Abstract:Global food demand is increasing due to population growth and dietary transitions, resulting from rising incomes, are associated with increased prevalence of non-communicable diseases. Improving the sustainability of the food sector is also critical for achieving the Sustainable Development Goals. This study assesses for the first time the greenhouse gases emissions (Carbon Footprint - CF), the water footprint (WF) and the cost of three omnivorous diets recommended in Spain due to their health benefits: the Mediterranean diet (MD), the Southern European Atlantic diet (SEAD) and the Spanish dietary guidelines (NAOS). Analysis was conducted using standard Life Cycle Assessment and WF methods together with current Spanish food price data. The dietary energy recommendation of the SEAD is greater than that of MD and NAOS (11 and 15% respectively), and SEAD also has greater animal source food content than the other two diets. SEAD has a concomitantly higher CF, WF and cost scores in comparison with MD (+30%, +23% and +21% respectively) and NAOS (+15%, +9% and +21% respectively). Adjusting recommendations to meet the suggested Spanish adult dietary energy of 2228 kcal.capita-1.day-1 changed the environmental profiles of the diets and the NAOS has the highest environmental impact. However, the isocaloric diets had approximately the same cost. Analysis of the WF of the diets identified the major contribution of precipitation (the green WF) to the overall WF (88% of the total) and the significant contribution of animal-source foods to dietary WF. Regardless of the dietary scenario, better scores were identified for the Spanish recommendations analysed than those reported for other healthy diets identified in Europe. Differences in the recommended intake levels of certain food groups, cooking techniques and the origin of food products are behind these results. Environmental indicators should be considered alongside nutrition and health metrics when defining national dietary guidelines. Supporting citizens to follow healthy and environmentally-friendly dietary recommendations through, among other things, information campaigns and nutritional education programmes is essential. It is recommended the incorporation not only of health, but also of environmental indicators of these dietary options in the national dietary guidelines, as well as implementation of information campaigns and nutritional education programs among citizens to promote their adhesion since balanced dietary habits rich on plant-based products and low on animal-based ones involve multiple health and environmental benefits.
The Environmental Impact of an Italian-Mediterranean Dietary Pattern Based on the EAT-Lancet Reference Diet (EAT-IT)

Abstract:The definition of a healthy and sustainable diet is nowadays considered pivotal, but data related to environmental outcomes are still debated. In this study, we compared the carbon (CF) and water footprints (WF) of an Italian-Mediterranean (EAT-IT) dietary pattern designed on the "Planetary diet", with a pattern based on the Italian Dietary Guidelines (IDG). The influence of different food categories and food choices on environmental impact was assessed. To this aim, weekly dietary patterns were developed, considering food categories and related portions and frequencies of consumption. Results show that the EAT-IT dietary pattern, compared to the IDG, had a significantly lower CF (2.82 ± 1.07 and 3.74 ± 0.92 kg CO2/day, respectively) but not WF. Protein-rich foods were the main contributors to CF and WF in both dietary patterns. The increased substitution of frozen instead of fresh foods, imported instead of local fruits, greenhouse-grown instead of seasonal vegetables, and processed legume-based foods instead of unprocessed legumes caused an increasing worsening of the CF in both patterns, but with different magnitudes. Our analysis indicated that the EAT-IT dietary pattern can be considered sustainable for CF, but individual choices are likely to largely affect the final environmental outcomes.

