Abstract:Surgical Innovation, Ahead of Print. IntroductionVideo-assisted telescope operating monitor (VITOM®) with 3D Visualization technology was developed and has been used with favorable results by several surgical specialties. Our study aims to be a preliminary report for initial experience using the VITOM® 3D system for microsurgical varicocelectomy on varicocele patients.MethodsWe performed 35 microsurgical varicocelectomy procedures using the VITOM® 3D system on varying types and grades of varicoceles. The surgeon had the option of using either a 2.5 or 3.5 magnifying loupe in addition to the exoscope for each operation evaluated. The exoscope is a standalone camera head with an integrated 3D telescope and remote control with zoom and focus functions. It is connected to the 3D monitor via a mechanical holding arm. During the procedure, surgeons, assistants, and observers were able to view the 3D high-definition stream displayed on a 26-inch 3D monitor at a convenient viewing angle and distance. The varicocele ligation was performed using a Carl Zeiss Meditec AG microscope.ResultsThere were 35 patients with varicocele aged 31.51 years old on average, which were included in this report. Most patients had grade 3 bilateral varicocele (n = 13, 37.1%). All procedures were performed without any intraoperative complications. After the procedures, only a few patients suffered from postoperative complications. Three patients suffered scrotal edema (8.6%), while another had hydrocele (2.9%). The postoperative pain results were also very minimal from .89 1 day after the operation to .26 3 days after the operation.ConclusionThe VITOM® 3D system showed promise in microsurgical varicocelectomy.

Clinical effectiveness of microsurgical subinguinal varicocelectomy with enhanced recovery after surgery for varicocele

Abstract:Background: Varicocele (VC) is one of the most common causes of infertility in men, and microscopic varicocelectomy is currently the major surgical procedure for VC. We assessed the clinical effectiveness of microsurgical subinguinal varicocelectomy (MSV) with enhanced recovery after surgery (ERAS) in the treatment of VC in terms of semen quality improvement rate, pregnancy rate, pain relief rate, recurrence rate, and complication rate after MSV and explored the indications for VC surgery. Methods: In total, 216 patients undergoing MSV in our center between June 2019 and July 2020 were enrolled in this study. All patients received the surgery under local anesthesia and were admitted and discharged within 24 hours. All patients were followed up for more than 6 months, and the rates of semen quality improvement, pregnancy, pain relief, recurrence, and postoperative complications were recorded. We can evaluate the pain degree of patients through the numerical rating scale (NRS). (I) 0 points for painless; (II) 1-3: mild pain; (III) 4-6: moderate pain; (IV) 7-10 points are severe pain. 0 is the most slightly, indicating comfort, and 10 is the most painful and unbearable. Results: The sperm concentration, total sperm count, progressive motility rate, sperm viability, and morphology were significantly improved after the surgery (all P values <0.05). The rate of semen quality improvement was 88.2%, and the semen indicators returned to normal in 26.6% of the patients. Among the patients who were followed up for 1 year, the natural conception rate reached 27.1% and was accompanied by a 95.5% pain relief rate, a 0.5% VC recurrence rate, and a 2.3% postoperative complication rate.We obtained data through laboratory examination of semen DNA fragments index (DFI). Compared with preoperative and postoperative DFI, postoperative DFI was improved, and the pregnancy outcome was improved. Conclusions: MSV under local anesthesia increases the rates of semen quality improvement, pregnancy, and pain relief while lowering the rates of recurrence and postoperative complications. MSV may also help to improve the pregnancy outcomes in patients with VC accompanied by sperm DNA fragmentation or nonobstructive azoospermia, but this should be verified by further investigation.

Abstract:The time necessary for the breakdown of a lymphosarcoma is measured in a stock of genetically uniform non-susceptible mice. This breakdown time is measured by allowing grafts of the tumour to lie in the non-susceptible mice for different lengths of time, and then transplanting the grafts into susceptible mice to test their viability. If the non-susceptible mice are immunized by previous inoculation with the tumour, the breakdown time is considerably accelerated. The breakdown time of the tumour is then tested in non-susceptible mice which have received various tissues from immunized mice of the same stock. By this test, serum, peritoneal exudate and lymphocyte suspensions from immunized animals do not confer immunity, nor is immunity transferred from immunized females to their offspring. Immunity is, however, transferred by grafts of lymph nodes from immunized animals, and this transfer is shown to be genuinely passive. The power to transfer immunity lasts only a short time following implantation of the tumour, and is confined to the nodes draining the site of implantation.

