Abstract:H transplantation remains the strategy allowing the largest therapeutic benefit in selected patients with advanced heart failure, even in the era of long-term mechanical circulatory support. The major limitation to this therapeutic approach, however, is the large discrepancy between need and availability of suitable organs. In particular, heart procurement suffers from the additional limitation of reduced organ-specific suitability for transplantation: heart is procured and successfully transplanted in only 15% to 40% of effective organ donors (Figure 1A). In the latest years, legislation of several countries permitted organ donation after circulatory death (DCD). The majority of DCD donors are patients in whom poor general clinical conditions deemed futile prolongation of intensive care, thus allowing withdrawal of life support (Maastricht category III). Despite ethically accepted even by traditionally strict moral conceptions, withdrawal of care is not permitted in several countries, including, for example, Spain, Italy, and France. In countries that specifically developed a DCD donor program in the context of withdrawal of care protocols, this approach lead to marked increase in organ transplantation (Figure 1B). However, utilization of hearts from DCD donors has not become a routine clinical practice because the severity of ischemic myocardial injury suffered during the cardiac arrest and the subsequent standoff period, which would not allow recovery of cardiac function with the standard preservation and transportation techniques. Opposing this concept, several experimental, and now clinical, experiences demonstrated the feasibility of human heart transplantation from DCD donors, thanks to the availability of a device for distant transportation and recovery of circulatory function by ex vivo perfusion of the procured heart. Abstract:Purpose: Exercise therapy and key regulators of bone quality exert anti-hyperglycemic effects on type 2 diabetes mellitus (T2DM) mice. A number of programs have been reported to have an effect on bone disease in T2DM. Major unanswered questions concern the potential correlation of exercise with the improvement of bone quality in T2DM mice and how the nonlinear optical properties of bone are correlated with changes to its crystal structure. Methods: Subjects were randomly divided into six groups: 1) control (C) group, which was fed a normal diet ( n = 8); 2) T2DM quiet group, which was given a high-fat diet and quiet ( n = 8); 3) T2DM plus swimming (T2DM+S) group, which received T2DM and swim training ( n = 8); 4) T2DM plus resistance exercise (T2DM+RE) group, which was given T2DM and resistance exercise ( n = 8); 5) T2DM plus aerobic exercise (T2DM+AE) group, with T2DM and medium-intensity treadmill exercise ( n = 8); and 6) T2DM plus high-intensity interval training (T2DM+HIIT), with T2DM and high-intensity variable-speed intervention ( n = 8). The levels of runt-related transcription factor 2 ( RUNX2 ), osterix ( OSX ), and alkaline phosphatase ( ALP ), as well as the bone microstructure and morphometry, were measured at the end of the 8-week exercise intervention. Results: Compared with the C group, the bone microstructure indexes [bone mineral density (BMD), bone volume/tissue volume (BV/TV), cortical thickness (Ct.Th), and connectivity density (Conn.D)], the bone biomechanical properties (maximum load, fracture load, yield stress, and elastic modulus), and the osteogenic differentiation factors ( RUNX2 , OSX , and BMP2 ) of the T2DM group were significantly decreased (all p < 0.05). Compared with the T2DM group, there were obvious improvements in the osteogenic differentiation factor ( OSX ) and Th.N, while the separation of trabecular bone (Tb.Sp) decreased in the T2DM+AE and T2DM+HIIT groups (all p < 0.05). In addition, the bone microstructure indicators BV/TV, tissue mineral density (TMD), Conn.D, and degree of anisotropy (DA) also increased in the T2DM+HIIT group, but the yield stress and Ct.Th deteriorated compared with the T2DM group (all p < 0.05). Compared with the T2DM+S and T2DM+RE groups, the BV/TV, trabecular number (Tb.N), Tb.Sp, and Conn.D in the T2DM+AE and T2DM+HIIT groups were significantly improved, but no significant changes in the above indicators were found between the T2DM+S and T2DM+RE groups (all p < 0.05). In addition, the BMD and the expression of ALP in the T2DM+AE group were significantly higher than those in the T2DM+HIIT group (all p < 0.05). Conclusion: There was a significant deterioration in femur bone mass, trabecular bone microarchitecture, cortical bone geometry, and bone mechanical strength in diabetic mice. However, such deterioration was obviously attenuated in diabetic mice given aerobic and high-intensity interval training, which would be induced mainly by suppressing the development of T2DM. Regular physical exercise may be an effective strategy for the prevention of not only the development of diabetes but also the deterioration of bone properties in patients with chronic T2DM.

