Abstract:Intestinal neuronal malformation (INM) is a rare and refractory pediatric disease [1]. Its definitive diagnosis is generally confirmed by an invasive full-thickness biopsy [2]. This biopsy is required because the nerve plexus is located in the deep submucosal and muscle layers, resulting in poor diagnostic ability with endoscopic suction biopsies [3]. In our experience, even specimens obtained by endoscopic submucosal dissection fail to provide an accurate histological evaluation owing to the burn effects. As a result, full-thickness specimens must be obtained to make a diagnosis of INM. A new type of over-the-scope clip (OTSC), called a full-thickness resection device (FTRD; Ovesco Endoscopy, Tübingen, Germany), provides a moderate rate (75%) of histologically complete resection, so indicating a need to modify procedures [4, 5]. In this animal study, we introduced a productive endoscopic full-thickness sampling method with the original OTSC system targeting INM. A flexible gastrointestinal endoscope was used. First, a 10-mmmucosal pocket was created in the lower rectum using a needle knife (KD-650Q; Olympus, Tokyo, Japan) until the muscle layer was visible (▶Fig. 1 a). Next, after the artificial pocket had been anchored into the application cap with a retraction device (Anchor; Ovesco Endoscopy) that captured the exposed muscle layer, the OTSC was successfully deployed (▶Fig. 1b). A 10-mm incision was then made with the needle knife in the muscle layer immediately above the clip to prevent slippage of the snaring device. Finally, a full-thickness resection was completed with the snare, without complications, using the Endo Cut Q mode on an electric generator (VIO300D; ERBE, Tübingen, Germany) (▶Fig. 1 c, d; ▶Video1). A 10-mm specimen with a sufficient muscle layer was acquired (▶Fig. 2). Histological examination revealed an adequate full-thickness layer including the myenteric plexus and ganglia cells (▶Fig. 3). This study emphasizes that a minimal incision-assisted OTSC procedure can facilitate full-thickness sampling and minimally invasive diagnosis of INM.

Abstract:Summary Using hemoglobin markers to identify donor- and host-type erythrocytes, we have demonstrated that cells in the leukocyte fraction of leukemoid blood are capable of transplanting and giving rise to mature erythrocytes of donor type in lethally irradiated mice. Modulatory Properties of Vitamin D in Type 2 Diabetic Patients: A Focus on Inflammation and Dyslipidemia

Abstract:Background: Evidence from preclinical studies has found a correlation between the development of type 2 diabetes (T2D) and vitamin D deficiency. However, evidence from randomized controlled trials (RCTs) revealed inconclusive results on vitamin D supplementation. We explored the effect of vitamin D on inflammation and dyslipidemia in T2D. Methods: We comprehensively searched for RCTs evaluating the effect of vitamin D in T2D on PubMed. Data were analyzed using Review Manager 5.3 and reports, such as standardized mean difference (SMD) and 95% confidence intervals (CI) at a 5% significant level using a random effect model. Results: This study revealed a significant reduction in tumor necrosis factor-alpha (TNF-α) SMD = (-0.51, 95%CI (-0.93, -0.09); p = 0.02), high sensitivity C-reactive protein (hs-CRP) SMD = (-1.06, 95%CI (-1.67, -0.45); p < 0.05) in vitamin D compared to placebo. Additionally, interleukin-6 (IL-6) exhibited a marginal effect SMD = (-0.52, 95%CI (-1.05, 0.01), p = 0.05). Furthermore, a significant reduction in the level of triglycerides SMD = (-0.65, 95%CI (-1.11, -0.18), p < 0.05) was observed, concomitant to a significantly increased high-density lipoprotein (HDL) level SMD = (0.53, 95%CI (0.08, 0.98), p = 0.02). However, no statistically significant changes were observed in total cholesterols SMD = (-0.16, 95%CI (-0.57, 0.24), p = 0.43) and low-density lipoprotein (LDL) SMD = (-0.06, 95%CI (-0.37, 0.24), p = 0.67). Conclusions: These findings suggest that vitamin D supplementation may be beneficial in ameliorating inflammation and dyslipidemia in T2D patients.
Influence of vitamin D supplementation on glycemic control in type 2 diabetics

