Abstract:Introduction/objective: The influence of cognitive behavioral therapy (CBT) and its modalities on various neuropsychiatric conditions is herein explored together with their impact on specific cardiovascular (CV) diseases (CVD). Methods: A comprehensive review of the literature was undertaken via the PubMed, Scopus and Google Scholar on the above relevant topics. The focus was on large randomized controlled trials and meta-analyses. Results: Among the various neuropsychiatric disorders, depression and anxiety commonly occur in CVD patients, frequently eluding clinician's attention. This reciprocal liaison may incur higher rates of morbidity/mortality, through physiological and behavioral mechanisms. Multimodal psychiatric interventions, using medications and psychotherapies, such as CBT, seem promising. Such mindfulness-based interventions have the potential to be an efficacious complementary strategy to address psychological stress in CVD patients. As the cost of CBT is relatively low, such a supportive approach for stress management provides high patient acceptability, with a positive impact on improving quality of life, by promoting CV health and mitigating CV complications. Conclusion: There is ample evidence of a reciprocal liaison between heart and mind. Several CV risk factors are strongly affected by diseases of the mind, and the clinical course of various CVDs is influenced by affective or other psychiatric disorders. CBT and relevant mindfulness-based interventions have a significant supportive role in patients with various CVDs by targeting CV risk factor(s) or the underlying specific CVD and by identifying and addressing psychosocial issues. In this direction, various CBT interventions can provide the means to favorably influence both CV risk factors and CVDs.
[Treatment of dislocated lenses in patients with Marfan's syndrome using vitrectomy and intraocular lenses].

Abstract:PURPOSE To evaluate results of surgical complex treatment of lens dislocation in Marfan syndrome cases. MATERIAL AND METHODS Nineteen eyes of 13 patients were operatively treated for lens dislocation. Pars plana vitrectomy was done in all eyes. Dislocated lenses were removed by lensectomy in 5 eyes and with an intracapsular method in 14 eyes. The scleral fixation technique was used for primary posterior chamber intraocular lens implantation in 18 eyes, and one eye remained aphakic. Time of observation ranged between 6 and 71 months (mean 3.5 years). RESULTS All eyes achieved improvement in visual acuity or good preoperative visual acuity was maintained. The vision between 6/6 and 6/8 was achieved in 14 patients. There were no serious intraoperative or early postoperative complications. Retinal detachment appeared in two eyes in a long observation time and it was successfully operatively treated. The not good visual results were due to amblyopia (n = 2), complications of retinal detachment surgery (n = 2) and glaucomatous optic disc atrophy coexisting with macular degeneration (n = 1). CONCLUSIONS Pars plana vitrectomy and primary scleral-fixated IOL implantation gives quick and good visual rehabilitation in adult patients with Marfan's syndrome. The described surgical method is complex and need a prolonged operative time, but gives very good functional results and very few complications.

Abstract:We report on a de novo submicroscopic deletion of 20q13.33 identified by subtelomeric fluorescence in situ hybridization (FISH) in a 4‐year‐old girl with learning difficulties, hyperlaxity and strabismus, but without obvious dysmorphic features. Further investigations by array‐based comparative genomic hybridization (array‐CGH) and FISH analysis allowed us to delineate the smallest reported subterminal deletion of chromosome 20q, spanning a 1.1–1.6 Mb with a breakpoint localized between BAC RP5‐887L7 and RP11‐261N11. The genes CHRNA4 and KCNQ2 implicated in autosomal dominant epilepsy are included in the deletion interval. Subterminal 20q deletions as found in the present patient have, to our knowledge, only been reported in three patients. We review the clinical and behavioral phenotype of such “pure” subterminal 20q deletions. © 2007 Wiley‐Liss, Inc.

Abstract:From the analysis of 190 case histories of elderly primiparas, the following conclusions were drawn concerning their pregnancy and delivery. Frequency = 0.6%. The high frequency of spontaneous abortion (26.8%) and infertility treatment (20.5%) should be stressed. Age-related obstetric pathology in the sample group is dominated by the high frequency of toxaemia (32.4%), premature delivery (14.3%) and uterine fibromata (17.3%). Dystocia is frequent: normal, unassisted vaginal deliveries: 32%; forceps: 33.2%; Caesarean section: 34.2%. Neonatal mortality remains high: 5.2% as does morbidity (Apgar less than 7: 29.6%).

