Abstract:Abstract Background: Malnutrition continues to be a critical public health problem in sub-Saharan Africa. For example, in East Africa, 48% of children under- five are stunted while 36% are under weight. Poor health and poor nutrition are now more characteristics of children living in urban area than of children in rural areas. Therefore this study assessed the effect of maternal employment on nutritional status of under-five children at Mendera Kochi kebele of Jimma town South West of Ethiopia.Method: Community based cross sectional study design was conducted in Jimma town Mendera Kochi kebele from April15-May 05. Systematic sampling technique was used. Pre-tested structured questionnaires used for data collection. A total of 264 study subjects were interviewed. The collected data analyzed using SPSS 20 version.Result: Out of 264 total households the majority, 96(36.4%) of mothers were living in households with 3-4 or more family members. Out of 264 respondents 155(58.7%) were government employee, 93(35.3%) and the remaining were private and NGO employees. The income level of majority of respondents, 54.5% was between 1000 and1500. A total of 254 children aged 0–59 months were enrolled in this study. The age of the children in the study area, 6-12 months 89(35%), 12-24 months 111(43.6%), and 24-59 months 54(21.4%) respectively. Weight of children’s 53(21.1%) were between 1500-2500 gm, 40(16.1%) were between 2500-4000gm, 25(9.8%) were >4000gm and the rest were below 1500gm. 151(57.2%) children of working mothers had a mean upper arm circumference value of >=12.5. 51.1% of the children had height for age ranged from 90-95% percentiles. Conclusion- The children of working mothers were observed to achieve better nutritional status from their anthropometric measurement. Maternal employment is not a protective factor against diarrhea and acute respiratory infection.

Abstract:Even though maternal employment can increase family income, several studies suggest that it could have adverse consequences on children’s health. In this study, we use a nationally representative sample of 12,888 children, aged 0–5 years from Egypt to examine the impact of maternal employment on child nutritional indicators, namely: stunting, wasting, and being underweight and overweight. We adopted various estimation methods to control for observable and unobservable household characteristics in order to identify the causal effect of maternal employment. These different techniques include, propensity score matching (PSM), OLS regression with controlling for a wide range of individual characteristics, and an instrumental variable two-stage least squares (IV 2SLS) approach. Results of the PSM and OLS suggest that maternal employment is weakly associated with having a malnourished child. On the other hand, the IV 2SLS suggests a stronger and significant association between maternal employment and poor nutritional status among children.

Abstract:About 40%, 15%, and 38% of children in Bangladesh are stunted, wasted, and underweight, respectively. This paper explores the causal relationship between maternal employment and under-five children malnutrition in Bangladesh by using the instrumental variable approach utilizing the augmented cross-sectional samples of 25,667 and 1665 ever-married women at the national level and in metropolitan cities, respectively. This study finds a higher likelihood of stunting children of employed mothers at the national level, at 11.7%. The probabilities of stunting, wasting, and underweight children of employed mothers are also very high in metropolitan cities at 15.8%, 2.9%, and 11.6%, respectively. These impacts on the children of formal-working mothers are comparable to those of both formal and informal-working mothers, almost double at the national level and in metropolitan cities. The likelihood of stunting children of formal-working mothers at the national level is 20.2% and the probabilities of stunting, wasting, and underweight children of formal-working mothers in metropolitan cities are 28.3%, 5.3%, and 20.9%, respectively. This study is a good reference for the policymakers of Bangladesh; with it, they can adopt effective measures to curb early childhood malnutrition.

