Abstract:Background Tooth loss is a common oral health problem that can significantly impact an individual's quality of life. Despite a general decrease in severe tooth loss, it remains prevalent globally and can be caused by multiple factors, including chronic inflammation and systemic diseases such as diabetes and cardiovascular disease. To investigate the relationship between peripheral blood indices and tooth loss by analyzing data from the National Health and Nutrition Examination Survey (NHANES) and uncover any potential clinical biomarkers in peripheral blood for tooth loss. Methods This study analyzed data from the NHANES collected from 2017 to March 2020. Age, gender, race, C-reactive protein, and complete blood count were employed as variables. Multivariate logistic regression models to examine the association between the important variables and tooth loss. Odds ratios (OR) with 95% confidence intervals (95%CI) was used to evaluate the significance of association. Results A total of 9094 participants were finally included in the trial. In peripheral blood, tooth loss was highly linked with higher levels of segmented neutrophils (OR = 1.12; 95% CI: 1.05–1.20; p = 0.0017) and red cell distribution width (OR = 1.12, 95% CI: 1.07–1.16; p < 0.001). The degree of the connection between these factors and tooth loss varied by age, gender, and ethnicity, as shown by subgroup analysis. Conclusions The results suggest that higher segmented neutrophils and red cell distribution width levels in peripheral blood may be associated with a higher risk of tooth loss and could serve as risk markers for tooth loss.

Abstract:Objectives The purpose of this cone beam computed tomography (CBCT) study was to describe facial alveolar bone alterations and gray value (GV) changes around implants in aesthetic anterior maxilla with simultaneous guided bone regeneration (GBR) and to investigate potential influence of factors related to vertical alveolar bone stability, such as particle sizes of the deproteinized bovine bone mineral (DBBM) used. Materials and Methods A retrospective study design was adopted. Four facio-palatal cross-sectional CBCT images of 166 implants from 124 patients who had maxillary anterior implants were obtained. Measurements of the vertical facial alveolar bone level (VFBL), horizontal facial alveolar bone thickness (HFBT), and GV were identified of 1-3 years. Then, correlation coefficients for those parameters were calculated. Finally, linear mixed models were established to investigate potential factors influencing vertical alveolar bone resorption. Results Facial alveolar bone underwent fast bone modeling and remodeling during the first 6 months, with decreases of 1.00 +/- 1.19 and 0.74 +/- 0.86 mm in VFBL and HFBT in implant shoulder, respectively, and there was positive and significant correlation between the alterations in VFBL and HFBT1 (r(s) = .516, p = .000). Linear mixed models identified particle size of DBBM as a critical factor associated with vertical bone resorption (p = .000). The GV gradually increased during the follow-up period. Conclusions Rapid and unavoidable peri-implant bone resorption usually happened during the first 6 months after implant placement. GBR is a predictable treatment for maxillary anterior implants, since GV has an increasing trend. Large bone particles of DBBM help maintain vertical alveolar bone stability.

Abstract:Periodontal disease is a site-specific disease affecting the supporting tissues of the teeth. It is useful for the clinician to have information about the prevalence and severity of alveolar bone loss (ABL) according to the site, location, and position of the teeth for a better treatment plan and expected prognosis. This study aimed to assess the prevalence and severity of ABL at mesial, distal, buccal and lingual sites of teeth in different locations, positions and sides of the dentition. The ABL of 20,620 sites of 5155 teeth in 212 patients was assessed using cone-beam computed tomography from the cemento-enamel junction to the crest of the alveolar bone. The prevalence of ABL was higher in the interproximal sites as well as anterior and mandibular teeth compared to their counterparts. Buccal sites and anterior teeth revealed higher ABL levels than the other tooth sites and posterior teeth, respectively. Furthermore, associations in the severity of ABL were observed between distal and mesial sites, buccal and lingual sites, maxillary and mandibular teeth, anterior and posterior teeth, and right and left sides. This study showed that the prevalence and severity of ABL differ from one tooth site to another and according to the tooth's location in the dentition. Higher prevalences were found in the interproximal sites, anterior teeth and mandibular teeth; higher ABL was found in buccal and distal sites, with the strongest associations between distal and mesial sites, buccal and lingual sites, and right and left sides.

