Abstract:Background: Variations in maternal blood parameters, which are mostly induced by the physiological changes that occur during pregnancy, have been reported in different gestational periods. The use of the established reference intervals for healthy adult females leads to the misclassification of healthy pregnant women as abnormal. Our aim was to establish appropriate reference intervals for biochemical, haematological and haemostatic parameters in the first and third trimesters of pregnancy.Methods: We included 565 healthy pregnant women with normal pregnancies. Blood samples were collected for biochemical analyses, complete blood counts and coagulation analyses at 8-12 and 28-37 weeks of gestation. The median and reference intervals (the 2.5th and 97.5th values) were calculated for each parameter during pregnancy and then compared to the established reference intervals for healthy adult females.Results: Significant increases in triglyceride, total cholesterol, low-density lipoprotein cholesterol, uric acid, alkaline phosphatase, white blood cell, mean platelet volume, fibrinogen and D-dimer reference intervals and clear decreases in total protein, albumin, blood urea nitrogen, creatinine, red blood cell, haemoglobin, haematocrit, platelet counts and thrombin time reference intervals were observed during pregnancy. According to the 'n%', most changes were observed beginning in the first trimester. Compared to the established reference intervals, the greatest misclassifications were observed for ALB, ALP and D-Di.Conclusions: Changes in maternal blood parameters during pregnancy were confirmed. We recommend that the reference intervals for most blood parameters be revised to account for the gestational period.

Abstract:The concentration of many biochemical parameters changes significantly during pregnancy and the postpartum period, causing a need for specific reference values for biochemical parameters in these life periods. We have collected blood samples in pregnancy week 18, 28 and 36 and six weeks, four months and six months postpartum from 139 healthy women. Blood samples from 59 healthy never-pregnant women were included as controls. Serum samples were analyzed for 23 common biochemical parameters and reference intervals for pregnancy and postpartum periods were computed with the non-parametric method in accordance with recommendations from IFCC and CLSI. The dynamics of different biochemical parameters during and after pregnancy vary considerably between parameters. Some analytes like serum amylase and HDL cholesterol do not differ from never-pregnant values. Of the 23 parameters, serum alkaline phosphatase, chloride, gamma-glutamyl transferase and sodium and urea which were still significantly changed at six months postpartum compared to never-pregnant women. The physiological changes related to pregnancy and lactation have profound effects on biochemical parameters, denoting the use of specific reference intervals for both pregnancy and the postpartum period.

Reference ranges for lipid profiles in Chinese pregnant women

Abstract:Background Pregnancy is a critical time of altered metabolic function and changed lipid profiles. Maternal hyperlipidemia has been shown to be associated with maternal and offspring adverse outcomes, yet no consistence reference values for lipids exist in pregnancy period, and there are no recommended lipid levels for Chinese pregnant women. The object of this study was to describe lipid levels during pregnancy and establish the reference ranges of lipids in Chinese pregnant women. Methods In total, 14,739 pregnant females and 183,787 non-pregnant females who underwent measurement of serum lipids were taken as parent dataset. For controlling confounding from age and BMI, a 1:1 matched pair analysis was used to compare the difference in lipid levels between pregnant and non-pregnant females. Pairs were matched by BMI (± 1 kg/m 2 ) and age (± 1 year). Multi-index method was used to determine reference ranges in healthy pregnant women with normal pre-pregnancy BMI. Results Concentrations of total cholesterol (TC), triglycerides (TG), low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C) were higher in pregnant women compared to non-pregnant women. With pregnancy development, TC, TG, and HDL-C increased from 1st trimester to 2nd and 3rd trimesters, and LDL-C decreased in the third trimester. The reference ranges were recommended as the levels of TC no greater than 9.02 mmol/L, TG no greater than 5.88 mmol/L; HDL-C no less than 1.03 mmol/L and LDL-C no greater than 5.65 mmol/L. Conclusion Obvious higher lipid concentrations were observed during pregnancy compared to non-pregnant females. Hence, reference ranges of lipid profiles from the general population are not appropriate for pregnant women. Furthermore, this study provided reference ranges of lipid profiles for Chinese pregnant women.
Research for the Reference Ranges of 36 Physicochemical Indexes in the Serum of Healthy Individuals in Hangzhou

