Abstract:Hirschsprung's disease (HSCR) is a congenital disorder characterized by failure of the neural crest cells to migrate and populate the distal bowel during gestation affecting different lengths of intestine leading to a distal functional obstruction. Surgical treatment is needed to correct HSCR once the diagnosis is confirmed by demonstrating the absence of ganglion cells or aganglionosis of the affected bowel segment. Hirschsprung's disease associated enterocolitis (HAEC) is an inflammatory complication associated with HSCR that can present either in the pre- or postoperative period and associated with increased morbidity and mortality. The pathogenesis of HAEC remains poorly understood, but intestinal dysmotility, dysbiosis and impaired mucosal defense and intestinal barrier function appear to play a significant role. There is no clear definition for HAEC, but the diagnosis is primarily clinical, and treatment is guided based on severity. Here, we aim to provide a comprehensive review of the clinical presentation, etiology, pathophysiology, and current therapeutic options for HAEC.

Abstract:Purpose of review: Hirschsprung's disease (HSCR) is characterized by an absence of ganglion cells in the distal hindgut, extending from the rectum to a variable distance proximally, and results from a failure of cranial-caudal neural crest cell migration. Hirschsprung's-associated enterocolitis (HAEC) is a condition with classic manifestations that include abdominal distention, fever and foul-smelling stools, and is a significant and life-threatening complication of HSCR. The purpose of this review was to critically evaluate recent findings regarding the pathophysiology of HAEC. Recent findings: Several recent studies have investigated the cause of HAEC in humans and mouse models. These studies suggest that alterations in the intestinal barrier, including goblet cell number and function, and Paneth cell function, impaired gastrointestinal mucosal immunity, including B-lymphocyte trafficking or function and secretory immunoglobulin A production, and dysbiosis of the intestinal microbiota may contribute to the development of HAEC. Summary: Recent studies add to the body of literature, suggesting that the intestinal defects observed in HSCR are not restricted to the aganglionic segment but extend to the mucosal immune system within and beyond the gastrointestinal tract. Future studies further dissecting the mechanisms of HAEC and validating these findings in humans will allow for the development of directed therapeutic interventions.

Abstract:Hirschsprung's disease (HSCR) is a common cause of neonatal bowel obstruction and the approach to diagnosis and surgical treatment is well defined and accepted. Hirschsprung's-associated enterocolitis (HAEC) remains a frequent cause of pre-operative and post-operative morbidity and mortality, with unchanged treatment guidelines over multiple decades. Recent advances in our understanding of the genetics underlying HSCR have allowed the development of animal models, some of which recapitulate the HAEC phenotype. These animal models, along with recent translational studies, have implicated multiple facets of mucosal immunity and microbiome dysbiosis in the development of HAEC. Here, we will review the established epidemiology, modes of diagnosis and treatment of HAEC. Furthermore, we will explore emerging concepts in the pathogenesis of this disease; including animal models, alterations in mucosal immunity, dysbiosis of the intestinal microbiome, specific genetic susceptibility, and novel treatment modalities.

Abstract:Introduction Increased duodenal acid exposure (DAE) is associated with ulcer and dyspeptic symptoms but our understanding is limited by cumbersome recording methods. We have recently shown that prolonged radiotelemetry pH monitoring (RpHM) in the duodenum is feasible. This study compared DAE in controls and patients with functional dyspepsia (FD) using RpHM. Methods FD and controls underwent 48-hour recording of DAE using endoclip-secured RpHM capsules in the distal duodenal bulb. Subjects were nil per os except for meals eaten twice daily. FD completed a 14-item symptom checklist. Sleep periods were determined from diaries. Meal periods were defined as the 3-hour period after meal initiation. Results Thirty-four FD and 25 controls were studied. Prolonged recordings were obtained in 32/34 FD and 17/25 controls. Reasons for incomplete recordings were capsule dislodgement (7), procedural complication (1), and suspected gastric prolapse of capsule (2). Within groups, pH values for recording periods did not differ. Between groups, FD had significantly lower mean pH values [median (25th to 75th percentile)] during meals [5.11 (4.44 to 5.59) vs. 5.63 (5.17 to 6.10); P=0.003] and upright periods [4.69 (3.92 to 5.64) vs. 5.35 (4.55 to 6.31); P=0.01] but not during sleep. Duodenal pH values did not correlate with symptoms except for complaints of inability to finish a meal with meal (r=−0.341; P=0.05) and sleep (r=−0.383; P=0.03) pH. Conclusions RpHM allows for prolonged recording of DAE. FD have l greater DAE than controls during daytime and meal periods. DAE and symptoms are poorly associated. The association of DAE with inability to finish a meal is consistent with prior studies demonstrating impaired proximal stomach function after duodenal acidification.

