Abstract:Objective To observe the effect of combined therapies for congenital dacrycystitis. Methods In proper order,114 cases(115 eyes)of congenital dacrycystitis were treated with squeezing and massaging lacrimal sac, irrigating lacrimal passage, probing and irrigating lacrimal passage. Results All eyes were recovered, and epiphora and pyorrhea disappeared after combined therapies. Conclusion Combined therapy is a safe, effective method for congenital dacrycystitis with lower complication. The way of irrigation and probing of lacrimal passage is suitable for all infants and children who are suffering from congenital dacrycystitis.Parents of children often play a key role in treatment.

Prenatal diagnosis and outcome of congenital dacryocystoceles

Abstract:Objective: To determine the incidence and present our experience with prenatal diagnosis and postnatal outcome of dacryocystocele. Material and methods: All cases of congenital dacryocystocele diagnosed in our center between 2020 and 2022 were identified in our database to establish the incidence of these defects. The medical records were then reviewed for gestational age, gender, size, and side of dacryocystocele and postnatal outcome. Results: A total of 26 cases with dacryocystoceles were found at a mean gestation age of 30 weeks (range, 29-33 weeks). The overall incidence was 1.35%, there was an obvious female predominance (73%), 69% of cases were unilateral and 31% were bilateral. There were no serious associated anomalies. The postnatal outcome was obtained in 88% of cases (23/26), in 39% (9 out of 23) cases the dacryocystocele was confirmed postnatally, and in 7 (77%) of these it was complicated by dacryocystitis. The spontaneous resolution was more likely in the right-sided lesions, and this was statistically significant. The treatment in cases with dacryocystitis involved massage and local antibiotics and was successful in 71% of cases. 2 cases (29%) suffer from recurrent dacryocystitis and are followed up with recurrent probing and local antibiotics. No breathing difficulties were described postnatally in our study group. Conclusion: The overall prenatal incidence of dacryocystocele was 1.35%. The outcome is favorable, 61% of dacryocystoceles in our study resolved spontaneously and in no case postnatal breathing complications were reported. Dacryocystitis was common in persisting cases but was usually treated successfully by massage and antibiotics. The right-sided dacryocystoceles are more likely to resolve spontaneously than left-sided, and this was the only significant factor predicting persistence.

Abstract:Congenital lacrimal sac mucocele occurs in approximately 3% of infants with congenital lacrimal duct obstruction. Lacrimal sac mucocele is sometimes associated with a intranasal nasolacrimal duct cyst presenting as an intranasal cystic mass, which needs the involvement of an ophthalmologist and otolaryngologist in diagnosis and management. Bilateral cysts may cause in the early neonatal varying degrees of nasal obstruction, sleep disturbance, feeding difficulty and even respiratory distress. We present a case of bilateral dacryocystocoele with intranasal extension presenting as respiratory distress in a 7-day-old male newborn. The diagnosis was confirmed clinically and by computed tomography scan.

Abstract:Chronic dacryocystitis refers to the inflammation of the lacrimal sac and nasolacrimal duct caused by infection, trauma, tumor, and other factors. The main clinical manifestations are epiphora, obstruction of nasolacrimal duct, and pyorrhea. If effective remedy is not given in time, the subject may suffer from corneal damage and visual acuity will be notably affected. [ 1–3 ] At present, the clinical remedy of chronic dacryocystitis includes lacrimal duct irrigation, antibiotic ophthalmic medicine, operation, and so on. [ 4–8 ] The most frequently used ones are surgical remedy, which aims to reestablish the internal nasal drainage channel, remove the obstruction of nasolacrimal duct, and eliminate inflammation. [ 9–11 ] Dacryocystorhinostomy (DCR) is the main surgical method to treat chronic dacryocystitis. In the past, external-route was often used, but postoperative complications were easy to occur. [ 12 , 13 ] The endoscopic nasal approach, in contrast to traditional methods, offers the significant advantage of avoiding any external scarring, a crucial factor in the aesthetic outcome. This technique, recognized for its minimally invasive nature, is increasingly preferred in the treatment of eye and nose diseases. Its ability to eliminate visible scars aligns with the growing demand for less intrusive surgical interventions, particularly in cases of chronic dacryocystitis. [ 6 , 14 , 15 ] This study conducted a retrospective analysis to explore the safety and effectiveness of END-DCR in the remedy of chronic dacryocystitis. The report is as follows.

