Towards an Integrated Care System in France: the "Pioneer Territories" experiment

Abstract:Starting in 2018, the French Hospitals Federation (FHF) has developped a clinical integration model based on "Responsabilité Populationnelle" (accountabilty towards a population). The model rellies on localy developped "action plans" covering targeted populations, from "at risk" people to severly ill patients. The model is developped and tested in five "Pioneer Territories", covering 1.3M habitants, and built around Territorial Hospital Groups. The Five Territories ""share"" two common target populations : people at risk or suffering from Type 2 diabetes and Heart Failure. These populations represent around 60 000 ill persons, and 300 000 ""at risk"" individuals, in the Five regions. They share a common methodology, developped by FHF's teams and leading medical experts in the fields of diabetology, cardiology and public health. The model relies on extensive use of healthcare data, in particular the ability to ""stratify"" target populations into clinical profiles. To each profiles are attached clinical guidelines that must be executed for every patient, according to its profile. It is up to local actors to create their "actions plans", leveraging local assets to create local pathways, and local ressources to adress local needs. Large scale deployment started in January 2022. In September 2022, more than 700 healthcare professionals, as well as 60 ""patient/partners"" were actively involved in the program. More than 450 outreach/prevention actions had been performed, and more than 5 300 at risk individuals had been screend for T2D or HF. More than 1 700 patients were included in ""clinical programs"", according to their clinical profiles. We are already measuring impacts in terms of hospital use for these populations : reduced ER utilization, reduced lenght of stay, increased planned admissions. Key Takeaways: The model is inspired by other Integrated health systems. However, FHF having no regulatory power, the model is primarely built around clinical practices and intrinsic motivations. We rely on the ability of local healthcare professionals and actors to develop actions plans that will improve coordination between them and the timely use of healthcare ressources for the benefit of the populations. So far, the model is producing positive results in five very different regions, providing valuable insight as to the scalabilty of the model in other regions, and/or for other target populations. The Five regions have shown tremedous ability to mobilize their local ecosystems, ranging from community groceries to local authorities, to patient organizations and other community assets. Data analysis and utilization, in particular the stratification methods developped for the program, are based on DRG's, making them highly transferable to other HC organizations that use ICD-10 coding. These tools were well received by local HC professionals and are used on a daily basis in the Five Territories. Among the key challenges at this stage, the question of a unified Information System is the most pressing. At this stage, there is still no single EHR in France, making the connection between hospitals and ambulatory providers cumbersome. Morevoer, if we want to model to endure and to be generalyzed in France, regulatory intervention by National authorities will be necessary.

Abstract:We find two relations between coherence and path information in a multipath interferometer. The first builds on earlier results for the two-path interferometer, which used minimum-error state discrimination between detector states to provide the path information. For visibility, which was used in the two-path case, we substitute a recently defined l_{1} measure of quantum coherence. The second is an entropic relation in which the path information is characterized by the mutual information between the detector states and the outcome of the measurement performed on them, and the coherence measure is one based on relative entropy.

Abstract:Rationale Home care allows patients with functional limitations to receive services at home and prevent health decline. Home care can reduce hospitalization and emergency department (ED) transfers. Integrating primary healthcare nurse practitioners (PHCNPs) in home care increases the supply of services, but little is known about their influence on patients' ability to remain at home. Aims and Objectives Evaluate the influence of PHCNPs' interventions on hospitalizations and ED transfers in patients receiving care from interprofessional home care teams with PHCNPs. Method The retrospective cohort study employed multiple logistic regression and Cox modelling techniques to assess the influence of PHCNP interventions on hospitalizations and ED transfers. Descriptive and bivariate analyses examined sample characteristics and variable associations. Four purposively selected sites in Québec, Canada, were included. Data were collected from electronic health records of all patients (n = 343) receiving home care from these teams. Results The characteristics of the sites, sample, and PHCNP interventions were described. Planned interventions led to a 5.8% decrease in hospitalizations and ED transfers (adjusted odds ratio [OR]: 0.058, p = 0.039), while unplanned interventions resulted in a 1.5‐fold increase in this outcome (adjusted OR: 1.518, p

Substitutional Effect of Long-Term Care to Hospital Inpatient Care?

