Abstract:Water, sanitation, and hygiene (WASH) are essential for human development. The lack of WASH facilities affects the livelihoods and the health of communities. Assessing the state, availability, and accessibility of WASH systems is vital for identifying gaps and formulating proactive solutions. This study assessed drinking water and sanitation access in the Central Gonja District (CGD) and Mtubatuba Municipal areas, using qualitative methods and in-depth interviews with participants. Findings revealed persistent challenges of inadequate, unaffordable, and inaccessible water, along with subpar sanitation services, negatively impacting community well-being and health. Issues like corruption, favouritism, and social networks influenced the distribution of water and sanitation systems. Infrastructure limitations, bureaucratic obstacles, and ineptitude of officials posed barriers to service delivery, exacerbated by rural–urban population shifts. A public–private partnership between Novubu and Mtubatuba Municipality enabled water treatment, showcasing a potential model for sustainable water provision and a possible replication for CGD. The study advocates for transparent governance, community-centric solutions, and inclusive strategies to address chronic challenges, emphasizing the importance of proactive leadership and maintenance to prevent environmental degradation and health risks. The local governance systems should ensure the distribution and maintenance of water and sanitation systems while emphasizing the need for inclusive, community-driven approaches for lasting improvements.

Abstract:The fear of the invisible but prevalent Coronavirus (COVID-19), disease cannot be overemphasized since there is the potential possibility of it wiping out the entire world population within a few months if adequate and quick steps are not taken to curb this menace, and the sub-Saharan African (SSAn) region is no exception. It is evident that water, as an essential daily commodity, has long been in a state of emergency in SSAn nations, which is largely attributed to decades of neglect by the successive governments, because it has not been possible to separate the existing bond between water, health, livelihood and the economy. The laudable Millennium Development Goals (MDGs) proposed by the United Nations had yet to achieve the stated objective of improving the standards of living and health conditions of the rural communities in the SSAn region before the COVID-19 pandemic outbreak. This failure has been masked by a sort of delusion in which the people of this region are subjected to the hardship of searching for clean and healthy water in their own ponds, rivers, streams and shallow hand-dug local wells on a continuous basis. Less than 17% of the rural population in all the SSAn communities can access basic hand-washing facilities and sanitation systems. The total water productivity, as measured by the Gross Domestic Product (GDP) per cubic meter of total freshwater withdrawn, for the people was less than 5 GDP.
COVID-19 and the call for 'Safe Hands': Challenges facing the under-resourced municipalities that lack potable water access - A case study of Chitungwiza municipality, Zimbabwe

Abstract:Billions of people living in developing countries lack access to safe drinking water, not to mention water for handwashing, one of the most effective ways to contain the fast spreading novel coronavirus (COVID -19). The recent global spread of COVID-19 has fostered diverse initiatives such as the 'Safe Hands' challenge led by the World Health Organization. Individuals are encouraged to regularly wash their hands for 40-60 s under running water with soap. This call for 'Safe Hands' comes at a time when water insecurity and limited access to handwashing facilities in Africa is heightened. In this article, Chitungwiza city in Zimbabwe is used as a case study to assess the implications of the 'Safe Hands' challenge for poor municipalities in developing countries and characterize the challenges they face. To do so, interviews were conducted at water points/boreholes used by residents during Zimbabwe's COVID-19 national lockdown. The calculation of water requirements for proper hand hygiene determined the capacity for water-stressed regions to effectively implement 'Safe Hands'. On average, it was established that one person consumes an extra 4.5 L per day of water when they practice WHO 'Safe Hands' in the context of COVID-19. This increases domestic water demand in Chitungwiza by 9%. Due to water scarcity, people in Chitungwiza were experiencing challenges with practicing 'Safe Hands'. With their 'dry taps' woes, they might not be able to meet the standards of this WHO challenge. Lack of soap also reduced the effectiveness of the 'Safe Hands' challenge. This paper proposes short- and long-term measures that would allow effective implementation of the 'Safe Hands' by means of sustainable potable water supply. These measures include extensive social awareness and temporary change of household water use behavior. Municipalities are recommended to establish public private partnerships (PPPs) to create immediate and long-term water investments. Structural and transformational reforms would enhance, through flexible planning, investments for both water infrastructure and governance. This narrative has the potential to improve the urban water systems resiliency against future pandemics.