Abstract:Background Although endoscopic dacryocystorhinostomy (DCR) is a standard procedure for nasolacrimal duct obstruction (NLDO), the failure rate remains approximately 10%. A small lacrimal sac is considered the main reason for surgical failure. We explored the efficacy of endoscopic DCR for the treatment of NLDO with a small lacrimal sac. Methods The clinical data of 72 patients (88 eyes) diagnosed with NLDO and undergoing endoscopic DCR from 2012 to 2020, with at least 24 months of follow-up were retrospectively collected. Intraoperatively, the Rosenmüller valves were fully exposed, mucosal flaps were preserved to cover the naked bone, and a silicone tube was implanted if necessary. Postoperative intervention was performed if necessary. The main outcome measures were symptomatic improvement and objective ostium patency. Results Eighty-eight eyes of 72 patients were divided into two groups: the refractory group (34 patients, 47 eyes), with a small lacrimal sac (≤ 5 mm in diameter), and the simple group (38 patients, 41 eyes). Patients with small lacrimal sacs were more prone to bilateral eye disease than those in the simple group ( P = 0.014) and required a longer postoperative follow-up ( P < 0.001). Refractory NLDO and simple NLDO had a success rate of 91.5% and 95.1%, respectively, with no significant difference. Conclusion Endoscopic DCR for refractory NLDO with a small lacrimal sac could achieve a beneficial result by exposing the Rosenmüller valves, preserving mucosal flaps, implanting necessary intubation, and intervening postoperatively. Thus, a small lacrimal sac should not be regarded as a contraindication to surgery.

Abstract:We describe 3 infants with congenital dacryocystocele resistant to conservative treatment who were treated with a novel, simple intranasal cyst marsupialization (ICM) technique. Otoscopy-guided ICM was performed by an otolaryngologist in the manner of otoscopic myringotomy for cases with nasal cyst distension. All 3 infants were treated successfully by a single surgical procedure under topical anesthesia in an office setting.

Abstract:Roebel cables made of high temperature superconducting (HTS) coated conductors can carry high currents with a compact design and reduced ac losses. Therefore, they are good candidates for manufacturing coils for HTS applications such as motors and generators. In this paper, we present the experimental dc and ac characterization of several coils assembled from a 5-m-long Roebel cable built at Karlsruhe Institute of Technology, which differ in the number of turns and turn-to-turn spacing. Our experiments, which are supported by finite-element-method calculations, show that a more tightly wound Roebel coil, despite having a lower critical (and therefore operating) current, can produce a higher magnetic field than a loosely wound one. For a given magnetic field produced at the coil's center, all the coils have similar ac losses, with the exception of the most loosely wound one, which has much higher losses due to the relatively large current needed to produce the desired field. The experiments presented in this paper are carried out on the geometry of pancake coils made of Roebel cables, but they are exemplary of a more general strategy, based on coupling experiments and numerical simulations, that can be used to optimize the coil design, with respect to different parameters, such as tape quantity, size, or ac loss, the relative importance of which is dictated by the specific application.

