Abstract:OMEGA - Journal of Death and Dying, Ahead of Print. This study examines the experiences of suicidal ideation among Kenyan youth through a qualitative analysis of online narratives, a sample that comprised 15 young individuals aged 17–26 who shared their stories on online platforms. Employing Interpretative Phenomenological Analysis (IPA), the study highlights complex interactions of societal pressures, mental health challenges, and identity struggles that contribute to suicidal ideation. Key themes identified include the impact of cultural expectations, mental health stigma, isolation, and identity crises. By prioritizing the personal voices of the youth, the study sheds light on the deeply contextual nature of suicidal ideation and offers insights into the meanings individuals ascribe to their experiences. The findings advocate for culturally sensitive interventions and underscore the need to address mental health stigma, aiming to foster a supportive societal environment for Kenyan youth grappling with these issues.

Abstract:BACKGROUND: Severe childhood illnesses present a major public health challenge for Africa, which is aggravated by a suboptimal response to the child's health problems with reference to the health-seeking behaviour of the parents or guardians. We examined the health-seeking behaviour of parents at the Kenyan coast because understanding impediments to optimal health-seeking behaviour could greatly contribute to reducing the impact of severe illness on children's growth and development.METHODS AND RESULTS: Health-seeking behaviour, and the factors influencing this behaviour, were examined in two traditional communities. We held in-depth interviews with 53 mothers, fathers and caregivers from two rural clinics at the Kenyan Coast. Biomedical medicine (from health facilities and purchased over the counter) was found to be the most popular first point of treatment. However, traditional healing still plays a salient role in the health care within these two communities. Traditional healers were consulted for various reasons: a) attribution of causation of ill-health to supernatural sources, b) chronic illness (inability of modern medicine to cure the problem) and c) as prevention against possible ill-health. In developing an explanatory model of decision-making, we observed that this was a complex process involving consultation at various levels, with elders, but also between both parents, depending on the perceived nature and chronicity of the illness. However, it was reported that fathers were the ultimate decision makers in relation to decisions concerning where the child would be taken for treatment.CONCLUSIONS: Health systems need to see traditional healing as a complementary system in order to ensure adequate access to health care. Importantly, fathers also need to be addressed in intervention and education programs.

Abstract:Abstract The study examined rural folks’ perception of suicide drivers in rural communities of Delta state, Nigeria and the resultant implications for societal and agricultural security. The objectives of the study are to: describe the socio-economic characteristics of the respondents, ascertain the perceived suicide rates in rural households; identify the main causes (or drivers) of suicide in rural communities, ascertain the means of committing suicide, and determine respondents’ perception of the effect of suicide on agricultural production and their families. A multi-stage sampling procedure was used in composing 351 respondents who furnished information on suicide situation in the study area. Results indicated some cases of suicide in the area; several perceived suicide drivers were identified by the respondents, and the respondents identified some perceived deleterious effects of suicide as illness among family members, depression of surviving members, loss of income and output of agricultural production and scattering of family of suicide victim. Logistic regression of relationship between perceived social economic characteristics and suicidal death indicated that marital status, education, family size, family income, membership of association and extension contact were significant in preventing suicide. From the findings, it was recommended that all stakeholders including the government, private sector, religion organizations, health and welfare institutions, rural communities, and individuals have crucial role to play in curbing suicidal deaths in rural communities.
Socioeconomic position and suicidal behaviour in rural Sri Lanka: a prospective cohort study of 168,000+ people

