Abstract:Objective: To estimate the association between body mass index (BMI) and health-related quality of life (HRQL) and examine whether joint pain and obesity-related comorbidities mediate the BMI-HRQL association.Research Methods and Procedures: Population-based survey data from the 1999 Behavioral Risk Factor Surveillance Survey. Adults (N = 155,989) were classified according to BMI as underweight (< 18.5 kg/m(2)), desirable weight (18.5 to 24.9 kg/m(2)), overweight (25 to 29.9 kg/m(2)), obese class I (30 to 34.9 kg/m(2)), obese class II (35 to 39.9 kg/m(2)), and obese class III ( greater than or equal to40 kg/m(2)). Data including general health status, unhealthy days in the past 30 caused by physical problems and mental problems, and total unhealthy days in the past 30 were collected.Results: After adjusting for age, sex, race, smoking, education, and income, we observed J-shaped associations between BMI and HRQL. Compared with desirable weight adults, underweight, overweight, and obesity classes I, II, and III adults [odds ratio (OR) = 1.57, 1.19, 1.95, 2.72, and 4.36, respectively] were significantly (p < 0.001) more likely to report fair/poor general health status. For unhealthy days caused by physical problems, the corresponding ORs were 1.51, 1.15, 1.66, 2.27, and 3.61 (p < 0.001). For unhealthy days caused by mental problems, the ORs were 1.35, 1.14 1.43, 1.57, and 2.25 (p < 0.001). For total unhealthy days, the corresponding ORs were 1.27, 1.09, 1.37, 1.73, and 2.46 (p < 0.01). Adding joint pain and obesity-related comorbidities into models attenuated BMI-HRQL associations.Discussion: Associations between BMI and HRQL indices were J-shaped. Joint pain and comorbidities may mediate BMI-HRQL associations.
Association of body composition and physical activity with pain and function in knee osteoarthritis patients: a cross-sectional study

Abstract:Objective The objective of this study is to delineate disparities between patients with knee osteoarthritis (KOA) based on obesity status, investigate the interplay among body composition, physical activity and knee pain/function in patients with KOA and conduct subgroup analyses focusing on those with KOA and obesity. Design Cross-sectional study. Setting Residents of eight communities in Shijiazhuang, Hebei Province, China, were surveyed from March 2021 to November 2021. Participants 178 patients with symptomatic KOA aged 40 years or older were included. Main outcomes and measures The primary outcome measure was knee pain, assessed using the Western Ontario and McMaster Universities Osteoarthritis Index-pain (WOMAC-P) scale. Secondary outcome measures included function, evaluated through the WOMAC-function (WOMAC-F) scale and the Five-Time-Sit-to-Stand Test (FTSST). Data analysis involved t-tests, Wilcoxon rank-sum tests, 2 tests, linear and logistical regression analysis. Results Participants (n=178) were 41–80 years of age (median: 65, P25–P75: 58–70), and 82% were female. Obese patients (n=103) had worse knee pain and self-reported function (p<0.05). In general patients with KOA, body fat mass was positively associated with bilateral knee pain (β=1.21 (95% CI 0.03 to 0.15)), WOMAC-P scores (β=0.25 (95% CI 0.23 to 1.22)), WOMAC-F scores (β=0.28 (95% CI 0.35 to 1.29)) and FTSST (β=0.19 (95% CI 0.03 to 0.42)), moderate-intensity to low-intensity physical activity was negatively associated with bilateral knee pain (β=–0.80 (95% CI –0.10 to –0.01)) and Skeletal Muscle Index (SMI) was negatively associated with WOMAC-F scores (β=–0.16 (95% CI –0.66 to –0.03)). In patients with KOA and obesity, SMI was negatively associated with FTSST (β=–0.30 (95% CI –3.94 to –0.00)). Conclusion Patients with KOA and obesity had worse knee pain and self-reported function compared with non-obese patients. Greater fat mass, lower muscle mass and lower moderate-intensity to low-intensity physical activity were associated with increased knee pain and poor self-reported function. More skeletal muscle mass was associated with the improvement of objective function.

