Abstract:Background Advanced non-small cell lung cancer (NSCLC) harboring epidermal growth factor receptor (EGFR) mutations has been successfully treated with tyrosine kinase inhibitors (TKIs). However, resistance to osimertinib, a third-generation TKI, can be difficult to overcome in this small subset of patients and is attributed to secondary resistant mutations. Here, we report a case of acquired EGFR L858R/L718Q mutation with advanced NSCLC that resistant to osimertinib, which was successfully overcome using dacomitinib. Case Presentation A 64-year-old non-smoker woman was diagnosed with stage IV non-small cell lung adenocarcinoma with EGFR L858R mutation and brain metastasis in November 2018. Treatment with gefitinib and gamma knife radiosurgery was started as the first-line treatment. After 7 months, she experienced disease progression with increased primary lung lesions and switched to osimertinib based on an acquired EGFR T790M mutation. After another 4 months, the disease progressed, and she was switched to chemotherapy. During chemotherapy, brain MRI showed an increasing number of parietal lobe metastases. Hence, gamma knife radiosurgery was performed again. After 12 months, the disease progression resumed, and an EGFR L718Q mutation was found on biopsy. The patient was then challenged with dacomitinib, and the disease was partially responsive and under control for 6 months. Conclusion Currently, there are no established guidelines for overcoming osimertinib resistance caused by the L718Q mutation. The acquired EGFR L718Q mutation in subsequent resistance to osimertinib could be overcome using dacomitinib, indicating a promising treatment option in the clinic.

Abstract:Osimertinib, the third generation of epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor, can overcome resistance to first- or second-generation EGFR tyrosine kinase inhibitor with secondary T790M mutation. Limited strategies were available after the disease of nonesmall-cell lung cancer patients developed resistance to osimertinib. Clearance of T790M with concomitant EGFR L858R and L718Q mutation had been reported as one of the mechanisms of osimertinib resistance. We report the first case of afatinib treatment after osimertinib resistance. The patient experienced a partial response in 1 month but finally experienced disease progression due to secondary KRAS (Kirsten rat sarcoma viral oncogene) mutation. Afatinib may be a promising strategy for non-small-cell lung cancer patients with concomitant EGFR L858R and L718Q mutation after osimertinib resistance. (C) 2020 Elsevier Inc. All rights reserved.

Abstract:For patients with advanced non‐small cell lung cancer (NSCLC) that have epidermal growth factor receptor ( EGFR ) mutations, EGFR tyrosine kinase inhibitors (TKIs) are the standard treatment and have significant clinical benefits. Third‐generation TKIs, such as osimertinib, almonertinib and furmonertinib, are effective for the treatment of NSCLC that is EGFR‐sensitizing mutation‐positive and T790M‐positive. Despite the efficacy of third‐generation TKIs, patients inevitably develop resistance and the resistance mechanisms are heterogeneous. Second‐generation inhibitors, such as afatinib, may be crucial in treating diseases that have developed resistance to first‐ or third‐generation inhibitors. However, the clinical effect of afatinib in patients with acquired multiple EGFR mutations is not well defined. To the best of our knowledge, the present report describes the first case of a patient with lung adenocarcinoma who had multiple co‐existing EGFR resistance mutations, including EGFR L718Q , EGFR C797S, EGFR C797G, EGFR L792H, EGFR V802F and EGFR V689L . These mutations conferred resistance to almonertinib, whilst maintaining sensitivity to afatinib.

