Abstract:Securing an airway enables the oxygenation and ventilation of the lungs and is a potentially life-saving medical procedure. Adverse and critical events are common during airway management, particularly in neonates and infants. The multifactorial reasons for this include patient-dependent, user-dependent and also external factors. The recently published joint ESAIC/BJA international guidelines on airway management in neonates and infants are summarized with a focus on the clinical application. The original publication of the guidelines focussed on naming formal recommendations based on systematically documented evidence, whereas this summary focusses particularly on the practicability of their implementation.
Safe emergency neonatal airway management: current challenges and potential approaches

Abstract:This review examines the airway adjuncts currently used to acutely manage the neonatal airway. It describes the challenges encountered with facemask ventilation and intubation. Evidence is presented on how to optimise intubation safety and success rates with the use of videolaryngoscopy and attention to the intubation environment. The supraglottic airway (laryngeal mask airway) is emerging as a promising neonatal airway adjunct. It can be used effectively with little training to provide a viable alternative to facemask ventilation and intubation in neonatal resuscitation and be used as an alternative conduit for the administration of surfactant.

Abstract:Practitioners can face significant challenges when managing the airways of infants and neonates because of their unique anatomical and physiological features. The requirement for emergency airway management in this age group is rare. Details of emergency airway techniques in paediatric practice guidelines are missing or lack consensus, and it is known that outcomes for affected children can be poor. Ideally, these children should be managed by experienced paediatric airway practitioners working in a team. However, situations can arise where practitioners, unfamiliar and inexperienced with infants, find themselves in charge. So, what happens when such a practitioner encounters this life-or-death scenario and feels ill-equipped to act? The ethical and legal issues surrounding the management of this emergency are clearly defined, but they can be unknown or misunderstood by doctors. Compounding the extreme stress of the scenario is the moral and ethical dilemma of whether to act or not. The following discussion explores these issues and examines the philosophical and psychological perspectives.

Abstract:INTRODUCTION: In physiological conditions, neonatal airways are well-protected against aspiration of fluid or particulate material into the lungs, with laryngeal chemoreflex (LCR) being the most powerful mechanism. Failure of this protection allows substances to enter the lower airways, which starts a series of pathophysiological events initiated by inflammation and surfactant inactivation. The condition is defined as neonatal acute respiratory distress syndrome (ARDS), and its symptoms can range from mild respiratory distress to respiratory failure, often accompanied by persistent pulmonary hypertension (PPHN), in turn even leading to death. The management, therefore, may be very challenging. Areas covered: This review covers protection mechanisms of the neonatal lower airways, the etiology, and pathophysiology of neonatal aspiration syndrome (NAS), its definition in view of current literature, possible treatment options, and future trends. Expert commentary: Inflammation and secondary surfactant deficiency stand in the foreground of neonatal aspiration. Management focuses mainly on appropriate oxygenation, ventilation, improvement in PPHN, and maintenance of systemic circulation, which is largely symptomatic and supportive. Future research is required to evaluate the justification of using exogenous surfactants, antibiotics, anti-inflammatory and antioxidative drugs, or their combinations.

Neonatal airway management training using simulation-based educational methods and technology

Abstract:Airway management is a fundamental component of neonatal critical care and requires a high level of skill. Neonatal endotracheal intubation (ETI), bag-mask ventilation, and supraglottic airway management are complex technical skills to acquire and continually maintain. Simulation training has emerged as a leading educational modality to accelerate the acquisition of airway management skills and train interprofessional teams. However, current simulation-based training does not always replicate neonatal airway management needed for patient care with a high level of fidelity. Educators still rely on clinical training on live patients. In this article, we will a) review the importance of simulation-based neonatal airway training for learners and clinicians, b) evaluate the available training modalities, instructional design, and challenges for airway procedural skill acquisition, especially neonatal ETI, and c) describe the human factors affecting the transfer of airway training skills into the clinical environment.
Anesthetic Management of Thoracoscopic Procedures in Neonates: a Retrospective Analysis of 45 Cases.

