Abstract:Prostate cancer (PCa) incidence and mortality rate vary among racial and ethnic groups with the highest occurrence in African American (AA) men who have mortality rates twice that of Caucasians (CA). In this study, we focused on differential expression of proteins in AA prostate cancer compared to CA using Protein Pathway Array Analysis (PPAA), in order to identify protein biomarkers associated with PCa racial disparity. Fresh frozen prostate samples (n=90) obtained from radical prostatectomy specimens with PCa, including 25 AA tumor, 21 AA benign, 23 CA tumor, 21 CA benign samples were analyzed. A total of 286 proteins and phosphoproteins were assessed using PPAA. By PPAA analysis, 33 proteins were found to be significantly differentially expressed in tumor tissue (n=48, including both CA and AA) in comparison to benign tissue (n=42). We further compared protein expression levels between AA and CA tumor groups and found that 3 proteins were differentially expressed (P<0.05 and q<5%). Aurora was found to be significantly increased in AA tumors, while Cyclin D1 and HNF-3a proteins were downregulated in AA tumors. Predicted risk score was significantly different between AA and CA ethnic groups using logistic regression analysis. In conclusion, we identified Aurora, Cyclin D1 and HNF-3a proteins as being differentially expressed between AA and CA in PCa tissue. Our study suggests that these proteins might be involved in different pathways that lead to aggressive PCa behavior in AA patients, potentially serving as biomarkers for the PCa racial disparity.

Abstract:Prostate cancer is the second most common cancer in men in the United States, and racial disparities are greatly observed in the disease. Specifically, African American (AA) patients have 60% higher incidence and mortality rates, in addition to higher grade and stage prostate tumors, than European American (EA) patients. In order to narrow the gap between clinical outcomes for these two populations, genetic and molecular signatures contributing to this disparity have been characterized. Over the past decade, profiles of prostate tumor samples from different ethnic groups have been developed using molecular and functional assays coupled with next generation sequencing or microarrays. Comparative genome-wide analyses of genomic, epigenomic, and transcriptomic profiles from prostate tumor samples have uncovered potential race-specific mutations, copy number alterations, DNA methylation, and gene expression patterns. In this study, we reviewed over 20 published studies that examined the aforementioned molecular contributions to racial disparities in AA and EA prostate cancer patients. The reviewed genomic studies revealed mutations, deletions, amplifications, duplications, or fusion genes differentially enriched in AA patients relative to EA patients. Commonly reported genomic alterations included mutations or copy number alterations of FOXA1, KMT2D, SPOP, MYC, PTEN, TP53, ZFHX3, and the TMPRSS2-ERG fusion. The reviewed epigenomic studies identified that CpG sites near the promoters of PMEPA1, RARB, SNRPN, and TIMP3 genes were differentially methylated between AA and EA patients. Lastly, the reviewed transcriptomic studies identified genes (e.g. CCL4, CHRM3, CRYBB2, CXCR4, GALR1, GSTM3, SPINK1) and signaling pathways dysregulated between AA and EA patients. The most frequently found dysregulated pathways were involved in immune and inflammatory responses and neuroactive ligand signaling. Overall, we observed that the genomic, epigenomic, and transcriptomic alterations evaluated between AA and EA prostate cancer patients varied between studies, highlighting the impact of using different methods and sample sizes. The reported genomic, epigenomic, and transcriptomic alterations do not only uncover molecular mechanisms of tumorigenesis but also provide researchers and clinicians valuable resources to identify novel biomarkers and treatment modalities to improve the disparity of clinical outcomes between AA and EA patients.