Abstract:Abstract Background Baby-friendly workplace is an effective evidence based initiative developed by the World Health Organization to protect and support maternal knowledge, beliefs, and confidence in infant and young child feeding practices. However, studies that show the effect of the baby-friendly workplace initiative on the nutritional status of infant and young children are not available in Ethiopia. Therefore, this study aimed to assess the nutritional status among baby friendly initiatives service utlizers and non utlizers children age 6–24 months in public health facilities of Southern Ethiopia. Methods We conducted a comparative cross-sectional study from 1 to 30 June 2022 among 220 mothers with children aged 6–24 months. Data were collected through face-to-face interviews using a structured questionnaire. Data were entered into Epidata Software version 4.2 and then exported to IBM SPSS version 26 software for analysis. Chi-square and Fisher exact test were used to assess the differences between users and non-users of the baby friendly workplace initiative. Logistic regression model was used to determine the association between dependent and independent variables. Adjusted odds ratio (AOR) with a 95% confidence interval was computed. P -values < 0.05 at a 95% confidence level were considered statistically significant. Result The mean (SD) scores of weight for age (WAZ), height for age (HAZ), and weight for height (WHZ) were − 0.38 (1.34),-0.17(2.62) and-0.35 (1.84) respectively. After adjusting for covariates, children aged 6–24 months who did not use baby friendly workplace initiatives were 2.26 times more likely to have stunting compared to the users of baby friendly workplace initiative (AOR 2.26, 95% CI: 1.05, 4.88). However, both wasting (AOR: 0.42; 95% CI:0.13, 1.37) and underweight (AOR: 1.09; 95% CI: 0.45, 2.60) were not significantly associated with the use of baby friendly workplace initiatives. Conclusion The use of baby friendly work place initiatives was successful in improving nutritional status, specifically chronic malnutrition in children. Strengthening and scaling up the baby friendly work place initiative program has the potential to reduce chronic malnutrition in Ethiopia and other similar settings with high burden of malnutrition areas, by implementing it in public facilities.

Abstract:Maternal malnutrition is pervasive throughout the world, notably in sub-Saharan Africa (SSA), including Ethiopia. This study aimed to assess the effect of nutrition education on the nutritional status of pregnant women in urban settings in Southeast Ethiopia. A community-based two-arm parallel cluster randomized controlled trial was conducted among 447 randomly selected pregnant women attending antenatal care (224 intervention and 223 control). We used a multistage cluster sampling technique followed by systematic sampling to select the pregnant women. Pregnant women who participated in the intervention arm received six nutrition education sessions. Women in the control group received standard care. A nonstretchable mid-upper arm circumference (MUAC) tape was used to measure the MUAC. A linear mixed effects model (LMM) was used to evaluate the effect of the intervention on MUAC, accounting for the clustering. The net mean ± standard error of MUAC between the intervention and control groups was 0.59 ± 0.05 (P < 0.0001) The multivariable LMM indicated that having received nutrition education interventions (β = 0.85, 95% CI 0.60, 1.12, P < 0.0001) improved the MUAC measurement of pregnant women. Thus, nutrition education during pregnancy will combat undernutrition among pregnant women.

Maternal and paternal employment in agriculture and early childhood development: A cross-sectional analysis of Demographic and Health Survey data

Abstract:Considerable literature from low- and lower-middle-income countries (LLMICs) links maternal employment to child nutritional status. However, less is known about the role of parental employment and occupation type in shaping child development outcomes. Additionally, little empirical work has examined the mechanisms through which parental occupation influences child outcomes. Our objective was to investigate the associations between maternal and paternal employment (comparing agricultural and non-agricultural employment) and child development and to examine childcare practices and women's empowerment as potential mechanisms. We pooled nine Demographic and Health Surveys (Benin, Burundi, Cambodia, Congo, Haiti, Rwanda, Senegal, Togo, and Uganda) with data on 8,516 children aged 36–59 months. We used generalised linear models to estimate associations between parental employment and child development, child stimulation (number of activities provided by the mother, father, and other household members), child supervision (not left alone or with older child for >1 hour), early childhood care and education programme (ECCE) attendance, and women's empowerment. In our sample, all fathers and 85% of mothers were employed. In 40% of families, both parents were employed in agriculture. After adjusting for child, parental and household confounders, we found that parental agricultural employment, relative to non-agricultural employment, was associated with poorer child development (relative risk (RR) 0.86 (95% CI 0.80, 0.92), more child stimulation provided by other household members (mean difference (MD) 0.26 (95% CI 0.09, 0.42)), less adequate child supervision (RR, 0.83 (95% 0.78, 0.80)), less ECCE attendance (RR 0.46 (95% CI 0.39, 0.54)), and lower women's empowerment (MD -1.01 (95% CI -1.18, -0.84)). Parental agricultural employment may be an important risk factor for early childhood development. More research using more comprehensive exposure and outcome measures is needed to unpack these complex relationships and to inform interventions and policies to support working parents in the agricultural sector with young children.
The Impact of Nuchal Cord on Fetal Outcome, Mode of Delivery, and Its Management: A Questionnaire Survey of All Hong Kong Obstetricians and Gynaecologists

