Abstract:Abstract The age-related decline in muscle function, particularly muscle power, is associated with increased risk of important clinical outcomes. Physical activity is an important determinant of muscle function, and different types of physical activity e.g. power-based versus endurance-based exercise appear to have differential effects on muscle power. Cross-sectional studies suggest that participation in power-based exercise is associated with greater muscle power across adulthood but this has not been investigated longitudinally. We recruited eighty-nine male and female power and endurance master athletes (sprint and distance runners respectively, baseline age 35–90y). Using jumping mechanography, we measured lower limb muscle function during a vertical jump including at least two testing sessions longitudinally over 4.5 ± 2.4y. We examined effects of time, discipline (power/endurance) and sex in addition to two- and three-way interactions using linear mixed-effects models. Peak relative power, relative force and jump height, but not Esslingen Fitness Index (indicating peak power relative to sex and age-matched reference data) declined with time. Peak power, force, height and EFI were greater in power than endurance athletes. There were no sex, discipline or sex*discipline interactions with time for any variable, suggesting that changes were similar over time for athletes of both sexes and disciplines. Advantages in lower limb muscle function in power athletes were maintained with time, in line with previous cross-sectional studies. These results suggest that improvements in lower limb function in less active older individuals following power-based training persist with continued adherence, although this requires further investigation in interventional studies.

Primary amoebic meningoencephalitis: a report of two cases and antibiotic and immunologic studies.

Abstract:In the summer of 1978, two children who had recently been swimming in freshwater lakes in Florida died from primary amoebic meningoencephalitis. Despite early and intensive treatment with amphotericin B, both patients died three to five days after the onset of illness. Amoebae were observed in wet preparations of cerebrospinal fluid and in sections of cerebral tissue and were identified as Naegleria fowleri by the indirect immunofluorescent antibody technique. The amoebae were highly virulent in mice. The isolate of N. fowleri was extremely sensitive in vitro to amphotericin B (minimal inhibitory concentration [MIC], 0.15 microgram/ml), somewhat sensitive to miconazole (MIC, 25 micrograms/ml), and resistant to rifampin (MIC, less than or equal to 100 micrograms/ml). Treatment with amphotericin B (7.5 mg/kg of body weight per day) administered intraperitoneally protected 60% of the mice. Lower doses of amphotericin B alone or in combination with miconazole (100 mg/kg) or rifampin (220 mg/kg) were not protective. These results suggest that amphotericin B remains the single effective agent in treatment of primary amoebic meningoencephalitis.
Biomechanical and Skeletal Muscle Determinants of Maximum Running Speed with Aging

Abstract:PURPOSE: Aging diminishes the ability to run fast, but the specific mechanisms responsible for this deterioration remain largely unknown. In the present study, we investigated the age-related decline in sprint running ability through a cross-sectional examination of biomechanical and skeletal muscle characteristics in 77 competitive male sprinters aged 17-82 yr.METHODS: Ground reaction force (GRF) and kinematic stride cycle parameters were measured during the maximum-velocity phase using a 9.4-m-long force platform. Knee extensor (KE) and ankle plantar flexor (PF) structural characteristics were investigated using ultrasonography and muscle biopsies (vastus lateralis). Force production characteristics of leg extensor muscles were determined by dynamic and isometric contractions.RESULTS: The main findings were as follows: 1) the progressive age-related decline in maximum running velocity (Vmax) was mainly related to a reduction in stride length (Lstr) and an increase in ground contact time (tc), whereas stride frequency showed a minor decline and swing time remained unaffected; 2) the magnitude of average braking and push-off resultant GRFs declined with age and associated with Lstr, tc, and Vmax; 3) there was an age-related decline in muscle thickness, Type II fiber area and maximal and rapid force-generating capacity of the lower limb muscles; and 4) muscle thickness (KE + PF) was a significant predictor of braking GRF, whereas the countermovement jump height explained most of the variance in push-off GRF in stepwise regression analysis.CONCLUSIONS: Age-related slowing of maximum running speed was characterized by a decline in stride length and an increase in contact time along with a lower magnitude of GRFs. The sprint-trained athletes demonstrated an age-related selective muscular atrophy and reduced force capacity that contributed to the deterioration in sprint running ability with age.

