Abstract:Epithelial ingrowth is a rarely reported complication after small-incision lenticule extraction (SMILE). The case of a 41-year-old man with epithelial ingrowth in his left eye 20 days after SMILE is presented. He presented to the refractive center for a routine follow-up visit without any complaint, but the left eye revealed a small strip of corneal opacity. A corneal foreign body was removed and was verified as a corneal epithelium block after the pathology examination. Follow-up examinations showed good results.

Abstract:AIM To report the safety, efficacy, and accuracy of small-incision lenticule extraction (SMILE) or femtosecond-assisted laser in situ keratomileusis (FS-LASIK) for the correction of myopia or myopic astigmatism in patients with deep corneal opacity denoted by anterior segment optical coherence tomography (AS-OCT). METHODS Four patients with monocular corneal opacity (3 due to mechanical injury, 1 due to a firecracker wound) were recruited and treated with refractive surgery (3 for SMILE, 1 for FS-LASIK combined with limbal relaxing incision (LRI). Preoperative ocular manifestations, surgical details, postoperative visual outcomes, corneal opacity parameters, and corneal topography were analyzed. RESULTS Preoperatively, spherical diopter ranged from -3.0 D to -4.75 D with cylinder ranging from -0.75 to -5.0 D, and corrected distance visual acuity (CDVA) ranging from 20/25 to 20/20. One eye's corneal opacity was located in the central zone and three were in the mid-peripheral optical zone. Three patients underwent uneventful SMILE in both eyes, whilst one patient underwent FS-LASIK for high astigmatism in both eyes and LRI in the right eye. CDVA of the eye with corneal opacity ranged from 20/22 to 20/20 one to six weeks postoperatively. Two patients achieved better CDVA and no patients lost Snellen lines. The postoperative diopter was within ±0.75 D for all eyes. Significant edema existed above the corneal opacity in one eye and dissipated soon. No eccentric corneal topography or morphological anomaly of the corneal cap or flap was observed. CONCLUSION The cases demonstrate that SMILE or FS-LASIK is safe and effective to treat myopic astigmatism combined with deep corneal opacity lesions after comprehensive preoperative evaluation and appropriate candidate selection. FS-LASIK combined with LRI is also sufficient for correcting high astigmatism due to corneal scarring.
Treatment of corneal perforation with lenticules from small incision lenticule extraction surgery: a preliminary study of 6 patients.

Abstract:Purpose: To evaluate the use of lenticules extracted using small incision lenticule extraction (SMILE) surgery as a surgical alternative for wound closure in corneal perforation. Methods: Corneal lenticules obtained through SMILE surgery with central thickness >100 mu m were fixed over corneal perforation sites using 10-0 nylon interrupted stitches. Patients were monitored for a minimum of 1 year and were assessed using slit-lamp microscopy, fluorescein, digital tonometry, and best spectacle-corrected visual acuity (BSCVA) measurements. Postoperative complications throughout the study period were recorded. Results: Corneal perforations were successfully sealed in all 6 patients; 3 patients (50%) exhibited improved postoperative BSCVA. Between 3 and 4 weeks after the operation, part of the lenticules became incorporated into the corneal stroma and complete reepithelialization was achieved. During the follow-up period of 12 months, no evidence of infection, relapse, or perforation was detected in any patient. Conclusions: These preliminary findings suggest that the use of corneal lenticules may be a safe and effective surgical alternative for corneal perforation closure, with potential clinical application as relatively simple and inexpensive temporary measures to improve the condition of the cornea for further definitive interventions.
In Vivo Confocal Microscopic Investigation of the Cornea after Autologous Implantation of Lenticules Obtained Through Small Incision Lenticule Extraction for Treatment of Hyperopia