Abstract:Objectives: To explore the view of Hong Kong obstetrics and gynaecology specialists on the impact of nuchal cord on fetal outcome, mode of delivery, and its management. Methods: A questionnaire was mailed to all registered Hong Kong specialists in obstetrics and gynaecology (n=381) in July 2012 with a prepaid return envelope. Results: The overall response rate was 50.7%. About one-third and one-fifth of specialists considered that nuchal cord could cause intrauterine death and intrapartum fetal death / neonatal death, respectively. In addition, approximately half believed that it reduced the possibility of a successful normal vaginal delivery, and increased the rate of assisted vaginal delivery. Nonetheless only 4.7% would advise patients to elect for Caesarean section in the presence of nuchal cord. There were no significant differences in the opinions of the impact of nuchal cord on fetal outcome and mode of delivery between specialists working in the Hospital Authority / public institutions versus those in private practice, between Maternal-Fetal Medicine (MFM) subspecialists versus non-MFM specialists, as well as between specialists with different years of practice after obtaining Fellowship of the Hong Kong Academy of Medicine. Around one-third in private practice routinely screened for nuchal cord on ultrasound, compared with none who practised in Hospital Authority / public institutions. Conclusion: A significant proportion of obstetrics and gynaecology specialists thought that nuchal cord would lead to adverse fetal outcome and affect the mode of delivery. Inequalities in maternal malnutrition in Ethiopia: evidence from a nationally representative data

Abstract:Background: Despite promising progress made in several maternal health indicators, maternal malnutrition (especially undernutrition) remained one of the greatest development challenges for Ethiopia. The main purpose of this study was to examine the disparities in maternal malnutrition and estimate the population level impacts of key risk factors in Ethiopia. Methods: The analysis was made based on the Ethiopian Demographic and Health Survey (EDHS 2016) data, containing 9949 non-pregnant women. Multiple logistic regression was used to examine the effects of selected explanatory variables on the two nutrition morbidity outcomes (undernutrition and obesity). Two population weighed regression-based measures (the Slope Index of Inequality/SII and Relative Index of Inequality/RII) and Population Attributable Fractions (PAFs) were employed to examine the inequalities in maternal malnutrition. Results: The prevalence of maternal undernutrition and overweight or obesity were 21% and 6%, respectively. In the multiple logistic regression, four variables appeared to have significant association with both undernutrition and overweight/ obesity, namely age of the women, residence, maternal education, and non-monetary wealth (p < 0.05). Both the absolute and relative measures of inequalities showed remarkable differences in maternal undernutrition, significantly favoring the higher socioeconomic groups (p < 0.05). Further analysis of inequalities, using the Population Attributable Fractions (PAFs), revealed that the combined population level impacts of all the significant risk factors amount 80.38%, from which 25% is attributed to the three socioeconomic variables (non-monetary wealth, maternal education and paternal education). Conclusion and policy implication: Given the high disparity in both under nutrition and overweight and/or obesity, the study emphasized the need for policy and program efforts to promote parental education in Ethiopia. Strengthening nutrition sensitive mass literacy programs is recommended along with improving women's employment and rural households' income for increased access to better diet.

