Abstract:Introduction: Patients with type 2 diabetes (T2D) may have inadequate (insufficient or excessive) sleep which could accelerate CVD onset, but existing evidence from prospective studies is limited. Hypothesis: We hypothesized that inadequate sleep duration would be associated with higher risks of CVD incidence and mortality among patients with T2D. Methods: This prospective study included 19,345 participants with T2D who were free of CVD and cancer at baseline from the UK Biobank. Sleep duration was obtained at baseline using questionnaires. Cox proportional hazards regression models were used to examine the association of sleep duration with CVD events. Results: During an average follow-up of 11.0-12.0 years, we documented 2,219 incident CVD cases (1,555 CADs and 673 strokes) and 652 CVD deaths. Compared with those sleeping for 7-8 h/day, the multivariable-adjusted hazard ratios (95% confidence intervals) of sleeping for ≤5 and ≥10 h/day were 1.39 (1.19-1.61) and 1.38 (1.13-1.68) for incident CVD, 1.26 (1.04-1.51) and 1.23 (0.95-1.57) for CAD, 1.68 (1.30-2.16) and 1.75 (1.27- 2.42) for stroke, and 1.36 (1.02-1.80) and 1.94 (1.42-2.63) for CVD mortality. These results largely remained similar in sensitivity analyses that aimed to address potential reverse causation and in the joint analysis of sleep duration and metabolic control or diabetes severity status. Conclusions: Our findings showed inadequate sleep duration was independently associated with increased risks of CVD onset and death among patients with T2D.
Association of a Healthy Lifestyle with All-Cause and Cause-Specific Mortality among Individuals with Type 2 Diabetes: A Prospective Study in UK Biobank

Abstract:Objective:To evaluate the association of a healthylifestyle, involving seven low-risk factors mentioned in diabetes managementguidelines (no current smoking, moderate alcohol consumption, regular physicalactivity, healthy diet, less sedentary behavior, adequate sleep duration, andappropriate social connection), with all-cause and cause-specific mortalityamong individuals with type 2 diabetes. Research Design andMethods: This study included 13,366 participants with baseline type 2 diabetesfrom the UK Biobank free of CVD or cancer. Lifestyle information was collectedthrough a baseline questionnaire.Results:During a median follow-up of 11.7 years, 1,561 deaths were documented, with 625from cancer, 370 from CVD, 115 from respiratory disease, 81 from digestivedisease, and 74 from neurodegenerative disease. In multivariate-adjusted model, eachlifestyle factor was significantly associated with all-cause mortality and hazardratios (95% CIs) associated with the lifestyle score (scoring 6-7 vs. 0-2unless specified) were 0.42 (0.34, 0.52) for all-cause mortality, 0.57 (0.41,0.80) for cancer mortality, 0.35 (0.22, 0.56) for CVD mortality, 0.26 (0.10,0.63) for respiratory mortality, and 0.28 (0.14, 0.53) for digestive mortality(scoring 5-7 vs. 0-2). In the population-attributable-risk analysis, 27.1% (95%CI: 16.1, 38.0%) death was attributable to a poor lifestyle (scoring 0-5). The association between a healthylifestyle and all-cause mortality was consistent, irrespective of factorsreflecting diabetes severity (diabetes duration, glycemiccontrol, diabetes-related microvascular disease, and diabetes medication).Conclusions:Ahealthy lifestyle was associated with a lower risk of mortality due toall-cause, CVD, cancer, respiratory disease, and digestive disease amongindividuals with type 2 diabetes.

