Abstract:BACKGROUND Highly cross-linked polyethylene (HXLPE) has been an excellent bearing for total hip arthroplasty (THA) due to improved wear characteristics compared to conventional materials. Patients 50 years of age or younger are at high risk for wear-related complications of their THA, and few studies have followed these patients who have HXPLE into the third decade. METHODS In a retrospective review of 88 consecutive THAs performed in 77 patients aged 50 years and younger (mean 41; range, 20 to 50), in which HXLPE was utilized, they were evaluated for their clinical and radiographic results at an average of 20-year follow-up (range, 18 to 24). The current study reports on longer-term follow-up from our previously published series at shorter follow-up times. Patients were categorized by femoral head material: cobalt chrome (n = 14), ceramic (n = 30), and oxidized zirconium (n = 22) and by femoral head size: 26 mm (n = 12), 28 mm (n = 46), and 32 mm (n = 8). Harris Hip Scores (HHS) were collected preoperatively and at the most recent follow-up. Radiographs were evaluated for linear and volumetric wear, radiolucent lines, and osteolysis. RESULTS Mean HHS improved from 47.1 (SD [standard deviation] 8.8) preoperatively to 92.0 (SD 7.7) (P < 0.0001) at 20-year follow-up. There was one hip that was revised for recurrent instability, and no hip demonstrated radiographic evidence of loosening or osteolysis. The mean polyethylene linear wear rate was 0.017 (SD 0.012) mm/year, and the mean polyethylene volumetric wear rate was 3.15 (SD 2.8) mm3/year, with no significant differences based on articulation type or head size. CONCLUSION Total hip arthroplasty with HXLPE in patients ≤ 50 years of age continues to demonstrate excellent long-term clinical and radiographic outcomes with low wear characteristics at 20-year follow-up, regardless of femoral head material or size.

No Revisions Attributable to Wear of Highly Cross-Linked Polyethylene Liners: A Long-Term Follow-Up Study

Abstract:Background: There is a paucity of data beyond 15 years on the survivorship of total hip arthroplasty since the introduction of highly cross-linked polyethylene (HXLPE) liners. Our aim was to evaluate implant survivorship, liner wear rates, and clinical outcomes after primary total hip arthroplasty using HXLPE liners implanted between 1999 and 2002. Methods: Between 1999 and 2002, 690 primary total hip arthroplasties utilizing 28-mm femoral heads and HXLPE liners of a single design were identified using our institutional total joint registry. Femoral heads were made of metal in 96% of cases and ceramic in 4%. The mean age was 56 years, 48% were women, and the mean body mass index was 30. Survivorship analyses were performed for the outcomes of implant revision, reoperation, and complications for the entire cohort. Linear HXLPE liner wear rates were determined on 197 hips with radiographs with more than 18.5 years of follow-up. Results: At 20 years, survivorship free of revision was 94%, free of reoperation was 92%, and free of any complication was 81%. There were no documented wear-related revisions. The linear wear rate at a mean of 20.3 years postoperatively was 0.02 mm/y. There was no statistically significant difference in measured wear observed between the first available postoperative radiographs and those taken at the final follow-up. The use of elevated liners, patient body mass index, age, preoperative diagnosis, acetabular component inclination, and anteversion angles were not associated with increased wear rates. Mean Harris hip scores improved from 52 preoperatively to 90 at greater than 18.5 years CONCLUSIONS: Primary total hip arthroplasties using a single first-generation HXLPE liner demonstrated excellent survivorship and clinical outcomes at long-term follow-up with no wear-related revisions. Wear rates of HXLPE liners at 20 years are exceedingly low and are not significantly impacted by acetabular component position or patient-dependent variables such as BMI. Level of evidence: IV.