Abstract:BACKGROUND:Advances in medical techniques and equipment have enabled the thoracoscopic repair of certain congenital abnormalities in neonates including congenital esophageal atresia/tracheoesophageal fistula (EA/TEF) and congenital diaphragmatic hernia (CDH). A retrospective analysis was conducted to examine the anesthetic management of neonates (7 days or younger) undergoing thoracoscopic surgery in our hospital department, and to determine the efficacy of anesthetic management in neonates.METHODS:Clinical data from 45 neonates who underwent thoracoscopic surgery in our hospital from December 2015 to March 2020 were retrospectively analyzed. A total of 25 patients underwent repair of CDH and 20 underwent repair of an EA/TEF.RESULTS:All patients received general anesthesia with endotracheal intubation, standard ASA monitoring, and arterial blood gas (ABG) analysis. All patients survived the surgery. A total of 14 patients experienced decreases in SpO2, pH, PaO2, and increases in PETCO2 and PaCO2 30 minutes after CO2 insufflation. Our anesthetic management protocols are outline and analyzed.CONCLUSIONS:Thorough preoperative preparation is critical for a desirable outcome in neonates undergoing a thoracoscopic repair of CDH or EA/TEF. In our cohort, intraoperative ventilation strategies included pressure control ventilation with peak airway pressure maintained at 15-25 cmH2O, a respiratory rate of 35-55 breaths/minute, a fraction of inspired oxygen (FiO2) of 60-80%, an inspiratory/expiratory ratio (I:E) of 1:1-1.5, and careful airway suctioning to clear secretions. Postoperatively, maintaining normovolemia and hemodynamic stability are critical for successful weaning of ventilatory support and extubation.
Intermittent apnoea and manual jet ventilation: A successful anesthetic management for infant with acquired Myer-Cotton class III subglottic stenosis undergoing endoscopic balloon dilatation

Abstract:Acquired subglottic stenosis is a common complication of endotracheal intubation in infants. The risk increases in trisomy 21, patients undergoing bypass surgery, and having gastroesophageal reflux disease. Less invasive endoscopic balloon dilatation of subglottic stenosis has become a more common treatment modality compared to open surgical technique. Airway-related surgery needs meticulous preparation and good communication between the anesthetist, surgeon, and staff. More precaution and more effective preparation and communication are needed in neonatal airway surgery as it is physiologically easier to desaturate and develop hypoxemia compared to adults. We report a case of successful balloon dilation of Myer-Cotton class III subglottic stenosis with intermittent supraglottic jet ventilation and bag-mask ventilation in infants with trisomy 21.
Airway Management of a Patient With a Giant Frontoethmoidal Encephalocele: A Pediatric Case Report

Abstract:Meningoencephalocele is a rare congenital midline defect of cranial bone fusion characterized by herniation of the brain and meninges through the skull. In addition to the challenges of managing a major neurosurgical procedure in a pediatric patient, airway management in this group of patients requires advanced skills, and a difficult airway should be anticipated from the start. Since awake intubation is not an option in most pediatric cases with airway anatomy abnormalities and maintaining an adequate seal with a pediatric face mask is often impossible, airway management in patients with these lesions is highly challenging. We present the case of a 12-month-old girl with a postnatal diagnosis of frontoethmoidal meningoencephalocele who underwent craniotomy, followed by encephalocele resection, subsequent frontal cranioplasty, and reconstruction of the nasal bone defect. We discuss the timely adaptation of an adult face mask (size five) rotated 180º over the patient's entire face to perform adequate preoxygenation and spontaneous ventilation assistance with hand-bag ventilation after the inhalational induction of general anesthesia. After obtaining adequate depth of anesthesia, an initial video laryngoscopy with pediatric Medan® was performed. The epiglottis and vocal cords were identified, and rocuronium was administered. After complete muscle relaxation, another video laryngoscopy was performed and orotracheal intubation was successful on the first attempt. As an alternative airway, we planned orotracheal intubation using a pediatric fiberoptic bronchoscope with the aid of a laryngeal mask airway if required. As a rescue measure, we also ensured that an otolaryngologist was present in the operating room if a tracheostomy was deemed necessary. We aim to raise awareness of the importance of safe practices in anesthesia, reinforce preventive measures during careful airway examination, and plan approach strategies.

