Abstract:Study Design. Retrospective cohort. Objective. To identify factors that were independently associated with increased postoperative length of stay (LOS) and readmission in patients who underwent elective laminectomy for lumbar spinal stenosis. Summary of Background Data. Lumbar spinal stenosis is a common pathology that is traditionally treated with decompressive laminectomy. Risk factors associated with increased LOS and readmission have not been fully characterized for laminectomy. Methods. Patients who underwent laminectomy for lumbar spinal stenosis during 2011 and 2012 were identified from the American College of Surgeons National Surgical Quality Improvement Program database. Patient characteristics were tested for association with LOS and readmission using bivariate and multivariate analyses. Patients with LOS more than 10 days were excluded from the readmission analysis as the American College of Surgeons National Surgical Quality Improvement Program only captures readmissions within 30 postoperative days, and the window for potential readmission was deemed too short for patients staying longer than 10 days. Results. A total of 2358 patients who underwent laminectomy met inclusion criteria. The average age was 66.4 ± 11.7 years (mean ± standard deviation). Average postoperative LOS was 2.1 ± 2.6 days. Of those meeting criteria for readmission analysis, 3.7% of patients (86 of 2339) were readmitted within 30 days postoperatively. Independent risk factors for prolonged LOS were increased age (P < 0.001), increased body mass index (P = 0.004), American Society of Anesthesiologists class 3–4 (P = 0.005), and preoperative hematocrit less than 36.0 (P = 0.001). Independent risk factors for readmission were increased age (P = 0.013), increased body mass index (P = 0.040), American Society of Anesthesiologists class 3–4 (P < 0.001), and steroid use (P = 0.001). The most common reason for readmission was surgical site-related infections (25.0% of patients readmitted in 2012). Conclusion. The identified factors associated with LOS and readmission after lumbar laminectomy may be useful for optimizing patient care. Level of Evidence: 3

Abstract:Globally, increasing rates of obesity are one of the most important health issues. The association between breakfast skipping and body weight is contradictory between cross-sectional and interventional studies. This systematic review and meta-analysis aims to summarize this association based on observational longitudinal studies. We included prospective studies on breakfast skipping and overweight/obesity or weight change in adults. The literature was searched until September 2020 in PubMed and Web of Science. Summary risk ratios (RRs) or β coefficients with a 95% confidence interval (CI), respectively, were estimated in pairwise meta-analyses by applying a random-effects model. In total, nine studies were included in the systematic review and three of them were included in the meta-analyses. The meta-analyses indicated an 11% increased RR for overweight/obesity when breakfast was skipped on ≥3 days per week compared to ≤2 days per week (95% CI: 1.04, 1.19, n = two studies). The meta-analysis on body mass index (BMI) change displayed no difference between breakfast skipping and eating (β = −0.02; 95% CI: −0.05, 0.01; n = two studies). This study provides minimal evidence that breakfast skipping might lead to weight gain and the onset of overweight and obesity.

Effect of breakfast on weight and energy intake: systematic review and meta-analysis of randomised controlled trials

Abstract:Abstract Objective To examine the effect of regular breakfast consumption on weight change and energy intake in people living in high income countries. Design Systematic review and meta-analysis. Data sources PubMed, Ovid Medline, and CINAHL were searched for randomised controlled trials published between January 1990 and January 2018 investigating the effect of breakfast on weight or energy intake. ClinicalTrials.gov and the World Health Organization’s International Clinical Trials Registry Platform search portal were also searched in October 2018 to identify any registered yet unpublished or ongoing trials. Eligibility criteria for selecting studies Randomised controlled trials from high income countries in adults comparing breakfast consumption with no breakfast consumption that included a measure of body weight or energy intake. Two independent reviewers extracted the data and assessed the risk of bias of included studies. Random effects meta-analyses of the effect of breakfast consumption on weight and daily energy intake were performed. Results Of 13 included trials, seven examined the effect of eating breakfast on weight change, and 10 examined the effect on energy intake. Meta-analysis of the results found a small difference in weight favouring participants who skipped breakfast (mean difference 0.44 kg, 95% confidence interval 0.07 to 0.82), but there was some inconsistency across trial results (I 2 =43%). Participants assigned to breakfast had a higher total daily energy intake than those assigned to skip breakfast (mean difference 259.79 kcal/day, 78.87 to 440.71; 1 kcal=4.18 kJ), despite substantial inconsistency across trial results (I 2 =80%). All of the included trials were at high or unclear risk of bias in at least one domain and had only short term follow-ups (mean period seven weeks for weight, two weeks for energy intake). As the quality of the included studies was mostly low, the findings should be interpreted with caution. Conclusion This study suggests that the addition of breakfast might not be a good strategy for weight loss, regardless of established breakfast habit. Caution is needed when recommending breakfast for weight loss in adults, as it could have the opposite effect. Further randomised controlled trials of high quality are needed to examine the role of breakfast eating in the approach to weight management. Breakfast Skipping, Body Composition, and Cardiometabolic Risk: A Systematic Review and Meta‐Analysis of Randomized Trials

