Abstract:Background: Total colonic atresia is an uncommon congenital anomaly associated with neonatal intestinal obstruction, often necessitating prompt diagnosis and surgical intervention to prevent life-threatening complications. Understanding the etiology and optimal management strategies is crucial for ensuring favorable outcomes. Case Presentation: We present a case of a full-term male neonate with total colonic atresia, who presented with failure to pass meconium, abdominal distension, and vomiting. Diagnostic imaging confirmed intestinal obstruction, leading to surgical exploration revealing an atretic colon extending from the Ileocolic Junction to the Rectosigmoid Junction. An ileostomy was performed, followed by meticulous postoperative care, resulting in a successful resolution of symptoms and discharge from the hospital. Conclusion: Timely recognition and staged surgical management are paramount in addressing total colonic atresia, emphasizing the importance of early intervention for optimal patient outcomes. Long-term follow-up and comprehensive care are essential to ensure the overall well-being of affected newborns.

Abstract:Background: Congenital ileal atresia is a rare neonatal disease, the most common type of intestinal malformation in newborns, and one of the most common causes of congenital intestinal obstruction. It can cause various digestive system symptoms, including abdominal distension, vomiting, abnormal bowel movements, etc. In severe cases, it can be life-threatening. A prenatal ultrasound examination can assist clinical diagnosis of congenital ileal atresia, and those with a clear prenatal diagnosis should undergo surgical treatment after birth. Case Presentation: We have, herein, reported two cases of congenital ileal atresia, both of which showed fetal intestinal dilation (>7mm) and excessive amniotic fluid on prenatal ultrasound. Both newborns underwent surgical treatment after delivery and were confirmed to have congenital ileal atresia during surgery. Due to the different prenatal ultrasound manifestations of the two patients, they were divided into two different subtypes based on intraoperative manifestations. We observed significant differences in the prognosis of the two patients after surgery. Conclusion: Accurately locating and classifying ileal atresia using prenatal ultrasound is challenging; however, it plays an effective role in disease progression, gestational assessment, and prognosis. Accurately identifying intestinal diseases and/or the location of lesion sites through direct and indirect ultrasound findings in the fetal abdominal cavity is an important research direction for prenatal ultrasound.

Colonic atresia and Hirschsprung's disease in a neonate: A case report

Abstract:Introduction: Colonic atresia is a rare form of intestinal atresia that can be encountered in neonates. Although uncommon, other disease processes can be found simultaneously including malrotation, additional atresias, gastroschisis, and Hirschsprung's disease. Case presentation: A 2-day-old female neonate with known maternal polysubstance use was found to have colonic atresia on contrast enema after emesis and failure to pass meconium. Abdominal exploration revealed a blind ending cecum with evidence of ischemia along with an atretic transverse colon. An ileocecectomy with end ileostomy and transverse colon mucous fistula creation were performed. After eventual ileostomy reversal at 5 weeks of age, she struggled with intermittent oral intolerance and inconsistent bowel function. Re-exploration with ileostomy and gastrostomy tube placement was performed with additional biopsies revealing Hirschsprung's disease. Clinical discussion: Concomitant colonic atresia and Hirschsprung's disease is a rare clinical entity that provides challenges in diagnosis and definitive surgical management. The suspected source of atresia in this case was presumed to be due to an intra-uterine vascular accident given maternal polysubstance use. Delays in diagnosis can lead to increased patient morbidity. Conclusion: Even with a clear suspected etiology for colonic atresia, surgeons must maintain a high clinical suspicion for additional pathologies including but not limited to Hirschsprung's disease. Rectal suction biopsies should be performed if clinical suspicion arises for Hirschsprung's disease.
The prevalence of ABO blood group antigens and antibodies in Lagos State, Nigeria.

Abstract:A total of 1239 normal donors from the Lagos University Teaching Hospital (LUTH) and 111 staff of the National Institute for Medical Research (NIMR) Yaba were screened for ABO antibodies. Of the number from LUTH, 220 (17.8%) were found to be in group A, 282 (22.8%) in group B, 85 (6.9%) in group AB and 652 (52.6%) in group O. The number from NIMR consisted of 20 (18.0%) in group A, 25 (22.5%) in group B 8(7.2%) in group AB and 58 (52.3%) in group O. The mean tile avidity time of sera from 789 (62.66%) potent LUTH donors was less than 35 seconds. Only 97 (6.91%) of this reacted within 10 seconds. On the other hand, only 11(9.9%) of the NIMR sera reacted within 35 seconds and none reacted within 10 seconds. Group O individuals from LUTH and NIMR did not always have anti-A and anti-B components of their sera with equal avidity or potency. It was also observed that high avidity of antibody did not necessarily correspond with high potency. The commonest titre for group B (anti-A) sera was 256 and that for group A (anti-B) was 512. In general, anti-B titres tended to be consistently higher than anti-A. There was a bimodal peak at titres 32 and 256 in group B (anti-A) sera. This repeated itself in the anti-A component of group O sera (i.e., anti-A+B), but here the peaks occurred at 32 and 128.(ABSTRACT TRUNCATED AT 250 WORDS)
Congenital duodenal obstruction in neonates: a decade’s experience from one center