Abstract:Abstract Background: In recent years remarkable ethnic/racial differences in the frequency of prostate cancer (CaP) driver gene alterations have emerged. Along the lines of ethnic differences in CaP genomes, oncogenic activation of ERG proto-oncogene and deletion of the PTEN tumor suppressor gene are most widely studied. These findings underscore distinct biology of prostate cancer in different ethnicities and races. We have reported the cumulative analyses of 435 patients (whole genome sequencing (WGS): 14, FISH evaluations: 101, SNP array: 320) comparing CaP genomes of African American (AA) and Caucasian American (CA) patients. An AA CaP genome-associated deletion was identified and mapped to the Limbic System-Associated Membrane Protein (LSAMP) gene locus. Further examination of the data indicated the AA CaP-genome associated deletion of the Chromodomain Helicase DNA Binding Protein 1(CHD1) gene. The goal of the current study is the comparative assessment of frequencies and prognostic associations of CaP genome alterations in the context of racial/ethnic differences. Methods: A prostate tumor tissue microarray (TMA) of benign and tumor foci (500+ cores) was generated by sampling 2-3 cores representing the range of cell morphology in matched cohort of 42 AA and 59 CA patients with up to 20 years of follow-up. Comparative evaluation of frequencies and prognostic associations of ERG oncoprotein by immunohistochemistry and the deletion of LSAMP, CHD1, and PTEN genes by FISH was performed. ERG frequencies were further assessed in index tumors of Chinese CaPs (N=100) and were compared to ERG frequencies in index tumors of patients from the United States equal-access military health-care system (N=336 AA and N=594 CA). Results: Higher frequencies of recurrent deletions of CHD1 (29% AA vs. 10% CA p=0.017) and LSAMP (26% AA vs. 7% CA, p=0.006) were identified in AA CaP compared to CA CaPs. These deletions were associated with significantly higher risk of rapid biochemical recurrence. In contrast, PTEN deletions (15% AA vs. 63% CA) and the frequency of ERG positive CaPs (26% AA vs. 64% CA) were more frequent among CA CaPs. Comparative evaluation of ERG frequencies in CaPs (N=1,292) underscores highest ERG frequency among CA patients (49.3%) followed by AA (23.2%) and Chinese (22%) men. Conclusions: In light of distinct biology of CaPs in ethnically/racially diverse CaP patient populations, there is a need for developing broadly applicable diagnostic, prognostic marker panels and therapeutic approaches. Higher frequency of CHD1 deletion in CaPs of AA patients may provide new therapeutic opportunities due to the sensitivity of CHD1 deletion harboring tumors to PARP inhibitors and platinum agents. Abstract:Abstract Racial disparities in prostate cancer have not been well characterized on a genomic level. Here we show the results of a multi-institutional retrospective analysis of 1,152 patients (596 African-American men (AAM) and 556 European-American men (EAM)) who underwent radical prostatectomy. Comparative analyses between the race groups were conducted at the clinical, genomic, pathway, molecular subtype, and prognostic levels. The EAM group had increased ERG ( P < 0.001) and ETS ( P = 0.02) expression, decreased SPINK1 expression ( P < 0.001), and basal-like ( P < 0.001) molecular subtypes. After adjusting for confounders, the AAM group was associated with higher expression of CRYBB2, GSTM3 , and inflammation genes ( IL33, IFNG, CCL4, CD3, ICOSLG ), and lower expression of mismatch repair genes ( MSH2 , MSH6 ) (p < 0.001 for all). At the pathway level, the AAM group had higher expression of genes sets related to the immune response, apoptosis, hypoxia, and reactive oxygen species. EAM group was associated with higher levels of fatty acid metabolism, DNA repair, and WNT/beta-catenin signaling. Based on cell lines data, AAM were predicted to have higher potential response to DNA damage. In conclusion, biological characteristics of prostate tumor were substantially different in AAM when compared to EAM.

Abstract:Men of predominantly African Ancestry (AA) have higher prostate cancer (CaP) incidence and worse survival than men of predominantly European Ancestry (EA). While socioeconomic factors drive this disparity, genomic factors may also contribute to differences in the incidence and mortality rates. To compare the prevalence of prostate tumor genomic alterations and transcriptomic profiles by patient genetic ancestry, we evaluated genomic profiles from The Cancer Genome Atlas (TCGA) CaP cohort (n = 498). Patient global and local genetic ancestry were estimated by computational algorithms using genotyping data; 414 (83.1%) were EA, 61 (12.2%) were AA, 11 (2.2%) were East Asian Ancestry (EAA), 10 (2.0%) were Native American (NA), and 2 (0.4%) were other ancestry. Genetic ancestry was highly concordant with self-identified race/ethnicity. Subsequent analyses were limited to 61 AA and 414 EA cases. Significant differences were observed by ancestry in the frequency of SPOP mutations (20.3% AA vs. 10.0% EA; p = 5.6×10-03), TMPRSS2-ERG fusions (29.3% AA vs. 39.6% EA; p = 4.4×10-02), and PTEN deletions/losses (11.5% AA vs. 30.2% EA; p = 3.5×10-03). Differentially expressed genes (DEGs) between AAs and EAs showed significant enrichment for prostate eQTL target genes (p = 8.09×10-48). Enrichment of highly expressed DEGs for immune-related pathways was observed in AAs, and for PTEN/PI3K signaling in EAs. Nearly one-third of DEGs (31.3%) were long non-coding RNAs (DE-lncRNAs). The proportion of DE-lncRNAs with higher expression in AAs greatly exceeded that with lower expression in AAs (p = 1.2×10-125). Both ChIP-seq and RNA-seq data suggested a stronger regulatory role for AR signaling pathways in DE-lncRNAs vs. non-DE-lncRNAs. CaP-related oncogenic lncRNAs, such as PVT1, PCAT1 and PCAT10/CTBP1-AS, were found to be more highly expressed in AAs. We report substantial heterogeneity in the prostate tumor genome and transcriptome between EA and AA. These differences may be biological contributors to racial disparities in CaP incidence and outcomes.