Abstract:BACKGROUND: Crosslinked polyethylene (XLPE) liners used for primary THA have demonstrated lower wear rates than noncrosslinked, conventional polyethylene (CPE) liners through the first decade of clinical service. However, little high-quality evidence is currently available regarding the second decade performance of these implants and it remains uncertain whether the onset of osteolysis has simply been delayed or if the wear associated with XLPE liners will remain low enough that osteolysis will not occur. It is also unknown how the potential reductions in wear and osteolysis will influence long-term revision rates.QUESTIONS/PURPOSES: Do patients who underwent THA with XLPE liners demonstrate (1) a lower rate of revision for wear-related complications; (2) a reduced wear rate; and (3) a lower frequency of osteolysis compared with those with CPE liners?METHODS: Over an 18-month period from 1999 to 2000, 226 patients who had 236 primary THAs consented to participate in a randomized controlled trial conducted at one institution. To be eligible for intraoperative randomization, patients had to be implanted with a 28-mm cobalt-chrome alloy femoral head, a 4-mm lateralized liner, and the same cup and stem design. Six patients with six THAs were excluded intraoperatively because they did not receive study components for reasons unrelated to the liner material. The remaining 230 THAs among 220 patients were randomized to XLPE liners or CPE liners. The mean age at surgery was 62 ± 11 years and there were no differences in age, gender, or body mass index among the groups. There was no differential loss to followup between the study groups; among patients not known to be deceased or having undergone revision, minimum 14-year radiographic followup is available for 85 THAs including 46 with XLPE and 39 with CPE liners. Polyethylene wear was measured radiographically using Martell's Hip Analysis Suite and areas of osteolysis were evaluated before revision or at most recent followup. Revision rates at 15 years using reoperation for any reason and revision for wear or osteolysis were calculated using cumulative incidence considering patient death as a competing risk.RESULTS: The cumulative incidence of revision at 15 years using reoperation for wear-related complications as an endpoint was lower in the XLPE group than the CPE group (0%, 95% confidence interval [CI], 0%-0% versus 12%, 95% CI, 7%-19%; p < 0.001). Among unrevised THAs with minimum 14-year radiographic followup, the mean steady-state linear wear rate for THAs with XLPE liners was lower than the mean linear wear rate for the THAs with CPE liners (0.03 ± 0.05 versus 0.17 ± 0.09 mm/year; mean difference, 0.14; 95% CI, 0.11-0.17; p < 0.001). Osteolysis of any size was noted among 9% (four of 46) of the hips in the XLPE group and 46% (18 of 39) of the hips in the CPE group (odds ratio, 0.19; 95% CI, 0.07-0.51; p < 0.001).CONCLUSIONS: This randomized study with followup into the second decade demonstrated reductions in revision, wear, and osteolysis associated with the use of XLPE. The low wear rates and absence of any mechanical failures among the XLPE liners at long-term followup affirm the durability of these components that did not incorporate antioxidants. Although osteolysis has not been eliminated, it occurs infrequently and has not caused any clinical problems to date.LEVEL OF EVIDENCE: Level I, therapeutic study.

Measurement of polyethylene wear and its clinical significance in total hip arthroplasty

Abstract:Objective To measure the linear polyethylene wear with use of a computer-assisted two-dimensional method, and to evaluate the relationship between wear and long-term clinic-radiological findings. Methods Radiographs of 104 cementless total hip arthroplasties were examined for osteolysis, radiolucent line, cup abduction angle, cup migration, cancellous condensation, stem subsidence, distal pedestal formation and so on. To measure the linear polyethylene wear, migration in the femoral head center relative to the cup shell center was calculated on digitized consecutive radiographs with special computer software program, the degree and direction of wear were also determined. Patients were followed-up for an average period of 8.4 years. Results The average Harris Hip Score was 53.8 points preoperatively and 88.5 points at the latest follow-up. Fourteen hips ( 13.5 % ) were loose. Total linear wear was 1. 14 mm,wear rate was 0. 15 mm/year. Acetabular osteolysis was correlated to polyethylene wear, and a high wear rate was associated with loosening of the components. Multivariate regression analysis showed a lack of correlation between wear rate and patients' age, body weight, cup size, cup abduction angle, activity level and Harris Hip Score. Conclusion The method is preferable for measuring the polyethylene wear of metal-backed acetabular component and other components of various designs. There was clear relationship between wear and acetabular osteolysis or loosening of the prostheses. Therefore, polyethylene wear is important in evaluating the clinical and radiological outcomes of total hip arthroplasty.
Is Wear Still a Concern in TKA With Contemporary Conventional and Highly Crosslinked Polyethylene Tibial Inserts in the Mid- to Long-Term?