Abstract:Purpose: The purpose of this study is to assess the long-term outcomes of modified transcanalicular diode laser dacryocystorhinostomy in a large cohort of patients affected by primary acquired nasolacrimal duct obstruction. Methods: This study, conducted from January 17 to June 2022, encompassed 141 patients (159 procedures) who underwent modified transcanalicular diode laser dacryocystorhinostomy (MT-DCR). The procedure employed an 810-nm diode laser. Patients were monitored for at least a year after the intervention. Anatomical success was determined by ostium patency upon irrigation, while functional success referred to epiphora resolution. Parameters studied included patient demographics, procedure duration, complications, and both anatomical and functional success. Statistical analysis was performed using the Statistical Package for the Social Sciences software, with results considered significant at a 95% confidence interval (p≤0.05). Results: A total of 159 lacrimal drainage systems (141 patients: 112 women and 29 men) were included in this study. Among them, 18 underwent bilateral procedures. The average patient age was 58 years (range: 34-91 years), and the average surgical duration was 24 minutes (range: 18-35 minutes). One year after the surgery, MT-DCR exhibited anatomical and functional success rates of 84.9% (135/159) and 83% (132/159), respectively. Conclusion: MT-DCR achieved an anatomical success rate of 84.9%, reflecting an excellent outcome. However, further extensive studies with larger sample sizes and longer follow-up periods are necessary to substantiate these findings.
Intranasal Endoscopic Dacryocystorhinostomy for Chronic Dacryocstitis

Abstract:Objective:To analyze the outcome after intranasal endoscopic dacryocystorhinostomy.Methods: Endoscopic dacryocystorhinostomy was performed in 22 sides of 18 patients who had chronic epiphora.No silicone tubing was used in any of the cases.Among them 7 with deviation of nasal septum or nasal polyps and chronic sinusitis were treated at same time.Results: After a follow-up of 3 to 18 months,15 out of 22 sides of the patients were free of symptoms.Four additional patients(5 sides) became symptom-free after surgery.The total success rate was 90.9%.Conclusion: Intranasal endoscopic dacryocystorhinostomy is a rapid,safe,and elegant method for the treatment of chronic dacryocstitis.

BackgroundUtilizing expanded endoscopic approaches to the maxillary sinus for the endonasal management of a variety of tumors is increasing in popularity. The nasolacrimal duct (NLD) may be injured inadvertantly or need to be removed during tumor resection or to augment visualization. Management of the NLD can take the form of transection alone, transection with stenting, or performing a formal dacryocystorhinostomy to avoid postoperative sequelae of injury. The purpose of this study was to review the literature and determine the optimal management of the NLD during expanded maxillary sinus approaches.MethodsA systematic review of Ovid, Embase, Medline, and Cochrane databases was performed to identify studies involving expanded approaches to the maxillary sinus and that explicitly reported the status of the NLD and postoperative outcomes.ResultsNineteen studies were included in the analysis and divided into two groups: NLD-preserving (n = 9 studies; n = 191 patients in aggregate) and NLD-involving (n = 10 studies; n = 296 patients in aggregate). In the NLD-preserving subgroup, one patient out of a subgroup aggregate of 191 patients (0.5%) developed epiphora. In the NLD-involving subgroup, sharp transection alone was the most common method of NLD removal and was associated with a low rate of epiphora (study rates: 0 to 18.2%; aggregated subgroup rate: 7.0%, 21 / 296). Spontaneous resolution of symptoms was common (60%-100% cases).ConclusionsThe NLD should be preserved when feasible from an exposure and tumor-control perspective. When pathology or approach requires the removal of the NLD, rates of persistent epiphora are very low, regardless of surgical technique. When expanded maxillary approaches are employed, particularly for benign tumors, and require removal of the NLD, sharp transection is the simplest method of removal, provides a low rate of postoperative epiphora, and is supported by the available literature.

