Abstract:Cardiometabolic diseases have substantially increased in China in the past 20 years and blood pressure is a primary modifiable risk factor. Using data from the China Health and Nutrition Survey, we examine blood pressure trends in China from 1991 to 2009, with a concentration on age cohorts and urbanicity. Very large values of blood pressure are of interest, so we model the conditional quantile functions of systolic and diastolic blood pressure. This allows the covariate effects in the middle of the distribution to vary from those in the upper tail, the focal point of our analysis. We join the distributions of systolic and diastolic blood pressure using a copula, which permits the relationships between the covariates and the two responses to share information and enables probabilistic statements about systolic and diastolic blood pressure jointly. Our copula maintains the marginal distributions of the group quantile effects while accounting for within-subject dependence, enabling inference at the population and subject levels. Our population-level regression effects change across quantile level, year and blood pressure type, providing a rich environment for inference. To our knowledge, this is the first quantile function model to explicitly model within-subject autocorrelation and is the first quantile function approach that simultaneously models multivariate conditional response. We find that the association between high blood pressure and living in an urban area has evolved from positive to negative, with the strongest changes occurring in the upper tail. The increase in urbanization over the last twenty years coupled with the transition from the positive association between urbanization and blood pressure in earlier years to a more uniform association with urbanization suggests increasing blood pressure over time throughout China, even in less urbanized areas. Abstract:Although the fractional polynomials (FPs) can act as a concise and accurate formula for examining smooth relationships between response and predictors, modelling conditional mean functions observes the partial view of a distribution of response variable, as distributions of many response variables such as blood pressure (BP) measures are typically skew. Conditional quantile functions with FPs provide a comprehensive relationship between the response variable and its predictors, such as median and extremely high-BP measures that may be often required in practical data analysis generally. To the best of our knowledge, this is new in the literature. Therefore, in this article, we develop and employ Bayesian variable selection with quantile-dependent prior for the FP model to propose a Bayesian variable selection with parametric non-linear quantile regression model. The objective is to examine a non-linear relationship between BP measures and their risk factors across median and upper quantile levels using data extracted from the 2007 to 2008 National Health and Nutrition Examination Survey (NHANES). The variable selection in the model analysis identified that the non-linear terms of continuous variables (body mass index, age), and categorical variables (ethnicity, gender, and marital status) were selected as important predictors in the model across all quantile levels.

Abstract:Improving health worldwide will require rigorous quantification of population-level trends in health status. However, global-level surveys are not available, forcing researchers to rely on fragmentary country-specific data of varying quality. We present a Bayesian model that systematically combines disparate data to make country-, region- and global-level estimates of time trends in important health indicators. The model allows for time and age nonlinearity, and it borrows strength in time, age, covariates, and within and across regional country clusters to make estimates where data are sparse. The Bayesian approach allows us to account for uncertainty from the various aspects of missingness as well as sampling and parameter uncertainty. MCMC sampling allows for inference in a high-dimensional, constrained parameter space, while providing posterior draws that allow straightforward inference on the wide variety of functionals of interest. Here we use blood pressure as an example health metric. High blood pressure is the leading risk factor for cardiovascular disease, the leading cause of death worldwide. The results highlight a risk transition, with decreasing blood pressure in high-income regions and increasing levels in many lower-income regions.

Abstract:This paper deals with the following hypertension diagnoses: essential hypertension and five types of secondary hypertension: fibrodysplasic renal artery stenosis, atheromatous renal artery stenosis, Conn's syndrome, renal cystic disease, and pheochromocytoma. Only blood pressures, general information and general biochemical data are taken into account. Nineteen items were finally selected, by statistical investigation of experimental data, as being both discriminative and independent. The marginal density distributions of every item, and then joint density distribution functions were determined within six types of hypertension. The frequency of a given hypertension type within the hypertensive patients was used as prior probability of this state. The loss matrix was established by medical arguments. The expected loss corresponding to six possible decisions could thus be calculated for all cases. Both the ratio of secondary hypertensions that could be inferred from our set of data (not including the results of complementary tests) and that of correct "essential" hypertension diagnosis proved to be satisfactory.