Factors affecting pain and physical functions in patients with knee osteoarthritis: An observational study

Abstract:Knee osteoarthritis (KOA) is more common as people age and have a higher body mass index (BMI). We must know the role of various factors in pain and physical functions in patients with KOA. Therefore, the present study sought to examine the factors associated with pain and physical functions in individuals with KOA. This cross-sectional observational study included patients with KOA (n = 125; 57 men, 68 women; mean age 52.9 years). Using the visual analogue scale and a reduced version of the Western Ontario McMaster Universities Osteoarthritis Index, pain severity and physical functions were assessed. Demographic factors such as age, BMI, sex, and Kellgren-Lawrence (K/L) radiographic grade of KOA were analyzed. Age (R = 0.263, P < .001), BMI (R = 0.379, P < .001), and K/L grade (R = 0.844, P < .001) were significantly associated with knee pain. Similarly, age (R = 0.310, P < .001), BMI (R = 0.374, P < .001), and K/L grade (R = 0.862, P < .001) were associated with physical functions. No significant association of sex with pain (R = 0.071, P = .440) and physical functions (R = 0.055, P = .545) was observed. Age, BMI, and K/L grade explained 71% and 74% of knee pain and physical functions, respectively. Age, BMI, and radiographic (K/L) grades were associated with pain and physical functions in patients with KOA. K/L grade was the most significant predictor of pain and physical functions in KOA.

Abstract:Background: Osteoarthritis is the most prevalent form of arthritis causing disability, particularly in elderly population. Aging, female sex, obesity, and possibly nutritional deficiencies trigger the pathogenesis of osteoarthritis. This study aims to identify the association of body mass index (BMI), with knee osteoarthritis grading viewed from Kellgren–Lawrence (KL) criteria in participants with osteoarthritis of the knee joint.Methods: This research is a cross-sectional study using analytical approach in 70 participants diagnosed with Knee Osteoarthritis at Vaidyaratnam Ayurveda College, Ollur, Thrissur. BMI was calculated by measuring the height and weight of participants. Grading of knee OA based on KL criteria was assessed from the antero-posterior X-ray radiograph.Results: Of 70 participants, 81.4% were female and 31.4% were in the age group of 50–55 years, 27.1% in the age group of 61–65 years. Among the total 70 participants, 45.7% belong to overweight category, 28.6% were obese. Knee OA grading based on KL criteria indicates that the most frequent grade is Grade 2 with 34.3% followed by Grade 3 30%. Spearman rank correlation showed a positive correlation between BMI and degree of joint damage, which was statistically significant at 0.01 level (r = 0.491**, P < 0.001).Conclusion: The higher the BMI value, the heavier the grading of OA of the knee
The Effect of Obesity on Pain Severity and Pain Interference

Abstract:ABSTRACT Background and objectives Obesity is one of the most prevalent comorbidities associated with chronic pain, the experience of which can severely interfere with activities of daily living and increase the utilization of clinical resources. Obesity is also a risk factor for increased pain severity (pain intensity) and pain interference (pain related disability). We hypothesize that a higher level of obesity, as measured by body mass index (BMI), would be associated with increased levels of pain severity and interference in a population of chronic pain clinic patients. Methods Participant data was pulled from a multi-site chronic pain outpatient database from 7/8/2011 to 10/17/2016. The Brief Pain Inventory (BPI), opioid prescriptions, and basic demographic information were queried and we categorized participants into three different ordinal categories based on recorded BMI levels (underweight, normal and overweight, obese). Bivariate analyses were performed to compare pain outcomes by BMI and by other demographic/clinical patient characteristics. Multivariable linear regressions were constructed to model each of four pain severity scores in addition to total pain interference score. All models examined BMI as the primary predictor, controlling for age, receipt of a pain procedure within 45 days prior to the pain clinic encounter, opioid prescription within 45 days prior to the encounter, and diagnosis. The total pain interference model additionally included pain severity (as measured by worst pain in the past 24 hours) as a covariate. Results 2509 patients were included in the study. The median BMI was 27 and the median age was 59 years. 77% of patients were diagnosed with musculoskeletal pain conditions. Bivariate tests revealed significant differences between BMI groups for all pain severity scores and for total pain interference score. On multivariable modelling controlling for age, pain procedure within 45 days prior to pain clinic encounter, opioid prescription with 45 days prior, and diagnosis, obese patients had significantly higher pain severity (as measured by worst, least, average, and current pain in the past 24 hours) as well as higher pain interference (as measured by the overall pain interference score) than normal weight and overweight patients. Conclusion In our study of pain clinic patients, obesity was found to be associated with increased pain severity and pain interference. We believe that this relationship is multifactorial and bidirectional. Pain phyisicans should consider the impact of obesity when addressing pain management for patients.
Osteoarthritis of the knee joint in the obese patient: Do metabolic factors play a role?