Abstract:Airway endoscopy (rigid and flexible bronchoscopy) is an important procedure that allows for visualization of the trachea and bronchi as well as treatment of a variety of airway disorders for diagnostic and therapeutic interventions. Excellent communication between the anesthesiologist and the endoscopist is required to ensure that adequate oxygenation and ventilation is maintained via the shared airway. Various anesthetic and airway management techniques can be used for airway management in pediatric foreign-body aspiration. This article highlights indications, techniques, and complications encountered during pediatric bronchoscopy, with particular focus on anesthetic management of the pediatric airway.

Abstract:Bronchoscopy-guided diagnostic and interventional airway procedures are gaining in popularity and prominence in pediatric surgery. Many of these procedures have been used successfully in the adult population but have not been used in children due to a lack of appropriately sized instruments. Recent technological advances have led to the creation of instruments to enable many more diagnostic and therapeutic procedures to be done under bronchoscopic guidance. These procedures vary significantly in their length and invasiveness and require vastly different anesthetic plans that must be easily adapted to situational and procedural changes. In addition to close communication between the anesthesiology and procedural teams; an understanding of the type of procedure, anesthetic requirements, and potential patient risks is paramount to a successful anesthetic. This review will focus on new rigid bronchoscopic procedures, goals for their respective anesthetic management, and unique tips and trick for how to maintain adequate oxygenation and ventilation in each scenario.

Airway management in paediatric anaesthesia in Europe-insights from APRICOT (Anaesthesia Practice In Children Observational Trial): a prospective multicentre observational study in 261 hospitals in Europe

Abstract:Background: Critical respiratory events are common in children in the peri-anaesthetic period and are caused by airway and ventilation management difficulties. We aimed to analyse current European paediatric airway management practices and identify the incidence and potential consequences of difficult airway management. Methods: We performed a secondary analysis of airway and ventilation management details of the European multicentre observational trial (Anaesthesia PRactice in Children Observational Trial, APRICOT) of children from birth to 15 yr of age. The primary endpoint was the incidence of difficult airway management. Secondary endpoints were the associations between difficult airway management, known pre-existing respiratory risk factors, and the occurrence of critical respiratory events. Results: Details for 31 024 anaesthetic procedures were available for analysis. Three or more tracheal intubation attempts were necessary in 120 children (0.9%) and in 40 children (0.4%) for supraglottic airways insertions. The incidence (95% confidence interval) for failed tracheal intubation and failed supraglottic airway insertions was 8/10 000 (0.08%; 0.03-0.13%) and 8.2/10 000 (0.08%; 0.03-0.14%) children, respectively. Difficulties in securing the airway increased the risk for a critical respiratory event for tracheal tube (2.1; 1.3-3.4) and supraglottic airway (4.3; 1.9-9.9) placement. History of pre-existing respiratory risk factors was significantly associated with critical respiratory events independently of the airway device used. Conclusions: Airway management practices vary widely across Europe. Multiple airway device insertion attempts and pre-existing respiratory risk factors increase the likelihood of critical respiratory events in children and require further stratification during preoperative assessment and planning. This study highlights areas where education, research, and training may improve perioperative care. Clinical trial registration: NCT01878760.

Abstract:Dramatic advances in the optimization of neonatal respiratory function have been crucial in improving preterm infant survival. Commonly used adjuvant therapies aim to decrease long-term morbidities and mortality including systemic corticosteroids, caffeine, inhaled steroids, diuretics, surfactant, inhaled bronchodilators and improvemnt in ventilators. This series of papers highlights recent advances in neonatal pulmonology focusing on the novel use of established therapies such as mechanical ventilation and high frequency oscillatory ventilation as well as newer frontiers in the management of pulmonary hypertension using cell-based therapies. The ever-receding threshold of viability has contributed to a change in pulmonary pathophysiology requiring alteration in ventilatory management and will continue to challenge families and clinicians internationally. At the time of birth the newborn lungs are fluid-filled and this liquid needs to be cleared from the lungs to ensure adequate gas exchange after clamping the umbilical cord. For most preterm infants clearing lung fluid is challenging and they often need non-invasive respiratory support to achieve this goal. However, applying adequate non-invasive support to preterm infants is technically difficult and sometimes ineffective because the larynx is closed. Spontaneous breathing may overcome some of these challenges. In this narrative review Dekker and colleagues describe interventions which focus on stimulation and maintenance of spontaneous breathing of preterm infants in the first few hours after birth. 1