Abstract:Objective To evaluate the clinical efficacy and safety of microsurgical subinguinal varicocelectomy (MSV) and high retroperitoneal varicocelectomy (HRV) in the treatment of massive varicocele (VC). Methods The patients with bilateral massive varicocele were randomly divided into MSV group and HRV group. The patient age (PA), VC width (VCW), operation time (OT), intraoperative blood loss (BL), postoperative hospital stay time (HST), postoperation hydrocele incidence (HI), varicocele relapse rate (VRR), testicular atrophy rate (TAR), pregnancy rates (PR), semen concentration (SC), semen motility (SM), semen deformation rate (SDR), sperm quality improvement rate (SQIR), scrotum VAS pain score, scrotum pain relief rate relief rate (SPRR), IIEF-5 score, follicle-stimulating hormone (FSH), luteotropic hormone (LH), testosterone (T) and free testosterone (FT) in serum were analyzed retrospectively. Results There was no differences in PA, VCW, BL, SPRR, HI, TAR, preoperative SC, SM, SDR, preoperative and postoperative LH, FSH, T, FT, VAS pain score, IIEF-5 score between the two groups (P>0.05). The postoperative SC, SM, and SDR was significantly improved in the two groups (P<0.01). The postoperative PR and SQIR in the MSV group was higher than that in the HRV group (P<0.05). The postoperative VRR in the MSV group was lower than that in the HRV group (P<0.05). The OT, postoperative SC and SM in the MSV group was higher than that in the HRV group (P<0.01). The VRR in the MSV group was lower than that in the HRV group (P<0.05). The SDR and HST in the MSV group was lower than that in the HRV group (P<0.01). Conclusion MSV can improve sperm quality and increase PR with satisfactory efficacy. MSV has more advantages and fewer complications, but long-term therapeutic effect of MSV needs to be further investigated.

Abstract:Objective To explore the efficacy,safety and feasibility of microsurgical subinguinal varicocelectomy (MSV).Methods The clinical data of 48 patients with single-side-varicocele were retrospectively analyzed.Results All operations were successful.The average operative time,blood loss,postoperative hospital stay was 65 minutes,5 mL,and 1.5 days,respectively.Recheck showed that the quality of semen was improved significantly.Recurrence occurred in 2 cases.No major postoperative complications were observed.Conclusion MSV is the first choice to treat varicocele.It is less invasive than open surgery and causes fewer complications.Therefore,it is worth wide clinical application.