Abstract:Abstract African-American (AA) men are more likely to be diagnosed with and die from prostate cancer than European American (EA) men. Despite the central role of the androgen receptor (AR) transcription factor in prostate cancer, little is known about the contribution of epigenetics to observed racial disparities. We performed AR chromatin immunoprecipitation sequencing on primary prostate tumors from AA and EA men, finding that sites with greater AR binding intensity in AA relative to EA prostate cancer are enriched for lipid metabolism and immune response genes. Integration with transcriptomic and metabolomic data demonstrated coinciding upregulation of lipid metabolism gene expression and increased lipid levels in AA prostate cancer. In a metastatic prostate cancer cohort, upregulated lipid metabolism associated with poor prognosis. These findings offer the first insights into ancestry-specific differences in the prostate cancer AR cistrome. The data suggest a model whereby increased androgen signaling may contribute to higher levels of lipid metabolism, immune response, and cytokine signaling in AA prostate tumors. Given the association of upregulated lipogenesis with prostate cancer progression, our study provides a plausible biological explanation for the higher incidence and aggressiveness of prostate cancer observed in AA men. Significance: With immunotherapies and inhibitors of metabolic enzymes in clinical development, the altered lipid metabolism and immune response in African-American men provides potential therapeutic opportunities to attenuate racial disparities in prostate cancer.

Abstract:1603 Background: African American men (AAM) suffer from the highest rates of prostate cancer in the world. In addition, AAM experience the highest rates of aggressive prostate cancer and prostate cancer-specific mortality of any ethnic group in the United States [1, 2]. The reasons behind the increased burden of prostate cancer poor outcomes in AAM remain unresolved. A large proportion of variation in prostate cancer mortality remains unexplained solely by epidemiologic risk factors. We investigated genomic differences between AAM and Caucasian men with metastatic castration resistant prostate cancer (mCRPC) utilizing circulating tumor DNA (ctDNA) analysis. Methods: AAM and Caucasian men with mCRPC were enrolled in this study. We collected and analyzed 88 plasma samples, including 66 from Caucasian and 22 from African American patients, with no significant age difference between the groups. The targeted sequencing libraries were prepared following the Agilent SureSelect XT HS2 target enrichment workflow coupled with the SureSelect custom prostate cancer panel (Agilent, Inc.). The final libraries were sequenced on an Illumina NextSeq 2000 to generate 150 bp paired-end reads. Results: A total of 88 men with mCRPC were enrolled. About 60% of African American patients exhibited organ metastasis at the time of diagnosis, while this proportion was 36% for Caucasian patients and they primarily reported metastasis during relapse. Analyzing 1824 variants in 138 genes meeting inclusion criteria, 281 significant mutations across 88 genes revealed racial differences. Notably, 29% of these variants were somatic. Approximately 60 cancer-related pathways, tissues, and diseases were identified from these datasets, encompassing 59 of our significant genes. Among these associated genes, the prevalence of significant variants, higher in AAM, exceeds that in Caucasians in 76% of cases. The top genes that have more contribution in considering pathways are shown (Table). Conclusions: Genomic variations were observed in ctDNA analysis between African American and Caucasian patients diagnosed with metastatic castration-resistant prostate cancer. Our findings highlight the potential inclusion of ctDNA analysis in the clinical assessment of these patients, offering valuable insights for informed therapeutic decision-making. Abstract:Purpose: African American (AFR) men have the highest mortality rate from prostate cancer (PCa) compared with men of other racial/ancestral groups. Differences in the spectrum of somatic genome alterations in tumors between AFR men and other populations have not been well-characterized due to a lack of inclusion of significant numbers in genomic studies. Experimental Design: To identify genomic