Abstract:Introduction: Osteoarthritis (OA), a disease process characterized by progressive softening of the articular cartilage, sclerosis of subchondral bone, and cyst formation often affects the knee joint. Although OA has traditionally been interpreted as wear and tear, its etiology is now thought to be multifactorial with both biomechanical and metabolic factors implicated. Of the metabolic factors, leptin a glycosylated peptide hormone produced by the adipocytes has received major attention because of its correlation to body size. The aim of this study was to determine the correlation between the clinical severity of knee OA and serum leptin in obese patients. Materials and Methods: This was a prospective study; recruiting 100 patients (50 obese and nonobese patients) with OA of the knee joint. The severity of knee OA was ascertained using the WOMAC scoring system, and serum leptin of all patients was determined. Results: The age group most commonly affected by knee OA was 51–60 years. The mean serum leptin level in nonobese patients was 4.88 ± 5.08, compared to 20.11 ± 15.04 in obese patients. There was a statistically significant correlation between the severity of knee OA (using WOMAC score) and serum leptin level; P = 0.001. Conclusion: This study shows a statistically significant positive correlation between serum leptin and severity of clinical symptoms in obese patients with knee OA.
Despite Greater Improvement in Pain and Function Among Obese Patients Shortly after Total Knee Arthroplasty, there is No Difference in Patient-Reported Outcomes and Satisfaction Between Body Mass Index Classes at 1 Year Postoperatively

Abstract:Background Although obesity may be associated with an increased risk of peri- and post-operative complications after total knee arthroplasty (TKA), body mass index (BMI) cutoffs for TKA patient selection remain a controversial topic. This study aimed to investigate patient-reported outcomes, satisfaction, and pain among BMI classes at 3 months and 1 year following TKA. Methods A total of 2,365 patients were categorized into Centers for Disease Control BMI classes according to preoperative values. The BMI distribution over the cohort demonstrated 7 underweight, 340 healthy weight, 731 overweight, 666 Class I, 391 Class II, and 230 Class III obesity. Pre- and post-operative Knee injury and Osteoarthritis Outcome Score for Joint Replacement (KOOS JR), satisfaction, and pain scores were compared by BMI class by Analysis of Variance (ANOVA) with pairwise comparisons. Results Preoperative KOOS JR scores and satisfaction scores were both significantly greater for healthy weight patients compared to Class III patients ( P < 0.0001). However, KOOS JR, satisfaction, and pain did not vary by BMI class postoperatively. Changes in scores from baseline suggest larger improvements with increasing BMI class, where Class III patients reported the greatest improvements in KOOS JR (23.24±15.67, P < 0.0001) and pain scores (-3.56±2.65, P < 0.0001) at 90 days. Significantly greater improvement with increasing BMI was also present at 1-year postoperatively for KOOS JR scores. A total of 85.8% of Class III patients reached the Minimal Clinically Important Difference (MCID) for KOOS JR at 90 days compared to 76.8% of the healthy weight group ( P = 0.03). Differences between BMI groups in the proportion reaching MCID at 1 year were not significant. Conclusion Patients of higher BMI reported greater and earlier improvements in satisfaction, knee function, and pain following TKA. Shared decision-making remains paramount to preoperative surgical evaluation. However, rationing of this surgical intervention based on BMI alone may not be warranted.