Carbon footprint and nutritional quality of different human dietary choices

Abstract:Apart from industrial activities, our eating habits also have a significant environmental cost associated with crop cultivation, manufacturing processes, packaging, refrigeration, transport cooking and waste management. In a context of growing social awareness of the role of different dietary choices in the environment, the review of different alternatives on the road to a healthy and sustainable diet should integrate relevant information on the nutritional quality of different eating habits. Since dietary choices have an effect on environmental sustainability and human health, a literature review on different dietary choices has been conducted to determine the differences in carbon footprint and nutritional quality identifying the main hotspots trying to give advice towards the identification of sustainable diets. After applying a set of criteria for reference selection, 21 peer-reviewed studies have been analysed in detail, allowing the comparison of 66 dietary scenarios. We identified that the so-called Mediterranean and Atlantic diets present high nutritional scores and low carbon footprints. On the contrary, the dietary choices identified in northern and Western Europe, as well as in the United States, have the highest carbon footprints, highlighting the contribution of dairy products as a basic source of high-quality nutrients and protein. Broadly speaking, dietary choices rich in vegetables (e.g., vegan, vegetarian as well as Indian and Peruvian) have a better environmental profile than those rich in meat (mainly ruminant meat). In line with these findings, the shift in meat consumption habits from beef and veal to chicken, pork and poultry, the introduction of alternative foods to animal protein (e.g. quinoa) and the consumption of olive oil as a major source of vegetable oil may be compatible with a healthier and more environmentally friendly diet. However, the complete elimination of meat and dairy products from the daily diet may not be feasible in case the supply of some micronutrients (e.g., calcium and vitamin D) is not guaranteed. Limitations were identified in the consulted studies related to the consideration of the different system boundaries, as well as underlying uncertainties related to data sources. Therefore, efforts should be made to develop consistent and agreed-upon methods for estimating both the carbon footprint and nutritional quality scores.
Impact of mediterranean diet promotion on environmental sustainability: a longitudinal analysis

Abstract:Objective: This article aims to estimate the differences in environmental impact (greenhouse gas [GHG] emissions, land use, energy used, acidification and potential eutrophication) after one year of promoting a Mediterranean diet (MD). Methods: Baseline and 1-year follow-up data from 5800 participants in the PREDIMED-Plus study were used. Each participant's food intake was estimated using validated semi-quantitative food frequency questionnaires, and the adherence to MD using the Dietary Score. The influence of diet on environmental impact was assessed through the EAT-Lancet Commission tables. The influence of diet on environmental impact was assessed through the EAT-Lancet Commission tables. The association between MD adherence and its environmental impact was calculated using adjusted multivariate linear regression models. Results: After one year of intervention, the kcal/day consumed was significantly reduced (-125,1 kcal/day), adherence to a MD pattern was improved (+0,9) and the environmental impact due to the diet was significantly reduced (GHG: -361 g/CO2-eq; Acidification:-11,5 g SO2-eq; Eutrophication:-4,7 g PO4-eq; Energy use:-842,7 kJ; and Land use:-2,2 m2). Higher adherence to MD (high vs. low) was significantly associated with lower environmental impact both at baseline and one year follow-up. Meat products had the greatest environmental impact in all the factors analysed, both at baseline and at one-year follow-up, in spite of the reduction observed in their consumption. Conclusions: A program promoting a MD, after one year of intervention, significantly reduced the environmental impact in all the factors analysed. Meat products had the greatest environmental impact in all the dimensions analysed.

Abstract:Background: Although Spain has traditionally followed the Mediterranean diet, in recent years, changes have become noticeable in the food preferences of the population. These changes include adopting new diets and dietary trends, such as plant-based diets, intermittent fasting and raw food diets. These choices are influenced by cultural, social and socio-demographic factors, and their impact on health should be studied in detail. Aim: The objectives of this research are: (1) to study the prevalence of different dietary patterns among the Spanish population, (2) to explore the dependence of dietary choice on socio-demographic factors, (3) to investigate the relationship between the social habits and lifestyle choices of the population and how it affects health. Methods: A descriptive cross-sectional study was carried out on the Spanish population. Using a questionnaire constructed and validated by the research team, socio-demographic data were collected and different nutritional, social and lifestyle habits of the targeted population were explored. Results: A valid sample of 22,181 people was collected among which 17,573 (79.2%) people claimed to follow a Mediterranean dietary pattern, 1,425 (6.4%) people followed a plant-based diet, 365 (1.6%) people are vegans, 1,018 people (4.6%) practiced intermittent fasting, 252 (1.1%) people followed a raw food diet and 1,548 people (7%) claimed to follow other types of diets. The data show that younger people (18–25 years old) tend to adopt more often a plant-based diet than older people and that women tend to follow this type of diet more often than men. On the other hand, men seem to practice intermittent fasting more frequently. A higher tendency to practice a raw food diet was found among people living in villages than those living in cities. Moreover, some trends were found in different regions of Spain, with greater adherence to a plant-based diet in Catalonia, while intermittent fasting is more common in the Region of Murcia. Finally, the results indicate that individuals who pay attention to their diet and experiment with various dietary patterns are generally more health-conscious. This is reflected in their adoption of healthier behaviors, such as exercising more and reducing their intake of alcohol and sugary drinks.