Abstract:Background: Advanced non-small cell lung cancer (NSCLC) is often complicated by leptomeningeal metastases (LMs), especially in patients carrying EGFR mutations. EGFR tyrosine kinase inhibitors (TKIs) are the first-line drug for patients with specific gene mutations, such as EGFR exon 19 deletion or exon 21 L858R mutation. However, after long-term TKI use, patients eventually develop drug resistance and acquire new mutations. Acquiring the EGFR T790 M mutation during TKI treatment is a marker for first/second generation TKI resistance. Osimertinib (a third-generation TKI) could overcome this resistance, especially for patients who have already developed NSCLC-LM. Treating NSCLC patients with osimertinib resistance is challenging. Our aim was to investigate whether afatinib is effective in NSCLC-LM patient who showed resistance to osimertinib. Herein, we report two patients with resistance to first- and third-generation TKIs who benefited from second-generation TKI. Case summary: Case one: A 43-year-old man was diagnosed with stage 3A NSCLC with EGFR exon 19 deletion. He underwent surgery and received 4 rounds of chemotherapy (oxaliplatin combined with liposomal paclitaxel), after which the disease was controlled by icotinib (a first-generation TKI) for 4 years. Then, he showed poor drug response and bone metastasis. A liquid biopsy was carried out, and the EGFR L858R/T790 M compound mutation was found. The patient was given osimertinib (a third-generation TKI). The patient was in a stable condition for 2 years and then he developed bilateral peripheral facial palsy. Brain MRI showed enhancement in the left temporal lobe and meninges, and cerebrospinal fluid (CSF) cytology detected tumour cells in the CSF. NSCLC-LM was diagnosed. His performance status (PS) score was 3-4. Liquid biopsy and next-generation sequencing were performed again. Different gene mutations and copy number alterations were found this time, including EGFR L858R, but without the EGFR T790 M mutation. His disease was controlled with intrathecal methotrexate and oral afatinib (a second generation TKI). The patient has shown clinical remission (PS score: 1-2) until now, which is longer than 10 months. Case two: A 50-year-old man was diagnosed with NSCLC in May 2020. He underwent one round of chemotherapy before thoracoscopic partial lobectomy of the right upper lung. Histological study of the lung tissue showed lung adenocarcinoma with the EGFR L858R mutation. Then, the disease was controlled with icotinib. One year later, he was diagnosed with NSCLC-LM. Liquid biopsy and sequencing showed an EGFR L858R/S768I compound mutation. The patient was treated with osimertinib. His condition was stable for 5 months before his central nervous system (CNS) symptoms were exacerbated. Liquid biopsy and sequencing were carried out again, and his gene mutation profile had not changed much. Then, the patient was given afatinib, and his condition has remained stable for 11 months. Conclusion: Afatinib might be suitable for NSCLC-LM patients with EGFR compound mutations who show resistance to icotinib and osimertinib, since it might help overcome first- and third-generation TKI resistance.
Durable response to afatinib in an advanced lung adenocarcinoma patient with an EGFR L858R/G729A compound mutation: a case report

Abstract:Of the epidermal growth factor receptor (EGFR) mutations in patients with non-small cell lung cancer (NSCLC), 10-15% are uncommon mutations. Most of the EGFR "major" uncommon mutations have shown responses to EGFR-tyrosine kinase inhibitors (TKIs). However, there is a lack of clinical data for other less common types of EGFR mutations and the response to EGFR-TKIs, occurring either alone or in combination with EGFR sensitizing mutations. We reported a 70-year-old Chinese man with no smoking history who was diagnosed with stage IVA lung adenocarcinoma. An exceptionally uncommon EGFR G729A mutation in EGFR exon 19 was detected concomitantly with EGFR L858R in exon 21 in tumor specimens by next generation sequencing (NGS). This patient obtained limited benefit from icotinib and the increase in symptoms of cough and chest tightness, so we decided to switch the treatment to afatinib. Our patient exhibited partial response to afatinib with progression-free survival of 10 months. Subsequently, an EGFR T790M mutation was detected in the second lung biopsy. Then, osimertinib was administered and the symptoms improved significantly and the progress-free survival was nearly 16 months. Our data suggests that patients with NSCLC who are positive for uncommon EGFR G729A mutations may benefit from treatment with afatinib.

Abstract:Osimertinib is a third-generation epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKI) used both as the first-line treatment of EGFR-mutated non-small cell lung cancer patients and in second-line after T790M-positive disease progression to first- or second-generation TKIs. Unfortunately, patients unavoidably experience disease progression to osimertinib and the current research is focused on resistance mechanisms and the relative therapeutic strategy. We report the case of a patient with advanced EGFR-mutated (exon 19 deletion and T790M-positive) non-small cell lung cancer who developed disease progression to osimertinib characterized by the loss of T790M concurrently with the emergence of G724S EGFR mutation, which was tackled by subsequent afatinib treatment. Next-generation sequencing molecular study of rebiopsy at time of progression to osimertinib revealed the persistence of EGFR exon 19 deletion, loss of T790M with a new G724S EGFR mutation; other concomitant mechanisms were excluded. Retrospective analysis of cell-free DNA revealed the emergence of G724S EGFR mutation four months before the radiologically-proven disease progression. The patient, after chemotherapy, was treated with afatinib with clinical and radiological benefit. Our case report contributes to increase the knowledge on acquired resistance mechanisms to osimertinib treatment, and it shows, for the first time, the efficacy of afatinib in the case of T790M loss and emergence of G724S EGFR mutation.