Abstract:Background Modern literature has brought into question if wear of tibial inserts made from conventional or highly-crosslinked polyethylene (HXL PE) is still a factor limiting longevity of total knee arthroplasty (TKA) in the mid- to long-term. It is the objective of this study to determine: 1) most common causes of mid- to long-term TKA failure, 2) the prevalence of delamination, and 3) the medial/lateral linear wear rates of conventional and HXL PE tibial inserts retrieved in the mid- to long-term. Methods A tibial insert retrieval cohort of 107 inserts (79 conventional, 28 HXL PE) with a minimum time in situ of 6.5 years (mean 11.7±4) was studied. Failure causes were determined from chart-review, delamination presence was assessed microscopically, and medial/lateral linear wear was determined by minimal thickness changes measured with a dial-indicator. Results The most common mid-to long-term etiologies for failure were instability (43.9%), PE wear 15%), aseptic loosening (14%) and infection (13.1%). Delamination occurred in 70.1% of inserts (75% conventional, 58% HXLPE). Gross material loss due to delamination appeared to be the underlying reason for at least 35% of cases exhibiting instability. Of the cases removed for infection, 75% exhibited no histopathological hallmarks of acute infection. The medial/lateral wear rates were 0.054/0.051 (conventional) and 0.014/0.001 (HXL) mm/year, respectively. Conclusions Polyethylene wear still appears to be a major primary and secondary cause for TKA revision in the mid- to long-term. Wear may manifest as destabilizing delamination or as continuous release of fine wear particles potentially resulting in inflammatory responses and subsequent failure.

Abstract:Objective: To explore the intermediate outcomes of third-generation alumina-on-alumina total hip arthroplasty (THA). Methods: A consecutive series of 165 primary alumina-on-alumina total hip arthroplasties were performed in 135 patients. Three patients died and 13 patients lost follow-up so that a total of 119 patients (146 hips) were available for study. The mean patient age was 53.4 ± 11.0 years old (range: 26-79). Hydroxyapatite. (HA)-coated press-fit acetabular cups were used in all cases. At femoral side, 123 cases were implanted with HA-coated uncemented stems and 23 cases implanted with high polished double taped cemented stems. All patients were evaluated clinically and radiographically after a minimal follow-up duration of 60 months. Results: The preoperative mean Harris hip score of 49.6 ± 7.9 points improved to 91.7 ± 3.0 points at the last follow-up (P < 0.05). The 5-year survival for any reason lead to revision was 96.6%. Five hips were under revision, 1 for acetabular shell loosening and migration, 1 for Vancouver B2 periprosthetic fracture, 1 for fracture of pure alumina liner and 2 for infections. All other prostheses demonstrated no radiographic evidence of loosening. No periprosthetic osteolysis was found. There were 2 cases of dislocations and 2 patients presented postoperatively with "squeaking-like" hips. Conclusion: The clinical and radiographic outcomes after primary THA with third-generation ceramic-on-ceramic bearing surfaces are favorable after a minimal follow-up duration of 5 years. Abstract:To measure the linear polyethylene wear with use of a computer-assisted two-dimensional method, and to evaluate the relationship between wear and long-term clinic-radiological findings.Between December 1991 and December 1995, the senior author performed 40 primary total hip arthroplasties with use of H/G porous-coated acetabular component. Radiographs of 40 H/G cementless total hip arthroplasties were examined for osteolysis, radiolucent line, cup migration and so on. To measure the linear polyethylene wear, migration in the femoral head center relative to the cup shell center was calculated on digitized consecutive radiographs with special computer software program.The minimum follow-up was 10 years. Total linear wear was 2-8 mm, the mean rate of linear polyethylene wear was (0.32+/-0.31) mm/year. Twelve acetabular components (30%) were revised at an average follow-up point of 12 years. Three types of polyethylene liner failure were identified: wear-through of the polyethylene liner with resultant metallosis (5 hips), excessive wear necessitating revision (5 hips), and polyethylene liner dissociation without gross evidence of wear (2 hips).High wear rates and femoral and acetabular osteolysis have been and still are the main problems in uncemented total hip replacement. There is clear relationship between wear and acetabular osteolysis or loosening of the prostheses. The poor long-term results with these uncemented total hip arthroplasties illustrate the necessity of regular radiographic evaluation in order to detect osteolysis and liner failure, which are both generally asymptomatic until catastrophic failure appears.