Abstract:With childhood hypertension emerging as a global public health concern, understanding its associated factors is crucial. This study investigated the prevalence and associated factors of hypertension among Chinese children. This cross-sectional investigation was conducted in Pinghu, Zhejiang province, involving 2,373 children aged 8–14 years from 12 schools. Anthropometric measurements were taken by trained staff. Blood pressure (BP) was measured in three separate occasions, with an interval of at least two weeks. Childhood hypertension was defined as systolic blood pressure (SBP) and/or diastolic blood pressure (DBP) ≥ age-, sex-, and height-specific 95th percentile, across all three visits. A self-administered questionnaire was utilized to collect demographic, socioeconomic, health behavioral, and parental information at the first visit of BP measurement. Abstract:Hypertension is a major global health concern, with deaths attributed to the condition expected to increase to 1.57 million by 2034, particularly affecting low-and-middle-income countries such as those within sub-Saharan Africa. Non-communicable diseases, with hypertension as a core contributor, account for 74.36% of global deaths. The burden of hypertension in sub-Saharan Africa is significant, with an estimated 10–20 million people currently affected. Systemic barriers, such as fragmented health services and socioeconomic inequalities, coupled with shifts in greater salt-intake, ultra-processed foods, more sedentary lifestyles, and overburdened healthcare services, have exacerbated elevated blood pressure and poorer management of people living with hypertension in sub-Saharan Africa. Most public health strategies focus on detecting, treating, and controlling hypertension through lifestyle modifications and medication. However, evidence suggests only 10% of population hypertension is well managed. This indicates a growing need to shift towards preventative efforts. Precision prevention, a tailored health intervention approach utilising individual and population-specific factors – genetic, environmental, and social determinants – offers a potential alternative. Precision prevention aims to deliver the right preventative measures to the right population at the right time, promising to enhance intervention efficiency and health outcomes. This paper highlights various intervention levers, including environmental, biological, and behavioural modifications, examines case studies from high-income countries, and discusses the potential for implementing precision prevention in South Africa. While precision prevention shows promise, we also discuss the significant barriers to its implementation in LMICs such as those within sub-Saharan Africa.

Abstract:BACKGROUND AND OBJECTIVE: Hypertension, a global burden, is associated with several risk factors and can be treated by lifestyle modifications and medications. Prediction and early diagnosis is important to prevent related health complications. The objective is to construct and compare predictive models to identify individuals at high risk of developing hypertension without the need of invasive clinical procedures.METHODS: This is a cross-sectional study using 987 records of Qataris and long-term residents aged 18+ years from Qatar Biobank. Percentages were used to summarize data and chi-square tests to assess associations. Predictive models of hypertension were constructed and compared using three supervised machine learning algorithms: decision tree, random forest, and logistics regression using 5-fold cross-validation. The performance of algorithms was assessed using accuracy, positive predictive value (PPV), sensitivity, F-measure, and area under the receiver operating characteristic curve (AUC). Stata and Weka were used for analysis.RESULTS: Age, gender, education level, employment, tobacco use, physical activity, adequate consumption of fruits and vegetables, abdominal obesity, history of diabetes, history of high cholesterol, and mother's history high blood pressure were important predictors of hypertension. All algorithms showed more or less similar performances: Random forest (accuracy = 82.1%, PPV = 81.4%, sensitivity = 82.1%), logistic regression (accuracy = 81.1%, PPV = 80.1%, sensitivity = 81.1%) and decision tree (accuracy = 82.1%, PPV = 81.2%, sensitivity = 82.1%. In terms of AUC, compared to logistic regression, while random forest performed similarly, decision tree had a significantly lower discrimination ability (p-value<0.05) with AUC's equal to 85.0, 86.9, and 79.9, respectively.CONCLUSIONS: Machine learning provides the chance of having a rapid predictive model using non-invasive predictors to screen for hypertension. Future research should consider improving the predictive accuracy of models in larger general populations, including more important predictors and using a variety of algorithms.