Abstract:Introduction: Several studies have suggested a benefic impact of vitamin D supplementation on glycemic control and insulin resistance among patients with type 2 diabetes mellitus. The aims of our study were to assess vitamin D status in individuals with type 2 diabetes mellitus and to investigate the effects of vitamin D supplementation on glycemic measures in patients having vitamin D deficiency. Methods: We conducted a comparative prospective study involved 100 Tunisian patients with type 2 diabetes followed at the National Institute of Nutrition and Food Technology of Tunis. Glycemic control and insulin resistance were evaluated in the beginning of the study and three months after supplementation. Results: Baseline mean 25-Hydroxy vitamin D (25(OH)D) level was 17.5±9.8 ng/ml. Vitamin D status was deficient in 60%, insufficient in 26% and sufficient in 14% patients. After vitamin D supplementation, mean serum 25(OH)D concentration increased significantly (p˂10-3). We observed a negative correlation between the variation of plasma 25(OH)D level and the waist circumference's variation (r=-0.266 and p=0.018). This correlation persisted after adjustment for therapeutic management. Vitamin D supplementation did significantly improve neither glycemic control nor insulin resistance parameters. Conclusion: Vitamin D deficiency is frequent in patients with type 2 diabetes mellitus. The metabolic effects of supplementation are controversial, hence the need of expanding studies to better demonstrate these effects.

Abstract:To compare the serum levels of brain-derived neurotrophic factor (BDNF) in type 2 diabetes mellitus (T2DM) patients with healthy controls (HC) and evaluate the BDNF levels in T2DM patients with/without cognitive impairment. PubMed, EMBASE, and the Cochrane Library databases were searched for the published English literature on BDNF in T2DM patients from inception to December 2022. The BDNF data in the T2DM and HC groups were extracted, and the study quality was evaluated using the Agency for Healthcare Research and Quality. A meta-analysis of the pooled data was conducted using Review Manager 5.3 and Stata 12.0 software. A total of 18 English articles fulfilled with inclusion criteria. The standard mean difference of the serum BDNF level was significantly lower in T2DM than that in the HC group (SMD: -2.04, z = 11.19, P <0.001). Besides, T2DM cognitive impairment group had a slightly lower serum BDNF level compared to the non-cognitive impairment group (SMD: -2.59, z = 1.87, P = 0.06). BDNF might be involved in the neuropathophysiology of cerebral damage in T2DM, especially cognitive impairment in T2DM.

Abstract:Two copper salts (Cu(CF3CO2)2 and IMesCuCl) were identified as earth-abundant, inexpensive, but effective metal catalysts together with diazo malonate for chemo-/regioselective and stereospecific deoxygenation of various epoxides with tolerance of common functional groups (alkene, ketone, ester, p-methoxybenzyl, benzyl, tert-butyldimethylsilyl, and triisopropylsilyl). In particular, the unprecedented regioselectivity allowed for the first time monodeoxygenation of diepoxides to alkenyl epoxides. Density functional theory mechanistic studies showed that the deoxygenation occurred by collapsing the free ylide, unfavoring the possible intuitive pathway via cycloreversion of possible oxetane.