Abstract:Background Congenital duodenal obstruction (CDO) is one of the most common anomalies in newborns, and accounting for nearly half of all cases of neonatal intestinal obstruction. This study aimed to review our single-center experience in managing congenital duodenal obstruction while evaluate the outcomes. Methods We conducted a retrospective analysis of the records of all neonates dianogsed with congenital duodenal obstruction admitted to our center between January 2003 and December 2012. We analyzed demographic criteria, clinical manifestations, associated anomalies, radiologic findings, surgical methods, postoperative complications, and final outcomes. Results The study comprised 287 newborns (193 boys and 94 girls). Birth weight ranged from 950 g to 4850 g. Fifty-three patients were born prematurely between 28 and 36 weeks’ gestation. Malrotation was diagnosed in 174 patients, annular pancreas in 66, duodenal web in 55, duodenal atresia or stenosis in 9, preduodenal portal vein in 2, and congenital band compression in 1. Twenty patients had various combinations of these conditions. Presenting symptoms included bilious vomiting, dehydration, and weight loss. X-rays of the upper abdomen demonstrated the presence of a typical double-bubble sign or air-fluid levels in 68.64% of patients, and confirmatory upper and/or lower gastrointestinal contrast studies were obtained in 64.11%. Multiple associated abnormalities were observed in 50.52% of the patients. Various surgical approaches were used, including Ladd’s procedure, duodenoplasty, duodenoduodenostomy, duodenojejunostomy, or a combination of these. Seventeen patients died postoperatively and 14 required re-operation. Conclusions Congenital duodenal obstruction is a complex entity with various etiologies and often includes multiple concomitant disorders. Timely diagnosis and aggressive surgery are key to improving prognosis. Care should be taken to address all of the causes of duodenal obstruction and/or associated alimentary tract anomalies during surgery.
Congenital Neonatal Intestinal Obstruction: Retrospective Analysis at Tertiary Care Hospital

Abstract:Background: The purpose of this study is to analyze the etiology, clinical presentation and outcome of neonatal intestinal obstruction at our institute.Materials and Methods: The medical record of all the patients, presented with intestinal obstruction in neonatal period during 2014 and 2015 was reviewed retrospectively for etiology, clinical features, investigations, management, and outcome.Results: Out of total 53 cases of neonatal intestinal obstruction, 27 were of intestinal atresia (9 cases (17%) were of duodenal atresia, 7 (13%) were of jejunal atresia and 8 (13%) were ileal atresias and 3 cases were found with colonic atresia); 7 were malrotation, 17 were Hirschsprung’s disease (HD). All the patients were investigated with abdominal radiography and sonography. All patients were managed surgically. Overall mortality was 10/53 (18.8%). Out of 27 cases of atresia, 9 patients died (33% mortality). Septicemia was the cause of death in 7 patients (58.3%). Anastomotic leak was present in one mortality case.Conclusion: The most common cause of neonatal intestinal obstruction is atresia. Duodenal atresia was the most common atresia in our study followed by ileal atresia. Postoperative complications like septicemia led to most of deaths in our series. Septicemia, wound infection, hypothermia, prematurity need special attention for survival of neonates.