Abstract:Muscle weakness in the elderly has been linked to recurrent falls and morbidity; therefore, elucidating the mechanisms contributing to the loss of muscle function and mobility with advancing age is critical. To this aim, we comprehensively examined skeletal muscle metabolic function and hemodynamics in 11 young (23 ± 2 years), 11 old (68 ± 2 years), and 10 oldest-old (84 ± 2 years) physical activity-matched participants. Specifically, oxidative stress markers, mitochondrial function, and the ATP cost of contraction as well as peripheral hemodynamics were assessed during dynamic plantar flexion exercise at 40 per cent of maximal work rate (WRmax). Both the PCr recovery time constant and the peak rate of mitochondrial ATP synthesis were not significantly different between groups. In contrast, the ATP cost of dynamic contractions (young: 1.5 ± 1.0, old: 3.4 ± 2.1, oldest-old: 6.1 ± 3.6 mM min-1 W-1) and systemic markers of oxidative stress were signficantly increased with age, with the ATP cost of contraction being negatively correlated with WRmax (r = .59, p < .05). End-of-exercise blood flow per Watt rose significantly with increasing age (young: 37 ± 20, old: 82 ± 68, oldest-old: 154 ± 93 mL min-1 W-1). These findings suggest that the progressive deterioration of muscle contractile efficiency with advancing age may play an important role in the decline in skeletal muscle functional capacity in the elderly.

Abstract:Background Previous research suggests that an ageing‐associated remodelling and loss of motor units due to motor neuron death contributes significantly to muscle weakness in old age. In histological sections, motor unit remodelling is reflected by increased fibre type grouping. While regular exercise may not attenuate the loss of motor units during ageing, it has been suggested to facilitate reinnervation resulting in larger motor units, and a higher number and larger fibre type groups in histological sections of muscles from aged individuals. Abstract:Most existing evidence regarding the effects of age on muscular fatigue has focused on prolonged isometric contractions, repeated maximum dynamic contractions and individuals beyond traditional retirement age (>65 years). In the present study, age-related differences in muscle fatigue during submaximal dynamic efforts were examined. There were 24 younger (18-25 years) and 24 older (55-65 years) participants, all of whom were healthy and active, with equal numbers of each gender within each age group. Participants performed repetitive, intermittent shoulder abductions until exhaustion, at peak moments of 30% and 40% of individual maximum voluntary isokinetic contraction (MVIC) and with cycle durations of 20 and 40 s. Fatigue development was determined based on changes in MVIC, electromyographic (EMG) signals and ratings of perceived discomfort (RPD). Following the exhaustive exercises, strength recovery was monitored using a series of MVICs over a 15-min period. Results indicated the existence of an age-related fatigue resistance, with the older group demonstrating significantly slower rates of MVIC decline and RPD increase and smaller modifications in EMG-based fatigue measures. These age effects were generally more pronounced at the higher effort level. Main effects of effort level and cycle duration were also significant, while gender effects appeared to be marginal. Rates of strength recovery were not significantly influenced by age. In addition, the utility of standard EMG-based fatigue measures was assessed. Findings indicated that time-dependent changes in static and dynamic EMG-based measures were roughly comparable in terms of sensitivity and variability, supporting the use of standard EMG analyses for fatigue monitoring during intermittent dynamic contractions.

Abstract:Postoperative results of 48 patients who underwent anterior resection using the EEA‐stapler were evaluated. In all but 2 cases the indication for surgery was colorectal carcinoma. In 24 patients an anastomosis was created above the 5 cm level (above the anal verge: colorectal anastomosis) and in 24 at the 0‐5 cm level (coloanal anastomosis). There was no perioperative mortality. The only complication observed in the colorectal anastomosis group was one case of late anastomotic stenosis. In the coloanal anastomosis group there were 4 cases with early anastomosis leaks, 3 cases with late stenosis and 5 cases with various degrees of late fecal incontinence, ultimately resulting in a permanent diverting stoma in 5 (10.4%) patients.
Comparative Bilateral Measurements of Vastus Lateralis Muscle Oxygen Desaturation Kinetics during 30 S Sprint Cycling Exercise: Effects of Age and Performance