Abstract:Purpose: Lower socioeconomic position (SEP) is associated with an increased risk of suicidal behaviour in high income countries, but this association is not established in low- and middle-income countries (LMIC). Methods: We investigated the association of SEP with suicidal behaviour in a prospective cohort study of 168,771 Sri Lankans followed up for episodes of attempted suicide and suicide. SEP data were collected at baseline at the household and individual level at the start of the follow-up period. We used multilevel Poisson regression models to investigate the association of SEP at community, household and individual levels with attempted suicide/suicide. Results: Lower levels of asset ownership [IRR (95% CI) suicide 1.74 (0.92, 3.28); attempted suicide 1.67 (1.40, 2.00)] and education [suicide 3.16 (1.06, 9.45); attempted suicide 2.51 (1.70, 3.72)] were associated with an increased risk of suicidal behaviour. The association of these measures of SEP and attempted suicide was stronger in men than women. Individuals living in deprived areas [1.42 (1.16, 1.73)] and in households with a young female head of household [1.41 (1.04, 1.93)] or a temporary foreign migrant [1.47 (1.28, 1.68)] had an elevated risk of attempted suicide. Farmers and daily wage labourers had nearly a doubling in risk of attempted suicide compared to other occupations. Conclusions: Improved employment opportunities, welfare and mental health support services, as well as problem-solving skills development, may help support individuals with poorer education, farmers, daily wage labourers, individuals in young female-headed households and temporary foreign migrant households.

Abstract:Introduction: Over 800 000 people die due to suicide each year and suicide presents a huge psychological, economic and social burden for individuals, communities and countries as a whole. Low-income and middle-income countries (LMICs) are disproportionately affected by suicide. The strongest risk factor for suicide is a previous suicide attempt, and other types of self-harm have been found to be robust predictors of suicidal behaviour. An approach that brings together multiple sectors, including education, labour, business, law, politics and the media is crucial to tackling suicide and self-harm. The WHO highlights that evaluations of the knowledge and attitudes that priority groups, not only healthcare staff, have of mental health and suicidal behaviour are key to suicide prevention strategies. The aim of this systematic review is to examine the knowledge, attitudes and experiences different stakeholders in LMICs have of self-harm and suicide. Methods and analysis: MEDLINE, Embase, PsycINFO, CINAHL, BNI, Social Sciences and Cochrane Library will be searched. Reviewers working independently of each other will screen search results, select studies for inclusion, extract and check extracted data, and rate the quality of the studies using the Strengthening the Reporting of Observational studies in Epidemiology and Critical Appraisals Skills Programme checklists. In anticipation of heterogeneity, a narrative synthesis of quantitative studies will be provided and metaethnography will be used to synthesise qualitative studies. Ethics and dissemination: Ethical approval is not required. A report will be provided for the funding body, and the systematic review will be submitted for publication in a high-impact, peer-reviewed, open access journal. Results will also be disseminated at conferences, seminars, congresses and symposia, and to relevant stakeholders. Prospero registration number: CRD42019135323.

Abstract:Introduction Suicide is a leading cause of death, making suicide prevention a major public health priority. Increasing understanding of factors influencing suicidal behavior is paramount. Previous research has implicated psychological closeness, characterized by perceptions of how close/distant or attached/detached one feels to a particular object, as a cognitive factor that influences suicidal behavior. However, a better understanding of how psychological closeness to suicide methods is conceptualized by relevant populations is needed to improve its assessment and understand how it may confer risk for suicide. Methods The goal was to refine the conceptualization of psychological closeness to suicide methods by incorporating feedback from relevant populations. We conducted 30 interviews with those primarily identifying as having lived experience of suicide (n = 10), clinicians who work with suicidal patients (n = 11), and suicide researchers (n = 9). A rigorous thematic approach using NVivo software was used to uncover common themes. Results Primary themes included familiarity, comfort, and attachment, with one emerging theme of symbolism. We define these themes, provide context to their meaning, and share exemplary quotes across diverse participants. Conclusion We consider clinical, research, and policy implications from an interdisciplinary lens and discuss the strengths and limitations of this study.