Abstract:PurposeTo investigate re-innervation in the implanted lenticule, as well as changes to the cornea, after correcting hyperopia with an autologous implantation of a lenticule obtained through small incision lenticule extraction (SMILE). MethodsThis study retrospectively analysed re-innervation in the implanted lenticule, as well as microscopic morphological changes in the corneal architecture of the recipient cornea in five patients (with myopia in one eye and hyperopia in the contralateral eye) who received SMILE in the myopic eye and femtosecond laser in situ keratomileusis (FS-LASIK) combined with lenticule implantation in the contralateral hyperopic eye. Re-innervation in the implanted lenticule, as well as microscopic morphological changes in corneal architecture, were evaluated by in vivo confocal microscopy. ResultsOne patient was examined at postoperative week six, two were examined at postoperative month two, one was examined at postoperative month nine, and one was examined at postoperative month 12. Partial subbasal nerve fibre regeneration was detected in two patients (Case 4 who was examined at postoperative month nine and Case 5 who was examined at postoperative month 12). Regenerated and branched nerve fibres were detected in the implanted lenticule in Case 5 who was examined at postoperative month 12. Both the anterior and posterior interfaces showed an absence or decrease of keratocytes and the presence of small particles of various brightnesses. Keratocytes in the implanted lenticule presented abnormal morphology in size and shape after surgery in all treated eyes, but showed partial morphological recovery in two patients (Case 4 who was examined at postoperative month nine and Case 5 who was examined at postoperative month 12). ConclusionsThese preliminary findings suggest that nerve fibres will regenerate into the implanted lenticule after autologous lenticule implantation. Keratocytes in lenticules demonstrated a gradual return to a normal morphology.
Femtosecond Laser-Assisted Allogenic Lenticule Implantation for Corneal Ectasia after LASIK: A 3-Year in Vivo Confocal Microscopic Investigation.

Abstract:PURPOSE: To investigate morphological changes in the cornea and the implanted lenticule in patients with corneal ectasia after laser in situ keratomileusis (LASIK) and implantation using a lenticule from small incision lenticule extraction (SMILE). METHODS: Eight eyes of 8 patients with ectasia after LASIK from 2015 to 2018 at the Eye and ENT Hospital of Fudan Unversity were analyzed. For recipient eyes, lenticules were transferred with a mean cryopreserved time of 20 ± 21 days extracted from SMILE donors to the exposed stromal bed after lifting the flaps and the flap was repositioned. Uncorrected and corrected distance visual acuity, manifest refraction, and corneal topography were performed. Patients were followed up for 3 years. Changes in lenticules and recipient corneas were analyzed using in vivo confocal microscopy. RESULTS: Activated keratocytes in the anterior and posterior stroma were observed near the lenticule at the first 6 months. Elongated, deformed keratocyte nuclei were found in the implanted lenticules in postoperative year 1. The anterior and posterior interfaces showed an absence or decrease of keratocytes and the presence of small particles of varying brightness. Nerve fibers in the implanted lenticules were detected in one case. No obvious difference was detected between cryopreserved and fresh lenticules. CONCLUSIONS: The preliminary findings suggest that recipient keratocytes repopulate the lenticules and the keratocytes in lenticules gradually return to their normal morphology. Abstract:A 25-year-old woman with moderate myopia presented for refractive surgery. Bilateral femtosecond laser–assisted small-incision lenticule extraction (SMILE) was scheduled and her right eye was completed smoothly. However, during the lenticule cutting of her left eye, a large inferior black area was noted. The operation was abandoned after an immediate optical coherence tomography examination, which revealed the corneal epithelium defect with no laser scanning line at the corresponding site. The secondary surgery was assigned to laser-assisted subepithelial keratomileusis (LASEK) after 1 week. The uncorrected distance visual acuity of her left eye recovered to 20/25 on the 12th day and to 20/20 at 3-month follow-up, with ideal corneal topography profiles. Corneal epithelium defect induced by accidental alcohol contact during disinfection was suspected to cause the black area. The management of black area had to be determined according to the location and size. LASEK was a rational substitution for the aborted SMILE.
Epithelial Remodeling and Epithelial Wavefront Aberrometry after Spherical vs. Cylindrical Myopic Small Incision Lenticule Extraction (SMILE)