Abstract:BACKGROUND:Epidermal growth factor receptor (EGFR) tyrosine kinase inhibitors (TKIs) have been adopted as the standard of care for non-small cell lung cancer (NSCLC) patients harboring EGFR sensitizing mutations. Besides the two common mutations exon 19 deletion and L858R, which together comprise approximately 85% of EGFR mutations in NSCLC, rare EGFR mutations also exist, including point mutations, deletions, and insertions spanning EGFR exons 18-25. However, the responsiveness of uncommon EGFR mutations to EGFR TKIs remains elusive and attracts increasing interest.CASE SUMMARY:Herein, we report a 55-year-old male patient with stage IV NSCLC harboring a rare EGFR L833F-L861Q compound mutation in cis. The patient achieved a partial response to first-line treatment with afatinib and a progression-free survival of 10 mo. After afatinib failure, the patient received multiple line treatments with chemotherapy. Upon disease progression, the heavily pretreated patient was treated with osimertinib and bevacizumab, and both lung lesion and brain metastases were stable for more than 3 mo. He had an overall survival of 25 mo.CONCLUSION:Our case revealed that both afatinib and the osimertinib + bevacizumab combination demonstrated clinical efficacy in NSCLC harboring an EGFR L833F-L861Q compound mutation. The results provide more therapeutic options for patients with rare compound mutations.
Haptic Perception in Multimodal Virtual Environments - The Haptic Paradigm for Teaching and Learning

Abstract:Multimodal Virtual Environments (MVEs) interface between humans and computers using multiple human modalities. A multimodal interface conveys information in a more naturalistic fashion. The user manipulates and experiences the environment through multiple sensory channels. Out of the interfaces for our five senses, the visual and auditory ones have been used most extensively however, interfaces already exist that allow us touch (haptic devices), and technologies for taste and smell may be the next step. Such interfaces give birth to new paradigms for human computer interaction (HCI). Involving students in the learning process has been a challenge for educators for many years. MVEs may contribute to raising interest and motivation in students, improving their learning and retention. In addition, MVEs provide an alternative method of presenting complex concepts during the educational process. In this paper we review MVEs and their use in K-16 education, with the emphasis on the potential combinations of audio, visual, and haptic modalities. We provide a brief historical perspective of MVEs and review their evolution in parallel with education. We present the results of an investigation into the potential benefits of incorporating haptic feedback into an MVE intended for college and high school physics curricula. Our HaptEK16 application helps students understand the difficult concepts underlying hydraulics and Pascal’s Principle. HaptEK16 has the potential to augment or replace traditional laboratory instruction with an approach offering enhanced motivation, retention and intellectual stimulation.
Case report: Durable response of ensartinib targeting EML4-ALK fusion in osimertinib-resistant non-small cell lung cancer

Abstract:Background: Despite significant benefits from targeted therapy in patients with driver mutations, inevitable drug resistance usually occurred in non-small cell lung cancer, highlighting the necessity for sequential treatments to prolong overall survival. Unfortunately, durable drug response has not been reported in posterior-line therapy of cases with acquired EML4-ALK fusion after resistance to osimertinib, urging the need of referable decision-making in clinical management. Case presentation: We present a case of a 71-year-old Chinese female, never smoker, diagnosed with invasive adenocarcinoma in the left inferior lobe of her lung, with metastases in regional lymph nodes. She received erlotinib treatment after the detection of coexistent EGFR L858R/G719S and BRAF V600E via next-generation sequencing of resected tumor tissue. Routine imaging revealed disease progression approximately 14 months after starting erlotinib treatment, followed by the detection of EGFR L858R through non-invasive liquid biopsy. Subsequently, osimertinib was administered, showing clinical activities for nearly 19 months until the emergence of an EML4-ALK fusion. Given the EML4-ALK fusion, a relatively rare resistance mechanism to osimertinib, she received third-line ensartinib treatment. One month later, alleviated tumor lesions plus normal serum marker levels demonstrated the effectiveness of ensartinib in overcoming resistance to osimertinib. Of note, the clinical response to ensartinib persisted for more than 14 months, superior to the previously reported efficacy of aletinib and crizotinib in osimertinib-failure cases. As of the last follow-up in July 2022, the patient showed no signs of recurrence and maintained a good life quality. Conclusion: We reported a third-line ensartinib therapy in a patient with lung adenocarcinoma who developed an acquired EML4-ALK fusion after sequential treatment with erlotinib and osimertinib. Given the rarity of the EML4-ALK fusion as a resistance mechanism to osimertinib, ensartinib emerges as a promising treatment option for this specific clinical challenge, offering superior efficacy and good safety.