Abstract:Abstract Background The aim of this study was to examine the determinants of nutritional status among children under age 5 (0–59 months) in Ethiopia. Child malnutrition is an underlying cause of almost half (45%) of child deaths, particularly in low socioeconomic communities of developing countries. In Ethiopia, the prevalence of stunting decreased from 47% in 2005 to 39% in 2016, but the prevalence of wasting changed little over the same time period (from 11 to 10%). Despite improvements in reducing the prevalence of malnutrition, the current rate of progress is not fast enough to reach the World Health Organization global target for reducing malnutrition 40% by 2025. Methods This study used data from the 2016 Ethiopia Demographic and Heath Survey (EDHS). The analysis used stunting and wasting as dependent variables, while the independent variables were characteristics of children, mothers, and households. Logistic regression was used to analyze the determinants of nutritional status among children. Bivariate analysis was also used to analyze the association between the dependent and independent variables. Results Study results show that child’s age, sex, and perceived birth weight, mother’s educational status, body mass index (BMI), and maternal stature, region, wealth quintile, type of toilet facility, and type of cooking fuel had significant associations with stunting. Child’s age, sex, and perceived birth weight, mother’s BMI, and residence and region showed significant associations with wasting. The study found that child, maternal, and household characteristics were significantly associated with stunting and wasting among children under age 5. Conclusion These findings imply that a multi-sectorial and multidimensional approach is important to address malnutrition in Ethiopia. The education sector should promote reduction of cultural and gender barriers that contribute to childhood malnutrition. The health sector should encourage positive behaviors toward childcare and infant feeding practices. More should be done to help households adopt improved types of toilet facilities and modern types of cooking fuels.

Abstract:Abstract Background Undernutrition is one of the major public health problems affecting children in developing settings. Despite impressive interventions like productive safety net program (PSNP), there is limited information on the association between stunting and PSNP implementation in Ethiopia. Methods Community-based comparative cross-sectional study design was used among systematically selected 1555 children and their mothers/caregivers from households enrolled in PSNP and not, respectively, in Meta District east Ethiopia from 5th–20th of March 2017. Data were collected using pretested structured questionnaire. Measuring board was used to measure length/height of children. Length/height for age Z -score was generated using World Health Organization (WHO) Anthro version 3.2.2. Descriptive statistics was used to describe all relevant variables. Bivariable and multivariable logistic regression analyses were used to identify predictors of stunting. Odds ratio along with 95% confidence intervals were estimated to measure the strength of association. The statistical significance was declared at p value less than 0.05. Results The prevalence of stunting was 47.7%, 95% CI (44.1%, 51.5%) and 33.5%, 95% CI (29.9%, 36.9%) among children from households enrolled in PSNP and non-PSNP ones, respectively. Lack of maternal education [AOR = 3.39; 95% CI (1.12, 5.11)], women’s empowerment [AOR = 3.48; 95% CI (2.36, 5.12)] and fourth antenatal care visit [AOR = 4.2, 95% CI (2.5, 6.8)], practicing hand washing [AOR = 0.46; 95% CI (0.28, 0.76)], living in mid-land [AOR = 1.94, 95% CI (1.12, 3.35)] and low-land[AOR = 0.27: 95% CI (0.16, 0.45)] agro-ecological zones, PSNP membership [AOR = 1.82, 95% CI (1.14, 2.89)], childhood illness [AOR = 8.41; 95% CI (4.58, 12.76)], non-exclusive breastfeeding [AOR = 3.6; 95% CI (2.30, 4.80)], inadequate minimum dietary diversity [AOR = 4.7; 95% CI (3.0, 7.40)], child’s sex [AOR = 1.73, 95% CI (1.18, 2.53)] and age (24–59 months) [AOR = 3.2; 95% CI (1.6, 6.3)] were independent predictors of stunting. Conclusions The prevalence of stunting was high among children from households enrolled in PSNP. Stunting was significantly associated with maternal- and child-related factors. Therefore, women empowerment on household’s issues and improving infant and young child feeding practices could reduce the prevalence of stunting and its adverse consequences.
Association among Household Wealth, MaternalEmployment, and Undernutrition in Children under Three Years of Age in Pakistan