Abstract:Objective: To estimate the environmental impact of a dietary intervention based on an energy-reduced Mediterranean diet (MedDiet) after one year of follow-up. Methods: Baseline and 1-year follow-up data were used for 5800 participants aged 55-75 years with metabolic syndrome in the PREDIMED-Plus study. Food intake was estimated through a validated semiquantitative food consumption frequency questionnaire, and adherence to the MedDiet was estimated through the Diet Score. Using the EAT-Lancet Commission tables we assessed the influence of dietary intake on environmental impact (through five indicators: greenhouse gas emissions (GHG), land use, energy used, acidification and potential eutrophication). Using multivariable linear regression models, the association between the intervention and changes in each of the environmental factors was assessed. Mediation analyses were carried out to estimate to what extent changes in each of 2 components of the intervention, namely adherence to the MedDiet and caloric reduction, were responsible for the observed reductions in environmental impact. Results: We observed a significant reduction in the intervention group compared to the control group in acidification levels (-13.3 vs. -9.9 g SO2-eq), eutrophication (-5.4 vs. -4.0 g PO4-eq) and land use (-2.7 vs. -1.8 m2). Adherence to the MedDiet partially mediated the association between intervention and reduction of acidification by 15 %, eutrophication by 10 % and land use by 10 %. Caloric reduction partially mediated the association with the same factors by 55 %, 51 % and 38 % respectively. In addition, adherence to the MedDiet fully mediated the association between intervention and reduction in GHG emissions by 56 % and energy use by 53 %. Conclusions: A nutritional intervention based on consumption of an energy-reduced MedDiet for one year was associated with an improvement in different environmental quality parameters.

Abstract:Providing a growing global population with healthy and sustainable diets is an immediate challenge. In the current study, estimates were obtained for the environmental footprints (land, water, and greenhouse gas (GHG) emissions) in association with the Mediterranean diet (MED) and the EAT-Lancet reference diet, which represents a healthy diet derived from sustainable food systems. We used a newly developed Sustainable Healthy Diet (SHED) index that was validated for the Israeli population by Tepper et al. in 2020. Methods A group of 525 participants were recruited via social media, email, and phone. Demographic characteristics, quality of life, and answers to the SHED-index questionnaire were obtained. Dietary assessment was performed using the 116-item Food Frequency Questionnaire (FFQ), which was developed for the Israeli population. Adherence to the MED was calculated using a 9-point score. Adherence to the EAT-Lancet reference diet was assessed through the consumption of 14 food components. The environmental pressure of these dietary patterns was determined based on the “footprint family indicators,” which include land, water, and carbon footprints per unit of agricultural and food products. We assigned values for each food comprising the FFQ and calculated the environmental load for each dietary pattern. Statistical analyses were performed using the R package version 4.1.1 to compare environmental footprint values according to tertiles of the MED score, EAT-Lancet score, and SHED score. Results The participants ( n = 525) were 49% women, educated (82% had academic education), and physically active, and only 13% were smokers. The highest tertiles of adherence to the MED, adherence to the EAT-Lancet reference diet, and the SHED index were associated with the lowest GHG emissions and land use, as well as higher water use. Meat consumption contributed the most to land use, while dairy contributed the most to GHG emissions, and fruits contributed the most to water use. Conclusions Our analysis reveals that animal protein is the highest contributor to GHG emissions and land use, while fruits and vegetables contribute the most to water consumption. Nevertheless, most of the fruits and vegetables are grown using treated wastewater, which reduces environmental pressure. Given these findings, we suggest that MED and EAT-Lancet dietary patterns should be included in national dietary guidelines.

