Abstract:Many groups have dedicated to develop elasticity-imaging methods that induce tissue displacements by an acoustic radiation force (ARF). In this paper, we employ a latest model, 2-D CMUT array transducer, to generate a rod-like radiation force. Compared to spot-like force excitation, the rod-like focal force excitation has the following advantages in future practice: First, it can extend the displacement field; further, a rod-like excitation induces a shear wave field in a preferential propagation direction, which will help to measure the elastic properties in the anisotropic tissues, such as biceps and cardiac muscles. The 2-D array transducer is lateral focused at several depths with no elevational focalization by setting the delay time between the elements. The shape of rod-like focus has a length of about 16mm along the elevational direction. The uniformity and the width of the rod-like focal region at each focal depth is explored in our simulation study. Simulated results show that both the nonuniformity and the width of the focal region will increase with the focal depth. The standard deviation (SD) of the normalized pressure in the focal region will also increase with the focal depth. When the focal depth is relative near (<25mm), both the uniformity and the width of the focal region reach a satisfactory level (SD < 0.12, width< 3.2mm).

Abstract:Soft matters are observed anomalous viscosity behaviors often characterized by a power law frequency-dependent attenuation in acoustic wave propagation. Recent decades have witnessed a fast growing research on developing various models for such anomalous viscosity behaviors, among which one of the present authors proposed the modified Szabo’s wave equation via the positive fractional derivative. The purpose of this study is to apply the modified Szabo’s wave equation to simulate a recent ultrasonic imaging technique called the clinical amplitude-velocity reconstruction imaging (CARI) of breast tumors which are of typical soft tissue matters. Investigations have been made on the effects of the size and position of tumors on the quality of ultrasonic medical imaging. It is observed from numerical results that the sound pressure along the reflecting line, which indicates the detection results, varies obviously with sizes and lateral positions of tumors, but remains almost the same for different axial positions.
Acoustic radiation force-induced longitudinal shear wave for ultrasound-based viscoelastic evaluation

Abstract:Acoustic radiation force (ARF) is widely used to induce shear waves for evaluating the mechanical properties of biological tissues. Two shear waves can be generated when exciting with ARF: a transverse shear wave, also simply called shear wave (SW), and a longitudinal shear wave (LSW). Shear waves (SWs) have been broadly used to assess the mechanical properties. Some articles have reported that the LSW can be used to evaluate mechanical properties locally. However, existing LSW studies are mainly focused on the group velocity evaluation using optical coherence tomography (OCT). Here, we report that a LSW generated with ARF can be used to probe viscoelastic properties, including shear modulus and viscosity, using ultrasound. We took advantage of the surface boundary effect to reflect the LSW, named RLSW, to address the energy deficiency of LSW induced by ARF. We systematically evaluated the experiments with tissue-mimicking viscoelastic phantoms and validated by numerical simulations. Phase velocity and dispersion comparison between the results induced by a RLSW and a SW exhibit good agreement in both the numerical simulations and experimental results. The Kelvin-Voigt (KV) model was used to determine the shear modulus and viscosity. RLSW shows great potential to evaluate localized viscoelastic properties, which could benefit various biomedical applications such as evaluating the viscoelasticity of heterogeneous materials or microscopic lesions of tissues.
Quasi-plane Shear Wave Propagation Induced by Acoustic Radiation Force with a Focal Line Region: a Simulation Study

Abstract:Shear wave propagation speed has been regarded as an attractive indicator for quantitatively measuring the intrinsic mechanical properties of soft tissues. While most existing techniques use acoustic radiation force (ARF) excitation with focal spot region based on linear array transducers, we try to employ a special ARF with a focal line region and apply it to viscoelastic materials to create shear waves. First, a two-dimensional capacitive micromachined ultrasonic transducer with 64 × 128 fully controllable elements is realised and simulated to generate this special ARF. Then three-dimensional finite element models are developed to simulate the resulting shear wave propagation through tissue phantom materials. Three different phantoms are explored in our simulation study using: (a) an isotropic viscoelastic medium, (b) within a cylindrical inclusion, and (c) a transverse isotropic viscoelastic medium. For each phantom, the ARF creates a quasi-plane shear wave which has a preferential propagation direction perpendicular to the focal line excitation. The propagation of the quasi-plane shear wave is investigated and then used to reconstruct shear moduli sequentially after the estimation of shear wave speed. In the phantom with a transverse isotropic viscoelastic medium, the anisotropy results in maximum speed parallel to the fiber direction and minimum speed perpendicular to the fiber direction. The simulation results show that the line excitation extends the displacement field to obtain a large imaging field in comparison with spot excitation, and demonstrate its potential usage in measuring the mechanical properties of anisotropic tissues.
Impact of Acoustic Radiation Force Excitation Geometry on Shear Wave Dispersion and Attenuation Estimates