Freemium Model in Cloud Computing

Abstract:Software as a Service (SaaS) is the most mature and popularly adopted cloud services, and freemium is a widely used business model in SaaS. Freemium, combining “free” and “premium”, involves giving away for free a certain level or type of consumption, while making money on premium consumption. For example, SaaS vendors offer their basic version products for free while they charge for the premium features in their full version products. Many SaaS vendors have jumped on the freemium bandwagon fast. However, not every companies work the freemium model successfully. The conversion rate from free users to paying customer are typically very small, roughly 3%~5% in successful cases. Many companies found the "freemium" strategy is turning out to be a costly trap, leaving them with higher operating costs and lots of freeloaders. SaaS vendors thus, are eager to know how to increase the conversion rate. What are the key factors that make free users become paying customers? What are the key differences between the two groups? How do the two groups of users see their SaaS products or services differently, in terms of quality, value, and satisfaction? To what extent should the quality or functionalities be embedded into free version to appropriately convert free users to paying customers? To answer these questions, this study builds on IS success model, which provides a theoretical foundation to understand the relationship between software quality, perceived value, satisfaction and customers’ intention to pay. Our study aims to explain a phenomenon that can’t be answered well by extant literature: why some users perceive high value of SaaS, are satisfied with SaaS, but they still have very low intention to pay for SaaS. Our study departures from extant SaaS literature by proposing two user specific characteristics -fit and price consciousness that will moderate the well-examined relationships in the IS success model. Different from extant studies that argue each of the perceived benefits, user needs, or satisfaction simply has a direct impact on users’ intention to pay, our study proposes that it is the “fit” between users’ needs and perceived value that will determine users’ final intention to pay. Moreover, users’ price consciousness will strongly moderate the impact of user satisfaction on their intention to pay. We plan to empirically test our proposed model by collecting 638 SaaS users.
A cross-sectional study for prevalence and risk factors of peri-implant marginal bone loss

Abstract:Statement of problem Progressive peri-implant marginal bone loss and peri-implantitis have become a growing problem, but cross-sectional studies on their prevalence and risk factors are sparse. Purpose The purpose of this cross-sectional clinical study was to investigate the prevalence of peri-implant marginal bone loss (MBL) and to identify systemic and local risk factors. Material and methods All adult patients who had received dental implants at the National Taiwan University Hospital (NTUH) during 2009 or 2010 were included. Their medical records were collected from the NTUH-integrative Medical Database. Consecutive follow-up radiographs were accessed for severity of MBL. The influence of each factor on MBL was estimated by using generalized estimating equations (GEEs). Results A total of 732 participants with 1873 implants were analyzed (mean follow-up: 5.30 years). The prevalence of MBL was 59.15% at the individual level and 49.55% at the implant level. The risk indicators identified for the presence of MBL were follow-up period of more than 2 years, diagnosis of diabetes within 12 months, radiation therapy (2 years after implant placement), implant location at maxillary canine (compared with mandibular molar), and implants from the Nobel Biocare brands (Brånemark System and NobelActive). A second multivariate GEE model confirmed the association of progressive MBL with implant location at the maxillary canine and mandibular incisor and implant brand or design. Conclusions The identified risk indicators for MBL were longer follow-up period, diagnosis of diabetes, radiation therapy, implant location at maxillary canine, and implant brand or design.

Abstract:Objectives The recently introduced Implant Disease Risk Assessment (IDRA) identifies a restoration margin-alveolar bone crest (RM-AC) distance of less than 1.5 mm as a key risk factor for peri-implant disease among eight major risk factors. This study evaluated the impact of the RM-AC distance on marginal bone loss (MBL) through radiographic analysis. Methods This retrospective cross-sectional study included 77 partially edentulous patients (39 females and 38 males, aged 22 to 76 years) with 202 platform-switched conical connection implants, cement-retained, implant-supported fixed restorations, and bone-level implants placed between 2016 and 2021. Dental implants were followed for least 6 to 36 months at follow up functional loading. Study participants were categorized into Group A (RM-AC distance ≤ 1.5 mm, n=69) and Group B (RM-AC distance > 1.5 mm, n=133). Twelve patients in Group B and five patients in Group A had no history of periodontal disease. The MBL was measured radiographically from the most coronal point of the implant shoulder to the alveolar bone, and the RM-AC distance was measured from the restoration margin to the alveolar crest. Multinomial logistic regression analysis was used for statistical evaluation. Results The incidence of MBL in Group A was statistically significant and 3.42 times higher than that in Group B. The rate of MBL in periodontitis Stage 4 was found to be 26.31 times higher than that in periodontitis Stage 2. The incidence of MBL was 6.097 and 5.02 times higher with increasing implant diameter and length, respectively. Conclusion This study conclusively demonstrates that RM-AC distance ≤ 1.5 significantly increases the risk of MBL, particularly in patients with a history of periodontal disease. Clinical significance This study highlights the critical role of maintaining an RM-AC distance greater than 1.5 mm in the prevention of MBL, particularly in patients with a history of periodontal disease. Since implant diameter and length have a significant impact on the risk of MBL, it emphasizes that implant demographics should also be carefully evaluated.

