses and assigned the cause of death based on the International Classification of Diseases (ICD-10). We included 175 maternal deaths in our analysis. Results: The maternal mortality ratio was 196 (uncertainty range = 159-234) per 100 000 live births. Thirty-eight per cent of maternal deaths occurred on the day of delivery and 6% on one day post-delivery. Nineteen per cent of the maternal deaths occurred at home, another 19% in-transit, almost half (49%) in a public facility, and 13% in a private hospital. Haemorrhage contributed to 31% and eclampsia to 23% of the maternal deaths. Twenty-one per cent of the maternal deaths occurred due to indirect causes. Ninety-two per cent sought care before dying, of which 7% sought care from home. Thirty-three per cent of women who died due to maternal causes sought care from three or more different places, indicating they were substantially shuttled between facilities. Eighty per cent of the deceased women who delivered in a public facility also died in a public facility. Conclusions: Two major causes accounted for around half of all maternal deaths, and almost half occurred during childbirth and by two days of birth. Interventions to address these two causes should be prioritised to improve the provision and experience of care during childbirth. Significant investments are required for facilitating emergency transportation and ensuring accountability in the overall referral practices.

Abstract:Background In developing countries, institutional delivery is a key proven intervention that reduces maternal mortality and can reduce maternal deaths by approximately 16%–33%. In Somalia, only 32% of births are delivered in a health facility with the assistance of a skilled healthcare provider. We aimed to investigate the factors hindering women from giving birth at healthcare facilities in major towns in Somalia, where most of the health facilities in the country are concentrated. Methods A community-based health survey was carried out in 11 major towns in Somalia between October and December 2021. A structured and pretested questionnaire was used to collect data from 430 women who gave birth in the last five years. Women were recruited through convenient sampling. Descriptive statistics were used to summarize the data, and binary and multivariable logistic regression analysis was performed. Adjusted odds ratios (AOR) with 95% CI were estimated to assess the associations. Results The overall prevalence of institutional delivery was 57%. Approximately 38% of women living in Mogadishu and 53% living in another ten towns give birth at home. Women who had poor knowledge of the importance of health facility delivery had nearly four times higher odds of delivering at home (AOR 3.64 CI: 1.49–8.93). Similarly, those who did not receive antenatal care (AOR 2.5, CI: 1.02–6.39) and those who did not receive a consultation on the place of delivery (AOR 2.15, CI: 1.17–3.94) were more likely to give birth at home. The reasons for home delivery included financial reasons, the long distance to the health facility, and the fact that it was easier to give birth at home. Conclusion The study found that home delivery is high in major towns in Somalia and is associated with a lack of understanding of the importance of health facility delivery, not using ANC, and not receiving consultancy about where to give birth. Primary health care should strengthen information, education, and communication activities. Since the health care system in Somalia is overwhelmingly private, the government may consider access to free and within-reach ANC and health facility delivery for women and girls from families who cannot pay the ANC and childbirth delivery cost.

Abstract:This study examines the inequality of the use of skilled delivery assistance by the rural women of Bangladesh using the 2007 Bangladesh Demographic and Health Survey data. Simple cross-tabulation and univariate and multivariate statistical analyses were employed in the study. Overall, 56.1% of the women received at least one antenatal care visit, whereas only 13.2% births were assisted by skilled personnel. Findings revealed apparent inequality in using skilled delivery assistance by socioeconomic strata. Birth order, women’s education, religion, wealth index, region and antenatal care are important determinants of seeking skilled assistance. To ensure safe motherhood initiative, government should pay special attention to reduce inequality in seeking skilled delivery assistance. A strong focus on community-based and regional interventions is important in order to increase the utilization of safe maternal health care services in rural Bangladesh.