Surgical Outcome of External Dacryocystorhinostomy With Silicone Intubation for Recurrent Lacrimal Abscess in Children Younger Than 6 Years

Abstract:Purpose: To assess the success rate of external dacryocystorhinostomy (DCR) with silicone intubation for recurrent lacrimal abscess in children younger than 6 years. Methods: A single-center retrospective analysis of 46 eyes of 40 children who underwent DCR with silicone tube intubation for recurrent lacrimal abscess was done. Probing done previously in these cases was unsuccessful. Only those children who underwent incision and drainage of the abscess at least once with antibiotic treatment were included in the study. In all cases, silicone tube removal was done after 3 months. A successful outcome was defined as the absence of subjective complaints of pain and swelling over the medial canthal area and watering and discharge at 6 months postoperatively. Objective assessment of patency of the lacrimal apparatus was done by sac syringing at 6 months postoperatively. Results: The mean age at surgery was 4.93 ± 0.93 years (range: 3 to 6 years) and the mean follow-up duration was 11.80 ± 11.87 months (range: 6 to 84 months). Intra-operative difficulties encountered were excessive perisac adhesion (n = 28) and severe bleeding/hemorrhage (n = 24). A total of 82.61% cases had a successful outcome after DCR with silicon tube intubation. One child had granuloma formation at the wound. Spontaneous tube extrusion occurred in three children. Conclusions: DCR with silicone tube intubation is a safe and effective surgical approach with satisfactory outcomes for treating recurrent lacrimal abscess with congenital nasolacrimal duct obstruction in children younger than 6 years. Abstract:Purpose: To evaluate the clinical usefulness of digital radiography dacryocystography in patients with primary acquired nasolacrimal duct obstruction prior to endoscopic dacryocystorhinostomy. Methods: All dacryocystography images from 129 patients with primary acquired nasolacrimal duct obstruction were analyzed. Each group was assessed for postoperative epiphora severity using Munk's score via telephone follow-up three years post-surgery. Receiver operating characteristic (ROC) curve was plotted to obtain a suitable cutoff value of the transverse diameter of the lacrimal sac (LS), used to categorize LS size into small (≤4.350 mm) and large (>4.350 mm). Results: Analysis of the transverse diameter of the LS among 129 patients showed a negative correlation between it and Munk's score (r = -0.282, p = 0.001). There was a statistical difference between the surgical outcomes and the sizes of the LS (p = 0.041). The ROC curve analysis showed that the transverse diameter of the LS at 4.350 mm was the ideal cutoff value for the outcome of endoscopic dacryocystorhinostomy, with a sensitivity of 42.2 %, and specificity of 92.3 %. After adjusting for the age and sex, the small LS was associated with an increased risk of postoperative failed outcome (adjusted odds ratio [95 % CI]: 8.628 [1.074, 69.335]). Conclusion: The small LS was independently associated with the failed surgical outcome. Furthermore, the preoperative measurement of the LS transverse diameter serves as one of the reliable predictors for postoperative epiphora severity.
Transcanalicular Diode Laser-assisted Dacryocystorhinostomy for the Treatment of Primary Acquired Nasolacrimal Duct Obstruction

Abstract:Today's gold standard in the treatment of infrasaccal primary acquired nasolacrimal duct obstruction (PANDO) is external dacryocystorhinostomy (DCR), a relatively invasive procedure that can be performed after failure of recanalizing treatments. However, with progress in the field of diode laser technology, new approaches have emerged. Laser-assisted transcanalicular DCR with subsequent bicanalicular silicon intubation is a new option showing great promise as a viable minimally invasive procedure. Under permanent endoscopic visual control from the nasal cavity, a diode laser fiber is inserted into the lacrimal sac and laser energy is applied to create a bony ostium between the lacrimal sac and the nasal cavity. Since no skin incision needs to be made, advantages of this method comprise the sparing of the skin as well as the medial palpebral structures and the physiological palpebral-canalicular pump mechanism. The duration of surgery as well as reconvalescence is generally shorter than with external DCR. Complications include silicon tube prolapse, mild swelling and, rarely, canalicular infection and thermal injury. One-year functional success rates, defined as complete resolution of symptoms and ostium patency, are high, yet still range behind those of external DCR. However, secondary external DCR after failure of laser-assisted DCR can be performed without difficulty. Thus, laser-assisted transcanalicular DCR is a valid option that should be considered as a second-step procedure after failure of recanalization procedures and before external DCR.
The Nasolacrimal Drainage System in 143 Children with the Microphthalmos-Anophthalmos Complex

