Abstract:Background. A sociological study has been conducted among anesthesiology interns of the second year of study whose results make it possible to interpret the formation of a body of knowledge, abilities and skills in them, which are gained in the process of training on the specialty “Anesthesiology and intensive care”. Among the tasks in postgraduate medical education, those stand out that involve the training of competent specialists capable of quickly solving practical tasks in health care with the introduction of innovative technologies. The aim of the study: to establish trends in the formation of professional competence among anesthesiology interns through their sociological survey. Materials and methods. At the end of 2022, a sociological survey was conducted among anesthesiology interns (a total of 63 doctors of both sexes, average age was 24.2 years). Results. When choosing the future profession of an anesthesiologist, 49.20 % of respondents indicated that it was their own choice; 23.80 % of interns noted the influence of parents on the choice of future specialty, 15.87 % indicated their friends or acquaintances or other factors. The interns noted the presence of personal experience of working in health care institutions — 88.88 % answered positively (at the same time, 14.28 % of respondents mentioned previous experience of working in an intensive care unit as a nurse) and 11.12 % denied such experience. Most respondents said what attracts them to anesthesiology as a specialty: the possibility of obtaining a quick positive result of their activity — 85.71 % and the satisfaction of saving a human life — 57.14 %. A significant part of the interviewees noted such a factor as prestige (38.09 %) and a small number of interns noted “other” (6.34 %). Among the subjects and disciplines during training at a higher educational institution, the most important for anesthesiologists, according to the interns, are physiology and pathophysiology — 93.65 %, pharmacology — 80.95 %, normal or pathological anatomy — 46.03 %, physics — 28.57 %, chemistry — 25.39 %; 1.58 % of the respondents answered that all subjects are important at the same time or clinical pharmacology. When choosing a future specialty, interns chose from such medical specialties as surgery (39.6 %), neurology (9.52 %), emergency medicine (19.04 %), and others. Among the directions in anesthesiology, interns consider obstetric anesthesiology (47.62 % of respondents), cardiac anesthesiology (66.66 %), neuroanesthesiology (53.96 %), trauma anesthesia (68.25 %), pediatric anesthesiology (46.03 %), anesthesia in abdominal surgery (46.03 %), intensive care (3.17 %), and 4.76 % of respondents answered that all directions in anesthesiology are important. Intern doctors noted the following necessary personality traits and human values for successful work as an anesthesiologist: determination (74.60 %), leadership qualities (44.44 %), excellent health (33.33 %), the ability to quickly make the right decisions (90.47 %), other (11.11 %). The interns indicated which directions in anesthesiology they would like to thoroughly study during the face-to-face training at the department, namely: cardiopulmonary resuscitation (92.06 % of respondents), intensive care in emergency situations (85.71 %), ultrasound diagnosis in anesthesiology (71.42 %), acute poisonings (68.25 %), prevention and treatment of pain syndromes (80.95 %), features of providing assistance to the victims (39.68 %) and others: emergency medical care and management of patients with combat trauma in the intensive care unit (4.76 %), features of anesthesia in various pathological conditions, regional anesthesia, pharmacotherapy of emergencies, intensive care for acute pathologies (1.58 % of respondents). Conclusions. The conducted sociological research among anesthesiology interns of the second year of study allowed to detect the following trends in the formation of professional competence of anesthesiology interns: a significant amount of information and high professional requirements for anesthesiologists imply constant improvement of their knowledge and skills that is impossible without a sufficiently high level of motivation. The presence of previous work experience of intern doctors in health care institutions emphasizes the meaningfulness of the choice of future specialty, social and professional understanding of the essence of practical medicine, ensures communication skills with colleagues. Among the subjects and disciplines taught in higher education institutions, in the opinion of anesthesiology interns, the most important are: physiology and pathophysiology, pharmacology, normal or pathological anatomy, physics and chemistry. When choosing the future specialty “Anesthesiology and intensive care”, interns most often chose from such medical specialties as surgery, neurology, and emergency medicine. For successful work as an anesthesiologist, according to anesthesiology interns, the most relevant are: determination, leadership qualities, excellent health, the ability to quickly make the right decisions, etc. Among the directions in anesthesiology, interns of the second year of study expressed their desire to thoroughly study: cardiopulmonary resuscitation, intensive care in emergency situations, ultrasound diagnosis in anesthesiology, acute poisonings, prevention and treatment of pain syndromes, features of providing assistance to the victims, and others.
THE ROLE OF SIMULATION TRAINING FOR TEACHING (IN TEACHING) ANESTHESIOLOGIST INTERNS: A TWO-WAY SURVEY

