Monitoring of selected anthropometric and biochemical parameters in men and women in Bardejov District

Abstract:Objectives: Metabolic and cardiovascular diseases represent a significant and ever-increasing problem, accounting for nearly one-third of all deaths and leading to significant morbidity. Lifestyle changes, which introduce new risk factors for cardiovascular disease, are leading to an increase in the risk of cardiovascular disease worldwide. The burden of cardiovascular disease can be reduced by careful risk reduction, and as such, primary prevention is an important priority for all. There is strong consensus among international guidelines regarding the necessity of smoking cessation, weight optimization, and the importance of exercise, while guidelines differ slightly in their approach to hypertension and vary greatly in their approach to the optimal lipid profile, which remains a controversial issue. Methods: The work is focused on the analysis of biochemical (glucose, cholesterol and triacylglycerols) and anthropometric (body mass index) parameters in selected probands (n = 140), including men (n = 70) and women (n = 70) in the Bardejov district. Probands were divided into groups depending on age (18-38, 39-50, 51-60, and over 61 years). For statistical evaluation, the program Statistica ver. 12 and ANOVA were used. Results: The results of the study showed statistically significantly higher values of BMI, total cholesterol (TC), triacylglycerides (TG), and glucose (GLU) (p < 0.001) between men and women in the age groups (18-38 years, 39-50 years). In probands from the age group 51-60 years and over 61 years, we found a high level of cholesterol above the reference limit, but a significant difference between men and women was not confirmed. Correlation analysis confirmed the positive correlation between glucose and BMI. Conclusion: Differences in risk-factor (e.g., biochemical and anthropometrics) burden translate into marked differences in the lifetime risk of cardiovascular disease. These differences are consistent across age groups.

Survival, Disabilities in Activities of Daily Living, and Physical and Cognitive Functioning among the Oldest-Old in China: a Cohort Study.

Abstract:BACKGROUND:The oldest-old (those aged ≥80 years) are the most rapidly growing age group globally, and are most in need of health care and assistance. We aimed to assess changes in mortality, disability in activities of daily living, and physical and cognitive functioning among oldest-old individuals between 1998 and 2008.METHODS:We used data from the Chinese Longitudinal Healthy Longevity Study. Three pairs of cohorts aged 80-89 years, 90-99 years, and 100-105 years (in total, 19 528 oldest-old participants) were examined; the two cohorts in each pair were born 10 years apart, with the same age at the time of the assessment in the 1998 and 2008 surveys. Four health outcomes were investigated: annual death rate, Activities of Daily Living (ADL), physical performance in three tests and cognitive function measured by Mini-Mental State Examination (MMSE). We used different tests and multivariate regression analyses to examine the cohort differences.FINDINGS:Controlling for various confounding factors, we noted that annual mortality among oldest-old individuals was substantially reduced between 0·2% and 1·3% in 1998-2008 compared with individuals of the same age born 10 years previously, and that disability according to activities of daily living had significantly reduced annually between 0·8% and 2·8%. However, cognitive impairment in the later cohorts increased annually between 0·7% and 2·2% and objective physical performance capacity (standing up from a chair, picking up a book from the floor, and turning around 360°) decreased anually between 0·4% and 3·8%. We also noted that female mortality was substantially lower than male mortality among the oldest-old, but that women's functional capacities in activities of daily living, cognition, and physical performance were worse than their male counterparts.INTERPRETATION:Advances in medications, lifestyle, and socioeconomics might compress activities of daily living disability, that is, benefits of success, but lifespan extension might expand disability of physical and cognitive functioning as more frail, elderly individuals survive with health problems, that is, costs of success.FUNDING:National Natural Science Foundation of China, National Institute on Aging/National Institutes of Health, United Nations Funds for Population Activities.
Coverage by examinations associated with early detection of colorectal neoplasia in the Czech Republic

Abstract:Background: Coverage by examinations is a crucial indicator of the future impact on the burden of colorectal cancer (CRC). The study aimed to evaluate coverage by examinations associated with CRC screening and early cancer detection of CRC in the Czech Republic. The burden of CRC was also assessed. Methods: The novel nationwide administrative registry with individual data (period 2010-19) was used to evaluate coverage by examinations for screening faecal occult blood test and colonoscopy. In the second step, additional examinations for early CRC detection were included in the coverage calculation (complete coverage). Age-specific trends in CRC incidence (period 1977-2018) were investigated using Joinpoint regression. Results: Coverage by screening examinations within recommended interval was around 30%. Complete coverage reached >37% and >50% at the 3-year interval. The coverage by examinations for the non-screening population aged 40-49 years was almost 4% and 5% (most of them were colonoscopies) at the 3-year interval. In age groups aged ≥50 years, we observed a significant annual decline, especially in the 50-69 age group, with recent annual decreases reaching up to 5-7%. The change in trend and the recent decline were also observed in the age group 40-49. Conclusions: More than half of the target screening population was covered by examinations potentially associated with early detection and subsequent treatment of colorectal neoplasms. The substantial coverage by potentially prophylactic examinations might be an explanation for the considerable decrease in CRC incidence.
Interpreting routine biochemistry in those aged over 65 years: a time for change