Abstract:Background: There is an abundance of studies explaining the separate impact of female employment and household wealth status in reducing malnutrition. However, our study has unraveled the combined impact of maternal employment and household wealth on undernutrition among children under three in Pakistan. Methods: Using a sample of 1093 children under three years of age from the Pakistan Demographic and Health Survey 2017-2018, a binary logistic model was employed to gauge factors influencing the children's undernutrition. Results: Our results indicated that children up to a certain age (three years old) with residence in certain regions (Pakistan) and recent episodes of diarrhea had an increased risk of undernutrition. Conversely, secondary and higher maternal education, access to improved water sources, and sanitation facilities lowered the chances of undernutrition in children under three in Pakistan. The interaction between maternal employment and household wealth showed that maternal employment significantly lowered the risk of stunting, being underweight, and wasting among the average, rich, and richest households; however, it did not contribute to child nutrition among the poorer and poor households. Notably, regardless of whether the mother was employed, the wealth status of being rich and richest reduced the risk of stunting, being underweight, and wasting. Conclusions: In overcoming undernutrition, maternal employment significantly contributed to middle-income households. However, in the richer and richest households, the wealth status played a more crucial role compared to the maternal employment. This indicates that while employment plays a supportive role in household resources, the wealth status is overall more influential in reducing undernutrition.

Abstract:Approximately two billion individuals, or 26.4% of the global population, live in moderate- to severely food-insecure circumstances. It was discovered that not receiving all recommended antenatal care throughout one's pregnancy is one of the effects of household insecurity. The majority of women in Ethiopia, one of the most food-insecure countries in the world, with 10% of the population facing food poverty, devote more of their time to household duties, making food insecurity in the home the primary effect of poor prenatal care utilization. The main objective this study was to assess the status of household food insecurity among postpartum women at Arba Minch town, southern Ethiopia A community-based cross-sectional study was conducted among 381 mothers who were enrolled from December 1, 2022, to January 30, 2023. The total sample size was allocated proportionately to the number of postpartum women living in each kebeles taking from the town registry of health extension workers for immunization. Thus, systematic sampling was applied. Kobo Toolbox was used for data collection and cleaning, which was then analyzed using the statistical package of Social Science Version 26 (SPSS). In this study, the prevalence of food insecurity was 30.2% (95% CI 25.5–34.5). The associated factors with household food-insecurity were maternal occupation (AOR = 0.5, 95% CI 0.27, 0.90), late antenatal care initiation (AOR = 3.5, 95% CI 2.13, 5.91), and low monthly income (AOR = 3.1, 95% CI 1.38, 6.93). Food insecurity among postpartum mothers in the study area is high. Families who are severely food insecure require quick assistance to lower poor maternal and neonate's outcomes. Furthermore, enhancing the occupation of mothers is crucial in reducing the morbidities and mortality of food insecure mothers, such as delayed prenatal care services, anemia, low birth weight, and stillbirth.