Abstract:Objective:The research project was in the interest of establishing the reference intervals of 36 physicochemical indexes for healthy adults in the location of Hangzhou, Zhejiang province. Methods:The respondents of the study were healthy residents in Hangzhou (including its 8 districts and 5 countries), the number of which was 523 (including 257 male and 266 female samples). All the samples were divided into five different age groups (19-29, 30-39, 40-49, 50-64 and 65 or more) to analyze 36 physicochemical indexes in serum. Result:Statistical significance (P>0.05) was obtained among various age groups and genders in ALB, ALT, AST, CK, CL, GGT, HDL,LDL,LIP,NA,TG,UREA,FOL. Only gender-related difference in reference intervals were observed in ALP,CR,TBIL,TP,TRF,UA,Fe,AFP,FER,CEA,CA125,PSA. The levels of ALP, CR, TBIL, UA, Fe, AFP, PSA were higher in male samples than female ones. The levels of TP, TRF, CA125 were higher in female samples. Age-related difference of reference intervals was observed in Ca, GLU, LDH, Mg, P, TC, PTH. GLU, LDH, Mg, TC, PTH have positive correlation with age, while Ca and P have negative correlation. There is no statistical significance in AMY, K, VB12, INS among various age groups or genders. Conclusion:There is significant difference in Fe, TG, HDL, LDL, CR, UA, UREA, AFP, CEA, CA125, PSA, FER between genders, considering healthy residents in Hangzhou. Distinguishing reference interval of those indexes in the classification of sexes is recommended in clinical diagnosis. Levels of UA, UREA, TC, TG, LDL, GGT, ALB of healthy adults in Hangzhou show gaps with related studies in other locations. As a result, it’s necessary to modify native reference intervals in consideration of those indexes.
Reference Standard Of Total Bile Acid Concentration Of Chinese Pregnant Women: Analyses Of 11022 Chinese Pregnant Women

Abstract:The criterion of total bile acid (TBA) concentration for the diagnosis of intrahepatic cholestasis in pregnant women remains debated. The purpose of this study was to establish a reference range of TBA concentration in Chinese pregnant women. Clinical data of Chinese women aged > 18 years with singleton pregnancy were retrospectively analyzed. The blood samples were collected during pregnancy and postpartum, respectively. The concentration of TBA was quantitatively measured by using enzymatic electrochemiluminescence. In total, 11022 pregnant women who attended for antenatal care in our hospital were recruited in this clinical trial. The serum bile acid levels were assayed at the 23rd to 25th, 27th to 29th and 35th to 37th weeks of pregnancy and at 1-3 days postpartum. Different reference ranges for the TBA concentration differs were determined for each trimester. The TBA level was steadily elevated from the 24th (1.54 +/- 0.89 mu mol/L) to 36th week (2.25 +/- 1.38 mu mol/L) of pregnancy and reached the peak level at the 36th week (2.97 +/- 1.28 mu mol/L) of pregnancy, and subsequently tended to decline after birth delivery (2.31 +/- 1.87 mu mol/L). The serum levels of TBA significantly fluctuate during each trimester in pregnant women. The TBA levels increase over the gestational age and then dramatically decline after birth delivery. The reference threshold of TBA in Chinese pregnant women was investigated, which might be refered to the diagnose of liver disorders for pregnant women.

