Abstract:To determine the predictors of outcome after blunt diaphragmatic rupture, the records of 44 patients aged 15-70 years (84.1% male) who underwent diaphragm repair between 1998 and 2007 were reviewed. There were 38 (86.4%) cases of acute diaphragmatic rupture and 6 (13.6%) of post-traumatic diaphragmatic hernia. The injury was left-sided in 30 (68.2%) patients, right-sided in 12 (27.3%), and bilateral in 2 (4.5%). The diagnosis of acute diaphragmatic rupture was made preoperatively in 28 (73.7%) patients and intraoperatively in 10 (26.3%). Repair was carried out through a thoracotomy in 31 cases, laparotomy in 4, and thoracolaparotomy in 3. The mortality rate after diaphragmatic rupture was 13.2% (5/38). The 33 survivors had a significantly younger mean age (37 vs. 57 years) and lower incidences of multiple injuries (36.4% vs. 100%) and shock (24.2% vs. 100%). Repair of diaphragmatic hernia was performed through a thoracotomy in all cases, with no mortality. It was concluded that age, associated severe multiple injuries, and clinical status were predictors of mortality after blunt diaphragmatic rupture.

Abstract:57-year-old man was involved in a head-on motor vehicle collision. His injuries included closed head injury, pulmonary contusion, and fractures involving the femur and pelvis. Acute intraabdominal hemorrhage was excluded by diagnostic peritoneal lavage. Initial chest radiography showed opacification of the left lower lobe with silhouetting of the left diaphragm (Fig. 1A). The patient developed acute lung injury that required ventilation with positive endexpiratory pressure levels of 15-cm H 2 O for adequate oxygenation. By the fourth hospital day the positive end-expiratory pressure was removed. At that time, repeated chest radiography again showed dense opacification of the left lower lung, but also showed interval malpositioning of the nasogastric tube overlying the thorax, consistent with diaphragmatic rupture with gastric herniation (Fig. 1B). At surgery, a 10-cm tear in the left hemidiaphragm with herniation of the stomach and spleen was found and repaired. Diaphragmatic rupture is more likely due to blunt injury than from penetrating trauma. The 0.8‐1.6% incidence of diaphragmatic rupture in all hospital admissions rises to 4.5% in severely injured patients [1, 2]. Most penetrating injuries are smaller than 2 cm. The larger tears associated with blunt injury result in a higher incidence of visceral herniation in this group. Ruptures from blunt mechanisms of injury often measure up to 10 cm in length, as in this case, and present acutely with herniation [2, 3]. The diaphragm separates the intraabdominal viscera, which are under positive pressure, from the negative pressure within the thoracic cavity. At rest, the pressure gradient varies from 7- to 20-cm H 2 O [2]. Once a defect is created, abdominal contents compelled by this pressure gradient can migrate through the rent into the thorax. This migration is aggravated by any increase in intraabdominal pressure such as that caused by bowel edema or retroperitoneal hemorrhage. In the proper clinical setting, a chest radiograph showing a nasogastric tube, with an otherwise expected course to the region of the gastroesophageal junction, apparently misplaced over the thorax is nearly pathognomonic of visceral herniation [4]. Diaphragmatic eventration can have a very similar and confusing appearance. Small lacerations are believed to increase over time because of the radial tension in the central diaphragm, with resultant late or delayed herniation usually within 2 years of the injury [1, 2]. These do not represent late occurrence of diaphragmatic breakdown, but only late recognition of what was a small injury initially. In this case, despite a large laceration of the hemidiaphragm, positive pressure ventilation was enough to maintain the intraabdominal position of the abdominal viscera. As positive intrathoracic pressure was decreased, the abdominal contents herniated into the thorax. When there is an apparent delayed-interval diaphragmatic rupture with intrathoracic visceral herniation, the radiographic findings should be correlated with changes in ventilator management for accurate interpretation.