Abstract:In China, many of the topand second-tier hospitals are overcrowded, this is partly due to the fact they are providing services which can provided by other medical facilities such as long-term care. The implementation of the Qingdao Long-term Care Medical Insurance (LTCMI) which began as a pilot in Qingdao in 2012 may alleviate the burden of overcrowding in these hospitals. In this pilot, the Qingdao government shifted patients who did not require hospital inpatient care from topand second-tier hospitals to lower tier facilities, care homes and home care to i) reduce the expenditure of patients, ii) reduce the burden on the topand second-tier hospitals and iii) improve delivery (from a time and geographic perspective) of long term care to those that need such attention. The purpose of this paper is to assess the impact of this policy from a burden and cost perspective. Our finding suggests that there is a reduction in costs to all stakeholders. The total cost to the government-subsidized medical insurance decreases by around $7918 RMB per recipient. The cost to the individual decreases by around $2324 RMB per recipient. Thus, netting a decrease of $10,242 RMB in total expenditure. Furthermore, we find that there is a 12% reduction in inpatient service after a recipient participates in the pilot. Given the reduction in costs and admissions, this does indicate some level of success with the program. This paper concludes by examining the policy implications of these results.
The impact of integrated care on health care utilization and costs in a socially deprived urban area in Germany: A difference-in-differences approach within an event-study framework

Abstract:We investigated the impact of an integrated care initiative in a socially deprived urban area in Germany. Using administrative data, we empirically assessed the causal effect of its two sub-interventions, which differed by the extent to which their instruments targeted the supply and demand side of healthcare provision. We addressed confounding using propensity score matching via the Super Learner machine learning algorithm. For our baseline model, we used a two-way fixed-effects difference-in-differences approach to identify causal effects. We then employed difference-in-differences analyses within an event-study framework to explore the heterogeneity of treatment effects over time, allowing us to disentangle the effects of the sub-interventions and improve causal interpretation and generalizability. The initiative led to a significant increase in hospital and emergency admissions and non-hospital outpatient visits, as well as inpatient, non-hospital outpatient, and total costs. Increased utilization may indicate that the intervention improved access to care or identified unmet need.

Abstract:Objective: To quantify the impact of community-based health insurance (CBI) on utilization of health care services in rural Burkina Faso.Methods: Propensity score matching was used to minimise the observed baseline differences in the characteristics of insured and uninsured groups such that the observed difference in healthcare utilisation could generally be attributed to the CBI.Results: Compared with those who were not enrolled in the CBI, the overall increase in outpatient visits given illness in the insured group was about 40% higher, while the differential effect on utilization of inpatient care between insured and non-insured groups was insignificant. Not only were the very poor less likely to enroll in CBI, but even once insured, they were less likely to utilize health services compared to their wealthier counterparts.Conclusions: The overall effect of CBI on health care utilization is significant and positive but the benefit of CBI is not equally enjoyed by all socioeconomic groups. The policy implications are: (a) there is a need to subsidize the premium to favor the enrolment of the very poor: and (b) various measures need to be placed in order to maximize the population's capacity to enjoy the benefits of insurance once insured. Why We Should Focus on Melanoma-Targeted Screening Strategies

Abstract:ma could be considerable. The main reason is the low incidence rate of the disease: basic statistics explain that the specificity of a screening procedure will decrease (and the number of false-positive results considerably increase), depending on whether the disease incidence is high (in selected populations) or low (in general unselected populations). In order to improve the balance between benefits and harm, further research on melanoma screening should focus on individuals at increased risk of melanoma. According to an individual perspective, we assume that the excision of a nevus which would appear benign after a pathological analysis is not a major harm: various patients obtain excision of nevi for functional or esthetic reasons and excisions of nevi are common. (3) Do we screen the right people? Do we really reach out to the risk population? Both Jørgensen and Braun report a major limitation of the EUROMELANOMA campaigns conducted in the general population at average risk of melanoma: only 24% of the participants (263/1,087) agreed to be followed up [8–10]. Braun analyzed the major question as “whether we screen the right people, i.e. whether we really reach out to the risk population which we would otherwise miss.” We assume that a major issue is health inequalities [11]. While significant funds are spent by policymakers to enhance melanoma awareness in the general population, patients are selfdirected to seek consultations. As a result, various populations (men, older patients, and socioeconomically disadvantaged populations) do not perform skin examination and have thick melanomas at the moment of diagnosis [12, 13]. In total, melanoma exhibits the highest inequalities of all cancers when focusing on diagnosis at advanced stages [12]. In order to address this major issue, policymakers should promote the development of organized targeted melanoma screening programs, focusing on individuals at increased risk of melanoma. In conclusion, screening campaigns in the general population – as reported by Braun et al. [2] – are not cost-effective, the balance between benefit and harm is unclear, and it does not address health inequality issues. A limit of developing no screening strategy [3] is that individuals consult on their own initiative, with no effect on health inequalities. We agree with Braun et al. that the development and validation of a new, tailored and more effective strategy is necessary to reduce inequalities at the screening stage. We invite researchers who would like to assess a melanoma-targeted screening intervention via an international randomized controlled trial to join a melanoma-targeted screening task force.