Abstract:Freshwater resources in coastal areas are under intense pressure from excessive groundwater extraction, which amplifies saltwater intrusion (SWI) into coastal freshwater aquifers, such as in the Mekong Delta. Studies that combine socioenvironmental data and households’ psychological factors next to salinity measurement data to design groundwater conservation strategies are rare. In this study, these aspects are combined to explore their influence on the public willingness to conserve groundwater using a Bayesian belief network model. We analyzed 313 household survey data spread over three districts in the coastal province of Tra Vinh, located in the Vietnamese Mekong Delta. The level of salinity is significantly correlated with the willingness to conserve groundwater. The top three socioenvironmental characteristics that influence willingness are the level of salinity, type of employment—i.e., being a farmer—and frequency of being exposed to groundwater or SWI promotional activities. Social norm, i.e., perceived social pressure, is the most influential psychological factor that determines willingness. This study reveals an urgency for the local government to intervene and create social pressure regarding the issue.

Abstract:The Mekong River provides water, food security, and many other valuable benefits to the more than 60 million Southeast Asian residents living within its basin. However, the Mekong River Basin is increasingly stressed by changes in climate, land cover, and infrastructure. These changes can affect water quantity and quality and exacerbate related hazards such as land subsidence and saltwater intrusion, resulting in multiple compounding risks for neighboring communities. In this study, we demonstrate the connection between climate change, groundwater availability, and social vulnerability by linking the results of a numerical groundwater model to land cover and socioeconomic data at the Cambodia-Vietnam border in the Mekong River Delta region. We simulated changes in groundwater availability across 20 years and identified areas of potential water stress based on domestic and agriculture-related freshwater demands. We then assessed adaptive capacity to understand how communities may be able to respond to this stress to better understand the growing risk of groundwater scarcity driven by climate change and overextraction. This study offers a novel approach for assessing risk of groundwater scarcity by linking the effects of climate change to the socioeconomic context in which they occur. Increasing our understanding of how changes in groundwater availability may affect local populations can help water managers better plan for the future, leading to more resilient communities.
Effects of COVID-19 on the provision of Water, Sanitation, and Hygiene (WASH) services in health care facilities in the Far-North Region of Cameroon

Abstract:Worldwide, the Joint Monitoring Program reports that one in four health care facilities (HCFs) lack functional water supply on premises, one in three lack hand hygiene facilities, and one in three lack adequate infectious waste disposal. The COVID-19 pandemics shed light on the lack of investments, the absence of infrastructures, education and policies related to WASH as well as revealed insufficient investment in healthcare safety and has brought WASH services as non-negotiable for HCFs. This study used a cross-sectional pre-post COVID-19 framework to determine the proportion of HCFs: meeting basic WASH services and, which WASH services improved post-COVID-19 in the Far-North Region of Cameroon. A total of 97 (23.04%) HCFs among the 421 that are found in the Far-North region were surveyed and located in eight (25%) of the 32 Health Districts. They corresponded to the integrated health centers category (79.4%) and the survey’s respondent was the chief of the HCF (92.8%). Approximately 75.3%, 0.0%, 48.5%, 46.4%, and 6.2% of HCFs respectively met thresholds for basic water, sanitation, hygiene, waste management, and environment cleaning services. When comparing pre- vs. post COVID-19 periods, a significant increase (8%) was noted in the proportion of HCFs as of optimal handwashing practices-related services (P=0.0026). There was also a significant increase (p=0.007) in the proportion of HCFs with cleaning protocols available. Further, none of the HCFs fulfilled all the criteria to meet basic services for all the five WASH services. In conclusion, the response to the COVID-19 pandemics only partially improved WASH services-related infrastructures in HCFs of the Far-North Region of Cameroon. The COVID-19 pandemics was a missed opportunity to strengthen WASH services. There should be a continuing encouragement of governments and funding agencies in planning and budgeting WASH in healthcare-related research and issues, and enabling the maintenance of existing WASH infrastructures in healthcare settings.