Abstract:The study assessed vastus lateralis oxygen desaturation kinetics (SmO2) in 32 male cyclists (16 Seniors, 16 Juniors) during a 30 s sprint, examining effects of age and performance. An incremental test was used to determine ventilatory thresholds (VT1, VT2) and maximal oxygen uptake (VO2kg), followed by a sprint test to evaluate anaerobic performance. Cyclists' performance phenotype was determined as the ratio of power at VT2 to 5 s peak sprint power. Juniors exhibited sprinter-like traits, excelling in all functional tests except for lactate levels post-sprint. SmO2 data showed no age-related or bilateral differences across participants. The combined mean response time (MRT) revealed stronger bilateral goodness of fit (R2 = 0.64) than individual time delay (TD) and time constant (τ). Higher VO2kg at VT2, peak power, and maximal uptake were linked to longer TD, while shorter TD correlated with higher lactate production and increased fatigue. Bilaterally averaged SmO2 kinetics distinguished between sprint and endurance athletes, indicating the potential to reflect the alactic anaerobic system's capacity and depletion. Age did not affect desaturation rates, but younger cyclists showed greater response amplitude, attributed to a higher initial baseline rather than maximal desaturation at the end of the exercise.
Changes in muscle coordination patterns during 400‐m sprint: Impact of fatigue and performance decline

Abstract:The purpose of this study was to compare the muscle synergies extracted from 14 unilateral lower‐limb and trunk muscles between the first and final parts of a 400‐m sprint in experienced sprinters to understand neuromuscular coordination of multiple muscles in the fatigued condition sprint. Nine male 400‐m sprinters (400‐m personal record: 48.11 ± 1.6 s) performed 400‐m sprints as with the real competition strategy. We defined the first part (100–150 m section) and the final part (350–400 m section), and obtained mean spatiotemporal variables (e.g., running speed, step frequency, and step length) for both parts. Electromyography (EMG) signals were obtained using wireless EMG sensors (2000 Hz) from 14 lower‐limb and trunk muscles. Non‐negative matrix factorization was performed to extract the muscle synergies for both parts. We observed significantly declined spatiotemporal variables in the final part induced by fatigue. The extracted number of synergies was 7.0 ± 0.7 (mean ± SD) for the first part and 7.2 ± 0.4 for the final part with no significant differences between parts. However, we identified specific muscle synergy, and alterations in the individual muscle weightings of several hip muscles (rectus femoris: RF, tensor fasciae latae: TFL, and glutes maximus: Gmax muscles) while there was no change in the muscle weighting of shank muscles and the temporal patterns of all muscles even following fatigue in the 400‐m sprint. Fatigue‐induced performance decline in a 400‐m sprint corresponds to alterations in muscle synergies, particularly in hip muscles, with notable shifts in RF, TFL, and Gmax activation.

Abstract:Poor neuromuscular control and fatigue have been proposed as a risk factor for non-contact injuries especially around peak height velocity (PHV). This study explored the effects of competitive soccer match-play on neuromuscular performance and muscle damage in male youth soccer players. 24 youth players aged 13-16y were split into a PHV group (−0.5 to 0.5y) and post PHV group (1.0–2.5y) based on maturity off-set. Leg stiffness, reactive strength index (RSI), muscle activation, creatine kinase (CK), and muscle soreness were determined pre and post a competitive soccer match. Paired t-tests were used to explore differences pre and post competitive match play and independent sample t-tests for between groups differences for all outcome measures. There was no significant fatigue-related change in absolute and relative leg stiffness or muscle activation in both groups, except for the gastrocnemius in the post PHV group. RSI, CK and perceived muscle soreness were significantly different after soccer match-play in both groups with small to large effects observed (ES:0.41–2.82). Abstract:Abstract This study investigated the impact of age, knee joint angle, and strength testing modality on lower body maximal and rapid strength production and assessed the transferability of these characteristics to mobility-related function. Twenty young (age=21.9 yrs) and eighteen elderly (71.1 yrs) adults performed single-joint and multiple-joint isometric maximal voluntary contractions at three knee angles. Outcome measures included peak torque (PT), rate of torque development (RTD), jump height, power, 10 and 400 m walk, and timed chair stand. Older adults exhibited greater reductions in RTD for the multiple-joint (45%) than for the single-joint mode (18%). The 10 m walk was best predicted by multiple-joint RTD at 90°, the 400 m walk by jump height, and the chair stand by single-joint PT at 20°. Single-joint strength tests may underestimate age-related rapid strength impairments, likely due to requirements to exert muscular force in excess of one’s body mass in an upright position.

