Abstract:AIM Evaluation of patient age and time of the prophylactic surgery as well as incidence of genital cancers and precancerous states observed in histopathology of the postoperative material from BRCA1 gene mutation carriers previously treated for breast cancer. MATERIAL AND METHODS 206 carriers of one of the three most common BRCA1 gene mutations (5382insC, C61G and 4153delA) in the Polish population, who were offered the option of prophylactic salpingo-oophorectomy The study group comprised 85 patients with the diagnosis of breast cancer before gynecological preventive surgery The study group was further divided into two subgroups for more detailed assessment of the tested variables. The first subgroup included 67 patients with breast cancer (unilateral or bilateral synchronous). The second subgroup included 18 patients with bilateral metachronous (the second diagnosis of breast cancer was at least 12 months after the first breast cancer diagnosis). The control group consisted of 121 patients with no cancerous lesions before preventive gynecologic surgery The patients undergoing prophylactic treatment had no prior symptoms in female sexual organ and no changes in the diagnostic tests. RESULTS The patients with a history of breast cancer underwent genetic testing and preventive surgery of the genital tract at a significantly later age than controls (respectively p = 0.0003, p = 0.0006). The patients with bilateral metachronous breast cancer underwent preventive surgery significantly earlier (p = 0.03). There was a trend indicating a 2.5 times higher risk of developing ovarian cancer among BRCA1 mutation carriers who had already been diagnosed and treated for breast cancer when compared to women without breast cancer diagnosis. The incidence of other genital cancers and precancerous states in BRCA1 gene mutation carriers with history of breast cancer was not statistically significant as compared to controls. Data on the clinical stage, morphological grade, histological type, age and type of pathology and the type of BRCA1 gene mutation did not show a statistically significant difference between the groups. CONCLUSIONS Each patient diagnosed with breast cancer should be strongly recommended a genetic test to reduce adverse consequences resulting from postponing the test and, if applicable, the preventive operation until later in life. Preventive surgery should be considered especially in BRCA1 gene mutation carriers previously treated for breast cancer because of the increased risk of ovarian cancer
Deep learning and wearable sensors for the diagnosis and monitoring of Parkinson's disease: A systematic review

Abstract:Parkinson's disease (PD) is a neurodegenerative disorder that produces both motor and non-motor complications, degrading the quality of life of PD patients. Over the past two decades, the use of wearable devices in combination with machine learning algorithms has provided promising methods for more objective and continuous monitoring of PD. Recent advances in artificial intelligence have provided new methods and algorithms for data analysis, such as deep learning (DL). The aim of this article is to provide a comprehensive review of current applications where DL algorithms are employed for the assessment of motor and non-motor manifestations (NMM) using data collected via wearable sensors. This paper provides the reader with a summary of the current applications of DL and wearable devices for the diagnosis, prognosis, and monitoring of PD, in the hope of improving the adoption, applicability, and impact of both technologies as support tools. Following PRISMA (Systematic Reviews and Meta-Analyses) guidelines, sixty-nine studies were selected and analyzed. For each study, information on sample size, sensor configuration, DL approaches, validation methods and results according to the specific symptom under study were extracted and summarized. Furthermore, quality assessment was conducted according to the Transparent Reporting of a multivariable prediction model for Individual Prognosis Or Diagnosis (TRIPOD) method. The majority of studies (74%) were published within the last three years, demonstrating the increasing focus on wearable technology and DL approaches for PD assessment. However, most papers focused on monitoring (59%) and computer-assisted diagnosis (37%), while few papers attempted to predict treatment response. Motor symptoms (86%) were treated much more frequently than NMM (14%). Inertial sensors were the most commonly used technology, followed by force sensors and microphones. Finally, convolutional neural networks (52%) were preferred to other DL approaches, while extracted features (38%) and raw data (37%) were similarly used as input for DL models. The results of this review highlight several challenges related to the use of wearable technology and DL methods in the assessment of PD, despite the advantages this technology could bring in the development and implementation of automated systems for PD assessment.