Abstract:Resume. In the modern system of training intern anesthesiologists, simulation training is becoming an increasingly common tool for achieving educational goals. The simulation ensures the acquisition of professional competencies without the risk of harm to the patient and enables teachers to objectively assess the level of professional training of each intern. There is a growing need to review traditional approaches to the interaction between a teacher and an intern with the increasing use of simulation technologies. The purpose of the study was to determine the role of simulation training in the teaching of anesthesiologist interns. Materials and method. A prospective audit was conducted at the Department of Surgery, Anesthesiology and Intensive Care of the O.O. Bogomolets National Medical University in the period from April to June 2023. Interns of the 2nd year of training in 2023 in the speciality «Anesthesiology and intensive care» were trained in a 1-day simulation training «Critical incidents during anaesthesia". We examine data (lower-level outcomes) assessing the role of simulation training on learning outcomes and willingness to work independently from the perspective of interns and faculty using an anonymous questionnaire at the end of the training. Results: the audit included 36 responses from interns and 10 from teachers. Many intern doctors noted an increase in their motivation to study, achievement of educational goals and correction of errors as a result of debriefing thanks to the conducted simulation training. Intern doctors noted that such training was very close to real conditions, it increased confidence in working with patients and readiness for emergencies, helped identify knowledge gaps and was more effective compared to traditional training (lectures, seminars). The results of the teachers' survey confirm the importance of simulation training as an effective means of training interns, which ensures the achievement of educational goals without risks for patients, accelerates the process of acquisition of competencies by learners, increases their motivation to study and objectively reflects the level of skills and knowledge of interns. Conclusions. Our research has proven that simulation training should be a mandatory component of the training program of the clinical internship in the speciality «Anesthesiology and intensive care» with an increase in their share in the training program of specialists. Creating conditions of mutual trust between interns and teachers is the key to the social mission of ensuring the quality of postgraduate education. Conducting a debriefing after working out each simulation scenario lays the foundation for the formation of critical thinking in future doctors and helps to avoid mistakes in real clinical practice. The role of systematic simulation training at the postgraduate level to improve outcomes in real clinical practice may be the subject of further research.

The Art and Science of Securing an Anaesthesiology Training Job: A Survey of the Key Factors in Two Australian Tertiary Hospitals

Abstract:Background There is no specific formal guidance on what prospective trainees must focus on to secure an anaesthetic training position in Australia, and there is little in the literature to advise both applicants and their mentors. Method This study aims to ascertain the views of anaesthetic clinicians from two Australian tertiary referral hospitals on what they consider most important for selection. A paper-based survey was conducted at both hospitals across three groups, totalling 104 participants with a 100% response rate. Results The characteristics most agreed upon to be of at least some importance were clinical anaesthetic knowledge (98%, 102/104), teaching (95%, 99/104), basic science and courses (94%, 98/104), other critical care experience (93%, 97/104), and anaesthetic experience for more than six months (92%, 96/104). Of these, anaesthetic experience of greater than six months, non-anaesthetic critical care experience, and the completion of relevant courses were felt to be most important. Furthermore, good referee reports (95%, 99/104), especially those that come from anaesthetists (75%, 78/104) as well as having previous experience working in the institution applied to (88%, 92/104) were also seen as important factors. 'Non-technical' skills (40%, 42/104) were also regarded as an important factor, with immense competition for a few training positions (45%, 47/104) as the greatest barrier. When it came to selection, prevocational trainees consistently ranked the majority of criteria higher than accredited trainees or specialists. Conclusion This staff survey in two Australian hospitals has shed light on factors considered critical in securing an anaesthetic training position. It underscores the significance of clinical anaesthetic knowledge, basic science proficiency, and relevant critical care experience.
[How are beginning students in surgery selected? Results of a survey of German consultant surgeons]