Abstract:Introduction Varicocele is the most common treatable cause of male infertility. Although the exact mechanisms by which varicocele causes infertility have not been fully identified, there is evidence of an improvement in seminal parameters and pregnancy rates after varicocelectomy, especially if the technique is performed with an operating microscope Objective This study seeks to compare pre- and post-operative seminal and hormonal parameters of infertile patients undergoing microsurgical varicocelectomy in a local cohort Methods Retrospective analytical study of male patients with a diagnosis of infertility and clinical varicocele, who underwent microsurgical varicocelectomy in a University Hospital, between April 2013 - April 2023. Demographic variables, varicocele grade and laterality, reproductive hormone levels, seminal parameters and postoperative complications were collected from the clinical history database. Pre and postoperative seminal parameters and reproductive hormones were compared using non-parametric statisticians were used for data analysis with the Jamovi Software. P<0.05 was considered statistically significant Results There were 136 varicocelectomies in the period, 56 patients were excluded due to incomplete data collection and finally 80 patients were analyzed. Median age was 34.5 years (IQR 31-38), with a median infertility duration of 2 years (IQR 1-4). All patents had primary infertility. The median testicular volume was 15 cc (IQR 15-20 cc). 82.5% of varicoceles were Grade ≥2, 70.6% patients presented left varicocele and 28% were bilateral. Of the reproductive hormones, only FSH showed a statistically significant decrease of the pre- and post-operative median values, being 5.8 IU/L (IQR 3.9-8.9) and 2.7 IU/L (IQR 2.5-2.9) (p=0.001), respectively. Median sperm concentration before and after surgery was 7 million/mL (IQR 2-23) and 18 million/mL (IQR 3.3-73.5), respectively (p=0.02). Median preoperative and postoperative sperm progressive motility was 29% (IQR 15-48) and 40% (IQR 27-60), respectively (p=0.004). Seminal volume and normal morphology did not show significative changes after surgery. Only 2 patients (2.5%) presented a grade 1 Clavien-Dindo postoperative complication (mild infection of surgical site) Conclusions In this study, infertile patients with a clinical diagnosis of varicocele showed a statistically significant improvement in sperm concentration and progressive motility and a significant decrease in FSH levels after microsurgical varicocelectomy. Abstract:Two pair-housed, 1-y-old common marmosets (Callithrix jacchus) had intermittent loose feces and weight loss for approximately 2 mo. Cryptosporidum parvum was identified by ELISA in the feces of both animals. CBC and blood chemistry values, including liver enzymes, were within normal range. Both marmosets were treated with the antibiotic paromomycin (15 mg/kg PO) twice daily for 28 d. Resolution of clinical signs coincided with treatment. Three follow-up samples, taken 2 wk apart after treatment was finished, were negative for cryptosporidium ELISA in both animals. Paromomycin should be considered for treatment of cryptosporidiosis in marmosets.

Abstract:There are more than 15% couples suffer from infertility within 1 year. One in eight couples encounters problems when attempting to conceive a first child and one in six when attempting to conceive a subsequent child. The causes of infertility both of male and female could share 50%. With the recent advances in assisted fertilization such as in vitro fertilization (IVF)/intracytoplasmic sperm injection (ICSI), the couples suffer from infertility would like to choose assisted fertilization and nearly 1 million cycles ICSI were carried out in China every year, even its pregnancy rate 40% and live birth rate 35%, with very costed and with risks of inherent and born defects. Among of male factors of infertility, varicoceles are highly relevant (38%) and can result in a myriad of deleterious effects on male reproduction. Varicocele repair may be effective in men with subnormal semen analysis, a clinical varicocele and otherwise unexplained infertility. Numerous therapeutic options are available for correcting varicoceles, including surgical varicocelectomy and radiographic venous embolization. However, recent studies demonstrated that microsurgery for varicocele correction, with the lowest recurrence rate and the highest pregnancy rate compared with other surgical approaches, is a more cost-effective therapeutic modality than both assisted fertility for in infertility couples and it is safe and effective compared to the others surgical approaches. Obstructive azoospermia (OA) are highly prevalent (13%) in male infertility results from obstruction of the excurrent ducts (due to many causes) at any location between the rete testis and the ejaculatory ducts. Recent advances in microsurgical approaches with reversal rate 63.8–79.2%, pregnancy rate 30.6–35.8%. According to the calculations of cost per delivery for vasectomy reversal vs. sperm retrieval/ICSI, under a wide variety of initial assumptions, it is clear that vasectomy reversal/epididymovasostomy is associated with a considerably lower cost per delivery and higher delivery rates. Sperm retrieval and ICSI must yield an 81% pregnancy rate per cycle to achieve equal costs to vasectomy reversal. For the treatment of non-obstructive azoospermia (NOA), microdissection TESE is a more effective approach than other TESE approaches for ICSI, with a patency rate 40–60%, which change the impossible to possible for NOA patients, who want their own inherent child. In our center, there are more than 4,000 cases of patients were treated by microsurgery, including microsurgical varicocelectomy/vasectomy reversal/epididymovasostomy/microdissection TESE, from 2006 to 2016. We think that microsurgical training is important for urologists and clinical. Up to now, in the recent decade, we have trained about 500 surgeons for microsurgery from 30 provinces in China. In summary, the excellent efficacies were noted by microsurgical approaches for male infertility on varicoceles and OA, with lower cost and risks of inherent and born defects compared to in assisted fertilization such as IVF and ICSI.
Clinical Effect Of Microscopic Subinguinal Varicocelectomy Or Bypass Surgery On Nutcracker Phenomenon-Associated Varicocele

