Abstract:Introduction A rising trend has occurred in the physical and mental health challenges faced by recovering UK service personnel. To support these individuals, bespoke inclusive multiactivity and adventurous training courses (MAC) have been developed. This study investigated the MAC’s influence on participants’ ability to sustain day-to-day changes that facilitate positive mental health and psychological need satisfaction. Methods The 146 UK service personnel who participated in this study attended a five-day MAC 12 months ago. To investigate how the supportive experience influenced participants’ lives, quantitative and qualitative data were collected via an online survey. Open-ended questioning and abductive analysis were conducted to understand mechanisms, influential aspects of the course and positive behaviour change. Results Positive behaviour changes were reported by 74% of the respondents. These changes align with positive psychological well-being (98%). Impactful elements of the course experienced by participants mostly aligned with the three basic psychological needs of autonomy (34%), competence (36%) and relatedness (61%). Conclusions Recovery support programmes that encompass health coaching adventurous activities, such as the MAC, can initiate long-term positive behaviour change for recovering military personnel. In this specific context, the concurrence of the self-determination theory concepts that underpin the course delivery and participant outcomes is a powerful endorsement of implementation fidelity.

Abstract:PURPOSE: To explore the psychosocial outcomes of an inclusive adapted sport and adventurous training course that aims to support the rehabilitation and personal development of military personnel who have sustained physical and/or psychological disability.METHOD: Narrative life story interviews were conducted with 11 men aged 20-43 taking part in one of the 5-day courses. A thematic narrative analysis was conducted, focusing on accounts that provided insights into personally meaningful psychosocial outcomes of the course.FINDINGS: We identified six themes, falling into two distinct clusters. "Bringing me back to myself" was achieved through the themes of (1) returning to activity, (2) rediscovering a sense of purpose, and (3) reconnecting to others. "New rooms to explore" was realised through (4) experiencing new activities, (5) being valued/respected/cared for and (6) being inspired by other people.CONCLUSION: Involvement in the course stimulated a balance of present- and future-oriented psychosocial outcomes through which participants both recreated aspects of themselves that had been lost through injury/trauma and moved forward with their lives as a result of new horizons of possibility.IMPLICATIONS FOR REHABILITATION: This 5-day inclusive adapted sport and adventurous training course offered meaningful psychosocial outcomes among military personnel who had experienced physical and/or psychological disability. The course helped participants recover aspects of their previous life and self through becoming physically active again, rediscovering a sense of purpose and reconnecting to others. Participants describe a broadening of life horizons as a result of the course, through new activities, being valued/respected/cared for, and being inspired by other people.

Abstract:Background: This study sought to capture the perspectives and experiences of two sub-groups of military veterans, namely those who had previously been identified as offenders and those employed as practitioners within the criminal justice system in the United Kingdom. Methods: The lead author undertook narrative inquiry in the form of life story research. The life stories of 17 in-depth interviews were conducted across England and Wales, allowing for insight into the lived experiences of two life story trajectories of the health of military veterans. Existing literature on salutogenesis, health, post-traumatic growth, and positive deviance has also been investigated. Results: Life story interviews were transcribed verbatim and analyzed concurrently using thematic analysis to identify emergent themes. The researchers used thematic analysis as an analytical framework to allow descriptive themes from the literature on salutogenesis, health, post-traumatic, and positive deviance to be compared with those of life-story interviews. Conclusion: The study adopts a salutogenic approach, which suggests that an important indicator for a sense of coherence is enabled through the positive utilization of resilience through the reconstruction of military identity and experience. Notably, the concept of salutogenesis demonstrates the ability to draw from internal and external resources as circumstances require, to survive, and because of this, the participants' health is maintained or improved. Hence, adaptation was notable through the coherence identified by the identity of being a veteran noted between the two subgroups and represents a continuous and dynamic process. The study suggests that the development of a sense of coherence is not restricted to the early decades of life but is a continuous process as contexts, personal circumstances, and opportunities arise. This later point is realized through the presence of post-traumatic growth and positive deviance, as they aid in the nurturing and development of a renewed sense of coherence via the known identity of military service and life.