Abstract:<h3>Background</h3><p>Comprehensive assessment of integrated care deployment constitutes a major challenge to ensure quality, sustainability and transferability of both healthcare policies and services in the transition toward a coordinated service delivery scenario. To this end, the manuscript articulates four different protocols aiming at assessing large-scale implementation of integrated care, which are being developed within the umbrella of the regional project Nextcare (2016–2019), undertaken to foster innovation in technologically-supported services for chronic multimorbid patients in Catalonia (ES) (7.5 M inhabitants).</p><p>Whereas one of the assessment protocols is designed to evaluate population-based deployment of care coordination at regional level during the period 2011–2017, the other three are service-based protocols addressing: i) Home hospitalization; ii) Prehabilitation for major surgery; and, iii) Community-based interventions for frail elderly chronic patients. All three services have demonstrated efficacy and potential for health value generation. They reflect different implementation maturity levels. While full coverage of the entire urban health district of <em>Barcelona-Esquerra</em> (520 k inhabitants) is the main aim of home hospitalization, demonstration of sustainability at Hospital Clinic of Barcelona constitutes the core goal of the prehabilitation service. Likewise, full coverage of integrated care services addressed to frail chronic patients is aimed at the city of Badalona (216 k inhabitants).</p><h3>Methods</h3><p>The population-based analysis, as well as the three service-based protocols, follow observational and experimental study designs using a non-randomized intervention group <em>(integrated care)</em> compared with a control group <em>(usual care)</em> with a propensity score matching method. Evaluation of cost-effectiveness of the interventions using a Quadruple aim approach is a central outcome in all protocols. Moreover, multi-criteria decision analysis is explored as an innovative method for health delivery assessment. The following additional dimensions will also be addressed: i) Determinants of sustainability and scalability of the services; ii) Assessment of the technological support; iii) Enhanced health risk assessment; and, iv) Factors modulating service transferability.</p><h3>Discussion</h3><p>The current study offers a unique opportunity to undertake a comprehensive assessment of integrated care fostering deployment of services at regional level. Abstract:Background Information and communication technology may provide domiciliary care programs with continuity of care. However, evidence about the effectiveness and cost-effectiveness of information and communication technology in the context of integrated care models is relatively scarce. Objective The objective of our study was to provide evidence on the clinical effectiveness and cost-effectiveness of the BeyondSilos project for patients enrolled in the Badalona city pilot site in Spain. Methods A quasi-experimental study was used to assess the cost-effectiveness of information and communication technology–enhanced integration of health and social care, including the third sector (intervention), compared to basic health and social care coordination (comparator). The study was conducted in Badalona between 2015 and 2016. Participants were followed for 8 months. Results The study included 198 patients: 98 in the intervention group and 100 in the comparator group. The mean Barthel index remained unchanged in the intervention group (mean change 0.14, 95% CI –4.51 to 4.78; P=.95) but decreased in the comparator group (mean change –3.23, 95% CI –5.34 to –1.11; P=.003). Instrumental Activities of Daily Living significantly decreased in both groups: mean changes of –0.23 (95% CI –0.44 to –0.02; P=.03) and –0.33 (95% CI –0.46 to –0.20; P<.001) in the intervention and comparator groups, respectively. No differences were found in the Geriatric Depression Scale (intervention: mean change 0.28, 95% CI –0.44 to 1.01, P=.44; comparator: mean change –0.29, 95% CI –0.59 to 0.01, P=.06). The intervention showed cost-effectiveness (incremental cost-effectiveness ratio €6505.52, approximately US $7582). Conclusions The information and communication technology–enhanced integrated domiciliary care program was cost-effective. The beneficial effects of this approach strongly rely upon the commitment of the professional staff involved. REDUCING UNPLANNED HOSPITALIZATIONS OF NURSING HOME RESIDENTS: A PROCESS EVALUATION OF INTERCARE