Abstract:ObjectiveThe objective of this study was to evaluate the effect of skipping breakfast on body composition and cardiometabolic risk factors. MethodsThis study conducted a systematic review and meta‐analysis of randomized controlled trials (RCTs) evaluating breakfast skipping compared with breakfast consumption. Inclusion criteria included age ≥ 18, intervention duration ≥ 4 weeks, ≥ 7 participants per group, and ≥ 1 body composition measure. Random‐effects meta‐analyses of the effect of breakfast skipping on body composition and cardiometabolic risk factors were performed. ResultsSeven RCTs (n = 425 participants) with an average duration of 8.6 weeks were included. Compared with breakfast consumption, breakfast skipping significantly reduced body weight (weighted mean difference [WMD] = −0.54 kg [95% CI: −1.05 to −0.03], P = 0.04, I2 = 21.4%). Percent body fat was reported in 5 studies and was not significantly different between breakfast skippers and consumers. Three studies reported on low‐density lipoprotein cholesterol (LDL), which was increased in breakfast skippers as compared with breakfast consumers (WMD = 9.24 mg/dL [95% CI: 2.18 to 16.30], P = 0.01). Breakfast skipping did not lead to significant differences in blood pressure, total cholesterol, high‐density lipoprotein (HDL) cholesterol, triglycerides, C‐reactive protein, insulin, fasting glucose, leptin, homeostatic model assessment of insulin resistance, or ghrelin. ConclusionsBreakfast skipping may have a modest impact on weight loss and may increase LDL in the short term. Further studies are needed to provide additional insight into the effects of breakfast skipping.

Abstract:Skipping breakfast is one of the most prevalent irregular eating habits. Several pieces of evidence have reported the association between breakfast omission and a higher risk of cardiovascular diseases. Numerous publications have focused on the impact of skipping breakfast on various cardiovascular risk factors. Therefore, the current systematic review and meta-analysis aimed to assess this impact, especially with regard to anthropometric measurements, serum lipid profiles, blood pressure, and glycemic control indicators. A comprehensive search was performed in PubMed, Web of Science, Embase, Scopus, and the Cochrane Central Register of Controlled Trials up to 1 April 2023. A total of 11 eligible trials were identified to evaluate the combined effects of skipping breakfast. Final integrated results demonstrated that breakfast omission significantly decreased the body weight (mean difference = -0.66, 95% CI: -1.09 to -0.24, p = 0.002, I2 = 0.0) and increased the level of serum low-density lipoprotein cholesterol (LDL-C) (mean difference = 9.89, 95% CI: 5.14 to 14.63, p = 0.000, I2 = 17.3). Subgroup analysis also revealed potential factors that may affect the outcomes, for example, the physiological condition of participants, duration, gender, and type of breakfast. In conclusion, skipping breakfast may reduce body weight while increasing the level of serum LDL-C at the same time. In view of the limited trials, further studies are needed to expound the role of breakfast omission in cardiovascular diseases.

Abstract:"Eat breakfast like a king, lunch like a prince and dinner like a pauper" (Adelle Davis, 1904–1974) is a concept that appears to align with some contemporary evidence concerning the appropriate proportioning of daily meals. At the same time, with the popular and scientific dissemination of the concepts of intermittent fasting and time-restricted feeding, well-controlled clinical trials have emerged showing the safety or even possible benefits of skipping breakfast. In this comprehensive literature review, we discuss recent evidence regarding breakfast intake, cardiovascular outcomes and cardiovascular risk markers. Overall, breakfast omission appears to be associated with a higher risk for atherosclerotic and adverse cardiovascular outcomes. However, caution should be employed when deciphering these data as many complex, unmeasured confounders may have contributed. Unfortunately, long-term randomized, clinical trials with detailed dietary control that have assessed clinical outcomes are sparse. Notwithstanding the observational findings, current trials conducted so far—albeit apparently smaller number—have shown that breakfast addition in subjects who do not habitually consume this meal may increase body weight, particularly fat mass, through caloric excess, whereas skipping breakfast may be a feasible strategy for some people aiming for calorie restriction. To date, definitive benefits of breakfast omission or consumption are not supported by the best evidence-based research, and the question of whether skipping breakfast per se is causally associated with cardiovascular outcomes remains unresolved.