A comprehensive study of long-term skeletal changes after spinal cord injury in adult rats

Abstract:Spinal cord injury (SCI)-induced bone loss represents the most severe osteoporosis with no effective treatment. Past animal studies have focused primarily on long bones at the acute stage using adolescent rodents. To mimic chronic SCI in human patients, we performed a comprehensive analysis of long-term structural and mechanical changes in axial and appendicular bones in adult rats after SCI. In this experiment, 4-month-old Fischer 344 male rats received a clinically relevant T13 contusion injury. Sixteen weeks later, sublesional femurs, tibiae, and L4 vertebrae, supralesional humeri, and blood were collected from these rats and additional non-surgery rats for micro-computed tomography (µCT), micro-finite element, histology, and serum biochemical analyses. At trabecular sites, extreme losses of bone structure and mechanical competence were detected in the metaphysis of sublesional long bones after SCI, while the subchondral part of the same bones showed much milder damage. Marked reductions in bone mass and strength were also observed in sublesional L4 vertebrae but not in supralesional humeri. At cortical sites, SCI induced structural and strength damage in both sub- and supralesional long bones. These changes were accompanied by diminished osteoblast number and activity and increased osteoclast number and activity. Taken together, our study revealed site-specific effects of SCI on bone and demonstrated sustained inhibition of bone formation and elevation of bone resorption at the chronic stage of SCI.

Abstract:Diabetic osteoporosis (DO) is a complication of diabetes mellitus. Our previous study showed that silibinin can attenuate high glucose mediated human bone marrow stem cells dysfunction through antioxidant effect. However, no study has yet investigated the effect of silibinin in diabetic rats. Therefore, we assessed the effects of silibinin on bone characteristics in streptozotocin-induced diabetic rats. The aim of our study was to determine whether providing silibinin in the different supplementation could prevent bone loss in diabetic rats or not. Rats were randomly divided into four groups: (1) control group (CG) (n=10); (2) diabetic group (DG) (n=10); (3) diabetic group with 50mgkg−1day−1 of silibinin orally (DG-50) (n=10); and (4) diabetic group with 100mgkg−1day−1 of silibinin orally (DG-100) (n=10). 12 weeks after streptozotocin (STZ) injection, the femora from all rats were assessed and oxidative stress was evaluated. Bone mineral density was significantly decreased in diabetic rats; these effects were prevented by treatment with silibinin (100mgkg−1day−1 orally). Similarly, in the DG and DG-50 groups, changes in microarchitecture of femoral metaphysis assessed by microcomputed tomography demonstrated simultaneous existence of diabetic osteoporosis; these impairments were prevented by silibinin (100mgkg−1day−1 orally). In conclusion, silibinin supplementation may have potential use as a possible therapy for maintaining skeletal health and these results can enhance the understanding of diabetic osteoporosis induced by diabetes.

Abstract:Type 2 diabetes interferes with bone remodeling mechanisms, requiring studies to reverse this damage, and resveratrol is a polyphenol with rich properties. This study aimed to characterize the long bone morphology and peri-implant biomechanics of normoglycemic and type 2 diabetic animals treated with resveratrol. Thirty-two male Wistar rats were used and divided into normoglycemic and diabetic with or without treatment. They had the installation of implants in the tibia and treatment with oral resveratrol within 45 days. Resveratrol was responsible for weight homeostasis and decreased glycemic levels in rats with type 2 diabetes. The three-point bending testing, resveratrol showed positive effects on the biomechanics of long bones, corroborating a more resistant bone in comparison to untreated diabetics. Micro-ct revealed how bone metabolism is affected by systemic disease, decreasing bone quality. The counter-torque of normoglycemic animals showed superior osseointegration to diabetes, with no differences in the administration of the polyphenol, showing the sovereignty of the deleterious effects of the disease when there is a tissue lesion and an inflammatory picture installed. Overall, resveratrol acted positively in the etiopathogenesis of type 2 diabetes and revealed positive effects on the strength of long bones.

