Abstract:Exposure to vapors from a formulation of 1% propoxur in oil caused an increase in movement and antennal and tarsal grooming in adult male German cockroaches, Blattella germanica (L.). Increased movement was the most immediate response, followed by increased antennal grooming. Increased tarsal grooming was the last response and was most likely a secondary response to other factors. Adult males of a resistant field strain were more active than those of a susceptible laboratory strain in control studies. However, susceptible males displayed greater responses to the controls than did resistant males after exposure to the insecticide.

Abstract:The effective implementation of myopia prevention and control interventions is the foundation of achieving the goal of myopia prevention and control in China. However, relevant guidance documents for the implementation and evaluation of myopia prevention and control interventions are still lacking. Therefore, after in-depth research and summarizing relevant practical experiences, the Vision Health Branch of Chinese Association for Student Nutrition and Health Promotion and the Public Health Ophthalmology Branch of Chinese Preventive Medicine Association discussed and proposed the Chinese expert consensus on comprehensive public health intervention for myopia prevention and control in children and adolescents. The consensus develops a theoretical framework of comprehensive intervention actions for myopia prevention and control from the perspective of public health, creates six comprehensive intervention actions, including the establishment of vision health records and myopia risk assessment, evaluation of vision care related behaviors and the development of proper eye-use behaviors, health education for teachers, students and parents, improvement of reading and writing environment at home and school, 2 hours of daytime outdoor activities, hierarchical management and medical orthosis of myopia, and proposes a system of indicators for the implementation and effectiveness evaluation of myopia intervention actions, so as to provide a cost-effective and universally appropriate public health prescription for myopia prevention and control in children and adolescents.
Effects of online family health education on myopia prevention in children by parental myopia: a randomized clinical trial

Abstract:CLINICAL RELEVANCE: Online family health education may be effective for myopia prevention in children, and the effects may be different between children with myopic and non-myopic parents.BACKGROUND: Myopia is a common cause of vision loss. The aims of this study were to evaluate the effects of online family health education on preventing the development of myopia in children, and to estimate whether the effects vary according to parental myopia.METHODS: A cluster randomised trial including grade 1 and grade 2 children from 12 primary schools was conducted in Guangzhou, China. Weekly online family health education messages were sent to parents in the intervention group. Data collection included eye examinations of children and questionnaires completed by parents.RESULTS: Among the 3123 children included at baseline (1703 boys [54.5%]; mean [SD] age, 6.83 [0.73] years), 2376 completed the follow-up after 3 years. The differences in the incidence of myopia and myopic shift between the study groups were not significant in total. However, the 3-year cumulative incidence rate of myopia in the intervention group (125 of 445 [28.1%]) was significantly lower than that in the control group (225 of 603 [37.3%]; difference, 9.2% [95% CI, -14.9% to -3.5%]; P = 0.001) among children with non-myopic parents. In parallel, among children with non-myopic parents, the mean myopic change in SER was less for the intervention group than for the control group (-1.10 D vs. -1.24 D; difference, 0.13 D [95% CI, 0.03 to 0.23 D]; P = 0.01).CONCLUSIONS: Compared with children with myopic parents, online family health education was more effective in children with non-myopic parents. The incidence of myopia and myopic shift in refraction have been reduced in children with non-myopic parents. Further studies are needed to assess these differences by parental myopia.

Abstract:Recently, a marked increase in screen time and a corresponding decrease in outdoor activities have led to a global surge in myopia, especially among children and adolescents. By 2020, about 30% of the global population was affected by myopia, with projections suggesting that this figure could approach 50% by 2050 [1]. The global economic impact of uncorrected myopia and myopic macular degeneration in 2015 was estimated at about 6 billion USD, respectively, with East Asia bearing the largest share [2, 3]. In China, the escalation of myopia, especially high myopia, particularly among school-aged children, poses a significant public health challenge. The 2020 prevalence among Chinese children and adolescents reached 52.7%, with high myopia at 17.6% among high school students [4]. Myopia occurs when the human eye, in a state of relaxed accommodation, focuses parallel light rays in front of the retina due to alterations in the refractive system of the eye. It is categorized into refractive myopia, where the refractive index anomalies cause excessive refractive power while maintaining normal axial length, and axial myopia, the more common type, characterized by an extended axial length beyond the normal [5]. Axial elongation, a key factor in ocular development, is strongly linked to refractive errors [6]. Genetic predispositions significantly influence high myopia, particularly in children of myopic parents, with increased risks correlated with the degree of parental myopia [5]. Additionally, environmental factors like extensive near-work exacerbate myopia development, whereas outdoor activity inversely impacts its progression [7].