Abstract:Background/Objectives: To compare the epithelial thickness changes and the changes in epithelial wavefront aberrometry following spherical versus astigmatic myopic small incision lenticule extraction (SMILE). Methods: Eighty-six eyes of 86 patients who underwent SMILE were included in this retrospective study. A total of 43 eyes underwent myopic spherical correction (spherical group) and 43 eyes underwent myopic cylindrical correction (cylindrical group). The groups were matched according to the spherical equivalent of surgically corrected refraction. Subjective manifest refraction as well as high-resolution anterior segment optical coherence tomography (MS-39; CSO; Florence, Italy) were obtained preoperatively as well as 3 months postoperatively. The latter was utilized for computing epithelial wavefront aberrometry in addition to epithelial thickness mapping. Results: Epithelial thickness increased significantly in both groups after SMILE (p < 0.01). In the cylindrical group, epithelial thickening was more pronounced on the flat meridian compared to the steep meridian (p = 0.04). In both groups, epithelial wavefront aberrometry showed a significant postoperative increase in the epithelium's spherical refractive power, causing a myopization of -0.24 ± 0.42 diopters (D) in the spherical group (p < 0.01) and -0.41 ± 0.52 D in the cylindrical group (p < 0.0001). While no significant changes in epithelial cylindrical refractive power were observed in the spherical group, a significant increase was noted in the cylindrical group from -0.21 ± 0.24 D to -0.37 ± 0.31 D (p = 0.01). In both groups, epithelial higher-order aberrations increased significantly (p < 0.001). Conclusions: Postoperative epithelial remodeling after SMILE alters lower-order (sphere and cylinder) and higher-order aberrations of the corneal epithelial wavefront and might contribute to refractive undercorrection, especially in astigmatic corrections. Epithelial wavefront aberrometry can be used to quantify the refractive effect of epithelial remodeling processes after keratorefractive surgery.
Risk Factors and Incidence of Suction Loss during Small Incision Lenticule Extraction (SMILE) in 8487 eyes

Abstract:Abstract Background: To report the incidence and risk factors of suction loss during small incision lenticule extraction (SMILE).Methods: This retrospective comparative case control study included 8487 eyes of 4261 patients. Patients underwent SMILE surgery between January 2014 and September 2019 were included. Videos of suction loss were reviewed, and the direct causes of suction loss were noted. An independent samples t-test was used for comparisons between the suction loss group and the control group. A binary logistic regression model was used to determine the possible significant risk factors that might increase the likelihood of suction loss during SMILE surgery.Results: Suction loss occurred in 31 (0.37%) eyes of 30 patients; 23 (74%) cases occurred in the right eye (the first operative eye) and 8 (26%) cases occurred in the left eye. Among the 30 patients, 23 (76.7%) were male and 7 (23.3%) were female. The incidence in the six consecutive years were 0%, 2.13%, 0.34%, 0.24%, 0.22%, and 0.23%. Head and eye movements during surgery caused suction loss in 15 (48.4%) and 16 (51.6%) eyes, respectively. Comparison between the suction loss group and the control group showed that the first operative eye and male gender are at a significantly high risk for suction loss (p < 0.05).Conclusions: Surgical experience may influence the incidence of suction loss during SMILE surgery. The two events that easily led to suction loss were head and eye movements. The risk factors were first operative eye and male gender.Trial registration: Retrospectively registered. Bilaterally Asymmetric Corneal Ectasia Following SMILE With Asymmetrically Reduced Stromal Molecular Markers