Abstract:BACKGROUND Type I collagen is an interstitial collagen, which is not present in normal glomeruli. As type I collagen was observed in advanced glomerular lesions, it appears to be associated with deterioration of renal function. However, the origins of cells expressing type I collagen mRNA in glomeruli of diseased kidneys remains controversial. METHODS We examined the expression of type I collagen in glomeruli at protein and mRNA levels in rat crescentic glomerulonephritis induced by anti-glomerular basement membrane (GBM) antibody. In addition, in situ hybridization and immunohistochemical staining of serial sections were performed to identify the cellular origin of type I collagen in glomeruli. RESULTS Semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR) in isolated glomeruli showed that mRNA expression of type I collagen was remarkably increased on days 7, 14, and 28 after anti-GBM antibody injection (12.2+/-1.4, 20.2+/-2.1 and 14.6+/-1.0-fold over day 0, respectively). Immunofluorescence for type I collagen demonstrated marked staining in the fibrocellular and fibrous crescents, and weak staining within glomerular mesangial areas. In close association with mRNA levels analysed by RT-PCR, in situ hybridization revealed predominant presence of alpha1(I) collagen mRNA in cells within crescentic areas and Bowman's capsules. Serial section analysis for immunostaining and in situ hybridization showed that some alpha1(I) collagen mRNA-positive cells were also positive for cytokeratin. In contrast, no alpha1(I) collagen mRNA-positive cells were stained by ED-1 and podocalyxin. CONCLUSIONS It appears that increased expression of type I collagen at the protein and mRNA levels in glomeruli is involved in the progression of glomerulonephritis. At least in this crescentic model, parietal epithelial cells (PECs) may partially contribute to the dysregulated production of type I collagen, which leads to glomerulosclerosis.
Distribution of G + C-rich regions in varicella-zoster virus DNA.

Abstract:The distribution of G + C-rich sequences in the genome of varicella-zoster virus (VZV) was investigated by partial denaturation, equilibrium sedimentation and Southern blot analyses. Portions of the IRS and TRS repeat sequences bounding the US region of the DNA were found to have a G + C content 10 to 20% greater than the overall 47% G + C content of the VZV genome. A stretch of DNA (approx. 1500 base pairs) at the UL-IRS junction and repeated at the terminus of the TRS sequences was found to be about 64% G + C, based on sedimentation equilibrium measurements. We also report the cloning of a novel fragment containing sequences from both the UL and TRS termini of the VZV genome. Our ability to clone this fragment suggests that unusual forms of VZV DNA including closed circular molecules and molecules with an inverted UL region can be packaged into nucleocapsids.
Effect of Athletic Training on Fatigue During Neuromuscular Electrical Stimulation

Abstract:The purpose of this study was to explore the effect an individual's exercise training type will have on muscle fatigability during repetitive contractions induced by Neuromuscular Electrical Stimulation (NMES). Thirty-four subjects comprising of competitive athletes and controls were recruited into three cohorts: Endurance (runners/cyclists) n = 13; nine male, four female; 27 ± 8 years old, Explosive (Lifters/Sprinters) n = 11; nine male, two female; 30 ± 7 years old, and controls n = 10, six male, four female, 26 ± 4 years old. Subjects were placed in a custom-made leg extension rig, and received NMES against a fixed resistance (NMES-FR), to the Vastus Medialis muscle resulting in isometric leg extensions, at a duty cycle of 1 s on/3 s rest, for 20 min. The force of the isometric contractions was recorded using a Hogan MicroFet2 dynamometer, and three separate fatigue metrics were calculated to compare the different cohorts, sports within each cohort, and gender within each cohort. For every fatigue metric, the endurance group fatigued significantly less than both the explosive and control cohorts, with no difference observed between the explosive and the controls. Within each cohort, no significant difference was observed in any fatigue metric between sport or gender, but these comparisons lacked power. The results show that only high capacity endurance activity will have any effect on reducing one's fatigability during repetitive NMES. The implications of this conclusion can aid in the development of NMES regimens for use in healthy populations, such as athletic training or astronaut musculoskeletal countermeasures, as well as clinical applications when fatigue is to be minimized.
Neuromuscular Fatigue and Recovery after Heavy Resistance, Jump, and Sprint Training