Abstract:A unique glutamic acid-rich protein was previously identified in bovine rod photoreceptors (Sugimoto et al., 1991, Proc. Natl. Acad. Sci. USA 88: 3116-3119) and later suggested to be a third subunit (gamma) of the rod cGMP-gated cation channel (Chen et al., 1994, Proc. Natl. Acad. Sci. USA 91: 11757-11761). Here, we report on the characterization of the GAR1 gene encoding a human homolog of bovine gamma. Sequence analysis of cDNA clones encoding human gamma revealed an open reading frame predicting a protein of 299 amino acids (approximately 32 kDa), half the size of the bovine gamma subunit. Comparison of the N-terminal half of bovine gamma with the predicted human gamma sequence revealed 90% identity within the first 31 amino acids, and only 60% homology was found throughout the remainder of the protein sequence. As in bovine gamma, the predicted isoelectric point of the human protein is very acidic despite the absence of the bovine C-terminal glutamic acid-rich domain. The integrity of the cDNA sequence was confirmed by analysis of several overlapping genomic clones that span the GAR1 gene. The protein coding region of the gene consists of 12 exons spanning approximately 11 kb with exon sequence identical to that of the cDNA clones. PCR of somatic cell hybrid DNA with primer pairs that amplify a portion of the GAR1 gene (locus designation CNCG3L) demonstrate localization to chromosome 16. Diagnosis and treatment of blunt diaphragmatic rupture:an analysis of 36 cases

Abstract:Objective: To study the clinical diagnosis and surgical intervention of blunt traumatic diaphragmatic rupture.Methods: The clinical data of 36 patients with traumatic diaphragmatic rupture from January 2000 to December 2008 were retrospectively reviewed,including etiology,traumatic position,complicated injury,score of injury severity,diagnosis course and surgical treatment.In the series,there were 29 cases of left-diaphragmatic rupture,6 right diaphragmatic rupture and 1 case in bilateral.All the patients received operation.Thoracotomy was performed in 6 patients,laparotomy in 30,thoracotomy plus laparotomy in 2 and combined thoraco-laparotomy in 1 case.Results: Thirty-four cases were cured.One patient died of liver rupture,hemorrhagic shock,wound shock and DIC,and one of severe cerebral injury and MODS.Conclusion: Diaphragmatic rupture caused by blunt injury could lead to severe thoracoabdominal injury.In spite of high mortality rate,the condition is usually under diagnosed.Early diagnosis and active operation including definite treatment of complicated injures are the keys to increase survival rate and reduce mortality.
Grave thoraco-intestinal complication secondary to an undetected traumatic rupture of the diaphragm: a case report

Abstract:Background: Diaphragmatic lesions as a result of blunt or penetrating trauma are challenging to detect in the initial trauma setting. This is especially true when diaphragmatic trauma is part of a polytrauma. Complications of undetected diaphragmatic defects with incarcerating bowel are rare, but as in our patient can be serious. Case presentation: A 57-year-old female presented to the Emergency Room of our Hospital in a critical condition with 3 days of increasing abdominal pain. The initial clinical examination showed peritonism with tinkling peristaltic bowel sounds of mechanical obstruction. A thoraco-abdominal CT scan demonstrated colon prolapsed through the left diaphragmatic center with a large sero-pneumothorax under tension. As the patient was hemodynamically increasingly unstable with developing septic shock, an emergency laparotomy was performed. After retraction of the left colon, which had herniated through a defect of the tendinous center of the left diaphragm and was perforated due to transmural ischemia, large amounts of feces and gas discharged from the left thorax. A left hemicolectomy resulting in a Hartmann-type procedure was performed. A fully established pleural empyema required meticulous debridement and lavage conducted via the 7-10 cm in diameter phrenic opening followed by a diaphragmatic defect reconstruction. Due to pneumonia and recurring pleural empyema redo-debridement of the left pleural space via thoracotomy were required. The patient was discharged on day 56. A thorough history of possible trauma revealed a bicycle-fall trauma 7 months prior to this hospitalization with a surgically stabilized fracture of the left femur and conservatively treated fractures of ribs 3-9 on the left side. Conclusion: This is the first report on a primarily established empyema at the time of first surgical intervention for feco-pneumothorax secondary to delayed diagnosed diaphragmatic rupture following abdomino-thoracic blunt trauma with colic perforation into the pleural space, requiring repetitive surgical debridement in order to control local and systemic sepsis. Thorough investigation should always be undertaken in cases of blunt abdominal and thoracic trauma to exclude diaphragmatic injury in order to avoid post-traumatic complications.
Ruptured diaphragmatic hernia with grade I splenic injury: A case report