The association of fatigue severity with pain level, obesity indices and functional performances in women with knee osteoarthritis

Abstract:Introduction: Knee osteoarthritis is most common among women with obesity. It may lead to physical inactivity that, in turn, causes fatigue or lack of physical enthusiasm to perform meaningful daily activities. Hence, this study aimed to examine whether pain level, obesity indices and functional performances are associated with fatigue severity in women with knee osteoarthritis (KOA). Materials and methods: This cross-sectional study recruited women referred to physiotherapy to manage OA. The measurements included fatigue severity (fatigue severity scale); pain level (numerical rating scale); obesity indices (body mass index, fat %, waist circumference); functional performances (upper limb strength, lower limb strength, mobility, exercise capacity and quality of life). A simple linear regression analysis was used to determine which independent variable may be associated with fatigue severity. Results: Ninety-six women with unilateral KOA participated in this study (Mean age, 55.70, Standard Deviation, SD 6.90) years; Mean fatigue severity, 34.51, SD 14.03). The simple linear regression analysis showed that pain level (β=4.089, p<0.001), fat % (β=0.825, p<0.001) and QoL (β=0.304, p<0.001) were significantly associated with fatigue. After controlling for pain level, only fat % was significantly associated with fatigue (β=0.581, p=0.005). Conclusion: Pain level, fat %, and QoL appear to be associated with fatigue severity in women with KOA. In addition, pain symptoms may interact with factors associated with fatigue severity.

Abstract:Knee osteoarthritis (OA) and obesity are major public health concerns that are closely intertwined. This intimate relationship was documented by considering obesity as the most significant preventable risk factor associated with knee OA. To date, however, the effects of obesity on the knee joint's passive-active structure and cartilage loading have been inconclusive. Hence, this study investigates the intricate relationship between obesity and knee OA, centering on the biomechanical changes in knee joint active and passive reactions during the stance phase of gait. Using a subject-specific musculoskeletal and finite element approach, muscle forces, ligament stresses, and articular cartilage contact stresses were analyzed among 60 individuals with different body mass indices (BMI) classified under healthy weight, overweight, and obese categories. Our predicted results showed that obesity significantly influenced knee joint mechanical reaction, increasing muscle activations, ligament loading, and articular cartilage contact stresses, particularly during key instances of the gait cycle—first and second peak loading instances. The study underscores the critical role of excessive body weight in exacerbating knee joint stress distribution and cartilage damage. Hence, the insights gained provide a valuable biomechanical perspective on the interaction between body weight and knee joint health, offering a clinical utility in assessing the risks associated with obesity and knee OA.

Abstract:Objective To analyse the relationship between body mass index(BMI)and the incidence and radiographic progression of osteoarthritis(Kellgren/Lawrence score,K/L score).Methods Cross-sectional and longitudinal studies were conducted to analyse the relationship between BMI and the incidence and radiographic progression of osteoarthritis.The knee osteoarthritis analyses included 223 participants aged 30 years and older.416 controllers participated in the trial.Radiographic OA was assessed using the Kellgren/Lawrence(K/L) grading system.Body mass index was measured in all participants.All participants were divided into two groups according to WHO criterion for obesity(obese group:BMI≥25,normal weight gtoup:BMI25).Odds Ratio,95% confidence interval were calculated by comparing BMI between two groups.Results 11,69,58,84 patients had K/L score of I～IV respectively.BMI in subjects,with KOA was significantly higher than that in controls(P0.0001).Simultaneoualy,there was a certain correlation between BMI and radiographic KOA(Pearsou coefficient=0.222,P0.01);Obesity was a great risk for KOA(OR=2.63,95%CI=1.876-3.688,χ2=31.5,P≤10-7).Conclusion Obesity is a great risk for KOA;Simultaneously,it is correlated with progression of radiographic KOA.Weight control is one of the efficient ways to prevent knee osteoarthritis.