Abstract:docrine cells of the intestines, are of endodermal origin [5] thus makes sense, rationalizing the former difficulties in understanding the pathogenesis of medullary carcinoma and mixed MTC tumours. The derivation of C cells from the endoderm rather than the neural crest may at first seem to make it more difficult to understand the association between MTC and phaeochromocytoma. However, the types of tumours found in inherited syndromes primarily depend on the mutated genes involved in growth control rather than on the embryological origins of the organs involved. Good examples include Lynch syndrome where a mutated DNA repair gene is associated with an increased incidence of colon, endometrial, ovarian and other cancers, and Li Fraumeni syndrome where germline mutations in p53 are associated with sarcomas and cancers of the breast, brain and adrenal glands, and with leukaemias. Similarly, the RET gene is associated with lesions of both neural crest origin (neuroma and phaeochromocytoma) and of endodermal origin (medullary carcinoma and parathyroid adenoma). We fully agree with Drs. Paschou and Vryonidou that understanding the genes and their mutations involved in carcinogenesis, such as RET for MTC, will play a major role in improving treatment. Nonetheless, it is also important to consider embryonic cell and tissue origin as this may determine organ-specific traits of tumour Dear Editor We thank Drs. The main discussion point they raise is about the similarities between the features of medullary thyroid carcinoma (MTC) and intestinal neuroendocrine tumours, and their differences from the cancers of the same organs showing epithelial differentiation. We believe it is important to distinguish neuroendocrine tumours not only from adenocarcinomas, but also from neuroectodermal tumours, and would like to clarify the reasons. Neuroendocrine cells share some morphological and functional features with neural cells (e.g. granule structure and secretion of signalling peptides), but they are clearly not of neural crest origin, they derive from the same stem cells as the proper epithelium [2] . This is true for neuroendocrine cells in gut and in other endodermal organs such as the lungs, pancreas and prostate and for their neuroendocrine tumours. In contrast, neuroectodermal tumours are a separate group that can be found in a variety of peripheral organs, and although tumour cell features may vary considerably between subgroups depending on the ancestral differentiated cell type, they are all of neural crest origin [3] . Abstract:Objectives Cardiovascular diseases (CVDs) are a leading cause of death worldwide, emerging from a combination of several factors. The aim of this review is to define the psychological factors that are significant in the development and progression of these disorders. Methods Studies published through 2023 concerning adults with psychological vulnerability factors and/or cardiovascular disease were selected through searches of PubMed, PsychINFO, Science Direct, and Google Scholar. Results Psychological stress may influence CVD, in combination with other risk factors, or it can act independently, as in cases of workplace stress, post-traumatic stress disorder, Takotsubo syndrome and bereavement. Coping strategies, anxiety and depression have also been identified as relevant psychological factors in cardiac patients. Adverse childhood experiences are linked to a reduced quality of life and have been identified as significant risk factors for the development of acquired CVDs. Conclusion This review demonstrates that several psychological factors affect cardiovascular function. An in-depth study of the psychological correlates of CVDs would allow healthcare professionals to design more effective prevention and intervention programs.

Abstract:Abstract Background Depression is associated with an increased risk for cardiovascular disease (CVD). Biological cardiac risk factors are already elevated in depressed patients without existing CVD. The purpose of this exploratory trial was to examine whether treating Major Depression (MD) with cognitive behavioral therapy (CBT) is associated with improvements in cardiac risk biomarkers and whether depressive symptom severity at baseline moderates treatment effects. Methods Eighty antidepressant-free patients with MD were randomly assigned to CBT or waiting list (WL). Biological outcomes included long-term recordings (24-h, daytime, nighttime) of heart rate, heart rate variability (HRV), and blood pressure, as well as inflammatory markers such as C-reactive protein (CRP), interleukin (IL)-6, and tumor necrosis factor (TNF)- α . A sample of 40 age- and sex-matched non-clinical controls was also involved to verify biological alterations in MD at study entry. Results Compared to WL, CBT was associated with a significant increase in overall HRV, as indexed by the 24-h and daytime HRV triangular index, as well as trend improvements in 24-h low-frequency HRV and daytime systolic blood pressure. Self-rated depressive symptom severity moderated (or tended to moderate) improvements in CBT for 24-h and daytime heart rate and several indices of HRV (especially daytime measures). Inflammatory treatment effects were not observed. Conclusions CBT increased overall HRV in patients with MD. Initially more depressed patients showed the most pronounced cardiovascular improvements through CBT. These exploratory findings may provide new insights into the biological effects of psychological treatment against depression and must be confirmed through future research.