Abstract:Background Hypertension is the most common modifiable risk factor for cardiovascular diseases in South Asia. Machine learning (ML) models have been shown to outperform clinical risk predictions compared to statistical methods, but studies using ML to predict hypertension at the population level are lacking. This study used ML approaches in a dataset of three South Asian countries to predict hypertension and its associated factors and compared the model's performances. Methods We conducted a retrospective study using ML analyses to detect hypertension using population-based surveys. We created a single dataset by harmonizing individual-level data from the most recent nationally representative Demographic and Health Survey in Bangladesh, Nepal, and India. The variables included blood pressure (BP), sociodemographic and economic factors, height, weight, hemoglobin, and random blood glucose. Hypertension was defined based on JNC-7 criteria. We applied six common ML-based classifiers: decision tree (DT), random forest (RF), gradient boosting machine (GBM), extreme gradient boosting (XGBoost), logistic regression (LR), and linear discriminant analysis (LDA) to predict hypertension and its risk factors. Results Of the 8,18,603 participants, 82,748 (10.11%) had hypertension. ML models showed that significant factors for hypertension were age and BMI. Ever measured BP, education, taking medicine to lower BP, and doctor's perception of high BP was also significant but comparatively lower than age and BMI. XGBoost, GBM, LR, and LDA showed the highest accuracy score of 90%, RF and DT achieved 89 and 83%, respectively, to predict hypertension. DT achieved the precision value of 91%, and the rest performed with 90%. XGBoost, GBM, LR, and LDA achieved a recall value of 100%, RF scored 99%, and DT scored 90%. In F1-score, XGBoost, GBM, LR, and LDA scored 95%, while RF scored 94%, and DT scored 90%. All the algorithms performed with good and small log loss values <6%. Conclusion ML models performed well to predict hypertension and its associated factors in South Asians. When employed on an open-source platform, these models are scalable to millions of people and might help individuals self-screen for hypertension at an early stage. Future studies incorporating biochemical markers are needed to improve the ML algorithms and evaluate them in real life.

Epidemiology of Hypertension Stages in Two Countries in Sub-Sahara Africa: Factors Associated with Hypertension Stages

Abstract:Studies using the revised hypertension classification are needed to better understand epidemiology of hypertension across full distribution. The sociodemographic, biological, and health behavior characteristics associated with different stages of hypertension in Ghana and South Africa (SA) were studied using global ageing and adult health (SAGE), WAVE 1 dataset. Blood pressure was assessed for a total of 7545 respondents, 2980 from SA and 4565 from Ghana. Hypertension was defined using JNC7 blood pressure classification considering previous diagnosis and treatment. Multivariate multinomial logistic regression analysis using Stata version 12 statistical software was done to identify independent predictors. The weighted prevalence of prehypertension and hypertension in Ghana was 30.7% and 42.4%, respectively, and that of SA was 29.4% and 46%, respectively, showing high burden. After adjusting for the independent variables, only age (OR = 1.32, 95% CI: 1.14–1.53), income (OR = 1.9, 95% CI: 1.04–3.47), and BMI (OR = 1.16, 95% CI: 1.1–1.22) remained independent predictors for stage 1 hypertension in Ghana, while, for SA, age (OR = 2.27, 95% CI: 1.53–3.36), sex (OR = 0.28, 95% CI: 0.08–1), and BMI (OR = 1.15, 95% CI: 1.07–1.25) were found to be independent predictors of stage 1 hypertension. Healthy lifestyle changes and policy measures are needed to promptly address these predictors.

Modelling the Impact of Obesity Reduction on the Prevalence of Hypertension in India: A Discrete-Event Microsimulation Approach