Abstract:Background: Several factors like altitude, age, sex, pregnancy, socioeconomic status, life style and race influence hematological reference interval (RIs), which are critical to support clinical decisions and to interpret laboratory data in research. Currently there are no well-established RIs for cord blood hematological parameters of newborns in Ethiopia. This study aims to generate RIs for umbilical cord blood hematological parameters of newborns from Addis Ababa, Ethiopia. Method: A cross-sectional study was conducted from January 1 to March 31, 2019 on healthy, term newborns (37-42 weeks) with normal birth weight born to apparently healthy pregnant mothers who had met the eligibility criteria. From 139 newborns, 2-3ml cord blood was immediately collected from the clumped cord using EDTA tube. The samples were analyzed using Sysmex KX 21 hematology analyzer. Data was entered and the 2.5th and 97.5th percentiles (upper and lower reference limit) were determined using non parametric method by SPSS version 23. The non-parametric independent Mann-Whitney U test (Wilcoxon rank-sum test) was used to compare the distribution of the parameters between genders, modes of deliveries and gestational age. P value less than 0.05 was considered to declare statistical significance. Result: The median values and 95 % reference interval for umbilical cord blood hematological parameters of newborns were as follows: WBC = 12.4 [6.6-19.4] x109/L, RBC = 4.51 [3.55-5.52] x1012/L, HGB = 15.8 [12.4-19.7] g/dL, HCT = 45.9[37.9-56.3]%, MCV = 102.1[83.9-111.6] fL, MCH = 35.3 [29.4-39.1] pg, MCHC = 34.3 [32.3-37.4] %, PLT = 236 [146-438] x109/L, LYM = 37.5 [16.6-63.0] %, MXD = 7.9[1.7-15.8] %, NEU = 53.7[30.3-78.4] %, RDW = 15.6[12.0-19.0]%, PDW = 11.0[9.1-15.7]% and MPV = 9.4[8.1-11.8] fL. The current study found no significant difference between genders, except RDW (P = 0.01), and gestational age group, but there was significant difference for WBC (p = 0.007), RBC (p = 0.018) and Absolute NEU (p = 0.001) by delivery type where newborns delivered through caesarean section had lower values for these three parameters compared to those with spontaneous delivery. Conclusions: hematological reference intervals in cord blood were established for the first time from healthy newborns of Addis Ababa and its surrounding. The values are applicable for newborns from this area. Larger study throughout the country is warranted.

Abstract:Accurate reference intervals generated from an apparently healthy population and stratified by crucial variables such as age and gender are required to guarantee appropriate interpretation of test results. Since there were no local reference intervals in the study area, the present study aimed to establish reference intervals on serum lipid profiles and electrolytes for children and adolescents in Addis Ababa. Hence, the established reference intervals in the current study revealed that both the lower and upper limits contradicted the manufacturer values as well as the available literature. Background Accurate reference intervals generated from an apparently healthy population and stratified by crucial variables such as age and gender are required to guarantee appropriate interpretation of test results. Since there were no local reference intervals in the study area, the present study aimed to establish reference intervals on serum lipid profiles and electrolytes for children and adolescents in Addis Ababa. Methods This community‐based cross‐sectional study was conducted from April to October 2019. Laboratory analysis was performed using the automatic biochemical analyzer Cobas 6000 (c501) from Roche. According to Clinical and Laboratory Standards Institute (CLSI) guidelines, reference intervals for lipid profile and electrolyte tests for apparently healthy children and adolescents were established. We used a non‐parametric method to calculate the 2.5th and 97.5th percentiles with a 90% confidence interval. Results In children, the reference intervals for serum potassium, sodium, chloride, calcium, magnesium, and phosphate in mmol/L were 4.37–5.20, 137–145.50, 101.90–107.90, 2.34–2.70, 0.74–0.97, and 1.42–1.85, respectively; and for total cholesterol, triglycerides, low‐density lipoprotein, and high‐density lipoprotein, the respective values were 100.76–171.70, 44.16–126.36, 60.60–105.60, and 31.60–53.70 in mg/dL, for both genders. For adolescents, the reference intervals were 4.03–5.58, 137–146, 98.90–120.90, 2.39–2.70, 0.73–0.96, and 0.96–1.80 for serum potassium, sodium, chloride, calcium, magnesium, and phosphate in mmol/L, respectively; and 97.20–189.10, 40.50–143.60, 41.70–120.90, and 21.30–57.0 in mg/dL for total cholesterol, triglycerides, low‐density lipoprotein, and high‐density lipoprotein, respectively, for both genders. Conclusion The established reference intervals in the current study revealed that both the lower and upper limits contradicted the manufacturer values as well as the available literature. The study also discovered significant gender differences in reference values for TC, TG, LDL‐C, potassium, phosphate, and chloride in the adolescent age groups.