Abstract:Objective: The current study explores whether there is a clinically relevant distinction in the measurement of marginal bone loss when comparing high-dose (HD) versus low-dose (LD) cone beam computed tomography (CBCT) protocols in small and large acquisition volumes. Material and Methods: CBCTs of four human cadaveric preparates were taken in HD and LD mode in two different fields of view 8 × 8 cm2 (LV) and 5 × 5 cm2 (SV). In total, 43 sites of 15 teeth were randomly chosen, and marginal bone loss was measured twice in all protocols at 43 sites of 15 teeth by one calibrated investigator. Bland-Altman plots and Lin’s concordance correlation coefficient (CCC) were calculated to assess the extent of agreement of the measurements. Additionally, the rater scored the certainty in each of the measurements. Results: For HD-CBCT CCC of measurements obtained using SV versus LV was 0.991. CCC of measurements obtained using SV versus LV of LD-CBCT was 0.963. Both CCC values indicated excellent agreement between the two volumes in both protocols. CCC also indicated high intramodality correlation between HD-CBCT and LD-CBCT independent of the acquisition volume (0.963 – 0.992). Bland-Altman plots also indicated no substantial differences. Results of certainty scoring showed significant differences (p = 0.004 (LV), p < 0.001(SV)) between the LD and HD-CBCT. Conclusions: Accuracy of measurements of bone loss shows no clinical noticeable effects depending on the CBCT volume in this ex vivo study. There appears to be no relevant advantage of SV over LV, neither in HD-CBCT nor in LD-CBCT and additionally no relevant advantage of HD versus LD in visualizing marginal bone loss.

Abstract:Aim To investigate whether a progressive marginal bone loss (PMBL) occurring beyond the initial bone remodeling (IBR) is linked with bleeding on probing. Materials and Methods A total of 70 partially edentulous patients exhibiting 112 two‐piece bone‐level implants were included in this retrospective study. Panoramic radiographs were obtained after implant insertion (T0), after delivery of a final prosthetic restoration (T1) and subsequently during the 1‐(T2), 5‐(T3), 10‐(T4), and 15‐years (T5) follow‐up visits. At each time point, radiographic marginal bone levels were assessed from the implant shoulder to the first bone‐to‐implant contact at mesial and distal aspects. The IBR was defined as a bone loss occurring up to prosthesis delivery, that is, from T0 to T1. The PMBL was defined as bone loss occurring after T1. At T2, T3, T4, and T5, the presence or absence of bleeding on probing (BOP) was recorded at four sites. A median regression with mixed models was performed to assess the difference of PMBL in PMBL + BOP+ and PBML + BOP− groups. Results Over the mean implant functioning time of 4.44 ± 4.91 years, 38 (34%) implants showed no PBML, whereas 74 (66%) implants featured PMBL. Of these, 35 (47%) and 39 (53%) implants were assigned to the PMBL + BOP− and PMBL + BOP+ groups, respectively. The mean PMBL after 1, 5, 10, and 15 years were comparable between implants featuring PMBL with or without BOP. At 1 year, BOP intensity significantly correlated PMBL, with each increase in one BOP‐positive site being associated with increase in PMBL by 0.55 mm (p = 0.038), whereas this association was not found at 5, 10, and 15 years. The IBR values in the no PBML, PMBL + BOP+, and PBML + BOP− groups were −0.24 ± 0.31, −0.41 ± 0.59, and −0.24 ± 0.33 mm, respectively, with no significant differences found among the groups. Conclusion Progressive bone loss at implant sites is not always linked with bleeding on probing.