Abstract:SuperC/sup 2/, a concurrent object-oriented language designed for the implementation of distributed software running on a network of computers, is described. The goal of superC/sup 2/ is to estimate the easiness of programming of distributed systems and harmony between the distributed systems and programming language. The design policy, basic ideas, and implementation issues for superC/sup 2/ are considered. Examples of the use of superC/sup 2/ and some discussions on the development environment are also presented. Although the first version of superC/sup 2/ only extended the concurrent processing facilities, the current version has extended the facilities to store the object to the file storage persistently. These facilities supply the basis of the distributed object-oriented database. SuperC/sup 2/ can manage the distributed objects by their name. Using these facilities, superC/sup 2/ provides a unified access method to the interprocess communication and file system.<<ETX>>

Abstract:During the Coronavirus disease pandemic, many U.S. veterans with posttraumatic stress disorder (PTSD) experienced increased symptomology and worsened mental health and well-being due in part to social isolation and loneliness. The Mission Alliance project explored these ramifications and prioritized critical issues expressed by U.S. veterans and stakeholders ( N = 182) during virtual regional meetings ( N = 32). Field notes created specifically for this project were recorded and thematically analyzed. Emerging themes included: (1) social isolation: missed opportunities, collapsed social circles, work-life balance, fostering relationships, and evolving health care delivery; (2) loneliness: deteriorated mental health, suffered with PTSD together but alone, looked out for each other, ambivalence toward technology, and strained and broken systems; (3) mental health: sense of chaos, increased demand and decreased access, aggravation, implementation of tools, innovative solutions, fear and loss, and availability of resources; (4) wellbeing: sense of purpose, holistic perspective on well-being, recognition of balance, persisting stigma, redefined pressures, freedom to direct treatment, and reconnection and disconnection. A PTSD-related patient centered outcomes research (PCOR)/comparative effectiveness research (CER) agenda was developed from these themes. Establishment of a veteran and stakeholder network is suggested to support, facilitate, and promote the PTSD-related PCOR/CER agenda. Furthermore, enhancement of opportunities for veterans with PTSD and stakeholders to partner in PCOR/CER is required to develop and conduct projects that lead to PTSD-related comprehensive care of veterans affected by traumatic events with the potential to translate findings to other populations.

Abstract:A major active metabolite of malononitrilamide FK778 (an immunosuppressant under development) is labeled M3. Due to a chiral center created during in vivo metabolism, the exploration of enantiomer profiles in clinical samples is critical to the characterization of the immunosuppressive activity of M3. An enantioselective liquid chromatography method with detection by tandem mass spectrometry (LC/MS/MS) was developed for the resolution of M3 enantiomers. It was experimentally confirmed that no interconversion between the two enantiomers occurred during sample preparation. This new approach was applied to measure the enantioselectivity of the M3 metabolite in human plasma samples from kidney transplanted patients. The assay results of 91 in vivo human samples from three subjects showed a ratio of 57:43 for the (-)-enantiomer (the 2nd eluter) vs. the (+)-enantiomer (1st eluter), indicating that the enantiometabolism of FK778 through human enzymes is essentially non-specific.

Abstract:Wellness adventure tourism is one attractive activity for tourists to gain a sense of self-achievement, ultimately leading to health and well-being. In recent years, mountain adventure tourism has made greater demands for the experience and sensation of such activities. The main aim of this paper is to seek an answer to the question of to what extent mountain-based adventure is the realization of eudaimonic well-being development. Qualitative research methods were employed in this study and a content analysis technique adopted to analyze data. The data were collected through 40 face-to-face interviews conducted at a mountain national park in Thailand. The results revealed that the motives driving tourists to participate in mountain hiking through eudaimonic experience could more easily achieve sustainable development of mountain-based tourism. It also helps to provide a greater marketing competitiveness advantage for a destination, helps to maintain and/or improve the physical and mental well-being of tourists, increases tourist numbers, and generates income to the community. Management implications It was found that the participants gave insight into the six components of the eudaimonic experience to generate well-being in life by participating in a wellness adventure tourism activity. The adventure activity helped the tourists to enhance their self-acceptance, purpose in life, autonomy, environmental mastery, positive relationship, and personal growth.