Abstract:Background: Preeclampsia, a pregnancy-specific complication occurring in the second half of human pregnancy and one of the leading causes for perinatal mortality and morbidity, is characterized by the onset of hypertension and proteinuria. We measured circulating adipocyte fatty acid binding protein (AFABP) and retinol binding protein-4 (RBP-4) in patients with preeclampsia.Methods: Twenty-seven healthy non-pregnant women, 27 healthy pregnant women at third trimester and 26 women with preeclampsia were recruited and blood samples were taken. Concentrations of serum AFABP and RBP-4 were measured with ELISA.Results: There were significant differences in serum AFABP (median: 0.99, 0.93 and 1.81 ng/ml, respectively, P < 0.001) and RBP-4 (mean: 3.16, 2.65 and 4.27 ng/ml, respectively, P = 0.022) among non-pregnancy, normal pregnancy and preeclampsia. Serum AFABP was significantly higher in preeclampsia than non-pregnancy (P < 0.001) and normal pregnancy (P = 0.001). Serum RBP-4 was significantly higher in preeclampsia than normal pregnancy (P = 0.007) but not non-pregnancy (P = 0.061). There were no significant differences in serum RBP-4 and AFABP between non-pregnancy and normal pregnancy. Serum RBP-4 was significantly higher in severe than mild preeclampsia (mean: 5.15 vs 2.84 ng/ml, P = 0.046). There was no significant difference in serum AFABP between mild and severe preeclampsia.Conclusion: Increased circulating AFABP and RBP-4 concentrations were demonstrated, suggesting it be an important pathophysiology of preeclampsia. [Analysis of Serum Lipids Levels and the Establishment of Reference Intervals for Serum Lipids in Middle and Late Pregnancy].

Abstract:OBJECTIVE:To explore the serum lipids levels in healthy pregnant women, and to establish the reference intervals of serum lipids in middle and late pregnancy.METHODS:Triglyceride (TG), total cholesterol (TCH), high density lipoprotein (HDL), low density lipoprotein (LDL), apo-lipoprotein-A(APO-A) and apo-lipoprotein-B (APO-B) were measured in 3 200 pregnant women and 3 200 healthy women of childbearing age(the control group) from January 2014 to Febuary 2015 in Obstetrics and Gynecology Hospital of Fudan University. In the healthy pregnant women, serum lipids were measured at 14-20, 24-28 and 37-40 gestational weeks, respectively. All the parameters were detected by Hitachi 7180 automatic biochemical analyzer. The test results were calculated and determined by the C28-A3 standard of the National Clinical and Laboratory Standards Institute. And the normal reference intervals of serum lipids in middle and late pregnancy were defined as 2.5%-97.5%.RESULTS:(1) The levels of TG, TCH, HDL, LDL, APO-A and APO-B in the control group were 0.8, 4.2, 1.0, 2.7 mmol/L and 1.1, 0.8 g/L, respectively. The levels of TG, TCH, HDL, LDL, APO-A and APO-B in middle and late pregnancy were significantly higher than those in the control group (P<0.05). (2) The serum lipids levels at 14-20, 24-28 and 37-40 gestational weeks in healthy pregnant women were compared with the control group as following. The TG levels were 1.9, 3.8 and 4.4 folds of the control group; the TCH levels were 1.1, 1.5 and 1.5 folds of the control group; the HDL levels were 1.2, 1.6 and 1.5 folds of the control group; the LDL levels were 1.1, 1.4 and 1.4 folds of the control group; the APO-A levels were 1.3, 1.4 and 1.5 folds of the control group; and the APO-B levels were 1.1, 1.5 and 1.5 fold of the control group respectively. The TG level was the most increased, and it increased gradually with gestational age (P<0.01). (3) The median of LDL to HDL cholesterol ratio in the healthy pregnancy group at 14-20, 24-28 and 37-40 gestational weeks were 2.7, 2.5, 2.6, respectively, which were significantly lower than that of the control group (2.8; P<0.05). (4) Reference intervals of serum lipids at 14-20, 24-28 and 37-40 gestational weeks in healthy pregnant women were as following. The TG levels were 0.7-3.9, 1.7-6.3 and 1.6-8.1 mmol/L, respectively; the TCH were 3.3-6.9, 4.3-8.3, 4.3-8.7 mmol/L, respectively; the HDL were 0.8-1.8, 1.0-2.1 and 1.0-2.1 mmol/L, respectively; the LDL were 2.1-4.5, 2.7-5.1 and 2.6-5.2 mmol/L, respectively; the APO-A were 1.1-1.8, 1.2-1.9 and 1.1-2.4 g/L, respectively; and the APO-B were 0.6-1.4, 0.9-1.8 and 0.8-2.1 g/L, respectively. The LDL/HDL ratios were 2.3-3.1, 2.2-2.9 and 2.1-3.0, respectively.CONCLUSIONS:Serum lipids increased physiologically with gestational age in middle and late pregnancy. The establishment of reference intervals for serum lipids in pregnancy will help to distinguish abnormal serum lipid levels in middle and late pregnancy.