Breakfast Skipping is Positively Associated with Incidence of Type 2 Diabetes Mellitus: Evidence from the Aichi Workers' Cohort Study.

Abstract:Background: Skipping breakfast has been suspected as a risk factor for type 2 diabetes (T2DM), but the associations are not entirely consistent across ethnicities or sexes, and the issue has not been adequately addressed in the Japanese population.Methods: We followed 4631 participants (3600 men and 1031 women) in a work-site cohort of participants aged 35-66 years in 2002 through 2011 for T2DM development. Frequency of eating breakfast was self-reported and was subsequently dichotomized to breakfast skippers, who eat breakfast 3-5 times/week or less, and to eaters. Cox proportional hazards models were used to adjust for potential confounding factors, including dietary factors, smoking and other lifestyles, body mass index (BMI), and fasting blood glucose (FBG) at baseline.Results: During 8.9 years of follow-up, 285 T2DM cases (231 men and 54 women) developed. Compared to participants who reported eating breakfast every day, maximally-adjusted hazard ratios and 95% confidence intervals (CI) of those with the frequency of almost every day and 3-5, 1-2, and 0 days/week were: 1.06 (95% CI, 0.73-1.53), 2.07 (95% CI, 1.20-3.56), 1.37 (95% CI, 0.82-2.29), and 2.12 (95% CI, 1.19-3.76), respectively. In a dichotomized analysis, breakfast skipping was positively associated with T2DM incidence (maximally-adjusted hazard ratio 1.73; 95% CI, 1.24-2.42). The positive associations were found in both men and women, current and non-current smokers, normal weight and overweight (BMI = 25 kg/m(2)), and normal glycemic status and impaired fasting glycemic status (FBG 110 to < 126 mg/dL) individuals at baseline (Ps for interaction all > 0.05).Conclusions: The present study in middle-aged Japanese men and women suggests that skipping breakfast may increase the risk of T2DM independent of lifestyles and baseline levels of BMI and FBG.
The link between breakfast skipping and overweigh/obesity in children and adolescents: a meta-analysis of observational studies

Abstract:<h3 class="Heading">Purpose</h3><p class="Para">Childhood overweight/ obesity is one of critical public health concern. It has been suggested that there is a link between breakfast skipping and obesity. However, results are conflicting. The aim of the present study was to summarize the association between breakfast skipping and overweight/obesity in children and adolescent.</p><h3 class="Heading">Methods</h3><p class="Para">We performed a literature search using Pubmed/Medline, Scopus, Web of Science and EMBASE electronic databases from 2000 through 28 February 2018 without language limitation. Observational studies in which risk measures were reported regarding the link between breakfast skipping and obesity in children and adolescent were included. Studies with at least the score of 5 from Newcastle-Ottawa Scale were considered as low risk of bias. Random effect model was used for data synthesis.</p><h3 class="Heading">Results</h3><p class="Para">Of 3276 publications, finally 16 studies (14 cross-sectional studies, 2 cohort studies) were included for meta-analysis. Based on cross-sectional studies, we found a positive association between breakfast skipping and obesity (Odd ratio (OR) <sub>trim & fill</sub>: 1.43; 95%CI: 1.32, 1.54), while cohort studies showed no significant link (OR:1.01, 95%CI: 0.93, 1.11; I2: 48%, <em class="EmphasisTypeItalic">p</em> = 0.14). Subgroup analysis in cross-sectional studies showed that the association between breakfast skipping and the risk of obesity in boys was OR: 1.64; 95% CI: 1.38, 1.95; I<sup>2</sup>: 38.3%, <em class="EmphasisTypeItalic">p</em> = 0.18, while it was 1.56 (95% CI: 1.38, 1.77, I<sup>2</sup>: 0.0%, <em class="EmphasisTypeItalic">p</em> = 0.49) in girls.</p><h3 class="Heading">Conclusion</h3><p class="Para">The risk of obesity in children and adolescents who skipped breakfast was 43% greater than those who ate breakfast regularly in cross-sectional studies, while no significant link was found in cohort studies. However, due to high heterogeneity and limited cohort studies, findings should be interpreted by caution.</p>