Abstract:Aim: After finishing medical school it is getting more and more difficult for medical students to get one of the rare positions in a surgical training program. Only a good career plan and an intelligent strategy for the first steps in surgical life seem to be an advantage for that competition. On the other hand the selection of basic surgical trainees only based on the curriculum vitae and brief interviews may not be satisfying. The aim of this investigation was to answer the following questions: 1. How do consultant surgeons select their basic trainees? 2. How important is the applicant's record and the interview? 3. Which educational and personal criteria are important for the selection? 4. How can medical students improve their chance of getting a position in a surgical training program. Methods: Using an anonymous and standardized questionnaire 39 chiefs of university departments of surgery and 44 consultant surgeons each from general and local hospitals were interviewed concerning their selection process and criteria required for application in their training program. The candidates were chosen from an official address list (Deutsche Chirurgie 98, Berufsverband der Deutschen Chirurgen). The questionnaire was sent to the candidates by official postal service together with a prepaid return envelope. Results: Twenty-eight chiefs of university departments of surgery (71.8%) and 29 (65.9%) and 20 (45.5%) of the interviewed consultant surgeons in general and local hospitals responded to the 89-item questionnaire. It turned out that the majority of applications were send to the clinics at the applicant's initiation. Personal relationships or networks are important. Position advertisements are not important. 85.1% of the responders are satisfied with their procedure of selection, nevertheless, 43.4% are interested in recruiting markets. In detail, chiefs of surgical departments in university clinics get a mean of 12.3 (5-50) applications for basic surgical training per month, consultant surgeons in general hospitals 6.5 (4-20), those in local hospitals 5.6 (2-10). The completion and the contents of the doctor's thesis, former research efforts, foreign languages, computer knowledge and other qualifications are the most important criteria in the selection process. Reliability, loyalty, physical and psychological stability, organising and planning ability, stamina, sociability and independence are the most valued personal skills. Conclusions: Despite the satisfaction with the established common selection process, the high rate of participation in our study shows how important this subject is ranked by leading surgeons. It turned out that besides professional qualification, personal skills are a major criterion in the selection of basic surgical trainees.
Specialty choices among graduating medical students in University of Calabar, Nigeria: implications for anesthesia practice

Abstract:BackgroundDespite its strategic role in health-care delivery, anesthesia does not attract medical manpower in developing countries, more so among medical trainees. This has resulted in an alarming lack of physician anesthetists. This study aims to determine the rate of selection of anesthesia as a specialty choice and factors that influence medical students when choosing specialties.Patients and methodsA cross-sectional study was conducted on final-year medical students in the University of Calabar. A semistructured self-administered questionnaire was distributed to a total of 105 final-year students who had undergone their posting in anesthesia. Summarization of data was done using frequencies/proportions and graphs, and significance level was set at P-value less than 0.05.ResultsThe mean age of respondents was 27.2 ± 4.15 years. Of the students, 72 (69%) were male, whereas 33 (31%) were female. Of them, 96 (91%) indicated interest in specializing, 34 (32%) preferred obstetrics and gynecology, 20 (19%) wanted pediatrics, 13 (12%) preferred family medicine and three (3%) preferred anesthesia. Factors that influenced choice of specialty among the graduating students included: personal interests in 85 students (81%), future job opportunities in the field in 66 students (63%) and requirement of specialized skill in 65 students (62%). In all, 46 respondents (44%) reported that their anesthesia posting experience was interesting, whereas 27 respondents (28%) reported it as very educative. The duration of the posting was rated very short by 25 students (24%).ConclusionWith less than 5% of the graduating medical students indicating interest in anesthesia specialization, improvement in training facilities and provision of incentives to intending trainees are strongly recommended.