Abstract:OBJECTIVE:To observe the changes of complete blood count (CBC) parameters during pregnancy and establish appropriate reference intervals for healthy pregnant women. METHODS:Healthy pregnant women took the blood tests at all trimesters. All blood samples were processed on Sysmex XE-2100. The following CBC parameters were analyzed: red blood cell count (RBC), hemoglobin (Hb), hematocrit (Hct), mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), red blood cell distribution width (RDW), platelet count (PLT), mean platelet volume (MPV), platelet distribution width (PDW), white blood cell count (WBC), and leukocyte differential count. Reference intervals were established using the 2.5th and 97.5th percentile of the distribution. RESULTS:Complete blood count parameters showed dynamic changes during trimesters. RBC, Hb, Hct declined at trimester 1, reaching their lowest point at trimester 2, and began to rise again at trimester 3. WBC, neutrophil count (Neut), monocyte count (MONO), RDW, and PDW went up from trimester 1 to trimester 3. On the contrary, MCHC, lymphocyte count (LYMPH), PLT, and MPV gradually descended during pregnancy. There were statistical significances in all CBC parameters between pregnant women and normal women, regardless of the trimesters (P<.001). The median obtained were (normal vs pregnancy) as follows: RBC 4.50 vs 3.94×1012 /L, Hb 137 vs 120 g/L, WBC 5.71 vs 9.06×109 /L, LYMPH% 32.2 vs 18.0, Neut% 58.7 vs 75.0, and PLT 251 vs 202×109 /L. CONCLUSION:The changes of CBC parameters during pregnancy are described, and reference intervals for Beijing pregnant women are demonstrated in this study.

Abstract:BACKGROUND: Leucine arylamidase (LAP) is a type of proteinase. During pregnancy, the LAP value of women will continue to rise, but there is currently no established normal range for this value.METHODS: A total of 26,208 records of LAP information from pregnant women who visited Qingdao Municipal Hospital between 2016 and 2022 were selected, including 25,582 records of healthy pregnant women. The study used the AU5800 Series Clinical Chemistry Analyzer as the detection equipment. The Wilcoxon method was appropriate for calculating confidence intervals in this study since LAP values in pregnant women have a skewed distribution.RESULTS: The reference ranges obtained were 64.00 - 70.50 U/L, 161.00 - 166.00 U/L, and 174.50 - 182.50 U/L for the first trimester, second trimester, and third trimester, respectively.CONCLUSIONS: LAP can rise significantly during pregnancy, so it is essential to establish an appropriate reference range for pregnant women. Abnormal values of LAP carry a risk of developing various pregnancy disorders.