Paediatric airway foreign body - The human factors influencing patient safety in our hospitals

Abstract:Foreign bodies in the pediatric airway are an uncommon emergency with a high morbidity and mortality rate. Morbidity ranges from 10 to 20% worldwide and this pathology accounts for up to 7% of accidental deaths in children under 4. Dealing with this emergency safely and effectively is complex, requiring a tight coupling of procedures and processes and optimal anesthetic and operating conditions to prevent errors. These factors are recognized by the World Health Organization as 'Human Factors'. We perform a multi-center assessment of human factors pertinent to this emergency. Specifically, equipment provision and staff training in this emergency. Data was collected from 13 sites in the United Kingdom, using two questionnaires for medical and nursing staff. Information including equipment availability, location of equipment, and surgeon and nursing experience was recorded. Royal Manchester Children's Hospital (RMCH) set the study standard. Our study shows there is huge variability in equipment provision across units. There is a lack of experience, confidence and training amongst middle grade otolaryngology surgeons and emergency theatre staff in handling this emergency. Issues with equipment and inexperience of both middle grade doctors and nursing staff could result in significant patient morbidity and mortality. We suggest a standardized age appropriate equipment list and staff training in use of this equipment. Implementation of these simple changes could reduce preventable error in this rare but serious emergency.
Anesthetic Management of a Patient With Subglottic Stenosis: The Crucial Role of Communication Between Teams

Abstract:Subglottic stenosis is characterized by the narrowing of the airway at the inferior edge of the cricoid cartilage level. It is either congenital or acquired, the latter being more commonly secondary to internal iatrogenic trauma. Airway management of these cases is challenging and requires multidisciplinary discussion. We present a case of a 17-year-old boy scheduled for tracheostomy in the context of subglottic stenosis probably caused by prolonged endotracheal intubation. On the day of surgery, it was decided to perform an asleep fiberoptic visualization of the lesion through a supraglottic device, which revealed a narrow circumferential fibrous membrane just below the vocal cords. Given the findings, a suspension laryngoscopy accompanied by supraglottic manual jet ventilation was performed. Balloon dilatation with the application of mitomycin C was the elected otorhinolaryngologic technique. At the end of the procedure, a fiberoptic exam was performed and only a minimal portion of the membrane remained. The patient was asymptomatic on follow-up visits. We aim to raise awareness of how the anesthetic management of patients with subglottic stenosis may prove challenging. Communication between anesthetic and surgical teams is essential for the achievement of the main goal, which is the acquisition of an adequate airway that allows normal patient activity associated with minimal postoperative morbidity.

Abstract:Failed intubation and ventilation are important causes of anesthetic-related maternal mortality. The purpose of this article is to review the complex issues in managing the difficult airway in obstetric patients. The importance of prompt and competent decision making in managing difficult airways, as well as a need for appropriate equipment is emphasized. Four case reports reinforce the importance of a systematic approach to management. The overall preference for regional rather than general anesthesia is strongly encouraged. The review also emphasizes the need for professional and experienced team cooperation between the obstetrician and the anesthesiologist for the successful management of these challenging cases.LEARNING OBJECTIVES: After completion of this article, the reader will be able to break down the complex issues in managing the difficult airway in the obstetric patient, outline the reasons for difficult intubations in pregnancy, and describe the evaluation used to predict a difficult intubation.