Posttraumatic growth-oriented peer-based training among U.S. veterans: evaluation of post-intervention and long-term follow-up outcomes

Abstract:Introduction: Exposure to trauma among U.S. military veterans occurs at a high rate, often resulting in continued difficulty with emotional adjustment and a diagnosis of posttraumatic stress disorder (PTSD). The present study provides data from 184 U.S. military veterans who completed a manualized posttraumatic-growth oriented training program during an integrative seven-day retreat. Methods: Data was collected at baseline, after program completion, and at 18-month follow-up. Results: Results on primary outcomes indicated significant increases, with medium to large effect sizes, in growth related outcomes. Specifically, there was a significant increase in scores by 54% on the posttraumatic growth outcome measure (PTGI-X) from baseline (M = 50.2, SD = 31.1) to endpoint (M = 77.4, SD = 29.6), t (183) = −8.78, p < 0.001. Also, results indicate that immediately following training (Day 7), participants reported a significant decrease of 49% on the PCL-5 from baseline (M = 39.7, SD = 17.6) to endpoint (M = 20.1, SD = 13.2), t (183) = 11.75, p < 0.001. Depression subscale scores decreased by 60% from baseline (M = 8.0, SD = 5.2) to endpoint (M = 3.2, SD = 3.0), t(183) = 10.68, p < 0.001; Anxiety scores decreased by 28% from baseline (M = 5.8, SD = 4.3) to endpoint (M = 4.2, SD = 3.5), t (183) = 4.08, p < 0.001; and Stress scores decreased by 50% from baseline (M = 10.0, SD = 4.4) to endpoint (M = 5.0, SD = 3.3), t (183) = 12.21, p < 0.001. Eighteen-month follow-up data was available for 74 participants and indicated that all significant changes in growth-related outcomes were maintained. Further, all significant changes in symptomatology-related outcomes were also maintained at follow-up. Discussion: These findings demonstrate both the immediate and the long-lasting impact of an integrative posttraumatic growth-oriented training program on psychological growth and PTSD symptom reduction among U.S. military veterans.

The Development and Acceptability of a Wilderness Programme to Support the Health and Well-Being of Adolescent and Young Adult Cancer Survivors: The WAYA Programme

Abstract:Detailed descriptions of theory, structure, and activities with causal links to specified outcomes of wilderness programs are lacking. Addressing this gap, the present qualitative study gives a thorough description of the development of the Wilderness programme for Adolescent and Young Adult (AYA) cancer survivors (WAYA). WAYA is adapted to the individual needs of AYA cancer survivors. It was conceived around Næss's ecosophy and the Positive Health Model, and refined based on findings from a scoping review and patient/public involvement. Programme aims were to increase physical activity, self-confidence, personal growth, joy, safety within nature, meaningful relationships, and self-efficacy. The programme was an eight-day expedition followed three months later by a four-day base-camp. Activities included hiking, backpacking, kayaking, rock climbing, mindfulness and bushcrafting. Evaluation of the programme through focus group and individual interviews with 15 facilitators and 17 participants demonstrated that a diverse group of participants, challenging activities, and mindfulness-based practices were found to positively influence group bonding and the learning process. Furthermore, including an expedition and base-camp component was found to be beneficial in supporting the development of participants' own personal outdoor practices. In conclusion, this study demonstrated that the WAYA programme is safe and well accepted by AYA cancer survivors.
The lasting impacts of outdoor adventure residential experiences on young people