Abstract:Starting from pheophorbide a methyl ester (5), a degradation product of chlorophyll a, routes for the preparation of 3-vinylporphyrins 6 and 9 and of an [A,C]-divinylporphyrin 16 are described. These porphyrins were obtained as a result of chemical modification of 5 on ring D and the exocyclic ring E. Diels–Alder reactions of these (di)vinylporphyrins with dimethyl acetylenedicarboxylate, to afford new regiochemically pure benzoporphyrin derivatives 29 and 30 and dibenzoporphyrin derivative 33 for potential use as sensitizers in photodynamic therapy of tumors, were also investigated. Polyethylene wear and cup migration of cemented total hip arthroplasty with femoral heads made of oxidized zirconium, steel, or cobalt chromium: a 10-year secondary analysis from a randomized trial using radiostereometry

Abstract:Background and purpose: We aimed to evaluate polyethylene (PE) wear, cup migration, and clinical outcome over 10 years in total hip arthroplasties (THA) using different articulations. Methods: This is a secondary analysis of 150 patients randomized into 5 groups, using different articulations: Charnley/Charnley Ogee for steel and conventional polyethylene (CPE), or Spectron EF/Reflection with either CPE or highly cross linked polyethylene (XLPE) cups, paired with heads made of either cobalt-chromium (CoCr) or oxidized zirconium (OxZr). All cups were cemented. Patients underwent repeated radiostereometric analysis (RSA) measurements for up to 10 years to assess wear and migration. Clinical outcome was assessed using Harris Hip Score (HHS). Results: After 10 years, the XLPE cups demonstrated low wear rates: 0.08 mm (95% confidence interval [CI] -0.11 to 0.26 mm) with CoCr heads and 0.06 mm (CI -0.14 to 0.26 mm) with OxZr heads, with a mean difference of 0.01 mm (CI -0.26 to 0.29 mm). In contrast, CPE cups exhibited significantly more wear: 1.35 mm (CI 1.16 to 1.55 mm) with CoCr heads and 1.68 mm (CI 1.44 to 1.92 mm) with OxZr heads, with a mean difference of 0.33 mm (CI 0.02 to 0.64 mm). The Charnley/Ogee group (CPE) showed PE wear of 0.34 mm (CI 0.12 to 0.56 mm). The CPE groups with OxZr and CoCr heads had 0.67 mm (CI 0.38 to 0.96 mm) and 0.35 mm (CI 0.09 to 0.61 mm) greater proximal migration respectively than the corresponding XLPE groups. HHS was similar across all groups. Conclusion: We found no significant advantage of OxZr over CoCr heads in reducing wear or migration. XLPE demonstrated a major reduction in wear as well as a reduction in cup migration compared with CPE. Charnley performed better than the other CPE cups in terms of PE wear and cup migration. No differences in clinical outcome were found.
Ceramic femoral head in highly cross-linked polyethylene cup. 5 year results of a randomized trial