Abstract:Background: Periodontal disease is caused by subgingival bacteria that adversely affect the host immune system and create and maintain unmitigated inflammation in gingival and periodontal tissues. The condition is also linked to systemic conditions including cardiovascular disease, diabetes, and arthritis. Periodontitis elevates the bacterial load and spreads systemic inflammation through infection and inflammation. The main radiographic sign of periodontitis is marginal bone loss. Risk factors, including medications, smoking, age, and gender, are known to influence periodontal health. However, there is little information about the impact of systemic conditions, and medications on tooth wear. Aim: To assess the association between systemic conditions and medications and radiographic signs of tooth wear (TW) and marginal bone loss (MBL) . Materials and methods: This retrospective analysis was conducted on a group of 2223 consecutive patients who came for dental treatment in the clinics of a large HMO in Israel. Data available for the study included details of concomitant systemic diseases and medication and full-mouth radiographic surveys. Odds ratio and logistic regression analysis were used to detect associations between systemic conditions and medication, and MBL and TW . Results: The results indicated an elevated odds ratio for TW associated with age, gender, and smoking across all age groups. Among young patients, those using proton pump inhibitors and psychiatric medications had an elevated risk of TW. Age, smoking, and diabetes conditions were associated with an increased odds ratio for MBL in all age groups. Psychiatric medications and gender elevated the odds ratio for MBL only among older patients. Conclusion: Our results highlight the significant impact of age, gender, and smoking on tooth wear and extend these risks to alveolar bone loss when combined with diabetes and psychiatric conditions.

Abstract:Background The aim of this study was to evaluate the incidence of preloading crestal bone loss (PLCBL) and to identify the patient‐related and implant‐related factors associated with PLCBL. Methods This retrospective cohort examined the dental records of patients who received at least one dental implant. PLCBL was defined as a reduction ⩾0.5 mm and severe PLCBL (primary variable) as a reduction ⩾1.5 mm in mesial and/or distal bone level, measured from the day of implant placement to uncovering or abutment installation/crown delivery. The incidence of PLCBL and patient and implant variables were recorded. Bivariate analysis and binary logistic regression identified factors associated with PLCBL ⩾0.5 mm and ⩾1.5 mm. Results A total of 746 dental implants placed in 361 patients from January 2011 to July 2021 was included in the analyses. Of the implants assessed, 24.4% (n = 182) exhibited PLCBL ⩾ 0.5 mm and 10.5% (n = 78) presented severe PLCBL (i.e., ⩾1.5 mm). Abstract:Aim To evaluate long‐term survival and success of dental implants and evaluate indicators affecting the long‐term outcome. Materials and Methods Implant survival, success and crestal bone loss (BL) over time were evaluated. For covariates at patient level, Kaplan–Meier estimates of implant survival were compared between groups with the log‐rank test. Observed mean bone loss (MBL) was plotted as a function of time. Cumulative frequencies of BL were plotted for different post‐op times. Uni‐ and multivariate analysis was performed. Simple linear mixed and multiple linear mixed models for BL at 1, 5 and 10 years were fitted. Results 407 patients (221 women, 186 men; mean age 64.86 years (range 28–92, SD 10.11)), with 1482 implants, responded. Absolute implant survival was 94.74%; MBL was 0.81 mm (SD 1.58, range 0.00–17.00) after an average follow‐up of 10.66 years (range 10–14, SD 0.87). Implant survival was influenced on implant level by smoking, implant width and early bone loss (EBL) > 0.5 mm; on patient level by a history of periodontitis. Indicators influencing MBL after the 1st year were abutment height, type of surgery and implant width, while after 5 and 10 years of function were abutment height, EBL > 0.5 mm and smoking. Conclusion Implant survival was significantly affected by a history of periodontitis on patient level and by smoking, implant width and EBL > 0.5 mm on implant level. Late bone loss was significantly affected by abutment height, EBL > 0.5 mm and smoking. Trial Registration: B670201524796

Influence of Examiner Experience on the Measurement of Bone-Loss by Low-Dose Cone-Beam Computed Tomography: An Ex Vivo Study