Access to Safe Water in Rural Artibonite, Haiti 16 Months after the Onset of the Cholera Epidemic

Abstract:Haiti has the lowest improved water and sanitation coverage in the Western Hemisphere and is suffering from the largest cholera epidemic on record. In May of 2012, an assessment was conducted in rural areas of the Artibonite Department to describe the type and quality of water sources and determine knowledge, access, and use of household water treatment products to inform future programs. It was conducted after emergency response was scaled back but before longer-term water, sanitation, and hygiene activities were initiated. The household survey and source water quality analysis documented low access to safe water, with only 42.3% of households using an improved drinking water source. One-half (50.9%) of the improved water sources tested positive for Escherichia coli. Of households with water to test, 12.7% had positive chlorine residual. The assessment reinforces the identified need for major investments in safe water and sanitation infrastructure and the importance of household water treatment to improve access to safe water in the near term.

Abstract:Despite legislative and policy commitments to participatory water governance in South Africa, and some remarkable achievements, there has been limited progress to improve the water infrastructure servicing in marginalized rural communities. Around five million South Africans still do not have access to safe water.This paper seeks to understand and advance processes to engage multisectoral stakeholders to respond to lack of safe water as a community-nominated health priority in rural South Africa.We engaged representatives from Mpumalanga Department of Health (MDoH), rural communities, other government departments and non-governmental organisations (NGOs) to cooperatively generate, interpret and act on evidence addressing community-nominated priorities. A series of participatory workshops were conducted where stakeholders worked together as co-researchers to develop shared accounts of the problem, and recommendations to address it. Consensus on the problem, mapping existing planning and policy landscapes, and initiating constructive dialogue was facilitated through group discussions in a collective learning process.Community stakeholders nominated lack of safe water as a local priority public health issue and generated evidence on causes and contributors, and health and social impacts. Together with government and NGO stakeholders, this evidence was corroborated. Stakeholders developed a local action plan through consensus and feasibility appraisal. Actions committed to behavioural change and reorganization of existing services, were relevant to the needs of the local community and were developed with consideration of current policies and strategies. A positive, collective reflection was made on the process. The greatest gain reported was the development of dialogue in 'safe spaces' through which mutual understanding, insights into the functioning of other sectors and learning by doing were achieved.Our process reflected willingness and commitment among stakeholders to work together collectively addressing local water challenges. Location in an established public health observatory helped to create neutral, mediated spaces for participation.

Abstract:Water security is critical to the health and well-being of people around the world, especially among populations experiencing water stresses and rapid urbanization in low- to middle-income countries (LMICs). Recent research suggests water insecurity is associated with negative mental health outcomes. Despite global improvement in access to safe water across the world, the World Health Organization (WHO) reports that access to safe water in urban areas has not changed significantly or has stagnated in certain countries. In most African cities, entrepreneurial water vendors have stepped up to fill supply gaps in the formal delivery system by selling vended water. As part of a larger research program that aims to assess and analyze public perceptions around vended water, this paper explores the links connecting water insecurity and emotional distress among urban slum dwellers who mostly use vended water in Accra, Ghana. We used a parallel mixed-methods approach. Our quantitative results show that water-insecure households (OR = 2.23, p = 0.01) were more likely to report emotional distresses compared to water-secure households. However, households with improved sanitation (OR = 0.28, p = 0.01) and those willing to participate for improved water and sanitation (OR = 0.28, p = 0.01) were less likely to report emotional distress. Our qualitative results offered support for the quantitative results, as participants not only hold various perceptions regarding the safety and quality of vended water but expressed emotional distresses such as fear of contamination, discomfort, worry over arbitrary change in prices, and anxiety. The implications of the results for policy and practice, specifically to ensuring access to safe water, are discussed.