Abstract:In 2018, a consortium of government bodies in China led by the Ministry of Education released the Comprehensive Plan to Prevent Nearsightedness among Children and Teenagers (CPPNCT), aiming to reduce the incidence of myopia and control myopic progression in China. Recommendations span from home-based to school-based interventions, including time outdoors, physical activity, light exposure, near-work activity, screen time, Chinese eye exercises, diet and sleep. To date, the levels of evidence for this suite of interventions have not been thoroughly investigated. This review has summarised the evidence of the interventions recommended by the CPPNCT in myopia prevention and control. Thus, the following statements are supposed by the evidence: (1) Increasing time outdoors and reducing near-work time are effective in lowering incident myopia in school-aged children. (2) All interventions have a limited effect on myopia progression. Ongoing research may lead to a better understanding of the underlying mechanisms of myopia development, the interaction of different interventions and recommendations, confounding variables and their true effect on myopia prevention, and the identification of those most likely to respond to specific interventions. This field may also benefit from longer-term studies of the various interventions or strategies covered within this review article, to better understand the persistence of treatment effects over time and explore more novel approaches to myopia control.

Abstract:Objective To investigate the associated factors leading to myopia in primary school students in a relatively developed region in Hangzhou.Methods This was a cross-sectional study.By cluster sampling,primary students from 5 schools (grades 1-6) in Zhaohui District,Hangzhou,were recruited for this investigation.The right eye was examined and data were collected on spherical refraction,cylinder refraction,axial length and corneal curvature.Information was also obtained on family history,time spent on near reading after school and on weekends,and time spent on outdoor activities both after school and on weekends.A one-way ANOVA was used for analysis.Results There were significant differences in gender,family history,time spent on near reading and outdoor activity between myopic and non-myopic groups.Females were more likely to be myopic than males (63.7％ vs.67.4％).The students with myopic parents (F=10.559,P＜0.05) or those who spent more time on near reading on weekends (F=7.555,P＜0.05) and after school (F=2.492,P＜0.05) were more likely to be myopic.On the other hand,outdoor activities both after school (F=11.351,P＜0.05) and during weekends (F=6.387,P＜0.05) reduced the prevalence of myopia.Conclusion Myopia in primary school students is associated with gender,family history,near reading and outdoor activity.So both genetic and environmental factors contribute to the development of myopia.The effect of environmental factors,especially near reading,is time-dependent,and ultraviolet exposure during outdoor activity plays a significant role.