alterations associated with race, we compared the frequencies of somatic alterations in PCa obtained from four publicly available datasets comprising 250 AFR and 611 European American (EUR) men and a targeted sequencing dataset from a commercial platform of 436 AFR and 3018 EUR men. Results: Mutations in ZFHX3 as well as focal deletions in ETV3 were more frequent in tumors from AFR men. TP53 mutations were associated with increasing Gleason score. MYC amplifications were more frequent in tumors from AFR men with metastatic PCa, whereas deletions in PTEN and rearrangements in TMPRSS2-ERG were less frequent in tumors from AFR men. KMT2D truncations and CCND1 amplifications were more frequent in primary PCa from AFR men. Genomic features that could impact clinical decision making were not significantly different between the two groups including tumor mutation burden, MSI status, and genomic alterations in select DNA repair genes, CDK12 , and in AR . Conclusions: Although we identified some novel differences in AFR men compared with other populations, the frequencies of genomic alterations in current therapeutic targets for PCa were similar between AFR and EUR men, suggesting that existing precision medicine approaches could be equally beneficial if applied equitably.

Further examination of these data indicated the AA CaP genome associated recurrent deletion of the Chromodomain Helicase DNA Binding Protein 1(CHD1) gene. Methods: Combined evaluation of frequencies and prognostic associations of ERG oncoprotein by immunohistochemistry and deletions of LSAMP, CHD1 and PTEN genes by FISH were performed using a multi-core TMA (42 AA and 59 CA patients) with longitudinal follow up data (median 16 years). CaP and matched blood genomic DNA samples (N=60) from AA patients were analyzed by WGS. Abstract:Introduction. Prostate cancer (PCa) of American men of African ancestry (AA) is diagnosed at an earlier median age and more advanced stage with a poorer prognosis and significantly higher mortality than in Americans of primarily Northern European ancestry (EA). The mechanism of how ancestry contributes to aggressive PCa in AA patients is not well understood. Given the inherent heterogeneity and multifocality of prostate cancer, Spatial transcriptomics is of particular interest because it allows distinct cell populations within the tumor microenvironment to be characterized, and their possible effects on local tumors to be identified. Here we compare transcription between four AA and four EA patients in regions of peritumoral stroma, inflammation, Benign Prostatic Hyperplasia (BPH), and Gleason score patterns 3 (G3) and 4 (G4). Methods: Formalin fixed, paraffin embedded (FFPE) prostatectomy tissue blocks were obtained from 8 patients (4 AA and 4 EA) prior to treatment of the patients and matched for clinical variables (i.e., biochemical relapse, age, Gleason score, and tumor stage). 10um tissue sections were mounted onto Visium Spatial Gene Expression Slides and processed following 10x Genomics protocol. Sample counts were processed through Space Ranger. 18000 unique genes, spatial barcodes, and clinician guided annotations were imported to the R program Seurat. Data was processed using SCTransform. Significance of each transcript differences between each tissue were calculated. Results: Digital histologic slides of prostate cancer specimens from AA (n=4) and EA (n=4) patients were annotated by a urologic pathologist to identify regions of peritumoral stroma, inflammation, BPH, G3, and G4. Each spot was interrogated for the expression of 18000 transcripts, with an average of 17796 being detected per spot. Among the largest differences observed between AA and EA, G3 and G4 had distinct transcriptome profiles in the four AA as compared to G3 and G4 in four EA. Also, when differential expression (padj < 0.05) in peritumoral stroma was compared to G4 in AA, we identified 290 differentially expressed genes, whereas the same comparison in EA cohorts identified 600 differentially expressed genes. Conclusion: Spatial transcriptomic analysis revealed differences in gene expression across multiple tissue types in prostate cancer patients of different ancestries. Unveiling Prostate Cancer's Molecular Tapestry: Ethnic Disparities and Prognostic Insights from Whole-Mount Prostatectomy Tissue Analysis