Abstract:BackgroundThe “Second Stage of the Physician Exam” at the end of the 5th year of medical school in Germany is the final step before the “Practical Year.” An exam preparatory class can cover the complete content of Obstetrics and Gynecology (OB/GYN) in two days. We raise the question of whether such training might promote students’ interest in the given specialty during occupational decision making and whether it could even be used by hospitals as a recruitment tool. This investigation is even more important in the context of fierce competition among young professionals at clinics and in different specialties.MethodsWe conducted a multimodal course evaluation for four exam preparatory courses (each of which lasted two days and involved 8.5 h of teaching), including pre- and post-course tests with 20 multiple-choice questions to quantify the level of skill gain. Additionally, a standardized evaluation of course satisfaction was performed, followed by a post-exam questionnaire that dealt with studying activities and individual professional objectives.ResultsOverall, n = 197 students took part in four identical courses. Among them, n = 121 completed the pre−/post-course tests, n = 170 completed the evaluation, and n = 110 completed the post-exam questionnaire. An average improvement from 13.9 to 17.2 correct answers was observed (max. 20; pre−/post-difference 95%-CI: [2.77; 3.86], t-test: p < 0.0001). By trend, the students noted that course participation positively influenced their later choice of specialty training (m = 3.63; scale 1 = “strongly disagree,” 5 = “strongly agree”).ConclusionsIn addition to self-studying, condensed classroom training is effective and reasonable and might also increase the attractivity of OB/GYN among students and have a positive effect on recruitment.

Abstract:Background: Demographic changes in the society and among doctors, as well as changing attitudes towards and norms of how living should be structured are creating challenges regarding the organization of work environment in the hospital. In addition, organization of medical training is increasingly being influenced by economic considerations as well as a high level of medical specialization. We asked young respiratory physicians how they assessed their current situation with respect to quality of medical training and organization of their work environment. Methods: From September to November 2019, we performed an online survey adressing young respiratory physicians in Germany. Participants were recruited via three emails (baseline and reminders after 2 and 6 weeks) sent by the German Respiratory Society (DGP) and the German Union of Pulmonologists (BdP). The questionnaire consisted of a maximum of 62 questions. Apart from own questions that had been aligned with other questionnaires from similar surveys in other medical specialties, we also assessed the effort-reward ratio (ER ratio) based on the short version of the effort-reward imbalance questionnaire (16 questions). Results: We recuited n = 224 participants (33.8 ± 4.5 years, 5.4 ± 2.9 years of medical training, 54.4 % female , 86.8 % with German nationality). A little under half of the interviewees (n = 103, 46 %) reported to be very or generally satisfied with their working conditions, while n = 60 (27 %) were unsure. The main reasons for not being satisfied were long working hours and high work-load, as well as a lack of streamlining the work environment in the hospital to the specific needs of doctors. Despite the fact that many participants were satisfied, a large majority (n = 166, 88.2 %) depicted an unfavorable effort-reward ratio imbalance (adjusted mean 1.89 ± 2.18). Conclusion: Compared to many other European countries and internationally, the German healthcare system offers high-quality patient care and a well-equipped work environment. Increasing demands in the health care sector, however, are leading to a gratification crisis that not only harms the health and work performance of doctors but is also leading to reduced attractiveness of the job that might possibly lead to the search for new fields of activity or migration. Respiratory medicine is a discipline of growing interest and motivating young doctors to secure the promotion of this discipline is increasingly important. Factors harming the growth of this discipline should be immediately addressed. The results of this survey might help leaders in the field to restructure the work environment and medical education according to the actual needs.