Consumer resistance diminishes environmental gains of dietary change

Abstract:The environmental gains of dietary change are often assessed in relation to average national diets, overlooking differences in individual consumption habits and people's willingness to change. This study combines microdata on food intake and consumer behaviour to elicit the likely environmental gains of dietary shifts. We focus on the Netherlands owing to the country's ambition to halve its dietary footprint by 2050. Linking food recall survey data from a cross-section of the population (n=4,313), life cycle inventory analysis for 220 food products, and behavioural survey data (n=1,233), we estimate the dietary footprints of consumer groups across water, land, biodiversity and greenhouse gas. We find that meat and dairy significantly contribute to the dietary greenhouse gas (GHG) footprint (59%), land footprint (54%), and biodiversity footprint (59%) of all consumer groups and that male consumers impose a 30-32% greater burden than women across these impact areas. Our scenario analysis reveals that simply replacing cow milk with soy milk could reduce the GHG, land and biodiversity footprints by ≈8% if widely adopted by the Dutch adult population. These footprints could be further reduced to ≈20% with full adoption of the EAT-Lancet diet but with a significantly increased blue water footprint. However, when incorporating gender- and age-specific willingness to reduce meat and dairy consumption, the environmental gains resulting from partial adoption of the No-Milk diet and EAT diet diminish to a mere ≈0.8% and ≈4.5%, respectively. Consequently, consumer motivation alone is insufficient to realise the significant environmental gains often promised by dietary change. Yet, substituting high-impact food products offers a near-term opportunity to accelerate a rapid sustainable dietary transition. Future studies on sustainable dietary transition must incorporate consumer behaviour to fully comprehend the lock-in of food consumption patterns and targeted policy action required to secure a sustainable food future.

Abstract:Background: Plant-based diets are becoming more and more widespread among the Spanish population, progressively replacing the Mediterranean dietary pattern. Different studies have shown the motivations for adherence to these diets, and others have highlighted some health advantages and disadvantages. Purpose of the study: Further studies are needed to define the socio-demographic determinants that influence the choice of a plant-based diet and to study the relationship that the choice of dietary pattern has on the health and lifestyle habits of the population. Methods: A descriptive, cross-sectional study was conducted on the Spanish population. The NutSo-HH questionnaire, developed and validated by the research team, was used to gather socio-demographic, nutritional, social, and lifestyle information through non-probabilistic snowball sampling. Results: The questionnaire was completed by 22,181 Spanish citizens, of whom only 19,211 were of interest to the study. The socio-demographic variables gender, age, educational level, income level, and place of residence do not seem to influence the prevalence of a plant-based diet (n = 1638) compared to a Mediterranean diet (n = 17,573). People following a vegetarian or vegan diet have a lower BMI, and they consume less fried food, fast food, and ultra-processed dishes and fewer energy drinks or sugary beverages. They also do more exercise and sleep longer hours, smoke less, and consume alcohol less frequently. However, there seem to be more diagnosed eating disorders among people who follow a plant-based diet than those who follow a Mediterranean diet. Conclusions: People who adopt a plant-based diet tend to exhibit healthier lifestyle patterns and consume fewer foods that are detrimental to their health. However, it is essential for such dietary choices to be supervised by healthcare professionals to mitigate the risk of maladaptive behaviors evolving into eating disorders.

Abstract:The EAT–Lancet Commission has proposed a global benchmark diet to guide the shift towards healthy and sustainable dietary patterns. Yet it is unclear whether consumers’ choices are convergent with those guidelines. Applying an advanced statistical analysis, we mapped the diet gap of 15 essential foods in 172 countries from 1961 to 2018. We found that countries at the highest level of development have an above-optimal consumption of animal products, fats and sugars but a sub-optimal consumption of legumes, nuts and fruits. Countries suffering from limited socio-economic progress primarily rely on carbohydrates and starchy roots. Globally, a gradual change towards healthy and sustainable dietary targets can be observed for seafood, milk products, poultry and vegetable oils. We show that if all countries adopted the EAT–Lancet diet, the water footprint would fall by 12% at a global level but increase for nearly 40% of the world’s population.