Abstract:Background: Prostate cancer is a complex and heterogeneous disease with multiple tumor foci, each potentially harboring distinct driver molecular aberrations. This complexity poses challenges to effective management. We took an innovative approach to gain a comprehensive understanding of the genetic underpinnings of each tumor focus and avoid overlooking more minor yet clinically significant foci. Instead of relying solely on a systematic sampling of dominant foci, we conducted molecular analysis on whole-mount radical prostatectomy specimens. Our study aimed to find distinct molecular subsets of prostate cancer and assess their correlation with clinical outcomes, focusing on Caucasians (CA) and African Americans (AA). Method: We randomly selected 2201 whole-mount radical prostatectomy cases, with 1207 (54.8%) from CA and 994 (45.1%) from AA patients evaluated for a 5-year biochemical recurrence-free survival rate (BCR). Of these 2201 cases, 834 (463 -56% were from CA and 371 -44% from AA patients) were subjected to molecular analysis using dual immunohistochemistry (IHC) for ERG and SPINK1, along with dual RNA in-situ hybridization (RNA-ISH) for ETV1 and ETV4 to evaluate tumor molecular heterogeneity on whole-mount specimens. The Chi-squared test examined racial disparities in aberrant oncogene expression. To assess BCR-free survival, we employed the Kaplan-Meier method and Cox-PH models for patients with distinct molecular subsets of prostate cancer. Additionally, Gleason Grade groups of prostate biopsies were summarized using a spaghetti plot and compared using linear mixed models. Results: Analysis of the 2201 cases revealed that AA with localized prostate cancer behaved differently with better 5-year BCR-free survival than CA after radical prostatectomy (AA: 0.82, 95% CI 0.80-0.85; CA: 0.71, 95% CI 0.68-0.75; p<.001). Molecular profiling of whole-mount specimens from 834 cases revealed that 16.4%, 58.4%, 21.7%, and 3.5% of patients with localized prostate cancer expressed none, one, two, and three of the four oncogenes, respectively. This finding identified new molecular subsets of prostate cancer with more than one driver mutation in a mutually exclusive manner within the multifocal disease. ERG and SPINK1 expression showed a negative correlation (p<.001). Notably, AA patients exhibited a lower incidence of ERG (38.8% vs. 60.3%) but a higher incidence of SPINK1 (63.3% vs. 35.6%) than CA patients. The incidences of ETV1 (9.4% vs. 9.3%) and ETV4 (4.6% vs. 3.9%) were not statistically significant between the two racial groups. However, significantly, ETV1 expression was associated with worse BCR-free survival in CA patients (hazard ratio [HR]=2.36, 95% CI 1.22-4.57, p=.02), while ETV4 expression was linked to worse BCR-free survival in AA patients (HR=2.65, 95% CI 1.15-6.09, p=.02). Moreover, ETV4 expression was associated with regional lymph node metastasis in AA patients (odds ratio [OR]=5.14, 95% CI 1.3-17.4, p=.01) but not in CA patients (OR=0.60, 95% CI 0.03-3.17, p=.63) at the time of radical prostatectomy. Additionally, in patients who underwent multiple biopsies before radical prostatectomy, the Gleason Grade group increased over time in AA patients (0.25 per year, p<.001) but remained unchanged in CA patients. ERG expression was associated with a lower Gleason Grade group (-0.20, p=.03), while ETV4 expression was linked to a higher Gleason Grade (0.54, p=.01). Conclusions: Our study reveals that AA with localized prostate cancer behaves differently and has better BCR-free survival than CA after radical prostatectomy, even after adjusting for known prognostic factors. Identification of new molecular subsets of prostate cancer with more than one ETS gene fusion within a multifocal prostate shows significant molecular heterogeneity between localized prostate cancer in CA and AA patients. Importantly, given the association of ETV1 and ETV4 expression with worse BCR-free survival in CA and AA, respectively, ETV1 and ETV4 emerge as potential prognostic markers, offering insights for clinical practice to predict prostate cancer recurrence after radical prostatectomy. Identification of new molecular subsets of prostate cancer with more than one ETS gene fusion and SPINK1 in a mutually exclusive pattern indicates the clonal origin of independent tumor foci, which is a rare and unique phenomenon in prostate cancer hitherto unidentified.