Abstract:Objective: The goal of this study was to assess the effectiveness and safety of microscopic subinguinal varicocelectomy or bypass surgery on nutcracker phenomenon-associated varicocele. Methods: A total of 96 patients with varicocele were retrospectively analyzed in our institution between August 2015 and August 2017. In group 1, 35 patients were included with nutcracker phenomenon-associated varicocele underwent microscopic subinguinal varicocelectomy. In group 2, 30 patients were included with nutcracker phenomenon-associated varicocele underwent bypass surgery. In group 3, 31 patients were included with varicocele underwent microscopic subinguinal varicocelectomy. Results: There was no significant difference among the three groups in term of left testicular volume, left internal spermatic vein diameter, and semen parameters. After 6 months postoperative, the mean semen parameters were significantly improved substantially in all groups. However, no significant difference was observed in left testicular volume, left internal spermatic vein diameter, and reflux time after surgery among group 1, group 2, and group 3. The improvement of semen parameters showed no significant difference in patients with nutcracker phenomenon-associated varicocele that underwent microscopic subinguinal varicocelectomy compared with those that underwent bypass surgery. None of the patients reported serious adverse events. Conclusion: Outcomes of microscopic subinguinal varicocelectomy and bypass surgery are similar in term of surgical effect and safety for the management of nutcracker phenomenon-associated varicocele.

Abstract:Objective: We investigate the effect and safty of simultaneous microsurgical subinguinal vasectomy and varicocelectomy. Methods: A 39-years old male, bilateral varicocele (leftⅠ, rightⅡ) with right testicular pain, request vasectomy. Under the operating microscope (10×), in the right side,cremasteric vein were identified and ligated. Arteries is microscopically dissected free of all surrounding tissue. All internal spermatic veins are Iigated with 5-0 suture, and lymphatics were preserved. The deferential vessels were carefully dissected and preserved. The vas was then double ligated and transected, through the scrotom approach, the left vas deferens was treated similar to the right side. Results:Overall , 2 testicular arteries and 3 lymphatics were preserved, 9 internal spermatic veins and 1 cremasteric veins were ligated. The follow-up was 3 month and 6 month respectively after surgery. No complications episodes of testicular atrophy, vasectomy failure, varicocelectomy recurrences, and scrotal uncomfort were found. Conclusions: Microsurgical varicocelectomy allows the identification of the artery, tiny vein and lymphatic. Men presenting for vasectomy with palpable and symptomatic varicoceles could benefit from simultaneous microsurgical vasectomy and varicocelectomy.

Abstract:Purine-rich stretches of nucleotides (GAGA boxes) are often found just upstream of transcription start sites in many genes, including insulin. Mutational analysis suggests that the GAGA box plays an important role in transcription of the rat insulin I gene. We identify here at least four different proteins that bind specifically to the insulin GAGA box. Using a GAGA oligonucleotide, we have isolated a cDNA encoding a sequence-specific protein from a HIT (hamster insulinoma cell line) lambda gt11 library. This protein, which we designate Pur-1 (for purine binding), binds to the GAGA boxes of the rat insulin I and II genes and the human islet amyloid polypeptide gene. Pur-1 is a potent transactivator in both pancreatic and nonpancreatic cells. Furthermore, Pur-1 is able to activate an intact insulin promoter in HeLa cells, where it is normally inactive.