Abstract:The efficacy of osimertinib is severely limited by the emergence of EGFR C797S, which is detected in either the cis or trans position with T790M when osimertinib is used as a second-line treatment, and which is largely identified in combination with an EGFR 19 deletion. The EGFR T790M-cis-G796S mutation, which also occurs in exon 20 as C797S, participates in osimertinib resistance. To date, limited data for overcoming this resistance mutation have been reported. Here, we report data for an advanced NSCLC patient who developed EGFR L858R-T790M-cis-G796S and EGFR L718Q resistance co-mutations following progression with osimertinib. Such a case has rarely been reported, and under chemotherapy guidelines for this situation, no other effective treatment is recommended. The patient in our case experienced remarkable clinical improvement and good tolerance to the combination target therapy of brigatinib and cetuximab plus icotinib. At the time of our patient’s last follow-up and prior to publication, our patient had reached more than 9 months of progression-free survival (PFS) and felt very well. Our finding provides clinical evidence that the combined target therapy of brigatinib and cetuximab may potentially be an effective treatment strategy for patients with an acquired EGFR T790M-cis-G796S resistance mutation following osimertinib treatment.

Abstract:The incidence and mortality of lung cancer rank first in the world for malignant tumors. [ 1 ] The latest statistics showed that the incidence of lung cancer in China not only ranks first among all cancers but also continue growing. [ 2 ] Lung cancer can be catergorized into small cell lung cancer (SCLC) and non-small cell lung cancer (NSCLC), with NSCLC comprising 80% to 85% of cases. Epidermal growth factor receptor (EGFR) gene mutations are commonly found in NSCLC patients and serve as therapeutic targets. Several large studies demonstrated that epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) were superior to cytotoxic chemotherapy in patients with EGFR mutations and have significant advantages in terms of objective response rate and progression-free survival (PFS). [ 3–7 ] However, acquired resistance remains inevitable within patients treated with the third generation tyrosine kinase inhibitor (TKI) osimertinib. The EGFR L718Q mutation is an infrequent mechanism of osimertinib resistance and its role in therapeutic application requires further investigation. [ 8 ] Here, we report 2 cases of advanced NSCLC patients with concomitant EGFR L858R/L718Q mutations after osimertinib resistance. In both cases, additional treatment with dacomitinib showed significant therapeutic effects, suggesting that dacomitinib could be a novel treatment method for NSCLC patients with EGFR L718Q mutation.

Abstract:Generations of epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) can significantly improve the outcome of EGFR-positive NSCLC patients. However, acquired TKIs-resistant mutations are inevitable. Except the common EGFR alterations, more and more rare mutations are revealed by next-generation sequencing (NGS), the clinical significance of which are still unclear. Here, we report an advanced lung adenocarcinoma patient who harbored two novel EGFR exon 19 deletions (750_758del and I759S) at the beginning and exhibited a short response to icotinib for 7.0 months. Then, secondary resistance EGFR T751_I759delinsS occurred. Chemotherapy combined with bevacizumab and erlotinib was administered in turn but failed. Standard-dose osimertinib (80 mg daily) obtained durable clinical remission for 16 months, and high-dose osimertinib (160 mg daily) further prolonged the survival of 9 months after leptomeningeal metastases (LM) occurring. This study presented the first case of intractable terminal NSCLC in a patient with EGFR 750_758del, I759S and T751_I759delinsS mutations, who responded positively to osimertinib and achieved a prolonged OS of 52 months, providing a potential therapeutic option for the patients harboring these particular EGFR mutations.