Co-evolution of machine learning and digital technologies to improve monitoring of Parkinson’s disease motor symptoms

Abstract:Abstract Parkinson’s disease (PD) is a neurodegenerative disorder characterized by motor impairments such as tremor, bradykinesia, dyskinesia, and gait abnormalities. Current protocols assess PD symptoms during clinic visits and can be subjective. Patient diaries can help clinicians evaluate at-home symptoms, but can be incomplete or inaccurate. Therefore, researchers have developed in-home automated methods to monitor PD symptoms to enable data-driven PD diagnosis and management. We queried the US National Library of Medicine PubMed database to analyze the progression of the technologies and computational/machine learning methods used to monitor common motor PD symptoms. A sub-set of roughly 12,000 papers was reviewed that best characterized the machine learning and technology timelines that manifested from reviewing the literature. The technology used to monitor PD motor symptoms has advanced significantly in the past five decades. Early monitoring began with in-lab devices such as needle-based EMG, transitioned to in-lab accelerometers/gyroscopes, then to wearable accelerometers/gyroscopes, and finally to phone and mobile & web application-based in-home monitoring. Significant progress has also been made with respect to the use of machine learning algorithms to classify PD patients. Using data from different devices (e.g., video cameras, phone-based accelerometers), researchers have designed neural network and non-neural network-based machine learning algorithms to categorize PD patients across tremor, gait, bradykinesia, and dyskinesia. The five-decade co-evolution of technology and computational techniques used to monitor PD motor symptoms has driven significant progress that is enabling the shift from in-lab/clinic to in-home monitoring of PD symptoms.

Abstract:Parkinson’s disease (PD) is the second most common neurodegenerative disorder affecting over 6 million people globally. Although there are symptomatic treatments that can increase the survivability of the disease, there are no curative treatments. The prevalence of PD and disability-adjusted life years continue to increase steadily, leading to a growing burden on patients, their families, society and the economy. Dopaminergic medications can significantly slow down the progression of PD when applied during the early stages. However, these treatments often become less effective with the disease progression. Early diagnosis of PD is crucial for immediate interventions so that the patients can remain self-sufficient for the longest period of time possible. Unfortunately, diagnoses are often late, due to factors such as a global shortage of neurologists skilled in early PD diagnosis. Computer-aided diagnostic (CAD) tools, based on artificial intelligence methods, that can perform automated diagnosis of PD, are gaining attention from healthcare services. In this review, we have identified 63 studies published between January 2011 and July 2021, that proposed deep learning models for an automated diagnosis of PD, using various types of modalities like brain analysis (SPECT, PET, MRI and EEG), and motion symptoms (gait, handwriting, speech and EMG). From these studies, we identify the best performing deep learning model reported for each modality and highlight the current limitations that are hindering the adoption of such CAD tools in healthcare. Finally, we propose new directions to further the studies on deep learning in the automated detection of PD, in the hopes of improving the utility, applicability and impact of such tools to improve early detection of PD globally.

Abstract:Parkinson's Disease is the second most common neurological disease in over 60s. Cognitive impairment is a major clinical symptom, with risk of severe dysfunction up to 20 years post-diagnosis. Processes for detection and diagnosis of cognitive impairments are not sufficient to predict decline at an early stage for significant impact. Ageing populations, neurologist shortages and subjective interpretations reduce the effectiveness of decisions and diagnoses. Researchers are now utilising machine learning for detection and diagnosis of cognitive impairment based on symptom presentation and clinical investigation. This work aims to provide an overview of published studies applying machine learning to detecting and diagnosing cognitive impairment, evaluate the feasibility of implemented methods, their impacts, and provide suitable recommendations for methods, modalities and outcomes. To provide an overview of the machine learning techniques, data sources and modalities used for detection and diagnosis of cognitive impairment in Parkinson's Disease, we conducted a review of studies published on the PubMed, IEEE Xplore, Scopus and ScienceDirect databases. 70 studies were included in this review, with the most relevant information extracted from each. From each study, strategy, modalities, sources, methods and outcomes were extracted. Literatures demonstrate that machine learning techniques have potential to provide considerable insight into investigation of cognitive impairment in Parkinson's Disease. Our review demonstrates the versatility of machine learning in analysing a wide range of different modalities for the detection and diagnosis of cognitive impairment in Parkinson's Disease, including imaging, EEG, speech and more, yielding notable diagnostic accuracy. Machine learning based interventions have the potential to glean meaningful insight from data, and may offer non-invasive means of enhancing cognitive impairment assessment, providing clear and formidable potential for implementation of machine learning into clinical practice.