Several investigators have proposed the use of hierarchical video as a solution for dissemination of video across heterogeneous networks of this sort. Abstract:E ffective clinical management and control of hypertension remains a major challenge for physicians operating in different clinical settings, worldwide. Despite the majority of physicians reporting relatively high rates of perceived blood pressure control [1], over the past decade, observational studies and epidemiological surveys still have consistently and systematically showed relatively low proportions of hypertensive patients who attained the recommended blood pressure targets [2–6]. Several factors have been claimed to explain this gap between perceived and attained blood pressure control, among which some are related to patients (e.g. lack of adherence and frequent discontinuations from prescribed antihypertensive therapy due to drug-related side effects or adverse reactions) and some others to physicians (e.g. use of inappropriate therapeutic regimens, extensive use of monotherapy, complex combination therapies, and association of nonsynergistic antihypertensive drugs) [7]. Mostly, therapeutic inertia – that is the lack of therapeutic intervention in the presence of treated uncontrolled blood pressure levels – has emerged as a frequent and potentially modifiable factor for improving blood pressure control in daily clinical practice. In this issue of the Journal, a study by Escobar et al. described the main findings of a systematic and comprehensive analysis performed on the PRESion arterial en la poblacion Espanola en los Centros de Atencion Primaria [PRESCAP] databases, which collected data on temporal trends on blood pressure control rates achieved in Spain (three dataset of studies performed in 2002, 2006 and 2010), reporting that 36.1, 41.4 and 46.3% of the included patients
On titanium frameworks and alternative impression techniques in implant dentistry.

Abstract:PURPOSE The objectives of this thesis were to compare the clinical and material performance of implant-supported titanium frameworks with gold-alloy frameworks in the edentulous jaw and to test a digitized impression technique to optimize material characteristics and clinical and laboratory procedures. MATERIALS AND METHODS Two different kinds of laser-welded titanium frameworks and one type of digitized computer numeric controlled (CNC)-milled titanium framework for the edentulous jaw were evaluated and compared with gold-alloy frameworks in two clinical 5-year follow-up studies. The CNC frameworks were also evaluated and compared with gold-alloy frameworks in two experimental studies according to fit, preload, and surface roughness. Furthermore, a digitized photogrammetric technique for recording implant positions in the edentulous jaw was experimentally tested and compared with two conventional impression techniques. RESULTS Laser-welded titanium frameworks had a lower success rate after 5 years in function than gold-alloy frameworks. The 5-year performance of CNC frameworks was similar to that of gold-alloy frameworks. The fit and precision of implant-supported CNC frameworks was significantly better compared with gold-alloy frameworks. Applications of veneering material did not significantly affect the fit of the titanium frameworks. The preload of CNC frameworks was similar to that of gold-alloy frameworks, both exhibiting significantly decreased preloads after repeated tightening. The preloads of titanium frameworks were similar before and after veneering. Unloaded milled titanium screw sites had significantly rougher surfaces than loaded sites, and loaded gold screws had rougher surfaces than unloaded. No correlation between surface of the screw joints and preload of veneered titanium frameworks was observed. The precision of photogrammetric impressions was comparable to that of conventional impression techniques. CONCLUSIONS Implant-supported CNC-milled titanium frameworks are a viable alternative to gold-alloy castings in the edentulous jaw, and because a digital platform is used to manufacture these titanium frameworks, photogrammetric impressions can be made.

Abstract:OBJECTIVE To compare intraluminal pressure at initial leakage (leakage pressure), leakage location, and maximum intraluminal pressure (MIP) for various staple line offset configurations of functional end-to-end stapled anastomosis (FEESA). SAMPLE Grossly normal jejunal segments from 4 canine cadavers. PROCEDURES 52 jejunal segments (4 control and 24 anastomosis constructs [2 segments/standard FEESA construct]) were prepared for testing. Segments were assigned to three 8-segment gastrointestinal anastomosis staple line offset groups: complete offset (CSO group), partial gastrointestinal anastomosis offset (PSO group), and no gastrointestinal anastomosis offset (NSO group). Results for leakage pressure, leakage location, and MIP were compared. RESULTS Mean ± SD leakage pressure differed significantly among all groups and was highest for the PSO group (34.4 ± 3.7 mm Hg), followed by the CSO group (25.9 ± 4.1 mm Hg) and the NSO group (18.8 ± 1.5 mm Hg). Leakage location did not differ significantly among groups but was most commonly associated with the thoracoabdominal staple line. The MIP did not differ significantly among groups (PSO, 83.1 ± 9.4 mm Hg; CSO, 81.7 ± 6.7 mm Hg; and NSO, 58.5 ± 7.7 mm Hg). CONCLUSIONS AND CLINICAL RELEVANCE In this study, partial staple line offset leaked at a significantly higher pressure, which represented the greatest leakage protection of tested constructs. The thoracoabdominal staple line was more susceptible to leakage than was the gastrointestinal anastomosis staple line. Results suggested that surgeons should avoid FEESA with no staple line offset, strive for partial offset of the gastrointestinal anastomosis staples, and provide precise placement of the thoracoabdominal staple line.
Simple Process Equations, Fixed-Point Methods, and Chaos