Abstract:Abstract Background: Diabetes mellitus causes metabolic disturbances and defectiveness in most tissues and cells of the body, especially in the nerves, blood vessels, eyes, kidneys, and other body tissues. It is known that type 2 diabetes mellitus is one of the most prevalent diseases in the city of Mosul, due to several reasons, such as excessive carbohydrate intake, psychological disorders, obesity, and lack of exercise. Because of these and other reasons, there was a need to conduct scientific research to clarify the main reasons for diabetes, modify the method of treatment and change the lifestyle to avoid falling into these chronic diseases. Objective: Studying the effect of vitamin D deficiency on patients with type 2 diabetes mellitus for the population of Mosul city for the purpose of modifying the metabolism in the body. Materials and Methods: Fifty eight study subjects were collected from patients with type 2 diabetes mellitus (diagnosed by a specialist and HbA1c test results) with 20 control samples of approximately the same ages and body mass index. Thereafter, We have investigate the durations of improving vitamin D for more than one year and record all medical history of the patients. Eleven study subjects were excluded due to drug interactions. Forty nine subjects of a study comprised 33 females and 16 males who were included in the study after making sure that they did not take medication. None of them were pregnant or breastfeeding. Their mean age was (51.8 ± 0.2 years) and their mean BMI was (26.8 ± 0.5). The examination was carried out using immunofluorescence and dry chemistry using the I CHROMA TM II device with the materials of the device, which were provided by the manufacturer with all the materials for calibration and quality control. Results: Forty nine study subjects were examined by HbA1c and vitamin D level by immunofluorescence technique, and the mean results were (9.3 ± 0.1%) for HbA1c and (14.3 ± 1 ng/mL) for vitamin D. After treatment with vitamin D for 1 month. The results showed a good improvement in the results of the HbA1c examination, as their average results were (7.1 ± 0.1%), with an improvement in the results of the vitamin D examination by (48.8 ± 1 ng/mL). Conclusions: There was a clear improvement in the level of sugar in the blood of patients with type 2 diabetes mellitus after treatment with vitamin D and an improvement in its level in the blood.

Abstract:Vitamins are an essential component of the human body for growth and maintaining health. All vitamins have a significant role in metabolism, as a prophylactic in preventing various diseases, and maintaining health. Literature studies have predicted the positive impact of vitamin D on sugar level in blood of type 2 diabetes. Vitamin D plays a significant role in treating diabetes mellitus because it helps produce insulin and helps in the growth of beta cells of the pancreas. The present study was conducted for evaluating the impact of oral vitamin D in reducing the hyperglycemic conditions in patients after treatment of 1-6 months duration. 52 type 2 diabetes patients were enrolled in the study. The results showed that Vitamin D supplementation of 16 weeks reduced fasting blood glucose and HbA1C significantly in Vitamin D deficient Type 2 diabetes.

Abstract:Background: A low serum 25-hydroxyvitamin D (25(OH) D) concentration has been associated with a higher risk of type 2 diabetes mellitus (T2DM), especially in older people. Our aim in this randomized controlled trial was to evaluate the effect of vitamin D treatment on inflammatory markers in non-obese Lebanese patients with T2DM, living in Beirut, Lebanon. Methods: Non-Obese patients with T2DM (n = 88), deficient/insufficient in vitamin D, were randomly assigned into one of two groups-a treatment group receiving 30,000 IU cholecalciferol/week for a period of six months, and a placebo group. Serum concentrations of TNF-α, high-sensitivity C-reactive protein (hs-CRP), and Interleukin-6 (IL-6) were the primary outcomes. A homeostatic model of insulin resistance (HOMA-IR) was assessed, in addition to serum concentrations of fasting blood glucose (FBG), HbA1C, (25(OH) D), and PTH. Results: The vitamin D group showed higher blood levels of (25(OH) D) (p < 0.0001), and a significant reduction in hs-CRP and TNF-α concentrations (p < 0.0001) compared to placebo. The decrease perceived in IL-6 concentrations was not significant (p = 0.1). No significant changes were seen in FBG (p = 0.9) and HbA1c levels (p = 0.85). Conclusion: Six months of vitamin D supplementation led to a decrease in some inflammatory markers in patients with T2DM. Additional studies with a larger sample and a longer period are advised in this regard. This trial was registered at ClinicalTrial.gov; Identifier number: NCT03782805.
Association of Glycosylated Hemoglobin Levels With Vitamin D Status