Abstract:Abstract Aims Depression and anxiety occur frequently in individuals with cardiovascular disease and are associated with poor prognosis. This Cochrane systematic review and meta-analysis assessed the effectiveness of psychological interventions on psychological and clinical outcomes in adults with coronary heart disease (CHD), heart failure (HF) or atrial fibrillation (AF). Methods and results CENTRAL, MEDLINE, Embase, PsycINFO, and CINAHL databases were searched from January 2009 to July 2022 for randomized controlled trials of psychological interventions vs. controls in adults with CHD, HF, or AF. Twenty-one studies (n = 2591) were assessed using random-effects models. We found psychological interventions reduced depression [standardized mean difference (SMD) −0.36; 95% confidence interval (CI) −0.65 to −0.06; P = 0.02], anxiety (SMD −0.57; 95% CI −0.96 to −0.18; P = 0.004), and improved mental health-related quality of life (HRQoL) (SMD 0.63, 95% CI 0.01 to 1.26; P = 0.05) (follow-up 6–12 months), but not physical health-related quality of life, all-cause mortality or major adverse cardiovascular events compared with controls. High heterogeneity was present across meta-analyses. Meta-regression analysis showed that psychological interventions designed to target anxiety, were more effective than non-targeted interventions. Conclusion This review found that psychological interventions improved depression, anxiety and mental HRQoL, with those targeting anxiety to show most benefit. Given the statistical heterogeneity, the precise magnitude of effects remains uncertain. Increasing use of multifactorial psychological interventions shows promise for incorporating patient needs and preferences. Investigation of those at high risk of poor outcomes, comparison of intervention components and those with AF is warranted.

Abstract:Ovarian cancer (OC) is the most lethal gynecological cancer. The molecular mechanism of it is complicated, and numerous researches suggest that microRNAs are key regulators for it. This study was to investigate the pivotal role of miR-629 in the progression of OC and to reveal the possible molecular mechanism of its action. Testis-specific Y-like protein 5 (TSPYL5) is a tumor suppressor gene in various cancers, but there is little for its role in OC. OC OVCAR3 cells were transfected with the miR-629 vector, miR-629 inhibitor, and/or small interfering RNA (siRNA) targeting TSPYL5 (si-TSPYL5), respectively. After transfection, cell apoptosis, the ability of migration, and invasion were explored, as well as the level of miR-629 and TSPYL5 protein expression were detected by quantitative polymerase chain reaction and western blot. Compared with the control, there was increasing of miR-629, and decreasing of TSPYL5 and caspase 3 in OC tissue. Overexpression of miR-629 promoted the cell ability of migration and invasion and reduced OC cell apoptosis. In addition, elevated cancer inhibition ability of TSPYL5 induced by the miR-629 inhibitor was significantly blocked by inhibition of TSPYL5 (si-TSPYL5). All the above results suggested that miR-629 could promote OC proliferation, migration, and invasion by directly suppressing TSPYL5 expression, and inhibition of miR-629 might serve as a therapeutic target for OC.

Abstract:Sexual health has a fundamental role in overall health and well-being, and a healthy and dynamic sex life can make an important contribution to a good quality of life. Sexual dysfunction, and especially erectile dysfunction (ED) in men, is highly prevalent in patients with cardiovascular disease (CVD). CVD and ED have shared risk factors and pathophysiological links, such as endothelial dysfunction, inflammation and low plasma testosterone levels. ED has been shown to be an independent and early harbinger of future CVD events, providing an important window to initiate preventive measures. Therefore, screening and diagnosing ED is essential for the primary and secondary prevention of CVD because the assessment of ED offers an easy and low-cost prognostic tool that is an alternative to other investigational cardiovascular biomarkers. Moreover, ED is a major contributing factor to the discontinuation of, or poor adherence to, cardiovascular therapy. Cardiovascular drugs have divergent effects on erectile function, with diuretics and β-blockers having the worst profiles, and renin–angiotensin–aldosterone system inhibitors and nebivolol having the best profiles. Pharmacological treatment of ED has an equivocal effect on the risk of CVD, suggesting a complex interaction between ED and drugs for CVD. In this Review, we discuss how sexual function could be incorporated into the patient history taken by physicians treating individuals with CVD, not merely as part of the diagnostic work-up but as a means to pursue tangible and essential benefits in quality of life and cardiovascular outcomes.