Abstract:Purpose: To evaluate extracellular matrix regulators and inflammatory factors in a patient who developed ectasia after small incision lenticule extraction (SMILE) despite normal preoperative tomographic and biomechanical evaluation. Methods: The SMILE lenticules from both eyes of the patient with ectasia and three control patients (5 eyes) matched for age, sex, and duration of follow-up were used for gene expression analysis of lysyl oxidase (LOX), matrix metalloproteinase 9 (MMP9), collagen types I alpha 1 (COLIA1) and IV alpha 1 chain (COLIVA1), transforming growth factor-beta (TGF-beta), bone morphogenetic protein 7 (BMP7), interleukin-6 (IL-6), cathepsin K, cluster of differentiation 68, integrin beta-1, and tissue inhibitor of metalloproteinase-1 (TIMP1). Furthermore, the functional role of LOX was assessed in vitro by studying the collagen gel contraction efficiency of LOX overexpressing in primary human corneal fibroblast cells. Results: Preoperatively, manifest refraction was -9.25 diopters (D) in the right eye and -10.00 D in the left eye. Corneal thickness, Pentacam (OCULUS Optikgeräte GmbH, Wetzlar, Germany) tomography, and Corvis biomechanical indices (OCULUS Optikgeräte GmbH) were normal. The ectatic eye lenticule (left) had reduced expression of LOX and COLIA1 compared to controls without ectasia. Increased mRNA fold change expression of TGF-beta, BMP7, IL-6, cathepsin K, and integrin beta-1 was noted in the ectatic left eye compared to controls; however, MMP9 and TIMP1 levels were not altered. Ectopic LOX expression in human corneal fibroblast induced significantly more collagen gel contraction, confirming the role of LOX in strengthening the corneal stroma. Conclusions: Reduced preexisting LOX and collagen levels may predispose clinically healthy eyes undergoing refractive surgery to ectasia, presumably by corneal stromal weakening via inadequately cross-linked collagen. Preoperative molecular testing may reveal ectasia susceptibility in the absence of tomographic or biomechanical risk factors. [J Refract Surg. 2019;35(1):6-14.].

Abstract:Epithelial ingrowth is a rare but potentially sight-threatening complication caused by the invasion of corneal or conjunctival epithelial cells into the eye during ocular surgeries. DMEK is emerging as a widely used surgery for endothelial keratoplasty with its improved safety profile. We describe a case of epithelial ingrowth in the graft-host interface after uneventful DMEK associated with vitreous prolapse in the anterior chamber.

Abstract:Among eating disorders, the incidence and prevalence of anorexia nervosa and bulimia nervosa are reported to be on the increase. The diverse--and possibly fatal--consequences of these disorders are reviewed by the authors. The process of fasting-dieting, the bingeing episode or the methods applied for weight reduction (self-vomiting, use of laxatives, diuretics, diet pills, extreme exercise) can be harmful to the organism, either alone, or in combination. After reviewing the gastrointestinal, cardiovascular, pulmonary, renal, hematological, gynecological, endocrine, neurological, orthopedic, dermatologic, laryngologic and dental complications, the authors high-light anamnestic data and clinical signs on the basis of which eating disorders can be diagnosed.
Using Donor Lenticules Obtained Through Smile for an Epikeratophakia Technique Combined with Phototherapeutic Keratectomy

Abstract:PURPOSE: To investigate the feasibility, safety, and efficacy of using a donor lenticule created during small incision lenticule extraction (SMILE) in an epikeratophakia technique combined with phototherapeutic keratectomy (PTK).METHODS: Six patients with decreased visual acuity due to a recurrence of corneal dystrophy were included in this prospective pilot study. PTK was performed using the MEL 80 excimer laser (Carl Zeiss Meditec, Jena, Germany) followed by the transplantation of donor lenticules onto the recipient eyes. These lenticules were extracted during SMILE procedures using the VisuMax femtosecond laser (Carl Zeiss Meditec) on 6 donors with myopia and served as the epikeratophakia tissue.RESULTS: The surgeries and postoperative follow- up examinations were uneventful and no complications were noted over 6 months of follow- up. The epithelium remodelling was achieved within 1 month. At the last measurement, the corrected distance visual acuity of all recipient eyes gained at least two lines. Five eyes (83.3%) gained at least two lines of uncorrected distance visual acuity compared to their preoperative levels. Mean keratometric power increased by 5.97 +/- 3.73 diopters and central corneal thickness increased by 55.25 +/- 36.38 mu m. The epithelium healed and the lenticules remained clear over the follow-up period as observed under slit-lamp examination. Anterior segment optical coherence tomography observation showed the lenticule was transparent with a visible demarcation line during the follow-up examination.CONCLUSIONS: The use of a SMILE lenticule as a donor lenticule for an epikeratophakia technique appears feasible and safe in the short term. The predictability and long-term effects need further investigation.