Abstract:Introduction: The association of an overall healthy lifestyle, involving 3 emerging factors (less sedentary time, adequate sleep duration, appropriate social connection) and 4 conventional factors (no current smoking, moderate alcohol consumption, regular physical activity, healthy diet) from the prevailing diabetes management guideline, with total and cause-specific mortality risk in patients with diabetes remains to be examined. Hypothesis: We hypothesized that an overall healthy lifestyle would be associated with a lower risk of total and cause-specific mortality in patients with diabetes. Methods: Analyses included 18,822 participants with diabetes at baseline in the UK Biobank. Lifestyle information was collected via a baseline questionnaire (2006-2010). Cox proportional hazard modeling was used to examine the association of a healthy lifestyle with mortality risk. Results: During a median follow-up of 11.2 years, 2,470 deaths occurred after 2 years of enrollment. The fully-adjusted hazard ratios (95% CIs) for patients with 6-7 low-risk lifestyle factors compared with those with 0-2 were 0.44 (0.38-0.51) for total mortality, 0.52 (0.40-0.67) for cancer mortality, 0.38 (0.26-0.56) for CVD mortality, 0.24 (0.13-0.45) for respiratory disease mortality, 0.96 (0.45-2.04) for neurodegenerative disease mortality, and 0.16 (0.04-0.62) for digestive system disease mortality. For each additional low-risk factor, the risk was 17% lower for total mortality and 13-26% lower for cause-specific mortality. The association for a summed score of the emerging factors was stronger in those with a lower score of conventional factors (P-interaction <0.05). The inverse association between a healthy lifestyle and total mortality risk was consistently observed regardless of diabetes duration, vascular disease, glycemic control, and diabetes medication. Besides, the association was stronger in those with earlier diabetes onset or lower socioeconomic status (both P-interaction <0.05). Conclusions: For patients with diabetes, a healthy lifestyle is associated with a lower risk of total and most cause-specific mortality. Our findings are consistent across sub-populations, especially for those differing in factors related to diabetes severity.

Abstract:Background/Introduction Evidence regarding the potential health effects of sleep characteristics among individuals with type 2 diabetes (T2D) is limited. Purpose We aimed to examine sleep characteristics in relation to subsequent risk of cardiovascular disease (CVD), including coronary heart disease (CHD), and stroke, and all-cause and cause-specific mortality among individuals with T2D. Methods We prospectively followed 21,902 men and women with T2D at baseline or diagnosed during follow-up (Nurses' Health Study: 1986-2018, Health Professionals Follow-Up Study: 1986-2018). Sleep characteristics, including sleep duration, snoring, sleep apnea, and sleep quality, were repeatedly assessed using self-reported questionnaires at baseline and follow-up. Associations of sleep characteristics with CVD risk and mortality were assessed using Cox proportional hazards models with adjustments for demographic, dietary and lifestyle factors, and medical history. Results During 170,355 and 196,458 person-years of follow-up in participants with T2D, there were 2,486 incident CVD events and 3,922 deaths, respectively. Compared with sleeping for 7-8 h/day, the multivariable-adjusted hazard ratios (HRs) of sleeping for ≤5 h/day were 1.21 (1.02, 1.45) for CVD incidence, 1.24 (1.20, 1.51) for CHD incidence, 1.40 (1.24, 1.58) for total mortality, 1.52 (1.20, 1.92) for CVD mortality, and 1.27 (1.01, 1.61) for other non-CVD and non-cancer mortality, while the HRs of ≥10 h/day were 1.42 (1.21, 1.66) for total mortality, and 1.63 (1.20, 2.10) for other non-CVD and non-cancer mortality. Furthermore, compared with participants without habitual snoring, those with habitual snoring had a 21%, 24%, 18%, and 33% higher risk for CVD incidence, CHD incidence, CVD mortality, and total mortality, respectively. Compared with participants without diagnosed sleep apnea, those with sleep apnea had a 27%-58% higher risk for CVD incidence, CVD mortality, and total mortality. In addition, lower habitual sleep quality, including sleep difficulties and restless sleep, was associated with a substantially higher risk of CVD, CHD, or CVD mortality. No associations were observed between sleep characteristics with risk of stroke and cancer mortality in individuals with T2D. Conclusions Short and long sleep durations as well as habitual snoring, sleep apnea, and lower sleep quality were independently associated with higher risks of CVD incidence and mortality, particularly CVD mortality in individuals with T2D.