Abstract:Coronary heart disease (CHD) is the leading cause of people's deaths in the world. Psychological intervention is a crucial factor in the treatment of CHD. One of the significant aspects of patient's forecast improvement is a compliance to treatment. Negative emotional states accompany the worsening of the course of the disease in patients with a cardiac profile. Patients with CHD are characterized by increased level of stress, depression and anxiety, non-adaptive coping strategies, defensive mechanisms and inharmonious internal picture of the disease. The psychological intervention improves compliance, the quality of life and thus speeds up physical recovery. It is crucial to determine what methods and approaches in the work of a medical psychologist are the most productive and contribute to the preservation of a long-term effect. According to several sources, the most effective and demanded approach when working with CHD patients is the cognitivebehavioral approach. It is distinguished by its shortterm impact; maintaining the duration of the effect; anxiety, stress and depression reduction; change of the internal picture of the disease, harmonizing the doctor-patient relationship, treatment adherence, compliance and improving cognitive functioning. In particular, researchers note the feasibility of methods of breathing and muscle relaxation, meditation, cognitive restructuring and diary method.

Abstract:Background Depression is an independent risk factor for cardiovascular disease (CVD), but it is unknown if successful depression treatment reduces CVD risk. Methods Using eIMPACT trial data, we examined the effect of modernized collaborative care for depression on indicators of CVD risk. A total of 216 primary care patients with depression and elevated CVD risk were randomized to 12 months of the eIMPACT intervention (internet cognitive-behavioral therapy [CBT], telephonic CBT, and select antidepressant medications) or usual primary care. CVD-relevant health behaviors (self-reported CVD prevention medication adherence, sedentary behavior, and sleep quality) and traditional CVD risk factors (blood pressure and lipid fractions) were assessed over 12 months. Incident CVD events were tracked over four years using a statewide health information exchange. Results The intervention group exhibited greater improvement in depressive symptoms (p < 0.01) and sleep quality (p < 0.01) than the usual care group, but there was no intervention effect on systolic blood pressure (p = 0.36), low-density lipoprotein cholesterol (p = 0.38), high-density lipoprotein cholesterol (p = 0.79), triglycerides (p = 0.76), CVD prevention medication adherence (p = 0.64), or sedentary behavior (p = 0.57). There was an intervention effect on diastolic blood pressure that favored the usual care group (p = 0.02). The likelihood of an incident CVD event did not differ between the intervention (13/107, 12.1%) and usual care (9/109, 8.3%) groups (p = 0.39). Conclusions Successful depression treatment alone is not sufficient to lower the heightened CVD risk of people with depression. Alternative approaches are needed. Abstract:N-Methyl-3,4-methylenedioxyamphetamine (MDMA), also called "Ecstasy," is a commonly abused psychoactive drug among the American youth. We present the case of a 23-year-old Korean-American woman who presented with seizure, delirium, and rigidity after MDMA ingestion. She was febrile (38.7°C), tachycardic (188 beats/min), tachypneic (26 breaths/min) with a borderline blood pressure (95/43 mm Hg). Examination revealed generalized muscle rigidity, tremors, hyperreflexia, and ocular clonus, leading to the diagnosis of serotonin syndrome. Urine toxicology screen was only positive for amphetamines, consistent with the history of MDMA ingestion. Initial laboratory testing showed thrombocytopenia, further testing showed deranged prothrombin time, partial thromboplastin time, decreased fibrinogen, and elevated D-dimer, suggesting disseminated intravascular coagulation. Hepatic transaminases trended up dramatically reflecting acute hepatitis. The patient received supportive care and improved by hospital day 3. MDMA toxicity manifested as serotonin syndrome, hepatitis, and coagulopathy is exceedingly rare. MDMA is metabolized by the hepatic CYP2D6 enzyme. Certain populations, such as Koreans, Chinese, and Japanese have a high prevalence of a polymorphism that confers reduced enzyme activity. We discuss this hypothesis as a possible cause for this severe presentation in our patient after a single ingestion.