Abstract:Background: Report of clinical findings relating to the lacrimal system in congenital clinical anophthalmos and severe blind microphthalmos (MAC-complex patients). Methods: A retrospective study of the notes of 207 consecutive patients treated surgically at least once with highly hydrophilic self-inflating expanders for MAC between 1998 and 2021. The lacrimal drainage system was always probed and irrigated under general anaesthesia before any other procedure was started. Results: 64 patients were excluded due to possible misdiagnosis because of previous lid or orbit surgery elsewhere or due to missing data. The analysis therefore included 67 girls and 76 boys aged between 1 and 126 months (median age: 5 months). 72 patients presented with unilateral and 42 with bilateral anophthalmos, and 24 had unilateral and 5 bilateral microphthalmos; consequently, 286 orbits (of which, 190 with probable pathology) were available for assessment. In unilateral cases the lacrimal system on the normal side was never affected. On the anophthalmic or microphthalmic side the lacrimal system was normal in 68 orbits only (35.8%). The most frequent finding was canalicular stenosis (91 orbits; 48%). Common canaliculus stenosis was observed in 12 orbits (6.3%) and nasolacrimal duct obstruction in 9 orbits (4.7%). There were four cases of punctal aplasia, but no other anomalies. In unilateral MAC pathologic findings during lacrimal probing were found to be associated with anatomical malformation of the contralateral fellow eye. Only in unilateral anophthalmos there was a significant association with cleft lip and palate, which was not found in the three other groups. Conclusions: In congenital clinical anophthalmos the lacrimal system is affected in up to 66.5% of cases, mostly due to canalicular stenosis. Even if there is no clear evidence of an embryological connection, this association is certainly not a random finding.

Abstract:OBJECTIVE:The objective of this study was to assess the efficacy of exposing Rosenmüller valve during endonasal endoscopic dacryocystorhinostomy (EES-DCR) in treating refractory nasolacrimal duct obstruction.METHODS:Thirty six patients (47 eyes) with refractory nasolacrimal duct obstruction were entered into this study, including 13 patients (15 eyes) underwent nasal lacrimal duct intubation before, 18 patients (26 eyes) with small lacrimal sac, 2 patients underwent DCR before, 1 patient with functional nasal lacrimal duct obstruction, 1 patient with lacrimal mucocele, 1 patient with nasal trauma. During the DCR, the bone window was enlarged as wide as possible and the valve of Rosenmüller was exposed. The intubation of the lacrimal apparatus was performed if the lacrimal sac was very small and the Rosenmüller valve was tight. Functional endoscopic sinus surgery and septoplasty were performed simultaneously if necessary. Postoperatively, each patient was assessed by the symptom and nasal endoscopy.RESULTS:The postoperative follow-up period ranged from 3 to 36 months. Thirty two patients(89.4%, 42/47 eyes) were cured, which presented as the disappearance of symptoms, the patency of the lacrimal ostium and the lacrimal duct irrigation was clear. One patient (2.1%, 1/47 eyes) was improved, which presented as the improvement of symptoms, a few hypertrophic scar around the lacrimal ostium and the lacrimal duct irrigation was clear. Four cases failed (8.5%, 4/47), which presented as symptomatic and obstruction of the lacrimal ostium. The total effective rate reached to 91.5%. No serious postoperative complications were found in all cases.CONCLUSIONS:To expose Rosenmüller valve during endoscopic DCR in treating patients with refractory nasolacrimal duct obstruction could obviously increase the operative success rate, fewer complications, with no facial surgical scar. It is an ideal way to treat the refractory nasolacrimal duct obstruction.