Abstract:In this study, we use water quality tests, epidemiological data, and ethnographic interviews gathered in Kathmandu in 2017, to provide an interdisciplinary analysis of water insecurity, illness, and targeted responses. Our findings fit the pattern of the past 20 years: the concentration of coliform bacteria across water sources remains unsafe. Our epidemiological data documents high rates of water-borne infectious diseases consistent with fecal contamination of water sources. Our ethnographic interviews suggest that social marginalization, poverty, and dislocation are major drivers in the incidence of illness. Evaluating some of the collective responses to water insecurity and contamination, we argue that effectively addressing water pollution, scarcity, and health outcomes in Kathmandu requires redirecting the focus of programs and the primary methods of implementation. Instead of large-scale top-down infrastructural interventions such as the Melamchi Water Supply Project, programs should target women's leadership and subsidize funding for small-scale, community-managed interventions.

Abstract:The coronavirus disease 2019 pandemic has posed severe threats to humans and the geoenvironment. The findings of severe acute respiratory syndrome coronavirus 2 (Sars-CoV-2) traces in waste water and the practice of disinfecting outdoor spaces in several cities in the world, which can result into the entry of disinfectants and their by-products into storm drainage systems and their subsequent discharge into rivers and coastal waters, raise the issue of environmental, ecological and public health effects. The aims of the current paper are to investigate the potential of water and waste water to operate as transmission routes for Sars-CoV-2 and the risks of this to public health and the geoenvironment. Additionally, several developing countries are characterised by low water-related disaster resilience and low household water security, with measures for protection of water resources and technologies for clean water and sanitation being substandard or not in place. To mitigate the impact of the pandemic in such cases, practical recommendations are provided herein. The paper calls for the enhancement of research into the migration mechanisms of viruses in various media, as well as in the formation of trihalomethanes and other disinfectant by-products in the geoenvironment, in order to develop robust solutions to combat the effects of the current and future pandemics.
Evaluating knowledge, awareness and associated water usage towards hand hygiene practices influenced by the current COVID-19 pandemic in Bangladesh

Abstract:Hand hygiene is considered as one of the most effective ways for preventing transmissible diseases, especially for preventing virus-borne diseases. The study has been conducted to evaluate changes in knowledge, awareness and practices of hand hygiene due to the outbreak of the coronavirus disease (COVID-19) in Bangladesh. The potential factors influencing human behaviours for maintaining hand hygiene have also been explored. Moreover, a probable increase in daily water demand associated with the changed situation has been assessed. An online survey was performed among a total of 367 Bangladeshi residents about their practices of hand hygiene during pre-corona, corona, and of their perceived future practices at post-corona period. It has been observed that a significant percentage (62.1%) of the respondents have received basic hygiene education at any level of their academic education. Nevertheless, their hygiene practices were very poor before the COVID-19 pandemic. The outbreak of the COVID-19 has reinforced their previous knowledge and greatly influenced their behavioural changes towards practicing hand hygiene as per World Health Organization guidelines for preventing the virus outbreak. The changes, however, have created increased water demand. The estimated water usage is found to be 2.68 times (9.15 L/c/d) and 2.52 times (8.59 L/c/d) higher in the corona and post-corona period respectively than that of the pre-corona situation (3.41 L/c/d). The principal component analysis (PCA) elucidated that an individual's practice of hand hygiene was associated with income, level of academic and hygiene education, and the COVID-19 outbreak itself. Moreover, the influence of hygiene education and COVID-19 outbreak affecting the duration of handwashing are found highly significant (p-value < 0.001) from the regression analysis. Raising awareness towards behavioural change of an individual about water usage, improvement of academic curriculum regarding hand hygiene and provision of water pricing are recommended to attain sustainable development goals of the country.