Abstract:With the popularity of small incision lenticule extraction (SMILE), the clinical application of SMILE-derived lenticules has become a research hotspot due to the sufficient sources, good biocompatibility, and the ability of long-term storage. A large number of animal experiments and clinical studies have confirmed that SMILE-derived lenticules can be used for the treatment of hyperopia, presbyopia, keratoconus, and corneal ulcer and perforation. The safety and efficacy of this treatment are supported by many research studies. In order to provide reference for further research in this field, this review sheds light on the clinical application of lenticules and the issues that should be tackled before using them effectively and efficiently.
Two-year Outcome of a Patient after LASIK Following SMILE Lenticule in Situ Implantation to Treat a Rare Complication

Abstract:Abstract Background: To report a case of femtosecond laser-assisted in situ keratomileusis (FS-LASIK) following small incision lenticule extraction (SMILE) lenticule in situ implantation to treat a rare complication. Case presentation: The hyperopic left eye of a 46-year-old patient with refraction of +7.75 diopters sphere (DS)/-1.25 diopters cylinder (DC)×5°and corrected distance visual acuity (CDVA) of 20/50 mistakenly underwent SMILE procedure for myopic astigmatism (-8.50DS/-1.50DC×175°) due to medical record error. The extracted lenticule was subsequently re-implanted in situ. After 8 months, the left eye underwent FS-LASIK to correct hyperopia and astigmatism （+5.0DS/-0.75DC×100°. Two years after FS-LASIK, corneal tomography showed no ectasia and microscopy revealed transparent cornea. The left eye obtained CDVA of 20/50 and the refraction was -0.75DS/-0.25DC×165°. Conclusion: SMILE lenticule in situ implantation offers a solution for rare complications and correction of refractive error following lenticule re-implantation with FS-LASIK is feasible and effective. [The Histopathological Observation of Rabbit Corneas after Small Incision Lenticule Extraction].

Abstract:OBJECTIVE:To evaluate the morphological changes during corneal wound healing after small incision lenticule extraction (SMILE).METHODS:Twenty-one New Zealand white rabbits were randomly divided into 2 groups (group A of 12 rabbits and group B of 9 rabbits) using the toss of a coin. All the right eyes were treated. Group A underwent SMILE, and group B underwent femtosecond laser-assisted in situ keratomileusis (LASIK), with the same refractive corrections of-6.00 DS/-1.00 DC. The surface regularity on the refractive corneal lenticules and flaps was evaluated by scanning electron microscopy. Light microscopy and transmission electron microscopy were applied to observe changes in the cornea on postoperative week 1, months 1 and 3. The contralateral eye of three rabbits served as control.RESULTS:All the operations were successful, and the postoperative corneal healing was uneventful in both groups. The recovery of corneas was faster in the SMILE group by slit-lamp examination. Light microscopy confirmed the mild edema of corneal stroma with a clear intrastromal crevice in the SMILE group and the adhesion of the flap and stromal bed in the LASIK group on postoperative week 1. The filling of epithelial cells at the flap margin and the irregular arrangement of collagens were demonstrated in both groups. On postoperative month 1, stromal edema was dissipated, and collagens became more regular in both groups. The intrastromal crevice in the SMILE group disappeared on postoperative month 3. Transmission electron microscopy also revealed the edema of keratocyte mitochondria within 1 month, and the keratocytes recovered to normal on month 3. The surface regularity of the front surface was better than that of the back.CONCLUSIONS:Smooth intrastromal lenticules can be safely prepared by femtosecond laser. The corneal healing in the SMILE group was better than that in the LASIK group. The intrastromal crevice in the early postoperative period after SMILE should be paid attention to clinically. Abstract:PurposeTo report the incidence and outcomes of suction loss during small incision lenticule extraction (SMILE). MethodsThe incidence of suction loss was measured over 4000 consecutive SMILE procedures and categorized by cause, the interface in which suction was lost and management (restart/continue SMILE, re‐SMILE thinner cap, convert to laser in‐situ keratomileusis [LASIK]). One‐year outcomes were compared to the fellow eye where no suction loss occurred. ResultsThere were 20 cases of suction loss (0.50%): during the lenticule interface in seven eyes, lenticule side cut in one eye, cap interface in nine eyes and small incision for three eyes. Small incision lenticule extraction (SMILE) was continued in seven eyes, thinner cap SMILE in four eyes, LASIK in eight eyes, and the small incision was manually completed in one eye. Suction loss was caused by a Bell's reflex in 10 eyes, fixation light tracking in six eyes, patient anxiety in two eyes, a nociceptive reflex in one eye and false suction in one eye. There was no difference in results for suction loss and fellow eyes, respectively: uncorrected distance visual acuity was 20/20 or better in 100% in both groups, spherical equivalent was within ±0.50 D in 85% and 79%, one line loss of corrected distance visual acuity in 5% and 0%, and no eyes lost two lines. ConclusionSuction loss can be managed depending on the interface during which suction is lost. Treatment was completed on the same day in all instances. Visual and refractive outcomes were unaffected compared to the fellow eye in this series.