Abstract:Objective:To summarize clinical information and assessment techniques relevant to respiratory therapists caring for adult patients on high-frequency oscillatory ventilation (HFOV). Data Source:Review of observational studies, controlled trials, case reports, institutional experience, and hospital HFOV guidelines for adult patients. Data Summary:Respiratory therapists require unique physical assessment skills and knowledge in managing patients on HFOV. Respiratory therapy procedures relevant to HFOV include setting endotracheal tube cuff leaks, performing lung recruiting maneuvers, endotracheal suctioning, and monitoring ventilator parameters. Respiratory therapists serve as essential team members in the creation and implementation of written HFOV guidelines (e.g., algorithms) to optimize patient care. Conclusion:Respiratory therapy assessment and procedural skills are essential in providing optimal care to adult patients on HFOV.

Abstract:Hirschsprung-associated enterocolitis (HAEC) is a common life-threatening complication of Hirschsprung disease (HSCR). It has been proposed that gut microbiota, which have an essential role in gut-homeostasis, are associated with HAEC. Recent studies demonstrated an increase in alpha diversity of fecal microbiota over time in HSCR mice and a decrease in diversity after surgery. In addition, clinical studies have reported a reduction in bacterial richness in HSCR children after surgery. Some studies revealed a difference in microbiota between the proximal ganglionic and distal aganglionic intestine and found a difference in bacterial character between fecal and colonic specimens. HAEC studies found an increase in Proteobacteria, especially Escherichia and Enterobacteriaceae, with a decrease in Firmicutes and Bifidobacterium in HAEC patients. However, the direction of alpha diversity in HAEC patients is still controversial. The self-comparison of microbiota in treatment periods suggested that probiotics might improve gut dysbiosis and decrease the frequency of enterocolitis, but some reported contradictory findings. This review comprehensively summarizes and discusses key findings from animal and clinical data of the distinct microbiome associated with HCSR and the association of gut dysbiosis with the development of HAEC. This information should be useful in the establishment of novel interventions to improve gut dysbiosis and prevent enterocolitis in HSCR patients.

A Pilot Study: Transcriptional Profiling, Functional Analysis, and Organoid Modeling of Intestinal Mucosa in Hirschsprung Disease

Abstract:Background: Hirschsprung disease (HSCR) is a congenital colonic aganglionosis. Many HSCR patients develop enterocolitis despite surgical resection. The pathophysiology of this inflammatory process is poorly understood. We compared transcriptional profiles and function of ganglionic and aganglionic tissue in HSCR patients. Methods: RNA sequencing was performed on mucosal tissues from HSCR patients (n = 6) and controls (n = 3). Function of matched ganglionic and aganglionic regions were investigated utilizing organoids generated from these tissues. Results: Transcriptional differences observed in ganglionic and aganglionic regions of HSCR patients included upregulation of genes involving inflammation, cell differentiation and proliferation as well as decreased expression of genes encoding mucins compared to controls. Organoids derived from ganglionic and aganglionic regions of HSCR patients were similar in epithelial cell differentiation, epithelial barrier formation and response to stimulation with bacterial metabolites and pro-inflammatory cytokines. Conclusions: Despite normal ganglionic structure, the section of colon adjacent to the aganglionic region in HSCR patients has perturbed gene expression which resembles the aganglionic segment. Transcriptional and functional changes in colonic epithelium are persevered in the ganglionic colon used for pull-through surgery. This may explain persistence of enterocolitis despite surgical excision of aganglionic colon and subsequent endorectal pull-through performed with ganglionic colon during correction of HSCR. Abstract:In a total of 38 operative survivors who underwent tricuspid annuloplasty for functional tricuspid regurgitation, Kay-Boyd's, DeVega's and Carpentier's methods were compared using a newly developed radioisotope technique together with routine examinations. Carpentier's ring method was most effective for all grades and types of regurgitation. Kay-Boyd's method usually left some residual regurgitation and DeVega's semicircular annuloplasty had unpredictable results. Although the results of tricuspid annuloplasty are influenced by the quality of the repair of the primary lesion as well as the technique of tricuspid annuloplasty itself, we recommend Carpentier's method in the first place. However, further study is mandatory for a definite conclusion.

Abstract:Hirschsprung's disease-associated enterocolitis (HAEC) remains the most life-threatening complication in Hirschsprung disease (HD) patients. The pathogenesis of HAEC has not been determined and many hypotheses regarding the etiology of HAEC have been proposed. These include a possible causal relationship between the abnormal enteric nervous system development in HD and the development of enterocolitis. Based on the complex genetic causes of HD that have been discovered and the resultant heterogeneous group of patients that exists, the causes of HAEC are likely multiple. New insights regarding the relationship of the role of the enteric nervous system and its interaction between intestinal barrier function, innate host immunity, and commensal microflora have been discovered, which may shed light on this perplexing problem. This review presents current known risk factors of HAEC and the proposed theories and supporting evidence for the potential etiologies of HAEC.