Abstract:Background For the evaluation of physical function in the population with knee osteoarthritis (KOA), one of the tests recommended by the Osteoarthritis Research Society International (OARSI) is the Stair Climb Test, so it is important to investigate which factors can influence performance in the test. Objective To investigate whether sex, age, BMI, and pain intensity interfere with the performance of the population with KOA in the Stair Climb Test. Methods The present study is an observational cross-sectional study. Participants over 45 years of age, of both sexes, with clinical diagnosis of KOA, according to the American College of Rheumatology (ACR) criteria, and pain intensity greater than 4, evaluated by the Numeric Rating Scale (NRS), were recruited. Anthropometric data were collected through an initial anamnesis, followed by the application of the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) questionnaire. Subsequently, participants were submitted to the Stair Climb Test. Participants were instructed to climb and descend a flight of 11 stairs, each 20 cm in height, quickly but safely. The sum of the times for climbing and descending was recorded by the evaluator. The Statistical Package for Social Sciences, version 21.0, was used for the multiple linear regression analysis, and the significance level was set at 5%. Results 100 participants were included, 52% of whom were female, with a mean age of 60 years. The mean BMI of the participants was 29.64 with a mean pain intensity of 9.5 by the specific domain of the WOMAC questionnaire and Stair Climb Test performance of 22.79 seconds. The final regression model (Table 1) indicated that sex (p = 0.029), age (p = 0.001), BMI (0.004), and pain by the specific domain of the WOMAC (p = 0.003) may explain 42% of the variability in Stair Climb Test performance. Conclusion The present study demonstrates that there is an association between sex, age, BMI, and painful symptoms in the performance of the Stair Climb Test, which may be potential factors that interfere with the performance of subjects with KOA. Implications Understanding the influence of such factors helps in the interpretation of the performance of patients with knee osteoarthritis in the stair climb test.

Abstract:Background: Increase body mass index (BMI) is often accompanied by metabolic diseases such as diabetes, which will increase the uncertainty of total knee arthroplasty (TKA) efficacy and the risk of postoperative complications. The present study was to study the relationship between increase BMI and clinical efficacy of knee arthroplasty in patients with knee osteoarthritis.Methods: A total of 97 patients (36 males and 61 females) with knee osteoarthritis (KOA) who underwent TKA surgery were selected. According to the preoperative body mass index (BMI), the patients were divided into a normal group (n=42), overweight group (n=35), and obese group (n=20). All patients received TKA after admission. Seven days after surgery, the American Knee Society (AKS) and the Hospital for Special Surgery (HSS) scales were used to evaluate the recovery of knee function. The recovery was poor if the scores of AKS and HSS were less than 70.Results: Seven days after TKA, the scores of AKS and HSS in different BMI groups were significantly different, and decreased with the increase of BMI (P<0.05). Age, increased BMI, diabetes, preoperative range of motion (ROM), intraoperative blood loss, postoperative C-reactive protein (CRP), postoperative posterior slope angle (PSA), postoperative infection, and postoperative deep vein thrombosis (DVT) of lower extremities were related to AKS score <70 (P<0.05). Diabetes, preoperative ROM, intraoperative blood loss, postoperative CRP, postoperative PSA, and postoperative infection were related to HSS score <70 (P<0.05). Increased BMI, diabetes, postoperative CRP, postoperative infection, and postoperative DVT were independent risk factors for AKS score <70 (HR =3.458, 1.152, 2.960, 1.023, 3.589, P<0.05). Increased BMI, diabetes, postoperative CRP, and postoperative infection were independent risk factors for HSS score <70 (HR =6.891, 1.263, 1.967, 1.235, P<1.235). The area under the curve (AUC) (95% CI) of BMI in diagnosing AKS <70 was 0.740 (0.641-0.839). The AUC (95% CI) of BMI in diagnosing HSS <70 was 0.809 (0.723-0.894).Conclusions: The increase of BMI is an independent risk factor for the poor recovery of knee function after TKA in patients with KOA.