Abstract:BACKGROUND The examination of 24-hour urinary total protein excretion is an essential parameter used for evaluating renal function and early detection of gestational complications. However, data on reference ranges of 24-hour urinary total protein excretion in normal pregnancy are scarce. OBJECTIVE The objective was to determine reference ranges for 24-hour urinary total protein excretion in a population with uncomplicated singleton pregnancies using a standard method for urinary total protein. Additionally, the values of 24-hour urinary total protein were stratified by maternal age and prepregnancy body mass index. STUDY DESIGN This study was based on a prospective cohort study in Shenzhen, China. The pregnant women were enrolled at their first prenatal clinical visit. All the participants were instructed to collect 24-hour urine samples during successive gestational periods: 6 +0 - 13 +6 weeks, 14 +0 - 27 +6 weeks, and 28 +0 - 41 +6 weeks. Total urinary protein excretion was analyzed by a turbidimetric immunoassay. Ultimately, the study encompassed a total of 4,844 pregnant women with uncomplicated pregnancies. The nonparametric percentile method was employed to determine reference ranges for 24-hour urinary total protein excretion during different trimesters in women with uncomplicated pregnancies (excluding those with previous kidney disorders, gestational or chronic hypertension, preeclampsia, and pregestational diabetes mellitus, among others). RESULTS The 24-hour urinary total protein levels expressed as medians and percentiles (5th, 95th) for each trimester were as follows: 72.0 (28.4, 165.0), 88.0 (34.0, 185.0), and 108.0 (37.5, 258.0) mg in the first, second, and third trimesters, respectively. A significant increase in 24-hour urinary total protein excretion was observed throughout pregnancy (all P values < 0.001). Moreover, 24-hour UTP levels were higher in the older (age ≥ 35 years) group than in the younger (age < 35 years) group from mid-gestation. Specifically, the median (with interquartile ranges, IQR) 24-hour urinary total protein levels by age were 72.2 (IQR 50.6-100.0) vs. 70.5 (IQR 50.5-100.0) mg, 85.8 (IQR 62.0-117.0) vs. 96.0 (IQR 68.0-127.8) mg, and 106.6 (IQR 76.0-146.2) vs. 114.7 (IQR 81.5-153.6) mg in the first, second, and third trimesters, respectively. Additionally, 24-hour proteinuria was significantly increased in higher-weight subgroups (overweight or obese women) compared to lower-weight subgroups (underweight or normal-weight women) (all P values < 0.05). CONCLUSION Our study provides reference values for 24-hour urinary total protein excretion with apparently uncomplicated pregnancies. Understanding these changes in low-risk pregnancies is essential for optimizing maternal management.

Numerical experiments of the planar equal-mass three-body problem: the effects of rotation.

Abstract:The planar three-body problem with angular momentum is numerically and systematically studied as a generalization of the free-fall problem (i.e., the three-body problem with zero initial velocities). The initial conditions in the configuration space exhaust all possible forms of a triangle, whereas the initial conditions in the momentum space are chosen so that position vectors and momentum vectors are orthogonal. Numerical results are organized according to the value of virial ratio k defined as the ratio of the total kinetic energy to the total potential energy. Final motions are mapped in the initial value space. Several interesting features are found. Among others, binary collision curves seem to spiral into the Lagrange point, and for large k, binary collision curves connect the Lagrange point and the Euler point. The existence of a lunar periodic orbit and a periodic orbit of petal-type is suggested. The number of escape orbits as a function of the escape time is analyzed for different k. The behavior of this number for different time and k shows most remarkably the effects of rotation of triple systems. The number of escape orbits increases exponentially for k<or=0.1, the number seems to saturate for k=0.3-0.5, and most of the escape orbits escape quickly for k=0.9. The area of chaotic motion is small for k=0.9.