Abstract:In clinical practice, routine biochemistry tests are often performed for diagnostic or screening purposes. We reviewed papers that have reported reference values for people older than 65 years with or without overt health problems. Wider intervals have generally been reported for sodium, potassium, calcium and C-reactive protein (CRP) in the elderly. Higher levels of creatinine and lower levels of total cholesterol (TC) and serum albumin (SA) are observed with ageing. Elderly people have been found to be at greater risk for overt abnormalities in sodium, potassium, creatinine, TC, SA and CRP. The consequences of these abnormalities could be severe. For instance, even mild hyponatremia was associated with increased risk of mortality, disability and myocardial infarction. Mild hypernatremia was associated with severe disability. Mild increases in CRP levels were associated with an increased risk of sarcopenia, disability, cardiovascular disease and cognitive decline. Mild decreases in TC levels were associated with an increased risk of disability or mortality, and mild decreases in SA were associated with an increased risk of mortality, disability, sarcopenia and frailty. Nutritional factors could not wholly explain these effects. Modified biochemical reference values are required for elderly patients, as biochemical results can serve as markers of vulnerability to age-related diseases, linked to metabolism. Careful diagnosis and corrective interventions are needed for patients in this age group.

Abstract:The oldest old (aged ≥80 years) are often the population subgroup at high nutritional risk due to age-related metabolic changes. We performed a validation analysis of a dietary screening tool (DST) which was developed for older adults among the oldest old. We examined dietary intakes using three 24-hour dietary recalls and the DST among 122 participants (aged 82-97) of the Geisinger Rural Aging Study. DST scores were compared with the Health Eating Index (HEI)-2015 scores, which were calculated based on three-day dietary recalls. Pearson correlations were used to characterize concurrent validity and Bland-Altman plots were used to identify potential bias. DST scores were significantly correlated with HEI scores (adjusted r = 0.68; p < 0.001) in an age- and sex-adjusted model. Those within the not-at-risk DST group had significantly higher HEI scores (adjusted means = 79.6 ± 3.68) compared with those who were in the at-risk (adjusted means = 51.2 ± 1.56) and the possibly-at-risk (adjusted means = 66.3 ± 1.79) groups (p-trend < 0.001). The DST appears to be a valid measure of diet quality in the oldest old when compared with the HEI and may be a potential tool to assess overall diet quality in this population.

Abstract:Objectives: The aim of this cross-sectional study was to determine the life satisfaction of older people living in a home environment and to find out what predictors influence it. Methods: The research involved 1,121 older people 60 years and above from the Moravian-Silesian region who live in a home environment. The short form of the Life Satisfaction Index for the Thirds Age (LSITA-SF12) was used to assess life satisfaction. The Geriatric depression scale (GDS-15), the Geriatric Anxiety Inventory Scale (GAI), The Sense of Coherence Scale (SOC-13), and the Rosenberg Self-Esteem Scale (RSES) were used to evaluate related factors. In addition, age, gender, marital status, education, social support, and subjective health assessment were evaluated. Results: The overall life satisfaction score was found to be 36.34 (s = 8.66). The satisfaction of older people was classified into four grades: high satisfaction (15.2%), moderate satisfaction (60.8%), moderate dissatisfaction (23.4%), and high dissatisfaction (0.6%). The predictors of the longevity of the lives of older people were confirmed, both health factors (subjective health assessment, anxiety, and depression [Model 1: R = 0.642; R2 = 0.412; p<0.000]) and psychosocial factors (quality of life, self-esteem, sense of coherence, age, and social support [Model 2: R = 0.716; R2 = 0.513; p<0.000]). Conclusion: In implementing policy measures, these areas should be emphasized. The availability of educational and psychosocial activities (e.g. reminiscence therapy, music therapy, group cognitive behavioural therapy, cognitive rehabilitation) within the community care of the older people and university of third age is appropriate to increase the life satisfaction of the older people. An initial depression screening is also required as part of preventive medical examinations to ensure early diagnosis and treatment of depression.
The potential of domiciliary care service in the Czech Republic to promote ageing in place