Abstract:Introduction Suicidal behaviour is a major public health concern in Africa and a cause of premature mortality. The availability of community epidemiological data in Ethiopia is limited. This study assessed the prevalence of suicidal behaviour and its associated factors in Jimma Town, Southwest Ethiopia. Methods Using the Suicide Behaviour Questionnaire-Revised (SBQ-R), a community-based cross-sectional survey was conducted between September and November 2021. Multistage sampling was used to screen 636 participants for suicidal behaviour. The association between suicidal behaviour and other variables was explored using binary and multivariable logistic regression analyses. Results The overall estimated prevalence of suicidal behaviour of the respondents in the study was 7.9% (95% CI 5.9% to 10.3%), using the SBQ-R score (>7). One-third of the individuals with lifetime attempts encountered stigma. Multivariable logistic regression models indicated that being female (AOR 2.81, 95% CI 1.48 to 5.31), having depression (AOR 6.9, 95% CI 1.98 to 24.57), family history of mental illness (AOR 4.11, 95% CI 1.38 to 12.19), poor social support (AOR 3.86, 95% CI 1.23 to 12.15) and good coping efficacy (AOR 0.91, 95% CI 0.88 to 0.94))were significantly associated for suicidal behaviour with p value less than 0.05. Conclusion The overall prevalence of suicidal behaviour was relatively higher in this population than in other studies done in Ethiopia. In addition, the current study indicates the presence of suicide stigma, which further influences poor health-seeking behaviours. The factors significantly associated with suicidal behaviour in this study included being female, having a family history of mental illness, poor social support, poor coping self-efficacy and depressive symptoms. Findings warrant the attention of policy-makers in early screening of depression, developing preventive measures targeting risk factors and enhancing access to mental healthcare services.

Abstract:Although global rates of suicide have dropped in the last 30 years, youth in low- and middle-income countries (LMICs) continue to be highly represented in suicide statistics yet underrepresented in research. In this review we present the epidemiology of suicide, suicidal ideation, and suicide attempts among youth in LMICs. We also describe population-level (attitudes toward suicide, socioeconomic, and societal factors) and individual-level clinical and psychosocial risk factors, highlighting specific considerations pertaining to youth in LMICs. These specific considerations in risk factors within this population can inform how multi-level prevention strategies may be targeted to meet their specific needs. Prevention and intervention strategies relying on the stepped-care framework focusing on population-, community-, and individual level targets while considering locally- and culturally relevant practices are key in LMICs. In addition, systemic approaches favoring school-based and family-based interventions are important among youth. Cross-culturally adapted multimodal prevention strategies targeting the heterogeneity that exists in healthcare systems, suicide rates, and risk factors in these countries should be accorded a high priority to reduce the burden of suicide among youth in LMICs.

Abstract:ABSTRACT Background Individuals with intellectual disability or autism exhibit suicidal behaviours at an equal or greater rate than the general population, yet little is known about associated risk factors. This study explores suicidality in these populations, focusing on their understanding of suicide and death and perceptions of their direct support staff regarding their suicidality. Method Semi‐structured interviews from 23 suicidal and non‐suicidal participants and their support staff were analysed qualitatively to extract common themes and compare between groups. Results A breadth of suicidal behaviours was documented. Generally, suicidal participants demonstrated a more elaborate understanding of suicide and death than non‐suicidal participants. While staff often acknowledged never having discussed the topic with their client, most assumed they understood death accurately. Conclusion Results suggest interactions between conceptions of death and suicidal behaviour and a need for discussion around death and suicide in clinical settings. Implications for future research and clinical practice are discussed.