Abstract:We conducted a review to resolve whether there is relationship between depression and coronary heart disease or not. Depression is a mental health problem which is spread widely and most of physicians are well acquainted with this concept of depression. Diagnosis of depression disorder based on continuity of illness, etiology of illness and number of symptoms - all these discriminations should be reported during inspecting the link between depression and coronary heart disease. We review the mechanism of linking depression and coronary heart disease i.e. relationship between depression and inflammation, depression and autonomic dysfunction, CHD and autonomic dysfunction, depression and Sleep architecture disruption, depression and circadian rhythm disruption, CHD and circadian rhythm disruption, and depression and behavioral mechanism. There is bidirectional association between depression and coronary artery disease i.e. coronary artery disease can cause major depressive disorder and depression is risk factor for CAD and its complications. Major depression is a devastating comorbid disease that can make recovery difficult and increase risk of cardiac mortality and morbidity. The antidepressant medications like SSRIs e.g. Sertraline, fluoxetine, citalopram etc. appears to be safe in individual with depression and concomitant CAD or unstable angina. However, some evidence suggests that SSRIs like tricyclics, may increase risk cardiac events and death when taken for long time. New classes of antidepressants have dual reuptake inhibition for serotonin and nor-epinephrine e.g. venlafaxine. These medications are slightly more successful than SSRIs in treating depression, but they also have some adverse effects. Selegiline transdermal form was recently licensed for the treatment of MDD. Oral selegiline is not effective antidepressant. When compared to oral selegiline, STS results in stable plasma levels of drug and increased drug concentration in the brain.

Abstract:The objective of this review is to evaluate the effectiveness of psychosocial interventions on mental health outcomes in adult patients with Cardiovascular Diseases (CVDs) living in low- and middle-income countries (LMICs). Mental health issues are highly prevalent among patients with CVDs leading to poor disease prognosis, self-care/ management, and Quality of Life (QOL). In the context of LMICs, where the disease burden and treatment gap are high and resources are inadequate for accessing essential care, effective psychosocial interventions can make significant contributions for improving mental health and reducing mental health problems among patients who live with cardiovascular diseases. This review will include studies published between 2010 and 2021 that evaluated the effect of psychosocial interventions on mental health outcomes (resilience, self-efficacy, QOL, depression and anxiety) on adult patients (aged ≥18 years) with any cardiovascular diseases using experimental and quasi experimental designs. The search will be conducted from the following databases: MEDLINE via OVID (1946—Present), EMBASE via OVID (1974 –Present), Cumulative Index for Nursing and Allied Health Literature (CINAHL) via EBSCOhost (1936—Present), PsycINFO via OVID (1806—Present), Scopus via Elsevier (1976—Present), and Cochrane Library via Wiley (1992—Present). Data will be critically appraised using standard tools and extracted by two reviewers and disagreement will be solved by the third reviewer. Meta-analysis will be performed, if possible, otherwise, data will be synthesized in narrative and tabular forms. The findings of this review will provide a key insight into contextually relevant psychosocial interventions for promoting mental health of patients with CVDs living in LMICs. The review findings will be potentially useful for health care providers and researchers to implement such interventions not only for reducing the burden of mental health issues but also for improving the overall well-being among patients with chronic illnesses. Abstract:OBJECTIVE: Cardiovascular diseases are associated with an increased risk of depression, but it remains unclear whether treatment with cardiovascular agents decreases or increases this risk. The effects of drugs on individual usage are also often unknown. This review aimed to examine the correlation between depression and common cardiovascular drugs, develop more potent interventions for depression in cardiovascular patients, and further research on the bio-behavioural mechanisms linking cardiovascular drugs to depression.DATA SOURCES: The data in this review were obtained from articles included in PubMed, EMBASE, and Web of Science.STUDY SELECTION: Clinical trials, observational studies, review literature, and guidelines about depression and cardiovascular drugs were selected for the article.RESULTS: We systematically investigated whether the seven most used cardiovascular drugs were associated with altered risk of incident depression in this literature review. Statins have been proven to have antidepressant effects. Some studies believe angiotensin-converting enzyme inhibitors (ACEIs)/angiotensin receptor blocker (ARB) can exert an antidepressant influence by acting on the renin-angiotensin system, but further clinical trials are needed to confirm this. Beta-blockers have previously been associated with depression, but the current study found no significant association between beta blockers and the risk of depression. Aspirin may have antidepressant effects by suppressing the immune response, but its role as an antidepressant remains controversial. calcium channel blockers (CCBs) can regulate nerve signal transduction by adjusting calcium channels, but whether this effect is beneficial or harmful to depression remains unclear. Finally, some cases have reported that nitrates and diuretics are associated with depression, but the current clinical evidence is insufficient.CONCLUSIONS: Statins have been proven to have antidepressant effect, and the antidepressant effects of ACEIs/ARB and aspirin are still controversial. CCBs are associated with depression, but it is unclear whether it is beneficial or harmful. No association has been found with β-blockers, diuretics, and nitrates.