Abstract:Obesity is one of the most significant risk factors of non-communicable diseases, disability, and premature death. Due to its profound impact on health, researchers have started classifying it as a disease rather than a mere abnormality. India, following the global trend, is experiencing a surge in obesity prevalence, posing a critical research question about the potential impact of obesity reduction on NCD incidence and related disorders. This study employs discrete-event dynamic microsimulation modelling to investigate how changes in BMI distribution in early years of life can influence the prevalence of hypertension, one of the most prevalent diseases in India. The microsimulation modelling approach enables the simulation of individual-level real-life behaviors and interactions within a given population. The model simulated the lives of 100,000 individuals aged 20 over the next 50 years till age 70. Baseline characteristics, prevalence rates, and transition probabilities were derived from diverse data sources, including Census 2011, the National Family Health Survey - V (NFHS-5), and the Longitudinal Aging Study in India (LASI, 2017-18). The study explores the impact of two scenarios on hypertension prevalence: (i) a one-unit reduction in mean BMI level at baseline, and (ii) a one-unit reduction in the standard deviation of BMI distribution at baseline. Results indicate that a one-unit reduction in mean BMI level at baseline could lead to a 5% reduction in hypertension prevalence at age 70, while a one-unit reduction in the standard deviation of BMI distribution at baseline could result in a 7.5% reduction. These findings underscore the importance of targeting children and adolescents with elevated BMI values to mitigate the later-life prevalence of hypertension. Additionally, the study highlights the significance of promoting the use of microsimulation modelling in health research in the Indian context.

Infant-Feeding Practices of Low-Income Vietnamese American Women

Abstract:Healthy People 2010 breastfeeding goals include 50% exclusive breastfeeding at 6 months. In California, US, Santa Clara County Women, Infants, and Children (SCCWIC) data indicated Asian participants had low (5.6%) 6-month breastfeeding rates. To examine infant-feeding practices, Vietnamese breastfeeding peer counselors surveyed 133 Vietnamese SCCWIC participants (≥ 18 years old, nonpregnant, < 28 months postpartum) using a structured questionnaire regarding intentions, attitudes, and subjective norms toward breastfeeding. Results indicated that 75% initiated breastfeeding in the hospital (33% exclusively, 42% partially), and 25% exclusively formula fed. At survey time, 49% had terminated breastfeeding (average duration 4.4 months). Feeding intentions during pregnancy predicted feeding method used (P < .001). Most prenatal advice was from SCCWIC employees, friends, and doctors who supported breastfeeding. Most postnatal advice was from nurses who supported breastfeeding and bottle-feeding equally. Stronger control beliefs, peer counseling, and education were correlated with breastfeeding (P < .001). Hypertension in sub-Saharan Africa: the current profile, recent advances, gaps, and priorities

Abstract:Recent global and regional reports consistently confirm the high and increasing prevalence of hypertension in sub-Saharan Africa (SSA), with poor detection, treatment, and control rates. This narrative review summarises the burden of hypertension in SSA and recent findings from community-based hypertension management strategies. We further outline prominent risk factors according to recent data and associated underlying mechanisms for hypertension development. An extensive review of literature showed that most countries have reported on the prevalence of hypertension during 2017–2023, despite limitations linked to the lack of nationally representative studies, heterogeneity of sampling and data collection methods. Task-shifting approaches that assign roles to model patients and community health workers reported improved linkage to healthcare services and adherence to medication, with inconsistent findings on blood pressure (BP)-lowering effects over time. The regularly reported risk factors include unhealthy diet, sedentary lifestyle, increased adiposity and underweight, ageing, level of education, and/or income as well as psychosocial factors. Newer data on the pathophysiological mechanisms leading to hypertension and potential areas of intervention are reported from children and adults and include, among others, salt-handling and volume overload, endothelial function, BP dipping patterns and the role of human immunodeficiency virus . To conclude, significant strides have been made in data reporting from SSA on the burden of hypertension in the region as well as biomarker research to improve understanding and identification of areas of intervention. However, gaps remain on linkage between knowledge generation, translation, and implementation research. Coordinated studies addressing both discovery science and public health are crucial to curb hypertension development and improve management in SSA.

Development and validation of prediction models for hypertension risks: A cross-sectional study based on 4,287,407 participants