Abstract:Introduction and importance: Traumatic diaphragmatic injury (TDI), although rare, is associated with high mortality and morbidity and timely recognition is important. Case presentation: We present a case of a 44-year-old male who fell from a three-story building and presented with complaints of shortness of breath and chest pain. On examination, his chest compression test and extended focused assessment with sonography in trauma (E-FAST) were positive. Chest X-ray showed loss of diaphragmatic contour, fracture of fifth to eleventh ribs on the left side, and opacities in the left hemithorax. Contrast-enhanced computed tomography of chest, abdomen, and pelvis showed bowel loops over the left hemithorax with grade 1 splenic injury. A diagnosis of ruptured diaphragmatic hernia with grade I splenic injury was made and emergency primary repair of the diaphragm was done via thoracotomy. Discussion: Clinical diagnosis of TDI is difficult and can be misdiagnosed as a pneumothorax. In addition, the subtle presentation can often be missed. CT scan of the chest and abdomen is the imaging of choice to reach a diagnosis. Once diagnosed, emergency surgery via laparotomy or thoracotomy is mandatory. Delay in diagnosis can have a fatal consequence or delayed complications which have high mortality. Conclusion: Diaphragmatic injury should be suspected in all blunt thoracoabdominal traumas, and the presence of this injury should be excluded to prevent late complications. Timely intervention can provide excellent outcomes.
Rupture of Main Bronchus after Blunt Chest Trauma in Children

Abstract:Objective To evaluate the clinical features of rupture of main bronchus after blunt chest trauma in children and the importance of early diagnosis and surgical treatment.Methods 11 consecutive patients from ages 3 to 7 years(5.1±1.4),with the main bronchial disruptions due to blunt chest trauma were reported(four right,seven left).8 boys and 3 girls were included.4 patients underwent immediate surgical correction (end-to-end anastomosis)within 24 hours after the accident.7 patients had a delayed diagnosis for 2-12 months after injury.Among these 7 patients,end￣to￣end anastomosis was used in 3 cases,four patients required total pneumonectomy.Results There were no deaths in this series.7 patients who received end-to-end anastomosis experienced an uneventful recovery.Conclusion Recognition of the clinical features of main bronchial rupture of main bronchus after blunt chest trauma in children,early diagnosis and bronchial repair could significantly decrease the incidence of complication and mortality.

Abstract:IntroductionDiaphragmatic injuries include wounds and diaphragm ruptures, due to a thoracoabdominal blunt or penetrating traumas. Their incidence ranges between 0.8 and 15 %. The diagnosis is often delayed, despite several medical imaging techniques. The surgical management remains controversal, particularly for the choice of the surgical approach and technique. The mortality is mainly related to associated injuries. The aim of our study was to evaluate the incidence of diaphragmatic injuries occuring in thoraco-abdominal traumas, and to discuss their epidemiology, diagnosis and treatment.Patients and methodsWe performed a retrospective study over a period of 21 years, between January 1994 and June 2015 at the Department of General Surgery of the Aristide Le Dantec hospital in Dakar, Senegal. All patients diagnosed with diaphragmatic injuries were included in the study.ResultsOver the study period, 1535 patients had a thoraco-abdominal trauma. There were 859 cases of blunt trauma, and 676 penetrating chest or abdominal trauma. Our study involved 20 cases of diaphragmatic injuries (1.3 %). The sex-ratio was 4. The mean age was 33 years. Brawls represented 83.3 % (17 cases). Stab attacks represented 60 % (12 cases). The incidence of diaphragmatic injury was 2.6 %. The wound was in the thorax in 60 % (seven cases). Chest radiography was contributory in 45 % (nine cases). The diagnosis of wounds or ruptures of the diaphragm was done preoperatively in 45 % (nine cases). The diaphragmatic wound was on the left side in 90 % (18 cases) and its mean size was 4.3 cm. The surgical procedure involved a reduction of herniated viscera and a suture of the diaphragm by “X” non absorbable points in 85 % (17 cases). A thoracic aspiration was performed in all patients. Morbidity rate was 10 % and mortality rate 5 %.ConclusionThe diagnosis of diaphragmatic rupture and wounds remains difficult and often delayed. They should be kept in mind in any blunt or penetrating thoraco-abdominal trauma. Diaphragmatic lesions are usually located on the left side. Surgery is an efficient treatment.
Transdiaphragmatic intercostal herniation in the setting of trauma