Abstract:Due to population ageing, the need for long-term care is increasing. In many European countries, there is now a firm policy preference for care in the home as opposed to institutional care and policies at the local level support this preference. The purpose of this study is to report on the position of domiciliary care service within the Czech social services for the elderly and to explore its potential to promote ‘ageing in place’. The aim of the research was to perceive this issue from the viewpoint of the different parties: service users, service workers, service managers as well as policy-makers. Therefore, the qualitative methodology (case study method) was used. The results revealed that users considered domiciliary care as the only service in the Czech Republic that allowed them to remain at home despite their worsening capacity to manage the activities of daily living. On the part of the domiciliary care service, however, we found that this was strong in the provision of practical help, as well as assistance with users' self-maintenance, whereas their supervision and care management were not explicitly included either in the concept or the practice of this type of service.

Abstract:Objective To investigate routine blood cell count among elderly people aged over 65 years,and to analyze the relationship between routine blood cell count and age.Methods 2014 elderly people were selected by randomly sampling methods from Panjiayuan community and Balizhuang community in Chaoyang District,Zhanlanlu community and Gungwai community in Beijing.Basic data,habits,illness and other information were collected by the survey questionnaires,and blood samples were drawn to analyze routine blood cell count testing and biochemical results.The relationship between routine blood cell count testing and age was analyzed by statistical analysis.Results In elderly people in Beijing community,the mean red blood cell count was 4.43 1012/L,the mean hemoglobin was 135.6 g/L,the mean hematocrit was 39.53％,the mean white blood cell count was 6.69 109/L,the mean platelet count was 245.8 109/L.The 95％ confidence interval was (3.50-5.26) 1012/L for red blood cell count,was (106.6-164.6)g/L for hemoglobin,was 31.6％-47.5％for hematocrit,was (3.26-10.00) 109/L for white blood cell,was (110.9-377.7) 109/L for platelet.The red blood cell count,hemoglobin and hematocrit were higher in men than in women (P＜0.05 or 0.001),and the platelet count was higher in women than in men (P＜0.001).The red blood cell count,hemoglobin,hematocrit and mean corpuscular hemoglobin concentration (MCHC) were decreased along with ageing (all P＜0.001),and the average red blood cell distribution width (RDWCV) and mononuclear cell count showed an increasing trend with ageing (both P＜0.001).Conclusions The reference range of venous blood cells count in elderly people is different from that in adult people,and whether to develop a separate reference range for the elderly needs more data analysis.The values of blood cells count in elderly people may change dynamically with aging,and the elderly health can be improved by analyzing the reasons for the changes.

Abstract:Emergency departments in the Czech Republic have been established in recent years. Seniors are typical patients of these departments. Emergency medicine´s approach is based on symptoms' evaluation and on deciding about the priority of the care needed. The approach to older patients is specific both in diagnostics and in therapy. The triage of geriatric patients is more accurate when we also evaluate patient´s cognition, when we use geriatric frailty scales and screening tools for detection of delirium. Comprehensive geriatric evaluation is a time demanding process and thus inadequate for emergency department however we must maintain its basic components. The therapeutical approach must be complex, and it must include biological, psychological, and social aspects and environmental risk analysis. Trauma management in seniors requires evaluation of different vital function´s values compared to common triage criteria, the influence of medication on adaptive mechanisms and the risk of low energy trauma mechanisms. Therapy of trauma must be timely and complex and the continuity of care between intensive and standard level and then rehabilitation must be ensured. Palliative approach is appropriate for terminally ill patients.

Abstract:The demographic transition poses a significant challenge for health systems, especially in Central and Eastern European (CEE) countries, where the healthcare needs of aging populations are on the rise. This study aimed to describe and compare the health status and utilization of health services among the elderly residing in urban and rural areas of the most deprived region in Hungary. A comprehensive health survey was conducted in 2022, involving a randomly selected sample of 443 older adults (≥ 65 years) in Northeast Hungary. Multivariable logistic regression models adjusting for age, sex, education, financial status, chronic diseases, and activity limitations were used to investigate the association between type of residence and health service use. Among the study participants, 62.3% were female, 38.3% attained primary education, 12.5% reported a bad or very bad financial situation and 52.6% lived in urban areas. Overall, 24% of the elderly rated their health as very good or good (27.8% in urban and 19.7% in rural areas), while 57.8% (52.6% and 63.5% in urban and rural areas) reported limitations in daily activities. Compared to urban residents, rural residents reported lower rates of dentist visits ( p = 0.006), specialist visits ( p = 0.028), faecal occult blood testing ( p < 0.001), colorectal cancer screening with colonoscopy ( p = 0.014), and breast cancer screening ( p = 0.035), and a higher rate of blood pressure measurement ( p = 0.042). Multivariable models indicated that urban residence was positively associated with faecal occult blood testing (OR = 2.32, p = 0.014), but negatively associated with blood pressure (OR = 0.42, p = 0.017) and blood glucose measurements (OR = 0.48, p = 0.009). These findings highlight the influence of residence on health service utilization among older adults in Hungary. Further comprehensive studies are needed to better understand the health needs of the elderly population and to develop policies aimed at promoting healthy aging in CEE countries.