Abstract:Background: The discovery of vitamin D is one of medicine's great achievements. Despite all the positive evidence emerging about the beneficial effect of vitamin D, we still find many are vitamin D deficient. The purposes of this study were to examine the association between serum 25-hydroxyvitamin D (25(OH)D) and glycosylated hemoglobin (HbA1c) levels, to test the hypothesis that lower 25(OH)D levels are associated with poorer glucose control in diabetes mellitus (DM) patients and to investigate the effect of vitamin D supplementation on HbA1c levels. Methods: This was a prospective observational cohort study of all patients with type 1 and type 2 diabetes (above 12 years) who attended the outpatient clinics of a tertiary center in Riyadh. HbA1c and vitamin D levels were recorded prior to supplementation and after 9 months of supplementation with vitamin D. All patients were divided into four groups according to their vitamin D level and an association between 25(OH)D and HbA1c was tested. Results: Results showed that 73.1% of the patients had 25(OH)D levels < 50 nmol/L. We observed lowering of HbA1c after vitamin D supplementation (from mean HbA1c of 10.55 to 7.70). We found HbA1c to be inversely related to serum vitamin D levels (r = -0.14 (P < 0.0000002) before supplementation and -0.16 (P < 0.000001) after supplementation with vitamin D). Conclusions: Advising patients with higher HbA1c to test their vitamin D level and correct any deficiency will result in better blood glucose control and benefit the patient's overall health.
Correlation of 25-Hydroxycholecalciferol and Glycaemic Status in Recent Onset Type 2 Diabetes Mellitus

Abstract:Introduction: Type 2 Diabetes Mellitus (T2DM) is a non communicable disease caused by increased insulin resistance and beta cells dysfunction. Recently vitamin D has sparked wide spread interest in pathogenesis of diabetes by playing a role in insulin resistance. As a major regulator of homeostasis of calcium, vitamin D directly and/or indirectly improves insulin exocytosis and glucose tolerance. Aim: To estimate the serum 25 hydroxy cholecalciferol level and also to correlate with fasting, two hour postprandial blood glucose and Glycated Haemoglobin (HbA1c) values in recent onset T2DM patients. Materials and Methods: This cross-sectional study was conducted at Government Kilpauk Medical College, Tamil Nadu, India from November 2017-April 2018. One hundred and thirty nine recently diagnosed T2DM patients aged between 30-60 years of both sex, who were on oral hypoglycemic drugs for less than three months duration were included in the study. The patients were grouped into three groups according to their vitamin D levels, Group 1 Vitamin D deficient- 52 patients (≤20 ng/mL), Group 2- Insufficient Vitamin D -33 patients (>20-30 ng/mL), Group 3- Vitamin D sufficient-54 patients (>30 ng/mL). Fasting and 2 hour postprandial glucose, 25 hydroxy cholecalciferol and HbA1c were estimated. Statistical analysis was performed using analysis of variance (ANOVA) and Pearson’s correlation. Results: Out of 139 participants, 64 were males and 75 were females. The mean age of the study population was 50.64±5.343 years. The mean fasting blood glucose values among the three groups were 168.13, 129.61, and 125.33 mg/dL respectively. The mean two hour postprandial blood glucose values among the three groups were 269.44, 212.45, and 194.11 mg/dL respectively. The mean HbA1c among the three groups were 7.481±1.16, 6.027±0.31, and 5.86±0.19, respectively. Decreased 25 hydroxy cholecalciferol level in patients of T2DM showed a statistically significant inverse correlation with fasting and two hour postprandial blood glucose and HbA1c with p-value <0.001. Conclusion: The study suggested that hypovitaminosis D was prevalent in T2DM. The study showed that decreased vitamin D level in T2DM patients was associated with increased fasting, postprandial blood sugar, and HbA1c. So, vitamin D screening in diabetics and supplementation can improve the glycemic status.
Is there an association between serum 25(OH)D3 and mental well-being in patients with type 2 diabetes? Results from a cohort study in primary care