Abstract:Study objectives: To investigate the implications of both sleep factors and sleep patterns on the prognosis of cardiometabolic multimorbidity. Methods: From UK Biobank, individuals with cardiometabolic multimorbidity , defined as the coincidence of at least 2 cardiometabolic diseases (hypertension, diabetes mellitus, coronary heart disease, and stroke) were included in this study. Four low-risk sleep factors, including early chronotype, sleep 7-8 h/d, free of insomnia, and no frequent excessive daytime sleepiness, were used to generate a healthy sleep score ranging from 0 to 4. Participants with a score of 0-1, 2, 3-4 were clustered into groups with poor, intermediate, and healthy sleep pattern, respectively. We assessed the adjusted hazard ratios and 95% confidence intervals for all-cause mortality using the Cox proportional hazards model. Results: Among included 35,757 participants, the mean age (standard deviation)) was 61.82 (6.3) years. After full adjustment, early chronotype, sleep 7-8 h/d, no frequent excessive daytime sleepiness, and free of insomnia were independently associated with 8%, 12%, 11%, and 8% lower risk of all-cause mortality among all persons with cardiometabolic multimorbidity. We found the fully adjusted hazard ratio (95% confidence interval) for all-cause mortality was 0.90 (0.88-0.92) for a 1-point increase in the healthy sleep score. Compared with the reference group, participants with the intermediate and healthy sleep pattern had 9% and 23% lower risk of all-cause death, respectively, in the fully adjusted model. Conclusions: A healthy sleep pattern combining 4 low-risk sleep factors could be regarded as a healthy lifestyle for individuals with cardiometabolic multimorbidity to lower the risk of all-cause mortality. Healthy Dietary Patterns and Risk of Cardiovascular Disease in Diabetic Patients: a Prospective Cohort Study.

Abstract:Purpose: Evidence is limited regarding the associations of different dietary patterns with cardiovascular disease (CVD) risk among the population with diabetes. Thus, we aimed to explore the associations between three dietary patterns and CVD incidence among the population with diabetes. Materials and methods: We prospectively followed 22 473 diabetic patients from the UK Biobank at the baseline. The healthy dietary pattern was derived from a food frequency questionnaire; meanwhile, the Alternate Mediterranean Diet (AMED) and the Dietary Approaches to Stop Hypertension (DASH) diet were assessed based on the Oxford WebQ online 24 h dietary questionnaire. Results: During an average of 10.8 years of follow-up, 5209 incident CVD cases, including 3552 coronary heart disease (CHD) events and 881 strokes, were documented. After multivariate adjustment, a higher healthy diet score was negatively associated with CVD, CHD, and stroke incidence. The hazard ratios and 95% confidence intervals across increasing quintiles of healthy diet score were 0.86 (0.79-0.95), 0.83 (0.75-0.93), and 0.71 (0.57-0.88) for CVD, CHD, and stroke, respectively. A similar protective association with CVD incidence was found for the AMED but not the DASH diet. Conclusions: Adherence to healthy dietary patterns is related to a lower risk of developing CVD among diabetic patients. Our findings further provide vigorous evidence for formulating dietary adherence guidelines for diabetic patients to reduce the burden of CVD complications. Evidence is limited regarding the associations of different dietary patterns with cardiovascular disease (CVD) risk among the population with diabetes.

Abstract:Abstract Aims To examine the association of a healthy sleep pattern with the risk of recurrent cardiovascular events among patients with coronary heart disease (CHD). Methods and results This prospective cohort study included 21 193 individuals with CHD from the UK Biobank. A healthy sleep score was generated based on a combination of chronotype, sleep duration, insomnia, and excessive daytime sleepiness. Cox proportional hazards regression models were applied to estimate the associations between healthy sleep score and recurrent cardiovascular events. During a median of 11.1 years of follow up, we documented 3771 recurrent cardiovascular events, including 1634 heart failure cases and 704 stroke cases. After multivariable adjustment, including lifestyle factors, medical history, and CHD duration, sleep 7–8 h/day, never/rarely insomnia, and no frequent daytime sleepiness were each significantly associated with a 12–22% lower risk of heart failure. In addition, compared with participants who had a healthy sleep score of 0–1, the multivariable-adjusted HR (95% CI) for participants with a healthy sleep score of 4 was 0.86 (0.75, 0.99) for recurrent cardiovascular events, 0.71 (0.57, 0.89) for heart failure, and 0.72 (0.51, 1.03) for stroke. Conclusions Adherence to a healthy sleep pattern was significantly associated with a lower risk of recurrent cardiovascular events among patients with CHD, especially for heart failure. These findings indicate that healthy sleep behaviours could be beneficial in the prevention of cardiovascular event recurrence.