Abstract:Maternal/caregivers’ mental health (MMH) and child nutrition are both poor in low- and middle-income countries. Links between the two are plausible but poorly researched. Our aim was to inform future malnutrition management programmes by better understanding associations between MMH and the nutritional status of infants aged u6m. We conducted a health facility-based cross-sectional survey of 1060 infants in rural Ethiopia, between October 2020 and January 2021. We collected data on: MMH status (exposure) measured using the Patient Health Questionnaire (PHQ-9) and infant anthropometry (outcome); length for age Z-score (LAZ), weight for age Z-score (WAZ), weight for length Z-score (WLZ), mid upper arm circumference (MUAC), head circumference for age Z-score (HCAZ) and lower leg length (LLL). Linear regression analysis was used to determine associations between exposure and outcome variables. Mean (SD) age was 13.4 (6.2) weeks. The median score for MMH problem was 0 (inter quartile range 0 - 2) and 29.5 and 11.2% reported minimal and mild to severe depression score of 1-4 and 5-25, respectively. Mean (SD) LAZ was -0.4 (1.4), WAZ -0.7 (1.3), WLZ -0.5 (1.2), MUAC 12.4 (1.3) centimetre, HCAZ 0.4 (1.3) and LLL 148 (13.9) millimetre. In adjusted analysis, minimal MMH problems was associated with infant LAZ marginally (β=-0.2; 95% CI: -0.4, 0.001) and LLL (β=-2.0; 95% CI: -3.8, -0.1), but not with other anthropometric measurements. Significant associations were not found between mild to severe depressive symptoms and infant anthropometric outcomes. Covariates positively associated with infant anthropometric measurements were higher wealth index with LAZ (β=0.08, 95% CI: 0.03, 0.13), WAZ (β=0.12, 95% CI: 0.08, 0.17), WLZ (β=0.09, 95% CI: 0.05, 0.13), MUAC (β=0.06, 95% CI: 0.02, 0.11), and HCAZ (β=0.07, 95% CI: 0.03, 0.12); higher maternal schooling with LAZ (β=0.24, 95% CI: 0.05, 0.43) and WAZ (β=0.24, 95% CI: 0.07, 0.41); female sex with WAZ (β=0.16, 95% CI: 0.01, 0.31) and HCAZ (β=0.16, 95% CI: 0.001, 0.31); higher maternal age with LLL (β= 0.29, 95% CI: 0.07, 0.52); and improved water, sanitation and hygiene status with MUAC (β=0.07, 95% CI: 0.01, 0.12) and LLL (β=0.64, 95% CI: 0.04, 1.24). Covariates negatively associated with infant anthropometric measurements include female sex with MUAC (β=-0.33, 95% CI: - 0.48, -0.18) and LLL (β=-2.51, 95% CI: -4.15, -0.87); higher household family size with WLZ (β=-0.08, 95% CI: -0.13, -0.02); exclusive breastfeeding with MUAC (β=-0.39, 95% CI: -0.55, - 0.24) and LLL (β=-7.37, 95% CI: -9.01, -5.75); and grandmother family support with WAZ (β=- 0.2, 95% CI: -0.3, -0.0001) and WLZ (β=-0.2, 95% CI: -0.4, 0.1). In conclusion, only minimal, but not mild, moderate or severe, maternal/caregivers’ depressive symptoms are associated with infant anthropometry outcomes. Whilst plausible relationship between maternal mental health problems and offspring nutritional status exist, we are not able to show this because of small number of participants with moderate to severe level of depression in our study population. Thus, further evidence to understand and establish robust relationship between maternal mental health and offspring nutritional status is required.

Suboptimal nutritional status of school-age children in Addis Ababa: evidence from the analysis of socioeconomic, environmental, and behavioral factors

Abstract:Background: School-aged children (6–14 years old) are susceptible to malnutrition and micronutrient deficiencies. Environmental and behavioral factors greatly influence their nutritional status. This study aimed to examine the association between dietary factors and the nutritional status of school-aged children attending public and private schools in Addis Ababa. Methods: A community-based cross-sectional study design was employed from March to August 2023. A total of 309 study participants were randomly selected from 10 schools. Data were entered into Epidata version 3.1 and exported to SPSS version 23.0 for analysis. WHO Anthro Plus 1.0.4 was used to assess the measurements of weight-for-age (WAZ), height-for-age (HAZ), and BMI-for-age (BAZ) for overweight, stunting, and underweight, while wasting was assessed using MUAC. A Poisson regression model was used to determine the association between predictor variables and stunting, wasting, underweight, and overweight, with adjusted prevalence ratios (APR). APR and 95% CI were used to identify statistically significant variables. Results: Findings revealed the prevalence of wasting (15%), stunting (24%), underweight (36%), and overweight (19%) among school-aged children. Factors influencing stunting include marital status, house ownership, education level of parents/caregivers, child sex, and meal skipping. Wasting was linked to wealth index, child age, dietary diversity, dietary habits, water access, and toilet facilities. Underweight predictors include parent/caregiver age, marital status, and meal frequency. Moreover, school type appeared as a significant factor for overweight. Conclusions: The overall nutritional status of school-aged children is suboptimal, influenced by sociodemographic, environmental, and behavioral factors. Addressing these factors through targeted interventions is crucial, particularly for the most vulnerable groups.