Abstract:Purpose: There are limited and inconsistent results on the correlation between vitamin D and mental health in patients with type 2 diabetes (T2D). Thus, our aim was to explore the association between vitamin D and mental well-being in a community-based sample of participants with T2D. Methods: We analyzed serum 25-hydroxyvitamin D3 (25(OH)D3) in 698 patients with T2D at the baseline examination. The cohort was reinvestigated after 4 years. Data from SF-36 questionnaires measuring vitality and mental health at baseline and after 4 years were used for analyses. Results: Serum 25(OH)D3 was inversely associated with poor mental health at baseline (odds ratio (OR) for 10 nmol/l increase in 25(OH)D3, 0.90 (95% confidence interval (CI) 0.83-0.96, p = 0.003)) but not at follow-up (p > 0.05). Serum 25(OH)D3 was not associated with vitality at baseline (p > 0.05). At follow-up, there was an inverse association between 25(OH)D3 and low vitality (OR for 10 nmol/l increase in 25(OH)D3, 0.89 (95% CI 0.82-0.97, p = 0.009)) but not after adjustment. Conclusion: We found an inverse association between 25(OH)D3 and mental health in patients with T2D at baseline. We found no association between 25(OH)D3 and vitality after adjustment. Future studies are needed to determine the association between vitamin D and mental well-being in patients with T2D.
Association between serum vitamin D and the risk of diabetic kidney disease in patients with type 2 diabetes

Abstract:Aims: This investigation explored the potential correlation between serum vitamin D concentration and diabetic kidney disease (DKD) among patients with type 2 diabetes mellitus (T2DM). Methods: This cross-sectional study assessed 4,570 patients with T2DM drawn from the National Health and Nutrition Examination Survey (NHANES) dataset. Restricted cubic splines were utilized to examine the dose-response relationship between serum vitamin D levels and the risk of DKD in patients with T2DM. Serum vitamin D concentrations were divided into quartiles for multivariable logistic regression analysis to evaluate the association between varying serum vitamin D levels and DKD risk in patients with T2DM. Additionally, sex-stratified analyses were conducted to determine consistency of the results. The influence of vitamin D concentrations on mortality risk was assessed using a Cox regression model. Results: Of the patients with T2DM, 33% were diagnosed with DKD. Restricted cubic spline plots revealed a U-shaped relationship between vitamin D levels and DKD risk, with a protective effect noted in the mid-range, indicating optimal serum vitamin D concentrations between 59.6 nmol/L and 84.3 nmol/L. The multivariate Cox regression analysis suggested that higher VID levels were associated with a reduced mortality risk, particularly in male patients. Conclusion: The regulation and monitoring of serum vitamin D levels within an optimal range may play a pivotal role in the prevention of DKD in patients with T2DM. Public health strategies should emphasize the regular monitoring of vitamin D levels, especially among populations at elevated risk, to mitigate the progression of DKD and decrease the associated mortality rates.

Abstract:Background: Non-alcoholic fatty liver disease (NAFLD) individuals are more prone to acquire type 2 diabetes (T2DM). However, the knowledge regarding this phenomenon is limited to individuals without diabetes mellitus (DM). Vitamin D deficiency may lead to a more pronounced shift in an individual's glucose metabolism. Moreover, hepatocytes and peripheral tissues may potentially benefit from vitamin D in terms of reducing insulin resistance induced by free fatty acids. Objective: In this study, serum vitamin D levels were examined in patients with NAFLD with T2DM, and its relationship with NAFLD without DM was evaluated.Subjects and Methods: A total of 120 subjects were included in this study, divided into three groups: Group I included 30 healthy subjects matched for sex and age, Group II included 50 patients with NAFLD who had diabetes, and Group III included 40 subjects without diabetes. Three groups were compared regarding clinical and biochemical characteristics concerning serum 25-hydroxyvitamin D levels using enzyme-linked immunosorbent assays.Results: According to this study, people with NAFLD diagnosed by ultrasound examination Group II had significantly reduced serum levels of 25(OH) vitamin D compared to patients without NAFLD diagnosed by ultrasound examination Group II. There was a statistically significant difference between groups and 25(OH) vitamin D levels than group III. In addition, a highly significant difference in serum 25(OH) vitamin D levels was observed between groups II and III and the control group. Moreover, 25(OH) vitamin D is an independent predictor of T2DM in patients with NAFLD.Conclusion: Serum 25(OH) vitamin D levels were lower in patients with NAFLD and DM, indicating that low 25(OH) vitamin D status contributes to the onset and progression of diabetes mellitus in NAFLD.Keywords:25-hydroxy vitamin D[25(OH)D)], Non-alcoholic fatty liver disease (NAFLD), diabetes mellitus.

