Abstract:Abstract Purpose: Melanoma brain metastases (MBM) and leptomeningeal melanoma metastases (LMM) are two different manifestations of melanoma CNS metastasis. Here, we used single-cell RNA sequencing (scRNA-seq) to define the immune landscape of MBM, LMM, and melanoma skin metastases. Experimental Design: scRNA-seq was undertaken on 43 patient specimens, including 8 skin metastases, 14 MBM, and 19 serial LMM specimens. Detailed cell type curation was performed, the immune landscapes were mapped, and key results were validated by IHC and flow cytometry. Association analyses were undertaken to identify immune cell subsets correlated with overall survival. Results: The LMM microenvironment was characterized by an immune-suppressed T-cell landscape distinct from that of brain and skin metastases. An LMM patient with long-term survival demonstrated an immune repertoire distinct from that of poor survivors and more similar to normal cerebrospinal fluid (CSF). Upon response to PD-1 therapy, this extreme responder showed increased levels of T cells and dendritic cells in their CSF, whereas poor survivors showed little improvement in their T-cell responses. In MBM patients, therapy led to increased immune infiltrate, with similar T-cell transcriptional diversity noted between skin metastases and MBM. A correlation analysis across the entire immune landscape identified the presence of a rare population of dendritic cells (DC3) that was associated with increased overall survival and positively regulated the immune environment through modulation of activated T cells and MHC expression. Conclusions: Our study provides the first atlas of two distinct sites of melanoma CNS metastases and defines the immune cell landscape that underlies the biology of this devastating disease.

Abstract:Abstract We interrogated the microenvironment of 43 clinical samples from melanoma skin, brain (MBM) and leptomeningeal metastases (LMM) using single-cell RNA-seq analysis to determine how therapeutic intervention shaped the immune environment and affected patient survival. LMM is a poorly-characterized, devastating complication of late-stage disease, typically refractory to treatment and associated with dismal survival time. Analysis of serial specimens over the course of therapy demonstrated reductions in melanoma cells and macrophages, coupled with increased levels of T cells and dendritic cells in the CSF of a rare extraordinary responder, whereas typical poor survivors showed no improvement in T cell responses. In MBM patients, both targeted therapy and immunotherapy was associated with increased immune infiltrate. Treatment with targeted therapy was associated with an enrichment of CD8 T cells, while immunotherapy was associated with a more diverse lymphocyte landscape and higher numbers of antibody-producing cells. These findings were confirmed by multiplex-IF staining of patient specimens and using an immune-competent mouse model of MBM. Interestingly, a history of prior radiation therapy was associated with a diminished myeloid compartment. Although immune infiltrate was significantly lower in the brain compared to skin tumors, the phenotypic make-up of the lymphocyte compartment was quite similar, suggesting that the immune cells may have trafficked from the periphery to the brain post-therapy. Correlation analysis across the entire immune landscape identified the presence of a rare, novel population of dendritic cells (DC3s) to be correlated with increased overall survival, regardless of disease site/treatment. The presence of DC3s positively regulated the immune environment of both patient samples and preclinical melanoma models through modulation of activated T cells and MHC expression in the tumor. Overall, we present the first ever comprehensive single-cell atlas of the tumor microenvironment in melanoma CNS metastases in response to therapy.

Abstract:Purpose: Acral melanoma is a rare subtype of melanoma that arises on the non-hair-bearing skin of the palms, soles, and nail beds. In this study, we used single-cell RNA sequencing (scRNAseq) to map the transcriptional landscape of acral melanoma and identify novel immunotherapeutic targets. Experimental Design: We performed scRNA-seq on nine clinical specimens (five primary, four metastases) of acral melanoma. Detailed cell type curation was performed, the immune landscapes were mapped, and key results were validated by analysis of The Cancer Genome Atlas (TCGA) and single-cell datasets. Cell-cell interactions were inferred and compared with those in nonacral cutaneous melanoma. Results: Multiple phenotypic subsets of T cells, natural killer (NK) cells, B cells, macrophages, and dendritic cells with varying levels of activation/exhaustion were identified. A comparison between primary and metastatic acral melanoma identified gene signatures associated with changes in immune responses and metabolism. Acral melanoma was characterized by a lower overall immune infiltrate, fewer effector CD8 T cells and NK cells, and a near-complete absence of gd T cells compared with nonacral cutaneous melanomas. Immune cells associated with acral melanoma exhibited expression of multiple check-points including PD-1, LAG-3, CTLA-4, V-domain immunoglo-bin suppressor of T cell activation (VISTA), TIGIT, and the Adenosine A2A receptor (ADORA2). VISTA was expressed in 58.3% of myeloid cells and TIGIT was expressed in 22.3% of T/NK cells. Conclusions: Acral melanoma has a suppressed immune envi-ronment compared with that of cutaneous melanoma from non-acral skin. Expression of multiple, therapeutically tractable immune checkpoints were observed, offering new options for clinical translation.