Abstract:Shear wave elasticity imaging (SWEI) characterizes the mechanical properties of human tissues to differentiate healthy from diseased tissue. Commercial scanners tend to reconstruct shear wave speeds for a region of interest using time-of-flight methods reporting a single shear wave speed (or elastic modulus) to the end user under the assumptions that tissue is elastic and shear wave speeds are not dependent on the frequency content of the shear waves. Human tissues, however, are known to be viscoelastic, resulting in dispersion and attenuation. Shear wave spectroscopy and spectral methods have been previously reported in the literature to quantify shear wave dispersion and attenuation, commonly making an assumption that the acoustic radiation force excitation acts as a cylindrical source with a known geometric shear wave amplitude decay. This work quantifies the bias in shear dispersion and attenuation estimates associated with making this cylindrical wave assumption when applied to shear wave sources with finite depth extents, as commonly occurs with realistic focal geometries, in elastic and viscoelastic media. Bias is quantified using analytically derived shear wave data and shear wave data generated using finite-element method models. Shear wave dispersion and attenuation bias (up to 15% for dispersion and 41% for attenuation) is greater for more tightly focused acoustic radiation force sources with smaller depths of field relative to their lateral extent (height-to-width ratios <16). Dispersion and attenuation errors associated with assuming a cylindrical geometric shear wave decay in SWEI can be appreciable and should be considered when analyzing the viscoelastic properties of tissues with acoustic radiation force source distributions with limited depths of field.

Abstract:Objective Ultrasound beams sometimes need to be steered from the edge of linear array transducers to reach the sample volume with a desired Doppler angle in vascular exams. This phantom study aims to evaluate the impact of apertures located at the array edge on peak velocity (PV) measurements. Methods Three ultrasound scanner systems equipped with eight transducers from 3 major ultrasound vendors were tested using a flow phantom with a horizontal tube. Five spectral Doppler measurements with the aperture positioned at one edge of the array and 5 with the aperture at the center of the array were obtained using all available scanner-transducer combinations while maintaining all scan parameters and the sample volume in the same tube location. Differences in PVs between center and edge apertures were compared across 4 constant flow rates. Results The averaged PVs for all phantom flow rates ranged from 24.4 cm/s to 138.2 cm/s from the array center. The averaged PVs from the center aperture were significantly greater than the corresponding measurements from the edge aperture for each flow rate (all p < 0.001). The relative PV differences ranged from 6.7% to 19.4% across all transducers and flow rates. Conclusion Significantly lower PVs were consistently shown with the Doppler beam aperture at the array edge compared to center among all tested systems. This may be due to a narrower aperture width, shifted central axis, and less intrinsic spectral broadening error at the array edge. Controlling variations in Doppler aperture location is important in clinical applications which depend on consistent velocity measurements.

Ultrasonographic tissue quantification of the breast using acoustic radiation force impulse technology: phantom study and clinical application

Abstract:Purpose: The aim of this study was to perform the phantom experiment and demonstrate the clinical usefulness of tissue quantification using a linear array transducer and acoustic radiation force impulse (ARFI) technology. Materials and methods: For the phantom study, the commercially available Elasticity QA Phantom Model 049 was used. First, we measured the shear wave velocity (m/s) for the four spheres and the background of the phantom. Then, the shear wave velocity at nine sites was measured, with the region of interest being moved gradually from a shallow region (3 mm) to a deeper region (38 mm). For the clinical study, the shear wave velocities of 15 solid breast mass lesions were measured. Results: The phantom study confirmed the feasibility of quantitative determination of the degree of tissue hardness. Dispersion of the measured values tended to be somewhat increased for the depths of 3 mm and 38 mm. The mean shear wave velocity was 2.07-2.93 m/s for five benign lesions, whereas higher shear wave velocities (n = 2) (7.15, 7.44 m/s) or "X.XX" (unmeasurable state) (n = 7) were found for malignant lesions other than mucinous carcinoma (2.44 m/s). Conclusion: ARFI tissue quantification is a potentially promising ultrasonographic technique for diagnosing breast lesions.
Enhanced Contractility of Renal Afferent Arterioles From Angiotensin-Infused Rabbits: Roles of Oxidative Stress, Thromboxane Prostanoid Receptors, and Endothelium