At the Common Cold Research Unit in Salisbury, ECHO 28 virus and many other viruses have been isolated from subjects with colds. These other viruses had been tentatively termed rhinoviruses (1), and the term was officially accepted by the Virus Subcommittee of the International Committee on the Nomenclature of Bacteria and Viruses meeting in Montreal (1962). It is these viruses that I wish to consider now.

Abstract:BACKGROUND: Obesity and knee osteoarthritis adversely affect activities of daily living in older adults. Together, the complexities of their interaction on mobility, including stair negotiation, are unresolved. The purpose of this study was to determine the relationship between obesity, pain, and stair negotiation in older adults with knee osteoarthritis.METHODS: Older adults with symptomatic knee osteoarthritis and overweight or obesity participated in the study (n = 28; age range = 57.0-78.0 yrs.; body mass index range = 26.6-42.8 kg•m-2). The Western Ontario and McMaster Universities Osteoarthritis Index pain subscale was used to measure knee pain. Measurements included a three-dimensional biomechanical analysis during descent on a set of force plate-instrumented stairs and a timed stair descent test. Pearson's r was used to determine associations between body mass index and pain, stair descent weight-acceptance phase vertical ground reaction force (vGRF) variables and lower extremity joint kinematics and kinetics, and timed stair descent performance.FINDINGS: Significant correlations existed between body mass index and pain (r = 0.41; p = 0.03), peak vGRF (r = 0.39; p = 0.04), vertical impulse (r = 0.49; p = 0.008), and peak ankle plantar flexor moments (r = 0.50; p = 0.007) in older adults with knee osteoarthritis.INTERPRETATION: Greater obesity in older adults with knee osteoarthritis was associated with greater knee pain and higher ankle joint loads during stair descent. These results support the recommendations of osteoarthritis treatment guidelines for weight-loss as a first-line of treatment for older adults with obesity and knee osteoarthritis.

Abstract:Purpose: To generate hypothesis regarding the associations of knee cartilage defects in an obese population with and without OA. Methods: Obese subjects (BMI> 30) were recruited from laparoscopic adjustable gastric banding or exercise and diet weight loss programs. ACR clinical criteria for OA knee were determined. MRI assessment was conducted on a 3T machine (Magnetom Trio; Siemens, Erlangen, Germany) at baseline. Cartilage defects were graded according to an established protocol at medial tibial, medial femoral, lateral tibial, lateral femoral and patellar sites (grade 0−4). Composite scores for medial tibiofemoral, lateral tibiofemoral and whole compartments were obtained. All subjects completed a general health and musculoskeletal (MSK) questionnaire, WOMAC, SF-36, AQoL, Moorehead-Ardelt Quality of Life Questionnaire II (developed in an obese population), and Multidimensional Assessment of Fatigue. Physical assessment was conducted by a single assessor. The MSK questionnaire assessed joint pain; activity level at work, home and leisure; medication use; previous injury and previous surgery. Physical assessment parameters included: range of knee and hip motion; crepitus; alignment; quadriceps strength; waist, hip and knee circumference. Correlations between cartilage defect scores and MSK outcomes were obtained (Stata software). Regression analysis corrected for age, gender, BMI and presence of knee OA. Results: There were 111 subjects (78 women and 33 men) with mean BMI 39.9±5.8 and mean age 51±12 years. Fifty-three (48%) subjects met ACR criteria for knee OA. Knee cartilage defects were significantly associated with BMI (p< 0.03), presence of knee OA (p< 0.0001), knee pain (p< 0.05), and duration of knee pain (p< 0.01) in the medial tibiofemoral and whole compartments. There was statistically significant association with BMI and the lateral tibiofemoral compartment (p< 0.01). Significant associations were also seen with physical function, role physical, bodily pain and general health on SF-36 (p< 0.05); pain, stiffness and function on WOMAC (p< 0.0001); and medication use and independence subscale (p< 0.02) on AQoL for both the medial tibiofemoral and whole compartments. The association between knee cartilage defects, and BMI (p< 0.04) and knee OA (p< 0.0001) remained significant in the medial tibiofemoral and whole compartments on regression analysis. ICCs for intra-observer reproducibility were >0.98 for all compartments. Conclusions: Knee cartilage defects scores are significantly associated with BMI, knee OA, the presence and duration of knee pain. Cartilage defects are also significantly associated with a number of quality of life measures indicating clinical significance. They provide a time-efficient means of assessing articular cartilage which is reproducible. With the rising prevalence of obesity, it is important to assess the natural history of cartilage defects in the obese population, a group at risk for both incident and prevalent OA.

