Abstract:Introduction and importance: Meckel's diverticulum is a rare congenital intestinal anomaly that can sometimes cause serious complications. The authors' aim is to review the literature on this condition by reporting the clinical case of a young adult with Meckel's diverticulum complicated by acute intestinal obstruction. Case presentation: This was a 24-year-old young man, operated on for open bladder stones received for occlusive syndrome. Abdominal computed tomography (CT) suggested a flange occlusion. Surgical exploration found a Meckel's diverticulum creating a flange around the last one. An intestinal resection was performed with direct anastomosis with simple consequences. Clinical discussion: Meckel's diverticulum is a rare congenital intestinal anomaly. It is discovered incidentally or in the face of serious complications such as intestinal obstruction. Intestinal resection with one-stage anastomosis emerges as a standard and safe management approach. Conclusion: A Meckel's diverticulum can be complicated by acute intestinal obstruction mimicking a postoperative flange that can err the diagnosis.

Abstract:Objective To evaluate the clinical characteristics and pathological features of Meckel's diverticulum(MD) in children.Methods 244 MD cases admitted between January 2010 and December 2014 were retropectively analyzed.Results In fifty patients,MD was an incidental finding at laparotomy or laparoscopy for unrelated entities.Among the remaining 194 symptomatic patients,there were 76 patients presenting GI bleeding,forty eight patients were identified with perforated Meckel's diverticulum,thirty six patients suffered from intestinal obstruction.34 patients had MD caused severe complications such as volvulus and intestinal necrosis,diverticular perforation and peritonitis.61 out of 76 GI bleeding patients underwent a 99mTc scan,and positive tracer was found in 42 patients.Among the 19 negative 99mTc scan patients,8 received capsule endoscopy and only 3 patients were suspected of diverticulum.242 patients underwent one stage resection of the diverticulum.Histology revealed ectopic gastric mucosa or ectopic pancreatic tissue in 128 patients.One patient died of volvulus and intestinal necrosis postoperatively,and two suffered from adhesive intestinal obstruction during one to five year's follow up.Conclusions It is necessary to maintain a high suspicion of MD in the pediatric age group with symptoms of abdominal pain,gastrointestinal hemorrhage or intestinal obstruction.Ectopic mucosa assumes the ultimate responsibility for major complications of MD.

Abstract:Purpose/background: Meckel's diverticulum (MD) is one of the most common congenital malformations of gastrointestinal tract in children. However, the nonspecific clinical manifestations of MD often cause a diagnostic as well as therapeutic challenge to pediatric surgeon. This study aimed to review our experience in managing this disease while evaluating the management strategies. Methods: We retrospectively analyzed the clinical data of all patients diagnosed with MD admitted to our center between January 2010 and December 2015. Factors documented including demographic criteria, clinical manifestations, preoperative examinations, surgical methods, histopathological characteristics, postoperative complications, and outcomes. Results: The patients included 210 males and 76 females, aged from 1 day to 15 years. In fifty three patients, the MD was an incidental finding at laparotomy or laparoscopy. The remaining 233 patients were symptomatic and presented with various clinical features. Ninety nine patients presented with episodes of bleeding per rectumor melena. Fifty six patients demonstrated symptoms of diverticulitis or perforated MD. Forty patients were diagnosed as intestinal obstruction, and 35 patients with intussusception requiring surgical reduction. Two cases of Littre hernia and one case of foreign body trapped in MD were also observed in this group. Six patients misdiagnosed as appendicitis at another institution were reoperated in our department. Among the 99 patients with bleeding per rectum, 78 underwent a Tc-99mscan that showed a positive tracer in 55 patients and negative in 23. All patients underwent resection of the diverticulum, except for 2 cases of postponed resection. Histology revealed ectopic gastric mucosa or ectopic pancreatic tissue in 154 patients; significant differences were observed between the symptomatic group and the accidentally found group. One patient died of peritonitis and sepsis postoperatively; one case of anastomotic leak and one case of adhesive intestinal obstruction were reoperated. Conclusion: Meckel's diverticulumhas various clinical presentations and it is difficult to make a precise diagnosis preoperatively. It is necessary to maintain a high suspicion of MD in the pediatric age group with symptoms of abdominal pain, gastrointestinal hemorrhage or intestinal obstruction. Heterotopic tissue is the main cause of complicated diverticulum, and it is safe and feasible to remove the incidentally found MD. Abstract:Meckel diverticulum (MD), a congenital gastrointestinal anomaly, is often involved in pediatrics, but less in the adult population. The patient in this report was a 69-year-old female presented with massive gastrointestinal bleeding causing hemorrhagic shock due to MD containing ectopic pancreatic tissue. A review of the literature revealed that gastrointestinal bleeding from MD containing ectopic pancreatic tissue is rare in adults and difficult to be identified preoperation. MD should be considered as one of the differential diagnosis for lower gastrointestinal bleeding, although scarce in adults, especially when the patient has massive painless bleeding.