Abstract:Background: Diabetes patients have a higher chance of developing dyslipidemia and increased release of free fatty acids, which participate in developing insulin-resistant fat cells. On the other hand, vitamin D insufficiency is linked to the evolution of type 2 diabetes mellitus (T2DM). This study examines the impact of vitamin D serum levels on lipid profiles and insulin resistance concerning glycemic indices in obese T2DM patients. Methods: During the data collecting stage, 47 diabetes patients were chosen from the out-patient clinic. The control individuals were selected from the general population and were equivalent to the matching patients, with a total of 43 healthy participants. After an overnight fast, a venous blood sample was collected from each individual to test insulin and vitamin D3 levels using particular ELISA kits. In addition, by colorimetric test, serum was used to estimate total cholesterol, triglyceride, and high-density lipoprotein cholesterol. Aside from that, fasting serum glucose levels were measured (FSG). Results: Fasting serum glucose (FSG), homeostatic model assessment-insulin resistance (HOMA-IR), total cholesterol, and triglycerides, all of these values were significantly elevated in people with diabetes as compared to controls (p-value <0.05) when the serum level of vitamin D was markedly low. In contrast, insulin and high-density lipoprotein values had decreased significantly in the diabetic population compared to controls (p-value <0.05) and were not correlated to vitamin D levels. Conclusions: Diabetes patients had higher FSG, HOMA-IR, hemoglobin A1c (HbA1c), fasting insulin, triglycerides, total cholesterol to high-density lipoprotein cholesterol ratios (TC: HDL-C), triglyceride to high-density lipoprotein cholesterol ratios (TG: HDL-C), and low-density lipoprotein cholesterol to high-density lipoprotein cholesterol ratios (LDL-C: HDL). In obese diabetic individuals, vitamin D levels had a significant impact on total cholesterol, LDL-C, and the TC:HDL-C ratio.
Decreased serum levels of 25-OH vitamin D and vitamin K in patients with type 2 diabetes mellitus

Abstract:Background: Insulin resistance and/or insulin secretion dysfunction are crucial causes of type 2 diabetes mellitus (T2DM). Although some studies have suggested potential roles for vitamins D and K in glucose metabolism and insulin sensitivity, there is limited and inconclusive research on their levels in T2DM patients and their relationship with blood glucose levels and insulin resistance. Additionally, there is a lack of large-scale clinical trials and comprehensive studies investigating the combined effects of vitamins D and K on T2DM. Methods: A total of 195 participants with newly diagnosed T2DM were included in the research group, while 180 volunteers undergoing physical examinations in our hospital served as the control group. Fasting plasma glucose (FPG) was estimated using the glucose-oxidase technique, and fasting serum insulin (FINS) was evaluated by radioimmunoassay. FPG and FINS were used to calculate the homeostasis model assessment-insulin resistance (HOMA-IR). Serum vitamin D levels were measured using 25-hydroxyvitamin D, and vitamin K levels were evaluated using phylloquinone (VK1) and menaquinone (VK2) via ultra-high performance liquid chromatography and tandem mass spectrometry. Receiver operating characteristic (ROC) analysis was performed to assess the predictive value of these vitamins for T2DM. Results: Circulating levels of 25-hydroxyvitamin D (25.95 ± 10.42 ng/mL), VK1 (1.24 ± 0.89 ng/mL), and VK2 (0.2 ± 0.21 ng/mL) in T2DM patients were significantly lower than in the control group (37.46 ± 13.95 ng/mL for 25-hydroxyvitamin D, 1.99 ± 1.39 ng/mL for VK1, and 0.33 ± 0.22 ng/mL for VK2; p<0.001 for all comparisons). ROC analysis indicated that 25-hydroxyvitamin D, VK1, and VK2 could predict the occurrence of T2DM, with AUC values of 0.75, 0.69, and 0.71, respectively. In T2DM patients, 25-hydroxyvitamin D levels were positively correlated with VK1 (r=0.43, p<0.001) and VK2 (r=0.40, p<0.001) levels. FPG and HOMA-IR in T2DM patients were negatively correlated with circulating levels of 25-hydroxyvitamin D (r=-0.57, p<0.001), VK1 (r=-0.44, p<0.001), and VK2 (r=-0.36, p<0.001). Conclusion: Circulating levels of vitamins D and K are lower in T2DM patients and show significant correlations with blood glucose levels and insulin resistance. These findings suggest that measurements of 25-hydroxyvitamin D, VK1, and VK2 could have predictive value for T2DM, highlighting the potential roles of these vitamins in T2DM management.