Methode Die "LandArztMacher" organisieren eine vierwöchige Gruppenfamulatur im Bayerischen Wald in Praxen und Kliniken mit gemeinsamen Lehrveranstaltungen. Teilnehmende wurden vor und nach dem Praktikum zu den Themen "Wichtigkeit eines Praktikums im ländlichen Raum"; "Attraktivität des ländlichen Raums" (Skala: 0/keine bis 10/volle Zustimmung) sowie die persönlichen Vorstellungen über die Aufgaben von AllgemeinmedizinerInnen (Skala: 0/ keine bis 100/exakte Vorstellung) befragt. Bei vorliegender Arbeit handelt sich um eine wiederholte Querschnittstudie, bzw. um eine Vorher-Nachher-Studie. Als Lagemaß wird der Median berichtet, als Streuungsmaß der Interquartilsbereich. Ergebnisse Die Teilnehmenden (N=363, 74% weiblich, n=267, Alter: Spannbreite 19–46 Jahre, Mittelwert: 23,2 Jahre, SD: 2,41 Jahre) aus dem klinischen Abschnitt des Medizinstudiums hielten ein Praktikum im ländlichen Raum vorher wie nachher für sehr wichtig (Median: 8 bzw. 9) und können sich gut vorstellen, später auf dem Land zu arbeiten (Median: 7 bzw. 8). Ihre Einstellung zum kulturellen Angebot oder der Infrastruktur änderte sich nicht (jeweils Median: 6). Nach dem Praktikum hatten die Studierenden eine sehr viel genauere Vorstellung über die Tätigkeit von AllgemeinmedizinerInnen (Median: 65 bzw. 90). Background The excellent project "LandArztMacher" is an attempt to work against the predicted shortage of rural doctors in Germany with diverse approaches. Method "LandArztMacher" is a clinical traineeship with four weeks of practical training in general practices and clinics in the Bavarian countryside, accompanied by joint professional teaching. Participants were asked before and after the internship about the topics "importance of an internship in rural areas"; "attractiveness of rural areas" (scale: 0/no agreement to 10/full agreement). Ideas about the tasks of a general practitioner were assessed (scale: 0/no idea at all to 100/exact idea). The present study is a repeated cross-sectional study. The median is reported as the location measure and the interquartile range as the dispersion measure. Results Participants (n=363, 74% female, n=267, age: range 19–46 years, mean: 23.2 years, SD: 2.41 years) from the clinical section of the medical studies considered an internship in the rural area before and after the internship very important (median: 8 and 9, respectively) and could well imagine working in the countryside (median: 7 and 8, respectively). Their attitude towards the cultural offerings or the infrastructure did not change (median: 6 in each case). After the internship, the students had a more precise idea of what a general practitioner tasks are (median: 65 and 90, respectively). Summary A well-structured four-week rural internship can enhance the appeal of future rural employment through individual supervision and collaborative training. Medical trainees' speciality considerations at their transition from under- to postgraduate education: a descriptive, cross-sectional study

Abstract:Purpose: This paper aims to provide knowledge on medical trainees' considerations about specialisation as they move from undergraduate to postgraduate medical education; especially their interest in general practice compared to other specialities. Method: We developed and content-validated a questionnaire to examine medical trainees' speciality considerations and conducted a descriptive, cross-sectional study. All medical trainees initiating their internship in Denmark in 2022 (N = 1,188) were invited to participate in the study. Medical specialities were categorised as hospital service specialities, internal medicine specialities, primary care, psychiatry specialities and surgery and emergency specialities. Descriptive statistics were used to describe the cohort and examine the participants' speciality considerations by assigning them to one of the following three orientations: committed, undecided or non-committed to a speciality. Results: The response rate was 38.8% (n = 461), and participants' mean age was 27.4 years with a majority of females (68.1%). Nearly 25% of the participants had general practice as speciality preference, and only 13.9% had excluded general practice for future specialisation. Overall, around half of the participants had general practice as a first, second or third preference for specialisation. Conclusion: Danish medical trainees show considerable interest in general practice at the time of their transition from undergraduate to postgraduate education. However, to meet future demands on the primary care, further recruitment of general practitioners is still needed. This knowledge of the specialities' recruitment potential will likely be of interest to medical educators and healthcare planners alike.
Joining to promote young talents: an experience report from the first German Summer School for Obstetrics and Gynaecology for medical students

Abstract:PurposeThe Commission for the Promotion of Young Talents of the German Society for Gynaecology and Obstetrics e.V. was founded in 2017, aiming to inspire medical students for a career in obstetrics and gynaecology by developing a concept for the first German Summer School in this field. Here, medical students shall be introduced to this multifaceted specialty and have their interest in it kindled.MethodsThis article reports about the experiences of the first gynaecological summer school which was held at the University Hospital Heidelberg for 2 days in August 2018. The programme included keynote presentations, discussion roundtables and skills-lab training. To assess students’ related satisfaction, and to improve future projects, an evaluation survey with seven items and two open-comment questions was given to each participant after the event.ResultsMostly female students [n (♀) = 37, 93%; n (♂) = 3, 7%] from 15 different medical universities from all over Germany participated. Available places were booked within 1 week. Participants were in their clinical part of their studies between the 5th and 16th semester. The average rating of the event was excellent with 1.1 points (1 = best–5 = worst), while the selection of topics scored lowest marks with an average rating of 1.7 points.ConclusionDue to the great success, the high demand and the student’s positive evaluation, annual summer schools in obstetrics and gynaecology are planned. Because most of the participating students have shown a high interest and have appropriate education in the domain, the information content of keynote presentations could be increased above basic level in future projects.