Abstract:Meckel's diverticulum (MD), a prevalent congenital gastrointestinal anomaly affecting about 2% of the population, arises from the incomplete closure of the vitelline duct. It encompasses all layers of the small intestine and can lead to various complications like obstruction, hemorrhage, and perforation. When symptomatic, it presents challenges in diagnosis due to the low sensitivity of imaging techniques. Comprehensive understanding and accurate diagnosis are crucial for managing the complications associated with MD and forming the scientific rationale for publishing this case report. We present two cases, one of them being the case of a 73-year-old male who presented for an ileostomy closure procedure. Intra-operatively, a 4.5 cm diverticulum was identified 10 cm from the stomatal opening on the efferent limb. This finding led to segmental resection of the intestines. Later, pathology was compatible with MD, which didn't contain any malignant cells or heterotopic tissue. The second case was that of a 40-year-old female who presented for severe abdominal pain, abdominal distention, and obstipation for two days. Radiographic imaging was suspicious of a foreign object compatible with fish bone with local inflammation in the small bowel. Laparoscopic exploration showed an inflamed MD with fish bone lodged inside. In front of an incidental MD, the decision to resect is still controversial. Those who are against resection of uncomplicated believe that complications from resecting an uncomplicated MD are higher than the complications that arise if resection is not performed. Those who support resection say that the complications that arise following the resection of a complicated MD are worse than those after resecting an incidental one. Criteria have been put in place to help guide the decision for resection.
Acute abdomen due to Meckel's diverticulitis with synchronous inflammatory myofibroblastic tumor in the terminal ileum: A case report

Abstract:Meckel's diverticulum (MD) is the most common congenital anomaly of the gastrointestinal system, occurring in approximately 2% of the population. It is rare for MD to be symptomatic or complicated in adulthood. In this case report, we describe a patient who was admitted to the clinic with Meckel's diverticulitis, which had fistulized to the anterior abdominal wall, and was incidentally found to have an ileal inflammatory myofibroblastic tumor (IMT) on radiological imaging. A 46-year-old male patient presented to the emergency department with abdominal pain. Physical examination revealed localized guarding on the right side of the umbilicus. Blood tests showed elevated acute-phase reactants, including a white blood cell count of 13,800/µL, and C-reactive protein (CRP) level of 165 mg/L. Abdominal computed tomography demonstrated Meckel's diverticulitis fistulizing to the anterior abdominal wall and a polypoid structure in the ileum distal to the MD. The patient underwent emergency surgery, during which segmental ileal resection and ileocolic anastomosis were performed. On the fourth postoperative day, the patient developed an anastomotic leak. Relaparotomy, right hemicolectomy with end ileostomy, and mucous fistula creation were subsequently performed. Pathological examination of the resected ileum from the initial surgery revealed a benign IMT distal to the MD. The patient was discharged on the 40th postoperative day after developing a surgical site infection following the second surgery. The end ileostomy was closed six months later. In this case, it appears that the ileal IMT located distal to the MD may have caused intermittent intestinal obstruction, fecal stasis, and the development of Meckel's diverticulitis. Furthermore, a detailed examination of the patient's history, laboratory results, and radiologic tests may contribute to the detection of incidental pathologies and influence treatment choices.