Abstract:Objectives: We calculated the prevalence of reproductive health services utilization among the currently married non-pregnant women in Bangladesh and investigated its association with demographics and socio-economic factors. Methods: Publicly available secondary country representative 2019 Multiple Indicator Cluster Survey (MICS) dataset was analyzed. A two-stage stratified random sampling technique was followed, and all administrative districts (N=64) of Bangladesh were covered in the MICS 2019. Contraceptive use, antenatal care and skilled birth attendant were considered as utilization of reproductive health services and both crude and adjusted Odd Ratio (OR) were reported using a total of 8917 (weighted) data. Results: In Bangladesh, 36.9% of married non-pregnant women utilized all three reproductive health services. However, at disaggregated level, contraceptive use (66.9%), antenatal care (83%) and skilled birth attendant (59.3%) were found 2-3 folds higher then cumulative prevalence. Increased likelihood of utilization of reproductive health services (either combined or at individual services) was observed among those women who had higher education [AOR= 2.63, 95% CI 1.99 to 3.47], belong to wealthy families [AOR= 2.46, 95% CI 1.94 to 3.12], residing in urban areas [AOR= 1.33, 95% CI 1.14 to 1.54], having a smaller number of children [AOR= 1.53, 95% CI 1.25 to 1.87], and exposure to media [AOR= 1.44, 95% CI 1.26 to 1.63]. Conclusion: Around one-third of Bangladeshi married women received combined contraceptive use, antenatal care and skilled birth attendant reproductive health services; however, variation exist at the individual service level. Targeted public health campaign focusing on women education and media advertisement may increase the utilization.

Abstract:BackgroundNotwithstanding the significant progress in reducing maternal mortality in recent years, social inequalities in the utilization of maternal care continue to be a challenge in Bangladesh. In this study, we aim to provide a comprehensive analysis of trends in social inequalities in utilization of antenatal care (ANC), facility based delivery (FBD), and skilled birth attendance (SBA) in Bangladesh between 1995 and 2010.MethodsData were extracted from the five latest rounds of Bangladesh Demographic Health Surveys (BDHS). The Theil index (T) and between-group variance (BGV) were used to calculate relative and absolute disparities in the utilization of three measures (ANC, FBD, and SBA) of maternal care across six administrative regions. The relative and slope indices of inequality (RII and SII, respectively) were also used to calculate wealth- and education-based inequality in the utilization of maternal care.ResultsThe results of the T-index suggest that relative inequality in SBA has declined by 0.2% per year. Nevertheless, the estimated BGV demonstrated that absolute inequalities in all three measures of maternal care have increased across administrative divisions. For all three measures of maternal care, the RII and SII indicated consistent socioeconomic inequalities favouring wealthier and more educated women. The adjusted RII suggested that wealth- and education-related inequalities for ANC declined by 9% and 6%, respectively, per year during the study period. The adjusted SII, however, showed that wealth- and education-related inequalities for FBD increased by 0.6% per year.ConclusionsAlthough socially disadvantaged mothers increased their utilization of care relative to mothers of higher socioeconomic status, the absolute gap in utilization of care between socioeconomic groups has increased over time. Our findings indicate that wealthier and more educated women, as well as those living in urban areas, are the major users of ANC, FBD and SBA in Bangladesh. Thus, priority focus should be given to implementing and evaluating interventions that benefit women who are poorer, less educated and live in rural areas.

Abstract:Postnatal care (PNC) is the care of a newborn and mother for up to six weeks from one hour of placenta birth. The postnatal period is one of the most hazardous stages for mothers and their baby's health. The PNC is influenced by several maternal, family, biological, and socio-economic factors and it is necessary to identify the most significant factors of PNC. Therefore, the authors focus on determining the significant determinants of postnatal care in Bangladesh. This study is based on a secondary dataset extracted from the Bangladesh Demographic and Health Survey (BDHS)-2017/18. Descriptive statistics, chi-square test, and multilevel logistic regression have been used to determine the contributing factors of PNC. The rate of postnatal care was highest in Sylhet (73.7%) and lowest in Dhaka (57.1%). Female babies had 10.1% less odds of having postnatal care than male babies. Findings depict that the children with 1–3 siblings have 1. 82 times more odds (odds ratio (OR):1.82, 95% confidence interval (CI):0.03–3.21) of PNC than babies without any siblings. Children who suffered from fever recently had 1.25 times (OR = 1.25, 95% CI:1.09–1.45) more odds of taking PNC than their counterparts. Children of working mothers had 1.33 times (OR:1.33, 95% CI:1.14–1.56) more odds of having PNC than children of non-working mothers. The sex of a child, birth order number, place of residence, region, receiving Bacillus Calmette-Guérin (BCG) vaccine, number of antenatal care visits, having fever recently, number of household members, media exposure, and household facilities are significantly linked with PNC in Bangladesh. To ensure the good health of a child, it is necessary to focus on the targeted groups and put emphasis on the identified variables. The authors believe that the findings will be helpful to the policymakers of Bangladesh to lessen childhood morbidities which will be helpful in achieving the target of the Sustainable Development Goals (SDGs) for reducing preventable maternal and under-five deaths by 2030.