Abstract:Abstract Objective: The current study aims to determine household-, maternal- and child-related factors influencing nutritional status among children under five in Mali. Design: Quantitative cross-sectional study using secondary data extracted from Mali DHS-VI 2018. Setting: Urban and rural areas of Mali. Participants: A total of 8908 children participated, with 3999 in the younger age group (0–24 months) and 4909 in the older age group (25–59 months). Results: In the younger age group, the prevalence of stunting, wasting and underweight was 18·8 % (95 % CI%: 17·5, 20·0), 24·6 % (95 % CI: 23·2, 26·0) and 13·2 % (95 % CI: 12·1, 14·3), respectively, while in the older age group, it was 24·9 % (95 % CI: 23·7, 26·2), 22·7 % (95 % CI: 21·5, 24·0) and 5·7 % (95 % CI: 5·0, 6·5), respectively. Being average or large size at birth, having piped source of water, receiving Zn, deworming, high maternal BMI, receiving Fe during pregnancy, higher maternal education and being rich were associated with lower odds of one or more form of undernutrition in both groups. On the other hand, children who were anaemic, drank from a bottle, maternal anaemia, current pregnancy of mothers and living in rural areas were associated with higher odds of stunting, wasting or underweight. Interestingly, children who received Fe supplementation had a higher odds of wasting in the younger group but lower odds of all forms of undernutrition in the older group. Conclusions: This study emphasised the potential risk factors associated with undernutrition in children. Children who consume non-potable water, have mothers with lower levels of education and BMI and reside in rural areas are more likely to experience undernutrition.

Abstract:Nutritional status is one of the most important causes of improper physical and mental development in children. The study attempts to assess the factors affecting the severity status of children aged 6–59 months' malnutrition based on the weight-for-age anthropometric index (z-score) and examine between-kebeles-level differences in determinants of the nutritional status of children. A community-based, cross-sectional study design was conducted from October 12 to November 12, 2022. A sample of 397 children aged 6–59 months primary data by applying multi–stage clustered sampling technique was used by considering their heterogeneity. The data were entered by SPSS and analyzed by using R version 3.4.0 and STATA 14.2 statistical software package using a multilevel ordinal logistic regression model and inferences were conducted at a 5% significance level. The results show that birth interval ≥ 24 months (OR = 1.431253, 95% CI 1.221337 1.6763421, P-value = 0.008), economic status of households medium (OR = 16.21466, 95% CI 1.221403 1.423929, P-value = 0.000), economic status of households rich (OR = 223.2856, 95% CI 1.34295 2.582325, P-value = 0.000), employment status of the mother unemployed (OR = 0.2291348, 95% CI 0.0529511 0.9966281, P-value = 0.049), No toilet facility (bush field) (OR = 0.3163329, 95% CI 0.1825356 0.5481975, P-value = 0.000), number of household members (OR = 0.9100682, 95% CI 0.8313481 0.9967315, P-value = 0.042), breastfeeding < 12 months (OR = 0.53803, 95% CI 0.322315 0.898135, P-value = 0.018), educational level of father Primary (OR = 4.601687, 95% CI 1.758009 2.22053, P-value = 0.000), educational level of father Secondary above (OR = 99.65229, 95% CI 2.533502 4.788896, P-value = 0.000) and geographical area (kebeles) were found to be important factors that affect a child's nutritional status between 6 and 59 months. 15% of the overall variation is attributable to the Kebeles level, according to two-level multilevel ordinal logistic regressions with estimates of the variation attributable to the Kebeles level equal to 0.569 and an intraclass correlation coefficient of 0.15. Due to the nature of the response variable random intercept model with random coefficients fitted the data adequately in predicting the severity status of children aged 6–59 months' malnutrition for the multilevel ordinal logistic regression model analysis. So, the researcher recommended that implementing primary health care and nutrition programs that would fit each kebeles' features in Itang Special Woreda to safeguard children from nutritional deficiency.