Abstract:Prostate cancer is the second leading cause of cancer-related deaths among men in the United States. Health disparities in men diagnosed with prostate cancer are observed between patients of African and those of European ancestry. To develop biomarkers and elucidate the molecular mechanisms behind prostate tumorigenesis and racial disparities, we collected formalin-fixed paraffin-embedded (FFPE) prostate tissues from African American (AA) and European American (EA) patients. By comparing global DNA methylation profiles among normal samples and different grades of tumor samples from AA and EA, we revealed distinct prostate cancer subgroups with variable DNA methylated regions. We identified thousands of differentially methylated regions between normal and tumor samples, defining hypermethylated and hypomethylated regions. When we examined the frequency of differentially methylated regions among different grades of tumor samples, we found that low-grade tumor samples exhibited fewer hypermethylated regions while high-grade tumors exhibited numerous hypermethylated regions across AA and EA samples. However, we found that a subset of low-grade tumors exhibited many hypomethylated regions similar to high-grade tumors. By clustering prostate tumor samples with the identified differentially methylated regions, we revealed distinct prostate cancer subgroups with unique DNA methylation patterns. By integrating immunohistochemistry data with DNA methylation profiles, we found that the identified subgroups are tightly linked to the overexpression status of ERG, surpassing the ethnicity-related variations. Associating DNA methylation patterns with clinicopathological features and integrating this data with other molecular features, we are characterizing the molecular mechanisms underlying prostate tumorigenesis and health disparities. This study will pave the way to identify novel DNA methylation biomarkers and therapeutic targets linked to different prostate cancer subgroups. Abstract:Abstract In the United States and globally, prostate cancer (PCa) mortality rates are highest among men of African descent. Previous studies suggested that tumor subtypes or gene expression profiles had the greatest influence on overall racial/ethnic survival accounting for 24% of the disparities in PCa and remains even when controlled for access to care and stage at presentation. Moreover, the tumor microenvironment plays an essential role in tumor progression, aggressiveness, therapeutic response, and patient outcomes. Additionally, the association of the genomic findings with patient ancestry and other characteristics, such as tumor biology and transcriptomic alterations, remains poorly understood. Here we performed a multi-Omics approach to unravel the complexity of tumor heterogeneity and understand disease progression & distinct tumor biology influenced by genetic ancestry. We performed Spatial NanoString high-plex GeoMx-DSP (Digital Spatial Profiler) at the protein and RNA levels for a total of 118 treatment-naive PCa patients (around 500 ROIs). Simultaneously, we ran matched genome-wide sequencing (whole transcriptome) for these patients. The cohort comprised 87 AAM, 3 unknown, and 28 EAM self-reported individuals. To verify the self-reported race, the genomic ancestry was qualified using genotype and Admixture analysis. To further validate differentially expressed genes at the protein level, we performed multi-plex histological staining of 40 markers to determine the spatial resolution and neighborhood clustering within the tumor and the microenvironment. In parallel, we performed scMultiOmics sequencing (scRNA & scATAC-Seq) at a single-cell resolution (6000 cells/sample at 25K reads per cell) from an additional 12 patients (9 AAM & 3 EAM). We will use the multi-omics integration method (CoGap) to build a structural, spatial map that reflects tissue/system phenotype and to understand the complementary role of multi-omics (genome, transcriptome, and metabolite) interrelation/influence on the disease processes. Our results demonstrate that patients who self-report as AAM or Nigerian are assigned to high African (> 70%) Ancestry with either Yoruba (Nigeria) and/or Bantu subpopulation in the Sub-Saharan area. Additionally, high African Ancestry patients are diagnosed at a younger age and show advanced pathology stages compared to patients with European Ancestry. Moreover, AAM with high African Ancestry has significantly (p-value 0.05) higher CD8 +ve cells infiltration. Interestingly, segmentation and gating of the immune cell populations showed increased expression of CD8+ve cells colocalization with PD-1 signaling within African Ancestry patients. Our study provides new insight into how genetic ancestry impacts immune signatures in AAM and contributes to PCa racial disparities. Our findings could lead to new therapeutic strategies using immune modulators drugs to decrease the global disease burden, especially among men at high risk for PCa, such as AAM of African descent. To verify the self-reported race, the genomic ancestry was qualified using genotype and Admixture analysis. To further validate differentially expressed genes at the protein level, we performed multi-plex histological staining of 40 markers to determine the spatial resolution and neighborhood clustering within