Abstract:The chaotic behavior of fixed-point methods for steady-state process simulation is studied. It is shown that direct substitution and Newton’s method exhibit all of the rich structure of chaos (period doubling, aperiodicity, fractal basin boundaries, and related properties) on simple process examples. These examples include finding roots to the SoaveRedlich-Kwong and Underwood equations, dew point and flash calculations for heterogeneous mixtures, and a simple process flowsheet. For single variable problems, it is shown that direct substitution follows a classical period-doubling route to chaos. On the other hand, the chaotic behavior of direct substitution and Newton’s method on multivariable problems is considerably more complex, and can give the appearance that no organized route to chaos is followed. For example, for the dew point problems, truncated period doubling, odd periodic cycles, and chaotic behavior can be observed, within which are embedded narrow regions of global convergence. Many numerical results and geometric illustrations are presented.

Abstract:BACKGROUND: Failure to pass meconium by a full-term neonate within the first 24 hours should raise a suspicion of bowel obstruction. The objective of this study was to determine pattern of presentation, diagnosis and outcome of management of intestinal obstruction in the early neonatal period in the Neonatal Care Unit of Tikur Anbessa Specialized Hospital in Addis Ababa, Ethiopia.METHODS: Retrospective chart review of admitted cases from January 2011 to December 2013 was done. Data were entered into SPSS software version 20. Descriptive analysis of data were done at first; then cross tabulation of variables with chi-square tests were done.RESULTS: Fifty-one (41 males and 10 females) cases of neonatal intestinal obstructions were described. ARM was the most common type of intestinal obstruction followed by intestinal atresia and Hirschsprung's disease.Delayed diagnosis of cases was observed in 72%. Associated congenital anomalies were documented in 13 % of the cases. Death occurred in 20% of the cases. One or another form of infection accounted for 90% of the deaths. Neonatal sepsis and being unfit for anesthesia were determinants of mortality with p-value <0.02 and < 0.0007 respectively.CONCLUSION: Anorectal malformation is the commonest cause of early neonatal obstruction followed by small intestinal atresia and Hirschsprung's disease. Low number of associated anomalies has been reported. High rate of death due to preventable causes such as sepsis is observed. Delayed diagnosis of cases were also prevalent.

Abstract:Introduction: One of the most prevalent congenital wall abnormalities in children, umbilical hernias are often linked to premature or small-for-gestational-age babies. In cases of intestinal malrotation or if the cecum is very movable, generalized peritonitis may facilitate the imprisonment of these hernias. Case report: We described a case of a 4-month-old baby who had a prior reducible umbilical hernia with a history of fever, vomiting, poor appetite, and constipation for around 48 h. The patient experienced significant intestinal bloating, vomiting, irreducibility of the umbilical hernia, skin pigmentation, and erythema at the umbilical site within 2 days after hospitalization. When there was no free abdominal air, a direct abdominal x-ray revealed evidence of hydro-gas stasis and various hydro-aerial levels that were pertinent to the ileum. In order to reduce the hernia, the patient had an emergency surgical treatment where the hernia sac was isolated and released from the ileal loop, which was securely attached to a fibrin plate. When the herniary sac was opened, a gangrenous and perforated appendix was found inside. On the seventh postoperative day, the patient was released from the hospital after an uncomplicated postoperative stay. Conclusion: Our patient's clinical presentation is similar to that of only one other case report involving a 25-day-old male patient. Our case presented with a variant of the clinical symptoms of the previously described umbilical hernia, which became unfixable and strangulated as a result of appendix inflammation. The appendix was discovered inside the hernia sac during surgery.

Abstract:Introduction and importance: Thoracoabdominal duplication and intestinal malrotation are extremely rare congenital alimentary tract anomalies that can manifest in any segment of the gastrointestinal tract. Still, tubular duplications are an even rarer subset of alimentary tract duplications. Misdiagnosis could occur and this will yield devastating ramifications. Therefore, consideration in the clinical settings is warranted to aid in conducting timely therapeutic interventions. Case presentation: In this article, we illustrate the overwhelmingly rare occurrence of thoracoabdominal duplication coexistent with intestinal malrotation in a 7-month-old male whose primary complaint was chronic dyspnoea since birth that progressed to involve cough and fever. Imaging analysis revealed a significant intrathoracic fluid-filled cyst. Clinical discussion: The intestinal malrotation was treated through Ladd's procedure, and surgical excision of the duplicated segments was accomplished. The subsequent analysis of the resected specimens via means of histopathology utilizing Hematoxylin and Eosin dyes established the definitive diagnosis of a foregut duplication cyst. Conclusion: Thoracoabdominal duplication is one of the most crucial topics in the field of Paediatric Surgery. It is exceptionally rare in occurrence, and the scarcity of available resources that document and describe this topic is evident in the published literature. The authors must opt to document, study, and broaden awareness regarding this life-threatening pathology so that they can circumvent the resultant complications by means of early detection and the performance of apt surgical interventions. Upon careful review of the available literature, we can state that ours is the first-ever case documented from their country regarding this topic and this co-incidence.
Accompanied anomalies in anal atresia or tracheo-esophageal fistula: Comparison with or without VACTERL association