Abstract:We report a case of small bowel obstruction (SBO) caused by internal hernia from Meckel's diverticulum (MD). Abdominal CT scan showed an abnormal dilated blind-ending structure in continuity with the distal ileum in the right lower quadrant, suggesting Meckel's diverticulum. MPR images revealed a "double beak-sign" at the point of MD and a collapsed closed loop with mesenteric vessels converging to the diverticulum. Since the patient has no prior history of abdominal surgery, the diagnosis of internal hernia caused by Meckel's diverticulum was considered. On laparoscopic exploration, an abnormal orifice for internal hernia created by adhesion from the tip of Meckel's diverticulum to the adjacent mesentery was revealed, confirming the diagnosis. The patient was discharged after 7 days without postoperative complications. MD-associated internal hernia is a rare cause of small bowel obstruction and should be considered to avoid delay in treatment. Multidetector Computed Tomography (MDCT) is the first-line imaging modality of choice and may offer some suggestive imaging features to make an accurate preoperative diagnosis.

Abstract:While bowel obstruction is the most common surgical disorder of the small intestine, small bowel obstruction due to Meckel's diverticulum is a relatively rare occurrence. We encountered a compelling case of small bowel obstruction that turned out to be more complex than anticipated, involving a Meckel's diverticulum with some unforeseen findings. We followed standard guidelines for history-taking, examination, investigations, and management of the intestinal obstruction. After exhausting conservative treatment options, we opted for surgical intervention, and the unexpected cause of the obstruction took us by surprise. This case report highlights an exceedingly rare entity: Meckel's diverticulum precipitating uncommon complications.

Abstract:Background: Meckel's diverticulum is the most prevalent congenital anomaly of the gastrointestinal tract, resulting from the incomplete obliteration of the omphalomesenteric duct during fetal development. Although often asymptomatic, it can present with a variety of complications, including bleeding, obstruction, and inflammation, necessitating timely and accurate diagnosis and intervention. Objective: This article aims to provide a comprehensive overview of Meckel's diverticulum, including its embryological origin, pathophysiological features, clinical manifestations, diagnostic strategies, and therapeutic approaches. Methods: A thorough literature review was conducted, focusing on studies and case reports detailing the clinical presentation, imaging modalities, surgical techniques, and outcomes associated with Meckel's diverticulum. Key databases such as PubMed, Scopus, and Web of Science were searched for relevant articles published up to August 2024. Results: Meckel's diverticulum typically presents in childhood but can manifest at any age. Its clinical presentation ranges from asymptomatic incidental findings to acute abdomen scenarios due to complications like hemorrhage, intestinal obstruction, diverticulitis, and, rarely, neoplastic transformation. Diagnostic imaging, including technetium-99m pertechnetate scintigraphy (Meckel's scan), CT, and MRI, plays a crucial role in the preoperative identification of the diverticulum. The management of symptomatic Meckel's diverticulum is predominantly surgical, with laparoscopic and open approaches being widely utilized. Prophylactic resection in asymptomatic cases remains controversial, with decision-making guided by patient-specific factors such as age, diverticulum characteristics, and the presence of ectopic tissue. Conclusion: Meckel's diverticulum, though often a silent anomaly, can present with significant clinical challenges. Early recognition and appropriate management are essential for optimal patient outcomes. This review highlights the importance of a multidisciplinary approach involving pediatricians, surgeons, and radiologists in the diagnosis and treatment of this condition.