Abstract:OBJECTIVES: To review systematically current literature on kidney function changes during pregnancy, in order to estimate the extent of adaptation over the course of both healthy physiological and complicated singleton pregnancies, and to determine healthy pregnancy reference values.METHODS: PubMed (NCBI) and EMBASE (Ovid) electronic databases were searched, from inception to July 2017, for studies on kidney function during uncomplicated and complicated pregnancies. Included studies were required to report a non-pregnant reference value of kidney function (either in a non-pregnant control group or as a prepregnancy or postpartum measurement) and a pregnancy measurement at a predetermined and reported gestational age. Kidney function measures assessed were glomerular filtration rate (GFR) measured by inulin clearance, GFR measured by creatinine clearance and serum creatinine level. Pooled mean differences between pregnancy measurements and reference values were calculated for predefined intervals of gestational age in uncomplicated and complicated pregnancies using a random-effects model described by DerSimonian and Laird.RESULTS: Twenty-nine studies met the inclusion criteria and were included in the analysis. As early as the first trimester, GFR was increased by up to 40-50% in physiological pregnancy when compared with non-pregnant values. Inulin clearance in uncomplicated pregnancy was highest at 36-41 weeks, with a 55.6% (53.7; 95% CI, 44.7-62.6 mL/min) increase when compared with non-pregnant values, and creatinine clearance was highest at 15-21 weeks' gestation, with a 37.6% (36.6; 95% CI, 26.2-46.9 mL/min) increase. Decrease in serum creatinine level in uncomplicated pregnancy was most prominent at 15-21 weeks, with a 23.2% (-0.19; 95% CI, -0.23 to -0.15 mg/dL) decrease when compared with non-pregnant values. Eight studies reported on pregnancies complicated by a hypertensive disorder. Meta-regression analysis showed a significant difference in all kidney function parameters when comparing uncomplicated and hypertensive complicated pregnancies.CONCLUSIONS: In healthy pregnancy, GFR is increased as early as the first trimester, as compared with non-pregnant values, and the kidneys continue to function at a higher rate throughout gestation. In contrast, kidney function is decreased in hypertensive pregnancy. © 2018 The Authors. Abstract:Abstract Established reference intervals (RIs) of coagulation parameters generally based on the general population are not applicable to specific women. In order to accurately evaluate the coagulation status of non-pregnant women and pregnant women, specific RIs should be established. Our study recruited 465 non-pregnant women and 1972 pregnant women aged 20–45 years. Eight tests including antithrombin (AT), protein C (PC), free protein S (fPS), lupus anticoagulant (LA), D-dimer, fibrin/fibrinogen degradation products (FDP), coagulation factor VII (FVII), and factor VIII (FVIII) were performed on ACL TOP automated coagulation instrument. The RIs for these tests were established in non-pregnant and pregnant women at different gestational weeks. Compared to the non-pregnant group, the medians of AT and fPS were lower, while the medians of PC, LA normalized ratio, D-dimer, FDP, FVII, and FVIII were higher. During pregnancy, AT and fPS activity showed a decreasing trend, with the increase of gestational age. PC activity, LA normalized ratio, D-dimer concentrations, FDP concentrations, FVII, and FVIII activity presented an increasing trend, with the increase of gestational age. The non-pregnant women-specific RIs and the gestational age-specific RIs of AT, PC, fPS, LA normalized ratio, D-dimer, FDP, FVII, and FVIII needed to be established for accurate clinical diagnoses.