Abstract:Epidermal growth factor receptor ( EGFR ) mutations have emerged as the most well‐studied oncogenic alterations in advanced non‐small cell lung cancer. The presence of single common or rare EGFR mutations and extra complex EGFR mutations correlates with the response sensitivity to EGFR tyrosine kinase inhibitors. Therefore, given the lack of evidence for the emergence of rare EGFR mutation types, the pathogenic mechanisms of uncommon EGFR mutations and the optimal treatment strategies remain to be explored further. The present study describes the case of a patient diagnosed with lung adenocarcinoma (LUAD) carrying two rare EGFR exon 18 indel/G719C and exon 19 L747S mutations, in which persistent lesion shrinkage was exhibited within 16 months of osimertinib treatment. Given the paucity of clinical trials for the treatment of LUAD harboring complex EGER mutations, the present detailed case description may provide clinicians with effective clinical experience in treating patients.

Abstract:Uncommon mutations account for 10-15% of epidermal growth factor receptor (EGFR) mutations in patients with non-small-cell lung cancer (NSCLC). However, in spite of the wealth of knowledge of the clinical significance and tyrosine kinase inhibitor (TKI) sensitivity of these mutations, acquisition of deeper insights is limited by the paucity of case reports and cohort studies of the exceptionally rare mutations, including compound mutations. In the present case, we describe the clinical efficacy of icotinib and osimertinib in a metastatic lung adenocarcinoma patient carrying a highly uncommon EGFR L858R/D761Y compound mutation. The progression-free survival (PFS) with osimertinib treatment was much longer than that with icotinib (19 mo vs 8.2 mo), and the overall survival (OS) has currently exceeded three years. To the best of our knowledge, this is the first report of durable osimertinib response in an NSCLC patient with a rare EGFR L858R/D761Y mutation.

Abstract:Rationale: With the advancement of targeted therapies, epidermal growth factor receptor tyrosine kinase inhibitors have become the preferred initial treatment for patients with advanced epidermal growth factor receptor (EGFR)-mutated non-small cell lung cancer. Osimertinib, a third-generation epidermal growth factor receptor tyrosine kinase inhibitor, is effective against exon 19 and 21 mutations as well as the T790M mutation. It has been approved by both the food and drug administration and European Medicines Agency for the treatment of non-small cell lung cancer patients with locally advanced or metastatic EGFR-mutated tumors, including those who have acquired T790M mutations. Patient concerns: To evaluate the effectiveness of osimertinib in treating patients with EGFR-mutated advanced lung adenocarcinoma and bone metastases, we present the treatment outcomes of 3 patients with EGFR 19 deletion-mutated advanced lung adenocarcinoma and bone metastases who received osimertinib treatment in recent years. All 3 cases involved elderly female patients, aged 62, 62, and 54, respectively. Diagnoses: All 3 cases exhibited a diagnosis of pulmonary adenocarcinoma accompanied by osseous metastases, with genetic testing revealing the presence of an EGFR 19del mutation. Interventions: In the first case, following 17 months of gefitinib therapy, disease progression prompted a switch to osimertinib treatment. In the second case, bone metastases were detected after 20 months of pemetrexed-carboplatin chemotherapy, leading to a transition to osimertinib therapy. In the third case, after 11 months of erlotinib treatment, bone metastases were identified. Subsequent interventions, including radiation therapy, pemetrexed-carboplatin chemotherapy, pemetrexed-bevacizumab maintenance therapy, and docetaxel chemotherapy, failed to arrest the progression of bone metastases. As a result, a combination of osimertinib and anlotinib targeted therapy was administered. Outcomes: All 3 patients experienced relatively good and favorable survival outcomes, with a progression-free survival of 22.7 months, 12 months, and 17.7 months, respectively. Lessons: These cases suggest that osimertinib is a promising treatment option for patients with EGFR 19 deletion-mutated lung adenocarcinoma and bone metastases, although further clinical studies are needed to confirm its efficacy.