Abstract:Background: Although few methods are used to estimate polyethylene liner wear from radiographs of total hip replacements, there is no consensus with regard to the accuracy of these methods. Highly cross-linked polyethylene (XLPE) is reported to have low rates of linear wear in the literature. However, there are not reports about wear rate of ceramic head in hxple cup at 5 years postoperatively, evaluated by Dorr method. We evaluated the wear rate of HXPLE, within the first five years after implantation using manual techniques of Dorr. Material and methods: We evaluated 143 patients (93 males and 50 females) with one design of uncemented acetabular component, a 40-mm (77 hips) or 44-mm (66 hips) ceramic femoral head and the annealed highly cross-linked crossfire polyethylene (X3) insert used in combination with Secur-Fit Advanced stems at a mean follow-up of 5 years. Selection of these femoral heads was based on several factors, including the perceived risk of dislocation, the outer diameter size of the acetabular component, and liner availability. Measurements of linear wear were performed by experienced surgeons with Dorr method and analyzed using the first-to-last method. Standard radiographs were used to detect periprosthetic osteolysis. Clinical records were used to determine all demographic data (age, height, weight, gender, months of follow-up, etc.). Results: For the entire cohort, the median linear wear rate was 0.0438 mm per year at 5 years and there was no any revision due to loosening, no liner fracture, and no patient with symptomatic corrosion. The head size was selected intraoperatively based on the size of the acetabular component and presumed risk of dislocation. There was no hip with pelvic or femoral osteolysis. The median linear wear rate was 0.0461 mm (±0.0183) and 0.0409mm (±0.0118), respectively, at head diameters of 40 and 44 mm. We found no association between femoral head size and the linear wear rate. This sentence can be excluded from here CONCLUSIONS: This acetabular component and HXLPEs with large ceramic heads had low rates of linear wear. Large ceramic femoral heads did not lead to liner fracture, loosening, or symptomatic trunnion corrosion in this patient population. However, we recommend longer clinical follow-up studies and caution in the routine use of larger ceramic femoral heads in other, younger patient populations. Abstract:Four hundred eighty-seven porous-coated anatomic (PCA) total arthroplasties were performed by the same surgeon between January 1982 and December 1989. Forty-three patients developed an effusion, pain, or decreased range of motion after a period of pain-free function. Average time to onset of symptoms was 4.5 years. Joint fluid aspirations were sterile and revealed the presence of high-density polyethylene crystals, best seen under a polarizing microscope. Thirty-three of these patients were arthroscopically evaluated. Extensive polyethylene wear and delamination were identified. Wear was most extensive on the medial tibial plateau. Patellar polyethylene wear was also identified. Substantial femoral component abrasions were present in areas where exposure of the tibial and patellar base plates had occurred. Diffuse granulomatous synovial tissue revealed an extensive foreign-body giant-cell reaction to polyethylene particles. In all patients, temporary symptomatic relief was obtained after arthroscopy. Thirty-two patients have, subsequently, had revision surgery. Intraoperative findings and biopsies at the time of revision confirmed the arthroscopic observations. Arthroscopy allowed the accurate diagnosis of polyethylene wear, provided temporary symptomatic relief, and facilitated preoperative revision planning. Polyethylene wear in PCA total knee replacements was related to patient characteristics (larger, younger, more active patients), nonconforming femotibial articular surfaces, thin polyethylene, heat-pressed polyethylene, and nonrigid mechanical attachment of polyethylene to metal base plate. Younger, more active, larger patients with total knee arthroplasties should be observed closely for evidence of polyethylene wear.

Abstract:We obtain a modular transformation for the function a(q, ζ,z )=Σ q n 2 +nm+m2 ζ n+m z n−m , namely, ϕ 3t ), and demonstrate how this unifies cubic modular transformations recently found by S. Cooper and their precursors established by J.M. Borwein and P.B. Bor- wein. Here and throughout the paper, unless otherwise stated, it is understood that the summation index or indices range over all integral values. We employ some properties of a(q, ζ,z) and its relation with the classical theta function (in Ramanujan's notation) f(a,b )=Σ a n(n+1)/2 b n(n−1)/2 simply es- tablished by the first author in an earlier publication. Also used are some sim- ple properties of f(a,b) and a well known modular transformation for f(a,b) as recorded by Ramanujan.
Minimum 10-Y Follow-Up of Vitamin-E-Diffused Highly Crosslinked Polyethylene Liners in Total Hip Arthroplasty: A Comparative Evaluation From a Prospective, International, Multicenter Cohort Study

Abstract:Background: Total hip arthroplasty (THA) is a proven and effective treatment for end-stage osteoarthritis. The success of THA is due in part to highly crosslinked polyethylene (XL) implants. In 2007, a new generation of polyethylene liners entered clinical use. The new liners infused polyethylene with vitamin E and became vitamin E infused polyethylene (EP). The EP liner was hypothesized to prevent the loss of mechanical properties caused by oxidation, extending the lifetime of the implant. This international prospective study aimed to quantify the clinical outcomes of a 977 patient cohort receiving EP and XL liners 10 years after surgery. Methods: The prospective cohort study began in 2007, including eight countries and 17 centers. The final cohort included 977 patients (EP liner: n = 520; XL liner: n = 457). Patients were followed preoperatively, postoperatively, and at 1, 3, 5, 7, and 10 years. Each follow-up visit involved clinical evaluation, radiography, and survey collection. The demographics and revisions were also recorded. Mann-Whitney U tests were used to evaluate statistical differences. Results: At 10 years following surgery, 534 patients were eligible for follow-up. Of those eligible, 352 patients returned for clinical evaluation (65.9% eligible; 36.0% overall). No statistical differences were found (P > 0.05) in the demographics of the followed-up cohort. The linear wear rates were 0.00338 mm/year for EP liners and 0.0236 mm/year for XL liners using individual regression (P < 0.0001). No significant difference was observed in wear rates using cohort regression. Similarly, surveys suggested no significant outcomes between the EP and XL liner cohorts. The overall incidence of revision was 2.3% for EP and 2.0% for XL liners. Conclusions: The EP liner demonstrated significantly lower wear than its XL counterparts at 10 years after implantation. Results are promising, as vitamin E polyethylene may improve the lifetime performance of THA implants and ensure that arthroplasty innovations reflect the changing patient population.