Abstract:Objective: To develop an optimal screening model to identify the individuals with a high risk of hypertension in China by comparing tree-based machine learning models, such as classification and regression tree, random forest, adaboost with a decision tree, extreme gradient boosting decision tree, and other machine learning models like an artificial neural network, naive Bayes, and traditional logistic regression models. Methods: A total of 4,287,407 adults participating in the national physical examination were included in the study. Features were selected using the least absolute shrinkage and selection operator regression. The Borderline synthetic minority over-sampling technique was used for data balance. Non-laboratory and semi-laboratory analyses were carried out in combination with the selected features. The tree-based machine learning models, other machine learning models, and traditional logistic regression models were constructed to identify individuals with hypertension, respectively. Top features selected using the best algorithm and the corresponding variable importance score were visualized. Results: A total of 24 variables were finally included for analyses after the least absolute shrinkage and selection operator regression model. The sample size of hypertensive patients in the training set was expanded from 689,025 to 2,312,160 using the borderline synthetic minority over-sampling technique algorithm. The extreme gradient boosting decision tree algorithm showed the best results (area under the receiver operating characteristic curve of non-laboratory: 0.893 and area under the receiver operating characteristic curve of semi-laboratory: 0.894). This study found that age, systolic blood pressure, waist circumference, diastolic blood pressure, albumin, drinking frequency, electrocardiogram, ethnicity (uyghur, hui, and other), body mass index, sex (female), exercise frequency, diabetes mellitus, and total bilirubin are important factors reflecting hypertension. Besides, some algorithms included in the semi-laboratory analyses showed less improvement in the predictive performance compared to the non-laboratory analyses. Conclusion: Using multiple methods, a more significant prediction model can be built, which discovers risk factors and provides new insights into the prediction and prevention of hypertension.

Implications of scaling up cardiovascular disease treatment in South Africa: a microsimulation and cost-effectiveness analysis

Abstract:Background: Cardiovascular diseases and their risk factors-particularly hypertension, dyslipidaemia, and diabetes-have become an increasing concern for middle-income countries. Using newly available, nationally representative data, we assessed how cardiovascular risk factors are distributed across subpopulations within South Africa and identified which cardiovascular treatments should be prioritised. Methods: We created a demographically representative simulated population for South Africa and used data from 17 743 respondents aged 15 years or older of the 2012 South African National Health and Nutrition Examination Survey (SANHANES) to assign information on cardiovascular risk factors to each member of the simulated population. We created a microsimulation model to estimate the health and economic implications of two globally recognised treatment recommendations: WHO's package of essential non-communicable disease interventions (PEN) and South Africa's Primary Care 101 (SA PC 101) guidelines. The primary outcome was total disability-adjusted life-years (DALYs) averted through treatment of all cardiovascular disease or microvascular type 2 diabetes complications per 1000 population. We compared outcomes at the aspirational level of achieving access to treatment among 70% of the population. Findings: Based on the SANHANES data, South Africans had a high prevalence of hypertension (24·8%), dyslipidaemia (17·5%), and diabetes (15·3%). Prevalence was disproportionately high and treatment low among male, black, and poor populations. Our simulated population experienced a burden of 40·0 DALYs (95% CI 29·5-52·0) per 1000 population per year from cardiovascular disease or type 2 diabetes complications at current treatment levels, which lowered to 32·9 DALYs (24·4-44·7) under WHO PEN implementation and to 32·5 (24·4-44·8) under SA PC 101 implementation. Under both guidelines, there were increases in blood pressure treatment (4·2 percentage points under WHO PEN vs 12·6 percentage points under SA PC 101), lipid treatment (16·0 vs 14·9), and glucose control medications (1·2 vs 0·6). The incremental cost-effectiveness of implementing SA PC 101 over current treatment would be a saving of US$24 902 (95% CI 14 666-62 579) per DALY averted compared with a saving of $17 587 (1840-42 589) under WHO PEN guidelines. Interpretation: Cardiovascular risk factors are common and disproportionate among disadvantaged populations in South Africa. Treatment with blood pressure agents and statins might need greater prioritisation than blood glucose therapies, which contrasts with observed treatment levels despite a lower monthly cost of blood pressure or statin treatment than of sulfonylurea or insulin treatment. Predicting high blood pressure using machine learning models in low- and middle-income countries