Abstract:Parkinson’s disease (PD) is a neurodegenerative disorder that primarily affects the elderly for over 55 years. PD can be characterised by patients exhibiting various non-motor and motor symptoms. It is significant to note that even though modern-day medical technology has grown exponentially over the years, there is still no cure for Parkinson’s disease. Hence, it is a scientifically exciting proposal to develop technologies that diagnose Parkinson’s disease earlier. Early diagnosis of PD can enhance the patient’s quality of life to a reasonable extent, as the disease’s nature is progressive, and it may take years to cripple the patient. It is also essential to observe that the symptoms will get intensified over time. Early diagnosis can also predict other types of neurodegenerative diseases, as the symptoms are pretty similar. The idea of Artificial Intelligence (AI) techniques is recently getting significant medical diagnosis attention, as these technologies can process massive data and come up with good statistical predictions. This study presents a detailed review of various machine learning and deep learning-based AI techniques applied to PD diagnosis and their impact in opening up newer research avenues. Furthermore, this paper explores the possible opportunities of data-driven AI technologies in PD diagnosis and its current status.

Abstract:The recent advancements in information technology and bioinformatics have led to exceptional contributions in medical sciences. Extensive developments have been recorded for digital devices, thermometers, digital equipments and health monitoring systems for the automated disease diagnosis of different diseases. These automated systems assist doctors with accurate and efficient disease diagnosis. Parkinson’s disease is a neurodegenerative disorder that affects the nervous system. Over the years, numerous efforts have been reported for the efficient automatic detection of Parkinson’s disease. Different datasets including voice data samples, radiology images, and handwriting samples and gait specimens have been used for analysis and detection. Techniques such as machine learning and deep learning have been used broadly and reported promising results. This review paper aims to provide a comprehensive survey of the use of artificial intelligence for Parkinson’s disease diagnosis. The available datasets and their various properties are discussed in detail. Further, a thorough overview is provided for the existing algorithms, methods and approaches utilizing different datasets. Several key peculiarities and challenges are also provided based on the comprehensive literature review to diagnose a healthy or unhealthy person.

Abstract:Parkinson's disease (PD) patient care is limited by inadequate, sporadic symptom monitoring, infrequent access to care, and sparse encounters with healthcare professionals leading to poor medical decision making and sub-optimal patient health-related outcomes. Recent advances in digital health approaches have enabled objective and remote monitoring of impaired motor function with the promise of profoundly changing the diagnostic, monitoring, and therapeutic landscape in PD. We recently demonstrated that by using a variety of upper limb functional tests iMotor, an artificial intelligence powered, cloud-based digital platform differentiated PD subjects from healthy volunteers (HV). The objective of this paper is to provide preliminary evidence that artificial intelligence systems may allow one to discriminate PD patients from (HV) further and determine different features of the disease within a cohort of PD subjects. The recently introduced Neural Network Construction (NNC) technique was used here to classify data collected by a mobile application (iMotor, Apptomics Inc., Wellesley, MA) into two categories: PD for patients and HV. The method was tested on a series of data previously collected, and the results were compared against more traditional techniques for neural network training. The NNC algorithm discriminated individual PD patients from HVs with 93.11% accuracy and ON vs. OFF state with 76.5% accuracy. Future applications of artificial intelligence-powered digital platforms can enhance clinical care and research by generating rich, reliable, and sensitive datasets that can be used for medical decision-making during and between office visits. Additional artificial intelligence-based studies in larger cohorts of patients are warranted.
Machine Learning Models for Parkinson Disease: Systematic Review