Abstract:Purpose: We evaluated the various accompanied malformations in patients with anal atresia or tracheoesophageal fistula (TEF). Furthermore, we determined the prevalence of VACTERL association and compared the clinical findings with those of patients without VACTERL association. Methods: We enrolled the patients with anal atresia or TEF with/without esophageal atresia. We collected the patient data pertaining to accompanied vertebral, cardiovascular, renal or limb anomalies, single umbilical artery, maternal diabetes mellitus or drug history, and gene research. Results: A total 155 patients (65 boys and 90 girls) were enrolled with 147 cases of anal atresia, 3 cases of TEF, and 5 cases of anal atresia with TEF. The prevalence of accompanied anomalies was 67.1% in cardiovascular, 27.1% in renal, 9.7% in vertebral, 2.6% in limb anomalies, and 3.9% in single umbilical artery. Thirty-six (23.2%) patients were diagnosed with VACTERL association. The patients with VACTERL association had a significantly higher number of male patients (58.3 vs. 37.0%, p = .033) and single umbilical artery (11.1 vs. 1.7%, p = .026), and had a significantly lower birth weight (2.8 vs. 3.1 kg, p = .033) than the patients without VACTERL association. Genetic studies were performed in 111 patients, and 8 (7.2%) had chromosomal abnormalities-3 in VACTERL and 5 in no VACTERL group. Conclusion: We recommend a careful evaluation for VACTERL association in patients with anal atresia or TEF. It is particularly important to screen for a single umbilical artery for features of VACTERL association as well as for other congenital anomalies.
Neonatal Renal Failure in the Setting of Anorectal Malformation: A Case Report and Literature Review

Abstract:Anorectal malformations (ARMs) can occur in isolation or in association with other anomalies, most commonly those of the genitourinary systems. Morbidity and mortality are highest among patients who develop end-stage renal disease (ESRD) either from severe congenital anomalies (dysplastic kidneys) or from repeated infections in those who have vesicoureteral reflux or persistent recto-urinary fistulas. We describe our management strategy for a patient born with an ARM and bilateral dysplastic kidneys to highlight the nuances and complex decision-making considerations required in taking care of this complex patient population. Our patient is a male twin born at 32 weeks' gestational age who was found to have bilateral dysplastic kidneys on prenatal ultrasound. On initial examination, an imperforate anus was identified along with a severe urethral stricture. Full workup also revealed sacral dysgenesis and confirmation of the dysplastic kidneys. On day of life 3, a laparoscopic diverting sigmoid colostomy was performed; urologic evaluation confirmed the severe urethral stricture, which required dilation to place an 8F council tip catheter. Due to his small size, peritoneal dialysis could not be initiated until five weeks of age. As full volumes could not be reached with peritoneal dialysis, he was soon transitioned to continuous renal replacement therapy. At five months of age, a laparoscopic-assisted posterior sagittal anorectoplasty (PSARP) was performed. As his urethral stricture had worsened, a suprapubic catheter had been placed for bladder decompression. Reversal of his colostomy was performed 15 days after PSARP. Unfortunately, the patient required three further surgical interventions due to abdominal wall and inguinal hernias contributing to filling and emptying dysfunction when utilizing peritoneal dialysis. He is currently 16 months of age and remains inpatient due to intermittent hemodialysis requirements along with autocycling of his peritoneal dialysis. He is working on developmental milestones, can pull to a stand, and is currently being evaluated for kidney transplantation. The development of ESRD in a neonate or infant with an ARM is rare and can be due to congenital dysplasia or agenesis of bilateral kidneys. While peritoneal dialysis is the preferred approach, catheter dysfunction can result from intra-abdominal adhesions or inadequate fluid removal from inguinal or abdominal wall hernias that form in the setting of increased intra-abdominal pressure required for peritoneal dialysis. Close collaboration is required between pediatric surgeons, nephrologists, and urologists to facilitate colonic and urologic reconstruction and manage catheter-related complications.
Analysis of 1,992 patients with anorectal malformations over the past two decades in Japan