Abstract:The aim of this study was to investigate the influence of examiner experience on measurements of bone-loss using high-dose (HD) and low-dose (LD) CBCT. Three diagnosticians with varying levels of CBCT interpretation experience measured bone-loss from CBCT scans of three cadaveric heads at 30 sites, conducting measurements twice. Between the first and second measurements, diagnostician 2 and diagnostician 3 received training in LD-CBCT diagnostics. The diagnosticians also classified the certainty of their measurements using a three-grade scale. The accuracy of bone-loss measurements was assessed using the absolute difference between observed and clinical measurements and compared among diagnosticians with different experience levels for both HD and LD-CBCT. At baseline, there was a significant difference in measurement accuracy between diagnostician 1 and diagnostician 2, and between diagnostician 1 and diagnostician 3, but not between diagnostician 2 and diagnostician 3. Training improved the accuracy of both HD-CBCT and LD-CBCT measurements in diagnostician 2, and of LD-CBCT measurements in diagnostician 3. Regarding measurement certainty, there was a significant difference among diagnosticians before training. Training enhanced the certainty for diagnosticians 2 and 3, with a significant improvement noted only for diagnostician 3. Examiner experience level significantly impacts the accuracy and certainty of bone-loss measurements using HD- and LD-CBCT.
Factors Influencing Early Marginal Bone Loss around Dental Implants Positioned Subcrestally: A Multicenter Prospective Clinical Study

Abstract:Early marginal bone loss (MBL) is a non-infective remodeling process of variable entity occurring within the first year after implant placement. It has a multifactorial etiology, being influenced by both surgical and prosthetic factors. Their impact remains a matter of debate, and controversial information is available, particularly regarding implants placed subcrestally. The present multicenter prospective clinical study aimed to correlate marginal bone loss around platform-switched implants with conical connection inserted subcrestally to general and local factors. Fifty-five patients were enrolled according to strict inclusion/exclusion criteria by four clinical centers. Single or multiple implants (AnyRidge, MegaGen, South Korea) were inserted in the posterior mandible with a one-stage protocol. Impressions were taken after two months of healing (T1), screwed metal-ceramic restorations were delivered three months after implant insertion (T2), and patients were recalled after six months (T3) and twelve months (T4) of prosthetic loading. Periapical radiographs were acquired at each time point. Bone levels were measured at each time point on both mesial and distal aspects of implants. Linear mixed models were fitted to the data to identify predictors associated with MBL. Fifty patients (25 male, 25 female; mean age 58.0 ± 12.8) with a total of 83 implants were included in the final analysis. The mean subcrestal position of the implant shoulder at baseline was 1.24 ± 0.57 mm, while at T4, it was 0.46 ± 0.59 mm under the bone level. Early marginal bone remodeling was significantly influenced by implant insertion depth and factors related to biological width establishment (vertical mucosal thickness, healing, and prosthetic abutment height). Deep implant insertion, thin peri-implant mucosa, and short abutments were associated with greater marginal bone loss up to six months after prosthetic loading. Peri-implant bone levels tended to stabilize after this time, and no further marginal bone resorption was recorded at twelve months after implant loading.

Forty patients with 201 implants were in the diseased-cohort, while thirty-three patients with 90 implants were in the control-cohort. Patient blood 25(OH)D levels were assessed through quantitative blood test. The correlation between abnormal 25(OH)D levels and disease status of each patient was assessed using Fisher's exact tests. The correlation of each implant's outcomes with vitamin-D status was assessed using Kaplan-Meier survival analysis and Mann-Whitney U tests. Results: Patients with blood 25(OH)D levels >70 ng/mL (hypervitaminosis-D) had a 21.1-fold increase in the risk of implant failure or severe peri-implant bone loss regarding patients with intermediate (>30, ≤70 ng/mL) levels. Kaplan-Meier survival analysis revealed that implants in the hypervitaminosis-D cohort had a survival probability of 73.7 % (95 % CI:56.5-84.5 %) at 19-years after surgery, compared to 95 % for implants in patients with intermediate 25(OH)D levels (95 % CI:88.3-97.9 %). Additionally, implants in the hypervitaminosis-D cohort lost bone faster than implants in the intermediate cohort. These results were specific to the patient cohort with elevated blood 25(OH)D levels and not observed in patients taking vitamin-D supplementation. The impact of hypervitaminosis-D was enriched for implants in the maxilla, and not as apparent for implants in the mandible. Conclusions: Blood 25(OH)D levels >70 ng/mL were correlated with adverse implant outcomes, including implant failure and peri-implant bone loss, especially in the maxilla. Clinical relevance: These results suggest that hypervitaminosis D may be a previously unidentified risk factor for dental implant complications and should be further investigated to elucidate the underlying mechanism.
Comparison of the sensitivity of detecting risk factors for periodontitis using conventional radiography