Abstract:Immune checkpoint blockade (ICB) targeting CTLA-4 or the PD1/PD-L1 axis have become a standard of care therapy for most patients with metastatic melanoma. However, for patients with resistance to front-line ICB, especially those with BRAFWT tumors who received combination therapy, subsequent therapeutic options remain limited. Thus, understanding how ICB alters the tumor ecosystem, and thereby may create new vulnerabilities, is key to understanding effective therapies for ICB-experienced patients. Here, enabled through innovations in biospecimen research, we performed multi-modal single-cell RNA, T-cell receptor and single-cell spatial transcriptomics, coupled with whole-genome sequencing (WGS) of 42 melanoma patient biopsies. This cohort includes 7 patients with paired before- and on-/post-treatment biopsies, and spanned treatment-naïve metastatic cutaneous melanoma patients (n=10), on-ICB (aPD-1, n=7; aCTLA-4, n=1; combination therapy, n=2; total, n=10), and post-ICB (aPD-1, n=10; aCTLA-4, n=5; combination therapy, n=6; L19IL2+L16TNF, n=1; total, n=22). This cohort further includes 6 rare samples of mucosal melanoma (oral, n=4; rectal, n=2). In total, we profiled >320,000 transcriptomes, including >240,000 malignant and ~80,000 non-malignant cells, and 14,714 matched TCRs. We used a series of novel analytical approaches, including KINOMO to mitigate biases introduced by differences in gene dosage, and novel artificial intelligence frameworks, to discover differentially expressed genes and gene programs associated with treatment exposures and treatment sites. Compared to untreated specimens, there is an increase in antigen presentation (Wilcoxon rank-sum test, P=0) and reduction in lineage expression (Wilcoxon rank-sum test, P=0) in ICB-experienced tumors. Furthermore, mucosal melanomas harbor unique gene regulatory networks characterized by expression of immune evasive programs (e.g. SOX4; MAST, P=0) and exhibit lower expression of oxidative phosphorylation (Wilcoxon rank-sum test, P=0). Together, these data indicate a high degree of cellular adaption and plasticity. Through integration of single-cell, spatial and WGS, we further identify cancer clones defined by their aneuploidy patterns that confer geographically restricted immune evasion with a distinctly cold tumor microenvironment (TME), despite global T cell activation and clonal expansion. This suggest significant intralesional heterogeneity with respect to response to ICB, which may be explained by cancer cell-intrinsic factors rather than insufficient T cell responses to ICB. This work provides initial insights into spatio-temporal changes induced by ICB in cancer cells and the TME in a relatively large patient cohort. Our results indicate that functional and spatial variability exists in these tumors and may provide avenues for novel therapeutic development in ICB-experienced patients. Abstract:Background Tumor-targeted therapy causes impressive tumor regression, but the emergence of resistance limits long-term survival benefits in patients. Little information is available on the role of the myeloid cell network, especially dendritic cells (DC) during tumor-targeted therapy. Methods Here, we investigated therapy-mediated immunological alterations in the tumor microenvironment (TME) and tumor-draining lymph nodes (LN) in the D4M.3A preclinical melanoma mouse model (harboring the V-Raf murine sarcoma viral oncogene homolog B (BRAF) V600E mutation) by using high-dimensional multicolor flow cytometry in combination with multiplex immunohistochemistry. This was complemented with RNA sequencing and cytokine quantification to characterize the immune status of the tumors. The importance of T cells during tumor-targeted therapy was investigated by depleting CD4 + or CD8 + T cells in tumor-bearing mice. Tumor antigen-specific T-cell responses were characterized by performing in vivo T-cell proliferation assays and the contribution of conventional type 1 DC (cDC1) to T-cell immunity during tumor-targeted therapy was assessed using Batf3 −/− mice lacking cDC1. Results Our findings reveal that BRAF-inhibitor therapy increased tumor immunogenicity, reflected by an upregulation of genes associated with immune activation. The T cell-inflamed TME contained higher numbers of activated cDC1 and cDC2 but also inflammatory CCR2-expressing monocytes. At the same time, tumor-targeted therapy enhanced the frequency of migratory, activated DC subsets in tumor-draining LN. Even more, we identified a cDC2 population expressing the Fc gamma receptor I (FcγRI)/CD64 in tumors and LN that displayed high levels of CD40 and CCR7 indicating involvement in T cell-mediated tumor immunity. The importance of cDC2 is underlined by just a partial loss of therapy response in a cDC1-deficient mouse model. Both CD4 + and CD8 + T cells were essential for therapy response as their respective depletion impaired therapy success. On resistance development, the tumors reverted to an immunologically inert state with a loss of DC and inflammatory monocytes together with the accumulation of regulatory T cells. Moreover, tumor antigen-specific CD8 + T cells were compromised in proliferation and interferon-γ-production. Conclusion Our results give novel insights into the remodeling of the myeloid landscape by tumor-targeted therapy. We demonstrate that the transient immunogenic tumor milieu contains more activated DC. This knowledge has important implications for the development of future combinatorial therapies.