Abstract:Many therapies are effective in treating varicoceles, including dilation of the pampiniform plexus in males. The most common method of treatment is varicocelectomy. We aimed to assess an alternative technique (microsurgical spermatic [distal end]-superficial or inferior epigastric vein anastomosis) that preserves the normal blood flow pattern for varicocele treatment. We retrospectively analyzed 27 men with varicocele between October 2019 and July 2020. All patients underwent microsurgical spermatic (distal end)-superficial or inferior epigastric vein anastomosis. The prognosis was reviewed retrospectively with an additional survey conducted 3 months after surgery. The mean ± standard deviation of the age was 26.1 ± 7.3 years in patients with microsurgical spermatic (distal end)-superficial or inferior epigastric vein anastomosis. The maximum diameter of the varicocele vein, perineal pain score, sperm density, and forward movement of sperm improved over 3 months after surgery. Microsurgical spermatic (distal end)-superficial or inferior epigastric vein anastomosis is a safe and efficient surgical treatment for varicoceles.

Abstract:Objective To evaluate the efficacy and safety of modified and simplified transumbilical laparoscopic technique in the treatment of varicocele.Methods Modified and simplified transumbilical laparoscopic technique was applied in the treatment of 5 6 patients with primary varicocele from July 2011 to January 2013.Then analyzing the operation time,amount of bleeding,hospitaliza-tion time and evaluating scrotal ultrasonography,semen quality and symptoms.Results All surgeries were successful,and the oper-ative time was 15-50 min,an average of(25.7±9.2)min;average amount of bleeding was less than 10 mL;hospital stays was 2~5 days,an average of(3.5±1.2)days.Patients were followed up for 3-24 months after operation.All patients′symptoms and sign significantly alleviated and semen quality in 46 patients was improved,no serious complications occurred during the follow-up peri-od.Conclusion Modified and simplified transumbilical laparoscopic technique is safe and effective in the treatment of varicocele. Moreover,it can reduce the cost of equipment and the difficulty of operation.
[Microsurgical varicocelectomy for male infertility]

Abstract:Varicoceles is the most common identifiable cause of male infertility, and varicocele repair is a major means for the treatment of the disease. Conventional techniques of varicocele repair including Palomo operation, open inguinal varicocelectomy and laparoscopic varicocelectomy have a relatively high rate of postoperative hydrocele formation, varicocele recurrence and testicular artery injury. Recently, microsurgical varicocelectomy has gained a worldwide application in the treatment of varicocele, which can preserve the testicular artery and lymphatic vessels ligate all the spermatic but vasal veins and significantly lower the incidence of the complications. It can improve the semen parameters of male infertility patients and increase the pregnancy rate in their female partners. It can also improve the semen parameters of the patients with non-obstructive azoospermia or severe oligoasthenospermia. Nowadays microsurgical varicocelectomy has become the "golden standard" for the treatment of varicocele.

Abstract:Objective: To determine efficacy of intraoperative microvascular Doppler ultrasound in assisting subinguinal microsurgical varicocelectomy for pain relief in the treatment of painful varicoceles, compared to the microsurgery without Doppler ultrasound. Methods: Total 153 patients underwent microsurgical varicocelectomy were randomly allocated to two groups: Groups 1 and 2 included 82 and 71 patients monitored with and without using intraoperative microvascular Doppler ultrasound, respectively. The assessments were compared between two groups, including intraoperative parameters (vessel numbers and operative time) and postoperative outcomes (pain resolution, complications and recurrence). Results: The average numbers of internal spermatic veins ligated (13.87±6.43 vs 11.72±5.66) and arteries preserved (1.96±0.87 vs 1.73±0.86) were significantly greater in Group 1 with Doppler ultrasound. Precisely, the smaller size of the internal spermatic veins was ligated and the more encircled arteries were also preserved in Group 1. In two groups with and without using Doppler ultrasound, 56 (68.3%) and 36 (50.7%) patients experienced a complete resolution of pain, 21 (25.6%) and 29 (40.9%) patients experienced partial resolution, whereas 5 (6.1%) and 6 (8.5%) patients experienced no change in the chronic pain, respectively. Thus, patients in Group 1 had a better outcome in chronic pain resolution (P = 0.033). The operative time, complications and recurrence rate were not different between the two groups. Conclusions: Subinguinal microsurgical varicocelectomy is an effective method to treat painful varicoceles. With the assistance of Doppler Ultrasound monitoring, greater numbers of vessels were identified and a better outcome of pain resolution was achieved.