Abstract:Meckel's diverticulum is a common congenital anomaly of the gastrointestinal tract and is often asymptomatic. This case report details the diagnosis and treatment of small bowel obstruction particularly in a segment of the jejunum in a 41-year-old woman due to Meckel's diverticulum. The patient presented with diffuse abdominal pain and bloating. Imaging and clinical evaluation revealed signs of ileus, leading to an exploratory laparotomy. Intraoperative findings included a mass consistent with Meckel's diverticulum causing adhesion and torsion of the jejunum. The surgical intervention involved adhesiolysis and segmental enterectomy with side-to-side anastomosis. Histopathological analysis confirmed Meckel's diverticulum with ectopic gastric mucosa. The patient had an uneventful postoperative recovery and was discharged on the seventh postoperative day.
Meckel's Diverticulum, A Rare Presentation in a Neonate

Abstract:Meckel's diverticulum is commonly symptomatic the first 2 years of life. Complications associated with Meckel's diverticulum are due to gastrointestinal (GI) bleeding or obstruction. A 5-day-old male presented to the emergency department (ED) with an episode of bright red blood per rectum (BRBPR) associated with emesis. Vital signs were normal and abdomen soft and non-distended. Serial abdominal radiographs progressed to show distention of small bowel and air fluid levels. Operative intervention was undertaken with diagnosis of intestinal obstruction. On exploratory laparotomy, 24 cm of a fibrosed, ischemic closed-loop ileal segment densely adherent to the tip of a Meckel's diverticulum was identified and resected, followed by primary reanastamosis. Histologic findings confirmed ectopic gastric tissue. Symptomatic Meckel's diverticulum is often secondary to intestinal obstruction and hematochezia, findings which are caused by incarcerated inguinal hernia or ileocolic intussusception. Our patient presented with a closed loop, which has not been previously reported.
Perforated Meckel's diverticulum in an adult: Case report and literature review

Abstract:Introduction and importance: Meckel's diverticulum (MD) is an unusual clinical condition that occurs in approximately 2-4 %. The complications are more common in children, with a low prevalence in adults, with the main complication in adults being intestinal obstruction followed by diverticulitis with or without perforation. Case presentation: We present a 30-year-old female patient with a history of an appendectomy. She attends the emergency room due to a four-day history of pain in the lower abdominal quadrants, tachycardia, and leukocytosis, with an abdominal CT scan identifying an inflammatory process of the ileum, suggesting a Meckel's diverticulum, urachus remnant, or foreign body. The patient was taken to surgery, and inflammatory adhesions, including a perforated Meckel's diverticulum with necrotic signs and purulent production, were evidenced. Intestinal resection and ileal anastomosis were performed with no complications at 90 days. Clinical discussion: Preoperative diagnosis is difficult in cases with associated inflammatory conditions and is generally made incidentally during surgery and less frequently with CT scans. Surgical management for patients with MD is still controversial and depends on many factors, including symptoms, related complications, characteristics of the diverticulum, and the patient's clinical condition. Currently, no standardized surgical technique exists, and cases should be individualized. Conclusion: Meckel's diverticulum perforation is a rare complication in the adult population and remains a diagnostic and surgical challenge, and the final decision must be individualized.

Abstract:Meckel's diverticulum is the most common gastrointestinal tract anomaly. It arises from the incomplete closure of the omphalomesenteric conduit, which is a true diverticulum at the antimesenteric border of the ileum. Although the majority of patients are asymptomatic, they can present with inflammation, hemorrhage, intussusception, intestinal obstruction, and perforation, among others; this constitutes an important differential diagnosis for acute abdomen. A 19-year-old female sought medical attention because of intermittent diffuse abdominal pain for two months, nausea, and diarrhea. In the requested imaging tests, tomography, and enterotomography, a diagnosis of Meckel's diverticulum with some degree of intussusception was suggested. The patient underwent elective surgical treatment without complications and was discharged on the second postoperative day with clinical improvement. In this section, we review publications on similar cases published in the last five years.