Abstract:BACKGROUND/PURPOSE: This report describes the results of a group study of the Japanese Study Group of Anorectal Anomolies (JSGA) to determine the relative incidence of specific types of anorectal anomaly in Japan, and includes discussion of rectourethral fistula regarding the relationship between the levels of the fistula and blind end of the rectum, low type deformity, rare types, and associated anomalies.METHODS: A total of 1,992 patients (1,183 boys and 809 girls) registered from 1976 to 1995 were analyzed.RESULTS: High-type deformities accounted for 26.0% of cases, intermediate 10.7%, low 57.2%, miscellaneous 4.5% and unclassified 1.8%. The most frequent deformity was male anocutaneous fistula (n = 364), followed by male rectourethral fistula (n = 333), and female anovestibular fistula (n = 241). There were 42 rectovesical fistulas in boys and 93 rectocloacal fistulas in girls. Covered anus complete occurred at the same frequency (10.1% of low deformities) as covered anal stenosis. In rectourethral fistula, the blind end of the rectum lay at or above the level of the P-C line in 40.3% of cases, at or above the M line in 39.6% and at the vicinity of the I line in 20.2%, respectively. There was no parallel relationship between the site of the fistula opening and the level of the rectal pouch. The overall incidence of patients having one or more associated anomalies was 45.2%: 70.6% in high deformity, 60.7% in intermediate, and 31.3% in low. The rate of association of Down's syndrome with deformities without fistula (40.3%) was significantly higher than with deformities with fistula (0.3%).CONCLUSIONS: Rectovesical fistula and covered anus complete were not infrequent deformities in this series. We consider that at least 20% of rectourethral fistula should be categorized as intermediate or low deformity from the viewpoint of the position of the rectal pouch. A significant preponderance of Down's syndrome in the deformities without fistula suggests that further investigation of associated anomalies in comparision with other congenital diseases may provide insights into the pathogenesis of anorectal malformation in the field of molecular genetics.

Abstract:Colon atresia is one of the rarest congenital anomalies of the gastrointestinal tract, with an incident range of between 1 in 10,000 and 66,000 live births. Type I colonic atresia affects only the mucosal layer of the intestine and spares the intestinal wall and mesentery. Hirschsprung Disease is a rare association of Colon atresia and is usually diagnosed as a complication of atresia treatment.

Abstract:We report a case of a 30-year-old female who presented with symptoms and signs of intestinal obstruction. The patient reported no previous history of abdominal surgery or trauma while clinical and radiographic examinations were not diagnostic. Patient planned for exploratory laparotomy in view of complete intestinal obstruction. Exploratory laparotomy with limited resection of gangrenous segment with primary closure of abnormal ileosigmoid communication with proximal ileostomy with distal mucous fistula. Intraoperatively distal 60cm of ileum, caecum and part of ascending colon was gangrenous there was a fibrous band extending from parities to ileum approximately 60 cm proximal to IC junction. Congenital bands are extremely rare. Their exact incidence is still unknown and usually observed in childhood. This case, therefore, represents an unusual surgical problem in an older individual in which the diagnosis was clinically unexpected. Small bowel obstruction is the most common surgical disorder of the small intestine. Adhesions are by far the most frequent causes followed by hernias, tumors, intussusception, foreign bodies, gallstones, and inflammatory bowel disease. Obstruction by a congenital band is extremely rare and usually observed in childhood. This report presents a 30-year-old female with symptoms of intestinal obstruction, subsequently treated by ligation and division of a congenital band.

Abstract:In a study of 526 students in two Philadelphia public high schools, the majority (135 of 265) who had been using drugs were found subsequently to have dropped out (failed to graduate) from high school, compared to only approximately one out of four (42 of 158) of the non-drug using students. In a multiple regression analysis, which controlled for twenty demographic, personal and family variables which had previously been found to have significant correlation with dropping out versus graduation from high school, the severity of the student's earlier drug use was still found to predict to failure to graduate, to a significant degree (F = 6.03). While drug use may not be the main cause of dropping out of high school, but only a concomitant effect of earlier, more basic state of disaffection from school, it is nevertheless clear that drug use by adolescents interferes with academic progress in high school.