Abstract:The acute reactions of skin of the thigh-leg of normal young adult C3Hf/Sed mice following five equal radiation doses (60Co) were studied in mice irradiated at various leg skin temperatures and while respiring various gas mixtures in control or anesthetized condition (sodium pentobarbital, 0.05 mg/g-1 body wt.) At 25 and 35 degrees C, anesthesia reduced the RD50 (2.5+ reaction) by 4-5%. An increase of temperature from 25 to 35 degrees C resulted in a decrease in RD50 by 10-12% for subjects respiring air or 5% for subjects respiring O2 at 1 or at 3 ATA. The major modifier of radiation response was to change from respiration of air to O2 at 1 or 3 ATA. Enhancement ratios for RD50 Air/O2 1 ATA were 1.4-1.5 for 25 and 35 degrees C. The ratios O2 1 ATA/3 ATA were 1-1.05.
Effects of moderate-intensity aerobic training on cardiac structure and function in type 2 mellitus diabetic rats: Based on echocardiography and speckle tracking

Abstract:Purpose: The present study aimed to evaluate the cardiac function changes using Layer-specific Speckle-tracking echocardiography (LS-STE) induced by Moderate-intensity aerobic training (MIAT) in Type 2 diabetes mellitus (T2DM) rats. Methods: Twenty-six rats were divided into four groups: the Control group (Con), the Training control group (CT), the T2DM group (DM), and the T2DM training group (DT). The CT and DT groups underwent an 8 weeks MIAT. Cardiac structure and function were evaluated by echocardiography and LS-STE. Results: Compared with the Con group, left atrial diameter (LAD), left ventricular end-diastolic diameter(LVEDD), relation wall thickness (RWT), and left ventricular mass (LVM) were significantly higher, and the endo-, mid-, and epi-longitudinal strain (LS), global radial strain (GRS), and endo- and mid-circumferential strain (CS) were significantly lower in DM rats (all p < 0.05). The endo-, mid-, and epi-LS, GRS, endo- and mid-CS were increased in DT rats compared with DM rats (all p < 0.05). Compared with Con rats, CT rats had a significant increase in LVEDD and LVM (all p < 0.05), meanwhile myocardial strains had no significant differences (all p > 0.05). Conclusion: LS-STE was a sensitive method to assess subclinical myocardial changes in T2DM rats. MIAT had the benefit of reversing cardiac systolic subclinical dysfunction in T2DM rats.

Abstract:Type 2 diabetes mellitus (T2DM) presents a challenge for individuals today, affecting their health and life quality. Besides its known complications, T2DM has been found to contribute to bone/mineral abnormalities, thereby increasing the vulnerability to bone fragility/fractures. However, there is still a need for appropriate diagnostic approaches and targeted medications to address T2DM-associated bone diseases. This study aims to investigate the relationship between changes in gut microbiota, T2DM, and osteoporosis. To explore this, a T2DM rat model was induced by combining a high-fat diet and low-dose streptozotocin treatment. Our findings reveal that T2DM rats have lower bone mass and reduced levels of bone turnover markers compared to control rats. We also observe significant alterations in gut microbiota in T2DM rats, characterized by a higher relative abundance of Firmicutes (F) and Proteobacteria (P), but a lower relative abundance of Bacteroidetes (B) at the phylum level. Further analysis indicates a correlation between the F/B ratio and bone turnover levels, as well as between the B/P ratio and HbA1c levels. Additionally, at the genus level, we observe an inverse correlation in the relative abundance of Lachnospiraceae. These findings show promise for the development of new strategies to diagnose and treat T2DM-associated bone diseases.