Abstract:We tested the hypothesis that cyclooxygenase (COX), thromboxane A2 synthase (TxA2-S), thromboxane prostanoid receptors (TP-Rs), or superoxide anion (O2−) mediates enhanced contractions of renal afferent arterioles (Aff) of angiotensin II (Ang II)-infused rabbits. Rabbits were infused with vehicle (sham), Ang II 60 ng·kg−1·min−1 (Ang II 60) or 200 ng·kg−1·min−1 (Ang II 200). There was a selective enhanced vasoconstriction of Affs from Ang II 60 rabbits to Ang II (Δdiameter−78±8% versus −43±9%; P <0.01) that was normalized by a TP-R antagonist but not by a superoxide dismutase (SOD) mimetic. Affs from Ang II 200 rabbits had increased (P <0.01) mRNA for COX-2 and enhanced vasoconstriction to Ang II, U-46 619 (TP-R mimetic), and endothelin-1 that was normalized by ifetroban plus tempol together. Endothelium removal enhanced Ang II responses of Affs from sham rabbits but blunted responses from Ang II 200 rabbits and abolished responses to ifetroban. Affs from Ang II 200 rabbits had an endothelium-dependent contraction factor (EDCF) response to that was blunted (P <0.001) by a SOD mimetic or antagonists of COX-1 or TxA2-S but normalized by antagonists of COX-2 or TP-R. Thus, enhanced Ang II responses in Affs from rabbits infused with slow pressor Ang II are mediated independently by O2− in the vascular smooth muscle cells and by an EDCF that is principally a vasoconstrictor prostaglandin generated by COX-2 > −1 activating TP-Rs, whereas enhanced responses in rabbits infused with a lower Ang II dose are dependent on TP-R but not O2−.
The effect of temperature dependent tissue parameters on acoustic radiation force induced displacements

Abstract:Multiple ultrasound elastography techniques rely on acoustic radiation force (ARF) in monitoring high-intensity focused ultrasound (HIFU) therapy. However, ARF is dependent on tissue attenuation and sound speed, both of which are also known to change with temperature making the therapy monitoring more challenging. Furthermore, the viscoelastic properties of tissue are also temperature dependent, which affects the displacements induced by ARF. The aim of this study is to quantify the temperature dependent changes in the acoustic and viscoelastic properties of liver and investigate their effect on ARF induced displacements by using both experimental methods and simulations. Furthermore, the temperature dependent viscoelastic properties of liver are experimentally measured over a frequency range of 0.1-200 Hz at temperatures reaching 80 C, and both conventional and fractional Zener models are used to fit the data. The fractional Zener model was found to fit better with the experimental viscoelasticity data with respect to the conventional model with up to two magnitudes lower sum of squared errors (SSE). The characteristics of experimental displacement data were also seen in the simulations due to the changes in attenuation coefficient and lesion development. At low temperatures before thermal ablation, attenuation was found to affect the displacement amplitude. At higher temperature, the decrease in displacement amplitude occurs approximately at 60-70 C due to the combined effect of viscoelasticity changes and lesion growth overpowering the effect of attenuation. The results suggest that it is necessary to monitor displacement continuously during HIFU therapy in order to ascertain when ablation occurs.

Abstract:Noninvasive diagnosis of bone density and mechanical properties using non-radiation imaging modality is an emerging area with promising in early prediction of osteopenia and treatment effectiveness in the clinic and functional disuse, i.e., long-term bedrest and space mission. Advances in quantitative ultrasound have shown advantages in measuring both bone density and mechanical strength, non-radiation, imaging capability, and easy to use. The challenge that remained is the poor penetration of ultrasound signals passing through trabecular and cortical bones and acoustic energy scattering. A new scanning confocal ultrasound technology is developed in this lab to detect the alteration of bone to provide diagnostic results in bone density and structure properties. A software-controlled flexible ultrasound system with 2-D dual array transducer is developed and proposed for the purpose of noninvasive bone density diagnosis and assessment of bone loss. Transmitting (Tx) transducer elements are divided into sub-blocks to excite the ultrasound signals in sequence to decrease the system complexity while maintaining beam pattern properties by the signal processing procedure at receiving (Rx) side. Apodization is also applied to reduce acoustic side lobes and to make the resolution in the ultrasound field of view (FOV) more uniform. This study may provide basic understanding of modulated confocal ultrasound beam forming for tissue characterization, such as trabecular bone structual and strength properties.