Association of Sleep and Circadian Patterns and Genetic Risk with Incident Type 2 Diabetes: a Large Prospective Population-Based Cohort Study.

Abstract:Objective: To examine the association of incident type 2 diabetes (T2D) risk with sleep factors, genetic risk, and their combination effects. Design: Large prospective population-based cohort study. Methods: This population-based prospective cohort study included 360 403 (mean (S.D.) age: 56.6 (8.0) years) participants without T2D at baseline from the UK Biobank. Genetic risk was categorised as high (highest quintile), intermediate (quintiles: 2-4), and low (lowest quintile) based on a polygenic risk score for T2D. Sleep scores, including long or short sleep duration, insomnia, snoring, late chronotype, and excessive daytime sleepiness, were categorized as an unfavourable, intermediate, or favourable sleep and circadian pattern. Results: During a median follow-up of 9.0 years, 13 120 incident T2D cases were recorded. Among the participants with an unfavourable sleep and circadian pattern, 6.96% (95% CI: 6.68-7.24%) developed T2D vs 2.37% (95% CI: 2.28-2.46%) of participants with a favourable sleep and circadian pattern (adjusted hazard ratio (HR): 1.53, 95% CI: 1.45-1.62). Of participants with a high genetic risk, 5.53% (95% CI: 5.36-5.69%) developed T2D vs 2.01% (95% CI: 1.91-2.11%) of participants with a low genetic risk (adjusted HR: 2.89, 95% CI: 2.72-3.07). The association with sleep and circadian patterns was independent of genetic risk strata. Participants in the lowest quintile with an unfavourable sleep and circadian pattern were 3.97-fold more likely to develop T2D than those in the lowest quintile with a favourable sleep and circadian pattern. Conclusions: Sleep and circadian patterns and genetic risk were independently associated with incident T2D. These results indicate the benefits of adhering to a healthy sleep and circadian pattern in entire populations, independent of genetic risk.

Abstract:Abstract Study Objectives To investigate whether a healthy sleep pattern would reduce the risk of cardiometabolic multimorbidity (CMM) among hypertensives. Methods This is a prospective cohort analysis from the UK Biobank. A total of 69 524 hypertensives without a history of diabetes mellitus, coronary heart disease, or stroke at baseline were enrolled. Five dimensions of healthy sleep at baseline including early chronotype, sleep 7–8 h/d, free of insomnia, no snoring, and no frequent excessive daytime sleepiness were used to generate a healthy sleep score ranging from 0 to 5 (one point was given for each dimension of healthy sleep). A higher score indicated a healthier sleep pattern. We set five groups corresponding to the healthy sleep score of 5, 4, 3, 2, and 0–1, respectively. The primary outcome was the incidence of overall CMM among enrolled hypertensives. We assessed the adjusted hazard ratios (HRs) and 95% confidence intervals (CIs) by Fine-Gray subdistribution hazard models. Results We found the full-adjusted HR (95% CI) for overall CMM was 0.93 (0.91–0.95) for a 1-point increase in the healthy sleep score. Compared to hypertensives with a healthy sleep score of 0–1, those with a score of 5 had a 27% lower risk of overall CMM, and 37%, 23%, and 20% lower risks of diabetes mellitus, coronary heart disease, and stroke, respectively, after adjusting for sociodemographic characteristic, lifestyle, and clinical factors. Conclusions Our results indicated that a healthy sleep pattern was associated with lower risks of CMM outcomes among hypertensives.