Abstract:Background and aims: Respiratory diseases are the most common cause of hospitalization and mortality of neonates in NICUs. Necessity of advanced and standard care of airways, especially when patient is under mechanical ventilation or respiratory support is obvious of every one. The usual intervention in NICUs is tracheal intubation, which lack of perfect care, can cause many side effects. One of the basic ways in reducing mortality rate in NICU is improvement of nursing care based on studies of Evidence Based Practice. Methods: This study has been extracted from scientific database such as Pub med, Science Direct, Trip Database and Cochrane Database which was confirmed with cooperation by physicians and experienced nurses of NICUs. Clinical guideline of neonatal airways as a practical guideline of care giving for specific respiratory care as below:1. Nursing care of Intubated neonates during Intubation and fixation of tracheal tube.2. Protocol of neonatal Indotracheal suctioning.3. Protocol of contrivance of Nasal-CPAP in neonates and required nursing care. Conclusion: By considering the spread and different level of health centers in the country and lack of standard clinical guide line, in many circumstances, respiratory care is with some deficiencies and sometimes with complications for neonates. Promoting of such clinical guide lines in NICUs will benefit a scientific pattern in order to a standard and safe nursing care. This guide line includes 3 sections: 1. Protocol of tracheal tube fixation: tracheal tube must be fixed by leucoplast strip and tracheal tube should be indirect position for perfect ventilation. 2. Protocol of Indotracheal suctioning: Indotracheal suctioning, if necessary must be done by assessment of neonates requirement. Suctioning indications: decrease of oxygen saturation, bradycardia, takycardia, decrease of chest movements, visible secretions in tracheal tube, agitation, decrease of respiratory sounds, increase of airway pressure and.... In order to prevent any possible damages, the length of suction catheter shouldn't exceed 1CM of tracheal tube. There are two methods of suctioning.1) Open method 2) Closed method. 3. Protocol of NCPAP care: Mask and nasal prong must be controlled regularly to prevent damage and necrosis. Nasal prong should be fixed 1-2MM from nasal septum and suitable size of hat and nasal mask and prong is required. Use of deoderm adhesive for protection of skin and nasal septum is recommended. Calming of neonate is necessary in prevention of CPAP pressure variation.

Abstract:Purpose of review: Airway management is one of the most challenging aspects in the field of anesthesia. This is also the case when an emergency airway situation arises in the postoperative patient. These airway procedures are often classified as advanced with high complexity. This article aims to address emergency airway management in the post anesthesia care unit. Recent findings: The way in which care and supervision are organized in the post anesthesia care unit (PACU) varies between hospitals. Airway management in the PACU has typical challenges related to the team, the location itself, and assembling the necessary equipment for airway rescue. Crew resource management is an important aspect of care in these situations. Summary: This article focuses on emergency airway management in the post anesthesia-care unit. A questionnaire for risk assessment and improvement of quality of care is presented. Different types of emergencies are discussed, i.e., due to medication, medical conditions (e.g., obstructive sleep apnea, pulmonary problems, stridor), procedural related emergencies (e.g., neurosurgery, head and neck surgery) and cardiac arrest. Each specific cause of emergency needs a different approach. A PACU airway rescue flowchart is presented.
Respiratory issues in the management of children with neuromuscular disease

Abstract:Most children with neuromuscular disease eventually require assistance with airway clearance and with breathing, especially during sleep. Techniques and devices for airway clearance and noninvasive ventilation that are commonly used in adults have been successfully adapted for use in infants and young children. Both physiological differences and small size of young patients with neuromuscular disease, however, can limit the applicability of such interventions or require special consideration. Measurements to identify the appropriate time to begin airway clearance assistance are lacking for young children, and the role of early introduction of noninvasive ventilation to preserve or enhance lung growth and chest-wall mobility remains to be elucidated. The paucity of nasal interfaces and headgear commercially made for small patients can reduce patient tolerance of noninvasive ventilation and exacerbate patient-ventilator dyssynchrony. Despite these issues, a greater number of children with neuromuscular diseases are living well past their second decade. Strategies to transition these patients to appropriate adult-care providers, to secure cost-effective health care for them, and to help integrate them into adult society must be developed.
Stridor and severe subglottic stenosis: emergency tracheostomy for a neonate with a pinhole trachea

Abstract:Congenital subglottic stenosis is a rare but potentially catastrophic condition. In this report, we describe the management of a term neonate who was noted to have biphasic stridor during preassessment for correction of an imperforate anus at 26 hours of life. The neonate was found to have a pinhole trachea secondary to congenital subglottic stenosis. It was impossible to pass an endotracheal tube, so the neonate underwent an emergency surgical tracheostomy with a good outcome. A high index of suspicion led to appropriate steps being taken to safely anaesthetise the neonate.