Abstract:Introduction. Periodontitis is a condition of tooth supporting structures, which are responsible for normal dental morphology and function. Bacterial biofilm formation in the presence of local risk factors plays a fundamental role in the development of periodontitis. Early and accurate detection of risk factors which promote dental plaque accumulation is the basis of treatment and prophylaxis. Aim. The aim of the study was to assess overhanging dental restorations and their impact on the maxillary alveolar process margin and the alveolar mandible based on panoramic radiography. Furthermore, the correlation between the type of restoration and the magnitude of bone loss was analysed. Material and methods. A retrospective analysis of 200 panoramic radiographs showing restorations in permanent dentition was performed using the Planmeca Romexis Viewer software. Further analysis included 113 radiographs with the presence of a homologous tooth on the opposite side of the dental arch. In the study, we analysed the following variables: age, gender, the bone level, occlusal contact points between the opposite teeth and the condition of the tooth adjacent to overhanging dental restoration. We also assessed the type of incorrect dental restoration according to G.V. Black classification. The obtained data was analysed statistically, with p < 0.005 considered statistically significant. The study was approved by the Bioethics Committee of the Medical University of Warsaw (approval no. KB/115/A/2012). Results. We obtained data on the prevalence and magnitude of overhanging dental restorations, the type of restoration and the coexisting bone lesion. The largest size of the overhanging dental restoration was associated with Class II (according to G. V. Black) mesio-occlusal-distal cavities (1.15 mm; p < 0.03), whereas the smallest ones were observed for occlusal-distal cavities (0.91 mm; p < 0.001). The Spearman’s rank correlation coefficient between mesio-occlusal-distal cavities and radiographically measured size of bone lesions was positive (R = 0.51; p = 0.03). Conclusions. The conducted study revealed a significant positive correlation between the mean size of overhangs and radiographically measured size of bone loss. Adjunctive radiology is an important aid in the detection of risk factors for periodontitis.
Evaluation of Root Resorption after Comprehensive Orthodontic Treatment Using Cone Beam Computed Tomography (CBCT): a Meta-Analysis

Abstract:Background: Orthodontic treatment can result in root resorption (RR). Traditional two-dimensional (2D) data exhibit magnification, deformation and positioning problems. Cone beam computed tomography (CBCT) contains more accurate three-dimensional (3D) information. This study identified and qualified the extent and location of root resorption using cone beam computed tomography (CBCT) after comprehensive orthodontic treatment. Methods: Studies comparing the RR before and after comprehensive orthodontic treatment using CBCT were identified using electronic searches of databases, including Cochrane, PubMed, EMBASE, China National Knowledge Infrastructure (CNKI) and Web of Science, and manual searches in relevant journals and the reference lists of the included studies until Oct 25, 2017. The extraction of data and the risk of bias evaluation were conducted by two investigators independently. The methodological quality of the included studies was assessed using the methodological index for non-randomized studies (MINORS). Studies that reported the length and volume of teeth were used for quantitative analyses. Results: Twelve studies were included in the meta-analysis. The length of all teeth after intervention was significantly shorter than that before treatment (MD = 0.80, 95% CI 0.56, 1.03, P < 0.00001). The sequence of RR from heaviest to lightest was maxillary lateral incisors, maxillary central incisors, mandibular anterior teeth, and maxillary canines. Studies were divided into two subgroups based on the use of tooth extraction. Root shortening after treatment was observed in both groups, and extraction caused more root resorption than was observed in the non-extraction group. Conclusions: There were different degrees of root resorption after orthodontics, but it was clinically acceptable. Root resorption established in CBCT research was less serious and more accurate than that observed in the two-dimensional research. Current evidence suggests that root length and volume were reduced after orthodontic treatment. The order of the amount of RR was maxillary lateral incisors, maxillary central incisors and mandibular anterior teeth. Most of the articles were complicated by different confounding factors. Therefore, more high-quality clinical trials are needed to determine the risk factors of root resorption and optimal protocols for treatment and to draw more reliable conclusions.
Prevalence of periodontitis and alveolar bone loss in a patient population at Harvard School of Dental Medicine