Abstract:Electrocochleographic changes were investigated together with morphological evaluation of cochlea in guinea pigs which received the antigen challenge by two different procedures after sensitization with type II collagen. Electrocochleogram was recorded from the round window using tone bursts and click as stimuli. Results were as follows: 1) Endolymphatic hydrops could be induced at 4 of 11 guinea pigs received the antigen challenge through the stylomastoid foramen. 2) A characteristic SP finding, that is,-SP recording in response to high frequency tone bursts was obtained in the hydropic guinea pigs. We suppose that the hair cell malfunction due to hydrops and immune responses might be responsible for such an abnormal SP finding. 3) In 3 hydropic animals, abnormal changes in AP, such as threshold elevation, decrease in amplitude and input-output curve with H-curve charater, were observed in addition to abnormal SP. In the cochlea of them, moderate spiral ganglion cell degeneration was demonstrated. 4) No endolympatic hydrops could be. produced in 7 guinea pigs received antigen challenge through stylomastoid foramen and 9 guinea pigs received antigen challenge by intradermal injection into the back, while slight or moderate degeneration of spiral ganglion cells was observed in some animals of them, which showed abnormal changes in AP: thershold elevation, decrease in amplitude, and input-output curve with H-curve pattern. 5) It became apparent that abnormal changes in AP response were closely related to the spiral ganglion cell degeneration. Because the organ of corti showed normal appearance morphologically, it seems very improbable that the abnormality in AP is caused by hair cell loss. 6) Guinea pigs with endolymphatic hydrops and those with spiral ganglion cell degeneration, which were induced by our immunologic method, can serve as the useful model of Meniere's disease and of "neural deafness", respectively. It may be suggested from our results that autoimmune response may probably play an important role on the etiopathogenesis of Meniere's disease and other inner ear diseases.

Abstract:Hirschsprung's disease (HSCR) is a classical model of enteric neuropathy, occurring in approximately 2-2.8 in 10,000 newborns. It is the commonest form of congenital bowel obstruction and is characterized by the absence of enteric ganglia in distal colon. Recent advances in genome-wide association analysis (GWAS) and next generation sequencing (NGS) studies have led to the discovery of a number of new HSCR candidate genes, thereby providing new insights into the genetic architecture and molecular mechanisms of the disease. Altogether, these findings indicated that genetic heterogeneity, variable penetrance and expressivity, and genetic interaction are the pervasive characteristics of HSCR genetics. In this review, we will provide an update on the genetic landscape of HSCR and discuss how the common and rare variants may act together to modulate the phenotypic manifestation. Translating the genetic findings to genetic risk prediction and to optimize clinical outcomes are undoubtedly the ultimate goals for genetic studies on HSCR. From this perspective, we will further discuss the major obstacles in the clinical translation of these latest genetic findings. Lastly, new measures to address these clinical challenges are suggested to advance precision medicine and to develop novel alternative therapies.
Hirschsprung disease associated enterocolitis: a systematic review and meta-analysis

Abstract:Introduction: Hirschsprung's disease-associated enterocolitis (HAEC) remains a substantial morbidity and mortality risk in Hirschsprung's disease. HAEC is a challenge. Its p athophysiology is still a mystery, and no adequate treatment strategy exists. The aim of the study is to analyse the pre-and post-operative, mortality and complications: strictures, anastomotic leak, constipation and incontinence associated with HAEC. Materials and methods: Adjust to the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) 2020; this study met all the requirements and was up-todate. The search approach was online publications between 2013 and 2023 in Pubmed and SagePub. It was decided not to consider review pieces that had already been published and half done. The STATA 18th version was used for metaanalysis. Results: Our search results included 370 PubMed and 149 SagePub articles. Since 2013, 134 PubMed and nine SagePub articles have been obtained, and seven studies have met the criteria. Conclusion: Disorders of intestinal motility in the aganglionic segment and accumulation of faeces disrupt the balanced microbiota population, which are factors of preoperative HAEC. Major congenital anomalies and low birth weight worsen pre-operative HAEC. Pre-operative HAEC can continue and affect the post-operative. Constipation and fecal incontinence are still the main challenges after HSCR surgery.

Neuroimmune regulation mediates the occurrence and development of enteritis, which is an important mechanism. However, there is a lack of review on the role of Neuroimmune regulation in enterocolitis associated with Hirschsprung's disease. Therefore, this paper summarizes the characteristics of the interaction between intestinal nerve cells and immune cells, reviews the neuroimmune regulation mechanism of Hirschsprung's disease associated enterocolitis (HAEC), and looks forward to the potential clinical application value.