Abstract:Introduction:- Vitamin D has been studied as modifiable risk factor in DM. Apart from its role in glucose homeostasis, the anti-inflammatory effect of vitamin D is claimed to have important effect on beta cell survival and on hepatic cells. Vitamin D is said to have anti-inflammatory, anti-proliferative and anti-fibrotic actions in liver. VDD is more prevalent in T2DM, obese and NAFLD even when these conditions occur separately. Literature states the protective effective of vitamin D on kidney. Association of VDD with albuminuria and chronic kidney disease in diabetics has also been reported. Material and Methods:- This is a type of comparative and interventional study. 63 T2DM patients aged 30 – 60 years with VDD were included. Baseline investigations determined blood levels of vitamin D, calcium, phosphate, liver enzymes (AST, ALT, ALP) and serum creatitine. Patients received vitamin D intervention orally in the dose of 2000 IU daily for 12 weeks. After 12 weeks blood levels of vitamin D, calcium, phosphate, liver enzymes (AST, ALT, ALP) and serum creatitine were determined. Results:- There was no correlation of vitamin D with urea, creatinine, calcium, phosphate, AST, ALT and ALP. There was extremely significant rise in vitamin D, significant fall in phosphate level, non-significant fall in creatinine, AST, ALT, ALP and non-significant rise in calcium, urea after 12 weeks of vitamin D supplementation. Conclusion:- There was no correlation of vitamin D with hepatic and renal parameters. Also 12 weeks of vitamin D supplementation had no significant improvement in these parameters in T2DM.

Abstract:Type 2 diabetes is a major public health problem, accounting for significant premature mortality and morbidity. It has been reported to be associated with several chronic complications which involve coronary artery disease and renal and liver diseases that are considered the primary cause of disability and mortality in diabetic patients. Different studies stated that vitamin D have functional role in glucose tolerance through its effects on insulin secretion and sensitivity and ameliorate type 2 diabetes complication. For that this study was carried out to investigate the effect of dietary vitamin D on type 2 diabetes complication on the liver, pancreas, lipid profile, and brain function. Rats were divided into 5 groups (control, diabetic, Vitamin D treated, Vitamin D preventive, and vitamin D control). Blood and tissue samples were collected after 6 weeks for measurement of fasting blood glucose (FBG), HOMA-IR, fasting serum insulin (FSI), serum lipid profile, blood antioxidants [reduced glutathione (GSH),superoxide dismutase (SOD), and catalase], serum lipid peroxidation product (malondialdehyde (MDA)), and histopathological examination of the pancreas and liver, besides brain acetylcholinesterase (AChE) gene expression. Our results indicated that dietary vitamin D supplementation to diabetic rats significantly increased FSI, decreased FBG and HOMA-IR, enhancing lipid profile by increase HDL-C and decrease LDL-C, TAG, and VLDL-C; Vitamin D also ameliorate GSH level and catalase activity and decrease the level of serum MDA, in addition to depress expression of brain AChE gene and improving liver and pancreas architecture. From these results, it can be concluded that vitamin D assured great protection against type 2 diabetes complications in rats.