Abstract:Breakfast skipping increases with age, and an association with a high risk of being overweight (OW) and of obesity (OB), cardiometabolic risk, and unhealthy diet regimen has been demonstrated in observational studies with children and adults. Short-term intervention trials in adults reported conflicting results. The purpose of this systematic review was to summarize the association of breakfast skipping with body weight, metabolic features, and nutrition quality in the groups of young people that underwent randomized controlled (RCT) or intervention longitudinal trials lasting more than two months. We searched relevant databases (2000–2021) and identified 584 articles, of which 16 were suitable for inclusion. Overall, 50,066 children and adolescents were included. No studies analyzed cardiometabolic features. Interventions were efficacious in reducing breakfast skipping prevalence when multi-level approaches were used. Two longitudinal studies reported a high prevalence of OW/OB in breakfast skippers, whereas RCTs had negligible effects. Ten studies reported a lower-quality dietary intake in breakfast skippers. This review provides insight into the fact that breakfast skipping is a modifiable marker of the risk of OW/OB and unhealthy nutritional habits in children and adolescents. Further long-term multi-level intervention studies are needed to investigate the relationship between breakfast, nutrition quality, chronotypes, and cardiometabolic risk in youths.

Abstract:Eating breakfast to control weight is a strategy presumed true without sufficient supporting evidence. We investigated whether biased reporting in primary literature exaggerates the protective effect of breakfast against obesity. From 92 identified studies of breakfast and obesity (all observational), ORs were calculated and meta‐analyzed from 88 study groups in 57 publications representing 30 countries. The resulting p‐value of p<10 −42 demonstrates gratuitous repetition of the breakfast‐obesity presumption. No publication bias was detected. Analysis of 88 of the 92 studies’ abstracts revealed that authors biased the reporting of their own findings: breakfast‐obesity associations were more likely to be mentioned in abstracts’ conclusions if results were pro‐breakfast, compared to associations that showed mixed, negative, or no relationships (65% vs. 39%, p=0.0492); and causal language was used in 26% of abstracts that mentioned breakfast and weight in the conclusions. Citing of others’ work was also biased: 67% of relevant citations of one breakfast RCT were overly positive toward breakfast (2% overly negative), while 48% of relevant citations of a case‐series paper inappropriately used associative or causal language. In conclusion, there is evidence of biases in the breakfast literature that misleadingly exaggerate the strength of the evidence for a presumed protective effect against obesity. Then the results of an initial comparison of the McCann99 Retinex and iCAM algorithms for use in rendering three high dynamic range gray-scale images are reported. The images are also rendered using an image-specific manual tone curve. Ten observers performed a rank ordering of the printed renderings, and then mean rankings were computed. The results varied by image, with iCAM being most preferred for one of the images, McCann99 Retinex being most preferred for one of the images, and a tie for all three renderings for the third image. Abstract:The incidence of skipping breakfast in pediatric subjects is rising, and a relationship with overweight (OW) and obesity (OB) has been shown. Associations with cardiovascular outcomes and skipping breakfast in adults have been reported. The purpose of this systematic review was to summarize the association of skipping breakfast with body weight and metabolic outcomes in the pediatric population. We searched relevant databases (2008–2018) and identified 56 articles, of which 39 were suitable to be included, basing on inclusion criteria (observational; defined breakfast skipping; weight and/or metabolic outcomes). Overall, 286,804 children and adolescents living in 33 countries were included. The definitions of OW/OB, skipping breakfast, and the nutrient assessment were highly heterogeneous. Confounding factors were reported infrequently. The prevalence of skipping breakfast ranged 10–30%, with an increasing trend in adolescents, mainly in girls. Skipping breakfast was associated with OW/OB in the 94.7% of the subjects. The lack of association was shown mainly in infants. Moreover, 16,130 subjects were investigated for cardiometabolic outcomes. Skipping breakfast was associated with a worse lipid profile, blood pressure levels, insulin-resistance, and metabolic syndrome. Five studies reported a lower quality dietary intake in breakfast skippers. This review supports skipping breakfast as an easy marker of the risk of OW/OB and metabolic diseases, whether or not it is directly involved in causality. We encourage intervention studies using standardized and generalizable indicators. Data on confounders, time of fasting, chronotypes, and nutrition quality are needed to establish the best practice for using it as a tool for assessing obesity risk.