Abstract:Globally, about 21–37% of total greenhouse gas (GHG) emissions are attributable to food systems. Dietary-related non-communicable diseases have increased significantly from 1990–2019 at a global scale. To achieve carbon emissions targets, increase resilience, and improve health there is a need to increase the sustainability of agricultural practises and change dietary habits. By considering these challenges together and focusing on a closer connection between consumers and sustainable production, we can benefit from a positive interaction between them. Using the 2019 EAT Lancet Commission dietary guidelines, this study analysed interview data and food diaries collected from members of Community Supported Agriculture (CSA) schemes and the wider UK population. By comparing the environmental sustainability and nutritional quality of their respective diets, we found that CSA members consumed diets closer to the EAT Lancet recommendations than controls. We identified significant differences in daily intakes of meat; dairy; vegetables; legumes; and sugar, and the diets of CSA members emitted on average 28% less CO2 compared to controls. We propose that agricultural and wider social and economic policies that increase the accessibility of CSAs for a more diverse demographic could support achieving health, biodiversity, and zero-emission policy targets.

Abstract:The importance of a sustainable diet is an emerging concept within sustainable food systems. Food systems emit 30% of greenhouse gases, which needs to change. A cross-sectional study was carried out to determine the knowledge, attitudes, and habits of students and professionals in the health sciences regarding a sustainable diet, comparing these to the Spanish population. We further aimed to analyse the consistency between the knowledge and attitudes of these individuals and their dietary habits and analyse the consumption of different food groups that are typical of a sustainable diet. A survey was completed by a total of 415 participants, both university students in the health sciences and health professionals. These two groups were more knowledgeable of sustainable diets than the general population, although certain concepts are unfamiliar to both populations. A positive attitude towards sustainable food habits was also observed among the population studied. The health sector reported having better eating habits than the overall population. A significant positive correlation was observed between higher fruit and vegetable consumption and deeper knowledge and more favourable attitudes. People with less knowledge and worse attitudes reported consuming more red and processed meat. The findings of this study could inform targeted interventions for health professionals given the need to promote a healthy diet but also a healthy and sustainable diet for planetary health.

Abstract:The Mediterranean diet (MD) has been shown to be a healthy dietary pattern (DP), and lately it is increasingly being studied as a sustainable DP. The aim of this study is to analyze whether the MD is a sustainable and healthy DP accounting for its carbon footprint, water footprint (WF), land use (LU), and/or energy use, based on the existing scientific literature. We conducted a systematic review following PRISMA guidelines and registered in PROSPERO (CRD42022309916). We included 35 studies: 25 modeling studies, 7 cross-sectional, and 3 longitudinal. Twenty-four studies compared the sustainability of the MD with that of other DPs; 21 assessed the sustainability of the MD compared with dietary consumption data; and 9 evaluated the MD's adherence and its environmental impacts. A total of 29 studies analyzed the carbon footprint, 11 the LU, 20 the WF, and 7 the energy use of the MD. Six articles assessed the health aspect of the diet apart from the environmental impact. The MD showed high nutritional quality, ranging between 122 and 178 points on the health score and between 13.51 and 90.6 points on the nutrient-rich food index. Using the results for environmental footprints in the same measurement units, we were able to quantitatively compare the most frequently assessed diets with MD. When compared with other diets, 91% of the studies referred to the MD as a sustainable DP, and most of the articles in which its adherence was assessed obtained an inverse correlation with the environmental footprints. Environmental footprints of the MD ranged from 1.03 to 5.08 kg CO2-eq/person-day for greenhouse gas emissions, 257.2-2735.2 L/person-day for WF, and 4-14.8 m2/person-day and 2.85-3.32 m2∗year/d for LU. In summary, the available evidence suggests that, in general, the MD is a sustainable and healthy DP, which aligns with planetary health.