Abstract:Background: A high body mass index (BMI) is associated with increased rates of complications after total knee arthroplasty (TKA). However, no study has examined the effect of BMI on lower limb alignment using the World Health Organization's (WHO) BMI classification. We believe that the WHO's BMI classification allows a uniform standard worldwide. We sought to investigate the potential association between a high BMI and the incidence of postoperative misalignment. We also evaluated whether a higher BMI is associated with worse clinical function.Methods: We retrospectively reviewed the data of 671 patients who underwent primary TKA for varus osteoarthritis between January 2010 and December 2015. The patients were divided into the following 5 groups based on their BMI: normal weight (<25.0 kg/m2), overweight (25.0-29.9 kg/m2), class I obese (30.0-34.9 kg/m2), class II obese (35-39.9 kg/m2), and class III obese (>40 kg/m2). Both weight and height were measured by nurses on admission. Patients' preoperative HKA, gender, age, and side of surgery were collected as baseline. All the patients underwent standing, weight-bearing, full-length radiography before and after surgery to measure the mechanical hip-knee-ankle angle (HKA). We followed up patients by telephone. Among the BMI subgroups, we compared the knee function scores, including the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score, Knee Society-Knee Score (KS-KS), Knee Society-Function Score (KS-FS), Forgotten Joint Score (FJS), and range of motion (ROM). A multivariate linear regression analysis and a logistic regression was conducted to examine the outcomes.Results: The study had a mean follow-up period of 8.16 years. The multivariate and logistic regression analyses revealed that preoperative alignment (P=0.002) and a higher BMI (P=0.015) were associated with a higher risk of postoperative misalignment. The WOMAC scores were higher in the normal and overweight groups than the other groups (P=0.022). The FJS and KS-KS gradually decreased as BMI increased.Conclusions: A higher BMI is associated with a greater risk of misalignment and worse long-term clinical outcome after TKA. When treating patients with high BMI, we should pay more attention to the adjustment of lower limb alignment intraoperatively.

Abstract:Background: Obesity is a crucial factor that increases the risk of initiating and advancing knee osteoarthritis. However, it remains unclear how obesity directly impacts the biomechanical experience of the lower limb joints, potentially triggering or exacerbating joint degeneration. This study investigated the interactive effects of BMI augmentation on lower limb kinematics, kinetics, and muscle activations during walking. Methodology: A group of 60 participants underwent a three-dimensional gait analysis. These individuals were categorized into three groups based on their body mass index (BMI): those with a BMI below 25 were classified as having a healthy weight, those with a BMI between 25 and 30 were categorized as overweight, and those with a BMI exceeding 30 were considered obese. This study analyzed the gait of 60 participants categorized by BMI. During walking trials, they recorded ground reaction forces electromyography of leg muscles like the gastrocnemii, hamstrings, and quadriceps. Lower limb joint angles and net moments were also calculated. Statistical mapping identified variations in kinematic, kinetic, and muscle activation patterns across the stance phase between BMI groups. Results: The results displayed distinct biomechanical patterns in obese individuals. Notably, there was a significant increase in flexion observed in the hip and knee joints (P < 0.001) during the initial stance phase and an increase in hip and knee adduction angles and moments throughout the entire stance phase (P < 0.001). Additionally, muscle activations underwent significant changes (P < 0.01), with a positive correlation noted with the BMI factor. This correlation was most pronounced during the early stance phase for the quadriceps and hamstring muscles and the late stance phase for the gastrocnemius. Conclusion: These findings represent a comprehensive picture that contributes to understanding how excess weight and obesity influence joint biomechanics, highlighting the associated risk of joint osteoarthritis.
The predictors to self-reported and performance-based physical function in knee osteoarthritis patients: a cross-sectional study