Abstract:Objective To identify the association between quantified sleep factors and the incidence of cardiovascular disease (CVD) through a 10-year prospective cohort study. Methods A total of 45,919 individuals were recruited in this population-based prospective study. The healthy sleep score was constructed by four sleep measures (sleep duration, insomnia symptoms, snoring and daytime sleepiness), which were collected by questionnaire. The hazard ratio (HR) and 95% confidence intervals (CIs) were calculated by the multivariate-adjusted Cox proportional hazards model. Restricted cubic spline analysis was used to examine the dose‒response relationships between healthy sleep scores and outcomes. Results During a median follow-up of 10.73 years (interquartile range: 10.08–11.72 years), 10,523 cases of total CVD incidence, 3766 cases of CHD, and 3967 cases of stroke incidence were documented. Our results found that participants who maintained four healthy sleep measures (including no insomnia, snoring, or frequent daytime sleepiness and sleeping 7–8 h/d) had a 12% (HR: 0.88, 95% CI: 0.84–0.93) and 16% (HR: 0.84, 95% CI: 0.78–0.92) lower risk of developing CVD and CHD, respectively, but not stroke. There was a dose‒response relationship between sleep scores and the risk of cardiovascular events. With an increasing healthy sleep score, the risk of cardiovascular events decreases. Compared to those with a sleep score of 0–1, participants with a score of 4 had 27% (HR: 0.73, 95% CI: 0.67–0.79), 25% (HR: 0.75, 95% CI: 0.65–0.87), and 24% (HR: 0.76, 95% CI: 0.66–0.86) reduced risks of CVD, CHD, and stroke, respectively. Conclusions In this large prospective cohort study, a healthy sleep pattern effectively reduced the risk of CVD, CHD, and stroke.

Abstract:Abstract Background Evidence is limited regarding the association of healthy lifestyle including sleep pattern with the risk of complicated type 2 diabetes mellitus (T2DM) among patients with hypertension. We aimed to investigate the associations of an overall healthy lifestyle including a healthy sleep pattern with subsequent development of T2DM among participants with hypertension compared to normotension, and to estimate how much of that risk could be prevented. Methods This study examined six lifestyle factors with T2DM cases among hypertension (227,966) and normotension (203,005) and their interaction in the UK Biobank. Low-risk lifestyle factors were defined as standard body mass index (BMI), drinking alcohol in moderation, nonsmoking, engaging in moderate- to vigorous-intensity physical activity, eating a high-quality diet, and maintaining a healthy sleep pattern. Results There were 12,403 incident T2DM cases during an average of 8.63 years of follow-up. Compared to those with 0 low-risk lifestyle factors, HRs for those with 5–6 were 0.14 (95% CI 0.10 to 0.19) for hypertensive participants, 0.13 (95% CI 0.08 to 0.19) for normotensive participants, respectively (p trend < 0.001). 76.93% of hypertensive participants were considerably less likely to develop T2DM if they adhered to five healthy lifestyle practices, increased to 81.14% if they followed 6-factors (with a healthy sleep pattern). Compared with hypertension adults, normotensive people gain more benefits if they stick to six healthy lifestyles [Population attributable risk (PAR%) 83.66%, 95% CI 79.45 to 87.00%, p for interaction = 0.0011]. Conclusions Adherence to a healthy lifestyle pattern including a healthy sleep pattern is associated with lower risk of T2DM in hypertensives, and this benefit is even further in normotensives.