Abstract:Background : Numerous studies reported inconsistent association between breakfast skipping and all-cause, cardiovascular disease (CVD) and cancer mortality. Therefore, we conducted a systematic review and meta-analysis to elucidate these associations. Methods : PubMed, Embase, and Web of Science databases were searched up to July 2023 for prospective cohort studies that assessed the association between breakfast skipping and all-cause, CVD and cancer mortality in general adults. A random effect model was used to estimate the pooled hazard ratio (HR) and 95% confidence intervals (CIs), with subgroup analysis and sensitivity analysis performed. The Newcastle-Ottawa Scale (NOS) was used to assess the study and the Grading of Recommendations Assessment, Development and Evaluation (GRADE) tool was used to assess the risk of bias. Results : The final analysis included 9 cohort studies including 242 095 participants, with 6 studies for all-cause mortality, 4 studies for CVD mortality, and 2 studies for cancer mortality. Compared to regular breakfast consumption, skipping breakfast was associated with a higher risk of all-cause (HR: 1.27, 95% CI, 1.07–1.51, I 2 = 77%), CVD (HR 1.28, 95% CI 1.10–1.50, I 2 = 0), and cancer (HR: 1.34, 95% CI: 1.11–1.61, I 2 = 0%) mortality. Sensitivity analysis revealed inconsistent results in all-cause and CVD mortality. Subgroup analysis showed significant association in studies with larger participants, longer follow-up, adjustments for energy intake, and high-quality articles. GRADE showed very low evidence for all-cause mortality and low evidence for CVD and cancer mortality. Conclusion : The findings underscore the importance of regular breakfast habits for health and longevity. However, these results require careful interpretation due to geographic limitations, potential heterogeneity, and instability.

Abstract:The effect of skipping breakfast on health, specifically in adults, remains a debatable topic. The objective of this study was to evaluate the effects of breakfast skipping on lipid profile parameters, obesity and metabolic disorders. Two hundred subjects were divided into three different groups (regular, often and rare breakfast eaters). Three days’ dietary intake over one weekend day and two weekdays were collected from each subject using a 24-hour recall and a 2-day daily record. Total cholesterol (TC), high density lipoproteins (HDL), low density lipoproteins (LDL), triglycerides (TG), body weight, body mass index, waist circumference and blood pressure were analyzed. This study supports the hypothesis that breakfast skipping has impact on selected metabolic measures, nutrient intakes and body weight in university going adults.

Abstract:Randomised controlled trials comparing low-v. high-fat diets on cardiometabolic risk factors in people with overweight or obesity have shown inconsistent results, which may be due to the mixed metabolic status of people with excess adiposity. The role of dietary fat manipulation in modifying cardiometabolic indicators in people with overweight or obese without metabolic disturbance is unclear. Thus, meta-analysis was conducted to compare low-v. high-fat diets on cardiometabolic indicators in people who are overweight or obese without metabolic disturbance in the present study. Databases were searched until October 2016. The pooled effects of outcomes with heterogeneity were calculated with a random-effects model, heterogeneities were analysed by subgroup and meta-regression. As a result, twenty studies with 2106 participants were included in the meta-analysis. Total cholesterol and LDL-cholesterol levels were lower following low-fat diets compared with high-fat diets: weighted mean difference (WMD) was -7.05 mg/dl (-0.18 mmol/l; 95 % CI -11.30, -2.80; P=0.001) and -4.41 mg/dl (-0.11 mmol/l; 95 % CI -7.81, -1.00; P=0.011), respectively. Conversely, significant higher level of TAG (WMD: 11.68 mg/dl (0.13 mmol/l), 95 % CI 5.90, 17.45; P<0.001) and lower level of HDL-cholesterol (WMD: -2.57 mg/dl (-0.07 mmol/l); 95 % CI -3.85, -1.28; P<0.001) were found following low-fat diets compared with high-fat diets. In conclusion, dietary fat manipulation has a significant influence on blood lipid levels in people with overweight or obesity without metabolic disturbances.