Abstract:Non-communicable diseases (cardiovascular diseases, cancer, chronic respiratory diseases, diabetes, etc.) are the leading cause of death worldwide, accounting for 71 % of global deaths. The risk factors for all of them are closely linked to lifestyle and in particular to diet. For this reason, public health policies in European Union countries, in line with World Health Organization (WHO) recommendations, include in their priority measures actions aimed at promoting healthier food environments. One of these measures are policies for the reformulation of processed foods to reduce critical nutrients: salt, saturated fats, trans fats and added sugars. In Spain, as part of the NAOS Strategy, food reformulation measures have been implemented since 2008. Of all of them, the Plan de Colaboración para la Mejora de la Composición de Alimentos y Bebidas y otras Medidas 2020 stands out, which has led to a reduction in the content of salt, sugar, saturated and total fat in foods belonging to 57 subcategories marketed by the main food manufacturing and marketing companies from 2016 to 2021. Despite this, to date no population-based study has been conducted to test the impact of the measure on the diet of the Spanish population. The present article, by way of example, shows results of the theoretical estimation of total sugar intake in the pre-reformulation period (2016) and post-reformulation period (2024) based on the consumption of beverages in Spain. Data highlight that reformulation policies could have positive effects that need to be evaluated in representative studies of the Spanish population assessing the overall diet, in order to solidly confirm their positive effects on the population's diet.

Abstract:In the context of the sustainable use of our planet's resources, there is a pressing need for environmentally friendly diets. Moreover, health issues linked to inadequate nutrition are rising, further emphasizing the need for more balanced diets. The aim of the planetary health diet developed by the EAT-Lancet commission is to sustainably feed 10 billion people in 2050 while not exceeding planetary limits. According to these guidelines, the EU should reduce red meat intake by 80 % and multiply legume intake by 15. This significant shift from animal to plant-based proteins could modify trade partnerships and EU dependency on plant-based proteins for animal feed, currently discussed within the EU protein strategy. This study evaluates land use changes and environmental consequences of this change in dietary patterns by applying a consequential life cycle assessment approach combined with an agri-economic model, additionally assessing if this diet shift could help to achieve EU Green Deal targets. For this simulation, the partial equilibrium model CAPRI was used, which is a global agricultural market model focusing on Europe, including bilateral trade worldwide. We found that the diet shift results in gaps between domestic demand and supply which are compensated by increased imports of legumes and additional exports of red meat from the EU due to an increased demand for red meat in non-EU countries as a consequence of decreased red meat prices. Land use changes within NUTS-2 regions indicate broad disparities depending on regional specialisation and profitability. The results from our life cycle assessment show limited benefits for most environmental impact categories within the EU given the considerable change in diets. GHG emissions from the agricultural sector would be reduced by 9 %, representing 4 % of the change needed to achieve overall EU GHG emission targets i.e., a reduction of 55 % by 2030 compared to 1990 levels. Yet, the mitigation potential allocated to EU production of shifting diets could be greater if international trade were not offsetting expected local environmental benefits. Indeed, if EU red meat and animal feed production were reduced proportionally to the demand shift, domestic legume production could hypothetically be extended on the available agricultural areas coming along with improvements in all environmental impact categories. Scientific contribution and media regularly point out the significant influence of individual consumer behaviour as an impulse for change: "the power is on our plates", while the results of our simulations show that this holds only to a limited extent. To minimise environmental pollution, strong supply-side policies are needed to complement the power on the consumers' plates.

Changes in Dietary Behaviours during the COVID-19 Outbreak Confinement in the Spanish COVIDiet Study

Abstract:The aim of this study was to evaluate whether dietary behaviours of the Spanish adult population were changed during the COVID-19 outbreak confinement. For that purpose, an online questionnaire, based on 44 items including socio-demographic data, Mediterranean diet (MedDiet) Adherence Screener (MEDAS) as a reference of a healthy diet, processed foods intake, changes in their usual food choices and weight gain was distributed using social media and snowball sampling. A total of 7514 participants (37% aged below 35 years, 70.6% female, 77.9% university-level education or higher) from all the Spanish territory completed the questionnaire. Results outlined healthier dietary behaviours during the confinement when compared to previous habits. Overall, the MEDAS score (ranging from 0 to 14, whereby higher a scoring reflects greater adherence to the MedDiet) increased significantly from 6.53 ± 2 to 7.34 ± 1.93 during the confinement. Multivariate logistic regression models, adjusted for age, gender, region and other variables, showed a statistically significant higher likelihood of changing the adherence to the MedDiet (towards an increase in adherence) in those persons who decreased the intake of fried foods, snacks, fast foods, red meat, pastries or sweet beverages, but increased MedDiet-related foods such as olive oil, vegetables, fruits or legumes during the confinement. COVID-19 confinement in Spain has led to the adoption of healthier dietary habits/behaviours in the studied population, as reflected by a higher adherence to the MedDiet. This improvement, if sustained in the long-term, could have a positive impact on the prevention of chronic diseases and COVID-19-related complications.