Abstract:Question Can school-based family health education via social media control the development of myopia in children?Findings In this cluster randomized clinical trial of 1440 children, the 2-year cumulative incidence rate of myopia in the intervention group (106 of 544 [19.5%]) was lower than that of the control group (171 of 700 [24.4%]), but the mean myopic shift difference was less than 0.25 diopters and was not accompanied by any axial length differences.Meaning The findings of this study suggest that school-based, weekly family health education via a social media platform could prevent and control myopia in children; however, whether these findings extrapolate elsewhere in the world or are clinically relevant in controlling myopia in the long term is undetermined.Importance Myopia is a common cause of vision loss, and its prevalence is increasing globally.Objective To evaluate the effects of school-based family health education via WeChat in raising parents' awareness of myopia prevention and behavior and in controlling the development of myopia in children.Design, Setting, and Participants A single-masked cluster randomized clinical trial of children was conducted from October 1, 2018, to December 31, 2020, among grade 1 students from 12 primary schools in Guangzhou, China. The 12 primary schools were randomly selected in 2 districts and randomized to the intervention and control groups. All grade 1 students were invited to participate, and 688 students were included in the intervention group and 752 in the control group.Interventions Weekly health education via the social media platform WeChat was provided to the parents in the intervention group.Main Outcomes and Measures Data include results of eye examinations of children and questionnaires completed by parents. The primary outcome was the 2-year cumulative incidence rate of myopia. Myopia was defined as a spherical equivalent (SE) refractive error (sphere of +0.5 cylinder) of at least −0.50 diopters (D). The secondary outcomes were the 2-year changes in the axis length and SE refraction, parental awareness, children's screen time, outdoor activities, and learning tools during COVID-19.Results Among the 1525 children included at baseline (835 boys [54.8%]; mean [SD] age, 6.3 [0.5] years), 1244 competed the final assessment; the 2-year cumulative incidence rate of myopia was 106 of 544 (19.5%) in the intervention group and 171 of 700 (24.4%) in the control group (difference, 4.9% [95% CI, 0.3%-9.5%]; P = .04). The mean myopic shift in SE refraction in the intervention group (−0.82 D) was lower than that in the control group (−0.96 D; difference, −0.14 [95% CI, −0.22 to −0.06] D; P < .001). No difference in change in axial length was detected (difference, 0.02 [95% CI, −0.06 to 0.09] mm; P = .70).Conclusions and Relevance School-based weekly family health education via WeChat resulted in a small decrease in the 2-year cumulative incidence rate of myopia with a difference in SE of less than 0.25 D not accompanied by any axial length differences. Abstract:Myopia is a common eye disease that has become the main cause of vision loss among Chinese children and adolescents. In 2020, the overall myopia rate of Chinese children and adolescents was 52.7%. The problem of low myopia age is still prominent, and the number of high myopia is increasing year by year. Although the country has raised myopia prevention and control to a strategic level, the current situation has not been completely changed. There are still many difficulties and bottlenecks that have not been solved. The etiology of myopia is complex, and there is no universal prevention and control strategy. In view of the current situation of myopia prevention and control, the author starts from the concept of children and adolescents' eye development and myopia cognition, thinks about the way of medical and industrial cross assistance, takes refractive development profile as guidance, establishes a standard system for data collection of refractive development archives, establishes longitudinal data prediction, so as to provide means for early warning and early prevention of children and adolescents' eye health. In order to provide solutions to break through the bottleneck of myopia prevention and control.
The gap between parental knowledge and children practice of myopia control and challenge under COVID-19: a web-based survey in China

Abstract:Objective: To evaluate parental knowledge of myopia control, investigate its association with children's practice and refractive status, and explore their change under the outbreak of COVID-19 pandemic. Methods: In this web-based survey, a self-administered questionnaire was made online available during the COVID-19 outbreak between February 1th, 2022 and August 31th, 2022 in China. Participants were recruited via social media by convenience and snowball sampling. Parents of both sexes whose children aged between 3 and 18 were eligible. The overall questionnaire was composed of four categories: demographic information, parental knowledge of myopia, children's myopia-related behaviors and their change after the COVID-19 pandemic, and children's refractive status. SPSS version 18.0 was applied to perform the statistics analysis and p < 0.05 was considered to be statistically significant. Results: A total of 423 eligible families were included in our online survey. The average age of children was 11.37 ± 2.83y (male 46.1%; female 53.9%), with a myopia incidence of 83.9% (355/423). Both children's age (OR = –0.6; 95%CI = –1.12 to −0.07; p = 0.026) and family income (OR = 2.60; 95%CI = 1.13 to 4.07; p = 0.001) had independently significant impacts on parental knowledge. Unexpectedly, parental knowledge was negatively correlated with children's onset age of myopia ( p = 0.002, r = −0.165) and positively correlated with spectacles wearing ( p = 0.014, r = 0.131), and no correlation was found between parental knowledge and the occurrence of children myopia, current diopter, annual myopia progression and the diopter of the first glasses (all p > 0.05). We found discordance phenomenon between parents' knowledge and children's behaviors, with parental knowledge being irrelevant to children's sleeping time ( p = 0.159, r = 0.069), the frequency of lying reading ( p = 0.462, r = −0.036) and keeping nutrition diet ( p = 0.142, r = 0.072), and positively correlated with daily homework time ( p = 0.012, r = 0.123). After the outbreak of COVID-19, 77.8% (329/423) of parents admitted that their children's daily routine had been changed, with children spending more time on sleeping ( p < 0.001) and electronic products ( p < 0.001), and taking less time to do outdoor activities ( p < 0.001). Conclusion: The ideal interaction mode that establishing positive impact between parental knowledge and children practice has not been reached in China, which might be the result of insufficient parents' cognition and discordance phenomenon between parental knowledge and children's behaviors. The pandemic of COVID-19 has obviously changed children's daily routine. More efforts should be made to narrow the gap between knowledge and behaviors of myopia control, and stay alert to the potential increased risk of myopia during COVID-19.