the tumor and the microenvironment. In parallel, we performed scMultiOmics sequencing (scRNA & scATAC-Seq) at a single-cell resolution (6000 cells/sample at 25K reads per cell) from an additional 12 patients (9 AAM & 3 EAM). We reconstructed a 3D model of the distinct tumor microenvironment at subcellular resolution using serially sectioned hematoxylin and eosin- stained tissue sections (CODA technology). Our results demonstrate that patients who self-report as AAM or Nigerian are assigned to high African (> 70%) Ancestry with either Yoruba (Nigeria) and/or Bantu subpopulation in the Sub-Saharan area. Additionally, high African Ancestry patients are diagnosed at a younger age and show advanced pathology stages compared to patients with European Ancestry. AAM of Yoruba descent expresses significantly higher immune-inflammatory signatures (STAT/IFNG-signaling pathway) compared to the Nigerian-Yoruba subpopulation. Our scRNA-Seq analysis shows that AAM has suppressive myeloid cells infiltrate within the tumor cells. The infiltrations of these cells change with age, Gleason Grade, and pathology stage. Moreover, AAM-tumor cells (scATAC-Seq) have increased open Chromatin accessibility with STAT motifs enrichment (p-value;0.0001), which could be the driver regulators of the immune-suppressive signature as well as influence the upregulation of STAT/IFNG signature within African Ancestry patients. Our study provides new insight into how genetic ancestry impacts immune signatures in AAM/African and contributes to PCa racial disparities. Comparative and integrative analysis of transcriptomic and epigenomic-wide DNA methylation changes in African American prostate cancer

Abstract:African American (AA) men have the highest incidence and mortality rate from Prostate cancer (PCa) than any other racial/ethnic group. To date, PCa genomic studies have largely under-represented tumour samples from AA men. We measured genome-wide DNA methylation in benign and tumor prostate tissues from AA men using the Illumina Infunium 850 K EPIC array. mRNA expression database from a subset of the AA biospecimen were used to assess correlation of transcriptome and methylation datasets. Genome-wide methylation analysis identified 11,460 probes that were significant (p < 0.01) and differentially methylated in AA PCa compared to normal prostate tissues and showed significant (p < 0.01) inverse-correlation with mRNA expression. Ingenuity pathway analysis and Gene Ontology analysis in our AA dataset compared with TCGA dataset showed similarities in methylation patterns: top candidate genes with significant hypermethylation and corresponding down-regulated gene expression were associated with biological pathways in hemidesmosome assembly, mammary gland development, epidermis development, hormone biosynthesis, and cell communication. In addition, top candidate genes with significant hypomethylation and corresponding up-regulated gene expression were associated with biological pathways in macrophage differentiation, cAMP-dependent protein kinase activity, protein destabilization, transcription co-repression, and fatty acid biosynthesis. In contrast, differences in genome-wide methylation in our AA dataset compared with TCGA dataset were enriched for genes in steroid signalling, immune signalling, chromatin structure remodelling and RNA processing. Overall, differential methylation of AMIGO3, IER3, UPB1, GRM7, TFAP2C, TOX2, PLSCR2, ZNF292, ESR2, MIXL1, BOLL, and FGF6 were significant and uniquely associated with PCa progression in our AA cohort.

Abstract:Introduction Men with African ancestry have the highest incidence and mortality rates of prostate cancer (PCa) worldwide. Methods This study aimed to identify differentially methylated genes between tumor vs. adjacent normal and aggressive vs. indolent PCa in 121 African American patients. Epigenome‐wide DNA methylation patterns in tumor DNA were assessed using the human Illumina Methylation EPIC V1 array. Results Around 5,139 differentially methylated CpG‐sites (q 0.2) were identified when comparing normal vs. tumor, with an overall trend of hypermethylation in prostate tumors. Multiple representative differentially methylated regions (DMRs), including immune‐related genes, such as CD40, Galectin3, OX40L, and STING, were detected in prostate tumors when compared to adjacent normal tissues. Based on an epigenetic clock model, we observed that tumors' total number of stem cell divisions and the stem cell division rate were significantly higher than adjacent normal tissues. Regarding PCa aggressiveness, 2,061 differentially methylated CpG‐sites (q .05) were identified when the grade group (GG)1 was compared with GG4/5. Among these 2,061 CpG sites, 155 probes were consistently significant in more than one comparison. Among these genes, several immune system genes, such as COL18A1, S100A2, ITGA4, HLA‐C, and ADCYAP1, have previously been linked to tumor progression in PCa. Conclusion Several differentially methylated genes involved in immune‐oncologic pathways associated with disease risk or aggressiveness were identified. In addition, 261 African American‐specific differentially methylated genes related to the risk of PCa were identified. These results can shedlight on potential mechanisms contributing to PCa disparities in the African American Population.