Abstract:Background: Osteoarthritis (OA) knee patients have limited ability in physical function, or difficulties with physical tasks and activities may develop disability. This study aimed to observe the predictors of self-reported and performance-based physical function in patients with knee OA by analyzing the impacts of demographic, pathological, and muscle impairment factors. Methods: 135 knee OA patients participated in this study to complete self-reported questionnaires using Knee Injury and Osteoarthritis Outcome Score (KOOS). When measuring performance-based physical function, a 6-meter gait speed (6MGS) test was measured to evaluate their mobility, and a 5-time Sit-to-Stand test (5STS) was assessed to evaluate their balance. Pain intensity, knee extensor and flexor muscle strength, age, body mass index (BMI), durations of symptoms, and radiographic severity were also collected. Spearman correlation and stepwise multiple linear regression were used to explore the association and predictors in self-reported and performance-based physical function. Results: BMI and durations of symptoms did not indicate any significant correlation with either self-reported or performance-based physical function. Age is significantly negatively associated with 6MGS ( r 2 = −0.383, p < 0.01), while knee extensor muscle strength has a moderate correlation with 5STS ( r 2 = −0.528, p < 0.01). In the stepwise multiple linear regression models, pain intensity (β = 0.712, p < 0.001), knee flexor muscle strength (β = 0.112, p = 0.042) were significantly associated with self-reported physical function in daily activities and contributed to 55.0% of the variance in KOOS-PF score. Knee muscle strength, including knee extensor (5STS: β = −0.428, p < 0.001) and flexor muscle strength (6MGS: β = 0.367, p < 0.001), were the main predictors with performance-based physical function. Conclusion: Pain intensity was the leading risk factor of self-reported physical function, and knee flexor muscle strength contributed as well. The severity of knee OA, durations of symptoms and BMI did not contribute to physical function. However, knee extensor and flexor muscle strength were the main predictors of performance-based performance. Our results show that strengthening of weak knee muscles in both quadriceps and hamstring muscle strength should be considered a priory consideration in knee OA no matter if people are in the early or end-stage of knee OA.

Abstract:Myeloproliferative disorders predispose individuals to bleeding and thrombosis, often with devastating consequences. We report a 41-year-old man who presented with headache, amnesia and dysphagia due to cerebral haemorrhage. Extensive investigation revealed the cause of the neurological syndrome as an underlying essential thrombocytosis. The patient made a full recovery following extensive inpatient and community rehabilitation, returning to work after 6 months. We discuss the diagnosis and management of stroke due to myeloproliferative conditions. This case report illustrates the importance of multidisciplinary teamwork in fully investigating and treating all patients with acute onset of stroke.

Abstract:Knee osteoarthritis (OA) is a growing source of pain and disability. Obesity is the most important avoidable risk factor underlying knee OA. The processes by which obesity impacts osteoarthritis are of tremendous interest to osteoarthritis researchers and physicians, where the joint mechanical load is one of the pathways generally thought to cause or intensify the disease process. In the current work, we developed a hybrid framework that simultaneously incorporates a detailed finite element model of the knee joint within a musculoskeletal model to compute lower extremity muscle forces and knee joint stresses in normal-weight (N) and obese (OB) subjects during the stance phase gait. This model accounts for the synergy between the active musculature and passive structures. In comparing OB subjects and normal ones, forces significantly increased in all muscle groups at most instances of stance. Mainly, much higher activation was computed with lateral hamstrings and medial gastrocnemius. Cartilage contact average pressure was mostly supported by the medial plateau and increased by 22%, with a larger portion of the load transmitted via menisci. This medial compartment experienced larger relative movement and cartilage stresses in the normal subjects and continued to do so with a higher level in the obese subjects. Finally, the developed bioengineering frame and the examined parameters during this investigation might be useful clinically in evaluating the initiation and propagation of knee OA.