Abstract:Abstract Background Although several studies assessed the prevalence of alveolar bone loss, the association with several risk factors has not been fully investigated. The aim of this article is to measure the prevalence of periodontitis by calculating the mean alveolar bone loss/level of posterior teeth using bitewing radiographs among the patients enrolled in the clinics at Harvard School of Dental Medicine and address risk factors associated with the disease. Methods One thousand one hundred thirty-one patients were selected for radiographic analysis to calculate the mean alveolar bone loss/level by measuring the distance between the cementoenamel junction and the alveolar bone crest on the mesial and distal surfaces of posterior teeth. Linear regression with Multi-level mixed-effect model was used for statistical analysis adjusting for age, sex, race, median household income, and other variables. Results Mean alveolar bone level of the whole sample was 1.30 mm (±0.006). Overall periodontitis prevalence for the sample was 55.5% (±1.4%). Moderate periodontitis prevalence was 20.7% (±1.2%), while 2.8% (±0.5%) of the whole sample had severe periodontitis. Adjusted mean alveolar bone loss was higher in older age groups, males, Asian race group, ever smokers, and patients with low median household income. Conclusion The effect of high household income on the amount of bone loss can be powerful to the degree that high household income can influence outcomes even for individuals who had higher risks of developing the disease. Public health professionals and clinicians need to collaborate with policy makers to achieve and sustain high quality of healthcare for everyone.

Abstract:PURPOSE: Patients with systemic scleroderma (SSc) often suffer from premature tooth loss. The aim of this study was to radiologically investigate bone loss at dental implants in patients with SSc and compare it with data from the literature on healthy patients.MATERIALS AND METHODS: Mesial and distal bone level changes at implants were independently determined on panoramic and intraoral radiographs. They were double-checked after determination of evaluability by three different raters. Cohen's kappa was used to test for interrater reliability. Mean bone loss was estimated using linear regression analysis considering the patient as a random-effect implant and performed separately in predefined implant regions for different time points and for the mesial and distal sides of the implants.RESULTS: Mesial and distal bone level changes were analyzed in 61 implants using periapical and panoramic radiographs. In total, 114 radiographs from 18 patients were evaluable in both the mesial and distal regions. After a maximum observation period of 60 months, the mean peri-implant bone loss was 1.68 mm (range: 0.83 to 2.54 mm) at the distal aspect and 1.65 mm (range: 0.81 to 2.48 mm) at the mesial aspect in the right posterior mandible (region 44 to 47 [FDI numbering system]), whereas in the left posterior maxilla (regions 24 to 27), the mean peri-implant bone loss was 0.61 mm (range: 0.32 to 0.91 mm) at the distal aspect and 0.59 mm (range: 0.16 to 1.03 mm) at the mesial aspect. The mean bone loss 60 months after surgery was 1.05 mm (range: 0.85 to 1.25 mm).CONCLUSIONS: Marginal bone loss at implants in patients with SSc is comparable to data from the literature collected in healthy subjects.

Abstract:Background/Objectives: The general condition of implantology patients is crucial when considering the long- and short-term survival of dental implants. The aim of the research was to evaluate the correlation between the new corticalization index (CI) and patients' condition, and its impact on marginal bone loss (MBL) leading to implant failure, using only radiographic (RTG) images on a pixel level. Method: Bone near the dental implant neck was examined, and texture features were analyzed. Statistical analysis includes analysis of simple regression where the correlation coefficient (CC) and R2 were calculated. Detected relationships were assumed to be statistically significant when p < 0.05. Statgraphics Centurion version 18.1.12 (Stat Point Technologies, Warrenton, VA, USA) was used to conduct the statistical analyses. Results: The research revealed a correlation between MBL after 3 months and BMI, PTH, TSH, Ca2+ level in blood serum, phosphates in blood serum, and vitamin D. A correlation was also observed between CI and PTH, Ca2+ level in blood serum, vitamin D, LDL, HDL, and triglycerides on the day of surgery. After 3 months of the observation period, CI was correlated with PTH, TSH, Ca2+ level in blood serum, and triglycerides. Conclusion: The results of the research confirm that the general condition of patients corresponds with CI and MBL. A patient's general condition has an impact on bone metabolism around dental implants. Implant insertion should be considered if the general condition of the patient is not stable. However, CI has not yet been fully investigated. Further studies are necessary to check and categorize the impact of corticalization on marginal bone loss near dental implants.