Abstract:Objective: To analyze the occurrence of osteolysis in total hip arthroplasty (THA) with highly cross-linked polyethylene prosthesis during a follow-up of more than 15 years. Methods: The clinical data of 84 patients (105 hips) treated with THA in the Affiliated Hospital of Kanazawa Medical University in Japan between June 2000 and April 2004 and met the selection criteria was retrospectively analyzed. There were 7 males and 77 females, aged from 41 to 75 years, with an average of 56.4 years. There were 94 hips with secondary hip osteoarthritis, 4 hips after pelvic osteotomy, 2 hips with primary hip osteoarthritis, 2 hips with traumatic hip osteoarthritis, 2 hips with osteonecrosis of the femoral head, and 1 hip with rheumatoid arthritis. According to Crowe classification, there were 79 hips of type Ⅰ, 19 hips of type Ⅱ, 6 hips of type Ⅲ, and 1 hip of type Ⅳ. The highly cross-linked polyethylene acetabular liner combined with a 26 mm zirconia femoral head were used in all patients. X-ray films were taken after operation to analyze the radiation transmission and osteolysis around the acetabular prosthesis. The vertical distance (the distance between the teardrop line at the lower edge of the pelvis and the perpendicular line of the hip rotation center), the horizontal distance (the horizontal distance between the hip rotation center and the interteardrop line and the vertical line at the lower edge of the teardrop), and the acetabular cup anteversion angle were measured at last follow-up. The acetabular and femoral osteolysis was analyzed by CT scan and three-dimensional multiplanar reconstruction (3D-MPR). Combined with X-ray film and CT results, osteolysis was evaluated according to the Narkbunnam score. Results: Deep vein thrombosis of lower limbs occurred in 2 cases. All patients were followed up 15-18 years, with an average of 15.9 years. One hip dislocation and 1 periprosthetic fracture occurred postoperatively, and no acetabular loosening or prosthetic lining ruptures occurred. Except for 1 patient who had a radiolucent line in the acetabulum after operation, the other 83 patients did not show any radiolucent line in the acetabulum or the femur. None of the patients underwent hip revision. X-ray films at last follow-up showed an acetabular cup anteversion angle of -10°-39°, with an average of 22°; a vertical distance of 3.5-47.1 mm, with an average of 24.6 mm; and a horizontal distance of 22.6-48.1 mm, with an average of 31.7 mm. There was no acetabular or femoral osteolysis in all patients on X-ray films and CT 3D-MPR images at last follow-up, and the Narkbunnam score was 0 in any region. Conclusion: Highly cross-linked polyethylene prosthesis does not increase the risk of long-term complications such as osteolysis after THA.