Abstract:Several aspects of EGFR TKI treatment have been discussed.The issue of TKIs resistance has been treated in some papers. In their Opinion, Bronte G et al discussed the topic of Osimertinib resistance and the possible combination strategies or the use of fourth generation EGFR-TKIs to overcome it. They supposed that in the next future oncologists will be able to address each patients who experience resistance to upfront osimertinib toward a different treatment strategy on the basis of molecular alterations highlighted by liquid biopsy or tumor biopsy. In this line, Carlo Bao et al presented two clinical cases harboring concomitant EGFR and BRAF alterations and treated with Osimertinib. Both cases were responsive to treatment and liquid biopsy results were in accordance with the clinical behaviour. Patients with advanced NSCLC with secondary T790M can benefit from Osimertinib, but the role of this drug in patients who exhibith resistance without T790M or with T790M unknown status is not well established. From the systematic review and meta-analysis performed by Yi XF and colleagues emerged that patients who undergo progression with brain metastasis on first or second generation TKIs can benefit from subsequent Osimertinib regardless T790M status. This represent an important finding that could orientate clinical decision management after Osimertinib resistance.Other types of resistance mechanisms are known, and the role of tumor-associated macrophages (TAMs) is also emerging (1). Previous studies have demonstrated that high infiltration of TAMs is significantly associated with an unfavorable prognosis in NSCLC patients treated with EGFR-TKIs (2). So, the inhibition of TAMs may a potential approach to improving resistance to EGFR-TKIs. In the review reported by Cheng D et al, a number of preclinical studies have been discussed, analysing the combination of EGFR-TKIs with several compound directed versus TAMs. In particular, inhibiting mTOR, AKT and STAT3 pathways, such as the lipid metabolic pathway, could all be possible potenatial strategies to overcome EGFR-TKI resistance.The addition of antiangiogenic treatments to EGFR-TKis was also studied. The systematic review and meta-analysis by Zheng H et al highlighted that the addition of bevacizumab to EGFR-TKI provide significant better PFS and OS, and the benefit is more evident for patients who have ever smoked, aged <75 years and of Asian population. Hsu PC and colleagues reported the results of a study in which first-line bevacizumab was combined with erlotinib or afatinib in EGFR-mutated NSCLC, demonstrating the development of T790M as resistance mechanism in 57.9% of cases. The addition of anti-angiogenic treatment in patients with an acquired resistance to Osimertinib was also demonstrated in a clinical case described by He W et al. In particular, a NSCLC patients harboring acquired EGFR 19Del/T790M/cis-C797S mutation resistance was treated with sintilimab, an anti-VEGF drug and chemotherapy. The patient remained progression-free for 15 months and the regimen was well tolerated. The addition of immunotherapy in EGFR-mutated patients is a controversial topic (3). Microsatellite instability is a rare event in NSCLC, but Yang M et al reported a case of apatients with a rare pulmonary enteric adenocarcinoma with EGFR mutation and MSI-H, who receive benefit from a combination approach of EGFR-TKI plus immune check point inhibitor.Rare EGFR mutations are another important issue object of several studies in the last years. It was reported that afatinib, an irreversible ErbB family inhibitor, is more efficacious in treating patients with uncommon EGFR mutations (4). Dong W et al performed a literature search on studies evaluating the efficacy of afatinib in any line of treatment in NSCLC patients with uncommon EGFR mutations. Their conclusion was that afatinib seems to be effective in patients with the more frequent uncommon EGFR mutations, whereas inconsistent data are present with regard to the other uncommon EGFR alterations, due to the high heterogeneity of them. Christopoulos P et al described seven clinical cases of patients carrying different uncommon EGFR mutations treated with afatinib. Overall, patients responded to afatinib, with six partial response and three durable responses. Interestingly, co-mutations did not preclude sensitivity to the drug, with durable response observed also in patients exhibiting cooccurring TP53 or CDKN2A mutations. Another case report was reported by Wang P et al, describing a patient carrying two exon 18 mutations and who acquired an EGFR amplification after treatment with osimertinib. The patient showed a partial response to neratinib. Also, a further case report of a patient carrying an EGFR kinase domain duplication was reported by Lin H and colleagues. Abstract:Third-generation tyrosine kinase inhibitors (TKIs), such as osimertinib, almonertinib and furmonertinib, overcome the mechanisms of resistance to first-generation inhibitors (such as gefitinib, erlotinib and icotinib) by incorporating an acrylamide group that alkylates the Cys797 of EGFR T790M. However, drug resistance is inevitable, even for third-generation TKIs. Screening for drug-resistant mutations by repeat biopsy and repeat gene sequencing is necessary after TKI treatment. Among various third-generation TKI-resistant mutations, secondary mutation of the L718 residue of EGFR exon 18 was found in approximately 8% of patients and is responsible for drug resistance in vitro and in vivo . Furthermore, there is limited clinical experience of targeted therapy for this mutation. Herein, we report for the first time that afatinib and cetuximab combination therapy can overcome such drug resistance.