Abstract:BRAF mutated colon cancer presents with poor survival, and the treatment strategies are controversial. The tumor microenvironment, which plays a key role in tumorigenesis as well as responses to treatments, of this subtype is largely unknown. In the present study, we analyzed the differences of immune microenvironments between BRAF mutated and BRAF wild-type colon cancer utilizing datasets from The Cancer Genome Atlas and Gene Expression Omnibus and confirmed the findings by tissue specimens of patients. We found that BRAF mutated colon cancer had more stromal cells, more immune cell infiltration, and lower tumor purity. Many immunotherapeutic targets, including PD-1, PD-L1, CTLA-4, LAG-3, and TIM-3, were highly expressed in BRAF mutated patients. BRAF mutation was also correlated with higher proportions of neutrophils and macrophages M1, and lower proportions of plasma cells, dendritic cells resting, and T cells CD4 naïve. In conclusion, our study demonstrates a different pattern of the immune microenvironment in BRAF mutated colon cancer and provides insights into the future use of checkpoint inhibitors in this subgroup of patients.

Abstract:Abstract Brain metastases (BrM) remain an intractable, deadly complication for advanced melanoma patients and efficient therapeutic strategies are desperately needed. The tumor microenvironment (TME) plays an important role in response to therapy. However, studies addressing the contribution of the TME to therapy efficacy for BrM are lacking, mostly due to limited access to human samples and scarcity of appropriate preclinical models. Here, we describe two novel isogenic immunocompetent BrM models generated by intracardiac injection of UV-induced mouse melanoma cell lines, representative of mutant-RAS human melanoma subtypes. We used these models to test immune checkpoint blockade (ICB) therapy and to interrogate the role of the TME in therapeutic efficacy. To evaluate response, we developed and applied a new machine-learning method to quantify metastatic burden. We investigated the TME by high-parametric flow cytometry and single-cell RNA sequencing (scRNA-seq). We showed that the models have distinct metastatic behaviors, with BR1 being mostly brain tropic and BR3 displaying widespread metastases. Notably, BR1 BrM were sensitive to ICB with a better response to anti-PD-L1/anti-CTLA-4 combination therapy as compared to monotherapies. In contrast, BR3 BrM were resistant to both mono- and combination therapies. Interestingly, we found that ICB efficacy on extracranial BR3 metastases is organ-dependent. Characterization of the BrM immune microenvironment before and after treatment revealed dramatic differences between the models. Untreated BR1 BrM showed significant recruitment of T cells, dendritic cells, and natural killer cells, while neutrophils were enriched in untreated ICB-resistant BR3 BrM. Moreover, we uncovered phenotypically distinct microglia populations exclusively present in ICB-sensitive BR1 BrM that positively correlated with T cell infiltration. Consistent with this finding, scRNA-seq showed upregulation of genes encoding for T cell-attracting chemokines and antigen presentation uniquely in the BR1-associated microglia. Post-treatment analysis of the brain TME highlighted beneficial changes induced by ICB in the responsive BR1 model, including increased recruitment of CD8 T cells with an activated phenotype, while a mild recruitment of exhausted T cells was observed in the resistant BR3 model. Altogether, our data emphasize the importance of interrogating the BrM TME to understand therapeutic response. Our unique BrM models, mirroring the diversity of ICB response observed in patients, provide a robust platform for the much-needed mechanistic studies to optimize BrM therapy. Deciphering the contribution of the newly identified BR1 BrM-associated microglia to ICB efficacy will be crucial to the identification of novel therapeutic targets. Abstract:Abstract Background: Immune checkpoint blockade (ICB) has revolutionized the treatment and prognosis of various cancers, including melanoma. Nevertheless, intrinsic or acquired resistance is common, and the interactions between tumor-intrinsic features and the microenvironment underlying these resistances remain