Abstract:Objective: To explore the relationship between sleep status and the risk of diabetes in adults. Methods: The baseline data of 53 260 subjects who were aged 30-79 years and had been enrolled into China Kadoorie Biobank (CKB) study from Suzhou, Jiangsu province were analyzed. Multiple logistic regression models were used to investigate the association between sleep status and diabetes after adjusting for potential confounders. Results: Among 53 260 subjects, 5.3% had diabetes. The proportions of difficultly falling asleep, early morning arousal and snoring frequently was 7.2%, 10.0% and 29.5%, respectively. There were 22.6% of subjects reporting sleep duration ≤6 hours. After controlling for possible confounders, the subjects with difficulty falling sleep (OR=1.63 for male, 95%CI: 1.30-2.05; OR=1.48 for female, 95%CI: 1.27-1.73), early morning arousal (OR=1.37 for male, 95%CI: 1.12-1.68; OR=1.31 for female, 95%CI: 1.14-1.51) or snoring frequently (OR=1.16 for male, 95%CI: 1.00-1.34; OR=1.39 for female, 95%CI: 1.23-1.57) had a higher risk of diabetes. Using hypnotics regularly was associated with the risk of diabetes in females (OR=1.42, 95%CI: 1.06-1.92). Compared with 8 hours sleep duration daily, shorter sleep duration (≤6 hours) was associated with risk of diabetes in both males (OR=1.37, 95%CI: 1.17-1.60) and females (OR=1.24, 95%CI: 1.08-1.41). No statistical significant association was found between longer sleep duration (≥9 hours) and the risk of diabetes. Conclusion: Sleep problems, including difficulty falling asleep, early morning arousal, snoring frequently and shorter sleep duration, were associated with the risk of diabetes, but no statistical significant association was observed between longer sleep duration and the risk of diabetes.

Abstract:Context A healthy sleep pattern has been related to a lower risk of type 2 diabetes mellitus (T2DM). Objective We aimed to identify the metabolomic signature for the healthy sleep pattern and assess its potential causality with T2DM. Methods This study included 78 659 participants with complete phenotypic data (sleep information and metabolomic measurements) from the UK Biobank study. Elastic net regularized regression was applied to calculate a metabolomic signature reflecting overall sleep patterns. We also performed genome-wide association analysis of the metabolomic signature and one-sample mendelian randomization (MR) with T2DM risk. Results During a median of 8.8 years of follow-up, we documented 1489 incident T2DM cases. Compared with individuals who had an unhealthy sleep pattern, those with a healthy sleep pattern had a 49% lower risk of T2DM (multivariable-adjusted hazard ratio [HR], 0.51; 95% CI, 0.40-0.63). We further constructed a metabolomic signature using elastic net regularized regressions that comprised 153 metabolites, and robustly correlated with sleep pattern (r = 0.19; P = 3x10(-325)). In multivariable Cox regressions, the metabolomic signature showed a statistically significant inverse association with T2DM risk (HR per SD increment in the signature, 0.56; 95% CI, 0.52-0.60). Additionally, MR analyses indicated a significant causal relation between the genetically predicted metabolomic signature and incident T2DM (P for trend < .001). Conclusion In this large prospective study, we identified a metabolomic signature for the healthy sleep pattern, and such a signature showed a potential causality with T2DM risk independent of traditional risk factors.

Abstract:Background: Sleep complaints were reported to be associated with stroke, however, the evidence on the association between healthy sleep pattern and stroke risk in Chinese is limited. Objective: The aim of this study was to investigate the association between healthy sleep pattern and stroke in Chinese, and the influence of metabolic diseases on the association. Methods: A total of 11,851 participants from the Kailuan study in China without stroke at baseline were included. We calculated a healthy sleep score according to four sleep factors, and defined the low-risk groups as follows: no insomnia, no excessive daytime sleepiness, no frequent snoring, and sleep 7-8h/d. Each low-risk sleep factor was assigned a score of 1. Cox proportional hazard models were used to assess the association between healthy sleep score and stroke. Mediation analysis was used to estimate the role of metabolic diseases (obesity, diabetes, and hypertension) in the healthy sleep score-stroke association. Results: During a mean follow-up period of 7.7 years, 504 cases of stroke were identified. A higher healthy sleep score was associated with a lower risk of stroke in a dose-response manner (P-trend=0.03). The adjusted hazard ratio (HR) for participants with a healthy sleep score of 4 versus ≤2 was 0.75 (95% confidence interval [CI]: 0.56, 0.96). In addition, obesity, diabetes, and hypertension collectively explained 21.9% (95% CI: 17.2, 26.5) of the association between healthy sleep score and stroke. Conclusion: Adherence to healthy sleep pattern was associated with a lower risk of stroke, and the favorable association was partially mediated by metabolic diseases.