Abstract:Inbred SJL/J and A.SW/SnJ mice have been bred to produce (SJL female X A.SW male)F1 and A.SW female X SJL male)F1 mice. F1 mice were then crossed to produce (SJL female X A.SW male)F2 and (A.SW female X SJL male)F2 mice. The inbred, F1, and F2 populations were followed for 2 years to determine the incidences of spontaneous reticulum and sarcoma (RCS) expression. SJL mice had a greater than 90% incidence of RCS, although F1 mice showed almost no RCS. The incidence of RCS in the F2 populations was 24.1%. These data were consistent with the hypothesis that in F1 and F2 populations a single dominant A.SW gene is able to suppress the expression of the pleomorphic RCS disease characteristic of the SJL mouse. This gene was not an H-2 gene, since the two strains were congenic with respect to the major histocompatibility complex; this gene was not on the Y-chromosome, and its phenotypic expression was not inherited in an X-linked manner.

Abstract:RATIONALE:Meckel's diverticulum (MD) is common congenital abnormality of gastrointestinal tract, only about 6.4% of patients become symptomatic. A smaller minority develop potential fatal complications such as hemorrhage, perforation, abscess, and bowel obstruction. PATIENT CONCERNS:A 15-year-old boy with history of appendicitis was admitted due to worsening abdominal pain and nausea for 1 day. The physical examination showed the abdomen was soft, with the diffuse tenderness to palpation and voluntary guarding. DIAGNOSIS:Abdominal computed tomography showed a probable MD in the distal ileum. Single photon emission computed tomography/computed tomography (SPECT/CT) fusion imaging revealed the focal concentration at the right lower quadrant of abdomen region. INTERVENTION:After the initial management including antibiotic administration and intravenous fluid resuscitation, MD with perforation and localized suppurative peritonitis was confirmed in surgery. The patient underwent a diverticulectomy. Histological examination was confirmed as MD with focal heterotopic gastric tissue. OUTCOMES:After surgery, the patient had uneventful recovery during 3 months follow-up. LESSONS:Spontaneous perforation and intraabdominal abscess due to MD is very rare. Accurate diagnosis of MD remains challenging as clinical symptoms from these complications occur nonspecifically. SPECT/CT fusion imaging is critical for prompt recognition and accurate diagnosis in the successful management of this rarely life-threating complication.

Abstract:BACKGROUND Meckel's diverticulum is a common congenital anomaly of the gastrointestinal tract, with a higher incidence rate in children under 7 years old. The condition is characteristically asymptomatic but may become a clinical concern when complications such as intestinal obstruction, bleeding, perforation, or diverticulitis precipitate acute abdominal presentations. CASE SUMMARY This report describes the case of a middle-aged man initially suspected of having acute appendicitis, which rapidly progressed to acute peritonitis with concomitant intestinal obstruction observed during preoperative assessment. Surgical exploration confirmed the diagnosis of Meckel's diverticulum-induced internal hernia, accompanied by intestinal obstruction and necrosis. In addition, the hernial ring base exhibited entrapment resembling a surgical knot. CONCLUSION Meckel's diverticulum is a rare cause of small bowel obstruction in adults, and it should be considered in a differential diagnosis.

Abstract:Ultrafine particle number and respirable particle mass concentrations were measured throughout an automotive grey iron foundry during winter, spring and summer using a particle concentration mapping procedure. Substantial temporal and spatial variability was observed in all seasons and attributed, in part, to the batch nature of operations, process emission variability and frequent work interruptions. The need for fine mapping grids was demonstrated, where elevations in particle concentrations were highly localized. Ultrafine particle concentrations were generally greatest during winter when incoming make-up air was heated with direct fire, natural gas burners. Make-up air drawn from roof level had elevated respirable mass and ultrafine number concentrations above ambient outdoor levels, suggesting inadvertent recirculation of foundry process emissions. Elevated respirable mass concentrations were highly localized on occasions (e.g. abrasive blasting and grinding), depended on the area within the facility where measurements were obtained, but were largely unaffected by season. Particle sources were further characterized by measuring their respective number and mass concentrations by particle size. Sources that contributed to ultrafine particles included process-specific sources (e.g. melting and pouring operations), and non-process sources (e.g. direct fire natural gas heating units, a liquid propane-fuelled sweeper and cigarette smoking) were additionally identified.