unclear for most patients. Methods: In this study, we developed a framework to analyze response and resistance, both intrinsic and acquired, via tumor-intrinsic programs, immune features, and tumor-stromal-immune interactions in a standardized, uniformly processed, and deeply clinically annotated cohort of metastatic melanoma patients (n=61) treated with ICB as part of the NCI funded human tumor atlas network (HTAN) initiative. From the same tumor samples (pre-treatment (n=33) and post-progression (n=28)), we conducted bulk whole-exome sequencing (WES), single-nucleus RNA sequencing, and for a subset of the samples high-resolution spatial imaging (including protein mIHC, CODEX, and transcriptomics MERFISH). Standardized processing and data pipelines allowed for integration of genomic, transcriptomic, and spatial features to elucidate shared tumor and microenvironmental states and their relationships with resistance. Results: Our preliminary analysis on a subset of 28 samples identified an enrichment of B cells, plasma cells, and T follicular helper-like cells in the tumor microenvironment (TME) of patients sensitive to treatment. The TME of non-responders was predominantly characterized by macrophage/monocyte cell populations. Post-treatment responder samples showed an enrichment of CD4+ T cells. Genomic heterogeneity, assessed through WES, was confirmed as a feature of intrinsic resistance across different ICB treatment settings, while ploidy was associated with treatment response in pre-treatment, ipilimumab-naïve patients treated with PD-1 inhibitors. Comparing the immune compartments of samples with high and low heterogeneity revealed NK cell enrichment in samples with a high proportion of subclonal mutations. Conclusions and Future Directions: Overall, our work provides a high-resolution understanding of the tumor-stromal-immune interaction in metastatic melanoma, shedding light on the factors contributing to therapy response and resistance, and contributing a repository of data from deeply clinically annotated, multimodally characterized and uniformly processed patient samples. Ongoing spatial analysis will further enhance our understanding of these compartments at a spatial level. The results of our study have the potential to guide more personalized and effective treatment strategies for metastatic melanoma in the future. Abstract:BACKGROUNDB-raf proto-oncogene (BRAF)-mutant melanoma brain metastases (BRAF MBM) are associated with poor prognosis. Amidst our large institutional experience treating patients with BRAF MBM with multimodality therapy, our aim was to identify disease and treatment-related prognostic features, accounting for longitudinal sequence of events, to inform clinical decision-making. METHODSWe retrospectively reviewed electronic health records of 100 patients with primary cutaneous melanoma diagnosed with BRAF MBM. Demographic, disease, and clinical factors including treatment sequencing were tabulated. Descriptive statistics were used to summarize the study population and non-parametric tests were used to evaluate differences across groups. A multivariable Cox regression identified factors associated with overall survival (OS) following BRAF MBM diagnosis. RESULTSMedian follow-up was 22 months and median OS was 33.5 months. Patients received an average [standard deviation (SD)] of 3.36 [1.06] unique therapies. After adjustment for covariates, greater number of brain metastases at initial diagnosis (hazard ratio (HR) 2.3 [1.14-4.62], p=0.01 for 3-10 metastases and HR 2.54 [1.01-6.36], p=0.04 for >10 metastases, respectively), receipt of combination IO and targeted therapy prior to BRAF MBM diagnosis (HR 2.91 [1.08-7.86], p=0.03), and receipt of no unique subsequent therapy after initial management strategy (HR 3.89 [1.48-10.16], p=0.005) were significantly associated with worse OS. CONCLUSIONSDespite heterogeneity in individual treatment sequences, patients with greater intracranial disease burden and those receiving combination targeted and immunotherapy prior to BRAF MBM diagnoses had the worst outcomes. Dynamic changes in therapy resistance may inform optimal treatment strategies for individuals with BRAF MBM.

Characterization of a 5-hydroxytryptamine receptor in mouse neuroblastoma N18TG2 cells.