Abstract:Abstract The INTERCARE study provided evidence for a complex intervention to reduce unplanned hospitalizations in 11 Swiss nursing homes. The six intervention components were strengthening of interprofessional collaboration, introduction of a nurse with in-depth geriatric training (INTERCARE nurse), comprehensive geriatric assessment, advanced care planning, evidence-based tools, and data-driven quality improvement. This process evaluation aims to describe the intervention components’ use in practice and understand the mechanisms of change to optimize intervention components for scale-up. We used qualitative data from individual and focus group interviews and meetings notes from the research team’s conversations with nursing home care workers and leadership, INTERCARE nurses and physicians from five nursing homes participating in the INTERCARE study. Data was extracted into spread sheets before starting inductive coding. Findings were summarized and consolidated in a conceptual model. In connection with the four intervention elements interprofessional collaboration, INTERCARE nurse, advanced care planning and evidence-based tools, we identified four intervention mechanisms, grouped into 1) those changing reasoning of care workers e.g., following a more structured approach to do things and 2) those enabling existing resources e.g., availability of a responsible contact person. We also identified behavior changes in care workers, including faster reaction to changes in resident situations, more comprehensive assessment of resident situations and improved communication which contributed to reductions in hospitalizations. Understanding how interventions work in practice is crucial for informing potential adaptations to the intervention and the implementation strategies. Our findings can help optimize the intervention components for scale-up, ultimately reducing unplanned hospitalizations from nursing homes.
Aligning social and health care services: The case of Community Care Connections

Abstract:<p>The Community Care Connections (CCC) program aims to align social and healthcare services to improve health outcomes in older adults with complex medical and social needs. This study assessed changes in healthcare utilization before and after CCC program participation. Between June 2016 and March 2019, 1214 adults with complete data who provided informed consent participated in the CCC program. CCC client data were linked with data on hospitalizations, ED visits, and observation stays 90 days before and after program start. Data analysis examined changes in health care utilization 90 days after program start, compared to 90 days before. Hospitalizations decreased by 30% (Change = −0.029, 95% Confidence Interval (CI) = −0.053, −0.005), ED visits decreased by 29% (Change = −0.114, 95% CI = -0.163, −0.066), and observation stays decreased by 23% (Change = −0.041, 95% CI = -0.073, −0.009) during the post period. ED visits decreased by 37% (Change = −0.140, 95% CI = -0.209, −0.070) for those with hypertension and by 30% (Change = −0.109, 95% CI = -0.199, −0.020) for those with high cholesterol, while observation stays decreased by 46% (Change = −0.118, 95% CI = -0.185, −0.052) for those with diabetes and by 44% (Change = −0.082, 95% CI = -0.150, −0.014) for those with high cholesterol during the post period. Connecting older adults with social services through the healthcare delivery system may lead to decreases in hospitalizations, ED visits, and observation stays. Implementation of cross-sector partnerships that address non-clinical factors that impact the health of older adults may reduce the use of costly healthcare services.</p>

Abstract:BackgroundThe increasing number of patients with chronic diseases represents a challenge for health care systems. The Chronic Care Model suggests a multi-component remodelling of chronic disease services to improve patient outcomes. To meet the complex and ongoing needs of patients, chronic disease prevention and management (CDPM) has been advocated as a key feature of primary care producing better outcomes, greater effectiveness and improved access to services compared to other sectors. The objective of this study is to evaluate the adaptation and implementation of an intervention involving the integration of chronic disease prevention and management (CDPM) services into primary health care.Methods/DesignThe implementation of the intervention will be evaluated using descriptive qualitative methods to collect data from various stakeholders (decision-makers, primary care professionals, CDPM professionals and patients) before, during and after the implementation. The evaluation of the effects will be based on a combination of experimental designs: a randomized trial using a delayed intervention arm (n = 326), a before-and-after design with repeated measures (n = 163), and a quasi-experimental design using a comparative cohort (n = 326). This evaluation will utilize self-report questionnaires measuring self-efficacy, empowerment, comorbidity, health behaviour, functional health status, quality of life, psychological well-being, patient characteristics and co-interventions. The study will take place in eight primary care practices of the Saguenay region of Quebec (Canada). To be included, patients will have to be referred by their primary care provider and present at least one of the following conditions (or their risk factors): diabetes, cardiovascular diseases, chronic obstructive pulmonary disease, asthma. Patients presenting serious cognitive problems will be excluded.DiscussionIn the short-term, improved patient self-efficacy and empowerment are expected. In the mid-term, we expect to observe an improvement in health behaviour, functional health status, quality of life and psychological well-being. At the organizational level, the project should lead to coordinated service delivery, improved patient follow-up mechanisms and enhanced interprofessional collaboration. Abstract:BACKGROUND: In nursing homes there is a gradual move from traditional care to integrated care. Integrated care means a demand-oriented, small-scale, co-operated and coordinated provision of services by different caregivers. This integration has direct effect on the work of these separate disciplines. With the introduction of integrated care the quality of work of these caregivers should be assured or even be improved.OBJECTIVES: The purpose of this study was to examine the implementation of integrated care in the nursing home sector and its effects on the quality of work of the caregivers (work content, communication and co-operation and worker's outcomes).DESIGN: A non-equivalent pre-test/post-test control group design was used in this study.SETTINGS AND PARTICIPANTS: Two nursing homes in the Netherlands participated in the study. One nursing home provided the five experimental nursing wards and the other nursing home provided four control wards.METHOD: Data were selected by means of written questionnaires.RESULTS: The results showed that the intervention appeared to be only successful on the somatic wards. The caregivers of these wards were more able to create a home-like environment for their residents, to use a demand-oriented working method and to integrate the provision of care and services. Regarding the effects of the intervention on quality of work factors, the results included an increase of social support by the supervisor, an increase of the degree of collaboration and a decrease in job demands. No changes were found for the worker's outcomes such as job satisfaction.CONCLUSIONS: The intervention on the psycho-somatic wards was unsuccessful. Although the introduction of integrated care on the somatic wards was successful, the effects on quality of work were limited. Next to quantitative research, more qualitative in-depth research is needed to examine models of integrated care and their effects on the work of caregivers within health care organisations, with special attention for specific characteristics of different types of nursing home care (somatic vs. psycho-geriatric).