Abstract:Objective: The objective of this study is to assess the impact of low-intensity pulsed ultrasound (LIPUS) therapy on the peri-implant osteogenesis in a Type II diabetes mellitus (T2DM) rat model. Methods: A total of twenty male Sprague-Dawley (SD) rats were randomly allocated into four groups: Control group, T2DM group, Control-LIPUS group, and T2DM-LIPUS group. Implants were placed at the rats' bilateral maxillary first molar sites. The LIPUS treatment was carried out on the rats in Control-LIPUS group and T2DM-LIPUS group, immediately after the placement of the implants, over three consecutive weeks. Three weeks after implantation, the rats' maxillae were extracted for micro-CT, removal torque value (RTV), and histologic analysis. Results: Micro-CT analysis showed that T2DM rats experienced more bone loss around implant cervical margins compared with the non-T2DM rats, while the LIPUS treated T2DM rats showed similar bone heights to the non-T2DM rats. Bone-implant contact ratio (BIC) were lower in T2DM rats but significantly improved in the LIPUS treated T2DM rats. Bone formation parameters including bone volume fraction (BV/TV), trabecular thickness (Tb.Th), bone mineral density (BMD) and RTV were all positively influenced by LIPUS treatment. Histological staining further confirmed LIPUS's positive effects on peri-implant new bone formation in T2DM rats. Conclusion: As an effective and safe treatment in promoting osteogenesis, LIPUS has a great potential for T2DM patients to attain improved peri-implant osteogenesis. To confirm its clinical efficacy and to explore the underlying mechanism, further prospective cohort studies or randomized controlled trials are needed in the future.

Abstract:Objective To explore the effects of Insulin on both bone mass and bone biomechanics of alloxan diabetic rat.Methods Divided 42 SD male mice into 3 groups: normal group,diabetic group,and insulin treatment group.All of them will be sacrificed after 3 months,then assay the bone mass of femur and epiphyseal tip,do biodynamical test on femur and lumbar,and give an appraisement.Results Compared with normal group,the bone mass of diabetic group decreased obviously;the maximum flexural carrying capacity of femur,contra-compression carrying capacity and strength of lumbar descent patently(P<0.01),however,elastic modulus has no significant change.Meanwhile,compared with diabetic group,bone mass of the group treated with insulin increased,and the maximum flexural carrying capacity of femur,compression carrying capacity and strength of lumbar enhanced,but elastic modulus rose abnormally.(P< 0.01).Conclusion Insulin can significantly increase bone mass of diabetic rat's trabecular bone,and improve its contra-compression and flexural capacity of its bone obviously,however,ill-effect of the increased fragility of lumbar will come out at the same time.
Effects of Linagliptin and Pioglitazone on Fracture Healing in an Experimental Type 2 Diabetes Rat Model

Abstract:AIM: Our study aimed to examine the effects of Linagliptin, Pioglitazone, and their combination on fracture healing in a diabetes rat femur fracture model.MATERIAL AND METHODS: Type 2 diabetes mellitus (T2DM) induced rats were randomly divided into four groups: non-treated diabetes group (TD), Pioglitazone group (P), Linagliptin group (L), and Pioglitazone and Linagliptin group (PL). Daily oral dosage of pioglitazone (10 mg/kg/day), linagliptin (10 mg/kg/day), and their combination were administered. Femur fractures were stabilized intramedullary. At weeks 2 and 6, rats were sacrificed for evaluation radiologically, biomechanically, histopathologically, histomorphometrically, and immunohistochemically.RESULTS: Flexural strength of the L and PL groups were significantly higher compared to the P group. The highest healing score was in the L group and lowest in the P group, while the highest inflammation score was in the P group and lowest in the L group. A cluster of differentiation (CD) CD 34 reactivity was highest in the L group and lowest in the PL group.CONCLUSION: Linagliptin treatment significantly increased histological healing scores, callus volume, biomechanical strength, and vascularity, however, minimized the inflammatory process, which was increased by pioglitazone. The combination of linagliptin and pioglitazone restored BMD and increased biomechanical strength. Linagliptin monotherapy is rarely indicated; hence, T2DM patients with a high risk of bone fractures can be considered for combined therapy of pioglitazone and linagliptin.
Effects of whole-body vibration on bone properties in type 2 diabetes model rats