Abstract:Evidence from four retrospective empirical research studies on lasting impacts (>12 months) of outdoor residential experiences for young people in the UK since 2015 are examined through a form of systematic review of papers and datasets. Thematic and comparative analysis identified lasting impacts as: self-confidence, independence and communication. Respondents also identified confidence, teamwork, life skills, intra-personal skills and the take up of new opportunities/activities as the impacts of use in young people's lives since their residential experience. A steps of change process within a theory of change model is used to examine the causal chains and attributes influencing outcomes. The intensity and challenge of the outdoor adventure residentials, and the power of groups, influence lasting impacts. These findings from large datasets across a range of contexts have implications for funders and policy makers for the provision of outdoor adventure residentials for young people.
Psychological aspects of the longest, solo, unsupported one-way polar ski expedition in Antarctica by a female adventurer

Abstract:Introduction Antarctic expeditions present exceptional physiological and mental challenges. Research data are lacking on psychological aspects of such endeavours. The aim of our study is to provide data on changes in mood, well-being, personality traits and personal experiences during an Antarctica crossing. Methods This is a study of a 33-year-old female British Army officer (height 175 cm; weight 75 kg; body mass index 24.49 kg/m 2 ; VO 2max 49 mL/kg/min) who completed the longest, solo, unsupported, one-way polar ski expedition. The expedition started at Hercules Inlet and finished at Ross Ice Shelf, lasting 70 days and 16 hours covering 1484.53 km, with temperatures estimating from −12°C to −50°C and wind speeds of up to 60 miles per hour. The adventurer pulled all equipment and nutrition in a pulk (sled), weighing approximately 120 kg. Five psychometric questionnaires were completed pre post and during the expedition, including the International Personality Item Pool – Neuroticism, Extraversion and Openness-60, Brief Assessment of Mood, Positive and Negative Affective Schedule, Profile of Mood States, Wellness questionnaire, as well as an unstructured open questionnaire. Results Mood generally deteriorated, particularly positive affective mood. Scores for fatigue and muscle soreness increased, with a reduction in sleep times. Personality traits of openness, agreeableness, and conscientiousness remained stable throughout the expedition, with some reduction in extraversion and an increase in neuroticism. Personal accounts give a unique insight into the increasing demands on the mental and physical impact of the expedition. Conclusions Meticulous preparation and planning may have led to a successful expedition, including physical preparation, prior on-field experience, and psychological preparedness and resiliance. Some of these strategies may be applicable to a range of settings, including future Antarctic expeditions, expeditions in extreme environments, or missions within a military context.

Can adventurous training have a role in improving clinical outcomes?

Abstract:The aim of military adventurous training (AT) is "to develop, through authorised challenging pursuits and within an outdoor environment, leadership and the qualities necessary to enhance the performance of military personnel during peace and war". An increasing amount of effort is being applied by all three services to increase participation in AT to achieve these, largely immeasurable, aims. Existing guidance to Commanding Officers dictates that, where possible, 20% of a unit strength should undertake some form of AT annually with 5% taking part in an overseas expedition. In a speech in 2008 Alan Johnson, the then Secretary of State for Health, acknowledged that for Armed Forces medical personnel 'just as important as clinical skills are issues such as leadership, communication, adaptability and teamwork." Controlled exposure to risk, discomfort and personal hardship is a common theme for both AT activities and military deployments. Both General Medical Council competencies for all doctors and the Royal College of Anaesthetists military module include elements which can effectively be taught and developed through training in an outdoors environment. These skills include communication skills, leadership, risk assessment and interdisciplinary working. In this review the value of AT in training doctors to develop those attributes is examined.
Extreme sport as an intervention for physically injured military veterans: the example of competitive motorsport