Abstract:Background and aim: The positioning of eliminating all forms of malnutrition within the spirit of the Sustainable Development Goals and the adoption of the United Nations resolution for a Decade of Action on Nutrition are a testament to strong global commitment to combat the double burden of malnutrition (DBM). Yet, there is a knowledge gap in sub-Saharan Africa (SSA) regarding the influence of socioeconomic status on DBM. We investigated the associative effect of socioeconomic status on DBM in SSA. Methods: Data for the study were extracted from the most recent Demographic and Health Surveys (DHS) of 29 countries in SSA conducted from 2010 to 2020. Bivariate and multivariate logistic regression models were fitted to examine the association between socioeconomic status and DBM. The results were presented using adjusted odds ratio (aOR) and 95% confidence interval (CI). Results: Children of obese mothers were less likely to be stunted compared to those born to mothers who were not overweight/obese [aOR = 0.70; 95% CI = 0.66-0.77]. The odds of stunting increased with wealth index, with children born to poorest mothers having the highest odds compared to those born to richest mother [aOR = 1.79; 95% CI = 1.64-1.95]. The odds of stunting among children was highest among those born to mothers with no formal education compared to those whose mothers had higher education [aOR = 2.73; 95% CI = 2.34-3.18]. Conclusion: DBM among children in SSA is predicted by maternal level of education, and wealth status. These results underscore the urgency of tailored interventions and policies that address DBM among women of reproductive age, with a particular focus on the socioeconomic disparities in SSA. To effectively combat this pressing public health issue, it is imperative to direct efforts towards empowering women to attain higher levels of education and to implement strategies that consider the specific needs of women across varying socioeconomic statuses.

Abstract:Bangladesh continues to face the persistent issue of child malnutrition. This study aims to investigate the influence of parental characteristics on undernutrition among children under the age of five in both urban and rural areas of Bangladesh. This study utilizes data from the Bangladesh Demographic and Health Survey (BDHS) 2017-18, which includes 7806 children under the age of five and their parents. The effects of parental education and occupation on undernutrition (e.g., stunting, underweight, and wasting) are examined using the binary probit regression technique. Among rural children, 32.6% are stunted, 22.5% are underweight, and 8.1% are wasted. The corresponding figures for urban children are 25.3%, 18.9%, and 8.9%, respectively. In all forms of undernutrition, children living in rural areas face greater challenges than those living in urban areas. The prevalence of stunting and underweight is low among children, irrespective of their rural or urban background, when both parents have the same level of education. The prevalence is also low when fathers are employers or traders and mothers are homemakers, or when highly educated fathers are employers or traders and highly educated mothers are homemakers. In both rural and urban areas, the risk of stunting, underweight and all forms of malnutrition among children decreases as parents' education levels increase. Children whose fathers work in service or business activities are less likely to experience stunting, being underweight, or wasting compared to children whose fathers work in agriculture or other professions in both areas. In urban areas, children born to mothers with lower levels of education are more vulnerable to wasting than children whose mothers have at least secondary education. To reduce child undernutrition nationwide, it is recommended that both parents have at least a secondary school education and that the father has a stable and sufficient income.

Abstract:We studied the efficacy of adenoidectomy in 213 children who had received tympanostomy-tube placement because of persistent and/or recurrent otitis media and had again developed otitis media after tubal extrusion. Ninety-nine of the children were assigned randomly to either an adenoidectomy group or a control group; in a separate trial, 114 children whose parents withheld consent for randomization were assigned according to parental preference. In both trials, control group outcomes appeared to have been biased favorably by the withdrawal of certain severely affected subjects from control status to receive adenoidectomy. Nonetheless, in both trials, adenoidectomy group outcomes were more favorable than control group outcomes during the first 2 follow-up years. Statistically significant differences were found mainly in the randomized trial, where during the first and second years, respectively, adenoidectomy subjects had 47% and 37% less time with otitis media than control subjects and 28% and 35% fewer suppurative (acute) episodes than control subjects. We conclude that adenoidectomy is warranted on an individualized basis for children who develop recurrent otitis media after extrusion of tympanostomy tubes.