Abstract:Cyclic AMP formation was found to increase in mouse neuroblastoma N18TG2 cells exposed to 5-hydroxytryptamine (5-HT). This response was concentration-dependent with an EC50 value of 0.22 microM. Tryptamine and other tryptamine-related compounds were also agonists in this assay with a rank order of potency of 5-methoxytryptamine > 5-HT > tryptamine > 2-methyl-5-HT > 5-carboxamidotryptamine >> alpha-methyl-5-HT. (D)-Lysergic acid diethylamide and 2-bromo-lysergic acid diethylamide were partial agonists in this system with maximal responses of 44 and 34%, respectively, compared to 5-HT. 5-HT-stimulated cyclic AMP formation was inhibited by clozapine, mianserin and methiothepin with pA2 values of 6.6, 6.5 and 6.4, respectively. Radioligand binding studies using [125I]iodolysergic acid diethylamide revealed a binding site present at a density of 10.4 fmol/mg of protein, with an affinity for the ligand of 1.18 nM. In competition studies this binding site displayed a pharmacology similar to that defined in studies of cyclic AMP formation. The pharmacological profile of this receptor, characterized by both radioligand binding and functional coupling to adenylyl cyclase, does not correspond to that of any of the currently classified subtypes of 5-HT receptor, but is similar to the 5-HT receptor cloned recently from rat striatum and referred to as the 5-HT6 receptor. The N18TG2 cell line represents a useful model system in which this novel 5-HT receptor may be characterized fully.
Unsupervised Analysis Reveals the Involvement of Key Immune Response Genes and the Matrisome in Resistance to BRAF and MEK Inhibitors in Melanoma

Abstract:Melanoma tumors exhibit a wide range of heterogeneity in genomics even with shared mutations in the MAPK pathway, including BRAF mutations. Consistently, adaptive drug resistance to BRAF inhibitors and/or BRAF plus MEK inhibitors also exhibits a wide range of heterogeneous responses, which poses an obstacle for discovering common genes and pathways that can be used in clinic for overcoming drug resistance. This study objectively analyzed two sets of previously published tumor genomics data comparing pre-treated melanoma tumors and BRAFi- and/or MEKi-resistant tumors. Heterogeneity in response to BRAFi and BRAFi/MEKi was evident because the pre-treated tumors and resistant tumors did not exhibit a tendency of clustering together. Differentially expressed gene (DEG) analysis revealed eight genes and two related enriched signature gene sets (matrisome and matrisome-associated signature gene sets) shared by both sets of data. The matrisome was closely related to the tumor microenvironment and immune response, and five out of the eight shared genes were also related to immune response. The PLXNC1 gene links the shared gene set and the enriched signature gene sets as it presented in all analysis results. As the PLXNC1 gene was up-regulated in the resistant tumors, we validated the up-regulation of this gene in a laboratory using vemurafenib-resistant cell lines. Given its role in promoting inflammation, this study suggests that resistant tumors exhibit an inflammatory tumor microenvironment. The involvement of the matrisome and the specific set of immune genes identified in this study may provide new opportunities for developing future therapeutic methods.

Abstract:Abstract Despite recent advances in mutation- targeted and immune therapies for melanoma, prolonged patient survival is still hampered by therapeutic resistance. For MAPKi, acquired resistance is common. For example, a majority of V600E/K mutant BRAF metastatic melanomas treated with MAPK inhibitor (MAPKi) will acquire resistance within 1-2 years of the treatment initiation, despite BRAF inhibitors (BRAFi) and their combination with MEK inhibitors (BRAFi+MEKi) being remarkably active initially. Our work and others' have shown that immune modulation by MAPKi may contribute to the in vivo anti-tumor activities of this oncogene-targeted approach. Importantly, my own recent study has shown that around half of all acquired or late MAPKi resistant melanoma display profound CD8 T-cell depletion and exhaustion, which were tightly associated with reduced antigen-presentation. Immunotherapies using antibodies against the T cell checkpoints, CTLA-4 and PD-1, have been shown to enhance the ability of endogenous T cells to destroy malignant cells in a variety of cancers, including melanoma. When successful, these therapies can result durable tumor control. However, the majority of patients do not respond to such therapies. Multiple studies including ours have delineated multiple factors limiting the efficacy of single agent anti-CTLA-4 or anti-PD-1 treatments, including the lack of tumor infiltration by T cells, insensitivity to IFN-gamma signaling and the activation of immune-suppressive processes represented by Innate anti-PD-1 RESistance (IPRES) signatures. Functionally, IPRES signatures include the downstream genes of potent immune-suppressive pathways and wound-healing processes