Abstract:Responding to the rising global prevalence of noncommunicable diseases (NCDs) requires improvements in the management of high blood pressure. Therefore, this study aims to develop an explainable machine learning model for predicting high blood pressure, a key NCD risk factor, using data from the STEPwise approach to NCD risk factor surveillance (STEPS) surveys. Nationally representative samples of adults aged 18-69 years were acquired from 57 countries spanning six World Health Organization (WHO) regions. Data harmonization and processing were performed to standardize the selected predictors and synchronize features across countries, yielding 41 variables, including demographic, behavioural, physical, and biochemical factors. Five machine learning models - logistic regression, k-nearest neighbours, random forest, XGBoost, and a fully connected neural network - were trained and evaluated at global, regional, and country-specific levels using an 80/20 train-test split. The models' performance was assessed using accuracy, precision, recall, and F1 score. Feature importance analysis identified age, weight, heart rate, waist circumference, and height as key predictors of blood pressure. Across the 57 countries studied, model performances varied considerably, with accuracy ranging from as low as 58.96% in some models for specific countries to as high as 81.41% in others, underscoring the need for region and country-specific adaptations in modelling approaches. The explainable model offers an opportunity for population-level screening and continuous risk assessment in resource-limited settings.
Development and Validation of Prediction Models for Hypertension Risks in Rural Chinese Populations

Abstract:BACKGROUND:Various hypertension predictive models have been developed worldwide; however, there is no existing predictive model for hypertension among Chinese rural populations.METHODS:This is a 6-year population-based prospective cohort in rural areas of China. Data was collected in 2007-2008 (baseline survey) and 2013-2014 (follow-up survey) from 8319 participants ranging in age from 35 to 74 years old. Specified gender hypertension predictive models were established based on multivariate Cox regression, Artificial Neural Network (ANN), Naive Bayes Classifier (NBC), and Classification and Regression Tree (CART) in the training set. External validation was conducted in the testing set. The estimated models were assessed by discrimination and calibration, respectively.RESULTS:During the follow-up period, 432 men and 604 women developed hypertension in the training set. Assessment for established models in men suggested men office-based model (M1) was better than others. C-index of M1 model in the testing set was 0.771 (95% confidence Interval (CI) = 0.750, 0.791), and calibration χ2 = 6.3057 (P = 0.7090). In women, women office-based model (W1) and ANN were better than the other models assessed. The C-indexes for the W1 model and the ANN model in the testing set were 0.765 (95% CI = 0.746, 0.783) and 0.756 (95% CI = 0.737, 0.775) and the calibrations χ2 were 6.7832 (P = 0.1478) and 4.7447 (P = 0.3145), respectively.CONCLUSIONS:Not all machine-learning models performed better than the traditional Cox regression models. The W1 and ANN models for women and M1 model for men have better predictive performance which could potentially be recommended for predicting hypertension risk among rural populations.

Abstract:Risk prediction models are frequently used to identify individuals at risk of developing hypertension. This study evaluates different machine learning algorithms and compares their predictive performance with the conventional Cox proportional hazards (PH) model to predict hypertension incidence using survival data. This study analyzed 18,322 participants on 24 candidate features from the large Alberta's Tomorrow Project (ATP) to develop different prediction models. To select the top features, we applied five feature selection methods, including two filter-based: a univariate Cox p-value and C-index; two embedded-based: random survival forest and least absolute shrinkage and selection operator (Lasso); and one constraint-based: the statistically equivalent signature (SES). Five machine learning algorithms were developed to predict hypertension incidence: penalized regression Ridge, Lasso, Elastic Net (EN), random survival forest (RSF), and gradient boosting (GB), along with the conventional Cox PH model. The predictive performance of the models was assessed using C-index. The performance of machine learning algorithms was observed, similar to the conventional Cox PH model. Average C-indexes were 0.78, 0.78, 0.78, 0.76, 0.76, and 0.77 for Ridge, Lasso, EN, RSF, GB and Cox PH, respectively. Important features associated with each model were also presented. Our study findings demonstrate little predictive performance difference between machine learning algorithms and the conventional Cox PH regression model in predicting hypertension incidence. In a moderate dataset with a reasonable number of features, conventional regression-based models perform similar to machine learning algorithms with good predictive accuracy.