Abstract:Background: Childhood bullying has been classified as a major public health concern by WHO, with negative effects on the health education and social outcomes of both bullies and victims. There is no current Kenyan data on the prevalence of face-to-face bullying and cyberbullying co-occurring in the same cohort of youth and how they are associated with different aspects of suicidality and socio-demographic characteristics. This study aims to fill these gaps in the Kenyan situation so as to inform current policy and practice. Methodology: This cross-sectional study involved 2,652 students from ten secondary schools in Kenya, selected from three regions representing different levels of public funded schools and socioeconomic spaces. The outcome variable was derived from the questionnaire which asked students questions related to self-harm, suicide thoughts, plans, and attempts. Predictor variables were based on response on experience of bullying in school, out of school, at home, and cyberbullying. Other variables such as gender, age, family background, and class were also collected from the self-reported questions. Data were analyzed using SPSS version 25, with descriptive summary statistics and chi-square tests used to examine variables, and logistic regression analysis used to determine the associations between suicidality and experience of bullying. Results: The mean age was 16.13 years. More than half of the participants were male, with the largest proportion living in rural areas. Face-to-face bullying was more prevalent than cyberbullying, with 82% of participants experiencing bullying and 68% experiencing it almost daily in the past six months. Both face-to-face bullying and cyberbullying were associated with suicidal thoughts, plans, and attempts. Predictors of suicidal attempts included being bullied outside of school and being a victim of group bullying, while being bullied every day and being bullied by adult men were predictors of suicidal attempts in cyberbullying. Conclusion: There is a high prevalence of face-to-face bullying both in and outside schools. There is also a high prevalence of cyberbullying. Both face-to-face and cyberbullying are associated with suicidality in Kenyan high school students.
Explaining Suicide Among Indian Women: Applying the Cultural Theory of Suicide to Indian Survivors of Gender-Based Violence Reporting Suicidal Ideation

Abstract:Journal of Interpersonal Violence, Ahead of Print. Indian women account for 36.6% of suicide-related deaths worldwide and gender-based violence (GBV) is a key social determinant. The cultural theory of suicide (CTS), which synthesizes risk factors and explanations of suicide among racial/ethnic minorities, posits four tenets: idioms of distress, cultural sanctions, and social discord. Our study applied the CTS to Indian women from slums reporting GBV to explore (1) culturally relevant risk pathways towards suicidal ideation using qualitative analyses, and test (2) the association between idioms of distress and suicidal ideation. 112 women from urban slums were recruited and 99 completed surveys. A subset were administered qualitative interviews. Aim 1 explored the CTS framework among participants describing suicidal ideation in qualitative interviews [n = 18]; Aim 2 explored if idioms of distress severity was associated with suicidal ideation through an ANCOVA [N = 99]. Idioms of distress such as 'tension' indicated suicidal ideation. Communities did not sanction suicidal ideation, leading to secrecy regarding disclosure. Women in 'love marriages' (versus arranged marriages) reported minority stress. Social discord heightened suicidal thoughts. Results of the ANCOVA confirmed that women reporting suicidal ideation had higher idioms of distress severity (M = 28.56, SD = 6.37), compared to women who did not (M = 21.77, SD = 6.07), F(1, 96) = 28.58, p < .001 (ηp2 = .23). Our study empirically validates the CTS among Indian GBV survivors. Culturally responsive suicide prevention can include assessing idioms of distress, improving family support, and educating to reduce stigma and enhance help-seeking.

Abstract:Background: In the new global socioeconomic development, suicidal deaths have increased which is progressively gaining more attention in the field of psychiatry, social work, psychology, and public health research. The present study aimed at examining the evidence for the attitudes towards suicide and risk factors for suicide attempts among university students in South Korea and suggested significant recommendations for future prevention programs and research. Method: A study was conducted by reviewing recent publications and significant research journals published since 2000 related to the study objective, themes, and drawn broader perspectives and implications. The recent journal articles from PubMed, Global Health journal database, Google Scholar, MEDLINE, Academic Journals Database, WHO, and PsychInfo data sources were included. The study key terms were "suicide", "attitudes", "risk factors", "university students", & "South Korea". Results: The negative attitudes towards self, confusion of meaning in life, anxiety and stress about academic achievement, family problems, hopelessness, depression, and bipolar disorder were the main factors. Limitations: We mainly included peer review journals in our study, but address the few relevant books and reports also. Conclusions: The authors discussed important implications for future actions, including conducting relevant studies and regular collaborative discussions among responsible stakeholders.