Abstract:The COVID-19 pandemic has already triggered global health, economic, and social crises, clouding the sky of achieving the 2030 Agenda for Sustainable Development adopted by the United Nations (UN) in 2015 to improve human lives while protecting nature. Water, vital for all forms of life, is essential to public health and human well-being and smooth the path of ending poverty through food and energy security. Hence, we argued that global water security can serve as a shining star to guide policymakers through the overwhelming task of achieving the UN 2030 Agenda for Sustainable Development, currently hindered by the coronavirus outbreak. To quickly attain global water security while taking the accelerators of Sustainable Development Goal 6 (SDG6 “Clean Water and Sanitation”) into account, this study proposed different water-related interventions. To successfully implement the UN 2030 Agenda for Sustainable Development after the coronavirus pandemic, we call for immediate and sustainable interventions to act rapidly towards achieving water security and SDG6 worldwide and support the constant battle against the ongoing fight with COVID-19 and prepare for possible future pandemics.
A Qualitative Study of Barriers to Accessing Water, Sanitation and Hygiene for Disabled People in Malawi

Abstract:Globally, millions of people lack access to improved water, sanitation and hygiene (WASH). Disabled people, disadvantaged both physically and socially, are likely to be among those facing the greatest inequities in WASH access. This study explores the WASH priorities of disabled people and uses the social model of disability and the World Health Organization's International Classification of Functioning, Disability and Health (ICF) framework to look at the relationships between impairments, contextual factors and barriers to WASH access. 36 disabled people and 15 carers from urban and rural Malawi were purposively selected through key informants. The study employed a range of qualitative methods including interviews, emotion mapping, free-listing of priorities, ranking, photo voice, observation and WASH demonstrations. A thematic analysis was conducted using nVivo 10. WASH access affected all participants and comprised almost a third of the challenges of daily living identified by disabled people. Participants reported 50 barriers which related to water and sanitation access, personal and hand hygiene, social attitudes and participation in WASH programs. No two individuals reported facing the same set of barriers. This study found that being female, being from an urban area and having limited wealth and education were likely to increase the number and intensity of the barriers faced by an individual. The social model proved useful for classifying the majority of barriers. However, this model was weaker when applied to individuals who were more seriously disabled by their body function. This study found that body function limitations such as incontinence, pain and an inability to communicate WASH needs are in and of themselves significant barriers to adequate WASH access. Understanding these access barriers is important for the WASH sector at a time when there is a global push for equitable access.

Pro-Poor Hydraulic Governmentality: Sustainable Water Management and Local Livelihoods in Mekong Delta

Abstract:This article considers and adds empirical nuances to the recent conceptualization of pro-poor water management. Using the concept of pro-poor hydraulic governmentality along the Vietnam-Cambodia border of Thường Phước commune, we argue that water management is linked to local rural livelihoods in a complex and dynamic pro-poor mechanism. While certain policies organize local populations according to cost-effectiveness ignoring local customs, the practicalities of dealing with such constraints are much more ambivalent. This article demonstrates the structural pro-poor complexity among sand excavation, riverbank landslides, water management, local livelihoods, and populace resettlement. The government's resettlement plans and the perceptions of residents of these plans are intertwined with a wider political, economic, social, and cultural significance in the context of strong institutional power in Vietnam. Limitations and future research agenda are also indicated in the discussion and conclusion section.

Abstract:Abstract Besides providing reliable water resources for agricultural production, rural development efforts in Myanmar should target rural water security in terms of safe water supply and sanitation, and by mitigating water-related hazards. However, very few studies are available over the status of water-related development in rural areas of the country, and consequently on suitable practical solutions. The present paper describes a participatory workshop undertaken involving 45 rural development officers of the Department of Rural Development (DRD) of the Ministry of Agriculture, Livestock and Irrigation (MOALI), aimed at identifying suitable sustainable land and water management (SLWM) practices to be developed in rural areas of the country. Adoption of water safety plans (WSP), water harvesting, and soil and water bioengineering were strongly supported, while the need for improving water sanitation, especially in the poorest areas, was made evident. Insights of the participatory process confirmed that the poorest regions of Myanmar have also the worst water management structures. The results of the present work can represent baseline information and a needs assessment for future development projects in the country. However, there is a strong need for more studies and reports targeting marginalized rural contexts of Myanmar, to support equitable development.
Effects of climate vulnerability on household sanitation access, functionality, and practices in rural Cambodia