Abstract:Abstract The l-tert-leucine-derived AmidPhos/silver(I) catalytic system has been developed for the asymmetric [3+2] cycloaddition of azomethine ylides with electronic-deficient alkenes with or without Et3N. Under optimal conditions, highly functionalized endo-4-pyrrolidines were obtained with modest to high yields (up to 99% yield) and enantioselectivities (up to 98% ee).
Logbooks alone are not enough: initial experience with implementing a logbook for medical students in a clinical internship in gynecology and obstetrics

Abstract:Abstract Background Logbooks are being increasingly widely used as a means of improving medical education and further training. They will in all probability continue to be mandatory in the Practical Year (PJ) in Germany even after the upcoming amendment of the Medical Licensing Regulations (ÄAppO). However, there are different approaches to their design and use, and these are also currently undergoing considerable change. This study for the first time examines and discusses the influence of logbooks on students’ evaluation of a gynecology internship. Methods The study was based on a well-established two-part 1-week internship course, with initially unstructured morning classes on wards and duty areas, along with precisely planned afternoon classes with skills training by peer teachers and seminars supervised by duty-exempted physicians. The postgraduate lecturers were prepared for the introduction of the logbook in a special course, and the aim was to optimize morning classes by introducing learning objectives adapted to the respective locations. The effects over 38 weeks of practical training were examined in evaluations by 235 prospectively group-randomized students with and without logbooks ( n = 166 and n = 66, respectively; three datasets were not evaluable). Results In the cohort comparison, the logbook group responded significantly more positively toward the internship at the start of the course ( P = 0.046). In the final evaluation, however, medical supervision during the entire internship was rated significantly more poorly ( P = 0.007). The logbook cohort also considered that guidance based on learning objectives was significantly worse, as was the extent to which wards and duty areas were prepared for the students ( P = 0.001 and P = 0.029). Conclusions Introducing a logbook to optimize clinical teaching in internships may raise expectations that cannot always be met. In addition to adapting the learning objectives to a general framework that is less favorable in comparison with the Practical Year, the least that is required appears to be simultaneous and continuous mentoring of the lecturers, as well as an increase in staffing resources.

Attractiveness of medical disciplines amongst Swiss first-year medical students allocated to different medical education tracks: cross-sectional study

Abstract:Abstract Background As most countries, Switzerland is experiencing a shortage of physicians especially in general practice and new medical education tracks with respective focusses have been started in response. This study investigated Swiss medical students’ career openness and attractiveness of different medical disciplines as well as the concordance of students’ career intentions with assigned medical education tracks. Methods Cross-sectional study surveying first year medical students assigned to four different Swiss medical education tracks with distinctive additional education focuses (ETH Zurich: medical technology and engineering, University of St. Gallen and University of Lucerne: primary healthcare and University of Zurich: no distinctive focus). Results We surveyed 354 medical students (response rate across all included medical education tracks 71.1%), 64.8% female, mean age 20 years. Regarding career openness, we found that 52.8% of medical students had neither a strong commitment nor a strong reservation for any of the proposed career options and 17.0% had a strong commitment. Among medical disciplines, medical subspecialties were attractive to the largest part of students (inpatient subspecialties attractive for 71%, outpatient for 58%), attractiveness of general practice was moderate (30%), academic (22%) and industrial sector (17%) careers were least attractive. Proportions of medical students attracted to general practice were similar at medical education tracks with focus on primary healthcare compared to other medical education tracks (32.2% vs. 25.8%, p = 0.391). Conversely, proportions of medical students attracted to academic or industry careers were significantly higher at the ETH Zurich compared to other medical education tracks (37.2%, vs. 13.1%, p < 0.001 and 31.9%, vs. 8.8%, p < 0.001 respectively). Conclusion While most first-year medical students were open to careers in many medical disciplines, attractiveness of disciplines varied strongly. Students attracted to academic or industrial careers accumulated at the medical education track with concordant teaching focus but students attracted to general practice did not accumulate at medical education tracks focused on primary healthcare. For medical education tracks with primary care teaching focus this is both a challenge and an opportunity to specifically counteract the shortage of general practitioners in Switzerland.