linked to suppression and exclusion of T cells (e.g., angiogenesis, extracellular matrix reorganization, mesenchymal/invasive transition). Unsupervised gene signature enrichment clustering revealed four subclasses with combinations of high/low enrichment scores of interferon (IFN)/T-cell inflammation and of IPRES related signatures in both our dataset and the TCGA melanoma. The cellular processes represented by the four subsets significantly resemble those induced during wound healing where: 1) the IFN-high stage matches the early, acute inflammation stage and 2) the IPRES-high stage matches the stage of neovascularization, extracellular matrix remodeling and suppression of inflammation. Importantly, the patients from different subclasses have statistically different overall survival where the IFN-high/IPRES-low group showed the best survival and IFN-low/IPRES-low group was the worst. In murine mutant BRAF melanoma YUMM1.7, regressing/responding tumors on the 6th day of BRAFi treatment showed IFN-high/IPRES-low pattern with a significant infiltration of T cells and other immune cell populations into the tumor. The residual tumors on the 15th day showed IFN-high/IPRES-high pattern and showed further immune infiltration. By the time of acquired resistance to BRAFi, however, all tumors were in the IFN-low/ IPRES-high stage and there was no or minimal immune infiltration in the tumor. We observed that MAPKi-induced an IFN-high/IPRES-high state in the residual melanoma tumors from patients undergoing MAPKi treatment. Given the immune suppressive nature of IPRES, we hypothesized that its activation may cause the observed intratumoral immune (T-cell, in particular) dysfunction and depletion. Indeed, combination of MAPKi with immune checkpoint blockade (ICB) therapy targeting the PD-1 only works when the ICB is administered before the IFN-high/IPRES-high stage. Also intriguing is the finding that IPRES was induced in vitro along with a tumor immune phenotype switch MAPKi resistant melanoma cell lines, suggesting, a tumor cell-intrinsic process. We found that IPRES signatures include potent immune-suppressive pathways like the VEGF and TGFB pathways. TGFB signaling is known to regulate T-cell inflammation and promote T-cell tolerance by downregulating TCR signaling. TGFB ligands (TGFB1/2/3 and ACTIVIN-A/B) are normally secreted by immune cells including T-cells, M2-macrophages and endothelial cells during the inflammation resolution phase of wound healing. In our data and the TCGA melanoma, we observed higher mRNA and protein expression of TGFB ligands in the IPRES-high melanoma subgroups and in tumors treated with MAPKi. We postulate that high levels of TGFB ligands, from tumor and stromal sources, could counteract the T-cell anti-tumor immunity. Of interest, a study has shown some efficacy of targeting the TGFB pathway by fresolimumab (a human monoclonal antibody against TGFB1-3) in melanoma (Clinical Trial ID: NCT0035646017). Another study currently combines nivolumab (anti-PD-1) with galunisertib (a TGFBR type I/II small molecule inhibitor) in multiple solid tumors (NCT02423343). In relation to the VEGF pathway, we observed an anti-correlation between the mRNA levels of VEGFA and CD8A in both our MAPKi and the TCGA melanoma cohorts. We also observed that VEGFA mRNA and its protein level were expressed higher in the non-IFN-/T-cell inflamed subclasses. Several publications had shown that tumors that were non-responsive to anti-PD-1 have higher VEGFA levels associated with either PTEN loss or beta-catenin activation. Tumor-secreted VEGFA was reported to inhibit T-cell diapedesis from the vasculature, and thus infiltration into the tumor bed. Our observations are thus in line with such a role of VEGFA in blocking T-cell homing into the tumor. Consistently, a recent report showed that anti-VEGFA antibody pre-treatment followed by co-treatment with aPD-L1 improved treatment responses among renal cell carcinoma (RCC) patients. The authors reported increased tumor-specific T-cell trafficking into the tumor with the dual treatment. An earlier study combining anti-CTLA-4 with anti-VEGFA also showed similar result. Thus, anti-VEGFA combined with anti-PD-1/L1 may have a role in addressing non-T-cell inflamed melanoma with innate/acquired aPD-1 resistance or acquired MAPKi-resistance. Thus, given the clinical availability of both TGFB and VEGFA antagonists, we are testing the efficacy of combinatorial MAPKi and anti-TGFB/VEGFA to improve T-cell mediated, antitumor immunity through suppression of IPRES. Finally, our recent study showed that MAPKi-treated, regressing melanoma already evolve epigenetic alterations, temporally far ahead of any clinical evidence of disease progression. However, it was not clear if the observed transition into an IPRES-high, immune-suppressive state in these melanoma has any epigenetic basis. In parallel studies in cell lines, we characterized two groups of melanomas derived from continuous treatment with BRAFi or BRAFi+MEKi: MAPK-redundant (Rr) vs. MAPK-reactivated (Ra). The Ra lines could be resensitized to additional MAPK inhibitors while the Rr lines could not (due to MAPK-independent growth and surival). Intriguingly, only the Rr-lines showed enrichments of the IPRES signatures. Importantly, we noted that chromatin enhancer regions (marked by significant H3K27ac peaks) of the Rr lines were correlated with more invasive melanomas. The same invasive melanoma enhancer profile can also be observed in the patient-derived MAPKi-on-treatment tumors. By analyzing the actual mRNA expression changes of the genes proximal to the invasive melanoma specific H3K27ac peaks, we found that the upregulated genes were enriched with IPRES-related processes. This implies that the tumor cell-intrinsic up-expression of IPRES-related genes is regulated at the epigenetic level. In summary, we anticipate that our study will lead to novel insights into the determinants and/or bottlenecks of effective and durable anti-tumor responses by tumor-specific T-cells in order to pave the way to a new combination therapies in human melanoma patients. Abstract:Complex tumor microenvironmental (TME) features influence the outcome of cancer immunotherapy (IO). Here we perform immunogenomic analyses on 67 intratumor sub-regions of a PD-1 inhibitor-resistant melanoma tumor and 2 additional metastases arising over 8 years, to characterize TME interactions. We identify spatially distinct evolution of copy number alterations influencing local immune composition. Sub-regions with chromosome 7 gain display a relative lack of leukocyte infiltrate but evidence of neutrophil activation, recapitulated in The Cancer Genome Atlas (TCGA) samples, and associated with lack of response to IO across three clinical cohorts. Whether neutrophil activation represents cause or consequence of local tumor necrosis requires further study. Analyses of T-cell clonotypes reveal the presence of recurrent priming events manifesting in a dominant T-cell clonotype over many years. Our findings highlight the links between marked levels of genomic and immune heterogeneity within the physical space of a tumor, with implications for biomarker evaluation and immunotherapy response.

Abstract:Targeted therapy directed against oncogenic BRAF mutations and immune checkpoint inhibitors have transformed melanoma therapy over the past decade and prominently improved patient outcomes. However, not all patients will respond to targeted therapy or immunotherapy and many relapse after initially responding to treatment. This unmet need presents two major challenges. First, can we elucidate novel oncogenic drivers to provide new therapeutic targets? Second, can we identify patients who are most likely to respond to current therapeutic strategies in order to both more accurately select populations and avoid undue drug exposure in patients unlikely to respond? In an effort to evaluate the current state of the field with respect to these questions, we provide an overview of some common oncogenic mutations in patients with metastatic melanoma and ongoing efforts to therapeutically target these populations, as well as a discussion of biomarkers for response to immune checkpoint inhibitors—including tumor programmed death ligand 1 expression and the future use of neoantigens as a means of truly personalized therapy. This information is becoming important in treatment decision making and provides the framework for a treatment algorithm based on the current landscape in metastatic melanoma.

Tumor immune microenvironment and immunotherapy efficacy in BRAF mutation non-small-cell lung cancer

Abstract:Previous small-size studies reported BRAF-mutated NSCLC patients have comparable sensitivity to immune checkpoint inhibitors (ICIs). However, how BRAF mutation affects the tumor immune microenvironment (TIME) is unknown. We performed Nanostring-panel RNA sequencing to evaluate TIME in 57 BRAF mutated and wild-type (WT) NSCLC specimens (cohort A). The efficacy of ICI monotherapy or combined therapies was determined in 417 patients with WT and BRAF mutated NSCLC (cohort B). We found that BRAF-mutant tumors had similar ratios of CD8+ T cells to Tregs, the balance of cytotoxicity gene expression signatures and immune suppressive features, and similar ICI-response-related biomarkers to WT NSCLC. A similar TIME pattern was observed between the BRAF V600E and Non-V600E subgroups of NSCLC. The further retrospective study confirmed that treatment with ICI monotherapy or combined therapies resulted in similar overall survival (OS) (HR: 0.85; 95% CI, 0.56 to 1.30; p = 0.47) and progress-free survival (PFS) (HR: 1.02; 95% CI, 0.72 to 1.44; p = 0.91) of patients with WT (n = 358) and BRAF mutant (n = 59) NSCLC. Similarly, both patients with BRAF V600E or Non-V600E NSCLC had similar responses to immunotherapy. Our findings support that BRAF mutation did not modulate TIME in NSCLC and therapeutic responses to ICIs. Patients with NSCLC harboring BRAF mutation should not be denied treatment with ICIs.