Abstract:Transdiaphragmatic intercostal herniation is a rare injury that can be associated with blunt trauma. Since its first documentation within the literature in 1946, there have been less than 50 cases reported. We present a case involving a 56-year old female who presented to our Trauma Center with transdiaphragmatic intercostal herniation caused by blunt trauma from a high-velocity T-bone vehicular collision. Upon presentation, she exhibited bilateral breath sounds; however, with labored breathing, chest pain, and hypoxia. The initial chest radiograph interpretation indicated the presence of "left lower lobe infiltrates", and subsequent computed tomography imaging identified "a small lateral hernia along the left mid abdomen". After initial resuscitation, her condition deteriorated, exhibiting respiratory distress and becoming increasingly hypercarbic, requiring intubation. Review of the imaging showed disruption of the left hemidiaphragm with intrathoracic herniation of colon and stomach through the thoracic wall between the ninth and tenth ribs. Consequently, a thoracotomy was performed in the operating room, revealing a large defect between the two ribs with disruption of the intercostal muscles and inferior displacement of rib space. Lung and omentum had herniated through the disrupted rib space and the diaphragmatic rupture was attenuated anteriorly, measuring 11x6cm. After reduction of the herniated organs, a biologic porcine mesh was placed and an intermediate complex closure of the thoracic wall hernia was performed. The patient was later extubated, recovered from her injuries with no complications and was discharged. With the low incidence of transdiaphragmatic intercostal herniation, there is no standardized surgical management. Recent literature suggests that these injuries should be managed with mesh, rather than sutures only, due to high rates of recurrence. Furthermore, diaphragmatic injuries may suffer a delay in diagnosis. Therefore, a high index of suspicion should be maintained in patients with respiratory distress following a blunt trauma, with close review of computed tomography.

Factors affecting mortality in traumatic diaphragm ruptures

Abstract:Traumatic diaphragm ruptures (TDR) are rarely seen. Although TDR does not cause morbidity in the acute period, undiagnosed TDR may cause clinical states, such as herniation, strangulation, pneumonia, pleural effusion, empyema, and cardiac tamponade, which have high morbidity and mortality rates in the late period. This study aims to evaluate the epidemiology, clinical characteristics, diagnosis and treatment methods of TDR encountered in thoracoabdominal trauma and to identify the factors affecting mortality. METHODS: A retrospective examination was carried out on the patients who were operated in our clinic because of traumatic diaphragm injury between January 2012 and December 2017. Each patient operated because traumatic diaphragm injury was evaluated in respect of age, gender, manner of injury, preoperative examination findings, laboratory test results, imaging methods, time of diagnosis, operation findings, concomitant injuries to other organs, operations performed, length of stay in hospital, the development of postoperative morbidity and mortality, and the calculated Abbreviated Injury Scale (AIS) and Injury Severity Score (ISS). RESULTS: Between January 2012 and December 2017, a total of 1066 patients were operated in our clinic because of thoracoabdominal trauma, and of 1066 patients, 45 of the patients were determined with TDR. Of the 45 patients, surgery was applied because of penetrating trauma in 32 cases (7 firearms injuries, 25 penetrating cutting injuries), blunt trauma in nine cases, and in four cases, diaphragm rupture was seen to have developed associated with iatrogenic injury during an operation. The most common injuries concomitant to traumatic diaphragm rupture were hemopneumothorax (70%), liver (43%), spleen (32%), colon (20%), stomach (17%) injuries and rib fractures (15%), respectively. Mortality developed in seven (17%) patients; five patients were lost because of hemorrhagic shock intraoperatively or in the early postoperative hours, and two because of multiorgan failure during follow-up in the intensive care unit. CONCLUSION: In high energy blunt and penetrating thoracoabdominal traumas, diaphragm injuries should be suspected. Factors affecting mortality were found to be the AISS, ISS, number of concomitant organ injuries and the combination with pneumohemothorax.
Life threatening rupture of the diaphragm after Covid 19 pneumonia: a case report