New trimester-specific reference intervals for clinical biochemical tests in Taiwanese pregnant women-cohort of TMICS

Abstract:Background Because there are no published biochemical reference intervals (RI) for pregnant Taiwanese women, we used an established islandwide birth cohort, the Taiwan Maternal and Infant Cohort Study, to establish RIs for important biochemical parameters in women during their 3rd trimester in Taiwan. Additionally, we compared the differences in these biochemical parameters between early third trimester (weeks 28 to 31) and late third trimester (weeks 37 to 40) of pregnant women as well as the differences in them between the third trimester and after delivery. Methods Between 2012 and 2015, we recruited a total of 2,136 pregnant women from nine hospitals located in northern (n = 3), central (n = 3), southern (n = 2), and eastern Taiwan (n = 1) to receive regular prenatal health examinations during their third trimester (weeks 28 to 40). After exclusion, samples obtained from 993 eligible pregnant women were analyzed. Results There were increases in both lower and upper normal limits for blood neutrophil, thyroid profile (triiodothyronine (T3) and thyroxine (T4)), testosterone, estradiol, and progesterone and decreases for RBC, hemoglobin (Hb), alanine aminotransferase (ALT) and creatinine (Cr) during their third trimesters. Women in their late third trimester (n = 378) had higher median RBC, Hb, aspartate aminotransferase (AST), Cr, thyroid-stimulating hormone (TSH), testosterone, estradiol, and progesterone and lower median platelet and insulin, compared with those in their early third trimester (n = 490). Twenty-three of the women had both third trimester and post-pregnancy data. After delivery, the women had lower median AST, ALT, insulin, T3, T4, testosterone, estradiol, and progesterone and higher median Cr, free T4, FSH, and luteinizing hormone (LH), compared to their third trimesters. Conclusions Gestation-related changes in important biochemical parameters should be considered when evaluating clinical laboratory values in pregnant women.

Abstract:Profound physiological changes during pregnancy may affect the requirement of retinol and tocopherol, which are essential micronutrients for the maintenance of maternal health and foetal development. However, the current reference intervals (RIs) of retinol and tocopherol are based on non-pregnant population. In the present study, a liquid chromatography-tandem mass spectrometry quantitation method for serum retinol and alpha-tocopherol was established and validated. In addition, we established trimester-specific RIs of retinol and alpha-tocopherol using the data from paired screening test for 31,301 outpatients who participated in the prenatal vitamins A/E evaluation program at our hospital using the Hoffmann method, which is a simple indirect RI estimation method that does not require the recruitment of healthy subjects. Further, to explore the associations between the levels of retinol and alpha-tocopherol and the parameters of complete blood count (CBC), the results of retinol, alpha-tocopherol, and CBC of 1,977 pregnant outpatients in the third trimester were analysed. The testing interval between the levels of vitamins and CBC was no more than 7 days. Although no significant changes were noticed in the levels of retinol, the alpha-tocopherol levels continuously increased with normal physiological changes throughout pregnancy. Lower retinol levels were associated with the higher incidence of anaemia, whereas higher levels of retinol and lower levels of alpha-tocopherol were associated with higher platelet count.

Abstract:Abstract Background Androgen could impact cervical remodeling during pregnancy, and a higher level is associated with adverse pregnancy outcomes. A population-based gestation age-specific reference interval of total testosterone (TT), androstenedione (A4), and 17-hydroxyprogesterone (17-OHP) can help to diagnose maternal hyperandrogenism. Methods We enrolled 600 healthy Chinese women to obtain longitudinal serum samples across gestation. The serum androgen profile was measured by liquid chromatography-tandem mass spectrometry. The equations for medians of TT, A4, and 17-OHP were generated by MedCal, and the variances adjusted for 2-level modeling were generated by MLwiN, a system for the specification and analysis of a range of multilevel models. Results A4 and TT levels increased across the gestation, and they closely correlated with each other (R = .90, P = <.001), whereas the 17-OHP level decreased from the 5th gestational week to the 16th gestational week and then increased afterward toward the end of pregnancy. Women diagnosed with preeclampsia (PE) were found to have a significantly higher level of A4, TT, and 17-OHP when compared with non-PE cases with P≤ .01, whereas mothers carrying male vs female fetuses have comparable levels of A4, TT, and 17-OHP. Conclusion The study highlights a methodology for constructing gestational age-specific TT, A4, and 17-OHP levels to provide a better interpretation of results in a cohort of healthy Chinese women. The observation in PE supports previous findings, and the higher levels of TT, A4, and 17-OHP were observed before the onset of PE.