Abstract:Physically or psychologically injured military veterans are motivated and benefited by physical activity or sport that may involve high levels of achievement. The aim of this study was to provide an in-depth insight into the impact of Competitive Motorsport (CM) on physically injured/disabled veterans' subjective well-being and in turn determine if it improves the quality of their lives.This is a qualitative study. Two sets of semi-structured interviews were conducted with 14 male British military veterans including a 6-month follow-up. All participants were subjected to a permanent, moderate or severe, physical injury/disability. Data were analysed in accordance with the principles of Thematic Analysis.Five key elements (familiar environment, team spirit, adrenaline rush, competition and equality) made CM a unique experience for physically injured/disabled veterans. Various psychological and physical benefits were found, including an increased sense of accomplishment, opportunity to socialise in a non-clinical environment, embracement of body image and adoption of a healthier lifestyle. Since physical activity and sport are among growing research on alternative interventions for military veterans, they deserve serious consideration as part of the treatment regimens and rehabilitation programmes to improve physically inured/disabled veterans' physical and mental health.Competitive Motorsport is an effective means of improving physical health and subjective well-being of physically injured/disabled military veterans.Multiple perceived psychological and physical benefits were identified over time, including motivation for living, embracement of body image and adoption of a healthier lifestyle.Competitive Motorsport deserves consideration as part of the treatment regimens and rehabilitation programmes to improve physically injured/disabled veterans' physical and mental health while facilitating transition to civilian life.

Adventure-based mindsets helped maintain psychological well-being during COVID-19

Abstract:Objectives: Research conducted during the COVID-19 pandemic underscores the value of physical activity and nature for psychological well-being in the general population when people's mobility and activities are restricted due to government mandates. Since restrictions may thwart the psychological benefits reported from participation in adventure recreation (e.g., rock-climbing, white-water kayaking), it is important to understand the psychological well-being of people who previously benefited from adventure opportunities. Thus, the purpose of this study was to investigate the lived experiences and psychological well-being of adventure recreation participants during COVID-19 restrictions. Design: A descriptive phenomenological approach was used. Method: Participants were fifteen men, four women, and one non-binary person who engaged in a variety of adventure recreation activities that included ski-mountaineering, free-diving, rock-climbing, white-water kayaking, back-country skiing, skydiving, SCUBA diving, BASE jumping, and mountaineering. Participants had an average of 11.7 years of experience in at least one of their preferred adventure recreation activities. Participants were invited to take part in a visual and audio-recorded semi-structured interview on Zoom. Interviews lasted on average 69.3 min. Reflexive thematic analysis was undertaken inductively. Results: Overall, participants discussed a range of nuanced impacts that COVID-19 restrictions had on their psychological well-being. Participants discussed how negative affect resulted from restricted opportunities for physical and mental challenges, emotion regulation, connections to nature and people, and excitement. However, participants also benefited from several silver linings, such as reflecting on past adventures, a reduced need to compare themselves socially, and opportunities to spend quality time with others. Participants also explained how 'adventure-based mindsets' (e.g., resilience, focusing on controllable elements, humility) were protective strategies they used to limit the ill-being impacts of COVID-19 restrictions. Conclusions: This study extends the alternative sport and exercise literature by illustrating how participation in adventure has the potential to enhance participants' resilience and their ability to maintain psychological well-being across diverse and novel contexts (e.g., the COVID-19 pandemic).
Longitudinal Development of Primary and Secondary Posttraumatic Growth in Aging Veterans and Their Wives: Domain-Specific Trajectories.

Abstract:Resumen Posttraumatic growth (PTG), the positive psychological transformations that follow traumatic events, affects both direct survivors (primary PTG) and their significant others (secondary PTG). Though primary and secondary PTG have been widely investigated in the literature, their long-term trajectories decades after a traumatic event, especially as survivors enter older age, remain largely uninvestigated. Furthermore, it remains contested whether PTG adds up to a monolithic construct or rather consists of relatively independent components. Addressing these issues, we assessed a sample of Israeli male veterans from the 1973 Yom Kippur war (N = 349) and their wives (N = 156) at three time points over the course of nearly three decades. Both the veterans (primary survivors) and their wives (secondary survivors) reported PTG relating to the veterans' experiences during the war and/or captivity. Latent growth mixture modeling was conducted to identify trajectories of PTG on the five subscales of the Posttraumatic Growth Inventory. Long-term trajectories of PTG followed heterogeneous patterns of fluctuation over time and particularly as participants entered older age. On most subscales, decreasing PTG scores were evident, a trend that was more pronounced among the primary survivors than the secondary survivors as primary and secondary PTG fluctuate considerably in the long-term and seem to decrease as individuals enter older age. Furthermore, it would seem that PTG should not be considered a holistic concept but rather a conglomeration of positive changes. Implications of the findings are discussed within the context of limitations and potential intervening factors.
Travelling with a purpose: An ethnographic study of the eudemonic experiences of volunteer expedition participants