Abstract:Dacryocystitis is a common ocular disease mainly characterized by tearing, swelling of the lacrimal sac and increased secretion, which seriously affects the quality of life and visual function of patients. [ 1 ] It is commonly caused by an obstruction within the nasolacrimal duct and subsequent stagnation of tears and lacrimal sac contents in the lacrimal sac. Dacryocystitis is mostly caused by microbial infections such as Streptococcus pneumoniae infection and Haemophilus influenzae infection. [ 2 ] The common treatments for dacryocystitis include medication, lacrimal irrigation, lacrimal dilation, and surgery. [ 3 ] Nowadays, in the treatment of chronic dacryocystitis, conservative medication and lacrimal irrigation cannot achieve satisfactory outcomes. In this case, surgery is now the main treatment for dacryocystitis. [ 4 ]

Abstract:Introduction: Nasolacrimal duct obstruction at the junction of the lacrimal sac and nasolacrimal duct causes acute or chronic dacryocystitis. Dacryocystorhinostomy (DCR) is the treatment of choice and has a high success rate. Objective: To analyze the success rate of external DCR surgery and its influencing factors at our hospital. Methods: It is a hospital-based descriptive study of cases that underwent external DCR surgery from June 2016 to February 2017 at Biratnagar eye hospital. The demographic data were recorded and the successful outcome of the surgery along with its influencing factors -tube status, operating surgeons, and gender was analyzed. The main outcome measure was defined as a patent lacrimal passage on syringing at 6 months postoperatively. Chi-square and Fisher’s exact test were used for statistical analysis. Results: In total 317 patients, the mean age of the patients was 38.46 (SD ± 13.6) years. The male to female ratio was 1:2.5. The commonest presenting symptom was watering with discharge in 224 (70.6%) patients. Of the total, 195(61.5%) had undergone the DCR surgery with intubation and 122 (38.5%) without intubation. Only 114 (35.9%) patients were followed up for 6 months. The success rate was 86.8%. Postoperative wound infection, bleeding or cheese wiring of the punctum was not seen. No statistically significant factors were seen to influence the success rate of DCR surgery. Conclusions: External DCR surgery has a high success rate and no significant influencing factors were found to define its success in this study.
Long-Term outcomes of pediatric dacryocystorhinostomy in a single medical center– a retrospective study

Abstract:European Journal of Ophthalmology, Ahead of Print. PurposeTo examine the long-term success rate of pediatric endoscopic DCR surgery via telephone questionnaires, as determined by patients and their parents.MethodsThis is a retrospective cohort study of all patients who underwent DCR surgery at the Schneider Children's Medical Center of Israel between 2010 and 2020. We performed long-term follow-ups to assess the quality of life, surgical complications, and satisfaction with surgical outcomes.ResultsOur study includes seventy-nine patients with a total of 108 eyes. The mean age at the time of DCR was 7.05 years (Std = 4, min = 0.3, max = 17.7) Mean follow-up time was 5.7 years (Std =2.5, min = 1.4, max = 11.1). Tubes were inserted for a mean of 129 days (Std = 101). Fifty-seven patients (72%) declared they had no complications after surgery, three patients (4%) reported pain after surgery, and 14 patients (17.7%) reported tube extrusion, which occurred 7–21 days after surgery. Forty-four patients (56%) reported no recurrence of symptoms, 29 (37%) complained of mild epiphora, and 18 (23%) reported some ocular discharge. Sixty-eight patients (86%) stated that they did not undergo additional surgery, while the remaining 11 (14%) reported undergoing a revision operation for symptom control. Satisfaction rate (1–7) mean score reported was 6.15 (Std = 1.6). Sixty-two (78%) reported improved quality of life, while 17 (22%) reported no improvement. Our questionnaire results have been compared with the TEARS scores with similar findings.ConclusionRegardless of its etiology, endoscopic DCR surgery in the pediatric population is safe and efficient, with a high long-term patient satisfaction rate, as reported via a telephone questionnaire.