Abstract:OBJECTIVE To evaluate the association between irregular sleep duration and incident diabetes in a U.K. population over 7 years of follow-up. RESEARCH DESIGN AND METHODS Among 84,421 UK Biobank participants (mean age: 62 years) who were free of diabetes at the time of providing accelerometer data in 2013–2015 and prospectively followed until May 2022, sleep duration variability was quantified by the within-person SD of 7-night accelerometer-measured sleep duration. We used Cox proportional hazard models to estimate hazard ratios (HRs) for incident diabetes (identified from medical records, death register, and/or self-reported diagnosis) according to categories of sleep duration SD. RESULTS There were 2,058 incident diabetes cases over 622,080 person-years of follow-up. Compared with sleep duration SD ≤ 30 min, the HR (95% CI) was 1.15 (0.99, 1.33) for 31–45 min, 1.28 (1.10, 1.48) for 46–60 min, 1.54 (1.32, 1.80) for 61–90 min, and 1.59 (1.33, 1.90) for ≥91 min, after adjusting for age, sex, and race. We found a nonlinear relationship (p nonlinearity 0.0002), with individuals with a sleep duration SD of >60 vs. ≤60 min having 34% higher diabetes risk (95% CI 1.22, 1.47). Further adjustment for lifestyle, comorbidities, environmental factors, and adiposity attenuated the association (HR comparing sleep duration SD of >60 vs. ≤60 min: 1.11; 95% CI 1.01, 1.22). The association was stronger among individuals with lower diabetes polygenic risk score (PRS; P interaction ≤ 0.0264) and longer sleep duration (P interaction ≤ 0.0009). CONCLUSIONS Irregular sleep duration was associated with higher diabetes risk, particularly in individuals with a lower diabetes PRS and longer sleep duration.

Abstract:Summary The diagnosis and clinical management of olfactory neuroblastoma are discussed. Computerized tomography is a valuable means of assessing the pre-operative extent of disease and the efficacy of subsequent major surgery. The tissue diagnosis is facilitated by electron microscopy, demonstrating the presence of dence core vesicles and cytoplasmic filaments. Radiotherapy followed by craniofacial resection is the treatment of choice for olfactory neuroblastomas that are locally extensive at presentation.

Abstract:Objective To prospectively examine the associations of combined lifestyle factors with incident cardiovascular disease (CVD) and mortality in patients with diabetes. Patients and Methods Patients with prevalent diabetes were included from 5 prospective, population-based cohorts in China (Dongfeng-Tongji cohort and Kailuan study), the United Kingdom (UK Biobank study), and the United States (National Health and Nutrition Examination Survey and National Institutes of Health–AARP Diet and Health Study). Healthy lifestyle scores were constructed according to non–current smoking, low to moderate alcohol drinking, regular physical activity, healthy diet, and optimal body weight; the healthy level of each lifestyle factor was assigned 1 point, or 0 for otherwise, and the range of the score was 0 to 5. Cox proportional hazards models were used to estimate hazard ratios for incident CVD, CVD mortality, and all-cause mortality adjusting for sociodemographic, medical, and diabetes-related factors, and outcomes were obtained by linkage to medical records and death registries. Data were collected from October 18, 1988, to September 30, 2020. Results A total of 6945 incident CVD cases were documented in 41,350 participants without CVD at baseline from the 2 Chinese cohorts and the UK Biobank during 389,330 person-years of follow-up, and 40,353 deaths were documented in 101,219 participants from all 5 cohorts during 1,238,391 person-years of follow-up. Adjusted hazard ratios (95% CIs) comparing patients with 4 or 5 vs 0 or 1 healthy lifestyle factors were 0.67 (0.60 to 0.74) for incident CVD, 0.58 (0.50 to 0.68) for CVD mortality, and 0.60 (0.53 to 0.68) for all-cause mortality. Findings remained consistent across different cohorts, subgroups, and sensitivity analyses. Conclusion The international analyses document that adherence to multicomponent healthy lifestyles is associated with lower risk of CVD and premature death of patients with diabetes.