Abstract:Purposeful travel is apparent in new modes of tourism and particularly in volunteer holidays where tourists are searching for meaningful experiences that provide a sense of physical, emotional or spiritual fulfilment. Based on a qualitative study using participant observation, this article adopted the concept of eudemonia to explore the experiences of participants on an elephant conservation expedition to Bardia National Park, Nepal. Volunteer travel is used to connect with and understand the wider world. Rather than an escape, these journeys allowed participants to experience first-hand the hardships and realities of people in other countries; creating greater perspective and making them ‘better people’ on their return. Feeling virtuous can only be mobilised, however, if participants felt themselves to be useful rather than a passive or ill-equipped bystander. Findings also revealed how the return home is not always easy; that the process of re-entry can be isolating.

Volunteering on Heritage at Risk sites and wellbeing: A qualitative interview study

Abstract:Introduction: We explored experiences of volunteering in Heritage at Risk (HAR) projects, intended to mitigate the deterioration to historic assets, and the relationship with wellbeing. We aimed to understand the value of HAR to volunteers' wellbeing and relationships between HAR programme characteristics such as location, asset type and type of activity. Methods: We used a qualitative design with semi-structured interviews of a purposive sample of volunteers recruited via Historic England (HE), employing Systematic Grounded Theory involving open, axial and selective coding. Findings: We interviewed 35 volunteers (18 male and 17 female) participating in 10 HAR projects. We identified six themes from the data analysis. (1) Purpose-was associated with volunteering motivations; there were some barriers to volunteering and many types of facilitators, including accessibility to local heritage sites. (2) Being-volunteers showed an appreciation and attachment to their place of residence. (3) Capacity-to learn heritage-specific skills and diversify experiences in learning new skills (life, technical and personal). (4) Sharing-community engagement, connectedness, and inclusivity captured diversity and inclusion within volunteers across age, ethnicity, ability, and gender. (5) Self-nurture-HAR volunteering created physical, psychological, and social benefits with limited risks and adverse outcomes. (6) Self-actualisation-described volunteers reflecting on their experiences. Conclusion: HAR volunteering was associated with positive physical, social and psychological wellbeing outcomes. The study provides an evidence base for specific wellbeing benefits of volunteering at Heritage at Risk sites, although we could not conclude that HAR project activity was the cause of increased wellbeing. Public contribution: Staff from HE were involved in designing the project brief. In selecting the HAR project sites, we took advice and recommendations from HE staff across all their six regional offices.
The influence of short-term adventure-based experiences on levels of resilience

Abstract:This exploratory study investigated the impact of participation in a three-week adventure education (AE) expedition upon levels of resilience of university students. Resilience is considered to be a dynamic process of positive adaptation to significant threat or adversity and may be an important variable to study as college students often live stressful lives involving academic workloads, relationship building and self-identity that often require the development of coping skills and resilience. Little research, however, has been conducted on the ability to enhance levels of resilience through AE activities within a college or university setting. The specific research questions studied included: (1) Do resilience scores change as a result of a short-term AE experience, and (2) What specific experiences did participants report that were related to the concept of resilience? Using a mixed method, with quantitative responses from the treatment group and the comparison group, paired sample t-tests resulted in a significant increase only in the treatment group. Following semi-structured interviews with 10 graduates two to three years after the expedition, six themes emerged as important aspects in developing a sense of resilience. These themes included perseverance, self-awareness, social support, confidence, responsibility to others, and achievement.