Prevalence of Hirschsprung-associated enterocolitis in patients with Hirschsprung disease

Abstract:Abstract Purpose Hirschsprung's associated enterocolitis (HAEC) is a complication of Hirschsprung's Disease (HD) with considerable morbidity and mortality. The variability in presentation leads to a wide variety of the reported prevalence pre-and postoperatively. This systematic review aimed to clarify the prevalence of HAEC in short—(S-HD), long (L-HD), TCA and the type of operation used. Methods A systematic literature-based search for relevant cohorts was performed using Pubmed/Medline, Cochrane Library from its inception to May 2021. Studies reporting on pre-and postoperative enterocolitis, segment length, and surgical procedure (Soave, Swenson, Duhamel) were included. Pooled prevalence and subgroup analysis have been calculated for pre-and postoperative HAEC. Results 4738 articles were identified from the literature search, among which 57 studies, including 9744 preoperative and 8568 postoperative patients, were included. The groups were sorted by length of the aganglionic segment for further analysis. The pooled prevalence for preoperative HAEC was 18.3% for all types, 15.2% for S-HD and 26.1% for TCA. The pooled prevalence for postoperative HAEC was in total 18.2% for all segment lengths and used techniques. Subgroup analysis showed no significant difference in the occurrence of postoperative enterocolitis between the three techniques. Conclusion The prevalence of preoperative HAEC increases with segment length. However, pooled data suggest that the postoperative risk for developing HAEC, independently of the employed method and segment length, is comparable to the preoperative risk.

Abstract:Background:Despite the prevalence of therapies available to patients at highest coronary heart disease risk, only a minority of type 2 diabetes mellitus (T2DM) patients reach desired cholesterol treatment levels, with limited data regarding their outcomes. Objective: To examine “real-world” effectiveness of initiating treatment with either colesevelam or ezetimibe among individuals with evidence of T2DM and hypercholesterolemia (HCh). Key outcomes included treatment patterns and cardiovascular (CV) events. Methods: This retrospective administrative claims-based study utilized medical, pharmacy, and enrollment data linked to laboratory results information from a large United States health plan (January 1, 2006, to March 31, 2011) and included individuals with recorded evidence of T2DM and HCh. The index date was the date of first pharmacy claim for colesevelam or ezetimibe, with cohort assignment based on index medication. Assessments included baseline characteristics, follow-up treatment patterns, and composite CV event, with propensity score matching to correct for sample selection bias. Results: In total, 4231 individuals were identified with evidence of HCh and T2DM (ezetimibe n = 3384; colesevelam n = 847). After matching, the baseline characteristics between cohorts were rendered to be similar. Mean days of persistent medication use was lower with colesevelam compared with ezetimibe (P < 0.001). Compared with ezetimibe, a smaller percentage of individuals in the colesevelam cohort experienced a follow-up composite CV event, and adjusted Cox model results suggested decreased risk (hazard ratio = 0.58; P = 0.004) of a follow-up composite CV event. Conclusion: In this health care database analysis among patients with HCh and T2DM, colesevelam was associated with decreased risk of a composite CV event compared with ezetimibe, despite lower persistence.

Our aim was to define predictive factors of the main postoperative complications: obstructive symptoms (OS) and Hirschsprung-associated enterocolitis (HAEC). In this prospective multicentre cohort study, samples of resected bowel were collected at time of surgery in 18 neonates with short-segment HSCR in tertiary care hospitals. OS and HAEC were noted during postoperative follow-up. We assessed the enteric nervous system and the intestinal epithelial barrier (IEB) in ganglionic segments by combining immunohistochemical, proteomic and transcriptomic approaches, with functional ex vivo analysis of motility and para/transcellular permeability. Ten HSCR patients presented postoperative complications (median follow-up 23.5 months): 6 OS, 4 HAEC (2 with OS), 2 diarrhoea (without OS/HAEC). Immunohistochemical analysis showed a significant 41% and 60% decrease in median number of nNOS-IR myenteric neurons per ganglion in HSCR with OS as compared to HSCR with HAEC/diarrhoea (without OS) and HSCR without complications (p = 0.0095; p = 0.002, respectively). Paracellular and transcellular permeability was significantly increased in HSCR with HAEC as compared to HSCR with OS/diarrhoea without HAEC (p = 0.016; p = 0.009) and HSCR without complications (p = 0.029; p = 0.017). This pilot study supports the hypothesis that modulating neuronal phenotype and enhancing IEB permeability may treat or prevent postoperative complications in HSCR.