Acquired BRAF V600E Mutation as Resistant Mechanism after Treatment with Osimertinib

Abstract:Introduction: AZD9291 (osimertinib) is designed for acquired T790M mutation after first- and second-generation EGFR) tyrosine kinase inhibitors have been used. Some of the resistance mechanisms that present after osimertinib treatment, including a newly acquired EGFR C797S mutation, have been identified. It is unclear, however, whether the bypass pathway is also a mechanism of resistance in patients after osimertinib treatment. Methods: Cells from malignant pleural effusion were collected and cultured at the time of progression in a patient being treated with osimertinib. Tumor genotyping was done by matrix-assisted laser desorption ionization-time of flight mass spectrometry. EGFR, AKT, MEK, and ERK phosphorylation were determined. An anchorage-dependent colony formation assay was used for drug sensitivity. Results: An acquired mutation, BRAF V600E, was found in the patient at the time of progression while being treated with osimertinib. Cells grown from malignant pleural effusion were sensitive to BRAF V600E inhibitor and were more vulnerable to a combination treatment with osimertinib. Conclusions: A potential mechanism of acquired resistance to osimertinib in patients with T790M is through the BRAF pathway. Simultaneous blockade of the BRAF and EGFR had a significant inhibitory effect.

Abstract:Up to 20% of all non-small cell lung cancer patients harbor tumor specific driver mutations that are effectively treated with tyrosine kinase inhibitors. However, for the rare EGFR deletion-insertion mutation of exon 18, there is very little evidence regarding the effectiveness of tyrosine kinase inhibitors. A particular challenge for clinicians in applying tyrosine kinase inhibitors is not only diagnosing a mutation but also interpreting rare mutations with unclear therapeutic significance. Thus, we present the case of a 65-year-old Caucasian male lung adenocarcinoma patient with an EGFR Exon 18 p.Glu709_Thr710delinsAsp mutation of uncertain therapeutic relevance. This patient initially received two cycles of standard platinum-based chemotherapy without any therapeutic response. After administration of Osimertinib as second line therapy, the patient showed a lasting partial remission for 12 months. Therapy related toxicities were limited to mild thrombocytopenia, which ceased after dose reduction of Osimertinib. To our knowledge, this is the first report of effective treatment of this particular mutation with Osimertinib. Hence, we would like to discuss Osimertinib as a viable treatment option in EGFR Exon 18 p.Glu709_Thr710delinsAsp mutated lung adenocarcinoma.

However, almost all patients become resistant to treatment. In some patients, emergence of tertiary EGFR mutations is implicated as a resistance mechanism. This study describes patients with NSCLC who acquired the rare EGFR mutations, L718Q or G724S, following EGFR TKI treatment. Patients and Methods: This was a retrospective, observational study undertaken in France from Feb–Nov 2021, in patients with EGFR mutation-positive NSCLC with an acquired L718Q or G724S mutation. Primary objectives were description of tumor characteristics, progression, and progression under treatment. Results: Nine eligible patients were identified. Acquired resistance to initial EGFR TKI treatment was associated with T790M emergence in six patients, who then received osimertinib monotherapy. Overall, eight patients received osimertinib monotherapy treatment at some point (average treatment duration: 18.3 months). Following the emergence of L718Q or G724S, patients received chemotherapy (n = 4; two of whom subsequently received afatinib), nivolumab (n = 2), afatinib (n = 2), or immunochemotherapy (n = 1). In the four patients who received afatinib after identification of L718Q or G724S, 2 achieved a partial response, one had stable disease and one had progressive disease. Treatment duration was 1.6– 31.7 months. In patients with controlled disease (n = 3), progression-free survival was 6.1– 31.7 months. Two of these patients had previously received osimertinib. Conclusion: Currently, there is no consensus regarding the treatment of EGFR mutation-positive NSCLC following emergence of the osimertinib resistance mutations, L718Q or G724S. Afatinib appears to be a promising treatment option in this setting. Three generations of EGFR TKI are available which have different mechanisms of action; the first-generation reversible EGFR TKIs (erlotinib and gefitinib), the second-generation ErbB family blockers (afatinib and dacomitinib), and the third-generation irreversible EGFR TKI, osimertinib. 2 Head-to-head prospective trials have demonstrated the superiority of second- and third-generation TKIs over first-generation EGFR TKIs. 3–5 Based on the Phase III FLAURA trial, 4,6 which demonstrated overall survival (OS) benefit with osimertinib versus erlotinib/gefitinib and favorable tolerability, osimertinib is often used as treatment of choice for EGFR mutation-positive NSCLC.