Abstract:Abstract Purpose: DNA damage repair (DDR) defects are common across cancer types and can indicate therapeutic vulnerability. Optimal exploitation of DDR defects in prostate cancer requires new diagnostic strategies and a better understanding of associated clinical genomic features. Experimental Design: We performed targeted sequencing of 1,615 plasma cell-free DNA samples from 879 patients with metastatic prostate cancer. Depth-based copy-number calls and heterozygous SNP imbalance were leveraged to expose DDR-mutant allelic configuration and categorize mechanisms of biallelic loss. We used split-read structural variation analysis to characterize tumor suppressor rearrangements. Patient-matched archival primary tissue was analyzed identically. Results: BRCA2, ATM, and CDK12 were the most frequently disrupted DDR genes in circulating tumor DNA (ctDNA), collectively mutated in 15% of evaluable cases. Biallelic gene disruption via second somatic alteration or mutant allele–specific imbalance was identified in 79% of patients. A further 2% exhibited homozygous BRCA2 deletions. Tumor suppressors TP53, RB1, and PTEN were controlled via disruptive chromosomal rearrangements in BRCA2-defective samples, but via oncogene amplification in context of CDK12 defects. TP53 mutations were rare in cases with ATM defects. DDR mutations were re-detected across 94% of serial ctDNA samples and in all available archival primary tissues, indicating they arose prior to metastatic progression. Loss of BRCA2 and CDK12, but not ATM, was associated with poor clinical outcomes. Conclusions: BRCA2, ATM, and CDK12 defects are each linked to distinct prostate cancer driver genomics and aggression. The consistency of DDR status in longitudinal samples and resolution of allelic status underscores the potential for ctDNA as a diagnostic tool.

Abstract:Abstract Men of African ancestry have the higher incidence and mortality rates of prostate cancer (PCa) worldwide. These health disparities may be contributed by environmental or biological factors, such as epigenomics. Differential DNA methylation can influence carcinogenesis and disease progression. Indeed, the most common molecular event in PCa is dysregulation of DNA methylation. Among these epigenetic changes, some specific changes may be associated with poor outcomes. Numerous studies reported that differential DNA methylation influences the likelihood of developing PCa and affects its progression. However, most studies were based on European American patients. There is a need to investigate methylation profiles to evaluate potential African American-specific methylated genes, since differential DNA methylation may influence health disparities in PCa. This study aimed to investigate differentially DNA methylated genes between tumor vs. adjacent normal, and aggressive vs. indolent PCa (based on Gleason Score) in 120 African American patients from Florida. Genomic DNA samples were extracted by macro-dissection from FFPE. DNA methylation patterns were assessed using the human Illumina Infinium Methylation EPIC array. We identified 5,097 differentially methylated CpG-sites (q<0.01, lΔβl > 0.2) A few representative differentially methylated regions (DMRs) include immune genes, such as CD40, OX40L, Galectin 3, and STING, in prostate tumor tissues as compared with normal tissues. There was also a clear global increase of methylation level in the tumor samples compared to the normal tissues. Regarding PCa aggressiveness, 6,775 (Hypo:4,252, and Hyper: 2,523) differentially methylated CpG-sites (q<0.05, lΔβl > 0.1) were identified when two groups GG1 (Gleason score 6) vs GG4/5 (Gleason score 8≤) were compared. Among these 6,775 CpG-sites, many CpG-probes are consistently significant in more than one comparison. For example, 1,182 hyper and 1,660 hypo-methylation sites were identified in the comparison between GG1 and GG2/3 (Gleason score 7), while 362 hyper- and 1,640 hypo-methylation sites were found in the GG2/3 vs GG4/5 comparison. A small fraction of probes (51 hyper and 155 hypomethylated probes) was consistently found in all comparisons. These genes, MMP16, CDH13, CCND2, and SEPT9, were previously reported to have a role in PCa progression. This study identified several differentially methylated genes, associated with risk or aggressiveness. Most of these changes appear to overlap in different comparisons, thus GG1 vs. GG2/3 or GG2/3 vs. GG4/5 comparison. Many differentially methylated genes identified here have previously been associated with PCa risk or tumor aggressiveness. These results will shed light on potential mechanisms contributing to PCa disparities in African American population.