Abstract:In vivo measurement of the mechanical properties of thin-walled soft tissues (e.g., mitral valve, artery and bladder) and in situ mechanical characterization of thinwalled artificial soft biomaterials in service are of great challenge and difficult to address via commonly used testing methods. Here we investigate the properties of guided waves in immersed pre-stressed plates and tubes, and show that they can address this challenge in a nondestructive manner. To this end, we carry out (i) a theoretical analysis based on incremental wave motion in finite deformation theory (ii) finite element simulations and we validate these results with (iii) experiments on cryogel phantoms immersed in water.

Abstract:Arterial stiffness is highly correlated with the functions of the artery and may serve as an important diagnostic criterion for some cardiovascular diseases. To date, it remains a challenge to quantitatively assess local arterial stiffness in a non-invasive manner. To address this challenge, we investigated the possibility of determining arterial stiffness using the guided circumferential wave (GCW) induced in the arterial wall by a focused acoustic radiation force. The theoretical model for the dispersion analysis of the GCW is presented, and a finite element model has been established to calculate the dispersion curve. Our results show that under described conditions, the dispersion relations of the GCW are basically independent of the curvature of the arterial wall and can be well-described using the Lamb wave (LW) model. Based on this conclusion, an inverse method is proposed to characterize the elastic modulus of artery. Both numerical experiments and phantom experiments had been performed to validate the proposed method. We show that our method can be applied to the cases in which the artery has local stenosis and/or the geometry of the artery cross-section is irregular; therefore, this method holds great potential for clinical use.

Abstract:OBJECTIVE: Acoustic radiation force (ARF)-based approaches to measure tissue elasticity require transmission of a focused high-energy acoustic pulse from a stationary ultrasound probe and ultrasound-based tracking of the resulting tissue displacements to obtain stiffness images or shear wave speed estimates. The method has established benefits in biomedical applications such as tumor detection and tissue fibrosis staging. One limitation, however, is the dependence on applied probe pressure, which is difficult to control manually and prohibits standardization of quantitative measurements. To overcome this limitation, we built a robot prototype that controls probe contact forces for shear wave speed quantification.METHODS: The robot was evaluated with controlled force increments applied to a tissue-mimicking phantom and in vivo abdominal tissue from three human volunteers.RESULTS: The root-mean-square error between the desired and measured forces was 0.07 N in the phantom and higher for the fatty layer of in vivo abdominal tissue. The mean shear wave speeds increased from 3.7 to 4.5 m/s in the phantom and 1.0 to 3.0 m/s in the in vivo fat for compressive forces ranging from 2.5 to 30 N. The standard deviation of shear wave speeds obtained with the robotic approach were low in most cases ( 0.2 m/s) and comparable to that obtained with a semiquantitative landmark-based method.CONCLUSION: Results are promising for the introduction of robotic systems to control the applied probe pressure for ARF-based measurements of tissue elasticity.SIGNIFICANCE: This approach has potential benefits in longitudinal studies of disease progression, comparative studies between patients, and large-scale multidimensional elasticity imaging.

Abstract:Since WLAN (wireless LAN) systems share the 2.4-GHz frequency band with microwave ovens, interference caused by radiated oven noise is a serious problem in practical WLAN application. To mitigate the oven noise interference in DS-SS (direct-sequence spread spectrum) WLAN systems, the use of adaptive filters is proposed. This method is based on the fact that oven noise behaves like CW (continuous wave) interference within a short duration. In contrast to previous reduction techniques for oven noise, this method can be implemented without changing any specifications of current WLAN systems. The results of numerical and experimental analyses clearly demonstrate the effectiveness of adaptive filters for improving the bit error rates of WLAN links subject to oven noise interference.

Abstract:Many cardiovascular diseases can alter arterial stiffness; therefore, measurement of arterial wall stiffness can provide valuable information for both diagnosis of such diseases in the clinic and evaluation of the effectiveness of relevant drugs. However, quantitative assessment of the in vivo elastic properties of arterial walls in a non-invasive manner remains a great challenge. In this study, we found that the elastic modulus of the arterial wall can be extracted from the dispersion curve of the guided axial wave (GAW) measured using the ultrasound elastography method. It is shown that the GAW in the arterial wall can be well described with the Lamb wave (LW) model when the frequency exceeds a critical value fc, whose explicit form is determined here based on dimensional analysis method and systematic finite-element simulations. Further, an inverse procedure is proposed to determine both fc and the elastic modulus of the arterial wall. Phantom experiments have been performed to validate the inverse method and illustrate its potential use in the clinic.