Abstract:Background: The incidence of diaphragmatic rupture is low; however, it may be life threatening. Normally caused by blunt trauma, some cases are reported after pulmonary infections with extensive coughing. Covid 19 causes pulmonary infections and pneumonia and has been associated with weakening of the diaphragm after prolonged ventilation. We present a patient who suffered from diaphragmatic rupture 2 months after recovering from a severe Covid 19 pneumonia. Case: A 71 years old male patient presented with massive thoraco-abdominal pain and severe dyspnea. At the time of admission, the patient was diagnosed with rupture of the diaphragm and developed cardiogenic shock. Intraoperatively there was a 4 cm diameter large rupture of the diaphragm with enterothorax (transverse colon, stomach, spleen, parts of the jejunum). Avulsion of the mesenteric arteries made a segmental resection of the jejunum together with the spleen necessary. A jejuno-jejunostomy was performed and organs were replaced into the abdomen. The rupture of the diaphragm underwent primary closure with non-resorbable suture material. The patient has shown an uneventful post-operative course, fully recovered and was discharged on day 11 after surgery. Conclusion: Covid 19 is a disease that is known to have various effects on different organs. The diaphragm is only paid heed in case of dysfunction. Also in the setting of Covid 19 it is not known as prominent effector organ. Nevertheless its affection by coughing caused by Covid 19 can lead to life threatening complications.
Sixty-three cases of traumatic injury of the diaphragm

Abstract:In the examination of patients with severe thoracic and/or abdominal trauma not requiring surgical exploration, special attention should be given to signs of traumatic diaphragmatic herniation (TDH). We analysed the hospital records of 63 patients with traumatic injuries of the diaphragm. Of these patients, 39 had suffered a blunt trauma in a traffic accident, 21 penetrating trauma, and three had fallen from a great height. There were 51 patients (81 per cent) with left-sided diaphragmatic injuries, ten (16 per cent) with right-sided injuries, and in two patients (3 per cent) the injuries to the diaphragm were bilateral. There were 22 patients (35 per cent) who had intrathoracic migration of abdominal viscera. Surgical treatment was given in all cases. Twelve patients (19 per cent) died due to massive haemorrhage, neurological lesions, or septicaemia. Based on the literature and our own experience, we developed a diagnostic protocol for the management of diaphragmatic injuries. Chest radiographs should be made routinely. If diaphragmatic injury is suspected, ultrasound investigation must be performed. If the physician is still in doubt, computed tomography should be performed. At laparotomy, the diaphragm should always be thoroughly examined for lacerations.

Abstract:Diaphragmatic injury secondary to blunt trauma is a relatively rare entity. Early diagnosis and appropriate management define patient outcomes. This set of images describes a gentleman in his late teens who was referred from another center after left-sided intercostal drainage for a blunt thoracoabdominal injury following a motor vehicle collision. A CT scan revealed suspicion of a diaphragmatic injury with gastric herniation and possible perforation; however, intra-operative findings revealed a distal esophageal perforation with a normal diaphragmatic contour. The radiological picture could be explained based on intra-operative findings, resulting in an interesting injury pattern.

The serum levels of cytomegalovirus (CMV) antibodies and the antibody titers of herpes simplex virus (HSV) type I has been determined by the ELISA test and the complement fixation method respectively. The viral factors that play a role in the pathogenesis of SS have been discussed in regard to the results of the study.

Goethe-University -a 1380-bed major tertiary care teaching hospital- during a study period of six month. During the study period a single physician evaluated all patients with signs and symptoms of an infection during his daily rounds. NI was defined as body temperature >38 degrees C and evidence of an infection not before the third day of admission to the hospital. NI was diagnosed in 127 patients (3.5%) of the 3605 patients studied. The data of 126 patients with NI could be collected and analyzed completely. Of the 126 patients 34 patients died. The mean length of hospitalization before the diagnosis of NI was 12.0 days (standard-deviation: +/-13.1 days; median: 7 days). Compared to all patients with NI significantly more patients of the Internal Intensive care unit (11.3%), of the HIV-ward (10.3%), and of the hematology / oncology ward (5.8%) acquired a NI (p<0.05). With respect to other groups of patients the frequency of NI ranged from 0.5 to 4.6 per 100 admitted inpatients. The lower rate was in patients admitted for invasive diagnostic procedures who were hospitalized only for 3 days or less.