Impact of the implementation of a fast-track on emergency department length of stay and quality of care indicators in the Champagne-Ardenne region: a before–after study

Abstract:Objectives We aimed to evaluate the effect of the implementation of a fast-track on emergency department (ED) length of stay (LOS) and quality of care indicators. Design Adjusted before–after analysis. Setting A large hospital in the Champagne-Ardenne region, France. Participants Patients admitted to the ED between 13 January 2015 and 13 January 2017. Intervention Implementation of a fast-track for patients with small injuries or benign medical conditions (13 January 2016). Primary and secondary outcome measures Proportion of patients with LOS ≥4 hours and proportion of access block situations (when patients cannot access an appropriate hospital bed within 8 hours). 7-day readmissions and 30-day readmissions. Results The ED of the intervention hospital registered 53 768 stays in 2016 and 57 965 in 2017 (+7.8%). In the intervention hospital, the median LOS was 215 min before the intervention and 186 min after the intervention. The exponentiated before–after estimator for ED LOS ≥4 hours was 0.79; 95% CI 0.77 to 0.81. The exponentiated before–after estimator for access block was 1.19; 95% CI 1.13 to 1.25. There was an increase in the proportion of 30 day readmissions in the intervention hospital (from 11.4% to 12.3%). After the intervention, the proportion of patients leaving without being seen by a physician decreased from 10.0% to 5.4%. Conclusions The implementation of a fast-track was associated with a decrease in stays lasting ≥4 hours without a decrease in access block. Further studies are needed to evaluate the causes of variability in ED LOS and their connections to quality of care indicators.

Does integrated governance lead to integrated patient care? Findings from the innovation forum

Abstract:Good integration of services that aim to reduce avoidable acute hospital bed use by older people requires frontline staff to be aware of service options and access them in a timely manner. In three localities where closer inter-organisational integration was taking place, this research sought patients' perceptions of the care received across and within organisational boundaries. Between February and July 2008, qualitative methods were used to map the care journeys of 18 patients (six from each site). Patient interviews (46) covered care received before, at the time of and following a health crisis. Additional interviews (66) were undertaken with carers and frontline staff. Grounded theory-based approaches showed examples of well-integrated care against a background of underuse of services for preventing health crises and a reliance on 'traditional' referral patterns and services at the time of a health crisis. There was scope to raise both practitioner and patient awareness of alternative care options and to expand the availability and visibility of care 'closer to home' services such as rapid response teams. Concerns voiced by patients centred on the adequacy of arrangements for organising ongoing care, while family members reported being excluded from discussions about care arrangements and the roles they were expected to play. The coordination of care was also affected by communication difficulties between practitioners (particularly across organisational boundaries) and a lack of compatible technologies to facilitate information sharing. Finally, closer organisational integration seemed to have limited impact on care at the patient/practitioner interface. To improve care experienced by patients, organisational integration needs to be coupled with vertical integration within organisations to ensure that strategic goals influence the actions of frontline staff. As they experience the complete care journey, feedback from patients can play an important role in the service redesign agenda.
Transition of care from post-acute services for the older adults in Quebec: a pilot impact evaluation