Abstract:HIP International, Ahead of Print. Introduction:Ceramic-on-ceramic bearings have been widely used since their introduction in the 1970s. First-generation ceramics have very high fracture (breakage) rates. To overcome this, in the 1990s, modular cotyloid insert designs were developed, consisting of a ceramic and polyethylene composite (sandwich-type) liner; however, high implant fracture rates were observed in the medium term. We aimed to estimate the cumulative incidences of revision surgery (implant failure) and implant failure due to fractures, survival rates (time-to-revision surgery and time-to-fracture), and the long-term clinical and radiological outcomes in our series.Methods:This was an observational, longitudinal, ambispective, single-centre study based on patients who underwent primary total hip arthroplasty (THA) using a sandwich-type liner (Cerasul), only available in our tertiary hospital between January 1999 and December 2002. Cumulative incidences were estimated and the 95% confidence interval (95% CI) was calculated. The Kaplan-Meier method was used to assess the time-to-revision surgery and time-to-fracture.Results:54 patients (49 men) were included, accounting for 59 sandwich-type linear implants. The mean (range) age was 47.4 (22–57) years. The primary THA indications were osteoarthritis (28 patients), osteonecrosis (14), childhood pathology sequelae (11), and inflammatory arthritis (6). The cumulative incidence of revision surgery by implants was 8.5% (5/59, 95% CI, 3.5–19.2%), 9.3% by patients (5/54, 95% CI, 4.0–19.9%), and 5.1% by implant fractures (3/59, 95%CI, 1.7–13.9%). The median (Interquartile Range, IQR) time-to-revision surgery was 158 (72.5–161) months, and the time to fracture was 182 (138–215) months. All primary THAs had good clinical and long-term survival outcomes. All implants had signs of solid fixation.Conclusions:After a 20-year follow-up period, the polyethylene-ceramic sandwich-type liner showed a long survival rate and low cumulative incidence of implant fracture; however, implant fractures remain the main complication. Orthopaedic surgeons should be aware that some patients still have this type of prosthesis and must be capable of responding quickly if a fracture occurs.

Abstract:Background: The literature has reported that supero-lateralization of the femoral head increases the rates of aseptic loosening and prosthesis revision. However, there are few reports on the influence of different hip center positions on liner wear with more than a 15-year follow-up period. Methods: From April 2000 to August 2003, 91 patients underwent 108 total hip arthroplasties using a highly cross-linked polyethylene liner combined with zirconia femoral head and cup components. Pelvic radiographs were used to assess the vertical and horizontal distances to the center of the hip and the amount of liner wear. Mean patient age at the time of surgery was 54 years (range, 33 to 73), and mean follow-up duration was 19 years (range, 18 to 21). Results: Average liner wear was 0.221 mm, with average annual wear of 0.012 mm/year. Mean vertical and horizontal distances for the hip center were 24.9 and 31.8 mm, respectively. There was no difference in linear wear between patients who had different hip center heights (<20, 20 to 30, and >30 mm), and quadrant partitioning showed no differences across the 4 quadrant zones. Conclusion: At a minimum of 18 years of follow-up in patients having developmental dysplasia of the hip who had different Crowe subtypes and different hip centers, elevated hip center and uncemented fixation techniques using a highly cross-linked polyethylene on ceramic components were associated with very low wear rates and excellent functional scores.

Abstract:Since 1977 we did implant all alumina (Al2O3) bearings total hip prostheses. A lot of data were documented concerning tissue reaction, in vivo wear behaviour, fractures, and clinical outcome. Ceramic materials retrieved at revision were analysed. In some cases, wear was as low as a few microns for a 15-year period in use. This is two thousand times less than a regular metal on polyethylene sliding couple. and 100 times less than a metal on metal prosthesis. Fracture mechanism is related to crack propagation into the material. During the first period, the fracture rate was in the range of 2%; it then dropped to less than 0.1 %. Few fractures could not be explained by technical or design mistakes. Clinical outcome: More than 4000 total hips in selected young and /or active patients were implanted. In a recently reported series of consecutive patients operated by P.Boutin during the year 1980, 118 hips in 106 patients were included. Mean age was 62 years. At the twenty-year follow-up evaluation, forty-five patients (fifty-one hips) were still alive and had not been revised, twenty-five patients (twenty-five hips) had undergone revision of either or both components, twenty-seven patients (thirty hips) had died from unrelated causes, and nine patients (twelve hips) were lost to follow-up. The mean Merle d’Aubigne hip score was 16.2 ± 1.8 at the latest follow-up. Survival of the cup at twenty years with revision for any reason as the end-point was 85.6 percent for cementless cups versus 61.2 % for cemented cups, respectively. Survival of the stem at twenty years with revision for any reason as the end-point was 84.9 % for cementless stems versus 87.3% for cemented stems. Wear of the prosthetic components was undetectable on plain radiographs. No fracture of the alumina socket or head was recorded. Another study concerned a more recent design of the socket which consisted in a metal back titanium alloy shell covered with a pure titanium mesh with an alumina liner. The nine year survival rate was 98.4% with revision for aseptic loosening as the end point. Conclusion This alumina on alumina bearing provides interesting results without any physical limitation specially in young and active patients.