Abstract:Objectives: We examined the effects of whole-body vibration (WBV) on bone properties in type 2 diabetes (T2DM) model rats. Methods: Twenty male Hos:ZFDM-Lepr fa , fa/fa rats (divided into DM and WBV groups, N = 10 each) and 10 Hos:ZFDM-Lepr fa , fa/+ rats (as the control (CON) group) were used. The WBV group underwent WBV at 45 Hz frequency, with 0.5 g acceleration (15 min/day, 5 days/week) for 8 weeks. Trabecular and cortical bone mass, trabecular bone microstructure (TBMS), and cortical bone geometry (CBG) were analyzed via micro-computed tomography. Bone mechanical strength (maximum load, break point, and stiffness) was also measured. Additionally, bone metabolic and DM-related markers were determined. Results: The bone mechanical strength of the femur improved in the WBV group, although muscle atrophy and bone deterioration were observed in the DM and WBV groups. The serum levels of bone-specific alkaline phosphatase and tartrate-resistant acid phosphatase-5b were significantly higher in the DM and WBV groups than in the CON group. Serum glucose and blood urea nitrogen levels were significantly lower in the WBV group than in the DM group. Conclusions: This study suggests that WBV potentially improves the decrease in the bone mechanical strength of the femur, although it does not prevent the deterioration of bone mineral content, TBMS, and CBG parameters. Further studies are needed to investigate the effective timing and duration of WBV and the conditions that prevent T2DM and deterioration of bone properties and clarify the mechanism underlying WBV effects on bone properties in DM animals.
Improved fatigue properties, bone microstructure and blood glucose in type 2 diabetic rats with verapamil treatment

Abstract:Background: Type 2 diabetes mellitus is a global epidemic disease, which leads to a severe complication named increased bone fracture risk. This study aimed to explore if verapamil treatment could improve bone quality of type 2 diabetes mellitus. Methods: Rat models of control, diabetes and verapamil treatment with 4/12/24/48 mg/kg/d were established, respectively. Blood glucose was monitored during 12-week treatment, and bilateral tibiae were collected. Microstructural images of bilateral metaphyseal cancellous bone and high-resolution images of cortical bone of left tibial shafts were obtained by micro-computed tomography. Fatigue properties of bone were evaluated via cyclic compressive tests of right tibial shafts. Findings: Verapamil treatment had no significant effect on blood glucose, but blood glucose tended to decline with the increase of verapamil-treated time and dose. Compared with controls, osteocyte lacunar and canal porosities in diabetes and verapamil-treated groups were significantly decreased (P < 0.05), trabecular separation and degree of anisotropy were significantly increased (P < 0.05), while trabecular tissue mineral density, trabecular bone volume fraction and trabecular number in verapamil-treated (48 mg/kg/d) group were significantly higher than those in diabetes (P < 0.05). Compared with diabetes, initial compressive elastic moduli in verapamil-treated (12/24/48 mg/kg/d) groups were significantly increased (P < 0.05), while secant modulus degradations in verapamil-treated (24/48 mg/kg/d) groups were significantly decreased (P < 0.05). Interpretation: Verapamil could improve bone microstructure and fatigue properties in type 2 diabetic rats; and high-dose verapamil presented a significant effect on improving bone quality. These findings provided a new possibility for preventing the high bone fracture risk of type 2 diabetes mellitus in clinics.

Abstract:Background: Diabetes mellitus is a systematic disease which exert detrimental effect on bone tissue. The repair and reconstruction of bone defects in diabetic patients still remain a major clinical challenge. This study aims to investigate the potential of bone tissue engineering approach to improve bone regeneration under diabetic condition. Methods: In the present study, decalcified bone matrix (DBM) scaffolds were seeded with allogenic fetal bone marrow-derived mesenchymal stem cells (BMSCs) and cultured in osteogenic induction medium to fabricate BMSC/DBM constructs. Then the BMSC/DBM constructs were implanted in both subcutaneous pouches and large femoral bone defects in diabetic (BMSC/DBM in DM group) and non-diabetic rats (BMSC/DBM in non-DM group), cell-free DBM scaffolds were implanted in diabetic rats to serve as the control group (DBM in DM group). X-ray, micro-CT and histological analyses were carried out to evaluate the bone regenerative potential of BMSC/DBM constructs under diabetic condition. Results: In the rat subcutaneous implantation model, quantitative micro-CT analysis demonstrated that BMSC/DBM in DM group showed impaired bone regeneration activity compared with the BMSC/DBM in non-DM group (bone volume: 46 ± 4.4 mm 3 vs 58.9 ± 7.15 mm 3 , * p < 0.05). In the rat femoral defect model, X-ray examination demonstrated that bone union was delayed in BMSC/DBM in DM group compared with BMSC/DBM in non-DM group. However, quantitative micro-CT analysis showed that after 6 months of implantation, there was no significant difference in bone volume and bone density between the BMSC/DBM in DM group (199 ± 63 mm 3 and 593 ± 65 mg HA/ccm) and the BMSC/DBM in non-DM group (211 ± 39 mm 3 and 608 ± 53 mg HA/ccm). Our data suggested that BMSC/DBM constructs could repair large bone defects in diabetic rats, but with delayed healing process compared with non-diabetic rats. Conclusion: Our study suggest that biomaterial sacffolds seeded with allogenic fetal BMSCs represent a promising strategy to induce and improve bone regeneration under diabetic condition.