Abstract:Background: Regular breakfast consumption is associated with better health status and healthier food intake throughout the day, but this association is a complex interaction of several factors. Objective: This study aimed to investigate the effect of nutritional and cognitive-perceived characteristics of breakfast on metabolic and behavioral variables related to food intake. Methods: The study was a randomized, crossover, controlled trial, with 4 experimental conditions consisting of 3 iso-energetic breakfasts and 1 energy-free control meal. Breakfasts had similar nutritional profiles but differed for glycemic index (GI), glycemic load (GL), and perceived healthiness, satiety, palatability, or energy content. Fifteen healthy normal-weight men [means ± SDs; age: 24 ± 2 y; body mass index (BMI; kg/m2) 23.4 ± 1.6] underwent each experimental condition in random order during 4 different weeks, separated by ≥1-wk washout. On the third day of each intervention week, postprandial blood variables (with insulin as primary outcome), satiety ratings, and food intake during an ad libitum lunch consumed 4 h after breakfast (secondary outcomes) were measured for each experimental condition. Results: A main effect of time, treatment, and time × treatment was found for postprandial insulin, glucose, and nonesterified fatty acids (P < 0.001 for all) after having the 3 iso-energetic breakfasts or the energy-free control one. Postprandial satiety was similar for the 3 energy-containing breakfasts, but higher when compared with the energy-free control (P < 0.001). No difference in energy intake was observed for the ad libitum lunch, whereas prolonged breakfast skipping was compensated by an increase (around +10%) in the average energy intake during the rest of the day, resulting in no differences in the total daily energy intake among the 4 conditions. Conclusions: Although other advantages might exist for breakfasts based on low-GI/low-GL foods, our findings support the hypothesis that minor differences in nutritional and perceived characteristics of breakfast are of limited importance regarding medium-term energy intake in healthy men. Abstract:This paper presents a novel two-stage low-frequency square-wave-driven electronic ballast for high-intensity-discharge (HID) lamps. The ballast is comprised of a PFC stage to achieve power factor correction and a low/high-frequency-driven half-bridge inverter to supply low-frequency square-wave sources to the lamp without causing acoustic resonance. The features of the proposed ballast are high power factor (>0.99), cost-effectiveness, and lack of acoustic resonance. This paper also presents operational principles and experimental results for the proposed ballast for one 70 W HID lamp operating at 200 Hz switching frequency with 90 Vrms to 265 Vrms input utility-line voltage.

Abstract:BACKGROUND: Obesity has become a worldwide epidemic and its prevalence continues to increase at a rapid rate in various populations and across all age-group. The effect of meal skipping, both behaviorally and physiologically, may have an impact on the outcome of weight-loss efforts.AIMS AND OBJECTIVES: Therefore, the aim of this study was to examine the prevalence of breakfast skipping and obesity in subjects.MATERIALS AND METHODS: A retrospective analysis of the patients visited to a metabolic clinic of the city was done.RESULTS: One hundred and eighty-six eligible subjects were included for the study. A questionnaire was used for data collection which included information regarding dietary factors and exercise schedule/physical activity. A 24-hour dietary recall method was used to assess the amount of food consumed. Anthropometric measurements were taken.CONCLUSION: The higher prevalence of overweight and obesity in the present study could be because of imbalance in the diet and faulty food habits prevalent in the region.

Abstract:Background: Meal skipping has become an increasing trend of the modern lifestyle that may lead to obesity and type 2 diabetes.Objective: We investigated whether the timing of meal skipping impacts these risks by affecting circadian regulation of energy balance, glucose metabolism, and postprandial inflammatory responses.Design: In a randomized controlled crossover trial, 17 participants [body mass index (in kg/m2): 23.7 ± 4.6] underwent 3 isocaloric 24-h interventions (55%, 30%, and 15% carbohydrate, fat, and protein, respectively): a breakfast skipping day (BSD) and a dinner skipping day (DSD) separated by a conventional 3-meal-structure day (control). Energy and macronutrient balance was measured in a respiration chamber. Postprandial glucose, insulin, and inflammatory responses in leukocytes as well as 24-h glycemia and insulin secretion were analyzed.Results: When compared with the 3-meal control, 24-h energy expenditure was higher on both skipping days (BSD: +41 kcal/d; DSD: +91 kcal/d; both P < 0.01), whereas fat oxidation increased on the BSD only (+16 g/d; P < 0.001). Spontaneous physical activity, 24-h glycemia, and 24-h insulin secretion did not differ between intervention days. The postprandial homeostasis model assessment index (+54%) and glucose concentrations after lunch (+46%) were, however, higher on the BSD than on the DSD (both P < 0.05). Concomitantly, a longer fasting period with breakfast skipping also increased the inflammatory potential of peripheral blood cells after lunch.Conclusions: Compared with 3 meals/d, meal skipping increased energy expenditure. In contrast, higher postprandial insulin concentrations and increased fat oxidation with breakfast skipping suggest the development of metabolic inflexibility in response to prolonged fasting that may in the long term lead to low-grade inflammation and impaired glucose homeostasis.