Abstract:Purpose Training methods that require maximal intensity efforts against light and heavy resistance are commonly used for athletic development. Typically, these sessions are separated by at least 48 h recovery on the assumption that such efforts elicit marked fatigue of the central nervous system (CNS), but this posit has not been well studied. The aim of the study was to assess the etiology and recovery of fatigue after heavy-resistance (strength), jump, and sprint training methods. Methods Ten male athletes completed three training sessions requiring maximal efforts that varied in their loading characteristics: (i) heavy-resistance exercise (10 × 5 back squats at 80% one-repetition maximum [1RM]) (STR), (ii) jumping exercise (10 × 5 jump squats) (JUMP), and (iii) maximal sprinting (15 × 30 m) (SPR). Preparticipants, postparticipants, and 24-, 48-, and 72-h postparticipants completed a battery of tests to measure neuromuscular function using electrical stimulation of the femoral nerve, and single- and paired-pulse magnetic stimulation of the motor cortex, with evoked responses recorded from the knee extensors. Fatigue was self-reported at each time point using a visual analog scale. Results Each intervention elicited fatigue that resolved by 48 (JUMP) and 72 h (STR and SPR). Decrements in muscle function (reductions in the potentiated quadriceps twitch force) persisted for 48 h after all exercise. Reductions in voluntary activation were present for 24 h after JUMP and SPRINT, and 48 h after STR. No other differences in CNS function were observed as a consequence of training. Conclusion Strength, jump, and sprint training requiring repeated maximum efforts elicits fatigue that requires up to 72 h to fully resolve, but this fatigue is not primarily underpinned by decrements in CNS function.

Abstract:ABSTRACT Purpose This study aimed to determine physiological and metabolic responses to two different sprint interval exercises (SIE) matched for total sprint duration and sprint-to-rest ratio. Methods After having measured peak oxygen uptake (V̇O 2peak ), fourteen healthy males (27.1 ± 4.8 years, 169.6 ± 6.0 cm, 64.5 ± 8.4 kg, V̇O 2peak : 47.2 ± 7.7 ml·kg -1 ·min -1 ) performed four 10-s sprints with 80-s recovery (SIE10) and two 20-s sprints with 160-s recovery (SIE20) on different occasions in a counterbalanced crossover manner. Pulmonary V̇O 2 and changes in tissue oxygenation index (∆TOI) at vastus lateralis (VL) and rectus femoris (RF) were measured during the SIEs. Furthermore, T2-weighted MRI was taken immediately before and after the SIEs to determine the activation levels of VL, RF, vastus medialis, vastus intermedius, adductor magnus, biceps femoris long head, semitendinosus and semimembranosus at 50% of right thigh length. Results In SIE10, increases in V̇O 2 and ∆TOI at VL and RF plateaued after the second sprint, whereas session-averaged ∆TOI was greater in SIE20 than SIE10 in both muscles (VL: 20.9 ± 7.4 vs. 14.2 ± 5.9%, RF: 22.8 ± 9.3 vs. 12.9 ± 6.6%, p = 0.00). While both SIEs significantly increased T2 values in all eight muscles, those magnitudes were similar between the conditions (SIE10 vs. SIE20: 5-16 vs. 8-16%). Conclusions This study showed blunted responses of whole-body (V̇O 2 ) and peripheral (∆TOI) oxidative responses with successive sprints (sprint 1 < sprint 2-4) in SIE10, suggesting that increasing sprint repetitions does not necessarily induce greater oxidative metabolism or stimulus. Moreover, greater peripheral oxygen extraction (∆TOI) was achieved with SIE20, whereas %changes of T2 indicates that the thigh muscles were similarly activated between the SIE conditions.

Abstract:The portion of society aged ≥60 years is the fastest growing population in the Western hemisphere. Aging is associated with numerous changes to systemic physiology that affect physical function and performance. We present a narrative review of the literature aimed at discussing the age-related changes in various metrics of physical performance (exercise economy, anaerobic threshold, peak oxygen uptake, muscle strength, and power). It also explores aging exercise physiology as it relates to global physical performance. Finally, this review examines the vascular contributions to aging exercise physiology. Numerous studies have shown that older adults exhibit substantial reductions in physical performance. The process of decline in endurance capacity is particularly insidious over the age of 60 years and varies considerably as a function of sex, task specificity, and individual training status. Starting at the age of 50 years, aging also implicates an impressive deterioration of neuromuscular function, affecting muscle strength and power. Muscle atrophy, together with minor deficits in the structure and function of the nervous system and/or impairments in intrinsic muscle quality, plays an important role in the development of neuromotor senescence. Large artery stiffness increases as a function of age, thus triggering subsequent changes in pulsatile hemodynamics and systemic endothelial dysfunction. For this reason, we propose that vascular senescence has a negative impact on cerebral, cardiac, and neuromuscular structure and function, detrimentally affecting physical performance.