Abstract:Background: With the increasing availability of data, computing resources, and easier-to-use software libraries, machine learning (ML) is increasingly used in disease detection and prediction, including for Parkinson disease (PD). Despite the large number of studies published every year, very few ML systems have been adopted for real-world use. In particular, a lack of external validity may result in poor performance of these systems in clinical practice. Additional methodological issues in ML design and reporting can also hinder clinical adoption, even for applications that would benefit from such data-driven systems. Objective: To sample the current ML practices in PD applications, we conducted a systematic review of studies published in 2020 and 2021 that used ML models to diagnose PD or track PD progression. Methods: We conducted a systematic literature review in accordance with PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines in PubMed between January 2020 and April 2021, using the following exact string: "Parkinson's" AND ("ML" OR "prediction" OR "classification" OR "detection" or "artificial intelligence" OR "AI"). The search resulted in 1085 publications. After a search query and review, we found 113 publications that used ML for the classification or regression-based prediction of PD or PD-related symptoms. Results: Only 65.5% (74/113) of studies used a holdout test set to avoid potentially inflated accuracies, and approximately half (25/46, 54%) of the studies without a holdout test set did not state this as a potential concern. Surprisingly, 38.9% (44/113) of studies did not report on how or if models were tuned, and an additional 27.4% (31/113) used ad hoc model tuning, which is generally frowned upon in ML model optimization. Only 15% (17/113) of studies performed direct comparisons of results with other models, severely limiting the interpretation of results. Conclusions: This review highlights the notable limitations of current ML systems and techniques that may contribute to a gap between reported performance in research and the real-life applicability of ML models aiming to detect and predict diseases such as PD.

Abstract:The dynamic nature of Parkinson's disease (PD) is that it gradually impacts regions of the brain that are responsible for the production of the dopamine hormone. Despite continuous efforts, no effective treatment or preventative approach exists for PD. Nonetheless, the disease can be detected. Our goal is to create a Machine Learning and Deep Learning‐based system that can detect Parkinson's disease from a variety of data sources with high accuracy, sensitivity, specificity and interpretability. However, there have been significant advancements in the field of research, especially the use of artificial intelligence in the Parkinson's disease diagnostic process. We reviewed articles that were released between 2018 and 2024, concentrating on the most current studies that had been published. We chose 70 research articles for our review paper based on a set of criteria from a variety of online databases, including IEEExpress, medical databases like PubMed, Google Scholar, ResearchGate and ScienceDirect, and various publishers, including Elsevier, Taylor & Francis, Springer, MDPI, Plos One and so forth. According to our review, the majority of works make use of voice data. Our review study found that the highest accuracy level of most papers was above 90%, and the most commonly used algorithms were CNN and SVM. The main goal of this review study is to look into and put together information about the different ways that artificial intelligence, especially Machine Learning, can be used to find Parkinson's disease. Using diverse data gathered from multiple public and private datasets, we can infer that the application of artificial intelligence, particularly Machine Learning algorithms, for identifying Parkinson's disease plays a crucial role in the medical field.

Abstract:Parkinson's disease (PD) is the second most common neurodegenerative disorder, globally affecting men and women at an exponentially growing rate, with currently no cure. Disease progression starts when dopaminergic neurons begin to die. In PD, the loss of neurotransmitter, dopamine is responsible for the overall communication of neural cells throughout the body. Clinical symptoms of PD are slowness of movement, involuntary muscular contractions, speech & writing changes, lessened automatic movement, and chronic tremors in the body. PD occurs in both familial and sporadic forms and modifiable and non-modifiable risk factors and socioeconomic conditions cause PD. Early detectable diagnostics and treatments have been developed in the last several decades. However, we still do not have precise early detectable biomarkers and therapeutic agents/drugs that prevent and/or delay the disease process. Recently, artificial intelligence (AI) science and machine learning tools have been promising in identifying early detectable markers with a greater rate of accuracy compared to past forms of treatment and diagnostic processes. Artificial intelligence refers to the intelligence exhibited by machines or software, distinct from the intelligence observed in humans that is based on neural networks in a form and can be used to diagnose the longevity and disease severity of disease. The term Machine Learning or Neural Networks is a blanket term used to identify an emerging technology that is created to work in the way of a "human brain" using many intertwined neurons to achieve the same level of raw intelligence as that of a brain. These processes have been used for neurodegenerative diseases such as Parkinson's disease and Alzheimer's disease, to assess the severity of the patient's condition. In the current article, we discuss the prevalence and incidence of PD, and currently available diagnostic biomarkers and therapeutic strategies. We also highlighted currently available artificial intelligence science and machine learning tools and their applications to detect disease and develop therapeutic interventions.