Abstract:Background Early discharge of frail older adults from post-acute care service may result in individuals' reduced functional ability to carry out activities of daily living, and social, emotional, and psychological distress. To address these shortcomings, the Montreal West Island Integrated University Health and Social Services Centre in Quebec, Canada piloted a post-acute home physiotherapy program (PAHP) to facilitate the transition of older adults from the hospital to their home. This study aimed to evaluate: (1) the implementation fidelity of the PAHP program; (2) its impact on the functional independence, physical and mental health outcomes and quality of life of older adults who underwent this program (3) its potential adverse events, and (4) to identify the physical, psychological, and mental health care needs of older adults following their discharge at home. Methods A quasi-experimental uncontrolled design with repeated measures was conducted between April 1st, 2021 and December 31st, 2021. Implementation fidelity was assessed using three process indicators: delay between referral to and receipt of the PAHP program, frequency of PAHP interventions per week and program duration in weeks. A battery of functional outcome measures, including the Functional Independence Measure (FIM) and the Patient-Reported Outcomes Measurement Information System (PROMIS) Global-10 scale, as well as fall incidence, emergency visits, and hospitalizations were used to assess program impact and adverse events. The Patient's Global Impression of Change (PGICS) was used to determine changes in participants' perceptions of their level of improvement/deterioration. In addition, the Camberwell Assessment of Need for the Elderly (CANE) questionnaire was administered to determine the met and unmet needs of older adults. Results Twenty-four individuals (aged 60.8 to 94 years) participated in the PAHP program. Implementation fidelity was low in regards with delay between referral and receipt of the program, intensity of interventions, and total program duration. Repeated measures ANOVA revealed significant improvement in FIM scores between admission and discharge from the PAHP program and between admission and the 3-month follow-up. Participants also reported meaningful improvements in PGICS scores. However, no significant differences were observed on the physical or mental health T-scores of the PROMIS Global-10 scale, in adverse events related to the PAHP program, or in the overall unmet needs. Conclusion Findings from an initial sample undergoing a PAHP program suggest that despite a low implementation fidelity of the program, functional independence outcomes and patients' global impression of change have improved. Results will help develop a stakeholder-driven action plan to improve this program. A future study with a larger sample size is currently being planned to evaluate the overall impact of this program. Abstract:The present study further assessed the role of the endolymphatic sac (E.S.) in the generation of inner ear immune responses. Guinea pigs underwent unilateral endolymphatic sac obliteration (Rt) and the opposite ears (Lt) served as controls. The animals were then systemically sensitized to KLH and both inner ears were subsequently challenged with KLH. Absence of the E.S. significantly reduced perilymph anti-KLH levels and greatly reduced cellular infiltration within the cochlea as compared to the control ears (Lt). Control studies demonstrated that neither surgical trauma to the semicircular canal nor the chemical effects of silver nitrate caused immunosuppression of the normal inner ear immune response. From these findings, the E.S. appears to be the main site of antigen processing in the inner ear and a site analogous to the gut with respect to absorptive and immunologic functions.

Abstract:Objective: The evaluation of a new model of care for older people with complex health care needs that aimed to reduce their use of acute hospital services. Method: Older people (over 55 years) with complex health care needs, who had made three or more presentations to a hospital emergency department (ED) in the previous 12 months, or who were identified by community health care agencies as being at risk of making frequent ED presentations, were recruited to the project. The participants were allocated a ?care facilitator? who provided assistance in identifying and accessing required health care services, as well as education in aspects of self management. Data for the patients who had been participants on the project for a minimum of 90 days (n = 231) were analysed for their use of acute hospital services (ED presentations, admissions and hospital bed-days) for the period 12-months pre-recruitment and postrecruitment. A similar analysis on the use of hospital services was conducted on the data of patients who were eligible and who had been offered participation, but who had declined (comparator group; n=85). Results: Post recruitment, the recruited patients displayed a 20.8% reduction in ED presentations, a 27.9% reduction in hospital admissions, and a 19.2% reduction in bed-days. By comparison, the patients who declined recruitment displayed a 5.2% increase in ED presentations, a 4.4 % reduction in hospital admissions, and a 15.3% increase in inpatient bed-days over a similar timeframe. Conclusion: A model of care that facilitates access to community health services and provides coordination between existing services reduces hospital demand.