Surgical experiences of final-year undergraduates and the impact on their career aspiration stratified by sex/gender

Abstract:Objectives: Surgical undergraduate training takes place in a male-dominated work environment that struggles with recruitment problems. Experiences of cultural and sex/gender-specific barriers of women in surgery have been reported worldwide. Overall, the experiences that students have in coping with the emotional impact of surgery as a profession are thought to be crucial to their subsequent career choices. We investigated whether students' self-reported experiences differed by sex/gender in terms of frequency and content, and whether they were related to their career aspirations. Methods: In Germany, the final year represents the last part of the undergraduate medical study program. At the Mannheim Medical Faculty, a 12-week surgical training is mandatory. After completing their surgical training, the students were asked about their experiences and their later career aspirations. These experiences were analysed using quantitative content analysis. The relationship between the quality of experience and career aspirations as well as sex/gender differences were statistically measured. Results: In the 475 questionnaires analysed (response rate 52%), the number of positive and negative mentions does not differ by sex/gender. However, male students feel more actively involved and female students feel poorly briefed and think that supervisors are less interested in their training. A significant influence on career aspirations was found in the performance category for female and male students, and in the supervision category for female students. Conclusion: The positive experiences that students gain while performing surgical activities have an impact on their subsequent career choices. In particular, a good settling-in period and encouragement to actively participate could help to attract more women to surgery.

Abstract:Abstract Aim This study aims to explore the impact of emergency department internships on the attitudes towards death among undergraduate nursing students and their preferences for end‐of‐life care settings. Additionally, the study analyzes the reasons behind nursing students' choices of end‐of‐life care settings and provides insights for improving undergraduate education on attitudes towards death and end‐of‐life care, and provide reference for the development of emergency hospice care. Design This study adopts an observational design with a self‐controlled before‐and‐after approach. Methods A questionnaire survey was conducted with 96 nursing interns between July 2021 to June 2022. Demographic information and data on attitudes towards death, and preferences for end‐oflife care location were collected by online questionnaire. Paired test were conducted to compare differences between groups. Results The study included a total of 96 nursing students with an average age of 21.11 years. The scores for the avoidance‐acceptance dimension of death attitudes before and after the internship were 2.40 (1.80, 3.00) and 2.20 (1.60, 3.00), respectively, showing a significant difference ( Z = −2.084, p = 0.037). Factors such as gender, experience in caring for critically ill or dying patients, knowledge of death education, and discussions about death at home were found to influence nursing students' attitudes towards death. Nursing students expressed a preference for receiving end‐of‐life care and treatment in their homes or in hospice/palliative care wards, while the intensive care unit, emergency department, and nursing homes were the least preferred settings. There were significant differences in nursing students' preferences for end‐of‐life care settings before and after the internship ( p = 0.000). Importantly, the number of nursing students expressing a desire to receive end‐of‐life care in the emergency department increased from 2 to 7 after the internship, while the number of students not wanting end‐of‐life care in the emergency department decreased by 5.

European otorhinolaryngology training programs: results of a European survey about training satisfaction, work environment and conditions in six countries

Abstract:ORL-students and residents have an ongoing debate about the "best" programme in Europe. Aim of this study was to comparatively assess differences among programmes in training, satisfaction, quality of life (QoL) of residents and recent otorhinolaryngologist (ORL) specialists in France, Germany, Spain, Italy, Austria, and Belgium. A self-administered anonymous questionnaire, structured in ten sections including general information, provided guidance, working environment, training structure, teaching of medical students, publication work, QoL, and satisfaction with training, were emailed to residents and recent ORL specialists. 476 returned questionnaires from 6 countries revealed that daily work hours were the highest in France and Belgium with 11 and 10.4 h on average, respectively. QoL, work conditions, and salary were best in Germany followed by Austria in terms of possibility of part-time contracts, better respect for post-duty day off, and compensation for overtime. Satisfaction with training including support and guidance of seniors was lowest in Italy, but, on the other hand, the publication work and support had a more important place than in other countries. In Belgium, there was some gap between the quality of teaching and feedback from seniors as well as apprenticeship. The highest satisfaction with training was in France and Spain followed by Austria. The study results provide guidance before choosing an ORL training programme in Europe. Country-specific strengths could be included into future harmonization efforts to improve all programmes, facilitate professional exchange and, finally, establish standards-of-care carried out by well-trained doctors also looking after a satisfying work-life balance.