Abstract:Purpose: This study aims to increase our understanding of the relationship between family and myopia in Chinese children. Methods: Students had a physical examination and were required to provide the necessary demographic information. Children and their guardians from different family types were required to fill in a questionnaire concerning myopia factors. Results: In this study, the prevalence of myopia in enrolled students aged 6-17 is 55.5%. The proportion of the nuclear family, extended family, single-parent family, and left-behind family is 40.6%, 43.7%, 11.1%, and 4.6%, respectively. Myopia rates from different family types by the order (nuclear family, extended family, single-parent family, and left-behind family) are 60.0%, 52.0%, 54.7%, and 50.9% taking on a decreasing trend, which shows an opposite trend comparing with elevated blood pressure, dental caries, and obesity. The interaction effect of the family type and region, physical examination, lifestyle (including diet habits, near work, outdoor activities, and sleep), and types of lamps and whether scolded by parents can have a significant impact on myopia. For primary school students (grade: 1-5), the prevalence of myopia in the nuclear family was a bit higher than that of myopia in the left-behind family, but for children in junior and senior high schools, both prevalences stayed similar. Conclusions: In this study, education pressure and time outdoors are still at play, and this kind of effect shows different phenomena in different families. Therefore, previous interventions would still work, and then the most critical challenge would be to ensure that left-behind children completed more schooling.
Barriers and Facilitators to Using Ophthalmic Clinical Health Services Following School Vision Screening: a Mixed-Methods Study

Abstract:Objective To identify determinants of the utilisation of ophthalmic clinical health services among students who failed school vision screening.Methods This study employed a sequential explanatory mixed methods design, underpinned by Andersen’s Behavioural Model of Health Service Utilisation. Data were initially gathered through interviews with 27 stakeholders—comprising 5 ophthalmologists, 7 community doctors, 7 public health professionals and 8 teachers. The qualitative insights informed the construction of a questionnaire, which subsequently garnered responses from 6215 participants. Qualitative data underwent thematic analysis with NVivo V.12, while quantitative data were analysed using multivariable multinomial logistic regression in SAS V.9.4. Data integration was performed using the Pillar Integration Process for a deductive, evidence-based synthesis of findings.Results The research revealed that students attending vision demonstration schools and receiving encouragement from schools or communities to access clinical ophthalmic services demonstrated higher adherence to referral (OR=1.66, 95% CI 1.30 to 2.12; OR=1.54, 95% CI 1.33 to 1.80). Conversely, older students and those from higher-income families exhibited lower adherence rates (OR=0.31, 95% CI 0.23 to 0.44; OR=0.34, 95% CI 0.25 to 0.46). Moreover, students with less urgent medical needs were more likely to adhere to referrals compared with those needing immediate referrals (OR=1.24, 95% CI 1.06 to 1.45).Four pillars emerged: (a) adherence decreased with age, (b) financial constraints did not pose an obstacle, (c) public health services played a critical role, (d) referral urgency did not linearly correlate with adherence.Conclusion The utilisation of ophthalmic clinical health services following vision screening failure in students is significantly influenced by public health services provided by schools or communities, such as prompting those with abnormal screening results to access ophthalmic clinical health services.
Improving myopia awareness via school-based myopia prevention health education among Chinese students