Abstract:Background: Diet plays an important role in the development of cardiovascular diseases and in maintaining sustainable planetary boundaries. The EAT-Lancet Planetary Health Diet could potentially provide co-benefits for human and environmental health, yet evidence on the association between adherence to the EAT-Lancet Planetary Health Diet and risk of cardiovascular events and environmental impact is limited. Methods: We investigated the association between adherence to the EAT-Lancet diet and coronary heart disease (CHD) and stroke risk, and with greenhouse gas (GHG) emissions, land use, and dietary species richness (DSR). We included 364,745 adult men and women participating in the European Prospective Investigation into Cancer and Nutrition (EPIC) study. Food frequency questionnaires were used to create a score reflecting adherence to the EAT-Lancet diet (EAT-Lancet diet-score), ranging from 0 (no adherence) to 140 (complete adherence). A (pro-) vegetarian version of the score, the EAT-Lancet dietVV-score, was also created, which rewarded low to no consumption of all animal-based foods. Cox proportional hazard regressions were used to study the association of adherence to the EAT-Lancet diet with CHD and stroke incidence. Linear regression analyzed the association with GHG emissions, land use, and DSR. Findings: Over a median follow-up of 12.8 years, we identified 12,690 CHD and 7,088 stroke cases. After multivariable adjustment, those most adherent to the EAT-Lancet diet had lower risk of incident stroke (HRQ5vsQ1: 0.59, 95%CI = 0.54 to 0.64), and of incident CHD for those younger than 60 years old at baseline (HRQ5vsQ1: 0.86, 95%CI = 0.79 to 0.93). High adherence to the EAT-Lancet diet reduced GHG emissions by 1.7% (95%CI = -1.9 to -1.5) and land use by 6.2% (95%CI = -6.4 to -5.9). The EAT-Lancet dietVV-score further reduced GHG emissions and land use by 14.3% (95%CI= -14.5 to -14.0) and 18.8% (95%CI = -19.0 to -18.5), respectively, when comparing extreme quintiles, while hazard ratios for CHD and stroke remained unchanged. Those most adherent to the EAT-Lancet diet consumed 16.1% (95%CI = 15.9 to 16.4) more plant species and 19.7% (95%CI = -20.11 to -19.40) fewer animal species. Interpretation: Higher adherence to the EAT-Lancet diet was associated with co-benefits for both cardiovascular outcomes and environmental indicators, including dietary species richness. Lower GHG emissions and land use were achieved by further reducing consumption of animal-based products. Abstract:Background. Climate change and global health are inextricably linked. Thus, health systems and their professionals must adapt and evolve without losing quality of care. Aim(s). To identify health and environmental co-benefits derived from a sustainable diet and promotion strategies that favor its implementation. Methods. A systematic search for articles published on sustainable diets and human/planetary health published between 2013 and 2020 was conducted on the databases PubMed, Cinahl, Scopus and Trip from 4 to 7 May 2020 in accordance with the PRISMA guideline. Results. A total of 201 articles was retrieved, but only 21 were included. A calorie-balanced diet mainly based on food of plant origin that would allow the attainment of 60% of daily caloric requirements and a low protein intake from animal foods (focusing in fish and poultry) could significantly reduce global morbi-mortality and the dietary environmental impact maintaining a framework of sustainability conditioned by the consumption of fresh, seasonal, locally produced and minimally packaged products. Discussion. The implementation of sustainable diets requires working on the triangulation of concepts of food–health–environment from schools and that is permanently reinforced during all stages of the life by healthcare workers, who should establish the appropriate modifications according to the age, gender and health situation.