Abstract:In myeloid malignancies, mutations have occurred in epigenetic regulator genes, including Ten-Eleven-Translocation 2 (TET2). TET2 is an enzyme that catalyzes the conversion of 5-methylcytosine into 5-hydroxymethylcytosine (5-hmC) which is a key intermediate for oxidative DNA demethylation. We analyzed the in vivo phenotype of TET2 failure using Ayu17-449 (TET2(trap/trap)) mice created by the gene-trap method in which TET2 mRNA levels were decreased to about 20% of the level in wild-type (WT) mice. In TET2(trap/trap) mice the levels of 5-hmC in genomic DNA from bone marrow (BM) cells were decreased in comparison to WT mice. TET2(trap/trap) mice were born at an expected Mendelian frequency but died at a high rate by postnatal day 3, indicating TET2 to be essential for survival. In analysis of the hematopoietic system, transplantation of TET2(trap/trap), but not WT fetal liver cells, led to mild myeloid hyperplasia and splenomegaly in WT recipient mice, but no onsets of lethal hematological malignancies were observed during a follow-up period of 12 months. TET2 knockdown led to an increased serial replating capacity of BM cells in vitro and increased hematopoietic stem cell (HSC) self-renewal in vivo in competitive repopulation and serial transplantation assays. These data indicate that TET2 has a critical role in survival and HSC homeostasis.
Correlates for the severity of suicidal risk in participants with common mental disorders with comorbid chronic medical conditions in rural primary healthcare settings in India

Abstract:Context: Suicide is on the rise in low- and middle-income countries (LMICs), including India. There are limited studies assessing factors linked to the severity of suicidal risk in patients with depression and comorbid chronic medical disorders in primary healthcare (PHC) settings. Aim: This study examines factors linked to suicidal risk among participants in a cluster randomized controlled trial of collaborative care intervention (Healthier Options through Empowerment (HOPE Study)). Settings and design: The setting was at 49 PHC in the rural Ramanagara District of Karnataka State in southern India. Study eligibility criteria included being ≥30 years with at least mild depression or generalized anxiety disorder and at least one medical condition (cardiovascular disorder or type 2 diabetes mellitus). Methods and material: The severity of suicidal risk at baseline was assessed using the Mini International Neuropsychiatric Interview (MINI), and other measures included the severity of depression (Patient Health Questionnaire-9-items (PHQ-9)), the severity of anxiety (Generalized Anxiety Disorder Scale-7-items (GAD-7)), disability, social support, quality of life, number of comorbid chronic medical illnesses, and body mass index (BMI). Statistical analysis used: Chi-square tests and independent-samples t-tests were used to compare the demographic and clinical characteristics of the no-low and mod-high suicidal risk groups. Logistic regression analysis was used to identify correlates associated with the mod-high suicidal risk group. Results: Mod-high suicidal risk was significantly positively associated with the severity of depression and disability and significantly negatively associated with social support. Conclusion: The severity of depression, higher disability scores, and lower social support were found to be independent correlates of mod-high suicidal risk. Screening, managing depression, and facilitating social support for patients with chronic medical illness in PHC settings may reduce suicidal risk.

Suicide Among Rural Primary School Learners in Zimbabwe: An Epidemic in Need of Mitigation

Abstract:The alarming rise of suicide cases among rural primary school students in Zimbabwe has become a major concern, as there is a significant lack of research on the root causes of suicide and effective prevention methods. This article aimed to address that gap by exploring Zimbabwe rural primary learners' perspectives on the factors driving them to engage in suicide and possible interventions by the Government of Zimbabwe and other stakeholders in order to curb this troubling trend. This qualitative study, drawing upon socio-cultural theory, held focus groups with 12 learners from the Mahoto rural area in Masvingo North Constituency. The aim was to understand the lived experiences and factors that contributed to suicidal thoughts among primary school learners. Through thematic analysis, key patterns were identified. This research on suicide among rural primary school students in Zimbabwe filled a crucial gap in the literature and held both theoretical and practical implications. The unique context of suicide within this demographic in a rural setting warrants further investigation. By shedding light on the factors contributing to this epidemic, the study can inform targeted interventions and mitigation strategies, ultimately leading to the protection and well-being of vulnerable young learners in Zimbabwe's rural areas. Early research indicates that many rural primary school students who lack access to counseling may be at risk of academic difficulties and even premature death by suicide. This highlights the need for Zimbabwe's Government through Ministry of Primary and Secondary Education to urgently deploy professional counsellors to provide professional counselling to learners. By amplifying the voices of marginalised rural learners who have been directly impacted by the suicide epidemic, this study aimed to develop practical solutions to prevent further tragedies and improve the mental health of Zimbabwean rural learners.