Abstract:Aim: To investigate the myopia awareness level, knowledge, attitude, and skills at baseline and to implement and evaluate the efficacy of myopia prevention health education among Chinese students. Methods: A total of 1000 middle school students from 2 middle schools were invited to participate in the study, and myopia prevention health education was conducted. The students were assessed at baseline, followed by a survey. The efficacy of health education was evaluated using the self-comparison method pre- and post-health education. Results: The study included 957 and 850 pre- and post-health education participants, respectively. The baseline knowledge of all respondents on myopic symptoms (87.5%), myopia is a risk of eyes (72.9%), myopia prevention (91.3%), myopia increases with age (86.7%), performing periodic eye examinations (92.8%), and one first, one foot, and one inch (84.8%) significantly increased after health education (P<0.001 for all). However, the percentage of students who still did not think it necessary to take breaks after 30-40min of continuous near work was 27.0%. The opinion that "myopia can be cured" was still present in 38.3%. Conclusion: Implementing school-based myopia prevention health education improves knowledge, attitudes, and skills regarding myopia among Chinese middle school students.

Abstract:Importance: Because of the high prevalence of myopia in Taiwan, understanding the risk factors for its development and progression is important to public health. Background: This study investigated the risk factors for myopia and their influence on the progression of myopia in schoolchildren in Taiwan. Design: Patients’ clinical records were obtained retrospectively from ophthalmologists. Questionnaires were given to collect demographic information, family background, hours spent on daily activities, myopia progression, and treatment methods. Participants: From a regional medical hospital in northern Taiwan, 522 schoolchildren with myopia participated in the study. Written informed consent was obtained from participants of legal age or the parents or legal guardians of younger children. Methods: Multivariable regression analyses were performed. Myopia measured in cycloplegic spherical equivalent (SE) was analysed, controlling for patients’ family and demographic information as well as their daily activity behaviours. Main Outcome Results: Children with high myopic parents were more myopic. Earlier onset age of myopia was associated with a higher level of myopia and greater annual myopic progression. Children reporting longer time usage of electronic devices had greater progression of myopia. Boys tended to be more myopic than girls. Lower levels of myopia were associated with more outdoor activities, and better vision care knowledge in children and parents. Conclusions and Relevance: In addition to genetics, education and environment can influence the development of myopia. Health policies for schoolchildren should promote protective activities and vision care knowledge at a young age, to protect the eyesight of schoolchildren.

The impact of parental migration on left-behind children’s vision health in rural China

Abstract:Abstract Background Parental migration is an important factor affecting left-behind children’s health. However, few studies have addressed the effect of parental migration on children’s vision health in China. To fill the gap, this study aimed to assess the impact of parental migration on left-behind children’s vision health and to explore the possible mechanisms of the effect. Methods Data were obtained from the baseline survey of the China Education Panel Survey (CEPS), which included over 10,000 junior high school students. This study used myopia, the most common vision problem among junior high school students, and tried to analyze whether myopia was corrected with eyeglasses as indicator variables of vision health. The impact of parental migration on vision health was assessed using an instrumental variables approach. Results The results show that parental migration reduced the likelihood of myopia in left-behind children and decreased the possibility of myopic left-behind children being corrected. This result passed a series of robustness tests. The mechanism analysis indicated that compared to non-left-behind children, left-behind children spent more time on outdoor activities and less time on after-school classes, reducing their risk of being myopic. Further, because left-behind children live apart from their parents, their myopia problem is more difficult for parents to notice, and left-behind children are less likely to inform their parents of their myopia than non-left-behind children actively. This helps to explain why left-behind children have a lower correction rate with eyeglasses. Conclusions Our findings suggest that parental migration, while not increasing the prevalence of myopia in left-behind children, has led to inequity in myopic left-behind children’s correction. Given the severe consequences of uncorrected myopia, action is required to enhance the correction rate of myopic left-behind children.