Long-term results of allogenic corneal lenticule of hyperopic SMILE for post-LASIK ectasia

Abstract:Post-laser-assisted in situ keratomileusis (LASIK) corneal ectasia is a severe complication of corneal refractive surgery, and cryopreserved lenticules from hyperopic small incision lenticule extraction (SMILE) may offer a promising treatment though their long-term safety and efficacy are still under investigation. In this prospective case series, six eyes from six patients with post-LASIK ectasia received lenticules (mean cryopreserved time: 63 days). The procedure involved lifting the corneal flap, implanting the lenticule, and repositioning the flap. Over a follow-up period of at least one year, uncorrected distance visual acuity (UDVA) improved from 1.52 ± 0.40 preoperatively to 0.74 ± 0.28 LogMAR. Two eyes gained one line of corrected distance visual acuity (CDVA), three gained two lines, and one gained over three lines. Spherical equivalents decreased from -14.67 ± 2.36 D to -8.75 ± 4.03 D (p = 0.02). Mean anterior K and total corneal refractive power decreased (p < 0.05). Thinnest corneal thickness increased from 359.2 ± 39.3 μm to 401.7 ± 53.4 μm (p = 0.02). These findings support the potential of cryopreserved lenticules for treating post-LASIK ectasia, though further refinement in refractive predictability is needed.

Abstract:A 22-year-old male presented with complaints of blurred vision in his right eye over the past 2 years following small incision lenticule extraction (SMILE) surgery conducted 4 years ago. Following a thorough ocular examination and evaluation, he was diagnosed with corneal ectasia in the right eye after the SMILE procedure. Subsequently, the patient underwent corneal cross-linking (CXL) treatment in the right eye to prevent the progression of the condition. After 3 months of treatment, the corneal ectasia remained stable. This article outlines the process of diagnosis and treatment, reviews the corneal conditions prior to the SMILE surgery, and analyzes the possible reasons behind the occurrence of postoperative corneal ectasia.

Abstract:Purpose: To describe the development of bacterial keratitis after small incision lenticule extraction in 5 patients and to explore its appropriate therapies. Methods: We retrospectively summarized the clinical treatments of five patients with postoperative bacterial infection after small incision lenticule extraction, who were referred to our hospital from 2019 to 2021. Results: Five male patients had undergone bilateral SMILE in the local hospital due to myopia aged from 18 to 26 years. The onset of keratitis during 1-3 days postoperatively and four of them were severe infection (2 bilateral, 2 unilateral). In five cases, 1 patient (1 eye) who was infected mild keratitis after SMILE was treated with only topical antibiotics; the others who respond poorly to topical antibiotics require surgical treatment, which 1 patient (1 eye) infected necrotic mass of the corneal cap was scraped and irrigated with antibiotic, and 3 patients (5 eyes) were treated by converting the cap to flap, curetting the necrotic tissue and irrigating with the antibiotic solution. In all patients, the duration from onset to resolution was 1-5 weeks. The final uncorrected visual acuity was above 20/32. Conclusion: Owing to the upward popularity of refractive surgery, the incidence of keratitis after SMILE should not be ignored. Early diagnosis and timely treatment of post-SMILE keratitis are essential. For severe keratitis that fails to respond to topical antibiotics, the corneal cap should be opened as a flap.