Abstract:Introduction Varicocele is the most common correctable cause of male factor infertility. It has always been a debatable subject as regard how it affects fertility and the best way to treat. However, there is a wide consensus nowadays that varicocelectomy improve male fertility potentials. Proper assessment of the disease bilaterality is crucial not to miss one side and not to jeopardize treatment outcome. Objective The primary outcome measure was to assess disease bilaterality and the secondary outcome measure was to report on improvement in semen 3 and 6 months post varicocelectomy and pregnancy rate 1 year post surgery. Methods This prospective study was conducted between January 2019 and January 2022. Infertile males with varicoceles underwent microsurgical subinguinal varicocelectomy by expert uroandrologists. Patients who were referred as unilateral varicoceles had their CDUS repeated for assessment of missed concomitant contralateral varicocele. Then, patients who were diagnosed with bilateral varicoceles underwent bilateral subinguinal ligation straight away. Those who were referred with unilateral varicocele had their palpable sides operated on first; then the contralateral side was also explored using an intraoperative Doppler device and the maximal vein diameter was measured. Patients came for follow for at least 6 months after surgery to evaluate any complications and assess improvement in semen parameters. Gynecologists also reported on normal pregnancy rate 1 year after surgery. Results A total of 329 cases completed the study. A hundred cases (30.4%) were initially referred as unilateral varicoceles and 229 (69.6%) as bilateral varicoceles. After reassessment of the study population, bilaterality of varicocele was found to be as high as 98.5% (324/329). This is statistically significantly higher than initial US (P<0.001). Repeat CDUS strongly correlated with the intraoperative measured varicocele diameter [r=0.9, P<0.001]. Moreover, sperm parameters showed significant improvement 3 and 6 months post varicocelectomy. Normal pregnancy after 1 year of surgery occurred in 118 cases (35.9%). Conclusions Varicocele bilaterality is underreported. Thorough assessment by expert radiologists and andrologists is of paramount importance not to miss significant pathology or hazard treatment outcome. Disclosure No.

Abstract:Abstract The influence of varicocele and microsurgical varicocelectomy on semen quality remains unclear. Few studies have investigated the relationship between semen metabolism and the abnormalities in reproductive function caused by varicocele, however, there is no study on the changes of semen metabolism after microsurgical varicocelectomy. Here, we used the non-targeted and targeted metabolic analysis to investigate the different metabolites in seminal plasma within normal, varicocele, and varicocelectomy groups. We clearly showed that varicocele significantly affects semen metabolism, and microsurgical varicocelectomy can reverse this metabolic abnormality. Moreover, we characterized the landscape of three dipeptides in the seminal plasma of patients with varicocele that have not been identified previously in human tissues or biofluids. Interestingly, the levels of these three dipeptides decreased after microsurgical varicocelectomy coincident with an improvement in semen quality. Western blotting confirmed the downregulation of DPEP3 (dipeptidase 3) in the varicocele group and the upregulation of DPEP3 in the varicocelectomy group. Furthermore, we found that eight metabolites may be helpful to distinguish varicocele patients from normal subjects. Our results may be applied to earlier diagnosis or to predict the outcome of microsurgery for varicocele.

Abstract:Objective: To compare the outcomes of two approaches (inguinal and subinguinal) of microsurgical varicocelectomy (MV) in Chinese infertile men. Patients and Methods: 120 infertile men who met the inclusion criteria were randomly assigned to two groups and operated with MV using different approaches. The outcomes and complications were then compared. Results: The two groups were similar in all aspects except the percentage of patients using analgesic at 27 h postoperatively (57.6% in the inguinal group, 37.5% in the subinguinal group; p < 0.05). Spontaneous pregnancy was achieved at a ratio of 33.9% in the inguinal group and 30.4% in subinguinal group. The median values of semen parameters were significantly improved postoperatively in both groups. Conclusions: Both approaches are effective methods in the repair of varicocele in infertile men, but more analgesic medication at the first day after operation may be required in inguinal group.