Abstract:Over the last two decades, the prevalence of myopia has gradually increased in China. Numerous epidemiological studies suggest that education and inadequate time spent outdoors are the major causes of the current myopia epidemic. China is one of the few countries that has begun to address the myopia epidemic with a national-level strategy, implementing nationwide education reform, cost-reduction measurements, and dissemination of information on myopia prevention and control. These "natural experiments" will provide insights into areas that may face similar or potential myopia problems.

Abstract:What is already known about this topic?: Myopia has been identified as a significant emerging challenge and policy priority among children and adolescents in China by the Ministry of Education and seven other departments. Limited research has been conducted to investigate the collective impact of outdoor time and other modifiable factors on the incidence of myopia.What is added by this report?: This study provides support for the protective effect of combining increased outdoor time with other prevention strategies in reducing the incidence of myopia. The results indicate the presence of a dose-response relationship.What are the implications for public health practice?: To effectively prevent myopia, it is important to implement comprehensive interventions that encompass various aspects such as outdoor time, eye-use habits, eye-use environments, and lifestyle modifications.

Abstract:Objectives To characterise the prevalence of myopia and eye diseases among school adolescents and children in Southwest China, and to evaluate the effectiveness of myopia control tools. Design Retrospective cohort study. Setting Across 95 basic education institutions in Southwest China. Participants 96 146 children aged 3–17 years from a school-based survey conducted between 2019 and 2021. Primary outcome measures The data of vision assessment and eye disease examination of school students were analysed, including a total of four surveys once per semester. The prevalence of myopia categorised as low (–0.5D to –3.0D), moderate (–3.0D to –6.0D) and high (≥–6.0D), along with the prevalence of significant ocular diseases, was assessed. Stratified analyses were conducted to investigate the impact of correction time on visual acuity (VA) and biological parameters. Subsequently, the subjects across the groups were matched using the nearest neighbour method, followed by multidimensional statistical analysis. Results The prevalence of myopia among the surveyed students was 38.39%. After controlling for confounding variables, the statistical analysis revealed a 0.1 increase in mean VA within the orthokeratology group and a 0.1 decrease in VA within the spectacle group (p<0.001), with statistically significant differences in corneal radius, corneal curvature and equivalent spherical lens (p 1), and that female students exhibited a higher risk than male students (OR=1.5). Conclusions The current high prevalence of myopia and eye diseases among Southwest China's school youths demands public health attention. Minors wearing orthokeratology lenses at night, especially in primary school, exhibit significantly improved naked-eye vision. However, vigilant eye healthcare during the correction period is crucial, especially for girls.

Abstract:The prevalence of myopia has markedly increased in East and Southeast Asia, and pathologic consequences of myopia, including myopic maculopathy and high myopia-associated optic neuropathy, are now some of the most common causes of irreversible blindness. Hence, strategies are warranted to reduce the prevalence of myopia and the progression to high myopia because this is the main modifiable risk factor for pathologic myopia. On the basis of published population-based and interventional studies, an important strategy to reduce the development of myopia is encouraging schoolchildren to spend more time outdoors. As compared with other measures, spending more time outdoors is the safest strategy and aligns with other existing health initiatives, such as obesity prevention, by promoting a healthier lifestyle for children and adolescents. Useful clinical measures to reduce or slow the progression of myopia include the daily application of low-dose atropine eye drops, in concentrations ranging between 0.01% and 0.05%, despite the side effects of a slightly reduced amplitude of accommodation, slight mydriasis, and risk of an allergic reaction; multifocal spectacle design; contact lenses that have power profiles that produce peripheral myopic defocus; and orthokeratology using corneal gas-permeable contact lenses that are designed to flatten the central cornea, leading to midperipheral steeping and peripheral myopic defocus, during overnight wear to eliminate daytime myopia. The risk-to-benefit ratio needs to be weighed up for the individual on the basis of their age, health, and lifestyle. The measures listed above are not mutually exclusive and are beginning to be examined in combination.