Abstract:Shear wave elastography (SWE) is an ultrasound (US) diagnostic method for measuring the stiffness of soft tissues based on generated shear waves (SWs). SWE has been applied to bulk tissues, but in arteries it is still under investigation. Previously performed studies in arteries or arterial phantoms demonstrated the potential of SWE to measure arterial wall stiffness-a relevant marker in prediction of cardiovascular diseases. This study is focused on numerical modelling of SWs in ex vivo equine aortic tissue, yet based on experimental SWE measurements with the tissue dynamically loaded while rotating the US probe to investigate the sensitivity of SWE to the anisotropic structure. A good match with experimental shear wave group speed results was obtained. SWs were sensitive to the orthotropy and nonlinearity of the material. The model also allowed to study the nature of the SWs by performing 2D FFT-based and analytical phase analyses. A good match between numerical group velocities derived using the time-of-flight algorithm and derived from the dispersion curves was found in the cross-sectional and axial arterial views. The complexity of solving analytical equations for nonlinear orthotropic stressed plates was discussed.

The influence of acetysalicylic acid on changes in platelet function due to physical exertion.

Abstract:The influence of acetylsalicylic acid (ASA) on alterations of the platelets resulting from physical exertion was studied in 10 healthy volunteers. Before and 2 h after the administration of placebo or 1 g ASA, venous blood samples were taken for platelet counts, thrombelastography (TEG) and tests of ADP- and epinephrine-induced platelet aggregation. The blood in which the 2-h values were determined was sampled at the highest work-load during sub-maximum exercise on a bicycle ergometer (5 min each at 50, 100 and 150 watts). ASA had no effect on the alterations of the platelets due to physical effort: the increase in the first phase of epinephrine-induced aggregation, the elevation of the platelet count and the hypercoagulability demonstrable by TEG still persisted after the administration of ASA. Some effects were noted, however, that are also observed after the administration of ASA at rest: the second phase of epinephrine-induced aggregation was suppressed and disaggregation after induction with ADP was accelerated.
Guided waves in pre-stressed hyperelastic plates and tubes: Application to the ultrasound elastography of thin-walled soft materials

Abstract:In vivo measurement of the mechanical properties of thin-walled soft tissues (e.g., mitral valve, artery and bladder) and in situ mechanical characterization of thin-walled artificial soft biomaterials in service are of great challenge and difficult to address via commonly used testing methods. Here we investigate the properties of guided waves generated by focused acoustic radiation force in immersed pre-stressed plates and tubes, and show that they can address this challenge. To this end, we carry out both (i) a theoretical analysis based on incremental wave motion in finite deformation theory and (ii) finite element simulations. Our analysis leads to a novel method based on the ultrasound elastography to image the elastic properties of pre-stressed thin-walled soft tissues and artificial soft materials in a non-destructive and non-invasive manner. To validate the theoretical and numerical solutions and demonstrate the usefulness of the corresponding method in practical measurements, we perform (iii) experiments on polyvinyl alcohol cryogel phantoms immersed in water, using the Verasonics V1 System equipped with a L10-5 transducer. Finally, potential clinical applications of the method have been discussed.
Multichannel Processing for Dispersion Curves Extraction of Ultrasonic Axial-Transmission Signals: Comparisons and Case Studies.

Abstract:Some pioneering studies have shown the clinical feasibility of long bones evaluation using ultrasonic guided waves. Such a strategy is typically designed to determine the dispersion information of the guided modes to infer the elastic and structural characteristics of cortical bone. However, there are still some challenges to extract multimode dispersion curves due to many practical limitations, e.g., high spectral density of modes, limited spectral resolution and poor signal-to-noise ratio. Recently, two representative signal processing methods have been proposed to improve the dispersion curves extraction. The first method is based on singular value decomposition (SVD) with advantages of multi-emitter and multi-receiver configuration for enhanced mode extraction; the second one uses linear Radon transform (LRT) with high-resolution imaging of the dispersion curves. To clarify the pros and cons, a face to face comparison was performed between the two methods. The results suggest that the LRT method is suitable to separate the guided modes at low frequency-thickness-product ( fh) range; for multimode signals in broadband fh range, the SVD-based method shows more robust performances for weak mode enhancement and noise filtering. Different methods are valuable to cover the entire fh range for processing ultrasonic axial transmission signals measured in long cortical bones.