Abstract:With climate events increasing in frequency and severity, effects on human life, particularly those most vulnerable, are projected to increase in coming decades. Focusing on the effects of floods, storms, and drought, we investigate how climate vulnerability correlates with toilet dysfunction and abandonment in rural Cambodia using two household surveys, a latrine sales database, two flood-extent maps, and a composite climate vulnerability index. Using multiple linear regression and measures of association, we show that together and individually, increasing climate vulnerability and poverty correlated with increasing toilet abandonment. Toilet dysfunction occurred more frequently in flood-prone regions during the rainy season and increased with more household members. How living in flood-prone regions affected households' perceptions, practices, and experiences with sanitation was also characterized in detail. The results highlight the need for more scientifically rigorous evidence to improve climate-resilient toilet designs that provide access to a functional toilet year-round. Beyond technology, household behavior towards and maintenance of sanitation infrastructure is a critical concern, as unsafe practices were found to be more common among climate-vulnerable households. The sanitation sector, particularly including government, should focus on improving access to safely managed sanitation in rural communities, particularly ones who are most affected by climate change, to prevent pollution of natural resources and further protect public health.

Abstract:Introduction: Community engagement (CE) is an effective public health strategy for improving health outcomes. There is limited published knowledge about effective approaches to CE in ensuring effective responses to COVID-19 throughout lockdowns, travel restrictions and social distancing. In this paper, we contribute to bridging this gap by highlighting experience of CE in Vietnam, specifically focusing on migrant workers in Vietnam. Methods: A cross-sectional qualitative study design was used with qualitative data collection was carried out during August-October 2020. Two districts were purposefully selected from two large industrial zones. Data was collected using in-depth interviews (n = 36) with individuals and households, migrants and owners of dormitories, industrial zone factory representatives, community representatives and health authorities. Data was analyzed using thematic analysis approach. The study received ethics approval from the Hanoi University Institutional Review Board. Results: The government's response to COVID-19 was spearheaded by the multi-sectoral National Steering Committee for the Prevention and Control of COVID-19, chaired by the Vice Prime Minister and comprised different members from 23 ministries. This structure was replicated throughout the province and local levels and all public and private organizations. Different activities were carried out by local communities, following four key principles of infection control: early detection, isolation, quarantine and hospitalization. We found three key determinants of engagement of migrant workers with COVID-19 prevention and control: availability of resources, appropriate capacity strengthening, transparent and continuous communication and a sense of trust in government legitimacy. Discussion and conclusion: Our results support the current literature on CE in infection control which highlights the importance of context and suggests that future CE should consider five key components: multi-sectoral collaboration with a whole-of-community approach to strengthen governance structures with context-specific partnerships; mobilization of resources and decentralization of decision making to encourage self-reliance and building of local capacity; capacity building through training and supervision to local institutions; transparent and clear communication of health risks and sensitization of local communities to improve compliance and foster trust in the government measures; and understanding the urgent needs ensuring of social security and engaging all parts of the community, specifically the vulnerable groups.

Abstract:Safe and enduring water, sanitation, and hygiene (WASH) for all is the key target of Sustainable Development Goal 6. Despite progress in global WASH monitoring to support SDG 6, significant data gaps persist, especially in urban informal settlements in the Melanesian Pacific Region. This study aimed to improve understanding of existing sanitation systems, residents' preferences, and climate hazard exposure in these areas. Household surveys were conducted with 393 households in Suva (Fiji) and Port Vila (Vanuatu). Water availability for sanitation was high in Suva (99%) and Port Vila (70%), with noted improvements in sanitation superstructure. However, dry pits without slabs remain significant (15%) in Port Vila. The preference for water-based systems highlights the need for a resilient sanitation service chain, including adequate faecal sludge management, which was problematic in both locations. Reports of climate-driven hazard damage were more frequent in Vanuatu, with one in three respondents reporting impacts on their sanitation services.