Abstract:Parkinson's disease (PD) is a common neurodegenerative disorder. It has a delitescent onset and a slow progress. The clinical manifestations of PD in patients are highly heterogeneous. Thus, PD diagnosis process is complex and mainly depends on the professional knowledge and experience of the physician. Magnetic resonance imaging (MRI) could detect the small changes in the brain of PD patients, and quantitative analysis of brain MRI may improve the clinical diagnosis efficiency. However, due to the complexity of clinical courses in PD and the high dimensionality in multimodal MRI data, traditional mathematical analysis could not effectively extract the huge information in them. Up to now, the accuracy of PD diagnosis in large sample size is still unsatisfying. As artificial intelligence (AI) is becoming more mature, varieties of statistical models and machine learning (ML) algorithms have been used for quantitative imaging data analysis to explore a diagnostic result. This review aims to state an overview of existing research recently that used statistical ML/AI methods to perform quantitative analysis of MR image data for the study of PD diagnosis. First we review the recent research in three subareas: diagnosis, differential diagnosis, and subtyping of PD. Then we described the overall workflow from MR image to classification result. Finally, we summarized a critical assessment of the current research and provide some recommendations for likely future research developments and trends.
Machine Learning Approaches for Detecting Parkinson’s Disease from EEG Analysis: A Systematic Review

Abstract:Background: Diagnosis of Parkinson’s disease (PD) is mainly based on motor symptoms and can be supported by imaging techniques such as the single photon emission computed tomography (SPECT) or M-iodobenzyl-guanidine cardiac scintiscan (MIBG), which are expensive and not always available. In this review, we analyzed studies that used machine learning (ML) techniques to diagnose PD through resting state or motor activation electroencephalography (EEG) tests. Methods: The review process was performed following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. All publications previous to May 2020 were included, and their main characteristics and results were assessed and documented. Results: Nine studies were included. Seven used resting state EEG and two motor activation EEG. Subsymbolic models were used in 83.3% of studies. The accuracy for PD classification was 62–99.62%. There was no standard cleaning protocol for the EEG and a great heterogeneity in the characteristics that were extracted from the EEG. However, spectral characteristics predominated. Conclusions: Both the features introduced into the model and its architecture were essential for a good performance in predicting the classification. On the contrary, the cleaning protocol of the EEG, is highly heterogeneous among the different studies and did not influence the results. The use of ML techniques in EEG for neurodegenerative disorders classification is a recent and growing field.

Abstract:Parkinson's disease (PD) can gradually affect people's lives thus attracting tremendous attention. Early PD detection and treatment can help control the disease progress, relief from the symptoms and improve the patients' life quality. However, the current practice of PD diagnosis is conducted in a clinical setup and administrated by a PD specialist due to the early signs of PD are not noticeable in daily life. According to the report of CDC/NIH, the diagnosed time of PD ranges from 2-10 years after onset. Therefore, a more accessible PD diagnosis approach is urgently demanded. In recent years, mobile health (for short mHealth) technology has been intensively investigated for preventive medicine, particularly in chronic disease management. Notably, many types of research have explored the possibility of using mobile and wearable personal devices to detect the symptom of PD and shown promising results. It provides opportunities for transforming early PD detection from clinical to daily life. This survey paper attempts to conduct a comprehensive review of mHealth technologies for PD detection from 2000 to 2019, and compares their pros and cons in practical applications and provides insights to close the performance gap between state-of-the-art clinical approaches and mHealth technologies.