Abstract:ABSTRACT IMPACT: Understanding dietary patterns and nutrient intakes of the aging population may help address concerns and dietary guidelines regarding their nutritional needs. OBJECTIVES/GOALS: The objective of this study is to test the hypothesis that a healthy dietary pattern in the oldest old (aged 80 years and older) is related to greater compliance with dietary recommendations and better nutrient intake profiles. METHODS/STUDY POPULATION: We conducted a cross-sectional study of 122 participants aged 82 to 97 years old from the Geisinger Rural Aging Study (GRAS) cohort in rural Pennsylvania (n = 56 men and 66 women). The main outcome measures of the investigation were the daily nutrient intakes and food group intakes evaluated from the average of three 24-hour dietary recalls. The dietary patterns were determined by cluster analysis from 28 food groups. Diet quality and adherence to the Dietary Guidelines for Americans was assessed by the Healthy Eating Index (HEI)-2015 and the Dietary Screening Tool (DST). Recommended intakes were determined by the Recommended Dietary Allowances (RDAs) or Adequate Intakes (AIs). RESULTS/ANTICIPATED RESULTS: Less than 50% of participants met the dietary recommended intakes for vitamins D, E, K, B6, dietary fiber, zinc, potassium, and calcium. The more-nutrient-dense cluster was characterized by higher intakes of fruits and vegetables. The less-nutrient-dense cluster was characterized by higher intakes of foods including desserts and sweets. After adjusting for age, sex, and energy intake, participants in the more-nutrient-dense dietary pattern had a higher intake of vitamins A, D, K, C, fiber, and potassium (p < 0.05 for all). After adjusting for age and sex, participants in the more-nutrient-dense pattern had better diet quality assessed by the (HEI)-2015 (p < 0.001) and DST (p = 0.006). DISCUSSION/SIGNIFICANCE OF FINDINGS: Among the oldest old, many participants were found to have nutrient intakes lower than the recommended levels for fundamental nutrients suggesting that dietary guidance in addition to a dietary pattern more aligned with dietary guidelines may be beneficial for supporting healthy aging.

Abstract:Biomarkers defining biological age are typically laborious or expensive to assess. Instead, in the current study, we identified parameters based on standard laboratory blood tests across metabolic, cardiovascular, inflammatory, and kidney functioning that had been assessed in the Berlin Aging Study (BASE) ( n = 384) and Berlin Aging Study II (BASE-II) ( n = 1517). We calculated biological age using those 12 parameters that individually predicted mortality hazards over 26 years in BASE. In BASE, older biological age was associated with more physician-observed morbidity and higher mortality hazards, over and above the effects of chronological age, sex, and education. Similarly, in BASE-II, biological age was associated with physician-observed morbidity and subjective health, over and above the effects of chronological age, sex, and education as well as alternative biomarkers including telomere length, DNA methylation age, skin age, and subjective age but not PhenoAge. We discuss the importance of biological age as one indicator of aging.

Abstract:Abstract Alzheimer’s disease is a progressive neurodegenerative disorder with extensive neuropathological heterogeneity. We aimed to identify distinct latent subgroups of neuropathology and differentiate them using sociodemographic, contextual, and cognitive factors. Participants without dementia from the Alzheimer’s Disease Neuroimaging Initiative (ADNI; n=1703) and the Czech Brain Aging Study (CBAS; n=385) were included. Latent profile analysis was used to detect profiles using 10 volumetric structural magnetic resonance imaging regions of interest derived using Freesurfer algorithm (hippocampus, middle temporal, superior temporal, precuneus, anterior cingulate, medial orbitofrontal, pericalcarine, precentral, lingual, and caudate regions). Four profiles emerged in ADNI: Minimal Atrophy (n=192), Mild Atrophy (n=691), Moderate Atrophy (n=567), Severe Atrophy (n=253). Two profiles emerged in CBAS: Mild Atrophy (n=208) and Severe Atrophy (n=177). For all but the caudate region, there were significant pairwise differences in volumetry between each profile. Further, Minimal Atrophy was the youngest, had the highest proportion of participants with normal cognition, had the lowest progression to dementia, and the best cognition compared to all other groups – the remaining pairwise comparisons suggested that participants were older, more likely to have mild cognitive impairment, progress to dementia, and have poorer cognition, transitioning from Mild to Moderate to Severe Atrophy. Findings were consistent in CBAS. In ADNI, Minimal and Mild Atrophy had higher education and premorbid abilities compared to Moderate and Severe Atrophy. In CBAS, Mild Atrophy had better premorbid abilities compared to Severe Atrophy. Biological heterogeneity among older adults without dementia should be considered to understand who may be at risk of cognitive decline.