Abstract:BACKGROUND: We present a discussion of the management of diaphragmatic injury and the factors that influence the choice of surgical approach based on our experience and a review of the literature.METHODS: Data of 41 patients with diaphragmatic injuries treated between 1996 and 2007 were analyzed retrospectively.RESULTS: A total of 29 men and 12 women between 15 and 56 years of age (mean age 34) were included in the study. Twenty-one patients had penetrating injuries and 20 patients had blunt injuries. The diagnosis was done preoperatively in 21 cases and intraoperatively in 20 cases. Thoracotomy was used in 23 cases, laparotomy in 11 cases, laparotomy-thoracotomy in 5 cases, sternotomy-laparotomy in 1 case, and sternotomy in 1 case. Emergency surgery was performed in 30 cases. Five cases were operated within 24 hours after admission to the emergency department. In 6 cases, surgery was performed 6 months to 4 years after the initial trauma. The mortality rate was 14.6 % and the operative morbidity was 2.4 %.CONCLUSION: A high index of suspect is vital for the diagnosis of diaphragmatic injuries in an emergency setting. The type of approach is closely related to the associated injuries.

Abstract:We report the case of a right diaphragmatic rupture associated with bilateral hemothorax and hemoperitoneum following closed thoracoabdominal trauma in an 11-year-old adolescent. The diagnosis was made intraoperatively. Treatment consisted of a simple abdominal suture and thoracic drainage. The post-operative course was straightforward.

Abstract:Purpose: Over the last decade Surgical Stabilisation of Rib Fractures (SSFR) gained popularity in our hospital. With increased numbers, we noted that frequently injuries were missed during primary/secondary survey and radiological imaging that were found during the surgical procedure. With this observation, the research question was formulated: What is the value of diagnostics thoracotomy or thoracoscopy during surgical stabilisation of rib fractures? Methods: In a single-centre, retrospective study between February 2010 and December 2019, trauma patients who underwent Surgical Stabilisation of Rib Fractures (SSFR) and an inspection thoracotomy were included. All radiological injuries were compared with intraoperative findings. Missed injuries that were discovered during the surgical procedure that were not analysed during primary/secondary survey or on radiological imaging were recorded and retrospectively analysed by an independent radiologist. Results: Fifty-one patients were included. Eight patients had additional injuries; all had a diaphragmatic rupture, one patient had an additional stomach laceration, and another patient had a significant lung laceration in need of surgical repair. On a CT scan there are 7 signs of predictive value for a diaphragmatic rupture. Only 13 out of the total of 56 diaphragm rupture CT signs were confirmed on the primary CT scans of the eight patients with diaphragmatic injuries; therefore, still 77% of signs could not be confirmed by initial radiological findings. Conclusion: With the recent shift towards surgical stabilisation of rib fractures, an inspection thoracoscopy or thoracotomy during SSFR should be considered to minimise the incidence of missed intrathoracic injuries requiring early or late surgical treatment.

Abstract:PurposeWe review our 11-year experience of treating diaphragmatic injury (DI), to identify the factors determining mortality and morbidity.MethodsWe analyzed the effects of demographic characteristics, type of injury (blunt or penetrating), number of injured organs, injury severity score (ISS), revised trauma score (RTS), Glasgow coma score, and intensive care unit and hospital stay, on complications and mortality, in 51 patients treated for DI between January 1995 and December 2005.ResultsTwenty-six (51%) patients suffered blunt injury and 25 (49%) suffered penetrating injury. The left diaphragm was injured in 40 (78%) patients, the right in 10 (19%), and both sides in 1 (2%). Only three (5.8%) patients had no concomitant injury. The diagnosis was made by the findings of laparotomy on 34 patients (65%), preoperative chest X-ray on 13 (25%), computed tomography on 2 (3.9%), and laparoscopy on 2 (3.9%). Complications developed in 23 (44%) patients and overall mortality was 19.6% (10/51). An ISS > 13 was found to be an independent prognostic factor for morbidity, whereas an RTS ≤ 11, age ≥ 48 years, and a major postoperative complication were independent prognostic factors for mortality.ConclusionEstablishing a preoperative diagnosis of DI is still problematic. Aggressive treatment and close monitoring of patients with an ISS > 13, an RTS ≤ 11, an age ≥ 48 years, or a postoperative complication may decrease morbidity and mortality.