Abstract:Background: Diabetes mellitus weakens bone strength due to deterioration of bone quality; however, the histological mechanisms are still unknown. We hypothesized that histological assessment of cortical bone would enable us to determine the cause of the bone strength reduction associated with diabetes mellitus. Our aim was to evaluate the histomorphometric changes of cortical bone associated with deterioration of intrinsic bone properties and bone quality in diabetes mellitus. Methods: We compared the outcomes of mechanical tests, bone mineral density measured using micro-computed tomography, and histological assessments, by applying Villanueva's bone stain, to the tibial bones of 40-week-old diabetic and control male rats. Results: With respect to mechanical testing, the maximum load and energy absorption were significantly lower in the diabetic than in the control group, although fracture displacement and stiffness were not significantly different between the two groups. Bone mineral density was significantly higher in the diabetic group than in the control group. Bone histomorphometry revealed that the diabetic rats had fewer osteocytes, greater cortical porosity, and increased mineralization in cortical bone compared with the control group. Conclusions: Increased mineralization of the cortical bone with greater cortical porosity leads to a weakening of bone strength in diabetes mellitus.

Abstract:Objective:To investigate the effect of diabetes mellitus on bone-defect healing in diabetic rats.Methods: A total of 90 male Wister rats were randomly divided into the diabetic group(n=54) and the control group(n=36).Diabetic rat models were made by injection alloxan into its abodominal cavities.Then a standardized bone-defect model was chosen on tibia.Four animals from each group were killed respectively at 1th,2th,3th,4th,5th and 6th weeks after bone defect and specimens were processed undecalcified for histological and immunohistochemical studies.Biomechanical tests were undergone at 4th,5th and 6th weeks.Results: Biomechanical analysis showed a decrease in the strength of healing fractures in diabetic animals.Immunohistochemical staining showed that BMP-2 was widely distributed in normal rats 1 and 3 weeks after fracture,especially in the callus,whereas much less BMP-2 was detected in diabetic rats.Conclusion: These results suggest that diabetes affects the phase of fracture healing by inhibiting cell proliferation through decreasing expression of cell growth factor.

Abstract:A 61-year-old man with severe chronic obstructive pulmonary disease presented to our hospital with recurrence of a right-sided spontaneous secondary pneumothorax. Thoracoscopic abrasion of the parietal pleura was performed, but an important air leak persisted. Presumed to originate from a bulla in the right upper lobe, bullectomy and pleural decortication were performed, but leakage remained. Lobectomy was considered, and quantitative ventilation/perfusion SPECT was performed to predict the functional outcome.Fused high-resolution CT/Tc Technegas images localized leakage not only to a bleb in the right upper lobe but also to the subcutaneous emphysema in the thoracic wall. The air leak resolved after conservative treatment.

Abstract:vere articular manifestations may persist. In these cases, the use of an immunomodulator—particularly methotrexate—is only intended to obtain a control of articular symptoms. Second, the addition of an immunomodulator to vedolizumab monotherapy can improve clinical benefit when vedolizumab monotherapy did not obtain complete control of intestinal symptoms. In this line, a recent study by Allegretti et al showed that the addition of an immunomodulator to vedolizumab was an independent predictor of clinical response or remission at week 54 among patients with Crohn’s disease, while a recent case series comprising 4 patients showed that the addition of methotrexate to vedolizumab did not obtain a clinical response and did not improve the pharmacokinetic profile of the drug. Consequently, we believe that our promising—but preliminary—results may be secondary to the addition of the therapeutic effect of the immunomodulator to that of vedolizumab, rather than to a pharmacokinetic benefit similar to that observed with anti-TNF combination therapies.