Abstract:This article describes a geriatric wellness program in which social work practitioners played a major role. The focus of this article is twofold: to examine the use of a telephone screening test for depression among a well elderly population and to compare the results of that screening with the clinical judgment of social workers. Overall findings indicated that a telephone screening instrument incorporating the Rand Mental Health Inventory and the Center for Epidemiological Studies Depression Scale was an efficient tool for assessing a population with a higher rate of major depression. Furthermore, the social workers identified many previously undetected cases of major depression, and a majority of people referred for treatment completed those referrals.

Stereotactic radiosurgery for intracranial dural arteriovenous fistulas: a systematic review.

Abstract:OBJECT The goal of this study was to evaluate the obliteration rate of intracranial dural arteriovenous fistulas (DAVFs) in patients treated with stereotactic radiosurgery (SRS), and to compare obliteration rates between cavernous sinus (CS) and noncavernous sinus (NCS) DAVFs, and between DAVFs with and without cortical venous drainage (CVD). METHODS A systematic literature review was performed using PubMed. The CS DAVFs and the NCS DAVFs were categorized using the Barrow and Borden classification systems, respectively. The DAVFs were also categorized by location and by the presence of CVD. Statistical analyses of pooled data were conducted to assess complete obliteration rates in CS and NCS DAVFs, and in DAVFs with and without CVD. RESULTS Nineteen studies were included, comprising 729 patients harboring 743 DAVFs treated with SRS. The mean obliteration rate was 63% (95% CI 52.4%-73.6%). Complete obliteration for CS and NCS DAVFs was achieved in 73% and 58% of patients, respectively. No significant difference in obliteration rates between CS and NCS DAVFs was found (OR 1.72, 95% CI 0.66-4.46; p=0.27). Complete obliteration in DAVFs with and without CVD was observed in 56% and 75% of patients, respectively. A significantly higher obliteration rate was observed in DAVFs without CVD compared with DAVFs with CVD (OR 2.37, 95% CI 1.07-5.28; p=0.03). CONCLUSIONS Treatment with SRS offers favorable rates of DAVF obliteration with low complication rates. Patients harboring DAVFs that are refractory or not amenable to endovascular or surgical therapy may be safely and effectively treated using SRS.

Abstract:Background: Dementia is the fourth leading cause of death in people > 65 years old in western countries. Objective: This cross-sectional assisted survey aimed to evaluate a multidisciplinary team approach of specialists of the Associazione Geriatri Extraospedalieri a favore di Anziani Svantaggiati and pharmacists to facilitate progress in the early identification and management of cognitive decline in patients > 60 years. Methods: A multidisciplinary team conducted this cross-sectional assisted survey. Patients (>60 years) with independent and/or assisted walking, subjective memory impairment, mild cognitive impairment or mild Alzheimer's disease (AD) who regularly attended pharmacies underwent the survey. An internal medical examination, a cardiovascular visit, and a short neuropsychological evaluation were conducted for each patient. Demographic, anamnestic, and clinical data were collected anonymously. Results: 279 eligible patients underwent the screening phase. 44% were overweight, 23% obese and 29% hypertensive. 62% of cases showed alterations of supra-aortic trunk with different percentages of stenosis. The neuropsychological evaluation highlighted that 67% of cases were normal according to age and education level, while 18% were in a state condition of cognitive frailty. Mild/moderate cognitive decline, or probably AD, was identified in 14% of cases. Conclusions: A multidisciplinary collaboration between pharmacists and specialist medical doctors is essential in early identification of prodromal symptoms of cognitive impairment and AD. The Prompt detection of the condition in this group of patients allowed the specialists to recommend in-depth diagnostic tests and follow-up procedures to slow the course of the disease. This would give time to carry out adequate caregiver training.