Abstract:This paper reviews related work and state-of-the-art publications for recognizing motor symptoms of Parkinson's Disease (PD). It presents research efforts that were undertaken to inform on how well traditional machine learning algorithms can handle this task. In particular, four PD related motor symptoms are highlighted (i.e. tremor, bradykinesia, freezing of gait and dyskinesia) and their details summarized. Thus the primary objective of this research is to provide a literary foundation for development and improvement of algorithms for detecting PD related motor symptoms.
Entropy-based machine learning model for diagnosis and monitoring of Parkinson's Disease in smart IoT environment

Abstract:The study presents the concept of a computationally efficient machine learning (ML) model for diagnosing and monitoring Parkinson's disease (PD) in an Internet of Things (IoT) environment using rest-state EEG signals (rs-EEG). We computed different types of entropy from EEG signals and found that Fuzzy Entropy performed the best in diagnosing and monitoring PD using rs-EEG. We also investigated different combinations of signal frequency ranges and EEG channels to accurately diagnose PD. Finally, with a fewer number of features (11 features), we achieved a maximum classification accuracy (ARKF) of ~99.9%. The most prominent frequency range of EEG signals has been identified, and we have found that high classification accuracy depends on low-frequency signal components (0-4 Hz). Moreover, the most informative signals were mainly received from the right hemisphere of the head (F8, P8, T8, FC6). Furthermore, we assessed the accuracy of the diagnosis of PD using three different lengths of EEG data (150-1000 samples). Because the computational complexity is reduced by reducing the input data. As a result, we have achieved a maximum mean accuracy of 99.9% for a sample length (LEEG) of 1000 (~7.8 seconds), 98.2% with a LEEG of 800 (~6.2 seconds), and 79.3% for LEEG = 150 (~1.2 seconds). By reducing the number of features and segment lengths, the computational cost of classification can be reduced. Lower-performance smart ML sensors can be used in IoT environments for enhances human resilience to PD.

Abstract:Parkinson's disease (PD) is the second most common movement disorder in the world. It is characterized by motor and non-motor symptoms that have a profound impact on the independence and quality of life of people affected by the disease, which increases caregivers' burdens. The use of the quantitative gait data of people with PD and deep learning (DL) approaches based on gait are emerging as increasingly promising methods to support and aid clinical decision making, with the aim of providing a quantitative and objective diagnosis, as well as an additional tool for disease monitoring. This will allow for the early detection of the disease, assessment of progression, and implementation of therapeutic interventions. In this paper, the authors provide a systematic review of emerging DL techniques recently proposed for the analysis of PD by using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The Scopus, PubMed, and Web of Science databases were searched across an interval of six years (between 2018, when the first article was published, and 2023). A total of 25 articles were included in this review, which reports studies on the movement analysis of PD patients using both wearable and non-wearable sensors. Additionally, these studies employed DL networks for classification, diagnosis, and monitoring purposes. The authors demonstrate that there is a wide employment in the field of PD of convolutional neural networks for analyzing signals from wearable sensors and pose estimation networks for motion analysis from videos. In addition, the authors discuss current difficulties and highlight future solutions for PD monitoring and disease progression.

Leveraging the Potential of Digital Technology for Better Individualized Treatment of Parkinson's Disease

Abstract:Recent years have witnessed a strongly increasing interest in digital technology within medicine (sensor devices, specific smartphone apps) and specifically also neurology. Quantitative measures derived from digital technology could provide Digital Biomarkers (DMs) enabling a quantitative and continuous monitoring of disease symptoms, also outside clinics. This includes the possibility to continuously and sensitively monitor the response to treatment, hence opening the opportunity to adapt medication pathways quickly. In addition, DMs may in the future allow early diagnosis, stratification of patient subgroups and prediction of clinical outcomes. Thus, DMs could complement or in certain cases even replace classical examiner-based outcome measures and molecular biomarkers measured in cerebral spinal fluid, blood, urine, saliva, or other body liquids. Altogether, DMs could play a prominent role in the emerging field of precision medicine. However, realizing this vision requires dedicated research. First, advanced data analytical methods need to be developed and applied, which extract candidate DMs from raw signals. Second, these candidate DMs need to be validated by (a) showing their correlation to established clinical outcome measures, and (b) demonstrating their diagnostic and/or prognostic value compared to established biomarkers. These points again require the use of advanced data analytical methods, including machine learning. In addition, the arising ethical, legal and social questions associated with the collection and processing of sensitive patient data and the use of machine learning methods to analyze these data for better individualized treatment of the disease, must be considered thoroughly. Using Parkinson's Disease (PD) as a prime example of a complex multifactorial disorder, the purpose of this article is to critically review the current state of research regarding the use of DMs, discuss open challenges and highlight emerging new directions.