Abstract:The development of BRAF and MEK inhibitors (BRAFis and MEKis) and immune checkpoint inhibitors have changed the management of advanced stage melanoma and improved the outcomes of patients with this malignancy. However, both therapeutic approaches have limitations, including a limited duration of benefit in subsets of BRAF-mutant melanoma patients treated with targeted therapy and a lower overall response rate without a clear predictive biomarker in patients treated with checkpoint inhibitors. Preclinical and translational data have shown that BRAFis and MEKis alter the tumor microenvironment to make it more amenable to immunotherapy and have provided the scientific rationale for combing BRAFis and MEKis with immunotherapy. In this review, the initial studies demonstrating the impact of BRAFis and MEKis on the expression of melanoma differentiation antigens, T-cell infiltration, and the balance of immune stimulatory and immune suppressive cells and cytokines are addressed. Preclinical work on the combination of targeted therapy with BRAFis and MEKis with immunotherapy are reviewed, highlighting improved tumor responses in mouse models of BRAF-mutated melanoma treated with combinatorial strategies. Lastly, data from early clinical trials of combined targeted therapy and immunotherapy are discussed, focusing on response rates and toxicities.

Abstract:Intrapatient heterogeneity among different metastatic sites contributes to immunotherapy (ICi) and targeted therapy resistance. Determining the evolution of metastatic melanoma lesions that develop within the same patient could help elucidate adaptive mechanisms that drive tumor progression during therapy. To assess how metastatic melanoma persists through immunotherapy and targeted inhibitor treatments, we utilized samples from a single patient who had received anti-CTLA4, BRAFi/MEKi, and BRAFi/MEKi+CDK4/6i treatments during the LOGIC2 clinical trial. Eight short-term cultures were established from different metastatic site biopsies at chronological time points throughout the patient's treatment and subsequent post-mortem resection of the bone, ovary, and brain metastatic sites. While the initial treatment naïve metastatic tumor in the breast was BRAFV600E mutant, NRASQ61 mutations were acquired after treatment with CDK4/6i in the bone and ovary tumors. Tumors that persist through treatment and colonize distant organs adapt to and rely on organ-specific microenvironments. To assess treatment- and organ-specific adaptations, single-cell RNA sequencing was performed on seven of the short-term cultures to identify subpopulations that persist through each treatment. We analyzed samples and subpopulations of each culture to determine differences in their transcriptional cell states. The regulon transcriptional network of each sample and subpopulation was evaluated to determine site- and treatment-specific transcriptional regulation differences. In addition, we identified the treatment naïve culture is split into two major subpopulations by therapy resistance features which are further made up of six distinct subpopulations with heterogeneous regulatory networks. We determined the longitudinal evolution of subpopulations in response to treatment and organ site to evaluate how the pressure of therapies and the microenvironment influence metastatic progression. We further evaluated site-specific resistance mechanisms and detected increased HER3 expression in the melanoma brain metastasis (MBM) sample. Treatment with Neuregulin 1, a ligand for the HER3 receptor that is abundant in brain tissue, partially rescued the growth of MBM cells and restored MAPK and AKT-mTOR signaling in the presence of BRAFi/MEKi. Targeting the NRG1-HER3 signaling axis could have therapeutic potential in melanoma patients with active brain metastases. Additionally, we evaluated treatment-specific resistance mechanisms and identified high Brevican (BCAN) expression in an early emerging subpopulation that arises after ICi in this patient and in others. BCAN is implicated in both ICi- and cross-resistance. We will functionally investigate whether targeting BCAN sensitizes cells to ICi. Abstract:Therapy targeting the BRAF-MEK cascade created a treatment revolution for patients with BRAF mutant advanced melanoma. Unfortunately, 80% patients treated will progress by 5 years follow-up. Thus, it is imperative we study mechanisms of melanoma progression and therapeutic resistance. We created a scRNA (single cell RNA) atlas of 128,230 cells from 18 tumors across the treatment spectrum, discovering melanoma cells clustered strongly by transcriptome profiles of patients of origins. Our cell-level investigation revealed gains of 1q and 7q as likely early clonal events in metastatic melanomas. By comparing patient tumors and their derivative cell lines, we observed that PD1 responsive tumor fraction disappears when cells are propagated in vitro We further established three anti-BRAF-MEK treatment resistant cell lines using three BRAF mutant tumors. ALDOA and PGK1 were found to be highly expressed in treatment resistant cell populations and metformin was effective in targeting the resistant cells. Our study suggests that the investigation of patient tumors and their derivative lines is essential for understanding disease progression, treatment response and resistance.