Abstract:Background: Hypertension, a global burden, increases mortality from cardiovascular and kidney diseases. Worldwide, in 2019, an estimated 1.3 billion adults developed hypertension. In Africa, the cumulative estimated prevalence of hypertension is ∼30.8%. Main Objective: To perform a comparative analysis between African countries focusing on hypertension prevalence, health policy, and guidelines mapping. Methods: A descriptive comparative analytic study compared hypertension data from nine African countries: Eswatini, Ethiopia, Ghana, Kenya, Malawi, Nigeria, Somalia, South Sudan, and Sudan, in 2024. Data included hypertension prevalence, health policy, guidelines’ mapping, and hypertension management responses. Mixed statistical methods of content analysis, percentages, means, and Spearman's correlations were used. Results: Hypertension prevalence varied from 27% in Ethiopia to 43% in Eswatini. Females consistently had higher prevalence than males, 30% in Ethiopia to 47% in Eswatini, suggesting gender-specific risk factors or disparities in healthcare access/utilization. Sudan had the second-highest prevalence 44%. Eswatini implemented strong blood pressure control measures, unlike Sudan. High prevalence was also found in countries with strong policies (Eswatini, Kenya, Nigeria, South Sudan), suggesting urgent improved healthcare delivery, policy enforcement, and addressing other risk factors. Ghana and Somalia had lower hypertension prevalence, though fewer policies. Spearman's test showed strong negative correlations between national responses and hypertension prevalence. Salt Intake Surveys had the strongest negative correlation (-0.707) with hypertension prevalence, highlighting countries like Ethiopia and Malawi with salt intake surveys and lower prevalence, reinforcing dietary factors’ effectiveness in managing hypertension. Kenya, Nigeria, and Ethiopia leverage trained non-physician health workers in hypertension management, thereby, improving treatment access and adherence. Treatment guidelines were moderately negatively correlated (-0.335) with hypertension prevalence. The mere presence of guidelines is insufficient, signifying their implementation and enhancement with targeted interventions Conclusions: These findings highlighted the multifaceted nature of hypertension in African countries which requires comprehensive approaches encompassing considerations of gender, socioeconomic factors, lifestyle, salt reduction, policy implementation, healthcare access, and a unified African hypertension guideline.

Abstract:Objective: To determine any differences in mental health (depression and anxiety traits) and poor psychosocial well-being by hypertension status, in adults at high-risk for diabetes. Design and method: In a population-based cross-sectional study, socio-demographic characteristics, lifestyle behaviours (cigarette smoking, alcohol misuse, physical inactivity), and mental health and psychosocial information were collected via administered questionnaires. Depression and anxiety traits were investigated using the Patient Health Questionnaire-9 (PHQ-9) and Generalised Anxiety Disorder (GAD-7) screening questions. Blood pressures (BP) were measured, and hypertension defined as systolic BP >=140 mmHg and/or diastolic BP >=90 mmHg, self-reported hypertension diagnosis or on antihypertension medication. Multivariable logistic regressions determined the associations with hypertension; the basic model comprised socio-demographic and lifestyle behaviours. Mental health and psychosocial variables were entered individually in separate models. Results: Among 678 participants, median age among those with (n=384) and without (n=290) hypertension was 54 vs. 49 years (p =10 (17.4% vs. 10.0%, p=0.004) and anxiety scores >=10 (11.7% vs. 8.2%, p=0.392) were higher in participants with than without hypertension but were only significant for depression. In the basic regression model, only increasing age was significantly associated with hypertension. Participants with depression score >=10 vs. <=4 were twice as likely to have hypertension (OR: 2.07, 95%CI: 1.23-3.46, p=0.006). Increasing anxiety scores were related to hypertension (OR: 1.05, 95%CI: 1.01-2.64, p=0.034). Poor psychosocial well-being, defined as 1) decreasing social support (OR: 1.03, 95%CI: 1.01-1.06, p=0.039), 2) physical health or emotional problems interfering with social activities all/most of the time (OR: 2.07, 95%CI: 1.02-4.17, p=0.043), 3) perception of worsening health (OR: 1.06, 95%CI: 1.02-1.11, p=0.002), and 4) a serious ongoing health problem (OR: 1.52, 95%CI: 1.01-2.30, p=0.048), was associated with hypertension in separate models. Quality of life and increasing number of adverse life events experienced were not significantly associated with hypertension. Conclusions: The high burden and associations of depression and anxiety traits, and poor psychosocial well-being with hypertension suggest a need for strategies to mitigate the effects of these conditions on hypertension.