Abstract:Objective Suicide is a crucial public health concern. However, the interactions between bio psychosocial vulnerabilities and stressors leading to deliberate self-harm behavior remain unexplored, especially in the Indian context. This study examined the experiences leading to self-harm behavior among people who presented to emergency departments with suicidal attempts. Methods In this mixed-methods study, we enrolled 44 patients who presented with self-harm behavior at three tertiary health care facilities between October and December 2019. To collect quantitative data, we employed standardized tools: General Health Questionnaire (GHQ-28), General Help-Seeking Questionnaire, Mini International Neuropsychiatric Interview, and the Brief Resilience Scale. Further, we conducted semi-structured interviews to qualitatively explore participants' life experiences and other risk factors. Qualitative analyses were performed using thematic analysis and quantitative descriptive and inferential statistics were performed using STATA software. Results The mean age of subjects were 29.8 years. The mean suicidality score for the patients was 26 (±8.7). In univariate analysis, depression and anxiety were positively associated with suicidality. While help-seeking behavior and resilience were negatively associated with suicidality. Qualitative results were centered on three major themes; life stressors, family related stressors, and social support-related vulnerabilities. The subjects' lived experiences were introduced in the backdrop of the interplay of vulnerabilities and stressors. Conclusion The biopsychosocial vulnerabilities remain dormant until it is activated by life stressors resulting in severe self-harm behaviors. Mental health team-driven assertive engagement, positive coping, and social support interventions would help prevent reattempts in people with self-harm behaviors.

Abstract:This qualitative study aims to explore the underlying causes of suicidal tendencies among male educators in Zimbabwe's rural regions and identify the strengths of current interventions targeting this issue. A purposive sampling technique was employed to select 15 knowledgeable participants who possessed relevant experiences and expertise. The study employed focus group discussions to generate data, which was subsequently analysed using content thematic analysis. The research questions addressed in this study were two-fold: (1) What are the underlying causes, including work-related, psychosocial, and personal factors, that contribute to the emergence of suicidal tendencies among male educators in Zimbabwe's rural regions? (2) What are the strengths of the current interventions targeting suicidal tendencies among male educators in these regions? The analysis of the data revealed two key findings. Firstly, the study identified multiple causal factors contributing to the emergence of suicidal tendencies among male educators in Zimbabwe's rural regions. These factors encompassed work-related challenges such as heavy workload, insufficient resources, and inadequate support systems. Additionally, psychosocial factors such as stigma, social isolation, and emotional distress were identified. Personal factors like financial difficulties and relationship problems were also found to contribute to suicidal tendencies. Secondly, the study identified several strengths within the current interventions targeting suicidal tendencies among male educators in Zimbabwe's rural regions. Notably, the provision of mental health support services, including counselling and awareness campaigns, was found to be a valuable aspect of the existing interventions. Moreover, the study highlighted the significance of community involvement and collaboration between educational institutions, healthcare providers, and relevant stakeholders in addressing this critical issue. Based on the findings, two key recommendations emerge. Firstly, it is crucial to enhance the existing support systems for male educators in rural regions, focusing on addressing work-related challenges and providing adequate resources and support. This may include workload management strategies, professional development opportunities, and the establishment of comprehensive mental health support networks. Secondly, there is a need for increased awareness and de-stigmatization of mental health issues among male educators. This involves implementing targeted awareness campaigns to promote mental well-being, fostering a supportive and inclusive work environment, and encouraging open dialogue on mental health concerns.
